Resumo

Purpose: Bone repair aims to restore the anatomical, biomechanical, and functional integrity of the affected structure. Here we study the effects of ascorbic acid (AA) and epidermal growth factor (EGF) applied in a single dose and in combination on the repair of a noncritical bone defect model. Methods: Twenty-four rats were divided into four groups: an intact G-1 control group, and three groups that underwent a noncritical bone defect in the right tibia: G-2 treated with AA, G-3 treated with EGF, and G-4 treated with AA in combination with EGF. After 21 days of treatment, rats were sacrificed, the tibias were dissected and a destructive biomechanical analysis of three-point flexion test was performed in a universal testing machine; the values of stiffness, resistance, maximum energy, and energy at maximum load were statistically compared. Results: G-3 and G-4 recovered the biomechanical properties of strength and stiffness of an intact tibia 3 weeks after their application. Not so the energy and energy at maximum load. For G-2, only the stiffness of an intact tibia was recovered. Conclusion: EGF and AA-EGF applied to a noncritical bone defect in the rat tibia favors the recovery of bone resistance and stiffness.

Resumo

Purpose: To create three-dimensional anatomical models of the thoracic and lumbar portions of the canine spine that reproduce the vertebral surgical approaches of dorsal laminectomy and hemilaminectomy, and to perform the respective radiographic evaluations of each approach. Methods: In a digital archive of the canine spine, digitally replicate the dorsal laminectomy and hemilaminectomy in the thoracic and lumbar portions and, then, make tridimensional prints of the vertebral models and obtain radiographs in three dorsoventral, ventrodorsal and laterolateral projections. Results: The anatomical models of the surgical spinal canal accesses of the thoracic and lumbar portions showed great fidelity to the natural bones. The created accesses have the proper shape, location and size, and their radiographic images showed similar radiodensities. Conclusions: The replicas of the dorsal laminectomy and hemilaminectomy developed in the anatomical models in the thoracic and lumbar portions are able to represent the technical recommendations of the specialized literature, as well as their respective radiographic images, which have certain radiological properties that allow to make a deep radiological study. Therefore, the models are useful for neurosurgical training.

Resumo

The use of endo-exo prosthesis is an alternative to improve the quality of life of amputees, allowing correct distribution of body weight, reducing skin problems and pain related to the implant, and reducing lameness. The aim of this paper is to report the use of a fixed bilateral endo-exo femur prostheses in a cat with amputation prior to treatment. The endoprostheses were made with titanium and the exoprostheses were made with 3D printing technology. A longitudinal craniolateral surgical approach was used and the skin incision was made over the knee-joint, then fascia lata incision, followed by biceps femoris and vastus lateralis muscles retraction, incision through the joint capsule to expose the tibiofemoral joint, disarticulation, and tibia and fibula removal. Afterwards, a femoral leveling osteotomy and insertion of the implant in the medullary canal were performed, being attached in the cis and trans cortical bone. The patient was evaluated in the immediate post-operative and periodically thereafter, showing satisfactory results, without macroscopic signs of rejection, wound dehiscence or infection, fracture, or injury of limbs, and presented adapted walking. Furthermore, it was observed the return to common behaviors for the species.


O uso de endoexoprótese é uma alternativa para melhoria na qualidade de vida de pacientes amputados, permitindo distribuição correta do peso corporal, redução de problemas de pele e dor ligadas ao implante e redução da claudicação. Este trabalho objetivou relatar a utilização de endoexoprótese bilateral fixa em fêmur em felino com amputação prévia ao atendimento. Foram modeladas endopróteses em titânio e exopróteses projetadas e impressas em 3D. O acesso cirúrgico, em ambos os membros, foi realizado por incisão craniolateral transarticular da pele, incisão da fáscia lata, rebatimento dos músculos bíceps femoral e vasto lateral, incisão da cápsula articular para exposição da articulação femorotibial, desarticulação e remoção cirúrgica da tibia e da fibula. Em seguida, realizou-se osteotomia niveladora femoral e inserção do implante no canal medular, sendo esse fixo nas corticais cis e trans. O paciente foi avaliado no pós-cirúrgico imediato e posteriormente de forma periódica, demonstrando resultados satisfatórios, sem sinais macroscópicos de rejeição, deiscência ou infecção da ferida cirúrgica, fratura ou lesão de membros, e apresentava deambulação adaptada. Além disso, foi observado o retorno de comportamentos comuns da espécie.

Background: Orthopedic implants are commonly used for different types of surgical procedures to gain optimal functionand to provide stability to both bones and tendon structures. When inserting these implants, the characteristics of the material are important for surgical success, and the ideal implant must be biocompatible and nonallergenic. However, whenmolding an implant to the bone structure, its resistance can change significantly. Implants can be temporary or permanentin the body, and metal possesses properties that make it acceptable for bone repair. In biomedical implants, 2 types aremost common, commercially pure (CP)-Ti and Ti-6A1-4V. They both provide stable fixation and low risk of loosening.Implants made with the same material and composition can perform differently if the material has been altered by processing techniques for different scenarios. Stress, strain and elastic modulus are the primary metrics used in the description ofimplant materials. They can be calculated based on mechanical tests of specimens with defined geometry, most commonlytensile, bending and torsional tests. In order to better evaluate those changes, we compared the mechanical characteristicsof titanium bone plates, before and after they were molded to the bone, to verify and quantify the loss of stiffness andresistance after molding the plate.Materials, Methods & Results: The study was prospective. Background: Coxofemoral dysplasia (CFD) is the abnormal development of the hip joint, mostly affecting large breeds, and is characterized by subluxation or complete luxation of the femoral head. Among the conservative therapeutic options, cell therapies with stem cells for CFD provides potential by the countless possibilities of therapeutic application, especially those related to the chronic and/or degenerative diseases treatment, which could be a key point for the bone and joint repair. The objective was to report a bilateral CFD case in a dog, treated with intra-articular application of mesenchymal stem cells (MSC), with 0, 30, 60 and 90 days, and further evaluations. Case: A 2-year-old male Akita dog was referred to the Veterinary Hospital Harmonia (HVH) located in the city of Recife, Pernambuco, presenting clinical signs of hip pain, lameness and pelvic limbs hopping. By orthopedic examination, pain was observed at the cranial and caudal extension of the pelvic limb, and at flexion, abduction and adduction of the limb, as well as moderate muscle atrophy and presence of joint crackling. By coxofemoral radiography performed in ventrodorsal projection, it were detected bilateral femoral head subluxation, thickening of the femoral neck and the presence of intraarticular osteophytes. These findings are compatible with the degeneration caused by the presence of CFD. Laboratory tests performed such as hemogram and biochemical had no changes. Thus, the patient had intra-articular application of allogeneic MSC, derived from adipose tissue, obtained by private company. For stem cell applications, arthrocentesis of the hip joints was performed one at a time, using a 5 mL syringe and 16G needle for aspiration of 2 mL synovial fluid, for discard and application of stem cells. O macaco-prego-preto é endêmico em áreas de mata atlântica, porém pode ser encontrado nas demais regiões, como na cidade de Maringá, Paraná, em áreas de reserva ambiental. É uma espécie quase em extinção. Acidentes envolvendo animais selvagens são comuns, principalmente em áreas de reservas próximas a ruas e avenidas. Objetiva-se relatar o caso de um Sapajus nigritus nigritus fêmea, adulta, 3,1kg, que foi resgatada e atendida após atropelamento. Após exame radiográfico no membro pélvico direito, foi constatada fratura exposta grau I, oblíqua curta, da tíbia e fíbula. Após limpeza e imobilização temporária do membro com tala, o animal foi submetido à cirurgia de osteossíntese tibial. O protocolo de medicação pré-anestésica incluiu dexmedetomidina associado à morfina. A indução anestésica foi realizada com propofol e cetamina por via intravenosa. Foi realizado ainda bloqueio epidural com lidocaína. Durante o procedimento cirúrgico foi aplicada placa bloqueada na função de apoio e cinco parafusos bloqueados. Os cuidados pós-operatórios farmacológicos envolveram administração de antibióticos, anti-inflamatórios e analgésicos opioides. Após três dias, o membro retornou à sua funcionalidade. O paciente permaneceu internado 45 dias pós-operatórios para recuperação com os cuidados veterinários. Concluiu-se que o método de fixação da fratura, bem como os protocolos sedativos e anestésicos empregados, são aplicáveis para a espécie citada.


The black capuchin monkey is endemic in areas of Atlantic Forest, but can be found in other regions, such as the city of Maringá, Paraná, in areas of environmental reserve. It is an almost endangered species. Accidents involving wild animals are common, especially in reserve areas close to streets and avenues. The objective is to report the case of a female, adult, 3.1 kg Sapajus nigritus nigritus, which was rescued and treated after being run over. After radiographic examination of the right pelvic limb, a short degree I, open oblique fracture of the tibia and fibula was found. After cleaning and temporary immobilization of the limb with a splint, the animal was submitted to tibial osteosynthesis surgery. The pre-anesthetic medication protocol included dexmedetomidine associated with morphine. Anesthetic induction was performed with propofol and ketamine intravenously. Epidural blocking was performed with lidocaine. During the surgical procedure, a blocked plate was applied to the support function and five blocked screws. Postoperative pharmacological care involved administration of antibiotics, anti-inflammatory drugs and opioid analgesics. After three days, the limb returned to functionality. The patient remained hospitalized 45 days postoperatively for recovery under veterinary care. It was concluded that the fracture fixation method, as well as the sedative and anesthetic protocols used, are applicable for the mentioned species.

Background: Acute spinal traumas can lead to irreversible damage associated with vascular and inflammatory changes in neural tissue. Since spine and spinal cord traumas have an unfavorable prognosis in small animals, and reports of the use of Steinmann pins and polymethylmethacrylate repair of lumbar vertebra fracture-luxation in puppies are rare in the literature, the present paper aimed to report the surgical treatment of transversal fracture through the body of the fourth lumbar vertebra, with dorsocranial displacement of the caudal fragment by using Steinmann pins and polymethylmethacrylate in a 7-month-old Labrador Retriever male dog. Case: A 7-month-old intact male Labrador Retriever dog, weighing 24.0 kg was attended at School Veterinary Hospital with a history of hit by car and paraplegia of the hind limbs. On neurological examination was observed no proprioception and no deep pain sensitivity on the both pelvic limbs. The lumbar spinal palpation showed intense pain, and the motor function and patellar reflexes were reduced. The values of haematological and biochemical analysis remained within the reference values for the species. Radiographs revealed a transversal fracture through the body of the fourth lumbar vertebra, with dorsocranial displacement of the caudal fragment, and was decided to perform a surgical treatment by open reduction and internal fixation of the fracture. The dog was positioned in ventral recumbency for surgery, and a dorsal midline incision was made from the second lumbar vertebra to the sixth lumbar vertebra. Two crossed 1.5 mm Kirschner wires were placed through the caudal articular facets of the fourth lumbar vertebra to provide initial stability...

As infecções ortopédicas são condições comumente observadas em equinos, afetando frequentemente estruturas nobres como ossos e tecido sinovial. Afecções de origem séptica são de extrema importância na espécie equina, pois podem resultar em claudicação permanente provocada por danos articulares irreversíveis. Desta forma, tratamentos que resultam em concentrações elevadas de antimicrobianos nas áreas infectadas atuam de maneira mais eficaz na eliminação da população bacteriana nestes tecidos. Neste sentido, a perfusão regional do membro tem se mostrado uma excelente alternativa. A presente revisão aborda os principais aspectos relacionados às técnicas de perfusão regional do membro com antimicrobianos em equinos. Serão descritas as técnicas intravenosa e intraóssea, as quais envolvem a administração de antimicrobianos, em uma região selecionada do membro, isolada previamente da circulação sistêmica por meio da aplicação de um torniquete. A perfusão regional tem sido descrita com sucesso no tratamento de condições sépticas da região distal do membro de equinos, visto que altas concentrações de antimicrobianos são alcançadas nos locais afetados. Além disso, é de fácil realização e apresenta baixo risco de efeitos adversos sistêmicos, bem como custo reduzido.


Orthopedic infections are conditions commonly observed in horses, often affecting important structures such as bone and synovial tissue. Septic source conditions are extremely important in horses as they may result in permanent lameness caused by irreversible joint lesions. Therefore, treatments that result in higher concentrations of antimicrobials in infected areas act more effectively eliminating the bacterial population in these tissues. Therefore, the regional limb perfusion has proven to be an excellent alternative. This review addresses the main issues of regional limb perfusion with antimicrobials in horses. It will describe intravenous and intraosseous techniques, which involve the administration of an antimicrobial solution, in a part of the limb, previously isolated from the systemic circulation by applying a tourniquet. Regional perfusion has been described as a successful treatment for septic conditions at the distal region of the equine limb, since high concentrations of antimicrobial are achieved at the sites affected. In addition, it is easy to perform and presents a low risk of adverse systemic effects as well as reduced cost.

A perfusão regional tem sido descrita com sucesso no tratamento de condições sépticas da região distal do membro de equinos, visto que altas concentrações de antimicrobianos são alcançadas nos locais afetados. Além disso, é de fácil realização e apresenta baixo risco de efeitos adversos sistêmicos, bem como custo reduzido.(AU)


Orthopedic infections are conditions commonly observed in horses, often affecting important structures such as bone and synovial tissue. Septic source conditions are extremely important in horses as they may result in permanent lameness caused by irreversible joint lesions. Therefore, treatments that result in higher concentrations of antimicrobials in infected areas act more effectively eliminating the bacterial population in these tissues. Therefore, the regional limb perfusion has proven to be an excellent alternative. This review addresses the main issues of regional limb perfusion with antimicrobials in horses. It will describe intravenous and intraosseous techniques, which involve the administration of an antimicrobial solution, in a part of the limb, previously isolated from the systemic circulation by applying a tourniquet. Regional perfusion has been described as a successful treatment for septic conditions at the distal region of the equine limb, since high concentrations of antimicrobial are achieved at the sites affected. In addition, it is easy to perform and presents a low risk of adverse systemic effects as well as reduced cost.(AU)

Resumo

The objectives of this study were to demonstrate the lesions that affect athlete bulls and to correlate the disorders with weight, age, affected limb and region of the limb. The present study was accomplished using radiographic images of athletic rodeo bulls collected from the medical and surgical records of the large animal service at the veterinary hospital. Radiographic images were evaluated for 136 bulls that were taken care of at the Veterinary Hospital, ranging in age from 4 to 13 years, with an average weight of 800kg and proven prior physical activity through participation in rodeos. The chi-square or Fisher's exact test was used to assess the association between the studied variables. It was observed that 71.6% of the bulls studied and suffering from lameness had radiographic lesions, predominantly in experienced animals. Enthesopathy in starter and experienced bulls, septic arthritis in starter bulls, and fractures and degenerative joint disease in experienced bulls were the most frequent radiographic lesions diagnosed. The region of limb where the majority of radiographic changes occurred was the digits. Risk factors and occurrence of diseases of the locomotor system in athletic bulls are similar to those in sport equines.(AU)


O objetivo desse trabalho foi demostrar quais lesões acometem touros atletas, correlacionando as desordens com peso, idade, membro e região afetada. O presente estudo foi realizado utilizando radiografias de touros atletas de rodeio, após levantamento dos prontuários clínicos e cirúrgicos do serviço de grandes animais de um hospital veterinário. Foram avaliadas radiografias de 136 touros que haviam sido atendidos em tal hospital, com idade variando de quatro a 13 anos, com média de peso de 800kg e realizavam participação em rodeios. Para avaliar a associação entre as variáveis estudadas foi utilizado o teste exato de Fischer e o Qui-quadrado. Observou-se que 71,6% dos touros estudados que claudicavam, apresentavam lesões radiográficas, predominantemente em animais mais experientes. A entesiopatia em todos os animais, artrite séptica nos touros iniciantes, fraturas e doença articular degenerativa nos touros mais experientes, foram as alterações radiográficas mais frequentes. As regiões com maior ocorrência de lesões radiográficas foram os dígitos. Os fatores de risco para ocorrência de doenças no sistema locomotor de touros atletas são diversos, similares aos que ocorrem nos equinos atletas.(AU)

Fractures of the distal femoral physis are frequently observed in animals with immature skeletons and may cause future orthopedic problems. With this type of fracture, early reduction and stabilization are necessary to prevent additional damage to the physis and to preserve the growth potential of the bone. This report aims to describe the clinical and radiographic findings, as well as the surgical method used that combined Rush pins with an intramedullary pin, associated with a Salter Harris type I fracture of the right distal femur in a rabbit.Case: A 4-month male domestic rabbit (Oryctolagus cuniculus) weighing 1.2 kg, was referred to the Veterinary Hospital with a right pelvic limb lameness after an episode of trauma. Orthopedic examination revealed swelling, instability, crepitation, and increased pain sensitivity in the distal aspect of the right femur. Following physical and radiographic examination, a Salter Harris type I fracture of the right distal femur was diagnosed. The patient was premedicated with a combination of xylazine (2 mg/kg IM) and ketamine (10 mg/kg IM), which allowed placement of an IV catheter for the administration of fluids and intubation using an endotracheal tube with an internal diameter of 2 mm. Anesthesia was maintained with isoflurane, and an epidural was performed with a combination of lidocaine (2 mg/kg) and bupivacaine (0.75 mg/kg). Osteosynthesis was performed with two Rush pins and an intramedullary pin. Radiographic examination 110 and 330 days after the surgical procedure showed good alignment of the bone and adequate healing of the fracture. The combination of techniques used in this case report proved to be effective, resulting in functional recovery of the limb and rapid bone healing.[...](AU)

Cranial cruciate ligament rupture (CCrLR) is a common condition found in the small animal routine, being correlated to traumas, obesity, genetic factors, and primary osteoarthritis (OA) in dogs. A Ruptura do Ligamento Cruzado Cranial (RLCCr) é uma afecção comumente vista na clínica de pequenos animais e, nos cães, pode ser decorrente de traumas, obesidade, fatores genéticos e osteoartrite (OA) primária. Todos os casos apresentam instabilidade articular e quando não tratados podem evoluir para OA secundária e perda funcional do membro. O objetivo do presente trabalho foi comparar os resultados, a curto prazo, da técnica intra-articular utilizando o botão cirúrgico associado ao fio de poliéster 5 (Grupo A), botão cirúrgico associado ao fio de náilon 5 (Grupo B), e grampo cirúrgico associado ao fio de poliéster 5 (Grupo C), para a estabilização da articulação do joelho após a RLCCr. Foram utilizados 18 cães com RLCCr, com peso variando entre 5 e 35 kg, com raça e sexo aleatórios. Os cães foram aleatoriamente divididos em três grupos de 6 indivíduos. ABSTRACT: The aim of this paper was to conduct a bibliometry search about Brazilian scientific publications that describe experimental orthopedic surgeries in dogs. It was analyzed 14 publications, and there was wide variation in the number of experimental animals, showing that there is no consensus on the appropriate number for science experiment with dogs. Most articles had been reviewed by an ethics committee (13/14, 92.85%) and only two articles (2/14; 14.28%) did not report the use of analgesic protocol. Among analgesics, morphine was the most widely used, noted in seven studies (7/14; 42.85%). In six of the articles (6/14; 42.85%), the final destination of animals was not informed, but in four experiments (4/14; 28.57%) the dogs were euthanized. In four studies (4/14; 28.57%) the animals were sent for adoption. In conclusion, the ethics in animal research in Brazil has advanced, but is necessary proper planning in sample size.


RESUMO: O objetivo do presente artigo foi realizar uma pesquisa bibliométrica sobre publicações científicas brasileiras que descrevem cirurgias ortopédicas experimentais em cães. Foram analisadas 14 publicações, havendo grande variação no número de animais experimentais, demonstrando que não há consenso sobre o número adequado para um experimento científico ortopédico com cães. A maioria tinha sido analisada por um comitê de ética (13/14; 92,85%) e apenas dois artigos (2/14; 14,28%) não informaram o uso de protocolo analgésico. Entre os analgésicos, a morfina foi a mais utilizada, tendo sido observada em sete estudos (7/14; 42,85%). Em seis publicações (6/14; 42,85%) o destino final dos animais não foi informado, mas em quatro experimentos (4/14; 28,57%) os cães foram eutanasiados. Também em quatro estudos (4/14; 28,57%) os animais foram encaminhados para adoção. Em conclusão, a ética em pesquisa animal no Brasil avançou, mas há necessidade de planejamento adequado no número amostral.
