Background: Mast cell tumors (MCTs) are neoplasms originating from mast cells, which can be well or poorly differentiated. They are considered the most commonly diagnosed malignant cutaneous neoplasm in dogs; however, intranasal forms are still little reported. Thus, this study seeks to report a case of unilateral intranasal MCT exhibiting submandibular lymph node metastasis. Case: A 11-year-old-and-4-month-old dog of undefined breed (UB), weighing 41 kg, was referred to the Veterinary Medical Teaching Hospital of the University of Passo Fundo (UPF), in the state of Rio Grande do Sul, Brazil. Presenting a clinical history of bilateral purulent nasal secretion, accompanied by sneezing in the two months prior to admission, in addition to vomiting and diarrhea. Auxiliary tests were requested, including skull X-ray, cytology of the nasal cavity with a swab, and collection of material from the submandibular lymph node directly through cytology with a needle. Cytological findings from the right nasal cavity were consistent with mast cell tumors (MCTs). Cytological analysis of the left nasal cavity was compatible with dysplasia/cellular reactivity. A heterogeneous population of cells was detected on cytology of the right submandibular lymph node. These findings were consistent with MCT lymph node metastasis. Skull radiography showed an increase in both opacity and soft tissue extension, surpassing the palate, from the canine tooth through the caudal region of the maxillary sinuses to the last molar, without bone destruction. The dog was then admitted for an abdominal ultrasound, which showed no changes in the spleen or liver. The leukocyte count showed mild lymphopenia and the presence of reactive lymphocytes. Through the buffy coat, the presence of rare round cells, compatible with circulating mast cells, was detected. Due to the biological behavior of the neoplasm and its anatomical location, the established therapy was based on the use of vinblastine and prednisolone. The patient did not show any clinical improvements. In a joint decision with the patient's guardian, the dog was euthanized. Discussion: Intranasal MCTs commonly present progressive and intermittent unilateral epitaxis, mucopurulent nasal discharge, dyspnea, and ocular discharge. Several anatomical sites were associated with more aggressive neoplastic phenotypes; those with an unfavorable prognosis were mainly those present in the oral and intranasal mucosa. Cytopathological examination is considered a highly sensitive method for the diagnosis of MCTs. Metastases are present in more than 90% of mucosal MCTs, usually affecting regional lymph nodes and associated with a poor prognosis. Radiography is considered a useful test in determining the size and location of tumors in the nasal cavity. Chemotherapy plays an important role in the treatment, especially in cases like the one described in this report, in which surgical excision is not possible due to the anatomical location of the neoplasm. Intranasal MCTs are uncommon in dogs. In this case, he presented aggressive, metastatic behavior and a poor response to antineoplastic therapy. Furthermore, due to the location of these tumors, they may be clinically similar to a number of other upper respiratory tract diseases, posing a diagnostic challenge. Therefore, it is essential that the search for differential diagnoses be carried out through auxiliary tests, such as cytology and imaging.

Background: Brachycephalic dogs have several abnormalities in the airways, which generate clinical manifestations that impair the quality of life of these animals. Primary alterations promote airflow obstruction and increase intraluminal negative pressure, causing secondary alterations due to a cycle of inflammation with consequent obstruction. With the onset of inflammation and other alterations in the airways, clinical manifestations can be observed, such as snoring, reverse sneezing, coughing, dyspnea, cyanosis, syncope, and vomiting. Endoscopic examination of the airways allows visualization and diagnosis of morphological changes in these animals. This study aimed to evaluate the endoscopic findings of the airways and determine the quantitative and qualitative assessment of the severity of clinical manifestations in 14 dogs with brachycephalic syndrome (BS). Materials, Methods & Results: Historical and anamnesis data and clinical, respiratory, and digestive manifestations were collected from the owners' reports. The clinical manifestations were classified as mild, moderate, or severe. All animals were subjected to endoscopy of the airways, and image findings were grouped according to the anatomical site where they were observed, then the abnormalities were correlated with the severity of the clinical manifestations. When comparing the frequency of endoscopic abnormalities with the severity of clinical manifestations, it is interesting to observe that animals with severe disease more frequently presented the following alterations: prolongation and thickening of the soft palate, laryngeal changes, presence of hyperemia, lymphoid hyperplasia and polyps in the nasopharynx, hypoplasia and presence of tracheal secretion and thickening of the dorsal tracheal muscle; bronchial collapse, hypoplasia of the main bronchi and bronchial hyperemia and the presence of aberrant nasal turbinates. Comparison between the means of nonparametric variables was performed using the Mann-Whitney test, with a 5% significance level. Discussion: A higher frequency of French Bulldog dogs included in the study was observed, probably due to their greater popularity, although any brachycephalic dog may be affected by BS. Considering that nostril stenosis is a congenital alteration, which is usually diagnosed early, and since the animals included in the study were dogs referred for the rhinoplasty procedure the higher frequency of young dogs in the study was not surprising. The results revealed a significant difference in the endoscopic findings with the severity of the clinical manifestations in different variables, such as prolongation and thickening of the soft palate, eversion of the laryngeal saccules, presence of laryngeal inflammation and collapse, presence of polyps or lymphoid hyperplasia in the nasopharynx, thickening of the dorsal muscle of the trachea, polyps in the trachea and hypoplasia in addition to bronchial hyperemia. The larynx was the most affected anatomical site, with alterations present in 92.8% of the evaluated dogs. Based on these findings, it can be concluded that airway obstruction caused by primary alterations can cause secondary alterations, with consequent clinical manifestations in brachycephalic dogs it is possible to suggest that dogs with this syndrome may present severe manifestations of the disease, regardless of age. Furthermore, it can be concluded that in this study, dogs with BS had frequent laryngeal alterations and that alterations in the main bronchi were present in all animals with severe manifestations of the disease.

Background: Chemodectomas, better known as tumors of the base of the heart, arise from aortic bodies, respiratory chemoreceptors located near or inside the aortic arch or originate from receptors located in the carotid arteries. Relatively rare, they affect dogs and, to a lesser extent, cats. They gain great importance when they influence the function of the cardiovascular system, with animals showing clinical signs related to congestive heart failure. Clinical diagnosis is based on symptomatology and complementary tests such as radiography, electrocardiography and echocardiography, while the definitive diagnosis is obtained by cytological and histopathological exams. This study aims to reports a case of malignant chemodectoma in a bitch, whose main symptomatology was neurological and not cardiovascular. Case: A 1-year-old Rottweiler bitch was attended with neurological alterations compatible with vestibular syndrome, hyporexia, dysphagia, apathy, melena, emesis, and purulent nasal discharge on the right nostril. On physical examination, the animal showed depressed level of consciousness, poor body condition, bilateral quemosis, paralysis of the right eye, inspiratory dyspnea and muffling of cardiac auscultation, besides a subcutaneous nodule between the scapulae. On neurological evaluation, horizontal nystagmus, head tilt to the right side, ventromedial strabismus and facial nerve paralysis on the right side were observed so that the localization of the lesion was set in peripheral vestibular system. During anesthesia for esophageal tube placement, a mass from the hard palate to the oropharynx was noted, making endotracheal intubation impossible to perform. Biopsy of this nodule was performed, and tracheostomy was indicated, but the owner opted for euthanasia before the procedure. Necropsy revealed white soft masses in the bilateral retromandibular region, on the subcutaneous tissue near the scapulae, in the right ear and since nasopharynx to the soft palate, in addition to sparse white nodules in the heart, lung, carotid artery, kidneys, right ovary, mesentery near to the spleen, and axillary lymph node. Histologically, the nodules were characterized by neoplastic cells population organized in short bundles or cords, arranged around small blood vessels surrounded by delicate connective tissue. Neoplastic cells infiltrated muscles and blood and lymphatic vessels were filled by multiple neoplastic emboli. The histological pattern of the cells allowed the diagnosis of chemodectoma. Discussion: The bitch from this case had 1-year-old when diagnosed with chemodectoma, differently from most cases from literature, that are between 7 to 15 years old. Furthermore, primarily cardiac tumors are considered rare, being chemodectoma the most common, often reported in Boxer and Boston Terrier dogs, but unusual in Rottweilers. Despites some articles mentioning seizure and Horner's Syndrome secondary to a carotid body chemodectoma, neurological signs are not commonly observed in these cases. The presence of the tumor in the middle ear region of the right side supports the occurrence of peripheral vestibular syndrome and facial nerve paralysis on the same side. Because it is a neoplasm that is usually detected late during the course of the disorder, most patients either cannot obtain diagnosis in vivo, as in this reported animal, which was in such a critical condition that underwent euthanasia, or there are no more possible therapeutic choices. In the patient described, there were numerous metastatic masses and nodules spread throughout the body. Although the typical clinical signs in animals with chemodectomas are often related to heart disease, neurological signs may also be present. This report emphasizes the importance of chemodectoma being included as a differential diagnosis in young dogs and even in breeds such as Rottweiler.

Introduction: The foramen magnum is located in the occipital bone and communicates the cranial cavity with the spinal canal of the vertebral column. Variations in the shape and size of this foramen, such as the presence of a notch in its dorsal contour, characterize occipital dysplasia and may occur due to a defect in the supraoccipital bone ossification process during the gestational period. Occipital dysplasia has been reported primarily in small, toy, and brachycephalic breeds, and its clinical relevance remains controversial. The aim of the present study was to evaluate the size of the foramen magnum in asymptomatic dogs of small and toy breeds. Materials, Methods & Results: The study was conducted at the Veterinary Hospital of the Center for Rural Health and Technology of the Federal University of Campina Grande (UFCG), Patos Campus, located in Paraíba state, Brazil. Twelve (n = 12) asymptomatic, small and toy breed dogs, with variable sex and over 10 months of age, were referred to the Veterinary Hospital for elective surgical procedures and were used in this study. All dogs underwent complete neurological examination to confirm the asymptomatic status and were radiographed in rostrocaudal position, with their mouths closed and the hard palate at an angle of approximately 70º to 80º with the table of the X-ray apparatus. The foramen magnum of each specimen was evaluated in the radiographs using a precision caliper. The analyzed parameters included height (H), the height of the dorsal notch (N), total height (H+N), and width (W), and the obtained data were used to establish the degree of occipital dysplasia and determine the shape of the foramen magnum. Of the 12 animals studied, 75% (n = 9) exhibited a dorsal notch in the foramen magnum, which varied between 3.00 and 10.00 millimeters (mm) in height, characterizing occipital dysplasia. Among the affected animals, 77.77% (n = 7) were classified as grade 1 for the alteration, 11.11% (n = 1) as grade 2 and one animal (11.11%) as grade 3. The prevalent shape observed for the foramen magnum was oval (83.3%). Discussion: Although occipital dysplasia has been associated with the occurrence of nonspecific neurological clinical signs, such as tremors, ataxia, and epileptic seizures, the presence of this alteration in asymptomatic dogs indicates that the formation alone is just an anatomical variation, as demonstrated herein and in previous studies conducted over the past few years. This hypothesis has been increasingly supported by scientific evidence through publications that portray occipital dysplasia in dogs of various breeds and sizes without clinical manifestations. The clinical signs attributed to occipital dysplasia may originate in situations where there is a coexistence of other conditions. Occipital dysplasia has been reported several times in conjunction with other pathologies, such as occipital hypoplasia and syringomyelia, in symptomatic dogs. The dorsal notch-shaped occipital defect is covered by a fibrous tissue membrane in dogs affected by occipital dysplasia. The presence of this soft tissue membrane has been related to the late onset of syringomyelia due to the decompressive effect that it provides to the flow of cerebrospinal fluid. When occipital dysplasia is identified in symptomatic dogs, it is suggested that the lesion be correctly located within the nervous system and that, according to its neurolocalization, a thorough investigation of other underlying causes for the occurrence of the neurological clinical manifestation be carried out. To date, there is no evidence characterizing occipital dysplasia as a single entity causing neurological deficits.

Background: Nasopharyngeal polyps are benign and inflammatory masses assumed to arise from the middle ear or the eustachian tube with extension into the pharynx. The most common clinical signs associated with nasopharyngeal polyps include respiratory stertor, dyspnea, and otic discharge. Neurological signs, including head tilt, facial nerve paralysis, and ataxia, might indicate concurrent involvement of the middle or inner ear. The objective of the current report is to describe a case of a feline nasopharyngeal polyp with a concurrent hiatal hernia and megaesophagus, both spontaneously resolved after removal of the polyp. Case: A 6-month-old female intact domestic shorthair cat was presented for evaluation of lethargy, anorexia, and upper respiratory signs, such as stridor, stertor, and dyspnea. A thoracic radiography revealed esophageal dilation caudal to the cardiac silhouette, suggestive of megaesophagus with gaseous filtration. An esophagram confirmed a hiatal hernia and megaesophagus. Computed tomography revealed a nasopharyngeal mass adjacent to the soft palate and a soft-tissue density in the right tympanic bulla. A tentative diagnosis of a nasopharyngeal polyp was made. After the ventral bulla osteotomy, the nasopharyngeal mass was removed by a gentle traction avulsion technique. Six days after the surgery, hiatal hernia and megaesophagus were spontaneously resolved. Based on histopathologic exam, the mass was found to be an inflammatory nasopharyngeal polyp. Two months after surgery, the owner reported that the patient's condition had returned to baseline with a good appetite, and the thoracic radiography was within normal limit. Discussion: For successful treatment of a nasopharyngeal polyp, traction avulsion of the polyp with or without a ventral bulla osteotomy is recommended. However, in patients with otitis media, a ventral bulla osteotomy followed by traction avulsion of the polyp is recommended in order to reduce the rate of polyp recurrence. Common clinical signs of a nasopharyngeal polyp are stertor, stridor, dyspnea, dysphagia, and open-mouth breathing, which are identified in a chronic upper airway obstruction. A hiatal hernia secondary to a nasopharyngeal polyp has not been reported so far. However, a relationship between chronic upper airway obstruction and hiatal hernias has been proposed previously. Moreover, hiatal hernia resolved spontaneously after removal of the nasopharyngeal polyp suggests that the occurrence of the hiatal hernia was secondary to the nasopharyngeal polyp. In addition to the hiatal hernia, megaesophagus was also identified in the present case. Megaesophagus secondary to a chronic upper airway obstruction from a nasopharyngeal obstruction has been reported. However, megaesophagus is also thought to occur secondary to hiatal hernias. Therefore, in the current study, it is unclear whether the megaesophagus was solely a result of the obstructive nature of the nasopharyngeal polyp or a combination of the hiatal hernia and the nasopharyngeal polyp. In conclusion, any cat with clinical signs of an upper airway obstruction and a concurrent hiatal hernia and megaesophagus should be thoroughly investigated for a nasopharyngeal polyp, as well as other gastrointestinal and systemic causes. Furthermore, this case suggests that the prognosis for a concurrent hiatal hernia and megaesophagus is good in cats if the nasopharyngeal polyp is properly removed.

Palatal malformations rarely occur in small animals. Palatal clefts can affect the upper lip, hard palate, and soft palate, and may be congenital or acquired in origin. The most common acquired forms result from the excision of oral neoplasia, teeth extraction, perforating lesions, automobile accidents, and electric accidents. Clinical signs of a fissured palate are sneezing, purulent nasal discharge, salivary secretion in nasal discharge, difficulty feeding and drinking, and pneumonia due to aspiration, which are the most severe alterations and can lead to death. The treatment of palatal clefts consists of occlusion of the oronasal communication, which can be performed using mucoperiosteal grafts, pedicled flaps, or palatal obturators. Herein, we report a case of an acquired palatal cleft treated with a palatal obturator in a feline patient who bit a high-tension wire. Two attempts of surgical correction had been unsuccessful, resulting in suture dehiscence and perpetuation of the oronasal fistula. After fixation of the obturator, the animal showed remission of all clinical signs. The animal has survived for over one year with improved quality of life.(AU)

Twenty-six brachycephalic dogs with nostril stenosis, 22 males and four females, underwent bilateral rhinoplasty. Sixteen dogs were French Bulldogs; two, English Bulldogs; Six, Pugs; and two, Shih tzus, ranging in age from six months to six years. Blood samples were collected for hemogasometric analysis in the preoperative period and 30 days after surgery. For each evaluation, a 0.5ml sample of blood collected from the femoral artery was obtained in a heparinized plastic syringe. Hemogasometric evaluation was then performed on a blood gas analyzer (I-stat-Abbot®). The results of hemogasometry before and after rhinoplasty showed a reduction in the mean values of pCO2, TCO2, HCO3- and BEecf, hematocrit and hemoglobin, and an increase in pH, pO2 and SO2, indicating an improvement in the ventilatory condition of the animals after surgical correction of Nostril stenosis. Therefore, it is concluded that arterial hemogasometry is an important diagnostic tool for the diagnosis of brachycephalic respiratory syndrome and is extremely useful in monitoring the patient's response to treatment. Rhinoplasty was effective in the treatment of respiratory syndrome, promoting improvement in hemogasometric parameters that indicate respiratory acidosis secondary to airway obstruction, common in the brachycephalic races.(AU)

Neuropathies of pharyngeal branches of glossopharyngeal and vagus are often associated with guttural pouches diseases; however, these branches of injury due to stylopharyngeus muscle compression are not reported. A case was reported of a quarter horse mare, 8 years old, 450 kg, presenting dyspnea and respiratory noise associated with weight loss. Clinical examination observed mixed dyspnea, tachycardia, dysphagia, sialorrhea, lung crackles and submandibular and parotid lymphadenopathy. Endoscopic exam showed right arytenoid chondritis, nasopharyngeal collapse, generalized larynx edema and dorsal displacement of the soft palate. Right guttural pouch evaluation showed swelling in the origin of stylopharyngeus muscle with consequent compression of the XII, X and IX cranial nerves. Tracheotomy, systemic treatment with corticosteroids, beta lactams and aminoglycosides antibiotics were performed. No resolution was observed and, after 16 days, the animal showed clinical worsening, developed pleuropneumonia, uveitis, severe sepsis, acute renal failure and was euthanized. The mixed neuropathy resulted in rapid clinical deterioration of the animal, due to the difficulty in swallowing and consequent associated respiratory processes. This report emphasizes the importance of evaluating stylopharyngeus muscle origin in cases of nasopharyngeal collapse associated with dysphagia in horses, given the possibility that structural changes in this muscle can result in laryngeal neuropathy.(AU)

To determine the prevalence of diseases of the upper respiratory tract and associated factors in the Colombian Creole Horse Valley Aburrá in 2015, a random sampling of 15 farms in southern Aburrá Valley under different management conditions was performed. A total of 105 CCC underwent general clinical examination, respiratory endoscopic evaluation and a detailed inspection of housing conditions and management. A low prevalence of upper respiratory tract diseases was observed, including Pharyngeal Lymphoid Hyperplasia 38.1%, Recurrent Laryngeal Neurophaty 12.38%, Dorsal Displacement of the Soft Palate 7.62%, and presence of tracheal secretions not associated with a specific pathology. A bivariate analysis showed association between tracheal secretions and frequency of training per week (OR = 9.86), duration of the training sessions (OR = 5.55) and inadequate ventilation of the stable (OR = 10.52). Association between HLR and inadequately ventilated barns was also observed (OR = 14.9). Logistic regression showed association between tracheal secretions and inadequate ventilation (OR = 7.18). We conclude that inadequate ventilation was the most important factor for the incidence of upper respiratory tract diseases in horses at the southern area of the Aburrá Valley.(AU)

A regressão logística mostrou associação entre secreção traqueal e ventilação inadequada (OR = 7.18).
