Background: It is essential to study the availability of reproductive health services and the capacities of providers, to provide evidence for improving service quality. Aims: To identify the role of midwives in the provision of reproductive health services and recommend improvements. Methods: A national review of government health resources in the Islamic Republic of Iran was conducted to explore available reproductive health services. Through semi-structured interviews with 30 midwives, information was collected about the compatibility of services with the capacity and scope of the activities of midwives. A panel of 12 experts was assembled to develop a proposed service package. The content analysis method was applied to data analysis and interpretation. Results: The service package developed covered 82 services that midwives can offer at the 8 healthcare facility groups. Although midwives were trained to manage a range of primary and gynaecological care services, certain essential aspects of reproductive health services were not being delivered on the frontline. These include sexually transmitted diseases and human papilloma virus management, diagnosis and treatment of common gynaecological problems, sexual education and counselling, and childbirth services. Midwives were not adequately engaged to provide reproductive health care at the secondary level. Conclusion: There are drawbacks to the current reproductive health service delivery in the Islamic Republic of Iran. The service package designed and proposed in this study aims to strengthen reproductive health care services and planning and better integration of midwife-led programmes.

Background: Needlestick injuries (NSIs) are one of the most serious occupational hazards for healthcare workers (HCWs). Aims: The aim of this study was to evaluate the incidence and causes of NSIs globally. Methods: A systematic review and meta-analysis of data from January 2000 to May 2020 collected from Scopus, PubMed, Embase, Web of Science, and Google Scholar. The Newcastle–Ottawa Scale was used to assess the quality of the included articles. The data obtained were analysed by R version 3/5/0, and 113 articles were retrieved. Results: There were 113 studies with a total of 525 798 HCWs. The incidence of NSIs was 43%. Africa had the highest rate of these injuries of 51%, and the World Health Organization (WHO) African Region had the highest incidence among WHO regions of 52%. Women were more frequently affected by NSIs than men. Hepatitis C virus infection was the disease most commonly transmitted via NSIs (21%). The highest rates of NSIs according to causes, devices, hospital locations, occupations and procedures were for recapping of needles, needles, general wards, nurses and waste disposal, respectively. Conclusion: The incidence of NSIs is gradually decreasing. The findings of this study can contribute to improving the decision-making process for reducing NSIs in HCWs.

Recognizing the critical contribution of the nursing and midwifery professions to health systems, population health and efforts to achieve the SustainableDevelopment Goals and universal health coverage,and in response to World Health Assembly resolution WHA 74.15, this technical brief aims to: support governments to strengthen government chief nursing and midwifery officers (GCNMOs) in their countries; identify the current roles and responsibilities of GCNMOs and senior nursing and midwifery leaders in the WHO European Region; and explore the enablers to support GCNMOs and/or senior nursing and midwifery leaders to work more effectively to support improved health outcomes. The ultimate goal is to increase the impact of GCNMOs and senior nursing and midwifery leaders on health, social care and health workforce policy to improve health outcomes

RESUMO

Имитационное моделирование широко используется в клиническом обучении студентов и в последипломном повышении квалификации медицинских работников. Данный методический подход к преподаванию, усвоению знаний и оценке клинических навыков применяется с большой пользой на различных уровнях сестринского и акушерского образования. По данным опубликованных исследований, применение имитационных моделей в обучении сестринскому и акушерскому делу приносит значительную пользу как студентам, так и пациентам. Моделирование позволяет эффективно обучать медицинских работников выполнению безопасных и своевременных вмешательств в соответствии с международными рекомендациями, тем самым формируя более ответственное отношение студентов к предстоящей клинической деятельности и улучшая общее качество медицинской помощи. Цель настоящего руководства – помочь преподавателям сестринского и акушерского дела, желающим внедрить применение имитационного моделирования в качестве одной из учебных стратегий. В руководстве дан обзор основных характеристик моделирования, определены ключевые понятия, приведено теоретическое обоснование, описаны типы моделирования и разъяснены оптимальные пути планирования, практического осуществления и оценки эффективности данного подхода. Также дается ряд рекомендаций для преподавателей и менеджеров, которые хотели бы использовать имитационное моделирование в программах додипломного обучения по сестринскому и акушерскому делу или в процессе непрерывного образования / повышения квалификации на рабочем месте.

Публикация «Содействие применению научно обоснованной практики в сестринском и акушерском деле в Европейском регионе ВОЗ» представляет собой подготовленное под эгидой Европейского регионального бюро ВОЗ руководство для государств-членов, призванное всемерно повысить вклад медицинских сестер и акушерок в продвижение научно обоснованной практики и инноваций применительно к сестринскому и акушерскому делу. Основное предназначение данного руководства – способствовать развитию общего понимания научно обоснованной практики в деятельности медицинских сестер и акушерок и укрепить ее основы в Регионе. Это поможет руководителям, отвечающим за разработку и проведение политики здравоохранения, медицинским работникам и другим заинтересованным сторонам эффективно внедрять принципы научно обоснованной практики в сестринское и акушерское дело. В руководстве приведены примеры в поддержку медицинских сестер и акушерок в их усилиях, направленных на применение принципов научно обоснованной практики в клинической работе.

Медсестры и акушерки составляют половину численности профессиональных медицинских работников во всем мире. Они взаимодействуют с людьми на протяжении всей жизни и во всех типах условий, что составляет около 90% от общего числа контактов между пациентами и медицинскими работниками. Медсестры и акушерки могут предоставлять безопасные, высококачественные, действенные и ресурсоэффективные медицинские услуги, особенно для уязвимых, малообеспеченных и труднодоступных групп населения. Предоставление медсестрам и акушеркам возможности работать с полной реализацией своего профессионального потенциала имеет большое значение для достижения Целей в области устойчивого развития, обеспечения всеобщего охвата услугами здравоохранения и для удовлетворения текущих и будущих потребностей населения в области охраны здоровья. Настоящая Дорожная карта отражает первоочередные задачи, поставленные в Европейской программе работы ВОЗ, и содержит набор мероприятий, организованный в соответствии с четырьмя ключевыми областями политики и 12 стратегическими приоритетами, изложенными в документе ВОЗ «Глобальные стратегические направления укрепления сестринского и акушерского дела на 2021–2025 гг.» Предложены мероприятия для государств-членов и описаны конкретные области технической поддержки, которую может оказать Европейское региональное бюро ВОЗ.

Background: Despite the importance of gender and intersectionality in policy-making for human resources for health, these issues have not been given adequate consideration in health workforce recruitment and retention in Africa. Aims: The objective of this review was to show how gender intersects with other sociocultural determinants of health to create different experiences of marginalization and/or privilege in the recruitment and retention of human resources for health in Africa. Methods: This was rapid review of studies that investigated the intersectionality of gender in relation to recruitment and retention of health workers in Africa. A PubMed search was undertaken in April 2020 to identify eligible studies. Search terms used included: gender, employment, health workers, health workforce, recruitment and retention. Criteria for inclusion of studies were: primary research; related to the role of gender and intersectionality in recruitment and retention of the health workforce; conducted in Africa; quantitative or qualitative study design; and published in English. Results: Of 193 publications found, nine fulfilled the study inclusion criteria and were selected. Feminization of the nursing and midwifery profession results in difficulties in recruiting and deploying female health workers. Male domination of management positions was reported. Gender power relationship in the recruitment and retention of the health workforce is shaped by marriage and cultural norms. Occupational segregation, sexual harassment and discrimination against female health workers were reported. Conclusion: This review highlights the importance of considering gender analysis in the development of policies and programmes for human resources for health in Africa.

Nurses and midwives comprise half of the professional health workforce globally, interact with people frombirth to death across all types of settings and account for about 90% of contacts between patients andhealth professionals. They can deliver safe, high-quality, effective and efficient health services, particularly forvulnerable, disadvantaged and hard-to-reach groups. Enabling and supporting nurses and midwives to work totheir full potential is essential to achieving the Sustainable Development Goals and supporting universal healthcoverage, and to meeting current and future population health needs. This Roadmap reflects the priorities ofthe WHO European Programme of Work to set out activities organized according to the four areas of focusof the WHO Global strategic directions for nursing and midwifery 2021−2025 report (education, jobs, leadershipand service delivery) across 12 policy priorities. It proposes activities for Member States of the WHO EuropeanRegion and outlines specific areas of technical support the WHO Regional Office for Europe can provide.
