Resumo

A hipertensão arterial sistêmica (HAS) é uma enfermidade silenciosa que pode afetar gatos e cães. Embora bastante descrita em felinos, a "Síndrome do Jaleco Branco", ou hipertensão situacional, é pouco relatada em cães. Assim, este trabalho tem como objetivo mostrar a importância do estresse na aferição da pressão arterial de cães adultos. O estudo foi realizado com 46 cães de ambos os sexos, dóceis, adultos, e aparentemente saudáveis. Após anamnese, exame físico, ambientação e manejo amigável dos animais, mensurou-se a pressão arterial sistólica (PAS) pelo método Doppler no membro torácico. Foram feitas várias aferições para obtenção de média aritmética. Animais com até 160mmHg de PAS, na primeira aferição, foram considerados normotensos (36 cães, correspondente a 78,26%). Para os que tiveram PAS maior ou igual a 160mmHg (dez cães, equivalente a 21,74%), realizou-se um segundo momento de aferição, após 15 a 30 minutos, com os animais em repouso. Assim, seis desses cães apresentaram PAS entre 154mmHg e 200mmHg, sendo, então, um considerado pré-hipertenso e cinco hipertensos, e outros quatro considerados normotensos. Para o animal com pré-hipertensão e para os hipertensos, realizaram-se exames complementares, pelos quais quatro cães foram diagnosticados com doença renal crônica e dois com dislipidemia familiar dos Schnauzers. Dessa forma, pode-se concluir que, dos 46 cães avaliados, 36 apresentaram-se normotensos (78,26%), um pré-hipertenso (2,17%), cinco hipertensos (10,87%) e quatro com hipertensão situacional (8,7%). Portanto, para cães em ambiente hospitalar, é importante realizar um manejo amigável e, quando necessário, mais de um momento de aferição de PAS para evitar diagnósticos errôneos de HAS.


Systemic arterial hypertension (SAH) is a silent disease that can affect cats and dogs. Although well described in felines, "white coat syndrome", or situational hypertension, is little reported in dogs. Thus, this paper aims to show the importance of stress in the measurement of blood pressure from adult dogs. The study was conducted with 46 dogs of both sexes, docile, adults, apparently healthy. After anamnesis, physical examination, ambiance, and friendly management of animals, systolic blood pressure (SBP) was measured by the doppler method in the thoracic limb. Several measurements were made to obtain an arithmetic average. Animals up to 160mmHg of SBP, in the first measurement, were considered normotensive (36 dogs, corresponding to 78.26%). For those with SBP higher than or equal to 160mmHg (10 dogs, equivalent to 21.74%), a second measurement was performed after 15 to 30 minutes, with the animals at rest. Therefore, six of these dogs presented SBP between 154mmHg and 200mmHg (one considered prehypertense and five hypertensives) and four normotensives. For the animal with prehypertension and the hypertensives, complementary examinations were performed, in which four dogs were diagnosed with chronic kidney disease and two with family dyslipidemia of the Schnauzers. Thus, it can be concluded that among the 46 dogs evaluated, there were 36 normotenses (78.26%), a one prehypertense (2.17%), five hypertensives (10.87%), and four with situational hypertension (8.7%). Therefore, regarding dogs in a hospital environment, it is important to perform friendly management and, when necessary, more than one moment of SBP measurement to avoid erroneous diagnoses of SAH.

Using Gas chromatography technique, the intensity of toxic pollutant residues in serum samples of Hypertensive patients were measured. Based on statistical analysis, the effects of different covariates namely pesticides, age, systolic blood pressure, diastolic blood pressure, and lipid profile duration was checked using the logistic regression model. Statistical computation was performed on SPSS 22.0. The P-values of F-Statistic for each lipid profile class are greater than 0.01 (1%), therefore we cannot reject the null hypothesis for all cases. The estimated coefficients, their standard errors, Wald Statistic, and odds ratio of the binary logistic regression model for different lipid profile parameters indicate if pesticides increase then the logit value of different lipid profile parameters changes from -0.46 to -0.246 except LDL which increases by 0.135. The study reports a significantly increased threat of cardiovascular disease with increased concentrations of toxic pollutants.


Resumo Este estudo determina as associações entre o perfil lipídico sérico, o risco de doença cardiovascular e os poluentes orgânicos persistentes. Por meio da técnica de cromatografia gasosa, mediu-se a intensidade dos resíduos de poluentes tóxicos em amostras de soro de pacientes hipertensos. Com base na análise estatística, os efeitos de diferentes covariáveis ou seja, pesticidas, idade, pressão arterial sistólica, pressão arterial diastólica e duração do perfil lipídico foram verificados usando o modelo de regressão logística. O cálculo estatístico foi realizado no SPSS 22.0. Os valores P da estatística F para cada classe de perfil lipídico são maiores que 0,01 (1%), portanto não podemos rejeitar a hipótese nula para todos os casos. Os coeficientes estimados, seus erros padrão, estatística de Wald e odds ratio do modelo de regressão logística binária para diferentes parâmetros do perfil lipídico indicam se os pesticidas aumentam, então o valor logit de diferentes parâmetros do perfil lipídico muda de -0,46 para -0,246, exceto LDL, que aumenta em 0,135. Background: Canine hypercortisolism (HC) is the most prevalent endocrinopathy in dogs in southern Brazil. The prognosis depends on several factors including the general health status, owners´ commitment, and the development of disease complications and comorbidities occurrence, such as cardiovascular complications including mitral valve disease (MVD), systemic arterial hypertension, and left ventricular hypertrophy. The main objective of the present study was to assess cardiovascular parameters in canine HC, based on investigating survival-related variables. The study also aimed to evaluate the influence of concurrent preclinical (MVD) on dogs` survival and the impact of HC on MVD progression. Materials, Methods & Results: A total of 25 dogs with spontaneous HC were enrolled and divided into 2 subgroups accordingly to their echocardiographic findings: group HC (normal echocardiography at first evaluation, n = 16); and group HC + MVD (concurrent presence of stages B1 and B2 MVD diagnosed at the first evaluation, n = 9). The patients were evaluated at diagnosis (T0); 6 months after treatment begging (T1); and after 12 months of treatment (T2). The owners were further contacted by phone or e-mail for 1 more year after T2 regarding survival information. A control group (CG, n = 20) was also evaluated at T0 and T1. At each evaluation, dogs were submitted to a complete clinical evaluation and physical exam, associated with a minimum database (CBC, serum biochemistry, and urine analysis) and cardiovascular evaluation composed of systolic blood pressure determination, electrocardiogram (ECG), and echodopplercardiography. In the HC group, 11/16 dogs underwent the evaluation at T2, while 4/9 dogs from the HC + MVD group and 17/20 dogs from the CG underwent the evaluation at T2. Five dogs (31.25%) from the HC group and 4 dogs (44.44%) from the HC + MVD group died before the end of the follow-up period. In the control group, only 1 dog (5.26%) died before the end of the study. Despite the higher mortality in the HC + MVD group during the follow-up period, there was no significant difference (P = 0.632) in survival when compared with the HC group. The MVD of 4 dogs included in the HC + MVD group was classified as stage B1, while the other 5 dogs were classified as stage B2. Only 1 dog from the CG developed stage B1 MVD in the period studied; however, progression of the MVD stage was documented in 1/4 of dogs in the HC + MVD group and MVD development was documented in 3/11 of dogs from the HC group from T0 to T2. The odds ratio (OR) and respective 95% confidence interval (95%CI) for HC as a risk factor for MVD progression were 4.267 (0.4268 - 42.65; P = 0.342). Exercise intolerance (12/16 dogs) and dyspnea (6/16 dogs) were the cardiorespiratory clinical signs with the highest incidence in the HC group at T0. When compared to the control group, both exercise intolerance (P < 0.001) and dyspnea (P = 0.03) occurrence were significantly higher in the HC group. The age (P = 0.001) and the occurrence of dyspnea (P = 0.036) at diagnosis were significantly higher in dogs with HC that died during the follow-up than those that remained alive. Regarding the occurrence of cardiac arrhythmias verified by ECG, no statistically significant differences were observed among groups. Discussion: The study suggests that systemic hypertension and altered echocardiographic measurements did not interfere with dogs' survival; however, dyspnea was associated with a worse prognosis. Finally, it is possible to conclude that mitral valve degeneration is a common comorbidity in dogs with HC, however, it was not evidenced their interference in the survival of dogs with this endocrine disease or even a role of the HC in the progression of the MVD.

Resumo

Nitric oxide (NO) is an important mediator responsible for vasodilation in pulmonary hypertension (PH) in humans. Based on human literature, it is suggested that in dogs there is also NO production decrease in lung tissue in the presence of PH with hypoxia. Therefore, the aim of this research was to determine the indirect plasmatic NO concentration in dogs with PH secondary or not to the left-side heart disease (LHD) and also with low, intermediate and high probability of PH to characterize the NO involvement on PH in dogs. Blood samples were collected from 35 dogs with probability of PH to NO measurement. NO concentration was estimated by the nitrite/nitrate concentration, and it was significantly different (p=0.002) in dogs with PH secondary to LHD (median=14 µM, range 11.19-16.59) and not secondary to LHD (median=25.88µM, range 15.08-36.71). However, this was not significant for the probability of low, intermediate, and high PH, although there was a tendency for NO concentration to be higher in dogs with high PH. The results of this study demonstrate that there is release of NO in dogs with PH, as well as that its dosage could differentiate dogs with PH secondary to LHD from dogs with non-secondary PH.


O óxido nítrico (ON) é um importante vasodilatador na hipertensão pulmonar (HP) em seres humanos. Baseado na literatura humana, sugere-se que em cães também ocorra a diminuição da produção de ON no tecido pulmonar na presença de HP frente à hipóxia. Dessa forma, o objetivo desta pesquisa foi determinar a concentração indireta do ON plasmático em cães com HP secundária ou não à doença do lado esquerdo do coração (LHD), bem como caracterizar os achados ecocardiográficos de diagnóstico e a probabilidade de HP (baixa, intermediária e alta). Para isso, foram coletadas amostras de sangue de 35 cães com probabilidade de HP para mensuração de ON. A concentração do ON foi estimada pela concentração de nitrito/nitrato, e esta foi significativamente diferente (P=0,002) nos cães com HP secundária à LHD (mediana=14µM, intervalo 11,19-16,59) e não secundária à LHD (mediana=25,88µM, intervalo 15,08-36,71), porém não significativa para a probabilidade de HP baixa, intermediária e alta, embora houvesse uma tendência a ser maior a concentração de ON nos cães com probabilidade alta. Os resultados deste estudo demonstram que há liberação do ON em cães com HP, bem como que sua dosagem conseguiu diferenciar cães com HP secundária à LHD de cães com HP não secundária.

Purpose: In this study, the aim to assess the combined effects of prone-positioning (PP) and minimal-flow (MF) general anesthesia on regional cerebral oxygenation (RCO) and systemic hemodynamics. Methods: This is a randomized prospective study aiming to evaluate changes in cerebral oxygenation and hemodynamic parameters in MF systemic anesthesia in patients undergoing surgery in PP. Patients were randomized to MF or normal-flow (NF) anesthesia. In the operating room, pulse rate, mean arterial pressure (MAP), peripheral hemoglobin oxygen saturation (spO2), and right- and left-side RCO (assessed by nearinfrared spectroscopy, NIRS) were measured perioperatively. Results: Overall, 46 patients were included (24 in the MF group and 22 in the NF group). The amount of anesthetic gas consumption was significantly lower in the low-flow (LF) group. In both groups, the mean pulse rate showed a decrease after PP. Before induction, RCO was significantly higher both at the right- and left-sides in the LF group compared to the NF group. This difference continued throughout the operation on the left-side and disappeared 10 min after intubation on the right-side. On the left side, mean RCO decreased after PP in both groups. Conclusion: MF anesthesia in PP did not reduce cerebral oxygenation compared to NF and was safe in terms of systemic hemodynamics and cerebral oxygenation.

Resumo

Background: The nonsteroidal anti-inflammatory drugs (NSAIDs) exert their analgesic effect through peripheral inhibition of prostaglandin synthesis and a variety of other peripheral and central mechanisms. However, NSAIDs are associated with some adverse effects, mainly related to the gastrointestinal, renal, and hepatic systems, highlighting the need for research to develop safer drugs. Therefore, the aim of this study was to evaluate the efficacy of preoperative oral administration of carprofen or grapiprant in female cats submitted to elective ovariohysterectomy on the quality of perioperative analgesia and the need for hypnotic and analgesic drugs. Materials, Methods & Results: Thirty-three adult female cats were selected, without defined breed and healthy based on physical examination, routine laboratory analyses (complete blood count, total protein, Heinz body investigation and serum quantification of alanine transaminase [ALT], aspartate transaminase [AST], gamma glutamyl transpeptidase [GGT], alkaline phosphatase [ALP], urea, frutosamine, and glucose) and negative tests for feline immunodeficiency virus (FIV) and feline leukemia virus (FeLV). After 3 days of adaptation, they were submitted to ovariohysterectomy by celiotomy and randomly allocated into 2 groups according to the preoperative drug used: GCAR [carprofen - 4 mg/kg, VO, 2 h before surgery; n = 11] and GGRA (grapiprant - 2 mg/kg IV, 2 h before surgery; n = 21]. The cats were pre-medicated with acepromazine 0.05 mg/ kg IV and later submitted to general anesthesia with propofol intravenously. Anesthesia was maintained with isoflurane in 100% oxygen. After anesthetic induction, a continuous infusion of remifentanil at a rate of 10 µg/kg/h was initiated. During the transanesthetic period, the parameters of heart rate; respiratory rate; systolic, mean, and diastolic arterial pressure using the oscillometric method; electrocardiogram; rectal temperature; partial pressure of CO2 at the end of expiration: and partial saturation of O2 in hemoglobin were continuously monitored. The evaluation of nociception was based on the changes in the aforementioned physiological parameters. The rate of remifentanil used did not change over time with the use of carprofen. However, animals that received grapiprant required a lower remifentanil dose at 20, 25, and 30 min during the procedure. The female cats that received carprofen showed an increase in mean heart rate at 30 min compared to that at 20 and 25 min. In the Grapiprant group, the heart rate at 35 min was higher only than that observed at 25 min. Discussion: The remifentanil rate did not differ between the groups, even between the times for GCAR. However, the remifentanil rate was lower from 20 min of the procedure for GGRA. This decrease may be related to a decrease in the need for anesthetics and analgesics by decreasing temperature, which causes decreases in metabolism and surgical stimulation. The increase in systolic, mean, diastolic, and heart rate arterial pressure parameters observed in both treatments after 15 min of anesthesia is related to the nociceptive stimulus resulting from traction and ligation of the ovarian pedicles and maneuvers for exteriorization of the uterus. These are considered the moments of greater surgical stimulus during ovariohysterectomy, evidenced by the greater release of cortisol and increase in physiological parameters. The results of this study show that the administration of carprofen or grapiprant was clinically similar when used preemptively for perioperative analgesia in cats submitted to elective ovariohysterectomy.

Laparoscopy procedures are useful tools to perform some assisted reproductive biotechnologies in ewes, it requi-res general anesthesia and manoeuvres that might result in alteration of the cardiopulmonary function. For this reason, this study aimed to investigate the effects of oxygen supplementation as a therapeutic measure to mitigate these alterations in ewes submitted to laparoscopic ovum pick-up (LOPU) under total intravenous anesthesia (TIVA). Twenty-four healthy adult ewes were submitted to two LOPUs with a 21 days interval, under ketamine-midazolam anesthesia, and receiving each of the two experimental in random order, oxygen treatment (OT) 50 mL/kg/min of oxygen via endotracheal tube and control treatment (CT) not receive any inhalation treatment. Heart rate (HR), respiratory rate (fR), peripheral oxygen saturation (SpO2), mean arterial pressure (MAP), rectal temperature (RT), end-tidal CO2 concentration (EtCO2) and recovery anesthesia time were evaluated during LOPU, arterial blood gases and electrolytes were evaluated after induction of anesthesia and at the end of the LOPU. Variables were compared between groups and moments using ANOVA. MAP, SpO2, PaO2, SaO2 and pH were higher in OT, while EtCO2, PaCO2, temperature loss and recovery time were lower. These results allow to conclude that oxy-gen supplementation in ewes submitted to LOPU under TIVA provides benefits in order to mitigate physiological alterations.(AU)


Os procedimentos de laparoscopia são ferramentas úteis para realizar algumas biotecnologias de reprodução assistida em ovelhas, requer anestesia geral e manobras que podem resultar em alteração da função cardiopulmonar. Por esse motivo, este estudo teve como objetivo investigar os efeitos da suplementação de oxigênio como medida terapêutica para atenuar as alterações em ovelhas submetidas à Aspiração Folicular guiada por Laparoscopia (LOPU) sob anestesia venosa total (TIVA). Vinte e quatro ovelhas adultas saudáveis foram submetidas a duas LOPUs com intervalo de 21 dias, sob anestesia com cetamina-midazolam, recebendo cada um dos dois experimentos em ordem aleatória, tratamento com oxigênio (OT) 50 mL/kg/min de oxigênio via tubo endotraqueal e tratamento de controle (CT) não recebem nenhum tratamento por inalação. Frequência cardíaca (FC), frequência respiratória (FR), saturação periférica de oxigênio (SpO2), pressão arterial média (PAM), temperatura retal (TR), concentração expirada de CO2 (EtCO2) e tempo de recuperação da anestesia foram avaliados durante LOPU, arterial gasometria e eletrólitos foram avaliados após a indução da anestesia e ao final da COL. As variáveis foram comparadas entre grupos e momentos por meio de ANOVA. PAM, SpO2, PaO2, SaO2 e pH foram maiores no TO, enquanto EtCO2, PaCO2, perda de temperatura e tempo de recuperação foram menores. Estes resultados permitem concluir que a suplementação de oxigênio em ovelhas submetidas a LOPU sob TIVA proporciona benefícios no sentido de atenuar as alterações fisiológicas.(AU)

Background: Ischemic neuromyopathy is the most common reason for amputation in cats. In veterinary medicine, the use of prosthetic limbs is not widespread; therefore, in most cases total limb amputation is indicated. However, hyperbaric oxygen therapy (HBOT) is an alternative with several benefits for the treatment of vascular disorders with reperfusion, ischemia, and infection. Therefore, this study aimed to report the positive effects of HBOT on the treatment of ischemic neuromyopathy secondary to arterial thromboembolism on the patient's clinical improvement, and on the preparation of the patient for insertion of an osseointegrated prosthesis. Case: A 6-month-old mixed-breed kitten returned for treatment after undergoing surgery seven days earlier for reduction of traumatic diaphragmatic hernia, during which it suffered a cardiorespiratory arrest. The patient presented with acute pelvic limb paralysis with 24-h evolution, absent femoral pulse, plantar cushions and dorsal part of the limbs cold and pale. After supportive therapy and diagnosis of aortic thromboembolism by arterial Doppler, the patient started adjunctive treatment with HBOT from the first day of hospitalization. Sessions took place in an exclusive hyperbaric chamber for animals and lasted 60 min at a pressure of 2.5 absolute atmospheres and 100% oxygen, initially every 12 h. However, during the first 5 days of hospitalization, the distal region of both pelvic limbs began to show tissue devitalization and edema, and hematologic parameters showed changes on the 7th day. The right pelvic limb (RPL) showed more involvement of superficial tissues, extending to the tarsometatarsal joint region. After 8 days of hospitalization, the devitalized tissue was debrided. The RPL had an extensive devitalized area with exposed bone in the phalanges and necrosis in the pads. The left pelvic limb (LPL) suffered minor complications, with involvement of the phalangeal region. After 12 days, with HBOT every 48 h, exuberant granulation tissue was observed. After 17 days, the patient was discharged, and HBOT sessions were performed weekly. Gangrene of the midfoot and lack of proprioception were observed in RPL, while LPL showed bone divulsion of the 1st, 3rd, and 4th phalanges. Because of the poor prognosis for limb viability, the RPL was partially amputated, and a self-threaded intraosseous prosthesis was inserted. Discussion: The cardiorespiratory arrest that occurred during the surgical procedure to reduce the diaphragmatic hernia without thromboprophylaxis may have contributed to the peripheral ischemia. HBOT was proposed for the adjuvant treatment of ischemic injury because it is especially indicated for cases of ischemia-reperfusion injury. The main hematological parameters were evaluated at an average interval of 7 days. While the platelet count and hematocrit increased, the leukocytosis decreased. This demonstrates the benefit of oxygen therapy in the reported patient. The use of HBOT in orthopedic injuries is known to result mainly in stimulation of osteoblasts, promoting osseointegration of the prosthesis. We conclude that the adjuvant treatment with HBOT helped to preserve a large segment of both pelvic limbs, prevent the progression of necrosis, and provide a healthy bed for fixation of an osseointegrated prosthesis in the RPL, resulting in clinical improvement of the patient.

Background: Photoplethysmography is widely used in human medicine, with few studies on its use in veterinary medicine. Its sensor detects fluctuations in blood volume at the site, providing direct readings of cardiac pulse and peripheral oxygen saturation, as well as estimating cardiac output, respiratory rate and blood pressure. This study aimed to evaluate the use of photoplethysmography and compare it to vascular Doppler ultrasound as an indirect method of measuring systolic blood pressure in bitches undergoing elective ovariohysterectomy, using the invasive assessment of systolic blood pressure as a reference. Materials, Methods and Results: After clinical and laboratory evaluation, 34 healthy bitches were selected to undergo elective ovariohysterectomy. After food and water fasting, patients received pethidine hydrochloride intramuscularly as pre-anesthetic medication, followed by anesthetic induction with fentanyl citrate and propofol intravenously. General anesthesia was maintained by inhalation with isoflurane diluted in 100% oxygen. Intraoperative analgesia consisted of continuous infusion of fentanyl citrate intravenously. The animals were randomly divided into 2 groups, the thoracic limb group (TLG) and the pelvic limb group (PLG). In each patient, non-invasive blood pressure measurement was obtained simultaneously with Doppler (DOP) and photoplethysmography (PPG). The sensors of both devices were placed on the end of the same limb. The PPG sensor was positioned in the interdigital region. In patients belonging to the TLG, the Doppler sensor was placed in the ventral region of the thoracic limb, under the ulnar artery. In PLG patients, the Doppler sensor was placed in the dorsal region of the pelvic limb, over the dorsal artery of the foot. The sphygmomanometer was positioned close to the sensors. For systolic blood pressure (SBP) measurement, the cuff was inflated until the Doppler sound signal and the plethysmographic wave were lost. The cuff was then deflated until the Doppler pulse sound resumed and the photoplethysmography showed at least 2 continuous waves on a regular basis. The corresponding pressure value observed on the manometer consisted of the SBP. The same 2 evaluators performed all SBP measurements: 1 responsible for the DOP method and the other for the PPG method; both were blind to the other's findings, thus minimizing potential bias in the results. All animals underwent cannulation of the auricular artery for invasive measurement of systolic blood pressure, using a multiparameter monitor. All blood pressure measurements were performed at 5-min intervals, as well as obtaining additional parameters (heart and respiratory rate, esophageal temperature, partial tissue oxygen saturation, carbon dioxide concentration) and electrocardiographic monitoring. All parameters were documented for further statistical analysis. A strong correlation (r² = 0.95) was obtained between the DOP and PPG methods regardless of the limb on which the sensors were placed. There was a low correlation between the invasive method of measuring systolic blood pressure and the other methods. There was better agreement between the DOP and PPG methods (r2 = -0.0061; P = 0.85) when systolic blood pressure was measured in the TLG. Discussion: In the PLG, the values obtained with the DOP and PPG methods were significantly higher than those obtained with the invasive method, while the values obtained in the TLG differed slightly. It was found that the best measurement site by non-invasive methods was the thoracic limb. It was concluded that the non-invasive methods showed a low correlation with the invasive method; however, both methods had similar characteristics and photoplethysmography can be used to replace the vascular Doppler method.

In the veterinary field,there are few reported cases of BJR induced by dopamine, which is often used to manage hypotension. Herein, 2 casesinvolving small dogs exhibiting BJR due to dopamine infusion during general anesthesia are described.Cases: Case 1: A 7-year-old, 7 kg, mongrel was referred for external skeletal fixator removal. The patient was premedicatedwith 0.3 mg/kg midazolam and 0.2 mg/kg butorphanol intravenously (IV). General anesthesia was induced with 6 mg/kgpropofol and maintained with 1.6% isoflurane in oxygen. The patient was given 5 mL/kg/h of Hartmann’s solution IV. Therespiratory rate (RR) was set to 9 breaths/min with a ventilator. The HR and MAP values were initially 120 bpm and 76mmHg and gradually decreased to 70 bpm and 40 mmHg, respectively. The end-tidal CO2 partial pressure (ETCO2) was 39mmHg, and the patient was administered 2.5 μg/kg glycopyrrolate IV. Then, 5 μg/kg/min dopamine was administered IVsince the MAP did not improve. The HR, MAP, and ETCO2 increased to 113 bpm, 72 mmHg, and 47 mmHg, respectively.Subsequently, HR and MAP dramatically decreased to 50 bpm and 43 mmHg, respectively. A anestesia locorregional, em pequenos animais, está em crescente utilização como forma de promover analgesia para procedimentos cirúrgicos, através do bloqueio reversível da condução do impulso nervoso. Assim, o objetivo deste trabalho foi relatar o uso do bloqueio locorregional do plano serrátil (SP block) associado ao bloqueio locorregional do plano transverso do abdômen (TAP block) em uma cadela da raça Lhasa Apso. A associação de tais bloqueios visou à insensibilização da musculatura na região da cadeia mamária para um procedimento cirúrgico de mastectomia unilateral total. A cadela castrada, foi encaminhada para a mastectomia unilateral total direita, apresentando nódulos nas glândulas mamárias torácica caudal e inguinal. Os bloqueios foram realizados com a paciente em decúbito dorsal, utilizando-se bupivacaína a 0,25%. Em seguida, a paciente foi liberada para o procedimento, o qual foi monitorado. Os parâmetros de eletrocardiografia, frequência respiratória, frequência cardíaca, oximetria de pulso, pressão arterial oscilométrica (composta por pressão arterial sistólica, pressão arterial diastólica e pressão arterial média), capnografia e temperatura esofágica foram anotados a cada dez minutos em uma ficha de avaliação anestésica. Durante a recuperação pós-operatória, foi realizada a avaliação de dor aguda por meio da escala de dor da Universidade de Melbourne. Os resultados demonstram que a associação do TAP block com o SP block foi eficiente em promover analgesia para uma mastectomia total unilateral de cadela.


Locoregional anesthesia in small animals is increasingly used as a way to promote analgesia for surgical procedures, through a reversible block of nerve impulse conduction. Thus, the objective of this study was to report the use of locoregional block of the serratus plane (SP block) associated with the locoregional block of the transverse abdominis plane (TAP block) in a bitch of the Lhasa Apso breed. The association of such blocks aimed to desensitize the musculature in the region of the mammary chain for a surgical procedure of total unilateral mastectomy. The castrated bitch was referred for total right unilateral mastectomy, showing nodules in the caudal thoracic and inguinal mammary glands. The blocks were performed with the patient in dorsal decubitus, using 0.25% bupivacaine. Then, the patient was released for the procedure, which was monitored. The parameters of electrocardiography, respiratory rate, heart rate, pulse oximetry, oscillometric blood pressure (composed of systolic blood pressure, diastolic blood pressure, and mean blood pressure), capnography, and esophageal temperature were recorded every 10 minutes in an anesthetic evaluation form. During the postoperative recovery, acute pain was evaluated using the pain scale of the University of Melbourne. The results demonstrate that the association of TAP block with SP block was efficient in promoting analgesia for a total unilateral mastectomy in a bitch.

Purpose: To investigate the role of renal denervation (RDN) on endogenous ouabain (EO) secretion in spontaneously hypertensive rats (SHR). Methods: Sixteen 12-week-old male SHR were randomly separated into the renal denervation group (RDNX group) and sham operation group (sham group), and eight age-matched Wistar Kyoto rats (WKY) were served as control group. EO concentrations, the Na+- K+-ATPaseactivity, and the expression of Na+-K+-ATPase were assessed. Results: EO levels in serum, kidneys and hypothalamus of sham group were higher than in RDNX group (p < 0.05). Renal Na+-K+-ATPase activity subjected to denervation surgery showed significantly reduction when compared with the sham groups (p < 0.05). A positive correlation existed between norepinephrine (NE) content and Na+-K+-ATPase activity in the kidney (r2 = 0.579). Renal Na+-K+-ATPase α1 subunit mRNA expression was down-regulated in the RDNX group compared with the sham group (P < 0.05), while renal Na+-K+-ATPase α1 subunit mRNA expression was no statistical significance between the groups (P = 0.63). Immunohistochemical analysis showed that there were significant differences in the renal expression of Na+-K+-ATPasebetween the three groups (P < 0.05). Conclusions: These experiments demonstrate that RDN exerted an anti-hypertensive effect with reduction of EO levels and Na+-K+-ATPase activity and Na+-K+-ATPase α1 subunit expression of kidney in SHR.

Anesthetic protocols have been developed to obtain the most effective and safe association in wildlife. This study compared the anesthetic effects and cardiorespiratory parameters of ketamine-S (+) (10 mg/kg)/dexmedetomidine (0.020 mg/kg) (KD ) and ketamine-S (+) (10 mg/kg)/midazolam (0.5 mg/kg)/methadone (1.0 mg/kg) (KMM ) in capuchin monkeys (Sapajus apella). Eight capuchin monkeys were randomly assigned to KD (n = 4) or KMM (n = 4) to evaluate induction, immobilization, and recovery scores, heart and respiratory rate parameters, besides systolic, mean, diastolic arterial pressure and arterial blood gas. There was no difference (P = 0.56) in the quality of induction, immobilization, and anesthetic recovery between the protocols. The time for anesthetic induction was 4 ± 1 min in the KD group and 5 ± 1 min in the KMM group, and these values were statistically equal (P = 0.28). The mean immobilization time in the KD and KMM groups were 35 ± 13 and 33 ± 15 min, respectively. Heart rate was lower in animals in the KD group (P < 0.001), while respiratory rate (P = 0.03), and mean blood pressure (P = 0.046) were higher than that of the animals in the KMM group. Respiratory acidosis occurred in the KMM group, with lower pH (7.25±0.047; P = 0.0055) and higher pCO2 (51 ± 6;mmHg; P = 0.008). Both protocols exhibited good induction quality, immobilization, and anesthetic recovery, despite cardiorespiratory and blood gas alterations observed, which warrants monitoring of cardiorespiratory variables during KD or KMM chemical restraint.


Protocolos anestésicos foram desenvolvidos para obter a associação mais eficaz e segura em animais selvagens. O objetivo deste estudo foi comparar os efeitos anestésicos e os parâmetros cardiorrespiratórios de cetamina-S (+) (10 mg / kg) / dexmedetomidina (0,020 mg / kg) (KD) e cetamina-S (+) (10 mg / kg) / midazolam (0,5 mg / kg) / metadona (1,0 mg / kg) (KMM) em macacos-prego (Sapajus apella). Oito macacos-prego foram distribuídos aleatoriamente em KD (n = 4) ou KMM (n = 4) para avaliar os escores de indução, imobilização e recuperação, parâmetros de frequência cardíaca e respiratória, além da pressão arterial sistólica, média, diastólica e gasometria arterial. Não houve diferença (P = 0,56) na qualidade da indução, imobilização e recuperação anestésica entre os protocolos. O tempo de indução anestésica foi de 4 ± 1 min no grupo KD e 5 ± 1 min no grupo KMM, sendo esses valores estatisticamente iguais (P = 0,28). O tempo médio de imobilização nos grupos KD e KMM foram 35 ± 13min e 33 ± 15 min, respectivamente. A frequência cardíaca foi menor nos animais do grupo KD (P < 0,001), enquanto a frequência respiratória (P = 0,03) e a pressão arterial média (P = 0,046) foram maiores do que nos animais do grupo KMM. Acidose respiratória ocorreu no grupo KMM, com menor pH (7,25 ± 0,047; P = 0,0055) e maior pCO2 (51 ± 6; mmHg; P = 0,008). Ambos os protocolos apresentaram boa qualidade de indução, imobilização e recuperação anestésica, apesar das alterações cardiorrespiratórias e gasométricas observadas, o que justifica o monitoramento das variáveis cardiorrespiratórias durante a contenção química com KD ou KMM.

Objetivou-se avaliar os efeitos cardiorrespiratórios da dexmedetomidina isolada e associada à morfina em pacientes submetidas à anestesia geral inalatória com isoflurano e submetidas à ovariohisterectomia eletiva. Vinte cadelas saudáveis foram selecionadas a partir de exames físico e laboratoriais. Os animais foram distribuídos aleatoriamente em dois grupos: dexmedetomidina (10 µg/Kg) (GD) e dexmedetomidina (10 µg/Kg) associado com morfina 0,3 mg/Kg (GDM), administrados pela via intramuscular (IM). Ato contínuo, o paciente foi induzido com propofol (à efeito) e mantido com isoflurano diluído em oxigênio 100% e administrado através de vaporizador calibrado. Foram aferidos os parâmetros fisiológicos: frequências cardíaca (FC) e respiratória (f), temperatura corporal (T°C), pressões arteriais sistólica (PAS), diastólica (PAD) e média (PAM), saturação de oxigênio na hemoglobina (SpO2), pressão parcial de gás carbônico no final da expiração (ETCO2) e isoflurano expirado (ETISO). A hemogasometria arterial foi utilizada para a análise do potencial hidrogeniônico (pH), bicarbonato (HCO3-) e pressão parcial de dióxido de carbono (PaCO2). O procedimento cirúrgico teve duração máxima de 45 minutos. Os parâmetros demonstraram diferença estatística entre os tempos e grupos avaliados, de acordo com os testes de Tukey e Bonferroni (p<0,05). Observou-se bradicardia 30 minutos após a aplicação da medicação pré-anestésica e acidemia no período trans-anestésico. Apesar disto, tanto a dexmedetomidina isolada quanto associada, conferiram estabilidade hemodinâmica e respiratória, apesar da bradicardia e acidemia observada.(AU)


This study aimed to evaluate the cardiorespiratory effects of dexmedetomidine alone and associated with morphine. Twenty healthy dogs were selected from physical and laboratory exams. The animals were randomly distributed into two groups: 10 µg/Kg dexmedetomidine (DG) and 10 µg/Kg dexmedetomidine group associated with 0.3 mg/Kg morphine (GDM), administered intramuscularly (IM). The patients were induced with propofol (to effect) and maintained with isoflurane diluted in 100% oxygen and administered through a calibrated vaporizer. Physiological parameters were measured: heart rate (HR), respiratory rate (f), body temperature (T°C), systolic arterial pressure (SAP), diastolic arterial pressure (DAP) and mean arterial pressure (MAP), oxygen saturation of hemoglobin (SpO2), end-tidal carbon dioxide partial pressure (ETCO2) and end-tidal isoflurane concentration (ETISO). Arterial blood gas analysis was used to analyze hydrogen potential (pH), bicarbonate (HCO3-) and partial pressure of carbon dioxide (PaCO2). The surgical procedure lasted up to 45 minutes. The parameters showed a statistical difference between the times and groups evaluated, according to the Tukey and Bonferroni tests (p<0.05). Bradycardia was observed 30 minutes after the application of pre-anesthesic medication and acidemia in the trans-anesthetic period. Despite this, both dexmedetomidine alone and associated, provided good hemodynamic and respiratory stability, despite the bradycardia and acidemia observed.(AU)


Este estudio tuvo como objetivo evaluar los efectos cardiorrespiratorios de la dexmedetomidina sola y asociada con la morfina en pacientes sometidas a anestesia general por inhalación con isoflurano y sometidas a ovariohisterectomía electiva. Se seleccionaron veinte perras sanas a partir de exámenes físicos y de laboratorio. Los animales se dividieron aleatoriamente en dos grupos: dexmedetomidina (10 µg/kg) (GD) y dexmedetomidina (10 µg/kg) asociada con 0,3 mg/kg de morfina (GDM), administrada por vía intramuscular (IM). Posteriormente, el paciente fue inducido con propofol (en efecto) y mantenido con isoflurano diluido en oxígeno al 100% y administrado mediante de un vaporizador calibrado. Se midieron parámetros fisiológicos: frecuencia cardíaca (FC) y respiratoria (f), temperatura corporal (T°C), presión arterial sistólica (PAS), diastólica (PAD) y media (PAM), saturación de oxígeno de hemoglobina (SpO2), presión parcial de carbono dióxido al final de la expiración (ETCO2) e isoflurano expirado (ETISO). Se utilizó gasometría arterial para analizar el potencial de hidrógeno (pH), el bicarbonato (HCO3-) y la presión parcial de dióxido de carbono (PaCO2). El procedimiento quirúrgico tuvo una duración máxima de 45 minutos. Los parámetros mostraron diferencia estadística entre los tiempos y grupos evaluados, según las pruebas de Tukey y Bonferroni (p <0.05). Se observó bradicardia 30 minutos después de la aplicación de medicación preanestésica y acidemia en el período transnestésico. A pesar de esto, tanto la dexmedetomidina aislada como la asociada proporcionaron estabilidad hemodinámica y respiratoria, a pesar de la bradicardia y acidemia observadas.(AU)

El objetivo de este artículo es describir un caso de neoplasia adrenocortical con manifestación de hiperadrenocorticismo. Los tumores adrenocorticales se originan a partir de diferentes tipos de células y presentan una variada manifestación clínica, que puede ser funcionante o no funcionante. Los adenocarcinomas son autónomos y funcionales en la mayoría de los casos, lo que lleva a una secreción excesiva de glucocorticoides, independientemente del control pituitario. Corroboran la aparición de hiperadrenocorticismo (HAC) al interferir con la síntesis de cortisol. Los signos clínicos observados pueden ser poliuria, polidipsia compensatoria, polifagia, alteraciones de la presión arterial, disfunciones cardíacas, renales y endocrinas, entre otras, estas manifestaciones clínicas pueden presentarse de forma aislada o en asociación. El diagnóstico se puede obtener de varias maneras, como mediciones de cortisol urinario, estimulación con hormona adrenocorticotrópica, pruebas de supresión con dosis bajas de dexametasona y pruebas de imagen. Sin embargo, el diagnóstico definitivo se basa en el uso de la histopatología. Este artículo reporta el caso de una mujer mestiza de 13 años con signos de poliuria y polidipsia. Tras descartar los diagnósticos iniciales (diabetes mellitus y/o alteraciones renales), se sospechó HAC, realizándose nuevas pruebas. Los resultados de los exámenes mostraro


The aim of this article is to describe a case of adrenocortical neoplasm with manifestation of hyperadrenocorticism. Adrenocortical tumors originate from different types of cells and present varied clinical manifestations, which can be functional or non-functional. Adenocarcinomas are autonomous and functional in most cases, leading to excessive secretion of glucocorticoids, regardless of pituitary control. They corroborate the occurrence of hyperadrenocorticism (HAC) due to interference in the synthesis of cortisol. Clinical signs can be observed, as polyuria, compensatory polydipsia, polyphagia, blood pressure alterations, cardiac, renal and endocrine dysfunctions, among others. Diagnosis can be made in various ways, such as urinary cortisol measurements, adrenocorticotropic hormone stimulation, low-dose dexamethasone suppression tests, and imaging tests and histopathology. This article reports the case of a 13-year-old mixed-breed female dog with signs of polyuria and polydpsia. After discarding the initial diagnoses (diabetes mellitus and/or renal alterations), HAC was suspected, with further tests being performed. The test results showed an increase in the adrenal region, and adrenalectomy and hormone replacement with trilostane were recommended. O objetivo deste artigo é descrever um caso de neoplasia adrenocortical com manifestação de hiperadrenocorticismo. Tumores adrenocorticais são originados de diversos tipos de células e apresentam manifestação clínica variada, podendo ser funcionantes ou não funcionantes. Os adenocarcinomas são autônomos e funcionantes na maioria dos casos, levando a secreção excessiva de glicocorticóides, independente do controle da hipófise. Eles corroboram com a ocorrência de hiperadrenocorticismo (HAC) por interferência na síntese de cortisol. Os sinais clínicos observados podem ser poliúria, polidipsia compensatória, polifagia, alterações pressóricas, disfunções cardíacas, renais e endócrinas, entre outros, estas manifestações clínicas podem se apresentar de forma isolada ou associada. O diagnóstico pode ser obtido de diversas formas, como dosagens de cortisol urinário, estimulação de hormônio adrenocorticotrópico, testes de supressão com baixa dose de dexametasona e por testes de imagem. Resumo

This study assessed the impact of different recumbency on sevoflurane-anaesthetised sheep. Seven female sheep were premedicated with 0.1 mg.kg-1 butorfanol and subsequently administered a combination of 3 mg.kg-1 ketamine and 0.5 mg.kg-1 midazolam. Animals were maintained on sevoflurane anaesthesia with pressure-controlled ventilation (12 cm H2O peak inspiratory pressure) and ƒ of 10 mpm. During the anaesthetic procedure, animals underwent one out of three different recumbency: dorsal, left lateral, or right lateral positions. Treatments lasted 120 min with a 48-h washout period in between the treatments. Arterial and central venous blood samples were withdrawn for blood gas and electrolytes analysis and pulmonary shunt fraction (Qs/Qt) CaO2, CcvO2, and CcO2 were calculated accordingly. Results showed that Qs/Qt greatly decreased from 0 to 120 min in all the groups (dorsal: 69.3% to 27.3%; left lateral: 59.1% to 25.0%; right lateral: 67.2% to 32.4%). CaO2, CcvO2 and CcO2 improved over time points, with no difference among treatments. PaO2 and PAO2 showed higher values for 60 and 120 min compared to the 0 min value in all groups, with no differences among treatments as well. PaCO2 and ETCO2 in the lateral groups were higher than those in the dorsal group at 120 min. Pressure-controlled ventilation improved gas exchanges in sheep, thereby reducing pulmonary shunt. Recumbency did not interfere with pulmonary shunt, nevertheless, special attention must be paid to lateral recumbency.


O estudo avaliou o impacto de diferentes decúbitos em ovelhas anestesiadas com sevoflurano. Sete ovelhas foram pré-medicadas com 0,1 mg.kg-1 de butorfanol e induzidas à anestesia com 3 mg.kg-1 de cetamina e 0,5 mg.kg-1 de midazolam. Os animais foram mantidos em anestesia por sevofluorano, em ventilação mecânica controlada por pressão, com pico inspiratório em 12 cm H2O e f de 10 mpm, sendo mantidos por 120 minutos. Durante esse período os animais foram submetidos a um dos três tratamentos: decúbito dorsal, lateral esquerdo ou lateral direito, com intervalo de no mínimo 48 horas entre eles. Amostras de sangue arterial e venoso central foram colhidas para análise de gases sanguíneos e eletrólitos, bem como para cálculo da fração de shunt pulmonar (Qs/Qt), CaO2, CcvO2 e CcO2. Os resultados mostraram que Qs/Qt diminuiu expressivamente de 0 a 120 minutos em todos os grupos (dorsal: 69,3% para 27,3%; lateral esquerdo: 59,1% para 25,0%; lateral direito: 67,2% para 32,4%). Os índices de CaO2, CcvO2 e CcO2 melhoraram ao longo do tempo, sem diferença entre tratamentos. PaO2 e PAO2 apresentaram valores maiores, em todos os grupos, nos minutos 60 e 120 em comparação ao momento 0, não havendo diferenças entre tratamentos. PaCO2 e ETCO2 apresentaram maiores valores nos grupos laterais em comparação ao grupo dorsal ao final do procedimento. Conclui-se que a ventilação controlada por pressão melhorou as trocas gasosas em ovelhas anestesiadas com sevoflurano, reduzindo o shunt pulmonar. O decúbito não interferiu na formação de shunt pulmonar, porém, deve ser dada atenção especial aos decúbitos laterais.
