Introduction and aim: Pancreatobiliary tumours are challenging to diagnose exclusively by imaging methods. Although the optimum moment for carrying out the EUS is not well defined, it has been suggested that the presence of biliary stents may interfere with the proper staging of tumours and the acquisition of samples. We performed a meta-analysis to evaluate the impact of biliary stents on EUS-guided tissue acquisition yield. Material and methods: We conducted a systematic review in different databases, such as PubMed, Cochrane, Medline, and OVID Database. A search was made of all studies published up to February 2022. Results: Eight studies were analyzed. A total of 3185 patients were included. The mean age was 66.9±2.7 years; 55.4% were male gender. Overall, 1761 patients (55.3%) underwent EUS guided tissue acquisition (EUS-TA) with stents in situ, whereas 1424 patients (44.7%) underwent EUS-TA without stents. The technical success was similar in both groups (EUS-TA with stents: 88% vs EUS-TA without stents: 88%, OR=0.92 [95% CI 0.55–1.56]). The type of stent, the needle size and the number of the passes were similar in both groups. Conclusions: EUS-TA has similar diagnostic performance and technical success in patients with or without stents. The type of stent (SEMS or plastic) does not seem to influence the diagnostic performance of EUS-TA. Future prospectives and RCT studies are needed to strengthen these conclusions. (AU)


Introducción y objetivo: Los tumores pancreatobiliares son lesiones difíciles de diagnosticar exclusivamente por métodos de imagen. Aunque no está bien definido el momento óptimo para la realización de la ecoendoscopia (EUS), se ha demostrado que la presencia de stents biliares puede interferir en la correcta estadificación de los tumores y la toma de muestras. Realizamos un metanálisis para evaluar el impacto de los stents biliares en el rendimiento de la adquisición de tejido guiada por EUS. Material y métodos: Realizamos una revisión sistemática en diferentes bases de datos, como PubMed, Cochrane, Medline y OVID Database. Se realizó una búsqueda de todos los estudios publicados hasta febrero de 2022. Resultados: Se analizaron 8 estudios. Se incluyeron un total de 3.185 pacientes. La media de edad fue de 66,9±2,7 años; el 55,4% fueron pacientes de sexo masculino. En total, 1.761 pacientes (55,3%) se sometieron a biopsias guiadas por EUS con stents in situ, mientras que 1.424 pacientes (44,7%) se sometieron a dichas biopsias sin stents. El éxito técnico fue similar en ambos grupos (EUS con stents: 88% vs. EUS sin stents: 88%, OR=0,92 [IC 95% 0,55-1,56]). El tipo de stent, el tamaño de la aguja y el número de pases fueron similares en ambos grupos. Conclusiones: La biopsia por EUS tiene un rendimiento diagnóstico y un éxito técnico similares en pacientes con o sin stents. El tipo de stent (SEMS o plástico) no parece influir en el rendimiento diagnóstico de la adquisición de tejido guiada por EUS. Se necesitan futuros estudios prospectivos y estudios aleatorizados controlados para fortalecer estas conclusiones. (AU)

Introducción y objetivos: El implante de stent es el tratamiento de elección en niños mayores y adultos con coartación aórtica (CoA). El objetivo fue determinar la incidencia de eventos tardíos después del tratamiento con stent. Métodos: Se analizó una cohorte de pacientes con CoA tratados en el centro entre 1993 y 2018 a los que se siguió periódicamente, incluyendo evaluación mediante tomografía computarizada (TC) y fluoroscopia. Resultados: Se incluyó a 167 pacientes con TC y fluoroscopia: 83 (49,7%) ≤ 12 años y 46 (28%) mujeres. El tiempo medio de seguimiento clínico fue de 17±8 años (rango 4-30) y de 11±7 años hasta la TC/fluoroscopia. Se detectó aneurisma en un 13% y se asoció al stent PALMAZ (OR=3,09; IC95%, 1,11-9,49; p=0,036) y a la longitud del stent (OR=0,94; IC95%, 0,89-0,99; p=0,039). La fractura del stent fue frecuente (34%) pero no asociada a la presencia de aneurisma. Se asoció con edad joven (OR=3,57; IC95%, 1,54-8,33; p=0,003), sexo masculino (OR=4,00; IC95%, 1,51-12,50; p=0,008) e inversamente con el stent PALMAZ (OR=0,29; IC95%, 0,12-0,67; p=0,005). La reintervención fue menor en adultos (10%), principalmente por aneurismas. Los pacientes que recibieron tratamiento cuando tenían 12 años o menos presentaron tasas de reintervención más altas (43%) debido al crecimiento somático. Conclusiones: Se observó una incidencia notable de eventos tardíos a largo plazo en pacientes con CoA tratados mediante stent. La reintervención fue más frecuente en pacientes tratados a edades más jóvenes. Parece aconsejable una vigilancia periódica mediante pruebas de imagen. (AU)


Introduction and objectives: Stent implantation is the preferred treatment in older children and adults with aortic coarctation (CoA). We aimed to determine the incidence of very late events after CoA stenting. Methods: We analyzed a cohort of CoA patients who underwent stent implantation at our center between 1993 and 2018. Patients were periodically followed up in outpatient clinics, including computed tomography (CT) and fluoroscopy assessment. Results: A total of 167 patients with CT and fluoroscopy data were included: 83 (49.7%) were aged ≤ 12 years and 46 (28%) were female. The mean clinical follow-up time was 17±8 (range 4-30) years and the mean time to CT/fluoroscopy was 11±7 years. Aortic aneurysm was present in 13% and was associated with the PALMAZ stent (OR, 3.09; 95%CI, 1.11-9.49; P=.036) and the stented length (OR, 0.94; 95%CI, 0.89-0.99; P=.039). Stent fracture was frequent (34%), but was not related to the presence of aneurysm. Stent fracture was associated with young age (OR, 3.57; 95%CI, 1.54-8.33; P=.003), male sex (OR, 4.00; 95%CI, 1.51-12.5, P=.008) and inversely with the PALMAZ stent (OR, 0.29; 95%CI, 0.12-0.67, P=.005). Reintervention was lower in adults (10%), mainly related to aneurysms. Those treated when aged ≤ 12 years had higher reintervention rates (43%) due to recoarctation somatic growth. Conclusions: This long-term follow-up study of CoA patients treated with stenting revealed a significant incidence of late events. Reintervention rates were higher in patients treated at younger ages. Periodic imaging surveillance appears to be advisable. (AU)

Introducción y objetivos Se desconoce el impacto pronóstico de los diferentes tipos de eventos adversos tras el infarto agudo de miocardio con elevación del segmento ST (IAMCEST). El objetivo de este trabajo es evaluar los predictores a largo plazo del objetivo combinado orientado al paciente (POCE) y si tener un fallo del vaso diana (FVD) como primer evento puede influir en los resultados. Métodos El ensayo EXAMINATION-EXTEND aleatorizó a pacientes con IAMCEST a tratamiento con stents liberadores de everolimus o a stents convencionales, con un seguimiento de hasta 10 años. En la población del estudio, se evaluaron los predictores de POCE (combinado de mortalidad por cualquier causa, infarto de miocardio y cualquier revascularización). Se clasificó a los pacientes según el tipo de primer evento (FVD primero o FDV no primero) y comparado en términos de POCE posterior. El FVD se definió como el compuesto de muerte cardiaca, IAMCEST del vaso diana y revascularización del vaso diana. Resultados De los 1.498 pacientes del estudio, 529 (35,3%) tuvieron POCE durante el seguimiento a los 10 años. Los predictores independientes de POCE fueron la edad, la diabetes mellitus, el infarto de miocardio previo, la enfermedad arterial periférica y la enfermedad coronaria multivaso. El primer evento fue un FVD o no FVD en 296 y 233 casos respectivamente. No hubo diferencias estadísticamente significativas entre quienes tuvieron primero un FVD y los que tuvieron un evento no FVD en cuanto a POCE (el 21,7 frente al 39,3%; razón de tiempo, 1,79; IC95%, 0,87-3,67; p=0,12) o sus componentes individuales. Conclusiones En el seguimiento a 10 años, alrededor de un tercio de los pacientes con IAMCEST tuvo al menos 1 evento de POCE, cuyos predictores independientes fueron la edad, la diabetes mellitus y una mayor extensión de la enfermedad ateroesclerótica... Introduction and objectivesAfter ST-segment myocardial infarction (STEMI), the impact of different adverse events on prognosis remains unknown. We aimed to assess very long-term predictors of patient-oriented composite endpoints (POCE) and investigate whether the occurrence of target vessel failure (TVF) vs a non-TVF event as the first event could potentially influence subsequent outcomes. Methods The EXAMINATION.EXTEND trial randomized STEMI patients to receive either an everolimus-eluting stent or a bare-metal stent. The follow-up period was 10 years. Predictors of POCE (a composite of all-cause death, any myocardial infarction, or any revascularization) were evaluated in the overall study population. The patients were stratified based on the type of first event (TVF-first vs non–TVF-first) and were compared in terms of subsequent POCE. TVF was defined as a composite of cardiac death, TV myocardial infarction, or TV revascularization. Results Out of the 1498 enrolled patients, 529 (35.3%) experienced a POCE during the 10-year follow-up. Independent predictors of POCE were age, diabetes mellitus, previous myocardial infarction, peripheral arterial disease, and multivessel coronary disease. The first event was a TVF in 296 patients and was a non-TVF in 233 patients. No significant differences were observed between TVF-first and non–TVF-first patients in terms of subsequent POCE (21.7% vs 39.3%, time ratio 1.79; 95%CI, 0.87-3.67; P=.12) or its individual components. Conclusions At the 10-year follow-up, approximately one-third of STEMI patients had experienced at least 1 POCE. Independent predictors of these events were age, diabetes, and more extensive atherosclerotic disease. Introducción y objetivos: Las oclusiones coronarias crónicas totales (OCT) que afectan a lesiones en bifurcación representan un subconjunto de lesiones difíciles de tratar y poco estudiadas en la literatura. Este estudio analiza la incidencia, la estrategia de tratamiento, los resultados hospitalarios y las complicaciones de la intervención coronaria percutánea (ICP) de las OCT en bifurcación (OCT-BIF). Métodos: Se evaluaron los datos de 607 pacientes consecutivos con OCT tratados en el Institut Cardiovasculaire Paris Sud (ICPS), Massy, Francia, entre enero de 2015 y febrero de 2020. Se compararon 2 subgrupos de pacientes (OCT-BIF, n=245; OCT-no BIF, n=362) en cuanto a estrategia de procedimiento, resultado hospitalario y tasa de complicaciones. Resultados: La media de edad de los pacientes fue 63,2±10,6 años; el 79,6% eran varones. Las lesiones en bifurcación estuvieron implicadas en el 40,4% de los procedimientos. La complejidad general de la lesión fue alta (valores medios de las puntuaciones J-CTO, 2,30 ± 1,16, y PROGRESS CTO, 1,37±0,94). El stent condicional fue la estrategia preferida para el tratamiento de las lesiones en bifurcación (93,5%). Los pacientes OCT-BIF presentaban una mayor complejidad de la lesión según la puntuación J-CTO (2,42±1,02 frente a 2,21±1,23 de los pacientes OCT-no BIF; p=0,025) y la puntuación PROGRESS CTO (1,60±0,95 frente a 1,22±0,90 de los pacientes OCT-no BIF; p<0,001). El éxito de la intervención fue del 78,9% y no se vio afectado por la presencia de bifurcación (el 80,4% en el grupo de OCT-BIF y el 77,8% en el grupo de OCT-no BIF; p=0,447) ni por el lugar de la bifurcación (OCT-BIF en segmento proximal, el 76,9%; OCT-no BIF en segmento medio, el 83,8%; OCT-BIF en segmento distal, el 85%; p=0,204). Introduction and objectives: Coronary chronic total occlusions (CTO) involving bifurcation lesions are a challenging lesion subset that is understudied in the literature. This study analyzed the incidence, procedural strategy, in-hospital outcomes and complications of percutaneous coronary interventions (PCI) for bifurcation-CTO (BIF-CTO). Methods: We assessed data from 607 consecutive CTO patients treated at the Institut Cardiovasculaire Paris Sud (ICPS), Massy, France between January 2015 and February 2020. Procedural strategy, in-hospital outcomes and complication rates were compared between 2 patient subgroups: BIF-CTO (n=245=and non–BIF-CTO (n=362). Results: The mean patient age was 63.2±10.6 years; 79.6% were men. Bifurcation lesions were involved in 40.4% of the procedures. Overall lesion complexity was high (mean J-CTO score 2.30±1.16, mean PROGRESS-CTO score 1.37±0.94). The preferred bifurcation treatment strategy was a provisional approach (93.5%). BIF-CTO patients presented with higher lesion complexity, as assessed by J-CTO score (2.42±1.02 vs 2.21±1.23 in the non–BIF-CTO patients, P=.025) and PROGRESS-CTO score (1.60±0.95 vs 1.22±0.90 in the non–BIF-CTO patients, P<.001). Procedural success was 78.9% and was not affected by the presence of bifurcation lesions (80.4% in the BIF-CTO group, 77.8% in the non–BIF-CTO-CTO group, P=.447) or the bifurcation site (proximal BIF-CTO 76.9%, mid–BIF-CTO 83.8%, distal BIF-CTO 85%, P=.204). Complication rates were similar in BIF-CTO and non–BIF-CTO. Conclusions: The incidence of bifurcation lesions is high in contemporary CTO PCI. Patients with BIF-CTO present with higher lesion complexity, with no impact on procedural success or complication rates when the predominant strategy is provisional stenting.(AU)

Biliary metal stent implantation is an effective treatment for malignant obstructive jaundice. But it's well known that stents put in for a long time can become occluded and cause jaundice and cholangitis. At this time, endoscopic intervention is usually required to replace the stent or re-insert the stent. Re-cannulation for metal stent occlusion is challenging because the guide wire may pass through the side holes of the uncovered metal stents, resulting in prolonged surgical time and exposure to radiation. Here we present a small tip that may help endoscopists complete the re-cannulation of an uncovered metal stent in a very short time.(AU)

Introducción y objetivos La estenosis ureteral es una condición crónica que puede provocar la obstrucción del flujo urinario de la unidad renal comprometida, con la consiguiente pérdida de función renal. Los tipos de tratamiento pueden clasificarse en 2categorías principales: endourológicos y reconstructivos. Nuestro objetivo fue investigar la eficacia y seguridad del stent ureteral autoexpandible Allium®, cuyo uso en el tratamiento mínimamente invasivo de la estenosis ureteral se ha extendido en los últimos años. Materiales y métodos El estudio incluyó a 20 pacientes a los que se colocó un stent ureteral autoexpandible entre 2017 y 2021. Se registraron y evaluaron de forma prospectiva sus características clínicas y demográficas, los detalles del tratamiento, las complicaciones perioperatorias y postoperatorias y su tratamiento, y los hallazgos durante el seguimiento. Resultados La etiología incluyó urolitiasis en 16 pacientes (80%), neoplasia en 3 pacientes (15%) y cirugía ginecológica previa en uno (5%). La obstrucción del stent fue la complicación más frecuente en 3 pacientes (15%), seguida de la migración del stent en 2 (10%). Se continúa el seguimiento de 15 pacientes sin obstrucción ni complicaciones relacionadas con el stent. El periodo medio de seguimiento fue de 28±15,7 meses. Conclusiones El stent ureteral constituye una alternativa eficaz y segura para el tratamiento mínimamente invasivo de la estenosis ureteral, con unas tasas de complicaciones aceptables y un manejo sencillo de ellas. Introduction and objectives Ureteral stricture is a chronic condition that can result in the obstruction of urinary drainage from the affected renal unit, leading to loss of renal function. reatment methods can be categorized into 2main headings: endourological and reconstructive procedures. We aimed to investigate the efficacy and safety of the self-expandable Allium® ureteral stent, which has been used in the minimally invasive treatment of ureteral stenosis in recent years. Materials and methods Twenty patients who were applied Allium® ureteral stent between 2017-2021 included in the study. The demographic and clinical characteristics of the patients, the details of the treatments applied to the patients, the perioperative and postoperative complications, the treatments applied for the complications and the findings in the follow-up were recorded and evaluated prospectively. Results Etiology included urolithiasis in 16 patients (80%), malignancy in 3 patients (15%), and a previous gynecological operation in one patient (5%). Stent obstruction was found to be the most common complication in 3 patients (15%). Stent migration was the second most common complication in 2 patients (10%). Our follow-up continues with 15 patients without stent-related complications and stent obstruction. The mean follow-up period was 28±15.7 months. Conclusions Allium ureteral stent is an effective and reliable method in the minimally invasive treatment of ureteral stricture with complications seen at acceptable rates and ease of treatment in the management of complications. Purpose: The performance of the Low-Profile Visualized Intraluminal Support (LVIS) stentdeployed following balloon angioplasty is unknown in treating intracranial atherosclerotic stenosis, and this study was to investigate the safety and efficacy of the LVIS stent in treatingintracranial atherosclerotic stenosis in the middle cerebral artery M1 segment.Methods: Thirty-five patients were enrolled with 35 atherosclerotic stenoses at the M1 segment. The stenosis was about 75% in 16 patients, 80% in 15, and 90% in the rest four. The LVISstent was used to treat these patients. Results: The success rate of stenting was 97.1%. The stenting procedure was failed in onepatient because of intraprocedural dissection of the stenotic (75%) segment, resulting in a 30-day periprocedural complication rate of 2.9% (1/35). Before stenting, the stenosis rate ranged75%—90% (mean 78.9% ± 4.7%), and after stenting, the diameter of the stented segment wassignificantly (P < 0.0001) increased to 1.5—3.4 mm (mean 2.1 ± 0.32 mm) ranging 68.2%—100%(mean 94.0% ± 5.8%) of the normal arterial diameter, with the residual stenosis ranging 0—31.8%(median 4.8%, IQR 2.4%—7.3%). Follow-up was performed at 6—20 months (mean 8.5) afterstenting. One patient (2.9%) had occlusion of the stented M1 segment with no symptoms, andtwo patients (5.7%) had slight asymptomatic instent stenosis (40%) at the M1 segment, with theinstent restenosis and occlusion rate of 8.6% (3/35). Conclusion: The braided LVIS stent can be safely applied for treatment of intracranialatherosclerotic stenosis in the middle cerebral artery with good safety and efficacy immediatelyafter stenting and at follow-up.(AU)


Objetivo: Nos propusimos analizar la seguridad y efectividad de la colocación de un stentlow-profile visualized intraluminal support (LVIS TM ) tras angioplastia con balón en pacientescon estenosis ateroscleróticas intracraneales en el segmento M1 de la arteria cerebral media(ACM). Métodos: Incluimos 35 pacientes con estenosis ateroscleróticas en el segmento M1 de la ACM;la estenosis era del 75% en 16 pacientes, del 80% en 15 y del 90% en los 4 restantes. En todoslos casos el tratamiento se basó en la colocación de un stent LVIS TM . Resultados: El stent se implantó con éxito en el 97,1% de los casos; en un paciente, el procedimiento no se pudo llevar a cabo a causa de una disección del segmento estenótico (estenosisdel 75%) durante la operación, lo que supone una tasa de complicaciones perioperatorias a los30 días del 2,9%. Antes de la colocación del stent, el grado de estenosis oscilaba entre el 75 yel 90% (media [DS]: 78,9% [4,7%]). Tras el procedimiento, el diámetro del segmento en el quese había colocado el stent aumentó de forma significativa (P < 0,0001) hasta los 1,5-3,4 mm(media: 2,1 mm [0,32]), logrando un 68,2-100% (media: 94,0% [5,8%]) del diámetro normal dela arteria, y una estenosis residual del 0 al 31,8% (mediana: 4,8%; p25-p75, 2,4-7,3%). Se realizóun seguimiento de entre 6 y 20 meses (media: 8,5) tras el procedimiento. Un paciente (2,9%)presentó una oclusión asintomática del segmento M1 intervenido, y 2 pacientes (5,7%) presentaron estenosis intrastent asintomáticas leves (40%) en M1, por lo que la tasa de reestenosisintrastent y oclusión fue del 8,6%. Conclusión: El stent LVIS TM es un tratamiento seguro y efectivo para las estenosis ateroscleróticas intracraneales en la ACM.(AU)

Objective:To determine the clinical efficacy of echocardiography in assessing diastolic function in football players who have experienced myocardial infarction with preserved ejection fraction heart failure. Methods:This study involved 54 football players with myocardial infarction and preserved ejection fraction heart failure, treated at our medical center from September 2020 to January 2021. They were compared to 50 healthy, physically active volunteers. Both groups underwent echocardiographic evaluations focusing on cardiac structure and function. The quality of life of the football players was assessed before and after the echocardiographic evaluations. Results:The observation group showed elevated serum levels of troponin I (cTnI) and N-terminal B-type natriuretic peptide (NT-proBNP) compared to the control group (P < 0.05). Echocardiographic findings indicated significant differences in diastolic function between groups, with the E/A ratio and E/E' ratio notably higher in the observation group. Post-treatment,the quality of life in the observation group improved significantly (P < 0.05). Conclusion:Echocardiography is a valuable clinical tool for evaluating diastolic function in football players who have suffered myocardial infarction with preserved ejection fraction heart failure, especially post-stenting. This study highlights the importance of specialized cardiac assessments in athletes to facilitate optimal recovery and athletic performance (AU)

Introduction Relatively constant surgical risks and rapid advances in endovascular treatment have caused a major shift toward endovascular management of posterior circulation aneurysms. This paper presents the results of a series of endovascularly treated posterior circulation aneurysms. Methods A total of 81 patients who underwent endovascular treatment of posterior circulation aneurysms performed by a single team between 2009 and 2019 were included. Demographic, clinical, radiologic, and management details were retrospectively obtained from hospital records. Results Among the included patients, 50 (61.7%) and 31 (38.3%) were female and male, respectively. Subarachnoid hemorrhage was observed in 30 patients (37%). Moreover, 40 (49.3%) aneurysms were treated with stent-assisted coiling, 1 (1.2%) aneurysm was treated with parent artery occlusion, 2 (2.4%) aneurysms were coiled using balloon assistance, 24 (29.6%) aneurysms were coiled primarily, 1 (1.2%) patient had an unsuccessful treatment attempt, and 13 (16.0%) aneurysms were treated with flow-diverter stents or stent monotherapy. During the last follow-up, 57 (83.8%) aneurysms were completely occluded, whereas 6 (8.8%) and 2 (2.9%) aneurysms did and did not have a residual neck, respectively. Flow diversion was used to treat 13 patients, among whom 8 had total occlusion or stable residue. A total of 7 deaths (8.6%) were encountered in this series. Conclusion Endovascular treatment should be considered as the primary treatment modality for posterior circulation aneurysms. Despite the high morbidity and mortality rates, promising results can be achieved with correct patient selection. Flow diversion can be a feasible alternative for complex aneurysms that are difficult to treat (AU)


Introducción Los riesgos quirúrgicos relativamente constantes y los rápidos avances en el tratamiento endovascular han provocado un viraje importante a dar preferencia al tratamiento endovascular en los aneurismas de la circulación posterior. Este artículo presenta los resultados de una serie de aneurismas de la circulación posterior tratados endovascularmente. Métodos Se incluyeron un total de 81 pacientes que fueron sometidos a tratamiento endovascular de aneurismas de la circulación posterior realizado por un solo equipo entre 2009 y 2019. Los detalles demográficos, clínicos, radiológicos y de manejo del paciente se obtuvieron retrospectivamente de los registros hospitalarios. Resultados Entre los pacientes incluidos, 50 (61,7%) eran mujeres y 31 (38,3%) eran hombres. Se observó hemorragia subaracnoidea en 30 de los pacientes (37%). Además, 40 (49,3%) de las aneurismas se trataron con enrollamiento asistido por stent, 1 (1,2%) aneurisma se trató con oclusión de la arteria madre, 2 (2,4%) aneurismas se enrollaron con asistencia de balón, 24 (29,6%) aneurismas se enrollaron en espiral, 1 (1,2%) paciente tuvo fallo en el intento del tratamiento y 13 (16,0%) aneurismas fueron tratados con stents desviadores de flujo o con monoterapia de stents. Durante el último seguimiento, 57 (83,8%) aneurismas estaban completamente ocluidos, mientras que 6 (8,8%) y 2 (2,9%) aneurismas tenían y no tenían cuello residual, respectivamente. La derivación de flujo se utilizó para tratar a 13 pacientes, de los cuales 8 tenían oclusión total o presentaban residuo estable. En esta serie se contabilizaron un total de 7 muertes (8,6%). Conclusión El tratamiento endovascular debería considerarse como la modalidad de tratamiento principal para los aneurismas de la circulación posterior (AU)

Introducción y objetivos: El uso de una guía de presión encarcelada para evaluar los resultados de la rama lateral durante la técnica del stent provisional parece factible. Sin embargo, existen dudas sobre su seguridad por el daño mecánico de la guía y son escasos los datos referentes al pronóstico de los pacientes. El objetivo de este estudio fue evaluar el daño estructural de la guía de presión en la técnica de la guía encarcelada y examinar los resultados clínicos a medio plazo. Métodos: Se incluyó a 99 pacientes con lesiones bifurcadas tratadas mediante la técnica de guía de presión encarcelada y, como control histórico, a 114 pacientes tratados mediante la técnica de la guía encarcelada con guías no poliméricas. Se evaluó el daño de la guía mediante microscopía estereoscópica. El objetivo primario fue localizar la presencia de daño microscópico significativo. Se examinaron eventos cardiovasculares adversos mayores a los dos años de seguimiento. Resultados: El daño microscópico significativo fue más frecuente en las guías de presión que en las no poliméricas (53,5% vs 22,8%, p <0,001). No hubo fracturas en ningún grupo. Hubo menos intervenciones de la rama lateral en el grupo de la guía de presión (posdilatación 32,3% vs 56,1%, p=0,001; stent, 0,0% vs 2,6%, p=0,104). El riesgo de eventos cardiovasculares a los 2 años fue similar en ambos grupos (HRadj=0,42; IC95%, 0,10-1,73; p=0,229). Conclusiones: La guía de presión fue menos resistente al encarcelamiento que la guía no polimérica. Los pacientes tratados con guía de presión requirieron menos intervenciones de la rama lateral, pero tuvieron similar riesgo de eventos a los 2 años de seguimiento. (AU)


Introduction and objectives: The use of a pressure wire as a jailed wire to evaluate side branch results during provisional stenting seems feasible. However, safety concerns exist due to the mechanical damage of the wire and the lack of prospective data evaluating the prognosis of patients treated using this technique. This study sought to evaluate the structural damage of the pressure wire in patients treated using the jailed pressure wire technique and to assess mid-term clinical outcomes. Methods: We enrolled 99 patients with single bifurcation lesions and provisional stenting as the strategy of choice. A jailed pressure wire was used to guide side branch intervention according to the instantaneous wave-free ratio (iFR). A total of 114 patients and the respective nonpolymer-coated jailed wires were used as historical controls. Guidewire damage was evaluated by stereomicroscopy. The primary endpoint was significant microscopic damage. Major adverse cardiac events were evaluated at 2-year follow-up. Results: Significant microscopic damage was more frequent in pressure wires than in nonpolymer-coated wires (53.5% vs 22.8%, P<.001). There were no fractures in either group. There were fewer side branch interventions in the pressure wire group (postdilation/kissing balloon, 32.3% vs 56.1%, P=.001; stenting, 0.0% vs 2.6%, P=.104). The 2-year rate of major adverse cardiac events was similar between the 2 groups (HRadj, 0.42; 95%CI, 0.10-1.73; P=.229). Conclusions: Pressure wires were less resistant to jailing than conventional nonpolymer-coated wires. Patients treated with iFR-guided provisional stenting required fewer side branch interventions but had similar 2-year clinical outcomes than patients treated with the angiography-guided technique. Introduction: The aim of this study was to compare the IsirisTM cystoscope with a common reusable flexible cystoscope in terms of patient perceived pain and endoscopy time in the ureteral stent removal setting. Materials and Methods: A non-randomized prospective study comparing the IsirisTM single-use cystoscope with a reusable flexible cystoscope. A visual analogue scale (VAS) was used for pain assessment and endoscopy time was recorded in seconds. Univariate and multivariate analyses were performed to assess the correlation between endoscope type and clinical variables with VAS score and endoscopy time. Results: A total of 85 patients were included in the study: 53 in the disposable cystoscope group and 32 in the reusable cystoscope group. Ureteral stent extraction was successful in all cases. The mean VAS score was similar between groups (single-use group was 2.09 +/– 2.53 vs 2.53 +/– 2.14 in the reusable cystoscope group) (p = 0.13). Same was observed endoscopy time (74.92 +/– 74.45 s. in the single-use group vs 98.87 +/– 153.33 s. in the reusable group) (p = 0.07). Age (coefficient β = –0.36, p < 0.04) and body mass index (BMI) (coefficient β = –0.22, p < 0.02) were inversely correlated with perceived pain during ureteral stent removal, measured by VAS score. Conclusions: Ureteral catheter removal with a flexible cystoscope is a well-tolerated procedure in patients. Older age and high BMI are associated with better intervention tolerance. Use of a single-use flexible cystoscope is comparable to that of a common flexible cystoscope in terms of pain and endoscopy time (AU)

Paciente masculino de 59 años con antecedentes de obesidad, hipertensión arterial y enfermedad renal crónica que presentó fracaso renal agudo de su riñón trasplantado, que requirió la colocación de un catéter transitorio. Durante la colocación de un catéter 12 Fr guiado por ecografía, inadvertidamente se realizó una fístula carotídea yugular (FYC). Después de que fallara la compresión guiada por ultrasonido, se desplegó en la carótida primitiva un stent autoexpandible cubierto con exclusión defi nitiva de la fístula. Estos resultan una opción segura y factible para el tratamiento de la fístula yugular carotídea en zonas de exposición quirúrgica complicada y pacientes de alto riesgo.(AU)


A 59-year-old male patient with a history of obesity, high blood pressure, and chronic kidney disease who presented acute renal failure of his transplanted kidney, requiring temporary catheter placement. During the placement of a 12 Fr catheter guided by ultrasound, a jugular-carotid fi stula (JCF) was inadvertently created. After ultrasound-guided compression failed, a covered self-expanding stent was deployed in the common carotid with defi nitive exclusion of the fi stula. These are a safe and feasible option for the treatment of carotid jugular fi stula in areas with complicated surgical exposure and for high-risk patients.(AU)
