Introducción: La lipoatrofia semicircular (LS) es un trastorno caracterizado por depresiones en forma de banda en la cara anterolateral de los muslos. Objetivos: determinar la prevalencia de los casos de LS en varias instituciones públicas y privadas de la Provincia de Barcelona, España. Material y Métodos: se realizó estudio retrospectivo, en 969 trabajadores de oficinas públicas y privadas, entre 2018 y 2022. A todos los casos se les realizó examen físico completo con mediciones, fotografía y seguimiento de las lesiones. Los resultados fueron analizados mediante el programa Instat Graphpad 3.10 y se aplicó la prueba de Fischer para determinar la significancia estadística. Resultados: se diagnosticaron 96 casos de los cuales 92 eran mujeres. Prevalencia: 9,9%. El promedio de edad fue 49 años (27-64). La mayoría de las lesiones se localizaron en los muslos en forma bilateral. Las lesiones de LS al final del estudio no desaparecieron en el 60% de los casos, mientras que en un 40% si lo hicieron. Conclusiones: Las lesiones de LS toman un tiempo muy variable para revertir, suele ser un proceso lento cuya duración se mide habitualmente en años. Ni el teletrabajo, ni el índice de masa corporal, tuvieron relación estadísticamente significativa en la desaparición de las lesiones. (AU)


Introduction: Semicircular lipoatrophy (LS) is a disorder characterized by band-shaped depressions on the anterolateral aspect of the thighs. Objectives: to determine the prevalence of LS cases in various public and private institutions in the Province of Barcelona, Spain. Material and Methods: a retrospective study was carried out on 969 workers from public and private offices, between 2018 and 2022. All cases underwent a complete physical examination with measurements, photography, and follow-up of injuries. The results were analyzed using the Instat Graphpad 3.10 program and the Fischer test was applied to determine statistical significance. Results: 96 cases were diagnosed, of which 92 were women. Prevalence: 9.9%. The average age was 49 years (27-64). Most of the lesions were located on the thighs bilaterally. LS lesions at the end of the study did not disappear in 60% of cases, while in 40% they did. Conclusions: LS lesions take a highly variable time to reverse, it is usually a slow process whose duration is usually measured in years. Neither teleworking nor the body mass index had a statistically significant relationship in the disappearance of the lesions (AU)

Introducción y objetivo: Presentamos 2 casos clínicos de extirpación de gran cantidad de tejido afectado en pacientes con inyección masiva de materiales extraños en glúteos y el procedimiento reconstructivo empleado.Material y método: Describimos el procedimiento quirúrgico seguido tras la extirpación del material de infiltración, mediante disección de un colgajo dermograso de amplia base lateral que permite su avance en dirección medial para proveer tejido de la región superior del muslo lateral y conformar el nuevo glúteo. Resultados: El contorno final de los glúteos y la ubicación de las cicatrices obtuvo un alto grado de satisfacción en ambas pacientes. El postoperatorio fue largo y algo tórpido por retardo de la cicatrización. Uno de los casos requirió revisión. Conclusiones: Consideramos que los resultados obtenidos pueden ser una alternativa interesante para el tratamiento de esta patología, muy frecuente en la actualidad y de difícil resolución. (AU)


Background and objective: We present 2 clinical cases of surgical removal of foreign materials from the buttocks and the following reconstructive procedures in patients with massive injections of foreign materials. Methods: We describe the reconstructive procedure after the removal of the foreign material, with a dermal fat flap with a wide lateral base that allows to move the flap towards the center of the buttocks and provide tissue in the upper region of the lateral thigh, effectively reshaping the new gluteus. Results: The final contour of the buttocks and the placement of the scars were accepted with great satisfaction by both patients. The postoperative was long and slow due to prolonged scarring. One case required revision. Conclusions: We consider that the obtained results are encouraging and the method could make for an interesting alternative to treat this pathology, which is frequent and of difficult solution. Background: Oromaxillofacial carcinomas frequently result in serious tissue defect due to enlarged resection for treating their extensive invasion, which require challenging reconstruction. Three-dimensional (3D) printing is an advanced technology which has greatly promoted the progress of craniomaxillofacial reconstructive surgery. This present study aimed to investigate the advantages of anterolateral thigh (ALT) perforator flap manufactured by 3D printing fixed positioning guide template in curing oromaxillofacial defect.Material and methods: Twenty patients with oromaxillofacial defects resulted from severe primary malignant tumors were divided into experimental group assisted by digital technique (n=8) and controlled group conventionally aided by ultrasound (n=12). The therapeutic effectiveness, flap preparation time, amount of bleeding, deviation of perforator vessel location, aesthetic satisfaction of donor site, postoperative complications, adverse symptom of flap, and LEFS scores were compared.Results: For experimental group, flap preparation time was significantly shorter; and it has obviously less bleeding, minor deviation of perforator vessel location, and better aesthetic satisfaction of donor site (P<.001). There was no statistical difference in postoperative complications and LEFS scores between two groups (P>.05).Conclusions: The study suggests 3D printing template of fixed positioning guide provides a brand-new method for orienting perforated vessels of ALT flap, which is more accurate in clinical application. It can improve the operative efficacy, and increase the successful rate of operation as well. (AU)

Esta investigación examinó las diferencias en la activación muscular en los músculos de la cadera y muslo en corredores y corredoras con y sin el síndrome de la banda iliotibial (SFBI). Se registró la actividad neuromuscular en 21 corredores durante la carrera (14 SFBI y 7 sanos). No se han encontrado diferencias significativas en la actividad muscular media entre los corredores y corredoras lesionados. Sin embargo, en el caso de las corredoras lesionadas, se han encontrado diferencias entre el vasto lateral y el tensor fascia lata, y entre el vasto lateral y el bíceps femoral (p<0,05 en ambos casos). En el caso de los corredores hombres lesionados, se han encontrado diferencias entre el glúteo mayor y el tensor fascia lata, y entre el glúteo mayor y el bíceps femoral (p<0,05 en ambos casos). Estos hallazgos proporcionan un mayor entendimiento de la lesión y ayudarían a un tratamiento más específico. (AU)


A study was performed to examine differences in hip and thigh muscle activation in male and female runners with and without iliotibial band syndrome (ITBS). The neuromuscular activity of 21 runners was recorded during run (14 ITBS and 7 healthy). No significant differences were observed in mean muscle activity in injured male and female runners. In contrast, in female runners with ITBS, there were differences between the vastus lateralis and the tensor fasciae latae and between the vastus lateralis and the femoral biceps (p<0.05 in the two cases). With regard to male runners with ITBS, differences in activity were observed between the gluteus maximus and the tensor fasciae latae, and between the gluteus maximus and the femoral biceps (p<0.05 in the two cases). These findings contribute to a better understanding of iliotibial band syndrome and may be useful for the design of targeted treatments. (AU)

RESUMO

El liquen estriado es una genodermatosis inflamatoria, autolimitada y generalmente asintomática, de diagnóstico clínico. Presenta un pico de incidencia entre los 2 y 12 años y su etiología es desconocida. Suele afectar a una sola extremidad y suele perdurar varios meses hasta su resolución espontanea sin requerir tratamiento. Presentamos el caso de una niña de cuatro años que acudió a consulta con una lesión compatible con este diagnóstico


Lichen striatus is a self-limited inflammatory genodermatoses. It is generally asymptomatic, and it has a clinical diagnosis. It affects predominantly children from 2 to 12 years old and its etiology is unknown. The eruption is typically unilateral involving the extremities. It usually has a spontaneous resolution within a few months. We describe the case of a 4-year-old girl who consulted for a compatible lesion

RESUMO

The gluteal muscles have all the same embryological origin and therefore show typical variations. The gluteus medius has its origin on the external surface of the ilium and inserts at the major greater trochanter. It lies between the gluteus maximus and minimus. Variations included in this case are the gluteus medius accessorius and the gluteus quartus or scansorius. Various sources of literature have described these two muscles on anthropoids and in man, but never with the two very rare variations appearing in one specimen, as we have found in this right lower limb of a 53-year-old woman. The gluteus medius originates from the iliac crest between the gluteus medius and the gluteus minimus and inserts onto the greater trochanter. The gluteus quartus arises from the anterior portion of the gluteus minimus and the fascia lata and inserts into the tip of the greater trochanter. As a clinical consequence, the occurrence of these muscles might lead to instability in the hip joint on the contralateral side, and these anatomical variations are also important to be known during different approaches of total hip replacement surgery

El sarcoma fibromixoide de bajo grado es una neoplasia poco frecuente que afecta preferentemente a adultos jóvenes. Se trata de un tumor fibroblástico de apariencia engañosamente benigna que puede presentar recurrencias locales y metástasis a distancia tardías. Presentamos el caso de una niña de 11 años que presentaba una lesión en el muslo, de la que se realizó exéresis completa. El estudio histológico mostró una neoplasia bien delimitada de estroma mixoide, con celularidad fusiforme sin atipia ni mitosis. La expresión inmunohistoquímica de MUC4, junto con el estudio de biología molecular que detectó la expresión del gen de fusión FUS-CREB3L2, confirmaron el diagnóstico de sarcoma fibromixoide de bajo grado. Revisamos el diagnóstico diferencial de los tumores mixoides de partes blandas, especialmente los de apariencia benigna, destacando la importancia del estudio inmunohistoquímico y molecular para descartar el sarcoma fibromixoide de bajo grado


Low-grade fibromyxoid sarcoma is a rare neoplasm, primarily affecting young adults. It is a fibroblastic tumor with a deceptively benign appearance, which may produce local recurrences and late distant metastases. We present a case of an 11-year-old girl with a lesion on her thigh that was completely excised. Histopathology revealed a well-delimited neoplasm with myxoid stroma and fusiform cellularity without atypia or mitosis. Immunohistochemical expression of MUC4, together with molecular biology, which detected FUS-CREB3L2 fusion gene expression, confirmed the diagnosis of low-grade fibromyxoid sarcoma. We review the differential diagnosis of soft-tissue myxoid tumors, especially those with a benign appearance, highlighting the importance of immunohistochemical and molecular studies to rule out low-grade fibromyxoid sarcoma

La piomiositis (PM) es una infección aguda bacteriana que afecta al músculo estriado, generalmente causada por Staphylococcus aureus y suele acompañarse de la formación de un absceso en el músculo. Esta entidad clínica fue descrita por primera vez en 1885 por Scriba1 como enfermedad endémica en los trópicos. En las últimas dos décadas se ha observado un incremento notable del número de casos en países de clima templado, probablemente asociado a un aumento de pacientes inmunodeprimidos. Las infecciones profundas de músculos pélvicos son difíciles de diagnosticar por ocasionar signos clínicos inespecíficos y que hacen pensaren otras patologías más comunes. Comunicamos el caso de una paciente de 33 años diabética que consultó por dolor inguinal acompañado de tumoración en la misma zona y fiebre de 4 días de evolución. Recibió tratamiento sintomático sin respuesta. La resonancia nuclear magnética pélvica fue compatible con piomiositis de músculo pectíneo. Recibió tratamiento antibiótico, evolucionando satisfactoriamente. La piomiositis es una entidad poco frecuente, que requiere un elevado índice de sospecha, para un adecuado diagnóstico y tratamiento, siendo la terapia antibiótica y drenaje en caso de absceso los pilares de éste último. Este tratamiento debe instaurarse de forma precoz, ya que su evolución puede ser potencialmente letal


Pyomyositis (PM) is an acute bacterial infection of skeletal muscle, usually caused by Staphilococcus Aureus and it is frenquently associated to a muscle abcess. This clinical entity was first described by Scriba1 at 1885 as a tropic endemic pathology. In the past two decades the number of cases at countries of temperate climate has rised notably due to the number of patients with inmunosuppression. Unfortunately, diagnosis of deep pelvic muscle infection is often delayed since they usually present with non-specific physical signs suggesting other more common diseases. The authors communicate a case of a diabetic33-year-old female who suffered inguinal acute pain and mass with history of fever since 4 days before. She initially received symptomatic treatment with no success. Pelvis magnetic resonance imaging showed pyomyositis of the piriformis muscle. Antibacterial treatment was administered, resulting in a good outcome. Pyomyositis is a rare condition that demands a high index of suspicion to make an adequate diagnosis and prompt treatment, including antibacterial treatment and drainage, particularly in case of abscess formation. This treatment should be established promptly since its outcome may be potentially lethal
