RESUMO
True spontaneous rupture of the diaphragm, as a consequence of pregnancy or delivery, is exceptionally rare; one only case has been recorded. We report a further case where diaphragmatic rupture was followed by herniation and subsequent infarction of the caecum, ascending colon and most of the transverse colon. The diagnostic difficulties, pathophysiology and management are discussed (AU)

The relatively contribution of the rib cage and abdomen-diaphragm to tidal volume in emphysematous patients and a control group of healthy individuals was determined during normal tidal inspiration. In the control group the distribution curve was right skewed, whereas it was bimodal in the emphysematous patients. The bimodal shape was attributed to the increased anteroposterior chest diameter and flattened diaphragm which reduced diaphragmatic contribution, and also tot he activity of the accessory muscles of respiration which resulted in a greater contribution to tidal volume by the upper thorax. The morphology is described of the tissue stages of Trypanosoma cruzi observed in strains isolated in British Honduras. The persistent presence of crithidia forms of the parasite in infected vertebrate tissues suggests that T. cruzi strains found in British Honduras differ from strains described from other geographical areas. It is suggested that such strains may be suitable for studies on trypanocidal drugs in vivo and for comparative studies on the morphology of reproduction in tissues. (AU)

A family planning service was instituted in Barbados in May, 1955. To evaluate the effectiveness of the service, 555 women who had previously attended the clinics and had fitted with a diaphragm were located and interviewed by nurses. The average period of follow-up was 17 months. Of women who were still in need of protection 63 per cent were found to be active cases. The majority of inactive cases had become inactive after using the prescribed method of contraception for a few months only. Whereas the pregnancy rate with use of diaphragm was 17 per 100 years of exposure, the corresponding rate based on the experience of all women after clinic attendance, including periods with and without use of diaphragm, was 29 per 100 years of exposure. It is estimated that the pregnancy rate among women who had attended the family planning clinics was reduced by approximately three-fifths, compared with the expected rate if they had not attended.
