RESUMO

A 16-year-old male, ORIF non-union of (R) medial epicondlye. He had no significant past medical history and his physical exam was essentially negative. He sustained the initial injury playing basketball. He was monitored with standard ASA monitors and induced width 1mg Midazolam 100macro grams Fentanyl and 250 mg Sodium Penthol. Endotracheal intubation was performed under relaxation using 50 mg Atracrium. Anesthesia was maintained with nitrous oxide, oxygen and Isoflourane changing to Desflurane at the end of the procedure, when he extubated and transferred to PACU where vital signs were stable but he had a harsh pan systolic murmur. On discharge from PACU, he was admitted and a Pediatric Cardiology consultation was requested. At 10:00p.m., six hours post-op the murmur was no longer heardand he remained asymptomatic. Our differential diagnosis was MVP IHSS and flow murmurs(AU)

RESUMO

Two unusual cases of triventricular hearts are described in which the right ventricle is partitioned into two chambers, the smaller of which is placed antero-superiorly and gives off the pulmonary trunk. Other anomalies present are high interventricular septal defect, overiding aorta and biscuspid pulmonary valves. Only a few cases of this anomaly appear to have been described. The possible pathogenesis is discussed and its relationship to Fallot's tetralogy is considered.(AU)

RESUMO

Over a period of 6 months a remarkable increase was seen in the incidence of acute nephritis among children in South Trinidad, and 263 cases were admitted to a general hospital. The patients were found to differ in their age, sex, and geographic distribution. The disease was associated with respiratory infection rather than skin infection, the form usually seen here, and the appearance of desquamation in many of the children suggested that the underlying organism was one capable of producing scarlet fever. The disease was mild. Only five children (1.9 percent) died, all from cardiac failure. Recovery appears so far to be complete in all but one of the survivors. (Summary)
