Resumo

Purpose: Acute mesenteric ischemia (AMI) is a condition in pediatric surgery that ranges from intestine necrosis to death. Ischemic postconditioning (IPoC) methods were developed to reduce the damage caused by revascularization. This study aimed to evaluate the efficacy of these methods in an experimental weaning rat model. Methods: Thirty-two 21-day-old Wistar rats were allocated into four groups according to the surgical procedure performed: control, ischemia-reperfusion injury (IRI), local (LIPoC) and remote IPoC (RIPoC). At euthanasia, fragments of the intestine, liver, lungs, and kidneys were submitted to histological, histomorphometric, and molecular analyses. Results: In the duodenum, intestines, and kidneys histological alterations promoted by IRI were reversed by remote postconditioning method. In the distal ileum, the histomorphometric alterations could be reversed by the postconditioning methods with more evident effects promoted by the remote method. The molecular analysis found that the levels of expression of Bax (proapoptotic) and Bcl-XL (antiapoptotic) genes in the intestine were increased by IRI. These alterations were equally reversed by the postconditioning methods, with more evident effects of the remote method. Conclusions: IPoC methods positively reduced the damage caused by IRI in weaning rats.

Background: Peritoneopericardial diaphragmatic hernia is a rare pathogenesis of congenital origin, which occurs due to a failure in the communication between the diaphragm and the pericardium during embryogenesis. Symptoms may be non-existent or non-specific, depending on the herniated organ involved and, in most cases, the diagnosis is incidental. Regarding the most indicated treatment, there are still divergences in the literature concerning the indication of conservative or surgical treatment. This study reports the case of a feline peritoneopericardial hernia, for which surgical correction was the treatment of choice. Case: A 3-month-old female kitten, no defined racial pattern, was referred to the Veterinary Hospital of the Federal Rural University of Pernambuco for treatment of a peritoneopericardial diaphragmatic hernia. This condition was diagnosed through radiographic examination after the patient having been submitted to pediatric castration and presented anesthetic complications in the transsurgery. Blood count, biochemical profile and Doppler echocardiogram were performed, which showed no significant changes. To obtain a better study and surgical planning, computed tomography was performed to observe the heart located cranially in the pericardial cavity. Caudally to the heart, hepatic parenchyma located in the pericardial cavity was observed; and hepatic vessels presenting slightly enlarged dimensions. These tomographic findings suggested peritoneopericardial diaphragmatic hernia; being the liver present in the pericardial cavity and signs of congestion in the hepatic parenchyma. Due to the likelihood of future worsening of the hernia, surgical correction was performed, with an abdominal midline incision in the preumbilical region to reposition the liver to its normal anatomy, followed by diaphragm reconstitution through a herniorrhaphy. After the surgical procedure, the patient was referred for observation in internment and, after 15 days, the skin sutures were removed. Complete correction of the hernial defect was observed on radiography performed 30 days after the surgical procedure. However, the examination showed the presence of deviation/ deformity in the topography of the sternum and costal cartilages, with slight cardiac displacement to the right hemithorax, suggesting the presence of pectus excavatum. Discussion: Peritoneopericardial diaphragmatic hernia is considered rare and, despite being one of the most common causes of congenital pericardial anomaly in felines, it has a low prevalence ranging from 0.06% to 1.45%. They are usually diagnosed from two years of age, with prevalence for older animals. However, due to having presented anesthetic changes, the patient of this case report could be diagnosed early. Among the most common organs that migrate to the thoracic cavity, the liver is the most commonly observed, which is also the hernia content of the present report. Peritoneopericardial hernia is often diagnosed through radiography and ultrasound, and these imaging tests proved to be sufficient for the diagnosis in this report. However, computed tomography was important for providing a better study of case and for the adoption of median celiotomy as a treatment. Associations with other malformations are described in the literature, with pectus excavatum being the most common and also observed in this report. Peritoneopericardial diaphragmatic hernia is a rare anomaly, rarely reported in the literature and with divergences regarding its treatment. The adoption of early surgical treatment performed in this report showed satisfactory evolution and the possibility of a favorable prognosis.

The Amazon basin is one of the seven major geographical areas where scorpionism is recorded. In French Guiana, 90 stings per 100,000 inhabitants are registered per year. As the severity of cases is higher in children, descriptive studies are needed to have a better understanding of this pathology. The aim of the present study is to describe pediatric scorpionism in French Guiana. Methods: We conducted a monocentric descriptive retrospective study on scorpion stings in all pediatric patients admitted to Cayenne General Hospital from January 1, 2002 to December 31, 2018. Results: In this survey, 132 patients were included. Of them, 63% were male. Patients with general signs of envenomation were younger and lighter (p = 0.04). The picture was "one sting" (95.3%) by a "big" (47.6%), "black" (60%) and "small pincer" (58%) scorpion on the extremity of the body (84%). Stings occurred mainly during the day, while patients changed clothes. There was no envenomation during night. The monthly evaluation highlights that the number of stings and percentage of general signs of envenomation were closely connected to a composite variable including the variation of the level of rivers (p = 0.005). Cardiac symptoms were recorded in 82% of cases with general signs of envenomation. The presence of pulmonary; ear, nose, and throat (ENT); or gastrointestinal symptoms are related to major envenomation (p = 0.001, p = 0.01, and p = 0.02 respectively). Leukocytosis and glycemia increased according to the envenomation grade whereas serum potassium and alkaline reserve decreased. Forty-six patients needed hospitalization and seven of them required intensive care. No patient died nor presented sequelae at discharge from the hospital. Conclusion: Pediatric scorpionism in French Guiana is closely associated with child activities and climatic conditions. Severe envenomation presented most of the time with cardiac, pulmonary, and gastrointestinal symptoms.(AU)

ABSTRACT Background: Neuroblastoma is a pediatric tumor with a mortality rate of 40% in the most aggressive cases. Tumor microenvironment components as immune cells contribute to the tumor progression; thereby, the modulation of immune cells to a pro-inflammatory and antitumoral profile could potentialize the immunotherapy, a suggested approach for high-risk patients. Preview studies showed the antitumoral potential of BJcuL, a C- type lectin isolated from Bothrops jararacussu venom. It was able to induce immunomodulatory responses, promoting the rolling and adhesion of leukocytes and the activation of neutrophils. Methods: SK-N-SH cells were incubated with conditioned media (CM) obtained during the treatment of neutrophils with BJcuL and fMLP, a bacteria-derived peptide highly effective for activating neutrophil functions. Then we evaluated the effect of the same stimulation on the co-cultivation of neutrophils and SK-N-SH cells. Tumor cells were tested for viability, migration, and invasion potential. Results: In the viability assay, only neutrophils treated with BJcuL (24 h) and cultivated with SK-N-SH were cytotoxic. Migration of tumor cells decreased when incubated directly (p 0.001) or indirectly (p 0.005) with untreated neutrophils. When invasion potential was evaluated, neutrophils incubated with BJcuL reduced the total number of colonies of SK-N-SH cells following co-cultivation for 24 h (p 0.005). Treatment with CM resulted in decreased anchorage-free survival following 24 h of treatment (p 0.001). Conclusion: Data demonstrated that SK-N-SH cells maintain their migratory potential in the face of neutrophil modulation by BJcuL, but their invasive capacity was significantly reduced.

Resumo

Este trabalho visa, por meio de levantamento de dados científicos, analisar a melhor abordagem anestésica para cada tipo de paciente de risco, mostrando um panorama geral e indicativo sobre os fármacos e sua funcionalidade para cada sistema. A revisão bibliográfica foi baseada na literatura encontrada em meios eletrônicos (plataformas PubMed, Scielo e Google Scholar) e físicos (livros de referência) nos últimos dez anos, dentro de artigos com relatos de caso, revisões de literatura e pesquisas inéditas. Dessa forma, obteve-se um levantamento amplo, abrangendo as principais classes farmacológicas utilizadas em procedimentos anestésicos em pacientes classificados como de risco, ou seja, cardiopatas, nefropatas, hepatopatas, pediátricos e senis, procurando determinar os que causam menor depressão cardiovascular e respiratória respeitando suas limitações. Sendo assim, para uma anestesia mais segura deve-se focar na prevenção de intercorrências anestésicas no transoperatório e pós-operatório. Observar, estudar, analisar o paciente e o protocolo a ser utilizado como um todo é necessário e indispensável na anestesiologia veterinária, associando a teoria com a prática.

Thus, a broad survey was obtained, covering the main pharmacological classes used in anesthetic procedures in patients classified as at risk, that is, cardiopath, nephropath, liver disease, pediatric and senile patients, trying to determine those that cause less cardiovascular and respiratory depression respecting their limitations. Therefore, for safer anesthesia, focus must placed on preventing anesthetic complications in the intraoperative and postoperative periods. Observing, studying, analyzing the patient and the protocol to be used as a whole is necessary and indispensable in veterinary anesthesia, associating theory with practice.

Resumo

Este trabalho visa, por meio de levantamento de dados científicos, analisar a melhor abordagem anestésica para cada tipo de paciente de risco, mostrando um panorama geral e indicativo sobre os fármacos e sua funcionalidade para cada sistema. A revisão bibliográfica foi baseada na literatura encontrada em meios eletrônicos (plataformas PubMed, Scielo e Google Scholar) e físicos (livros de referência) nos últimos dez anos, dentro de artigos com relatos de caso, revisões de literatura e pesquisas inéditas. Dessa forma, obteve-se um levantamento amplo, abrangendo as principais classes farmacológicas utilizadas em procedimentos anestésicos em pacientes classificados como de risco, ou seja, cardiopatas, nefropatas, hepatopatas, pediátricos e senis, procurando determinar os que causam menor depressão cardiovascular e respiratória respeitando suas limitações. Sendo assim, para uma anestesia mais segura deve-se focar na prevenção de intercorrências anestésicas no transoperatório e pós-operatório. Observar, estudar, analisar o paciente e o protocolo a ser utilizado como um todo é necessário e indispensável na anestesiologia veterinária, associando a teoria com a prática.(AU)

Neuroblastoma is a pediatric tumor with a mortality rate of 40% in the most aggressive cases. Tumor microenvironment components as immune cells contribute to the tumor progression; thereby, the modulation of immune cells to a pro-inflammatory and antitumoral profile could potentialize the immunotherapy, a suggested approach for high-risk patients. Preview studies showed the antitumoral potential of BJcuL, a C- type lectin isolated from Bothrops jararacussu venom. It was able to induce immunomodulatory responses, promoting the rolling and adhesion of leukocytes and the activation of neutrophils. Methods: SK-N-SH cells were incubated with conditioned media (CM) obtained during the treatment of neutrophils with BJcuL and fMLP, a bacteria-derived peptide highly effective for activating neutrophil functions. Then we evaluated the effect of the same stimulation on the co-cultivation of neutrophils and SK-N-SH cells. Tumor cells were tested for viability, migration, and invasion potential. Results: In the viability assay, only neutrophils treated with BJcuL (24 h) and cultivated with SK-N-SH were cytotoxic. Migration of tumor cells decreased when incubated directly (p < 0.001) or indirectly (p < 0.005) with untreated neutrophils. When invasion potential was evaluated, neutrophils incubated with BJcuL reduced the total number of colonies of SK-N-SH cells following co-cultivation for 24 h (p < 0.005). Treatment with CM resulted in decreased anchorage-free survival following 24 h of treatment (p < 0.001). Conclusion: Data demonstrated that SK-N-SH cells maintain their migratory potential in the face of neutrophil modulation by BJcuL, but their invasive capacity was significantly reduced.(AU)

A Síndrome da Angústia Respiratória Aguda (SARA) é uma resposta inflamatória pulmonar excessiva, devido a diferentes tipos de agressões, que leva ao aumento da permeabilidade capilar pulmonar e sistêmica e a instalação de uma insuficiência respiratória aguda. As causas mais frequentes incluem sepses, pneumonia grave, peritonite e politraumatismos e a SARA tem sido observada na prática clínica, em animais atropelados apresentando índice significativo de contusão pulmonar, que evolui agressivamente em períodos relativamente curtos e o tratamento prontamente empregado deve ser o de suporte. Objetiva-se, com esse trabalho, revisar os aspectos gerais relacionados a SARA, como a etiologia e fisiopatologia e, especialmente, auxiliar o veterinário na identificação dessa doença e posterior tratamento.(AU)


The acute respiratory distress syndrome (ARDS) is an excessive pulmonary inflammatory response, due to different kinds of aggressions, which leads to increase in systemic and pulmonary capillary permeability and installation of acute respiratory failure. The most common causes include sepsis, severe pneumonia, peritonitis and polytraumatized patient and SARA has been observed in clinical practice, to run over animals with significant rate of pulmonary contusion, which aggressively evolves in relatively short periods andtreatment promptly employee should be to support. This work aims to review the general aspects related to Acute Respiratory Distress Syndrome, such as etiology and pathophysiology, and especially to assist the veterinarian in identifying this disease and subsequent treatment.(AU)

Background: Multiple cartilaginous exostosis (MCE) is a rare neoplastic disease in dogs that is characterized by the presence of osteochondromas in multiple bones. MCE is a disorder of development during growth and maturation, the realetiology of which has yet to be elucidated, but is believed to have a genetic basis. Usually affected animals have a historyof a palpable mass on the bone surface, causing anatomical deformities and compression of adjacent structures. SinceMCE is a rare neoplastic disease in dogs it may be difficult to recognize in the clinic. The aim of this study was to reporta case of MCE in a pediatric dog.Case: A female 4-month-old Rottweiler dog with a history of bone neoformation on the left hind limb, anorexia, weightloss, and difficulty walking received clinical care. The disease had its initial onset 2 months prior. At physical examination, the animal showed paresis of the hind limbs and a bony tumor on the lateral part of the left tibia. A complete bloodcell count and serum biochemical profile were obtained. The only abnormality diagnosed was hyperproteinemia due tohyperglobulinemia. Other diagnostic tests were not authorized and the animal was euthanized. At the necropsy, an intrathoracic neoformation was observed from the sixth to the eighth rib on the right antimer. Some tumors were also observedon the fourth lumbar vertebra and between the first and the second sacral vertebra, and a left tibial tumor, which had beenobserved at physical examination. The sagittal section of the spine revealed the presence of extradural compression of thespinal cord due to vertebral proliferations with compression of the nerve roots. All of the proliferations had macroscopicsimilarities, being firm, sessile, irregular, and with complete adherence to deep planes. The antitumor properties of ticks salivary gland extracts or recombinant proteins have been reported recently, but little is known about the antitumor properties of the secreted components of saliva. The goal of this study was to investigate the in vitro effect of the saliva of the hard tick Amblyomma sculptum on neuroblastoma cell lines. SK-N-SK, SH-SY5Y, Be(2)-M17, IMR-32, and CHLA-20 cells were susceptible to saliva, with 80% reduction in their viability compared to untreated controls, as demonstrated by the methylene blue assay. Further investigation using CHLA-20 revealed apoptosis, with approximately 30% of annexin-V positive cells, and G0/G1-phase accumulation (>60%) after treatment with saliva. Mitochondrial membrane potential (m) was slightly, but significantly (p 0.05), reduced and the actin cytoskeleton was disarranged, as indicated by fluorescent microscopy. The viability of human fibroblast (HFF-1 cells) used as a non-tumoral control decreased by approximately 40%. However, no alterations in cell cycle progression, morphology, and m were observed in these cells. The present work provides new perspectives for the characterization of the molecules present in saliva and their antitumor properties.(AU)


As propriedades antitumorais de extratos de glândulas salivares de carrapatos ou proteínas recombinantes foram relatadas recentemente, mas pouco se sabe sobre as propriedades antitumorais dos componentes secretados da saliva. O objetivo deste estudo foi investigar o efeito in vitro da saliva bruta do carrapato duro Amblyomma sculptum sobre as linhagens celulares de neuroblastoma. Células SK-N-SK, SH-SY5Y, Be(2)-M17, IMR-32 e CHLA-20 foram suscetíveis à saliva, com redução de 80% na sua viabilidade em comparação com controles não tratados, como demonstrado pelo ensaio de Azul de Metileno. Investigações posteriores utilizando CHLA-20 revelaram apoptose, com aproximadamente 30% de células positivas para anexina-V, e G0/G1 (> 60%) após tratamento com saliva. O potencial de membrana mitocondrial (m) foi reduzido significativamente (p 0,05), e o citoesqueleto de actina foi desestruturado, como indicado pela microscopia de fluorescência. A viabilidade do fibroblasto humano (células HFF-1), usado como controle não tumoral, diminuiu em aproximadamente 40%. No entanto, não foram observadas alterações na progressão do ciclo celular, morfologia e m nestas células. O presente trabalho fornece novas perspectivas para a caracterização das moléculas presentes na saliva e suas propriedades antitumorais.(AU)

Resumo

Purpose: To develop a new low-cost, easy-to-make and available training model using chickens intestine for infant intestinal anastomosis.Methods: Segments of chicken intestine were used to create an intestinal anastomosis simulator. We tried to perform an end-to-end, end-to-side and side-to-side anastomosis. Handsewn sutured anastomosis were performed in single layered with interrupted prolene 5-0 suture. The parameters analyzed were cost, intestines diameter and length, anastomosis patency and flow-through and leakage amount. Results: In all cases it was possible to make the anastomosis in double layered without difficulties, different from the usual ones. There was a positive patency at all anastomoses after the end of the procedure, with no need for reinterventions. Conclusion: The new training model using chickens intestine for infant intestinal anastomosis is low-cost, easy-to-make and easy available.(AU)

This paper aimed to determine arterial partial pressure of carbon dioxide (PaCO2), end-expired CO2 pressure (ETCO2), and the difference between arterial and end-expired CO2 pressure (Pa - ETCO2) in prepubescent and adult bitches undergoing videolaparoscopic or conventional ovariohyterectomy (OH). Forty bitches were randomly assigned to four groups: Conventional Adult (CA), Conventional Pediatric (CP), Videolaparoscopic Adult (VA) and Videolaparoscopic Pediatric (VP). Pulse rate (PR), respiratory rate (RR), systolic, mean, and diastolic arterial pressures (SAP, MAP, DAP), ETCO2, peak inspiratory pressure (PIP), pH, arterial partial pressure of oxygen (PaO2), PaCO2, base excess (BE) and HCO3 - were measured. Based on the PaCO2 and ETCO2 values, Pa-ETCO2 was determined. There was no significant difference in PaCO2 between the VA (42.5±5.2 to 53.7±5.2) and VP (48.4±5.4 to55.4±5.7) groups. During the postoperative period, all groups presented with hypertension. However, mild hypertension (SAP 150 to 159mmHg) was observed in the VP group as compared to severe hypertension (SAP>180mmHg) in the CA group, suggesting that both the age range and videolaparoscopic OH are associated with lower levels of hypertension during the postoperative period in dogs.(AU)


O objetivo deste estudo foi determinar a pressão parcial de dióxido de carbono (PaCO2), pressão ao final da expiração de CO2 (ETCO2) e diferença artério-alveolar de CO2 (Pa-ETCO2) em cadelas pré-púberes e adultas submetidas à ovário-histerectomia (OH) videolaparoscópica ou convencional. Foram distribuídas 40 cadelas em quatro grupos: Convencional Adulto (CA), Convencional Pediátrico (CP), Videolaparoscópico Adulto (VA) e Videolaparoscópico Pediátrico (VP). Foram mensurados frequência de pulso (FP), frequência respiratória (FR), pressões arteriais sistólica (PAS), média (PAM) e diastólica (PAD), ETCO2, pressão de pico inspiratória (PIP), pH, pressão parcial arterial de oxigênio (PaO2), PaCO2, excesso de bases (EB) e HCO3 -. Com base nos valores de PaCO2 e ETCO2 encontrados, foi determinada a Pa-ETCO2. Não foram encontradas diferenças significativas nos valores de PaCO2 entre os grupos VA (42.5±5.2-53,7±5,2) e VP (48.4±5.4 - 55,4±5,7). Todos os grupos apresentaram hipertensão arterial no período pós-operatório. Entretanto, o grupo VP apresentou hipertensão moderada (PAS 150-159mmHg) em comparação ao grupo CA, que apresentou hipertensão severa (PAS>180 mmHg), sugerindo que tanto a faixa etária, quanto a execução de OH por videolaparoscopia, estão associadas a menores taxas de hipertensão pós-operatória em cadelas.(AU)
