Purpose: To investigate the Shikonin (SHI) induce autophagy of hypertrophic scar-derived fibroblasts (HSFs) and the mechanism of which in repairing hypertrophic scar. Methods: This study showed that SHI induced autophagy from HSFs and repaired skin scars through the AMPK/mTOR pathway. Alamar Blue and Sirius red were used to identify cell activity and collagen. Electron microscopy, label-free quantitative proteomic analysis, fluorescence and other methods were used to identify autophagy. The differences in the expression of autophagy and AMPK/mTOR pathway-related proteins after SHI treatment were quantitatively analyzed by Western blots. A quantitative real-time polymerase chain reaction assay was used to detect the expression of LC3, AMPK and ULK after adding chloroquine (CQ) autophagy inhibitor. Results: After treatment with SHI for 24 hours, it was found that the viability of HSFs was significantly reduced, the protein expression of LC3-II/LC3-I and Beclin1 increased, while the protein expression of P62 decreased. The expression of phosphorylated AMPK increased and expression of phosphorylated mTOR decreased. After the use of CQ, the cell autophagy caused by SHI was blocked. The key genes LC3 and P62 were then reexamined by immunohistochemistry using a porcine full-thickness burn hypertrophic scar model, and the results verified that SHI could induce autophagy in vivo. Conclusions: These findings suggested that SHI promoted autophagy of HSFs cells, and the potential mechanism may be related to the AMPK/mTOR signal pathway, which provided new insights for the treatment of hypertrophic scars.

Abstract Background: Tacrolimus is used to prevent unaesthetic scars due to its action on fibroblast activity and collagen production modulation. Objectives: To evaluate the action pathways, from the histopathological point of view and in cytokine control, of tacrolimus ointment in the prevention of hypertrophic scars. Methods: Twenty-two rabbits were submitted to the excision of two 1-cm fragments in each ear, including the perichondrium. The right ear received 0.1% and 0.03% tacrolimus in ointment base twice a day in the upper wound and in the lower wound respectively. The left ear, used as the control, was treated with petrolatum. After 30 days, collagen fibers were evaluated using special staining, and immunohistochemistry analyses for smooth muscle actin, TGF-β and VEGF were performed. Results: The wounds treated with 0.1% tacrolimus showed weak labeling and a lower percentage of labeling for smooth muscle actin, a higher proportion of mucin absence, weak staining, fine and organized fibers for Gomori's Trichrome, strong staining and organized fibers for Verhoeff when compared to controls. The wounds treated with 0.03% tacrolimus showed weak labeling for smooth muscle actin, a higher proportion of mucin absence, strong staining for Verhoeff when compared to the controls. There was absence of TGF-β and low VEGF expression. Study limitations: The analysis was performed by a single pathologist. Second-harmonic imaging microscopy was performed in 2 sample areas of the scar. Conclusions: Both drug concentrations were effective in suppressing TGF-β and smooth muscle actin, reducing mucin, improving the quality of collagen fibers, and the density of elastic fibers, but only the higher concentration influenced elastic fiber organization.

ABSTRACT BACKGROUND: Vesicourethral anastomotic stenosis (VUAS) following retropubic radical prostatectomy (RRP) significantly worsens quality of life. OBJECTIVES: To investigate the relationship between proliferative hypertrophic scar formation and VUAS, and predict more appropriate surgical intervention for preventing recurrent VUAS. DESIGN AND SETTING: Retrospective cross-sectional single-center study on data covering January 2009 to December 2019. METHODS: Among 573 male patients who underwent RRP due to prostate cancer, 80 with VUAS were included. They were divided into two groups according to VUAS treatment method: dilatation using Amplatz renal dilators (39 patients); or endoscopic bladder neck incision/resection (41 patients). The Vancouver scar scale (VSS) was used to evaluate the characteristics of scars that occurred for any reason before development of VUAS. RESULTS: Over a median follow-up of 72 months (range 12-105) after RRP, 17 patients (21.3%) had recurrence of VUAS. Although the treatment success rates were similar (79.5% versus 78.0%; P = 0.875), receiver operating characteristic (ROC) curve analysis indicated that dilatation using Amplatz dilators rather than endoscopic bladder neck incision/resection in patients with VSS scores 4, 5 and 6 may significantly reduce VUAS recurrence. A strong positive relationship was observed between VSS and total number of VUAS occurrences (r: 0.689; P < 0.001). VSS score (odds ratio, OR: 5.380; P < 0.001) and time until occurrence of VUAS (OR: 1.628; P = 0.008) were the most significant predictors for VUAS recurrence. CONCLUSIONS: VSS score can be used as a prediction tool for choosing more appropriate surgical intervention, for preventing recurrent VUAS.

Hypertrophic scar (HS) formation is a common complication that develops after skin injury; however, there are few effective and specific therapeutic approaches for HS. Emodin has previously been reported to inhibit mechanical stress-induced HS inflammation. Here, we investigated the molecular mechanisms underlying the inhibitory effects of emodin on HS formation. First, we conducted in vitro assays that revealed that emodin inhibited M1 and M2 polarization in rat macrophages. We subsequently established a combined rat model of tail HS and dorsal subcutaneous polyvinyl alcohol (PVA) sponge-induced wounds. Rats were treated with emodin or vehicle (DMEM). Tail scar specimens were harvested at 14, 28, and 42 days post-incision and subjected to H&E staining and Masson's trichrome staining. Histopathological analyses confirmed that emodin attenuated HS formation and fibrosis. Macrophages were separated from wound cells collected from the PVA sponge at 3 and 7 days after implantation. Flow cytometry analysis demonstrated that emodin suppressed in vivo macrophage recruitment and polarization at the wound site. Finally, we explored the molecular mechanisms of emodin in modulating macrophage polarization by evaluating the expression levels of selected effectors of the Notch and TGF-β pathways in macrophages isolated from PVA sponges. Western blot and qPCR assays showed that Notch1, Notch4, Hes1, TGF-β, and Smad3 were downregulated in response to emodin treatment. Taken together, our findings suggested that emodin attenuated HS formation and fibrosis by suppressing macrophage polarization, which is associated with the inhibition of the Notch and TGF-β pathways in macrophages.

La circuncisión es una cirugía practicada frecuentemente en urología. Sin tener en cuenta los principios básicos de la cirugía reconstructiva, puede generar procesos de cicatrización anómalos, con resultados estéticos y funcionales inadecuados, fimosis secundaria, cicatrización hipertrófica, queloide o "pene enterrado." El objetivo de este video es recordar la importancia de operar con las directrices de una cirugía reconstructiva, resaltando el cuidado de los tejidos y el uso de suturas de bajo calibre, para prevenir complicaciones y resultados insatisfactorios, resaltando la preservación de las zonas postuladas por Firlit. Se sospecha que las complicaciones de este procedimiento son subestimadas por los especialistas. El manejo cuidadoso de los tejidos, el uso de suturas de bajo calibre y los puntos subcuticulares con poca tensión, son medios para optimizar el proceso de cicatrización, evitando la isquemia y las cicatrices hipertróficas, basados en el fundamento de que esta es una cirugía no solo funcional, sino reconstructiva y estética. La circuncisión es un procedimiento que debe conservar la funcionalidad y estética del pene, por lo que debe ser realizado bajo las directrices de una cirugía


Circumcision is a frequently practiced surgical procedure in urology. Without considering the basic principles of reconstructive surgery, it can result in abnormal cicatrization processes, with inadequate aesthetic or functional results, secondary phimosis, hypertrophic or keloid scarring, or even "buried penis." The objective of this video is to remind the importance of operating with the principles of reconstructive surgery, highlighting the careful handling of tissues and the use of low-caliber sutures, to prevent complications and unsatisfactory results, preserving the areas postulated by Firlit. The complications of this procedure are suspected to be underestimated by specialists. Careful tissue management, the use of low-caliber sutures and subcuticular points with no tension are means to optimize the healing process, avoiding ischemia and hypertrophic scars, based on the fact that this is a surgery that is not only functional, but reconstructive and aesthetic as well. Circumcision is a procedure that must preserve the functionality and aesthetics of the penis and it must be performed under the principles of reconstructive surgery.

INTRODUCCIÓN. El proceso de cicatrización puede derivar en anomalías, que afectan el aspecto estético y funcional de la zona afectada; la combinación de tratamientos ha permi-tido resultados favorables. OBJETIVOS. Describir los factores que se dan en las recidivas de cicatriz queloide en pacientes tratados con resección quirúrgica más radioterapia. MA-TERIALES Y MÉTODOS. Estudio observacional, descriptivo y retrospectivo. Población de 2 960 Historias Clínicas, se tomó muestra de 100. Criterios de inclusión: diagnóstico de cicatriz queloide, edad de 12 a 75 años, combinación de tratamiento quirúrgico y radiotera-pia. Criterios de exclusión: edades fuera del rango, tratamiento diferente, en la Unidad de Plástica y Reconstructiva del Hospital de Especialidades Carlos Andrade Marín durante el período enero 2013 a diciembre 2019. Los datos fueron tomados del sistema AS400, el análisis se realizó en el programa estadístico International Business Machines Statistical Package for the Social Sciences, versión 22. RESULTADOS. La localización frecuente fue en el pabellón auricular con 83% (83; 100), de estos el 57% (57; 100) fueron poste-rior a perforación; la recidiva se presentó en el 24% (24; 100) y la principal complicación en los pacientes fue Radiodermitis. DISCUSIÓN. La evidencia científica guardó relación con el estudio referente a técnica, sexo, localización del queloide, causa y complicación. CONCLUSIÓN. Se pudo describir los factores que se dieron en las recidivas de cicatriz queloide en pacientes tratados con resección quirúrgica más radioterapia.


INTRODUCTION. The healing process can lead to anomalies, which affect the aesthe-tic and functional appearance of the affected area; the combination of treatments have allowed favorable results. OBJECTIVES. Describe the factors that occur in keloid scar recurrences in patients treated with surgical resection plus radiotherapy. MATERIALS AND METHODS. Observational, descriptive and retrospective study. Population of 2 960 Clini-cal Histories, a sample of 100 was taken. Inclusion criteria: diagnosis of keloid scar, age 12 to 75 years, combination of surgical treatment and radiotherapy. Exclusion criteria: ages outside the range, different treatment, in the Plastic and Reconstructive Unit of the Carlos Andrade Marín Specialty Hospital during the period January 2013 to December 2019. The data were taken from the AS400 system, the analysis was carried out in the program Sta-tistical International Business Machines Statistical Package for the Social Sciences, ver-sion 22. RESULTS. The frequent location was in the auricle with 83% (83; 100), of these 57% (57; 100) were after perforation; recurrence occurred in 24% (24; 100) and the main complication in patients was Radiodermatitis. DISCUSSION. The scientific evidence was related to the study referring to technique, sex, location of the keloid, cause and compli-cation. CONCLUSION. It was possible to describe the factors that occurred in keloid scar recurrences in patients treated with surgical resection plus radiotherapy.

Resumen Las quemaduras secundarias a agresión física con intención de desfigurar, torturar o incluso asesinar, se han convertido en un motivo de consulta común en el siglo XXI, siendo Bangladesh el país con la más alta incidencia en el mundo. Colombia es uno de los países con mayor incidencia a nivel de Latinoamérica. La mayoría de las lesiones ocurre en áreas expuestas como el rostro y se acompaña de graves secuelas físicas, estéticas y funcionales. Se realiza la presentación de caso de una paciente de 35 años con quemaduras de tercer grado en región frontal, periocular, malar bilateral, nasal, labial superior y pabellón auricular derecho, manejada con injertos de piel, quien posteriormente desarrolla cicatriz hipertrófica. Se describe el tratamiento con máscara termoplástica elaborada por los profesionales tratantes, con cubierta interna de silicona, fabricada sobre molde a medida y ajustada con bandas elásticas; integrando en un único dispositivo removible, cómodo y de bajo costo, diferentes alternativas terapéuticas que logran modular efectivamente el proceso de cicatrización y por su simplicidad favorecen la adherencia al tratamiento, la cual es indispensable para obtener resultados satisfactorios. Abstract Burns secondary to physical aggression with the intention of disfiguring, torturing or even murdering, have become a common reason for consultation in the 21st century, with Bangladesh being the country with the highest incidence in the world. Colombia is one of the countries with the highest incidence in Latin America. Most injuries occur in exposed areas such as the face and are accompanied by serious physical, aesthetic and functional sequelae. We present the case of a 35-year-old patient with third degree burns in the frontal, periocular, bilateral malar, nasal, upper labial and right ear region, managed with skin grafts, who later developed a hypertrophic scar. The treatment with a thermoplastic mask made by the treating professionals is described, with an internal silicone cover, made on a custom mold and adjusted with elastic bands; integrating in a single removable, comfortable and low-cost device, different therapeutic alternatives that manage to effectively modulate the healing process and, due to their simplicity, favor adherence to treatment, which is essential to obtain satisfactory results. Abstract Objective: To determine the effect type I collagen gene polymorphism alpha-2 (COL1A2) (rs42524) on the formation of scar tissue that is localized in the head and neck areas. Material and Methods: Sixty patients with scars in different areas of the head and neck were examined. The patients were divided into four subgroups, according to the types of scarring: G I: 15 patients with normotrophic scars; G ІІ: 15 patients with atrophic scars; G ІІІ: 15 patients with hypertrophic scars; and G IV: 15 patients with keloid scars. The age of patients ranged from 17 to 54 years. The single-nucleotide polymorphic site of the COL1A2 (rs42524) gene was detected by a polymerase chain reaction and subsequent analysis of restriction fragment lengths. Pearson's chi-squared test with Yates's correction and Fischer's exact test were used. Results: There were no significant changes between the control and basic groups (p=0.83) at analyzing the frequencies of G and C alleles. For the G allele, the calculation of odds ratio between the basic and control groups was 0.93 at 95% confidence interval (CI) (0.50-1.75), for the C allele - OR was 1.07 at 95% CI (0.57-2.01). Conclusion: Our studies may indirectly indicate the activation of the skin's protective reaction to physiological scarring and dosed scar formation in different areas of the head and neck.

Introdução: Abdominoplastia é um procedimento não apenas com características estéticas, mas também de reconstrução estrutural da parede abdominal. O objetivo do trabalho é mostrar a experiência dos autores em abdominoplastias, enfocando nos resultados estéticos da cicatriz e a evolução destes pacientes, aplicando as técnicas de ressecção em bloco do Professor Ronaldo Pontes (RP). Métodos: O estudo foi uma série retrospectiva de casos de 124 pacientes, entre março de 2014 a março de 2017, submetidos à abdominoplastia em bloco pela técnica RP. Resultados: Na nossa casuística, demonstrou-se uma mínima incidência de complicações encontradas com a técnica em Bloco RP, em comparação às encontradas na literatura, e, também, demonstrou um número pífio de casos de alterações na cicatrização. Conclusão: A técnica descrita e suas variantes atendem a necessidade de diversos tipos de casos e garantem cirurgias seguras e eficazes, com resultados muito satisfatórios, sendo uma técnica reprodutível.


Introduction: Abdominoplasty involves not only aesthetic characteristics but abdominal wall structural reconstruction. This study aimed to illustrate the authors' experience with abdominoplasty, focusing on the scar's aesthetic results and the evolution of application of the block resection technique of Professor Ronaldo Pontes (RP). Methods: The study included a series of retrospective cases of 124 patients treated between March 2014 and March 2017 who underwent RP block abdominoplasty. Results: In our studies, a minimal incidence of complications and a small number of healing alterations were noted with the RP block technique compared to those found in the literature. Conclusion: A técnica descrita e suas variantes atendem a necessidade de diversos tipos de casos e garantem cirurgias seguras e eficazes, com resultados muito satisfatórios, sendo uma técnica reprodutível.

Durante muchos años, el gel de silicona ha tenido un rol primario en el tratamiento y la prevención de cicatrices anómalas en forma de cicatrices hipertróficas y queloides luego de la epitelización. Los autores publican los hallazgos preliminares sobre el uso de un novedoso apósito de gel de silicona grado médico que forma una película y que fue aprobado para ser usado en heridas abiertas y en lesiones de la piel como único tratamiento y en combinación con otros tratamientos previos a la reepitelización. Un estudio observacional de 105 pacientes analizó la efectividad del gel de silicona en la estimulación de la epitelización acelerada, la reducción de la respuesta inflamatoria y la prevención de la formación de cicatrices. El estudio se realizó en una variedad de intervenciones quirúrgicas dermatológicas. Las observaciones de los autores confirmaron el rol de la silicona en la aceleración de la cicatrización de heridas, la prevención de la formación de cicatrices y la utilidad de este nuevo apósito de silicona que forma una película cuando se lo combina con otras modalidades de tratamiento. For many years, silicone gel has played a primary role in the treatment of prevention of abnormal scars in the form of after epithelialization. The authors publish the pre-elimination findings use of a novel medical grade silicone gel dressing that form a film that was approved for use in open wounds and in skin as the only treatment and in combination with other treatments prior to re-epithelialization. An observational study of 105 patients analyzed the effectiveness of silicone gel in the stimulation of accelerated epithelialization, reducing the inflammatory response and preventing the formation of scars. The study was conducted on a variety of dermatological surgeries. The authors' observations confirmed the role of silicone in acceleration of wound healing, prevention of the formation of the usefulness of this new silicone dressing that forms a film it is combined with other forms of treatment.

Abstract BACKGROUND: Tacrolimus, for its activity on modulation of collagen production and fibroblast activity, may have a role in the prevention of hypertrophic scars. OBJECTIVES: Evaluate macroscopic, microscopic, metabolic, laboratory effects and side effects of the use of topical tacrolimus ointment, in different concentrations, in the prevention of hypertrophic scars. METHODS: Twenty-two rabbits were submitted to the excision of 2 fragments of 1 cm of each ear, 4 cm apart, down to cartilage. The left ear of the animals was standardized as control and Vaseline applied twice a day. The right ear received tacrolimus ointment, at concentrations of 0.1% on the upper wound and 0.03% on the lower wound, also applied twice a day. Macroscopic, microscopic, laboratory criteria and the animals' weight were evaluated after 30 days of the experiment. RESULTS: Wounds treated with tacrolimus, at concentrations of 0.1% and 0.03%, when compared to control, showed a lower average degree of thickening (p = 0.048 and p <0.001, respectively). The average of scar thickness and lymphocyte, neutrophil and eosinophil concentrations are lower in the treated wounds compared to the control (p <0.001, p=0.022, p=0.007, p=0.044, respectively). The mean concentration of lymphocytes is lower in wounds treated with a higher concentration of the drug (p=0.01). STUDY LIMITATIONS: experiment lasted only 30 days. CONCLUSIONS: Tacrolimus at the 2 concentrations evaluated reduced the severity of inflammatory changes and positively altered the macroscopic aspect of the scar in the short term. Its use was shown to be safe, with no evidence of systemic or local adverse effects.

Introdução: Cicatrizes hipertróficas e queloides causam dano estético e funcional e são de difícil tratamento. O objetivo desta revisão foi identificar estudos prospectivos do tratamento com o laser fracionado de CO2, mostrando as alterações clínicas e histológicas e a metodologia utilizada para a avaliação das cicatrizes antes e após intervenção. Métodos: Foi realizada uma revisão eletrônica (LILACS, Medline e SciELO) de estudos publicados entre janeiro de 2004 e dezembro de 2017, com os termos "keloid/queloide", "hypertrophic scar/cicatriz hipertrófica" e "laser CO2", de acordo com o PRISMA Statement, sendo selecionados os estudos que comparassem as cicatrizes antes e depois de tratamento isolado com laser fracionado de CO2. Os dados foram analisados por dois revisores independentes. Resultados: Foram analisados 102 artigos, sendo que 7 cumpriam os critérios estabelecidos. Destes, os 7 analisaram cicatrizes hipertróficas, 2 deles também analisaram queloides, e 3 estudaram alterações histológicas. Houve diferença estatística entre os escores clínicos medidos antes e após tratamento de cicatrizes hipertróficas na maioria dos estudos, com melhora nos sintomas, na flexibilidade e altura da cicatriz. Entre os 2 estudos que analisaram os queloides, 1 deles demonstrou diferença clínica após tratamento. Nas alterações histológicas, houve diferença na orientação e densidade das fibras de colágeno e na espessura da epiderme. Conclusão: O laser fracionado de CO2 deve ser considerado como opção promissora no tratamento de cicatrizes patológicas, visto que melhora os sinais e sintomas clínicos como cor, espessura e prurido.


Introduction: Hypertrophic scars and keloids cause aesthetic and functional damages, and are difficult to treat. This review aimed to identify prospective studies on fractional CO2 laser to present the clinical and histological changes and the methodology used for the evaluation of scars before and after intervention. Methods: We conducted an electronic review (LILACS, Medline, and SciELO) of studies published between January 2004 and December 2017, using the search terms "keloid/queloide," "hypertrophic scar/cicatriz hipertrófica," and "CO2 laser ," according to the PRISMA Statement. Studies that compared scars before and after isolated treatment with fractional CO2 laser were selected. Two independent reviewers analyzed the data. Results: One hundred two articles were analyzed, of which 7 met the inclusion criteria. Of the 7 articles, all analyzed hypertrophic scars, 2 analyzed keloids in addition to hypertrophic scars, and 3 analyzed histological changes. Most studies showed a statistically significant difference in clinical scores between before and after treatment of hypertrophic scars, with improvement in symptoms, flexibility, and scar height. Between the 2 studies that analyzed keloids, 1 reported a clinical difference after treatment. The histological changes showed significant differences in the orientation and density of the collagen fibers, and in the thickness of the epidermis. Conclusion: The use of fractional CO2 laser should be considered as a promising treatment option for pathological scars, as it improves clinical signs and symptoms such as color, thickness, and pruritus.

Introdução: As cicatrizes são muito prevalentes na população mundial e podem ter impacto psicológico importante na vida do paciente. Já existem métodos clínicos e cirúrgicos convencionais para seu tratamento. Entretanto, a recidiva e os efeitos colaterais apresentados impulsionam a busca por novas modalidades terapêuticas. Objetivo: Avaliar a melhora clínica de pacientes com cicatrizes hipertróficas tratados com laser fracionado ablativo associado a drug delivery de triancinolona. Material e métodos: Foram selecionadas 18 pacientes com cicatrizes para serem tratadas, por oito semanas, com quatro sessões quinzenais de laser fracionado ablativo associado a drug delivery de triancinolona 20mg/ml. Antes e após o tratamento, o grupo foi avaliado por três examinadores por meio da evolução fotográfica, considerando-se os parâmetros aparência, discromia e grau de hipertrofia. Resultados: Completaram o estudo 15 pacientes. Destas, 35,53% apresentaram pouca ou nenhuma melhora; 37,80%, melhora moderada a intermediária; 8,89%, melhora importante e 17,80%, melhora próxima à resolução. Conclusões: A associação proposta é uma opção terapêutica promissora para o tratamento de cicatrizes hipertróficas por ser de rápida realização e cicatrização tecidual, com melhores resultados em relação às terapêuticas aplicadas isoladamente, com menores efeitos colaterais e com mais tolerabilidade pelo paciente, além de ser facilmente reproduzível em consultório.


Introduction: Scars are very prevalent in the world's population and can have a significant psychological impact in the patient's life. There are already conventional clinical and surgical methods for their treatment. However, recurrence and existing side effects drive the search for new therapeutic modalities. Objective: To evaluate the clinical improvement of patients with hypertrophic scarring treated with fractional ablative laser associated to triamcinolone drug delivery. Material and methods: Eighteen patients with scars to be treated were selected during eight weeks, with four fortnightly sessions of fractional ablative laser associated to 20mg/ml triamcinolone drug delivery. Before and after the treatment, the group was evaluated by three examiners, through the photographic evolution , considering the parameters appearance, dyschromia and degree of hypertrophy. Results: Fifteen patients completed the study. Of these, 35.53% had no or little improvement; 37.80%, moderate to intermediate improvement; 8.89%, expressive improvement and 17.80%, improvement near resolution. Conclusions: The proposed association is a promising therapeutic option for the treatment of hypertrophic scars because it is quick to perform and to heal, with less side effects and better tolerated by the patient when compared to the therapies used alone, besides being easily replicable in the office.

Dermatofibrosarcoma protuberans (DFSP) is an uncommon soft tissue neoplasm of low metastatic potential notable for its progressive growth and high rate of local recurrence after surgical excision. Fibrosarcomatous transformation of DFSP (FS-DFSP) is a rare variant characterized by higher rates of local recurrence and metastasis. Trauma has been hypothesized as a potential risk factor for the development of DFSP, although clear evidence has been lacking. In this study, we report a case of FS-DFSP that was found arising from a previously stable scar following a traumatic injury. A 49-year-old male was diagnosed with keloid scars following a motor vehicle accident where he sustained trauma. 12 years later, a large tumor developed immediately after a second traumatic event to the primary scar. Pathology of the excisional biopsy specimen demonstrated FS-DFSP with focal areas consistent with keloid and hypertrophic scar. This observation demonstrates the development of DFSP from underlying scar following a clear history of trauma. Furthermore, it suggests trauma as a possible trigger for the fibrosarcomatous transformation of DFSP.

Abstract: This study describes a case of a 19-year-old patient with seven asymptomatic lesions on the chest, measuring between 0.5 to 1cm in diameter, with no history of trauma in the region. The immunohistochemical evaluation was positive for vimentin and smooth muscle actin, determining Dermatomyofibroma as definitive diagnosis. Dermatomyofibroma is a benign skin tumor, with a myofibroblastic origin, prevalent in young women. It usually presents as a single lesion, with very few reports of multiple lesions.

Introdução: O abdome é um segmento estético-funcional importante na caracterização do contorno corporal. Na abdominoplastia, o posicionamento e o formato da cicatriz umbilical são importantes para um resultado satisfatório. Este estudo teve como objetivo descrever a técnica de onfaloplastia vertical, bem como apresentar os resultados obtidos. Métodos: Estudo longitudinal retrospectivo descritivo composto por pacientes submetidos à onfaloplastia vertical durante a abdominoplastia clássica no período de 2014 a 2017. A técnica consistiu em uma incisão linear vertical transcutânea sem retirada de fuso de pele e com mínimo desengorduramento da parede ao redor do umbigo, seguida da confecção de um ponto na porção lateral do pedículo umbilical, transfixante, permitindo sua invaginação e fixação rente à aponeurose do reto abdominal. Resultados: Do total de 128 pacientes avaliados, a faixa etária variou de 25 a 62 anos, com média de 32 anos. O tempo médio do ato operatório foi de 2 horas e 33 minutos. Não foram observadas complicações cirúrgicas como hematoma ou necrose do retalho abdominal. Foram observados 2 casos de deiscências, 4 casos de cicatriz hipertrófica, com 2 evoluindo para estenose umbilical e 1 caso de necrose umbilical. No seguimento ambulatorial 92,2% dos pacientes mostraram-se muito satisfeitos com o resultado final obtido. A análise técnica apresentou um grau de satisfação de 88,8% dos casos. Conclusão: A técnica descrita apresenta resultados satisfatórios, diminui a possibilidade do surgimento do anel cicatricial e permite minimizar um dos estigmas da abdominoplastia, que é uma cicatriz umbilical muito visível.


Introduction: The abdomen is an important aesthetic functional segment in the characterization of the body contour. In abdominoplasty, the placement and shape of the umbilical scar are important for a satisfactory result. This study aimed to describe the technique of vertical omphaloplasty and to present the results obtained. Methods: Retrospective longitudinal descriptive study composed of patients who underwent vertical omphaloplasty during classic abdominoplasty between 2014 and 2017. The technique consists of a vertical transcutaneous incision without removal of the skin spindle and with minimal degreasing of the wall around the umbilicus, followed by the transfixing a point in the lateral portion of the umbilical pedicle, allowing its invagination and fixation close to the aponeurosis of the rectus abdominis. Results: In the 128 patients evaluated, the patients' age ranged from 25 to 62 years, with a mean of 32 years. The mean time of surgery was 2 h and 33 min. No surgical complications occurred, such as hematoma or abdominal flap necrosis. Two cases of dehiscence, four cases of hypertrophic scar, two cases of stenosis, and one case of umbilical necrosis were observed. In outpatient follow-up, 92.2% of the patients were very satisfied with the final result. The technical analysis presented an 88% degree of satisfaction. Conclusion: The technique described presents satisfactory results, reduces the possibility of the appearance of a cicatricial ring, and allows minimizing one of the stigmas of abdominoplasty, which is a very visible umbilical scar.

Abstract: Background: Surgical sutures, wound tension, additional skin incisions and other factors may result in recurrence of tumor-like scar. Objective: To investigate the role of wound natural healing therapy in tumor-like hypertrophic scar. Methods: In this study, tumor-like hypertrophic scars of 47 cases were excised completely and the residual wounds were treated with natural healing. The short-term and long-term effects of treatment were evaluated. Results: All cases were successfully cured by natural healing therapy. The healing time of the maximum wound (80mm × 20mm) and the minimal wound (5mm× 5mm) was 25 days and 7 days respectively. The size of new skin scars ranged from 3mm to 11 mm. Clinical followed-up was performed in 34 cases for 36 months. Among them, no recurrence happened in 31 cases and new scar size ranged from 2mm to 8mm, while local recurrence happened in 3 cases whose scar size were less than 5 mm. Study Limitations: The cure rate of the therapy was 91.2%. Conclusion: The wound natural healing therapy is effective in treating tumor-like hypertrophic scar, which can prevent recurrence and has good cosmetic results.

BACKGROUND: Hypertrophic scarring (HS) is a severe disease, and results from unusual wound healing. Col1A1 could promote the hypertrophic scar formation, and the expression of Col1A1 in HS tissue was markedly higher than that in the normal. In present study, we aimed to identify miRNAs as post-transcriptional regulators of Col1A1 in HS. METHODS: MicroRNA-98 was selected as the key miRNA comprised in HS. The mRNA levels of miR-98 in HS tissues and the matched normal skin tissues were determined by qRT-PCR. MTT and flow cytometry were used to determine the influence of miR-98 on cell proliferation and apoptosis of HSFBs, respectively. Col1A1 was found to be the target gene of miR-98 using luciferase reporter assay. Luciferase assay was performed to determine the relative luciferase activity in mimic NC, miR-98 mimic, inhibitor NC and miR-98 inhibitor with Col1A13'-UTR wt or Col1A13'-UTR mt reporter plasmids. The protein expression of Col1A1 in HSFBs after transfection with mimic NC, miR-98 mimic, inhibitor NC and miR-98 inhibitor were determined by western blotting. RESULTS: The mRNA level of miR-98 in HS tissues was much lower than that in the control. Transfection of HSFBs with a miR-98 mimic reduced the cell viability of HSFBs and increased the apoptosis portion of HSFBs, while inhibition of miR-98 increased cell viability and decreased apoptosis portion of HSFBs. miR-98 inhibitor increased the relative luciferase activity significantly when cotransfected with the Col1A1-UTR reporter plasmid, while the mutant reporter plasmid abolished the miR-98 inhibitor-mediated increase in luciferase activity. Western blotting revealed that overex-pression of miR-98 decreased the expression of Col1A1. CONCLUSIONS: Overexpression of miR-98 repressed the proliferation of HSFBs by targeting Col1A1.
