RESUMO

This study assessed public acceptance for two new vectorial control techniques for Chagas' disease: insecticidal paint and fumigant canisters. The study compared the two with traditional fenitrothion insecticide spraying. An experimental field study was performed in an endemic area of central Honduras from August to November 1992, a year after the initial application of the treatments. The objectives of the study were to determine the acceptability of the tools on the part of the population whose homes were treated, and on the part of the personnel applying the treatments. The sample size was drawn up according to a uniform protocol applied in six Latin American countries. For this study a total of 651 persons were surveyed in 15 rural communities. Along with the surveys, focus groups were used to collect information to learn the reasons for accepting or rejecting particular treatments. The survey was done with heads of households. Focus groups were done with heads of households and also with the field operators who applied the treatments. The research showed that insecticidal paint had a low level of community acceptance (28.8 per cent). Field operators strongly disliked the paint because of problems with its transport, application, unpleasant smell, and very low effectiveness against triatomines and pest insects. The traditional insecticide was more acceptable to the community (93.9 per cent) and to the field operators, especially for its strong effect against the triatomines and pest insects. The results showed that in order to increase the public acceptance for insecticidal paint, it would be necessary to make the paint easier to transport and apply and to increase its effectiveness. Because of their very low effectiveness, fumigant canisters did not represent an acceptable alternative for triatomine vector control. A public educational effort should be a component of any new control method developed

RESUMO

En el presente artículo se analiza el uso actual de mosquiteros y de otros materiales impregnados con insecticida en las Américas. Se examinan diversos estudios efectuados en Brasil, Colombia, Ecuador, Guatemala, Perú, Suriname y Venezuela y se llega a la conclusión de que, en su mayor parte, estos estudios adolecen de graves deficiencias de diseño experimental, problemas de excesiva brevedad, y medición inadecuada de indicadores de salud. En el análisis se resalta la gran dificultad de llevar a cabo estudios científicos que buscan determinar el efecto de los materiales tratados con insecticida en la incidencia de malaria. En particular, la baja incidencia de malaria en las Américas, la elevada prevalencia de Plasmodium vivax y de casos recurrentes, y la relación existente entre los patrones de actividad del ser humano y los hábitos de picadura crepusculares de ciertos vectores de la malaria impiden hacer experimentos de fácil diseño y ejecución. Por ahora sería prematuro usar mosquiteros u otros materiales impregandos con insecticida como componentes principales de un programa integral para el control de la malaria. No obstante, se recomienda que se considere la posibilidad de realizar ensayos e intervenciones bien diseñados a gran escala, siempre que se basen en un conocimiento profundo de la dinámica de la transmisión de la malaria en la zona en estudio

RESUMO

Este artículo describe la evolución, características y eficiencia de diversos métodos desarrollados para el control y vigilancia de la enfermedad de Chagas en Mambaí, Goiás, Brasil, durante el período 1980-1992. El sistema de vigilancia se apoyó en un programa educativo en las escuelas rurales del municipio y contó con la participación de líderes comunitarios y de los habitantes, que utilizaron las "unidades de vigilancia" (cajas de Gómez-Nuñez, carteles o calendarios detectores de heces de triatominos, bolsas de plástico y "equipos cilíndricos peridomiciliarios") y notificaron la existencia de focos triatomínicos residuales a una red de puestos colectores distribuida en todo el municipio. Toda vivienda infestada fue fumigada por el guardia rural federal y, en las localidades más alejadas o de difícil acceso, por un líder de la comunidad previamente capacitado para esta labor. Las notificaciones de triatominos efectuadas por los propios habitantes y las "unidades de vigilancia" procedieron de la mayor parte de las casas infestadas, lo cual permitió eliminar el Triatoma infestans de los domicilios. Estos resultados asociados a los bajos costos del sistema, lo transforman en candidato viable para su aplicación a escala nacional en países con escasos recursos humanos y financieros donde la enfermedad de Chagas es endémica

RESUMO

Aircraft disinsection with aerosol insecticides during flight has generally been held to be inadvisable because it was assumed that the insecticides would be rapidly removed by the cabin air-conditioning system. We have developed protocols to deliver 2 percent d-phenothrin at a dose of 35 g per 100 m3 in various aircraft, and trials undertaken on Boeing 747 and 767 aircraft showed that their air-conditioning systems do not preclude effective disinsection. Mortality levels of 100 percent for Culex quinquefasciatus and Musca domestica test insects were recorded under normal operating conditions during routine scheduled passenger flights with disinsection procedures undertaken at "blocks-away" or at "top-of-descent". As a result, "top-of-descent" disinsection has been introduced as the recommended procedure for aircraft landing in Australia(AU)
