Resumo

Em guaxinins, comumente se observa a presença de osteoartrites acometendo a articulação coxofemoral. Em carnívoros domésticos, o tratamento pode ser conservativo ou cirúrgico e, dentre as técnicas cirúrgicas, cita-se a colocefalectomia. Por ser um procedimento que provoca dor moderada a severa, um protocolo de anestesia balanceada se faz necessário, podendo associar a técnicas de bloqueio locorregional, como a epidural. Protocolos descritos para a espécie são limitados, porém comumente empregados para contenções químicas, avaliações físicas e exames de imagem. Portanto, o objetivo deste trabalho é relatar a o protocolo anestésico realizado em guaxinim (Procyon cancrivorus) submetido à colocefalectomia. Foram utilizados dexmedetomidina (5µg/kg), cetamina (5mg/kg) e diazepam (0,3mg/kg) por via intramuscular como medicação pré-anestésica, indução com propofol e manutenção por anestesia inalatória com isoflurano. Para execução da anestesia epidural, realizou-se uma abordagem na região lombossacral, sendo administrados bupivacaína (1mg/kg) e morfina (0,1mg/kg). Não houve intercorrências e os parâmetros fisiológicos mantiveram-se estáveis. Descartou-se a necessidade de resgate analgésico e a recuperação anestésica foi rápida. Dessa forma, conclui-se que o manejo anestésico proposto promoveu estabilidade hemodinâmica e recuperação satisfatórias para a espécie.


In raccoons, the presence of osteoarthritis affecting the hip joint is commonly observed. In domestic carnivores, the treatment can be conservative or surgical and, among the surgical techniques, colocephalectomy is mentioned. Since it is a procedure that causes moderate to severe pain, a balanced anesthesia protocol is necessary, which can be associated with locoregional block techniques such as the epidural. Protocols described for the species are limited, being commonly used for chemical restraints, physical assessments, and imaging exams. Therefore, this work aims to report the anesthetic protocol performed on a raccoon (Procyon cancrivorus) submitted to colocephalectomy. Dexmedetomidine (5µg/kg), ketamine (5mg/kg), and diazepam (0.3mg/kg) were used intramuscularly as pre-anesthetic medication, in addition to induction with propofol, and the maintenance by inhalational anesthesia with isoflurane. For epidural anesthesia, an approach was performed in the lumbosacral region, with the administration of bupivacaine (1mg/kg) and morphine (0.1mg/kg). There were no complications and the physiological parameters remained stable. The need for analgesic rescue was ruled out and the anesthetic recovery was fast. Thus, it is concluded that the proposed anesthetic management promoted hemodynamic stability and a satisfactory recovery for the species.

Background: The Brazilian tapir (Tapirus terrestris), considered the largest land mammal in South America, is a vulnerable species in terms of its degree of conservation. In captivity, its health is evaluated through behavioral and physical observation and laboratory exams, and in some cases, chemical restraint, to reduce stress. Dissociative anesthetics and sedatives are used for the sedation of these animals, and few studies have reported the use of dexmedetomidine and its effects when associated with other drugs in chemical containment protocols; therefore, this work reports its use, in conjunction with ketamine and midazolam, in a young Brazilian tapir. Case: A male Brazilian tapir, male, weighing 89 kg, 1 and a half year old, housed at CETAS in Rio Branco, Acre, was chemically restrained with dexmedetomidine (7 µg/kg), ketamine (1.5 mg/kg), and midazolam (0.2 mg/kg) for venous blood collection, oral and rectal mucosal swabs, and microchipping. The protocol was administered intramuscularly to the right triceps brachii, after physical restraint. After 5 min of application, the animal assumed sternal recumbency and presented reflux. After 15 min, the patient was placed in the right lateral decubitus position. During collection, heart rate (48 ± 10 bpm), respiratory frequency (29 ± 1 mpm), rectal temperature (38.1 ± 0.18°C), oxyhemoglobin saturation (97 ± 1%), and electrocardiographic tracing were recorded. The tapir showed deep sedation, immobility, good muscle relaxation, discreet medial palpebral reflex, and bilateral rotation of the eyeball. After 40 min of protocol administration, sedative reversal was performed intramuscularly with 14 µg/kg atipamezole. Five min after administration, the tapir showed signs of mild sedation. After 10 min, he assumed the quadrupedal position, remained in this position for 8 min, and gently resumed the sternal decubitus. After only 20 min, he resumed the quadrupedal position, with mild ataxia and good muscular and conscious tone. After 50 min, the patient was discharged from anesthesia. Discussion: Domestic horses are phylogenetically close to tapirs, so the choice of drugs and doses of the protocol used was based on their use in horses, and on studies carried out with tapirs as well. Despite being docile and passive, the tapir was not conditioned and did not allow the manipulation and collection of samples collaboratively; therefore, it was chemically contained. The physical restraint performed did not generate satisfactory immobilization of the tapir, resulting in agitation and stress and causing the needle to break. The reflux presented by the tapir minutes after sedation and at recovery was induced by dexmedetomidine, and only the undigested banana pieces were offered to the animal. Reflux plus stress from extensive fasting and suboptimal physical restraint was responsible for the change in the tapir's eating behavior, with possible stress gastritis 24 h after chemical restraint. Only one study reported the use of dexmedetomidine in tapirs, associated with continuous infusions of ketamine, midazolam and guaiacol glyceryl ether for moderate to long-term field procedures. Sedative reversal of dexmedetomidine by atipamezole reduced the recovery time and the risk of death from cardiorespiratory depression. The anesthetic combination used was effective, promoting immobility, muscle relaxation, and stability of the physical parameters evaluated, with rapid and gentle induction and an adequate level of sedation for the objective, good sedative reversal, and anesthetic recovery.

Purpose: To analyze the effect and mechanism of dexmedetomidine (DEX) analgesia pretreatment on functional chronic visceral pain in rats. Methods: Rats were divided into six groups: W1, W2, W3, W4, W5, and W6. The behavioral changes and electrophysiological indexes of rats in each group before and after DEX treatment were detected. Results: The levels of abdominal withdrawal reflex (AWR) in W5 and W6 groups were significantly lower than those in group W3, while the levels of thermal withdrawal latency (TWL) and mechanical withdrawal threshold (MWT) were significantly higher than those in group W3 (p < 0.05). The electromyographic signals of W1, W5, and W6 groups showed little fluctuation, while those of groups W2, W3, and W4 showed obvious fluctuation. TLR4 mRNA expression, IRF3, P65, and phosphorylation levels in W4, W5, and W6 groups were significantly lower than those in group W2 (p < 0.05). Conclusions: Dexmedetomidine epidural anesthesia pretreatment could significantly inhibit visceral pain response in rats with functional chronic visceral pain, and its mechanism was related to the activation of TLR4 in spinal dorsal horn tissue of rats and the activation inhibition of IRF3 and P65 in the downstream key signals.

Objetivou-se avaliar os efeitos cardiorrespiratórios da dexmedetomidina isolada e associada à morfina em pacientes submetidas à anestesia geral inalatória com isoflurano e submetidas à ovariohisterectomia eletiva. Vinte cadelas saudáveis foram selecionadas a partir de exames físico e laboratoriais. Os animais foram distribuídos aleatoriamente em dois grupos: dexmedetomidina (10 µg/Kg) (GD) e dexmedetomidina (10 µg/Kg) associado com morfina 0,3 mg/Kg (GDM), administrados pela via intramuscular (IM). Ato contínuo, o paciente foi induzido com propofol (à efeito) e mantido com isoflurano diluído em oxigênio 100% e administrado através de vaporizador calibrado. Foram aferidos os parâmetros fisiológicos: frequências cardíaca (FC) e respiratória (f), temperatura corporal (T°C), pressões arteriais sistólica (PAS), diastólica (PAD) e média (PAM), saturação de oxigênio na hemoglobina (SpO2), pressão parcial de gás carbônico no final da expiração (ETCO2) e isoflurano expirado (ETISO). A hemogasometria arterial foi utilizada para a análise do potencial hidrogeniônico (pH), bicarbonato (HCO3-) e pressão parcial de dióxido de carbono (PaCO2). O procedimento cirúrgico teve duração máxima de 45 minutos. Os parâmetros demonstraram diferença estatística entre os tempos e grupos avaliados, de acordo com os testes de Tukey e Bonferroni (p<0,05). Observou-se bradicardia 30 minutos após a aplicação da medicação pré-anestésica e acidemia no período trans-anestésico. Apesar disto, tanto a dexmedetomidina isolada quanto associada, conferiram estabilidade hemodinâmica e respiratória, apesar da bradicardia e acidemia observada.(AU)


This study aimed to evaluate the cardiorespiratory effects of dexmedetomidine alone and associated with morphine. Twenty healthy dogs were selected from physical and laboratory exams. The animals were randomly distributed into two groups: 10 µg/Kg dexmedetomidine (DG) and 10 µg/Kg dexmedetomidine group associated with 0.3 mg/Kg morphine (GDM), administered intramuscularly (IM). The patients were induced with propofol (to effect) and maintained with isoflurane diluted in 100% oxygen and administered through a calibrated vaporizer. Physiological parameters were measured: heart rate (HR), respiratory rate (f), body temperature (T°C), systolic arterial pressure (SAP), diastolic arterial pressure (DAP) and mean arterial pressure (MAP), oxygen saturation of hemoglobin (SpO2), end-tidal carbon dioxide partial pressure (ETCO2) and end-tidal isoflurane concentration (ETISO). Arterial blood gas analysis was used to analyze hydrogen potential (pH), bicarbonate (HCO3-) and partial pressure of carbon dioxide (PaCO2). The surgical procedure lasted up to 45 minutes. The parameters showed a statistical difference between the times and groups evaluated, according to the Tukey and Bonferroni tests (p<0.05). Bradycardia was observed 30 minutes after the application of pre-anesthesic medication and acidemia in the trans-anesthetic period. Despite this, both dexmedetomidine alone and associated, provided good hemodynamic and respiratory stability, despite the bradycardia and acidemia observed.(AU)


Este estudio tuvo como objetivo evaluar los efectos cardiorrespiratorios de la dexmedetomidina sola y asociada con la morfina en pacientes sometidas a anestesia general por inhalación con isoflurano y sometidas a ovariohisterectomía electiva. Se seleccionaron veinte perras sanas a partir de exámenes físicos y de laboratorio. Los animales se dividieron aleatoriamente en dos grupos: dexmedetomidina (10 µg/kg) (GD) y dexmedetomidina (10 µg/kg) asociada con 0,3 mg/kg de morfina (GDM), administrada por vía intramuscular (IM). Posteriormente, el paciente fue inducido con propofol (en efecto) y mantenido con isoflurano diluido en oxígeno al 100% y administrado mediante de un vaporizador calibrado. Se midieron parámetros fisiológicos: frecuencia cardíaca (FC) y respiratoria (f), temperatura corporal (T°C), presión arterial sistólica (PAS), diastólica (PAD) y media (PAM), saturación de oxígeno de hemoglobina (SpO2), presión parcial de carbono dióxido al final de la expiración (ETCO2) e isoflurano expirado (ETISO). Se utilizó gasometría arterial para analizar el potencial de hidrógeno (pH), el bicarbonato (HCO3-) y la presión parcial de dióxido de carbono (PaCO2). El procedimiento quirúrgico tuvo una duración máxima de 45 minutos. Los parámetros mostraron diferencia estadística entre los tiempos y grupos evaluados, según las pruebas de Tukey y Bonferroni (p <0.05). Se observó bradicardia 30 minutos después de la aplicación de medicación preanestésica y acidemia en el período transnestésico. A pesar de esto, tanto la dexmedetomidina aislada como la asociada proporcionaron estabilidad hemodinámica y respiratoria, a pesar de la bradicardia y acidemia observadas.(AU)

Resumo

Os bloqueios locorregionais vêm sendo cada vez mais estudados na medicina veterinária, principalmente em pets não-convencionais e animais silvestres. O bloqueio do plano transverso do abdômen (TAP block) é uma técnica de anestesia locorregional capaz de promover anestesia e analgesia em regiões da pele, musculatura e peritônio parietal, as quais fazem parte da estratégia de analgesia multimodal. O objetivo deste trabalho é relatar o bloqueio do plano transverso em um coelho doméstico de quatro anos de idade submetido à mastectomia regional e ovariohisterectomia. Foram realizados dois pontos bilateralmente de bloqueio do espaço TAP guiado por ultrassom com 2mg/kg de bupivacaína a 0,25%, padronizando um volume total injetado de 2,4mL. Foram utilizados dexmedetomidina (5É¥g/kg) e midazolam (0,5mg/kg) por via intramuscular como medicação pré-anestésica, indução com isoflurano na máscara de oxigênio 100% e manutenção por anestesia inalatória com isoflurano. Os parâmetros fisiológicos mantiveram-se estáveis, sem necessidade de resgate analgésico, com extubação rápida e despertar tranquilo. Desta forma, conclui-se que o TAP block foi eficaz na analgesia para mastectomia regional e ovariohisterectomia como protocolo multimodal, aumentando o índice de segurança, além de ser de fácil execução mesmo com transdutores de baixa frequência.


The locoregional blocks have been increasingly studied in veterinary medicine, especially in non-conventional pets and wild animals. The transverse abdominis plane block (TAP block) is a locoregional anesthesia technique capable of promoting anesthesia and analgesia in the skin, muscle, and parietal peritoneum regions, which are is part of the multimodal analgesia strategy. This study aims to report the transverse plane block in a four-year-old domestic rabbit submitted to regional mastectomy and ovariohysterectomy. Two ultrasound-guided TAP space blocks were performed bilaterally with 2mg/kg of bupivacaine 0.25%, standardizing a total injected volume of 2.4mL. Dexmedetomidine (5É¥g/kg) and midazolam (0.5mg/kg) were used intramuscularly as pre-anesthetic medication, induction with isoflurane in a 100% oxygen mask, and maintenance by inhalation anesthesia with isoflurane. Physiological parameters remained stable, with no need for analgesic rescue, rapid extubation, and peaceful awakening. Thus, it is concluded that the TAP block was effective in analgesia for regional mastectomy and ovariohysterectomy as a multimodal protocol, increasing the safety index. Moreover, it is easy to perform even with low-frequency transducers.

Purpose: To determine whether dexmedetomidine aggravates hemodynamic, metabolic variables, inflammatory markers, and microcirculation in experimental septic shock. Methods: Twenty-four pigs randomized into: Sham group (n = 8), received saline; Shock group (n = 8), received an intravenous infusion of Escherichia coli O55 (3 × 109 cells/mL, 0.75 mL/kg, 1 hour); Dex-Shock group (n = 8), received bacteria and intravenous dexmedetomidine (bolus 0.5 mcg/kg followed by 0.7 mcg/kg/h). Fluid therapy and/ornorepinephrine were administered to maintain a mean arterial pressure > 65 mmHg. Hemodynamic, metabolic, oxygenation, inflammatory markers, and microcirculation were assessed at baseline, at the end of bacterial infusion, and after 60, 120, 180, and 240 minutes. Results: Compared to Shock group, Dex-Shock group presented a significantly increased oxygen extraction ratio at T180 (23.1 ± 9.7 vs. 32.5 ± 9.2%, P = 0.0220), decreased central venous pressure at T120 (11.6 ± 1 vs. 9.61 ± 1.2 mmHg, P = 0.0214), mixed-venous oxygen saturation at T180 (72.9 ± 9.6 vs. 63.5 ± 9.2%, P = 0.026), and increased plasma lactate (3.7 ± 0.5 vs. 5.5 ± 1 mmol/L, P = 0.003). Despite the Dex-Shock group having a better sublingual vessel density at T240 (12.5 ± 0.4 vs. 14.4 ± 0.3 mL/m2; P = 0.0003), sublingual blood flow was not different from that in the Shock group (2.4 ± 0.2 vs. 2.4 ± 0.1 mL/kg, P = 0.4418). Conclusions: Dexmedetomidine did not worsen the hemodynamic, metabolic, inflammatory, or sublingual blood flow disorders resulting from septic shock. Despite inducing a better sublingual vessel density, dexmedetomidine initially and transitorily increased the mismatch between oxygen supply and demand.

Anesthetic protocols have been developed to obtain the most effective and safe association in wildlife. This study compared the anesthetic effects and cardiorespiratory parameters of ketamine-S (+) (10 mg/kg)/dexmedetomidine (0.020 mg/kg) (KD ) and ketamine-S (+) (10 mg/kg)/midazolam (0.5 mg/kg)/methadone (1.0 mg/kg) (KMM ) in capuchin monkeys (Sapajus apella). Eight capuchin monkeys were randomly assigned to KD (n = 4) or KMM (n = 4) to evaluate induction, immobilization, and recovery scores, heart and respiratory rate parameters, besides systolic, mean, diastolic arterial pressure and arterial blood gas. There was no difference (P = 0.56) in the quality of induction, immobilization, and anesthetic recovery between the protocols. The time for anesthetic induction was 4 ± 1 min in the KD group and 5 ± 1 min in the KMM group, and these values were statistically equal (P = 0.28). The mean immobilization time in the KD and KMM groups were 35 ± 13 and 33 ± 15 min, respectively. Heart rate was lower in animals in the KD group (P < 0.001), while respiratory rate (P = 0.03), and mean blood pressure (P = 0.046) were higher than that of the animals in the KMM group. Respiratory acidosis occurred in the KMM group, with lower pH (7.25±0.047; P = 0.0055) and higher pCO2 (51 ± 6;mmHg; P = 0.008). Both protocols exhibited good induction quality, immobilization, and anesthetic recovery, despite cardiorespiratory and blood gas alterations observed, which warrants monitoring of cardiorespiratory variables during KD or KMM chemical restraint.


Protocolos anestésicos foram desenvolvidos para obter a associação mais eficaz e segura em animais selvagens. O objetivo deste estudo foi comparar os efeitos anestésicos e os parâmetros cardiorrespiratórios de cetamina-S (+) (10 mg / kg) / dexmedetomidina (0,020 mg / kg) (KD) e cetamina-S (+) (10 mg / kg) / midazolam (0,5 mg / kg) / metadona (1,0 mg / kg) (KMM) em macacos-prego (Sapajus apella). Oito macacos-prego foram distribuídos aleatoriamente em KD (n = 4) ou KMM (n = 4) para avaliar os escores de indução, imobilização e recuperação, parâmetros de frequência cardíaca e respiratória, além da pressão arterial sistólica, média, diastólica e gasometria arterial. Não houve diferença (P = 0,56) na qualidade da indução, imobilização e recuperação anestésica entre os protocolos. O tempo de indução anestésica foi de 4 ± 1 min no grupo KD e 5 ± 1 min no grupo KMM, sendo esses valores estatisticamente iguais (P = 0,28). O tempo médio de imobilização nos grupos KD e KMM foram 35 ± 13min e 33 ± 15 min, respectivamente. A frequência cardíaca foi menor nos animais do grupo KD (P < 0,001), enquanto a frequência respiratória (P = 0,03) e a pressão arterial média (P = 0,046) foram maiores do que nos animais do grupo KMM. Acidose respiratória ocorreu no grupo KMM, com menor pH (7,25 ± 0,047; P = 0,0055) e maior pCO2 (51 ± 6; mmHg; P = 0,008). Ambos os protocolos apresentaram boa qualidade de indução, imobilização e recuperação anestésica, apesar das alterações cardiorrespiratórias e gasométricas observadas, o que justifica o monitoramento das variáveis cardiorrespiratórias durante a contenção química com KD ou KMM.

Objetivou-se avaliar a eficácia analgésica pós-operatória da dexmedetomidina isolada e associada à morfina em pacientes submetidas à anestesia geral inalatória com isoflurano e submetidas à ovariohisterectomia eletiva (OH). Vinte cadelas saudáveis foram selecionadas a partir de exames físico e laboratoriais. Previamente ao estudo, foram submetidas a um período de 24 horas de adaptação ao ambiente e aos observadores. Ato contínuo, foram divididas aleatoriamente em dois grupos: grupo dexmedetomidina, na dose de 10 μg/Kg (GD) e grupo dexmedetomidina (10 μg/Kg) associado com morfina, na dose de 0,3 mg/Kg (GDM), administrados pela via intramuscular (IM). Ato contínuo, as pacientes foram induzidas com propofol e mantidas com isoflurano diluído em oxigênio 100% e administrado através de vaporizador calibrado. Para verificação da analgesia pós-operatória foram realizadas avaliações de dor por meio de escalas de Glasgow Modificada (EGM) e de Melbourne (EM). Para avaliação de sedação, por meio da escala de Dobbins (ED), em diferentes tempos: antes da administração da MPA (T0), e em mais 6 tempos no período pós-operatório, uma (T1), duas (T2), quatro (T3), oito (T4), 12 (T5) e 24 (T6) horas após a extubação orotraqueal. Não foram observadas diferenças estatísticas entre os grupos nas escalas de acordo com o teste de Kruskal-Wallis post-hoc de Dunn; e entre os tempos notou-se diferença estatística pelo teste de Friedman (p<0,05) para EGM e EM, porém não houve necessidade de resgate analgésico. Portanto, conclui-se que a dexmedetomidina isolada e associada a morfina produziu efeito analgésico adequado no período pós-operatório de cadelas submetidas a OH.


This study aimed to evaluate the postoperative analgesic efficacy of dexmedetomidine alone and when associated with morphine in patients under general inhalational anesthesia with isoflurane and undergoing elective ovariohysterectomy (OH). Twenty healthy bitches were selected via physical and laboratory examinations. Prior to the study, they underwent a 24-h period of adaptation to the environment and observers. They were then randomly divided into two groups: the dexmedetomidine group receiving a dose of 10 μg/kg, and dexmedetomidine group (10 μg/kg) associated with morphine receiving a dose of 0.3 mg/kg, administered via the intramuscular route. Thereafter, patients were induced with propofol and maintained with isoflurane diluted in 100% oxygen administered through a calibrated vaporizer. To verify postoperative analgesia, pain assessments were performed using the modified Glasgow (EGM) and Melbourne (EM) scales. For sedation assessment, the Dobbins scale was used at different times: before the administration of pre-anesthetic medication (T0) and at another six times in the postoperative period, 1 (T1), 2 (T2), 4 (T3), 8 (T4), 12 (T5), and 24 (T6) h after orotracheal extubation. No statistical differences were observed between groups in the scales according to Dunn’s Kruskal-Wallis post hoc test, and between the times a statistical difference was noticed by the Friedman test (p<0.05) for the EGM and EM scale scores, but there was no need for analgesic rescue. Therefore, we found that isolated dexmedetomidine and morphine produced adequate analgesic effects in the postoperative period of bitches submitted for OH.

Background: The brown bear (Ursus arctos) is considered one of the largest terrestrial carnivores, native from temperate forest regions of North America, Europe and Asia. In Brazil, they are founded in captivity and their safe capture and immobilization are obtained with one effective anesthesia for management and surgical and diagnostic procedures. Some anesthetic protocols are described for these purposes, however, there is a lack for data on the anesthetic and adverse effects they have on bears when used. The aim of this case is to report the use and effects of the association of dexmedetomidine with tiletamine and zolazepam in the chemical containment of a captive adult brown bear. Case: A 33-year-old female brown bear, weighing 100 kg, belonging to the Zoobotanic Park of Teresina, Piauí, Brazil, was chemically immobilized for an incisional biopsy of a cutaneous nodule with 1.0 cm diameter in the right face region. The anesthetic protocol included 6 µg/kg of dexmedetomidine associated with 3 mg/kg of tiletamine and zolazepam, administered intramuscularly by dart into the gluteal region of the right pelvic limb. The animal showed moderate ataxia at 5 min and assumed sternal decubitus 7 min after anesthetic administration. The bear's degree of sedation was considered adequate and safe to perform the biopsy at 10 min after administration. Heart rate (47 ± 3 bpm), respiratory rate (17 ± 2 mpm) and rectal temperature (38.7 ± 0.1ºC) were monitored. The bear remained immobile and unconscious throughout the procedure, with intense muscle relaxation, bilateral eyeball rotation, absent lateral palpebral reflex and mild medial reflex and without nystagmus. Complementary sedative doses were not necessary. At the beginning of anesthetic recovery, the bear received 6 µg/kg of atipamezole, intramuscularly. After 25 min of administration of atipamezole, the animal showed signs of recovery in the level of consciousness and reactivity to external stimuli, and assumed the quadrupedal position at 60 min after reverser application. Discussion: Even in captivity, the bears behavior is unpredictable and attack can occur, causing trauma or death to people. For this, the chemical immobilization is important to keep safety of everyone. This procedure was performed using blowgunassisted darts thrown by a staff member who had experience in using this method, who darted accurately and effectively. The latency time observed after administration of the anesthetic protocol used is similar to reported in other studies with bears that also received intramuscular dexmedetomidine and tiletamine and zolazepam and showed intense muscle relaxation and immobility. The doses used contributed to the absence of bradycardia and hypoventilation and, performing the procedure in the morning, when the temperature is milder in the city, minimized the chance of hyperthermia and thermal stress in the animal, not requiring body cooling. The use of dexmedetomidine in chemical containment protocols for short and minimally invasive procedures allows the subsequent use of its antagonist, atipamezole, contributing to a shorter recovery time, return of the animal's degree of consciousness and lower incidence of ataxia after assuming a quadrupedal position. The anesthetic protocol used was considered efficient, providing a quick and gentle chemical containment, adequate anesthetic plan and good anesthetic recovery in an adult brown bear from captivity.

Background: The anatomical, physiological, and pharmacological characteristics of reptiles make anesthesia in chelonians particularly challenging. Specific literature regarding safe anesthetic protocols that provide immobilization, antinociception, amnesia, and unconsciousness are scarce. Thus, this paper aims to report the case of a red-footed tortoise submitted to long-duration general anesthesia to celiotomy for foreign body removal. Case: An adult red-footed tortoise (Chelonoides carbonaria), 5.9 kg, was admitted due to hyporexia after ingesting a metallic fishhook. Serial radiographs confirmed the diagnosis and location of the foreign body in the stomach. The animal was premedicated with 0.03 mg/kg dexmedetomidine, 6 mg/kg ketamine, and 0.4 mg/kg butorphanol intramuscularly. After 90 min we inserted a 22G jugular catheter and proceeded to anesthesia induction with 5 mg/kg propofol. We intubated the animal with a 2.5 mm uncuffed endotracheal and started fluid therapy at a rate of 5 mL/kg/h. Surgical anesthesia was maintained with isoflurane in 0.21 oxygen, in a non-rebreathing circuit (baraka), under spontaneous breathing. Expired isoflurane was maintained between 3 and 4.5%. Due to reduced respiratory rate and hypercapnia, we opted for implementing manually-assisted positive pressure ventilation. Morphine (0.5 mg/kg) was administered at 10 and 87 min after the beginning of the surgery for further analgesia when the isoflurane requirement increased significantly. We did not detect any alterations in heart and body temperature. Surgical anesthesia lasted 6 h. During anesthesia recovery, voluntary head retraction and coordinated movement of the limbs occurred at 240 and 540 min after the extubation, respectively. In 2 days, the patient returned to voluntary feeding, being very active and responsive to stimulus. The post-surgical hematologic evaluation was unremarkable. Discussion: Pre-anesthetic medication aimed to promote sedation and preemptive analgesia. Due to its minimal cardiorespiratory depression, we chose the combination of ketamine, dexmedetomidine, and butorphanol. Dexmedetomidine reduced the ketamine dose and caused sufficient muscle relaxation and immobilization to perform the jugular catheter placement. Butorphanol is an agonist-antagonist opioid; that is why we decided to add it to the protocol for antinociception. However, due to signs of nociceptive response (increased isoflurane requirements and heart rate), and considering the evidence of a predominance of µ receptors in reptiles, we administered low-dose morphine twice during the procedure. Propofol was chosen as an induction agent at a dose sufficient to allow endotracheal intubation. Since reptilians often show apnea in the presence of 100% oxygen, we used a 0.21 oxygen fraction. Despite this, the patient showed respiratory depression. Due to right to left cardiac shunt, sudden changes in the direction of the blood can lead to very rapid changes in the serum concentrations of isoflurane, which leads to frequent oscillations in the anesthetic depth and consequently the need for vaporizer adjustments, which may justify the high expired isoflurane fraction during the procedure. Despite that, physiological parameters were maintained within normal ranges for the species, with slight variations during the surgical procedure. We conclude that the proposed anesthetic protocol is safe for long-duration anesthesia in chelonians, ensuring cardiovascular and respiratory stability. Thus, this report may help veterinarians to perform safe anesthesia in tortoises submitted to invasive surgical procedures.

Background: The spectacled bear (Tremarctos ornatus) is the only bear species inhabiting South America and is classified as vulnerable according to the International Union for Conservation of Nature (IUCN) Red List of Threatened Species. Among the few publications on the use of general anesthesia and advanced monitoring of ursids in veterinary hospital settings, little is described regarding chemical restraint, general anesthesia and monitoring of spectacled bears. This case series describes the use of a dexmedetomidine-tiletamine-zolazepam chemical restraint combination and its effects on cardiorespiratory variables and arterial blood gases observed in two spectacled bears undergoing isoflurane anesthesia for imaging and/or surgical procedures. Cases: Two female, one adult and one senile, all-term captive spectacled bears were referred to the Veterinary Teaching Hospital at the Universidade Estadual Paulista - Unesp, Botucatu campus, both with a presumable history of recent trauma. After immobilization with an intramuscular (IM) administration of tiletamine-zolazepam (3.8 - 4.3 mg/kg) and dexmedetomidine (6.4 - 7.6 µg/kg), induction of anesthesia was achieved by means of intravenous (IV) propofol (1 - 2 mg/kg). After orotracheal intubation animals underwent isoflurane anesthesia under mechanical ventilation through the remainder of the procedures. Initial settings of inspiratory flow rate were adjusted to obtain peak airway pressure (Ppeak ) of 10 cmH2 O and tidal volumes (Vt) of 10 mL/kg, as well as respiratory rates (ƒR) and inspiration-to-expiration (I:E) ratio of 10 breaths/min and 1:2, respectively, and were then adjusted throughout anesthesia to maintain normocapnia (end-tidal carbon dioxide concentrations between 35 and 45 mmHg). Purpose: To evaluate the protective effect of dexmedetomidine on gastric injury induced by ischemia reperfusion (I/R) in rats. Methods: A total of 18 male albino Wistar rats were divided groups as: gastric ischemia reperfusion (GIR), gastric ischemia reperfusion and 50 g/kg dexmedetomidine (DGIR) and sham operation (HG) group. After the third hour of reperfusion, the biochemical and histopathological examinations were performed on the removed stomach tissue. Results: Malondialdehyde (MDA) and myeloperoxidase (MPO) levels were found to be significantly higher in GIR compared to HG (p 0.05). A statistically significant decrease was observed at the DGIR compared to the GIR for oxidants levels. Total glutathione (tGSH) and superoxide dismutase (SOD) levels were statistically significantly decreased at the GIR, and antioxidants levels were found to be significantly higher in the DGIR (p 0.05) There was no significant difference between HG and DGIR in terms of SOD (p = 0.097). The DGIRs epitheliums, glands and vascular structures were close to normal histological formation. Conclusions: Dexmedetomidine is found to prevent oxidative damage on the stomach by increasing the antioxidant effect. These results indicate that dexmedetomidine may be useful in the treatment of ischemia-reperfusion-related gastric damage.(AU)

This study aimed to evaluate the postoperative analgesic efficacy of dexmedetomidine alone and when associated with morphine in patients under general inhalational anesthesia with isoflurane and undergoing elective ovariohysterectomy (OH). Twenty healthy bitches were selected via physical and laboratory examinations. Prior to the study, they underwent a 24-h period of adaptation to the environment and observers. They were then randomly divided into two groups: the dexmedetomidine group receiving a dose of 10 µg/kg, and dexmedetomidine group (10 µg/kg) associated with morphine receiving a dose of 0.3 mg/kg, administered via the intramuscular route. Objetivou-se avaliar a eficácia analgésica pós-operatória da dexmedetomidina isolada e associada à morfina em pacientes submetidas à anestesia geral inalatória com isoflurano e submetidas à ovariohisterectomia eletiva (OH). Vinte cadelas saudáveis foram selecionadas a partir de exames físico e laboratoriais. Previamente ao estudo, foram submetidas a um período de 24 horas de adaptação ao ambiente e aos observadores. Ato contínuo, foram divididas aleatoriamente em dois grupos: grupo dexmedetomidina, na dose de 10 µg/Kg (GD) e grupo dexmedetomidina (10 µg/Kg) associado com morfina, na dose de 0,3 mg/Kg (GDM), administrados pela via intramuscular (IM). Ato contínuo, as pacientes foram induzidas com propofol e mantidas com isoflurano diluído em oxigênio 100% e administrado através de vaporizador calibrado. Para verificação da analgesia pós-operatória foram realizadas avaliações de dor por meio de escalas de Glasgow Modificada (EGM) e de Melbourne (EM). Para avaliação de sedação, por meio da escala de Dobbins (ED), em diferentes tempos: antes da administração da MPA (T0), e em mais 6 tempos no período pós-operatório, uma (T1), duas (T2), quatro (T3), oito (T4), 12 (T5) e 24 (T6) horas após a extubação orotraqueal. Não foram observadas diferenças estatísticas entre os grupos nas escalas de acordo com o teste de Kruskal-Wallis post-hoc de Dunn; e entre os tempos notou-se diferença estatística pelo teste de Friedman (p<0,05) para EGM e EM, porém não houve necessidade de resgate analgésico. Portanto, conclui-se que a dexmedetomidina isolada e associada a morfina produziu efeito analgésico adequado no período pós-operatório de cadelas submetidas a OH.

Após exame radiográfico no membro pélvico direito, foi constatada fratura exposta grau I, oblíqua curta, da tíbia e fíbula. Após limpeza e imobilização temporária do membro com tala, o animal foi submetido à cirurgia de osteossíntese tibial. O protocolo de medicação pré-anestésica incluiu dexmedetomidina associado à morfina. A indução anestésica foi realizada com propofol e cetamina por via intravenosa. Foi realizado ainda bloqueio epidural com lidocaína. Durante o procedimento cirúrgico foi aplicada placa bloqueada na função de apoio e cinco parafusos bloqueados. Os cuidados pós-operatórios farmacológicos envolveram administração de antibióticos, anti-inflamatórios e analgésicos opioides. Após três dias, o membro retornou à sua funcionalidade. O paciente permaneceu internado 45 dias pós-operatórios para recuperação com os cuidados veterinários. Concluiu-se que o método de fixação da fratura, bem como os protocolos sedativos e anestésicos empregados, são aplicáveis para a espécie citada.

After cleaning and temporary immobilization of the limb with a splint, the animal was submitted to tibial osteosynthesis surgery. The pre-anesthetic medication protocol included dexmedetomidine associated with morphine. Anesthetic induction was performed with propofol and ketamine intravenously. Epidural blocking was performed with lidocaine. During the surgical procedure, a blocked plate was applied to the support function and five blocked screws. Postoperative pharmacological care involved administration of antibiotics, anti-inflammatory drugs and opioid analgesics. After three days, the limb returned to functionality. The patient remained hospitalized 45 days postoperatively for recovery under veterinary care. It was concluded that the fracture fixation method, as well as the sedative and anesthetic protocols used, are applicable for the mentioned species.

Resumo

A presente pesquisa comparou os efeitos cardiorrespiratórios, hemogasométricos e sedativos da associação midazolam (0,41mg/kg) e butorfanol (0,31mg/kg) acrescida de detomidina (157µg/kg) (DTMB) ou dexmedetomidina (36µg/kg) (DXMB) em catetos. Catetos adultos (n=20) foram distribuídos em dois grupos, DTMB ou DXMB. As variáveis (FC, f, PAM, SpO2, EtCO2 e TR) foram avaliadas após aplicação dos fármacos. A sedação foi avaliada por meio de escala analógica visual, relaxamento muscular, postura e resposta auditiva. Foi realizada ANOVA, seguida de teste t pareado (paramétricos) e teste de Mann-Whitney rank-sum test (não paramétricos), com P<0,05. Não foi observada diferença estatística entre os grupos para o período de latência. Observou-se diferença significativa entre grupos para as variáveis f, PAM e SpO2, com maiores valores para DTMB, e EtCO2, com maiores valores para DXMB. Os dois grupos apresentaram redução da FC e da concentração de lactato, bem como aumento da concentração de bicarbonato. A SpO2 permaneceu abaixo de 90%, durante todo o período experimental, nos dois grupos estudados. Os animais dos dois grupos apresentaram sedação profunda e relaxamento muscular máximo. Conclui-se que os dois protocolos testados proporcionaram adequada sedação, podendo ser indicados para contenção química de catetos adultos.(AU)

Collared peccaries (n= 20) were divided into two groups, either DTMB or DXMB. The variables (FC, f, PAM, SpO2, EtCO2 and TR) were evaluated after application of the drugs. Sedation was assessed by visual analogue scale, muscle relaxation, posture and auditory response. ANOVA followed by paired t-test (parametric) and Mann Whitney Rank Sum Test (non-parametric) with P< 0.05 were performed. No statistical difference was observed for the latency period. A significant increase was observed between groups for the variables f, PAM and SpO2 with higher values for DTMB and EtCO2 with higher values for DXMB. The two groups presented a reduction in HR and lactate concentration, and an increase in bicarbonate concentration. SpO2 remained below 90% throughout the experiment in both groups. The animals of the two groups presented deep sedation and maximum muscle relaxation. It is concluded that the two protocols tested provided adequate sedation and could be indicated for chemical containment of collared peccaries.(AU)

A presente pesquisa comparou os efeitos cardiorrespiratórios, hemogasométricos e sedativos da associação midazolam (0,41mg/kg) e butorfanol (0,31mg/kg) acrescida de detomidina (157µg/kg) (DTMB) ou dexmedetomidina (36µg/kg) (DXMB) em catetos. Catetos adultos (n=20) foram distribuídos em dois grupos, DTMB ou DXMB. As variáveis (FC, f, PAM, SpO2, EtCO2 e TR) foram avaliadas após aplicação dos fármacos. A sedação foi avaliada por meio de escala analógica visual, relaxamento muscular, postura e resposta auditiva. Foi realizada ANOVA, seguida de teste t pareado (paramétricos) e teste de Mann-Whitney rank-sum test (não paramétricos), com P<0,05. Não foi observada diferença estatística entre os grupos para o período de latência. Observou-se diferença significativa entre grupos para as variáveis f, PAM e SpO2, com maiores valores para DTMB, e EtCO2, com maiores valores para DXMB. Os dois grupos apresentaram redução da FC e da concentração de lactato, bem como aumento da concentração de bicarbonato. A SpO2 permaneceu abaixo de 90%, durante todo o período experimental, nos dois grupos estudados. Os animais dos dois grupos apresentaram sedação profunda e relaxamento muscular máximo. Conclui-se que os dois protocolos testados proporcionaram adequada sedação, podendo ser indicados para contenção química de catetos adultos.(AU)


The study compared the cardiorespiratory, hemogasometric and sedative effects of the combination of midazolam (0.41mg/kg) and butorphanol (0.31mg/kg) plus detomidine (157µg/kg) (DTMB) or dexmedetomidine (36µg/kg) (DXMB) in collared peccaries. Collared peccaries (n= 20) were divided into two groups, either DTMB or DXMB. The variables (FC, f, PAM, SpO2, EtCO2 and TR) were evaluated after application of the drugs. Sedation was assessed by visual analogue scale, muscle relaxation, posture and auditory response. ANOVA followed by paired t-test (parametric) and Mann Whitney Rank Sum Test (non-parametric) with P< 0.05 were performed. No statistical difference was observed for the latency period. A significant increase was observed between groups for the variables f, PAM and SpO2 with higher values for DTMB and EtCO2 with higher values for DXMB. The two groups presented a reduction in HR and lactate concentration, and an increase in bicarbonate concentration. SpO2 remained below 90% throughout the experiment in both groups. The animals of the two groups presented deep sedation and maximum muscle relaxation. It is concluded that the two protocols tested provided adequate sedation and could be indicated for chemical containment of collared peccaries.(AU)

Background: Several researches have shown the impacts of roads more directly to wildlife in Brazil. The crab-eating fox(Cerdocyon thous) is a frequent run over victim. Dissociative drugs are commonly used, but inhalation anesthesia is indicatedin cases of extensive and prolonged surgeries. Despite their similarity with domestic dogs, the literature is scarce regarding theassociation of new anesthetic techniques and protocols in wild canids. The aim of this paper was to report the viability of multimodal anesthesia in a crab-eating fox, victim of running over, undergoing hemilaminectomy and sacrococcygeal stabilization.Case: An adult male specimen of crab-eating fox was rescued after being run over and taken to a wild animal screening center.Physical examination showed superficial and deep pain, lack of support for the pelvic limbs and proprioception, increasedreflexes, and reduced tail mobility. Chemical restraint with intramuscular (IM) tiletamine-zolazepam (6.0 mg/kg) and morphine (0.5 mg/kg) was performed. Meloxicam (0.2 mg/kg IM) and enrofloxacin (5.0 mg/kg IM) were also administered. Theanimal was sequentially admitted to the veterinary hospital. Radiographic images showed compaction of the spinal columnof the T10 and T11 thoracic vertebrae and the sacrococcygeal region. Sixty min after chemical restraint, the anesthesia wassupplemented with IM tiletamine-zolazepam (4.5 mg/kg), and fluid therapy with 0.9% NaCl (10 mL/kg/h) was started. Ten minlater, intravenous propofol dose-effect (2.5 mg/kg) was administered and general anesthesia was maintained with isoflurane(FiO2 = 1.0). Thirty min after the induction of anesthesia, the animal was urdergoing hemilaminectomy and sacrococcygealstabilization. Constant rate infusions (CRI) of dexmedetomidine (0.5 μg/kg/h) and ketamine (0.6 mg/kg/h) were started. Resumo

Purpose: To evaluate protective effects of dexmedetomidine, calcitriol and their combination. Methods: Forty Wistar-albino rats were divided into 4 groups; group of Sham (Group Sham); group of dexmedetomidine (Group DEX); group of calcitriol (Group CAL) and group of dexmedetomidineandcalcitriol (Group DEX-CAL). Photographic analysis was used for macroscopic analysis and perfusion analyses were evaluated by scintigraphy. Additionally, tissue malondialdehyde (MDA) and total oxidant status (TOS) and total antioxidant activity (TAS) were recorded and oxidative stress index (OSI) was calculated. Each flap was assessed by histopathology. Results: Compared to Group Sham, the viable flap areas were higher in all treatment groups both by photographic image analyses and perfusion analyses (p 0.05). Group DEX-CAL had the highest viable flap percentage both in scintigraphic and photographic analyses; whereas Group Sham had the lowest viable flap percentage. Similarly, TAS and MDA levels were elevated and TOS levels were declined in all treatment groups compared to Group Sham (p 0.005). Histopathological analysis at flap demarcation zone confirmed neovascularization was significantly higher and edema, necrosis and inflammation were significantly lower in all treatment groups compared to Group Sham. Conclusion: The outcomes show that additional premedication with either dexmedetomidine or calcitriol or their combination reduces ischemia-reperfusion injury of flap area and show significant increase in the percentage of viable flap tissue.(AU)
