RESUMO

This report describes a unique case of a Coats-like presentation of familial exudative vitreoretinopathy in an 11-year-old girl. The patient was originally referred for evaluation of presumed Coats disease and presented with telangiectatic vessels, perivascular exudates, diffuse peripheral exudation, and intraretinal hemorrhages. Clinical and angiographical findings were consistent with familial exudative vitreoretinopathy, while genetic testing identified variants of uncertain significance in two associated genes, LRP5 and ZNF408. In silico analysis predicts the LRP5 variant to be pathogenic. Retinal vasculopathies often have phenotypic overlap, warranting angiographic examination of both eyes and genetic testing to uncover the correct diagnosis and guide proper treatment. A 37-week-old girl underwent ophthalmic examination. Born at 32 weeks, the infant weighed 680 grams and received high-flow nasal cannula for respiratory distress of the newborn. Dilated fundus examination of the right eye revealed an atypical chorioretinal coloboma; the left eye revealed hyperpigmentary changes in the macula. Fluorescein angiography of both eyes showed retinal vascularization to zone II. Genetic testing revealed a heterozygous variant of uncertain significance in the catenin Alpha 1 (CTNNA1) gene. CTNNA1 gene abnormalities have been implicated as causes of familial exudative vitreoretinopathy (FEVR). It is important to recognize possible simultaneous retinopathy of prematurity and FEVR. BACKGROUND AND OBJECTIVE: Retinal detachments (RDs) are a complication of X-linked retinoschisis (XLRS) with a poor prognosis. This study aims to report outcomes of XLRS-RD repair in pediatric patients. MATERIALS AND METHODS: The study is a retrospective analysis of pediatric patients undergoing vitreoretinal surgery for XLRS-RDs from 2000 to 2022. RESULTS: Nine patients (11 eyes) met inclusion criteria. All patients were boys, with a mean age of 8 years. Most RDs arose inferiorly (64%). Seven (64%) detachments were macula-involving. Seven (64%) detachments were repaired with combined scleral buckling and vitrectomy, two (18%) detachments were repaired with vitrectomy alone, one (9%) detachment underwent a primary scleral buckling procedure, and one (9%) asymptomatic detachment with a pigment demarcation line was observed. Silicone oil was used in eight of nine (89%) eyes undergoing vitrectomy. Final visual acuity was 20/200 or better in eight (73%) eyes. One eye that underwent surgical intervention remained unattached at follow-up. CONCLUSION: Surgical repair in patients with XLRSRDs was associated with successful reattachment, although visual prognosis varied. [Ophthalmic Surg Lasers Imaging Retina 2023;54:574-579.].

Methods: With and without agitation, the SR 1-ml insulin, Becton-Dickinson Ultra-Fine 0.3-ml Short Needle with a half-unit scale, HSW Norm-Ject Tuberculin, and Becton-Di­ckinson 1-ml Luer Lok Tip were examined with buffer and bevacizumab, aflibercept, and ziv-aflibercept. Flow imaging microscopy was performed to assess the particle numbers, concentrations, morphology, and size distribution. Results: Using the Becton-Dickinson Ultra-Fine syringe, the average particle count after agitation was higher than in the no-agitation group. For particles greater than 10 and 25 µm, differences were observed using the SR syringe between the two studied conditions. There were no significant differences in the means for the other syringes. Without agitation, the SR syringe had the highest number of particles (2,417,361.7 ± 3,421,575.5) followed by the Becton-Dickinson Ultra-Fine with 812.530,9 ± 996.187,2. The Becton-Dickinson Luer Lok Tip and HSW Norm-Ject performed equally with 398,396.8 ± 484,239.2 and 416,016.4 ± 242,650.1 particles, respectively. Conclusions: Flicking syringes to eliminate air bubbles results in increased numbers of particles released during intravitreal injections into the human vitreous.


RESUMO Objetivo: Visto que partículas são liberadas nas seringas durante as injeções intravítreas (IVIs), estas foram avaliadas quantitativamente após a agitação das seringas mais comumente usadas para injeções intravítreas. Métodos: A seringa SR de 1 ml de insulina, a agulha curta Becton-Dickinson Ultra-Fine 0,3 ml com escala de meia unidade, HSW Norm-Ject Tuberculin e a Becton-Dickinson Luer Lok Tip de 1 ml foram estudadas com placedo e com bevacizumabe, aflibercept e ziv-aflibercept, com e sem agitação. MicroFlow Imaging Mi­croscopy foi realizada para avaliar o número de partículas, con­centração, morfologia e distribuição das mesmas por tamanho. Resultados: A contagem média de partículas após agitação foi maior do que no grupo sem agitação usando a seringa Becton-Di­ckinson Ultra-Fine. Diferenças foram observadas usando a seringa SR entre as duas condições estudadas para partículas maiores que 10 e 25 µm. Para as demais seringas, não foram observadas diferenças significativas nas médias. A seringa SR apresentou o maior número de partículas sem agitação (2.417.361,7 ± 3.421.575,5) seguida da Becton-Dickinson Ultra-Fine com 812.530,9 ± 996.187,2. A BD Luer Lok Tip e a HSW Norm-Ject se comportaram de forma semelhante com 398.396,8 ± 484.239,2 e 416.016,4 ± 242.650,1 partículas, respectivamente. Conclusões: Agitar seringas para remover bolhas de ar resulta em um maior número de partículas liberadas durante Becton-Dickinson no vítreo humano.

ABSTRACT Retinal detachment (RD) is a common ophthalmic emergency that could bring permanent blindness if it is untreated or treatment is delayed. We conducted a review of Cochrane systematic reviews regarding retinal detachment interventions after a search strategy, we showed and analyzed the data narratively in Ophthalmologic Departure of Escola Paulista de Medicina-UNIFESP. As result, the group of pneumatic retinopexy was less susceptible to choroidal detachment and myopic shift as adverse events when compared with scleral buckle. Although there is no statistically significant difference in visual acuity between standard and heavy silicone oil and between C3F8 and silicone oil, the following interventions may present some benefits for RD: (a)LMWH with 5-FU versus placebo at high-risk of developing postoperative proliferative vitreoretinopathy (PVR); (b) silicone oil was favorable for macular attachment at 2 years compared to sulfur hexafluoride (SF6); (c) Retinal redetachment was reported in fewer participants in the Pars plana vitrectomy (PPV) group compared to the scleral buckling group. So, these points can be considered when choosing the technique to improve better results in cases of retinal detachment. In addition, there is still a need for studies with a prophylactic RD approach and studies with greater evidence of which surgical technique is most appropriate for each indication of RD considering the economic cost and the patient's quality of life.


RESUMO Descolamento de retina (DR) é uma emergência oftalmológica comum que pode evoluir como uma das causas de cegueira se não for tratada ou tiver o tratamento demorado. Esta é uma revisão de revisões sistemáticas da Cochrane sobre descolamento de retina, relacionada às intervenções realizadas no tratamento do DR, após uma estratégia de busca apresentamos e analisamos os dados narrativamente conduzida no departamento de oftalmologia da Escola Paulista de Medicina-UNIFESP. Como resultado, o grupo de retinopexia pneumática foi menos suscetível ao descolamento de coroide e miopização como efeito adverso em comparação ao grupo de introflexão escleral. Apesar de não ter diferença estatisticamente significante entre a acuidade visual entre o uso de óleo de silicone padrão versus pesado, nem entre C3F8 e óleo de silicone, as seguintes intervenções apresentaram benefícios para o tratamento: (a) Heparina de baixo peso molecular com 5 fluorouracil diminuíram o risco de evoluir com proliferação vítreo-retiniana; (b) Uso de silicone foi mais favorável como substituto vítreo na fixação macular em 2 anos comparado com o uso de hexafluoreto de enxofre; (c) Novo descolamento de retina foi menor em pacientes submetidos a Vitrectomia pars plana comparada ao grupo de introflexão escleral. Portanto, esses dados podem ser considerados na escolha da técnica empregada para obter melhores resultados nos casos de DR. Além disso, existe a necessidade de estudos de alto nível de evidência em busca do procedimento cirúrgico mais apropriado e profilático para DR, levando em consideração custo-benefício e qualidade de vida.

ABSTRACT Background: Retinopathy of prematurity (ROP) is a vasoproliferative retinal disorder that affects extremely premature infants and is the leading cause of irreversible blindness in childhood. Objective: This study presents a review of Cochrane systematic reviews about ROP. Methods: We analyzed and summarized the results of all intervention, prevention and treatment, from Cochrane Systematic Reviews (SR) for ROP designed to the highest standard of rigor to show the current position and propose studies that are missing for decision making. Results: Eight SR were analyzed in this study, four in prophylaxis and four in treatment of ROP. The outcome 'Beneficial effect of oral beta-blockers on progression to stage 3 ROP but not to stage 2 ROP with plus disease or to stage 4 or 5 ROP was the only one that presented moderate evidence quality, all the others outcomes presented evidence quality floating from low to very low. Conclusion: There is a lack of studies showing the quality of evidence in the treatment and prevention of retinopathy of prematurity, particularly in long-term outcomes after treatment in order to assess the impact and quality of life of these patients.


RESUMO Introdução: A retinopatia da prematuridade (ROP) é um distúrbio vasoproliferativo da retina que afeta recém nascidos extremamente prematuros e é a principal causa de cegueira irreversível na infância. Objetivo: Este estudo apresenta uma revisão das revisões sistemáticas da Cochrane sobre ROP. Métodos: Analisamos e resumimos os resultados de todas as intervenções, prevenção e tratamento, das revisões sistemáticas (SR) da Cochrane para ROP, projetada com o mais alto padrão de rigor para mostrar a posição atual e propor estudos que estão faltando para a tomada de decisão. Resultados: Oito RS foram analisados neste estudo, quatro em profilaxia e quatro em tratamento de ROP. O efeito benéfico dos betabloqueadores orais na progressão para a ROP do estágio 3, mas não para a ROP da fase 2 com doença positiva ou para a ROP da fase 4 ou 5 foi o único que apresentou qualidade de evidência moderada, todos os outros resultados apresentaram qualidade de evidência flutuante de baixo a muito baixo. Conclusão: Faltam estudos demonstrando a qualidade das evidências no tratamento e prevenção da retinopatia da prematuridade, principalmente em desfechos em longo prazo após o tratamento, a fim de avaliar o impacto e a qualidade de vida desses pacientes.

We conducted a case-control study using the Eyesi simulator to assess the surgical performance of 24 chronic cocaine users (CCUs) and 24 sex-/age-matched controls to numerically quantify ophthalmic microsurgical simulator performance and fine motor deficiencies. The inclusion criteria were no exposure to illicit drugs other than cocaine, marijuana, or alcohol within the previous month and no health conditions that could impact manual task performance. The outcomes included surgical scores (0-100, worst-best) and task completion time (minutes). Fisher's exact test, analysis of variance, Mann-Whitney U test, and Kruskal-Wallis test followed by Dunn-Bonferroni post-hoc were conducted for statistical analysis. The Eyesi scores were lower among CCUs compared to controls for bimanual tasks (4.50 ± 14.30 vs. 18.46 ± 26.64, p = 0.012), for exercises demanding upper and lower limb coordination (both hands and two foot pedals, respectively) (74.13 ± 35.01 vs. 85.21 ± 24.1, p = 0.045), and in the overall score for all three tasks (27.38 ± 15.06 vs. 39.5 ± 18.66, p = 0.021). CCUs took longer to complete tasks when performing exercises demanding upper and lower limb coordination compared to controls (1.26 ± 0.38 vs. 1.02 ± 0.44 min, p = 0.006). Individuals who used cocaine during the previous month had an independent lower bimanual score compared to controls (1.42 ± 4.91 vs. 18.46 ± 26.64, p = 0.018). No differences in performance among the CCUs were attributable to sporadic cannabis or alcohol use. Chronic use of cocaine negatively impacted fine dexterity as measured by bimanual tasks or maneuvers that required simultaneous coordination of the upper and lower limbs. This was most notable among individuals who used cocaine during the 1-month period before the simulation.

Intravitreal injections have become the most commonly performed intraocular treatments worldwide. Because intravitreal injections may induce severe adverse events, such as infectious and noninfectious endophthalmitis, cataract, ocular hypertension, vitreous hemorrhage, or retinal detachment, appropriate awareness of the materials and techniques used are essential to reduce these sight-threatening complications. This review provides insights into the needles, syringes, silicone oil coating, sterilization methods, devices to assist intravitreal injections, scleral piercing techniques using needles, syringe handling, anesthesia, and safety issues related to materials and techniques. It is paramount that physicians be aware of every step involved in intravitreal injections and consider the roles and implications of all materials and techniques used. The ability to understand the theoretical and practical circumstances may definitely lead to state-of-the-art treatments delivered to patients. The most important practical recommendations are: choosing syringes with as little silicone oil as possible, or, preferably, none; avoiding agitation of syringes; awareness that most biologics (e.g., antiangiogenic proteins) are susceptible to changes in molecular properties under some conditions, such as agitation and temperature variation; understanding that improper materials and techniques may lead to complications after intravitreal injections, e.g., inflammation; and recognizing that some devices may contribute to an enhanced, safer, and faster intravitreal injection technique.

Abstract This paper provides a fast and inexpensive technique to prevent slippage of the infusion cannula from the EVA DORC® 27-gauge system. After the usual retrobulbar anesthesia, antisepsis and asepsis, a 2% hydroxypropyl methylcellulose drop is placed around the infusion cannula in the trocar to hold it in position. This technique prevents the slippage of the infusion cannula and both the surgical time and creation of the first sclerotomy in the eye can be shortened slightly.


Resumo Este artigo fornece uma técnica rápida e barata para evitar o deslizamento da cânula de infusão do sistema EVA DORC® 27-gauge. Após a habitual anestesia retrobulbar, antissepsia e assepsia, uma gota de 2% de hidroxipropilmetilcelulose é colocada ao redor da cânula de infusão no trocater para mantê-la em posição. Essa técnica evita o deslizamento da cânula de infusão e o tempo cirúrgico e da criação da primeira esclerotomia no olho podem ser ligeiramente reduzidos.

Abstract Purpose: To summarize the evidence from Cochrane systematic reviews on interventions for Central or Branch Vein Occlusion. Methods: We included and summarized the results from Cochrane systematic reviews on interventions for both types of occlusion. The initial search retrieved was 21 reviews and four of them were selected. Results: The four systematic reviews included evaluated the effects of laser techniques and intravitreal injections of Anti-Vascular Endothelial Growth Factor (anti-VEGF) and corticosteroids on Branch and Central Retinal Vein Occlusions. Conclusions: In Branch Retinal Vein Occlusion was found some benefits in the use of grid laser when comparable to no intervention but insufficient evidence about the use of early grid laser, subthreshold laser, intravitreal triamcinolone or anti-VEGF over macular grid laser photocoagulation. In Central Retinal Vein Occlusion with Macular Edema was found insufficient evidence to determine the benefits of intravítreo steroids but ranibizumab may improve clinical and visual outcomes at six and 12 months and repeated intravitreal injection of anti-VEGF agents improved visual outcomes at six months when compared to no treatment.


Resumo Objetivo: Resumir as evidências das revisões sistemáticas da Cochrane sobre intervenções para oclusão de veia central ou de ramo. Métodos: Incluímos e resumimos os resultados das revisões sistemáticas da Cochrane sobre intervenções para os 2 tipos de oclusão. A busca inicial recuperada foi de 21 revisões e quatro delas foram selecionadas. Resultados: As quatro revisões sistemáticas incluídas avaliaram os efeitos das técnicas de laser e injeções intravítreas do Anti-Fator de Crescimento Endotelial Vascular (anti-VEGF) e corticosteroides nas oclusões de ramos e veias retinianas centrais. Conclusões: Na oclusão de veias retinianas do ramo foram encontrados alguns benefícios no uso do laser de grade, quando comparáveis a nenhuma intervenção, mas evidências insuficientes sobre o uso precoce do laser de grade, laser sublimiar, triamcinolona intravítrea ou anti-VEGF sobre a fotocoagulação a laser de grade macular. Na oclusão da veia central da retina com edema macular, foram encontradas evidências insuficientes para determinar os benefícios dos esteroides intravítreos, mas o ranibizumabe pode melhorar os resultados clínicos e visuais em 6 e 12 meses e a injeção intravítrea repetida de agentes anti-VEGF melhorou os resultados visuais em seis meses, quando comparado ao sem tratamento.

Abstract Purpose: To investigate the patients' perspectives regarding the introduction of the electronic medical record into use in an ophthalmologic hospital and its impact on the doctor/patient relationship. Methods: The cross-sectional study analyzed the impact of the electronic medical record on the doctor-patient relationship based on the patients' opinions after electronic medical record implementation compared with use of traditional paper records. The same doctor attended all patients and completed questionnaires during patient interviews that analyzed empathy, punctuality, efficiency, information clarity, doctor cordiality, respect, trustworthiness, patient benefits from the technology, confidentiality, and humanized care. The inclusion criteria included age of 18 years or older, adequate cognition, previous treatment in the same institution by the same doctor using paper medical records and later the electronic medical record, and free and informed written patient consent. The exclusion criteria included age below 18 years, inadequate time to answer the questionnaire, first patient visit, doubtful interview responses, and first visit before 6 months after electronic medical record implementation. The data were analyzed descriptively by relative and absolute frequencies. A previous pilot study of 20 patients yielded 95% confidence intervals for the percentages of agreement for the electronic medical record questionnaire responses obtained and found that 160 patients was adequate for performing the study. Results: The patients reported that the electronic medical record had a positive impact on the doctor-patient relationship in all areas considered. Over 94% of patients responded affirmatively when questioned about their confidence in the confidentiality of their data, 38.3% noted changes in the doctor's concern for service and 68% agreed that clarity of the information provided by the doctor was greater with the electronic medical record. Conclusion: Based on the patients' perceptions, the EMR positively affected the doctor-patient relationship after the implementation of the technology in a private ophthalmologic hospital.


Resumo Objetivo: Investigar as perspectivas dos pacientes em relação à introdução do prontuário eletrônico em uso em um hospital oftalmológico e seu impacto na relação médico / paciente. Métodos: O estudo transversal analisou o impacto do prontuário eletrônico na relação médico-paciente com base na opinião dos pacientes após a implementação do prontuário eletrônico em comparação com o uso de registros tradicionais em papel. O mesmo médico atendeu a todos os pacientes e completou questionários com pacientes que analisaram empatia, pontualidade, eficiência, clareza da informação, cordialidade do médico, respeito, confiabilidade, benefícios para o paciente da tecnologia, confidencialidade e cuidado humanizado. Os critérios de inclusão incluíam idade de 18 anos ou mais, cognição adequada, tratamento prévio na mesma instituição pelo mesmo médico, usando registros médicos em papel e, posteriormente, o prontuário eletrônico e consentimento livre e esclarecido por escrito do paciente. Os critérios de exclusão incluíram, idade abaixo de 18 anos, tempo inadequado para responder ao questionário, primeira consulta do paciente, respostas duvidosas à entrevista e primeira visita antes de 6 meses após a implementação do prontuário eletrônico. Os dados foram analisados descritivamente por frequências relativas e absolutas. Um estudo piloto prévio de 20 pacientes forneceu intervalos de confiança de 95% para as porcentagens de concordância para as respostas do questionário de prontuário eletrônico obtido e constatou que 160 pacientes eram adequados para realizar o estudo. Resultados: Os pacientes relataram que o prontuário eletrônico teve impacto positivo na relação médico-paciente em todas as áreas consideradas. Mais de 94% dos pacientes responderam afirmativamente quando questionados sobre sua confiança na confidencialidade de seus dados, 38,3% observaram alterações na preocupação do médico com o serviço e 68% concordaram que a clareza das informações fornecidas pelo médico era maior com o prontuário eletrônico. Conclusão: As vantagens do prontuário eletrônico foram o rápido acesso à informação, clareza dos dados, recuperação rápida e organizada da informação e agilidade nos serviços.

PURPOSE: We aimed to report the ocular manifestations observed in patients with psoriasis. METHODS: Patients were included and referred to our ophthalmology clinic from dermatology clinics of Universidade do Estado do Pará between October 2013 and August 2014. Clinical interviews were conducted to identify relevant epidemiological data, clinical features, and treatment details, and data were recorded using the same protocol. Subsequent dermatological examinations were performed and disease severity was rated using the Psoriasis Area and Severity Index and the Dermatological Life Quality Index. Complete eye examination was conducted, including visual acuity, biomicroscopy, tonometry, fundoscopy, Schirmer I test, tear breakup time, rose bengal staining, ocular surface disease index, and glaucoma tests. RESULTS: In total, we included 43 patients with psoriasis and 86 controls. Patients with psoriasis had statistically higher incidences of dry eye (16.28%), likely dry eye (32.56%), and blepharitis (16.28%). Furthermore, the rose bengal and ocular surface disease tests were more abnormal in patients with psoriasis (p<0.05). CONCLUSIONS: Patients with psoriasis should undergo regular eye exams, regardless of risk factors, to monitor for the progression of symptomatic or asymptomatic ocular manifestations.

RESUMO Objetivos: Relatar as manifestações oculares observadas em pacientes com psoríase atendidos no Ambulatório de Dermatologia da X e encaminhados ao Y, no período de outubro de 2013 a agosto de 2014. Métodos: A amostra foi constituída por um grupo composto por 43 pacientes com psoríase e um grupo controle com 86 pacientes sem psoríase. Foi realizada uma entrevista clínica com dados epidemiológicos, aspectos clínicos da doença e terapia empregada, sendo todas as informações registradas em protocolo próprio. Posteriormente, realizou-se o exame dermatológico, no qual foi avaliado o índice de gravidade da Psoríase por área (PASI) e índice dermatológico de qualidade de vida (DLQI), e o exame oftalmológico completo, incluindo: Acuidade Visual, Biomicroscopia, Tonometria, Fundoscopia, Teste de Schirmer I, Tempo de Ruptura do Filme Lacrimal (TBUT), rosa bengala, índice de doença da superfície ocular (OSDI) e exames para glaucoma. Resultados: Observou-se que nos pacientes com psoríase houve frequência estatisticamente maior de envolvimento ocular, como olho seco (16,28%), provável olho seco (32,56%) e blefarite (16,28%). Além disso, os valores do rosa bengala e do OSDI apresentaram-se mais alterados nos pacientes com psoríase (p<0,05). Conclusão: Dessa forma, sugere-se que esses pacientes realizem exames oftalmológicos periódicos, já que as manifestações oculares podem progredir sem sintomatologia e ocorrer independentemente de fatores de risco.

Undifferentiated pleomorphic sarcoma (UPS) is an extremely rare tumor that occurs in the head and neck region. Here, we report a unique case of a primary undifferentiated pleomorphic sarcoma in the orbital region. A 35-year-old woman presented with a progressive proptosis and periocular edema for 1 year. She had no previous history of surgery, skin malignancy, or radiation. Imaging tests showed an extraconal mass, not involving the muscles. The tumor was surgically removed and adjuvant radiotherapy was required after histological examination, which showed an undifferentiated pleomorphic sarcoma of the orbit. There was no recurrence after 1 year of follow-up. Though rare, undifferentiated pleomorphic sarcoma should be included in the differential diagnoses of orbital tumors.

RESUMO O sarcoma pleomórfico indiferenciado (SPI) é um tumor extremamente raro na região da cabeça e pescoço. Relatamos um caso de um sarcoma pleomórfico indiferenciado primário na região orbital. Uma mulher de 35 anos apresentou proptose progressiva e edema periocular há um ano. Ela não tinha histórico prévio de cirurgia, malignidade da pele ou radiação. Exames de imagem mostraram uma massa extraconal, poupando os músculos. O tumor foi removido cirurgicamente e foi necessária radioterapia adjuvante após o resultado histopatológico. O exame histológico demonstrou um sarcoma pleomórfico indiferenciado da órbita. Não houve recidiva após 1 ano de seguimento. Apesar de raro, o sarcoma pleomórfico indiferenciado deve ser incluído no diagnostico diferencial de qualquer tumor originado na órbita.

We report the case of a patient who received conjunctival tattooing for cosmetic purposes with associated complications. A 28-year-old male experienced severe ocular pain after undergoing conjunctival tattooing. Slit-lamp examination revealed blue deposits over the conjunctiva in association with intense conjunctival edema and a 4+ anterior chamber cellular reaction. The patient underwent clinical treatments to control ocular inflammation. Two months after the injury, the patient was still in follow-up to ensure that any further complications were managed and documented. This unusual case of conjunctival tattooing resulted in conjunctival edema and anterior uveitis. Because of the increasing popularity of eyeball tattooing, the potentially severe complications of this procedure may become more common.

RESUMO Relatamos um caso de um paciente que realizou tatuagem conjuntival para fins cosméticos com complicações associadas. Um homem de 28 anos de idade apresentou dor ocular importante após tatuagem conjuntival. O exame da lâmpada de fenda revelou depósitos azuis sobre a conjuntiva, associados a um intenso edema conjuntival e uma reação de câmara anterior de 4+ de células. O paciente foi submetido a tratamentos clínicos para controlar a inflamação ocular. Dois meses após o ocorrido, o paciente ainda estava em acompanhamento para garantir que quaisquer outras complicações fossem gerenciadas e documentadas. Um caso incomum de tatuagem conjuntival resultou em edema conjuntival e uveíte anterior. Devido à crescente popularidade da tatuagem no globo ocular, complicações potencialmente graves deste procedimento podem se tornar mais comuns.

PURPOSE: To report a case of conjunctival tattooing for cosmetic purposes with inadvertent globe penetration resulting in severe inflammation, capsular lens opacity, and secondary glaucoma. METHODS: Case report. RESULTS: A 25-year-old woman experienced severe ocular pain and decreased vision in the left eye after conjunctival tattooing for cosmetic purposes on the previous day. Slit-lamp examination revealed black deposits over the conjunctiva, corneal endothelium, anterior chamber angle, iris, and anterior capsular lens due to the tattooing pigment. In addition, severe anterior uveitis triggered by the pigments and, later, secondary glaucoma were diagnosed. The patient underwent clinical and surgical treatments to control ocular inflammation and intraocular pressure. Three months after the injury, the patient was still under follow-up treatment in an effort to control the complications of the eyeball tattooing. CONCLUSIONS: An unusual case of conjunctival tattooing resulted in severe inflammation, capsular lens opacity, and secondary glaucoma. Because of the increasing popularity of eyeball tattooing, coupled with the procedure being performed by untrained professionals, potentially severe complications of this procedure may become more common.

BACKGROUND: The cutaneous mycoses, mainly caused by dermatophyte fungi, are among the most common fungal infections worldwide. It is estimated that 10% to 15% of the population will be infected by a dermatophyte at some point in their lives, thus making this a group of diseases with great public health importance. OBJECTIVE: To analyze the clinical, epidemiological, and therapeutic profile of dermatophytosis in patients enrolled at the Dermatology service of Universidade do Estado do Pará, Brazil, from July 2010 to September 2012. METHOD: A total of 145 medical records of patients diagnosed with dermatophytosis were surveyed. Data were collected and subsequently recorded according to a protocol developed by the researchers. This protocol consisted of information regarding epidemiological and clinical aspects of the disease and the therapy employed. RESULTS: The main clinical form of dermatophyte infection was onychomycosis, followed by tinea corporis, tinea pedis, and tinea capitis. Furthermore, the female population and the age group of 51 to 60 years were the most affected. Regarding therapy, there was a preference for treatments that combine topical and systemic drugs, and the most widely used drugs were fluconazole (systemic) and ciclopirox olamine (topical). CONCLUSION: This study showed the importance of recurrent analysis of the epidemiological profile of dermatophytosis to enable correct therapeutic and preventive management of these conditions, which have significant clinical consequences, with chronic, difficult-totreat lesions that can decrease patient quality of life and cause disfigurement.
