Resumo

Conformações perineais inadequadas estão frequentemente associadas à pneumovagina, predispondo animais a processos inflamatórios vaginais e uterinos. Dentre as patologias reprodutivas que acometem éguas, a endometrite é a que ocorre com maior frequência, com forte impacto negativo sobre os índices reprodutivos de um criatório. Para essa afecção, a ozonioterapia se apresenta como tratamento em potencial. Assim, este estudo objetiva relatar um caso de endometrite em égua Quarto de Milha, multípara, de 17 anos de idade, atendida no Hospital Veterinário Sylvio Barbosa Cardoso, da Universidade Estadual de Ceará. Durante a avaliação ginecológica, observou-se alterações na conformação vulvar e teste de Windsucker positivo, indicando pneumovagina. Foram coletadas amostras endometriais para exames laboratoriais, os quais apresentaram citologia com contagem de neutrófilos polimorfonucleares ≥30% e cultura negativa para fungos e bactérias. O exame histopatológico revelou fibrose intersticial endometrial crônica acentuada. Diante do diagnóstico de endometrite, iniciou-se o tratamento com lavagem uterina usando solução ringer lactato ozonizada, seguido por insuflação uterina com ozônio. O animal foi submetido à técnica cirúrgica de Caslick para correção da má conformação vulvar. Após o tratamento, a égua foi inseminada, resultando na coleta de dois embriões viáveis. Diante do exposto, o caso reforça a efetividade da ozonioterapia associada à Caslick para o tratamento de endometrite em éguas.


Dysfunctional perineal conformations are often associated with pneumovagina, predisposing animals to vaginal and uterine inflammatory processes. Among the reproductive pathologies that affect mares, endometritis is the most frequent, with a strong negative impact on the reproductive indices of a farm. For this condition, ozone therapy presents itself as a potential treatment. The study aimed to report a case of endometritis in a 17-year-old multiparous quarter-mile mare, which was attended at the Veterinary Hospital Sylvio Barbosa Cardoso located in the State University of Ceará. During the gynecological evaluation, changes in vulvar conformation and a positive Windsucker test were observed, indicating pneumovagina. Endometrial samples were collected for laboratory exams, resulting in cytology with polymorphonuclear neutrophil count ≥30% and negative culture for fungi and bacteria. The histopathological examination revealed severe chronic endometrial interstitial fibrosis. Regarding the diagnosis of endometritis, the treatment with uterine lavage using ozonated ringer lactate solution was initiated followed by uterine insufflation with ozone. The animal was submitted to Caslick's surgical technique to correct the vulvar malformation. After treatment, the mare was inseminated, resulting in the collection of two viable embryos. In view of the above, the case reinforces the effectiveness of ozone therapy associated with Caslick`s technique for endometritis treatment in mares.

Resumo

Endometritis refers to an inflammation of the uterine mucosa that does not extend beyond the stratum spongiosum. Recent studies indicate it is a common finding in 30-50% of bitches suffering from infertility/subfertility. This disorder is subclinical, and its diagnosis involves sampling the uterus, whether it is by performing a cytology from flushing the organ or histology on collected uterine biopsies. Its pathophysiology remains unclear in 2021. Some cases are associated with cystic endometrial hyperplasia, which leads to disruption in the uterine clearance mechanisms after breeding ; while other appear as a pure inflammatory process. While we don’t have all the answers yet, there is no doubt today that these disorders must be included in the differential diagnosis of infertility in the bitch. The work that has already been done on this topic already offers some idea on how to approach these cases in our veterinary clinics.

The aim was to evaluate the use of ultrasonography color Doppler mode for diagnosis the subclinical endometritis (SE) in dairy cows. Lactating Holstein cows on a semi intensive system were evaluated (n= 152) and distributed into two groups (C1 and C2). The C1 (n = 60) was composed of primiparous and the C2 (n = 92) by pluriparous. All females (C1 and C2) were submitted gynaecological examination by transrectal palpation and evaluation with the Metricheck device (MD) and endometrial cytology (EC) to determine the percentage of neutrophils (PMNs). Ultrasonographic examinations were performed in the B mode for the presence of echogenic intrauterine fluid (IUF-e) and for color Doppler mode endometrial vascular perfusion evaluation (ENDOVP). Data were submitted to ANOVA with Pearson correlation and chi-square test at 5%. In the evaluation with MD, 16.7% of the cows in the C1 and 17.4% in the C2 were considered inapt. Regarding SE prevalence was 24.0% (C1) and 44.7% (C2). As far as the ultrasound evaluation of the IUF-e was present in 68.0% of the cows of the C1 and in 75.0% in the C2. In the ENDOVP, scores 1 (38.0%), 2 (36.5%), 3 (23.0%) and 4 (8.7%) were evidenced. There was a correlation between ENDOVP and PMN in C1 (r = 0.8336) and in C2 (r = 0.7196). Therefore, it is concluded that ENDOVP proved to be efficient for the diagnosis of SE, in view of its strong correlation with PMN.(AU)

Vacas da raça Holandesa foram avaliadas (n = 152) em lactação e mantidas em sistema semi-intensivo. Elas foram distribuídas em dois grupos (C1 e C2). O C1 (n = 60) foi composto por primíparas e o C2 (n = 92) por pluríparas. Todas as vacas (C1 e C2) foram submetidas a exame ginecológico por palpação transretal e avaliação com o dispositivo Metricheck (DM). Depois, amostras foram coletadas para citologia endometrial (CE) para determinar a porcentagem de neutrófilos (PMNs). Em relação a prevalência de ES, foi de 24,0% (C1) e 44,7% (C2). Quanto à avaliação ultrassonográfica do IUF-e, esteve presente em 68,0% das vacas do C1 e em 75,0% no C2. Gynecological examination is essential to assess the reproductive tract of mares and can provide important information about the uterine environment. It includes physical, vaginal, and rectal examination, ultrasound, cytology, culture, and endometrial biopsy. The present study aimed to perform gynecological examination and fertility to assess the fertility prognosis of Pantaneiro mares that have not been reproductively active and to determine their reproductive ability. Eight mares underwent ultrasound and gynecological examination and artificial insemination. Ultrasound revealed changes only in one mare. Histopathological findings were mild, such as periglandular and perivascular inflammatory cell infiltrates, fibrotic areas, glandular dilation, glandular island formation, and edema due to the phase of the estrous cycle. One animal was classified in category I and the others in category IIA. Cytological changes were found in only one mare. Endometrial culture from five mares resulted in isolation of Pseudomonas sp., Bacillus sp., Escherichia coli and Candida sp. Only four mares resulted pregnant through artificial insemination, using the same stallion with fresh semen, which has been proving fertility. Thus, mares with better uterine conditions will not always become pregnant and those with mild changes will not always be barren.

The study was carried out to find the relation between subclinical endometritis (SCE) and postpartum (pp.) ovarian resumption as well as to evaluate serum and endometrial TNF-α, IL-8 and serum CRP in buffaloes with and without SCE. Thirty-nine pluriparous buffaloes at the 3rd (W3), 5th (W5) and 7th (W7) week pp. were involved in this experiment. The parity of the buffaloes ranged from 4 to 8 with an average 5.8±0.2. Subclinical endometritis was diagnosed by the percentage of polymorphonuclear cells (PMNs) in uterine cytology obtained from endometrial cytobrush at W5 and W7. The cut-off point of PMNs% in buffaloes for SCE was ≥ 6% at W5 or ≥ 4% at W7. According to PMNs%, buffaloes were divided into SCE group (n=27) and non-SCE group (n=12). Ovarian cyclicity was monitored by rectal palpation, ultrasonography and progesterone assay at W3, W5 and W7. Serum and endometrial TNF-α, IL-8 and serum CRP were estimated at W5 and W7. Buffaloes with SCE (55.6%) showed delayed ovarian activity as compared to non-SCE (16.7%) animals (P=0.036). Significant increase in serum cytokines and CRP levels were detected at W5 (P ˂0.05) and W7 (P <0.01) in SCE buffaloes as compared to non-SCE. Endometrial levels of cytokines were significantly (P ˂0.05) elevated in SCE buffaloes. Serum and endometrial cytokines showed significant positive correlation. Furthermore, levels of TNF-α, IL-8 and CRP exhibited significant positive correlation with PMNs%. In conclusion, SCE delayed postpartum ovarian cyclicity in buffaloes. Moreover, TNF-α, IL-8 and CRP assessments could be efficient tools in prediction of SCE in buffaloes.

A endometrite é definida como uma inflamação do endométrio, podendo ser aguda ou crônica, infecciosa ou não infecciosa. Essa afecção é de grande importância econômica na medicina veterinária, podendo resultar em morte embrionária, aborto e até tornar a égua infértil, sendo uma das afecções que mais comprometem a sanidade uterina, colocando em risco os índices de prenhez obtidos pós-cobertura, pós-inseminação, e pós-inovulação de embriões. Existem diversos métodos para diagnóstico e o mais utilizado a campo é a citologia endometrial/uterina. Também existem algumas formas de coleta de amostra para análise laboratorial pela citologia, tais como: escova ginecológica, swab uterino guiado por mandril metálico ou plástico descartável e lavado uterino de baixo volume, seguido de sedimentação ou centrifugação podendo ser complementada por citocentrifugação. O diagnóstico pela citologia endometrial por meio da técnica da escova ginecológica é a mais utilizada entre os médicos veterinários, pois trata-se de um método prático para execução e rápido quanto à obtenção dos resultados sobre a presença ou não da endometrite, facilitando a conduta terapêutica quanto à utilização das éguas para reprodução ou descarte. Objetivou-se com esse estudo citaras formas de diagnóstico da endometrite em éguas, enfatizando a citologia endometrial e técnicas para obtenção de amostra, identificando a técnica mais indicada por diversos autores.(AU)


Endometritis is defined as an inflammation of the endometrium, which may be acute or chronic, infectious or noninfectious. This condition is of great economic importance in veterinary medicine, and may result in embryonic death, abortion and even render the mare infertile, being one of the conditions that most compromise uterine health, putting at risk the pregnancy rates obtained post-coverage, postinsemination, and post-embryo transfer. There are several methods for diagnosis and the most used in the field is endometrial/uterine cytology. There are also some forms of sample collection for laboratory analysis by cytology, such as: gynecological brush, uterine swab guided by metallic mandrel or disposable plastic and low volume uterine lavage, followed by sedimentation or centrifugation and may be complemented by cytocentrifugation. Diagnosis by endometrial cytology using the gynecological brush technique is the most used among Veterinary Practitioners, because it is a practical method to execute and fast to obtain the results in the presence of endometritis, facilitating the therapeutic conduct regarding the use of mares for breeding or discarding. The objective of this study was to cite the diagnostic methods of endometritis in mares, emphasizing endometrial cytology and its techniques for obtaining a sample, identifying the most indicated technique by several authors.(AU)

Uterine diseases in cattle occur at all stages of the reproduction cycle but the majority of cases is found in the postpartum period. The inflammation of the uterus is generally defined as metritis or endometritis, with several graduations, e.g. puerperal metritis, clinical metritis, clinical or subclinical endometritis. Whether uterine diseases have a negligible, moderate or detrimental effect on fertility is still under discussion and depends on definitions and classification. In the past, it was assumed that the pregnant uterus is free of pathogens, but recent studies found several species including pathogens in the uterus and endometrium of pregnant cows. After parturition, a broad diversity of bacteria with >200 different species has been found in the early postpartum period. Not all of these bacteria, however, are considered as pathogens. Furthermore, bacteriological findings provide only evidence for infection but not for inflammation. For some bacteria, particularly Escherichia coli and Trueperella pyogenes pathogenic mechanism resulting in metritis and endometritis have been elucidated in detail. The role of bacteria that can be regarded as opportunistic or potential pathogens, e.g. Bacillus pumilus, is still under investigation. The understanding of the uterine microbiota and its interactions is increasing with the use of modern high-resolution techniques such as Fouriertransform infrared spectroscopy. Endometrial cytology provides additional information about alterations in the endometrium. Knowledge of innate uterine defense mechanism in cattle has increased a lot in the recent past. It can be speculated that improving or modulating uterine defense mechanism will be part of future prevention and treatment approaches beyond the use of antimicrobials. In this context, cellular and molecular defense mechanisms have been in the focus of interest, e.g. the role of interleukins or mucins. This review gives a short overview on some aspects of recent research on uterine diseases in cattle.

Uterine diseases in cattle occur at all stages of the reproduction cycle but the majority of cases is found in the postpartum period. The inflammation of the uterus is generally defined as metritis or endometritis, with several graduations, e.g. puerperal metritis, clinical metritis, clinical or subclinical endometritis. Whether uterine diseases have a negligible, moderate or detrimental effect on fertility is still under discussion and depends on definitions and classification. In the past, it was assumed that the pregnant uterus is free of pathogens, but recent studies found several species including pathogens in the uterus and endometrium of pregnant cows. After parturition, a broad diversity of bacteria with >200 different species has been found in the early postpartum period. Not all of these bacteria, however, are considered as pathogens. Furthermore, bacteriological findings provide only evidence for infection but not for inflammation. For some bacteria, particularly Escherichia coli and Trueperella pyogenes pathogenic mechanism resulting in metritis and endometritis have been elucidated in detail. The role of bacteria that can be regarded as opportunistic or potential pathogens, e.g. Bacillus pumilus, is still under investigation. ABSTRACT: This study aimed to evaluate the effects of type of calving (eutocic vs. dystocic), season of the year (rainy vs. dry) on retained placenta (RP) and subclinical endometritis (SE) prevalence, to verify the relation between these diseases, as well as its effects on culling rate, days open and number of AI/conception. Retention of fetal membranes was recorded on first day postpartum. Endometrial cytology was performed between 30 and 80 days in milk (DIM) and positive cases were considered 5% neutrophils. Data were analysed by logistic regression and analysis of variance on Minitab program (P 0.05). The prevalence of RP was 14.93% (69/462) and of SE was 27.49% (127/462). A tendency of effect of RP on SE prevalence was detected (P=0.10). Dystocia increased RP prevalence (P 0.05). Cows that calved during rainy months had greater SE prevalence (P 0.05). RP increased culling rate (P 0.05), calving to conception interval (P 0.05) and number of AI/conception (P 0.05), although SE occurrence did not affect these variables (P>0.05). In conclusion, RP tended to be a risk factor for SE, dystocia is a predisposing factor for RP and calvings that happens during rainy period increases SE. There is a negative impact on reproductive efficiency of crossbred dairy cows that had retained placenta.

Os dados foram analisados por regressão logística e análise de variância no programa Minitab (P 0,05). A prevalência de RP foi de 14,93% (69/462) e de ES de 27,49% (127/462). Foi detectada tendência de efeito (P=0,10) da RP sobre a prevalência da ES. Partos distócicos aumentaram a prevalência de RP e vacas que pariram nos meses chuvosos tiveram maior prevalência de ES (P 0,05). A RP elevou a taxa de descarte (P 0,05), a duração do período de serviço (P 0,05) e o número de IA por concepção (P 0,05), enquanto a ES não afetou essas variáveis (P>0,05). Conclui-se que a retenção de placenta tende a ser um fator de risco para endometrite subclínica, distocia predispõe à retenção de placenta e partos ocorridos no período chuvoso aumentam os casos de endometrite subclínica. Há um impacto negativo na eficiência reprodutiva de vacas leiteiras mestiças acometidas por retenção de placenta.

O Cloprostenol reduziu (P < 0,01) o número de animais não gestantes (10%, 5/50) em relação ao controle (26%, 13/50). Não houve diferença (P < 0,09) entre os tratamentos na incidência de endometrites. A TP não diferiu entre os animais diagnosticados com endometrite subclínica. Conclui-se que a utilização de esteroides ou análogos da prostaglandina F2α neste período não é eficiente na prevenção de endometrites e na melhoria da TP e IPC. O Cloprostenol foi o único a reduzir o número de animais não gestantes em comparação ao controle.(AU)

Background: Clear cell carcinoma (CCC) of the cervix is a rarely seen malignant tumor which is classified in adenocarcinomas of the endometrium. It is responsible for 2.15-3.0% of endometrial tumors. The diagnosis is difficult and is still controversial in uterine carcinomas. Macroscopically, masses were protruding to endometrial surface. However, the definitive diagnosis was carried by microscopical evaluation. The cells contain clear cytoplasm. The cells are seen in this pattern because there is either glycogenation or secretoric activity in the cells.Case: In the present case, clinicopathologic and immunohistochemical findings of clear cell carcinoma of the cervix were defined in a 3 year-old spayed Golden retriever bitch. Previously spayed bitch, which had vaginal discharge for the last month-long, was submitted to the Department of Obstetrics and Gynecology, Ankara University, Faculty of Veterinary Medicine. In the general clinical examination, vaginoscopy was performed and vaginal cytology was obtained from smear. The source of discharge was detected as cervix uteri. In vaginal cytologic examination, erythrocyte, neutrophile and superficial cells were observed. In abdominal ultrasonography, a hypoechoic mass was detected at cranial part of the urinary bladder. To evaluate the general health condition of the dog, total blood counting and serum biochemistry were analyzed in addition to assessing its hormone prophlye. Estrogen and progesterone levels were evaluated. Estradiol (E2) level was measured as 23 pg/mL and progesterone level was measured as 1.96 ng/mL from collected serum. The cervical mass in diameters of 3x4x2.5 cm was removed in operation. In macroscopical examination, it had spherical and regularly shape. After the macroscopical examination, tissue samples were fixed in 10% buffered formalin. Then, the samples stained with Haematoxylin-Eosin (H&E), Masson’s trichrome and PAS stainings, respectively.[...]

Objetivou-se no presente estudo estimar o percentual de endometrites clínicas (EC) e subclínicas (ES) em tratos reprodutivos post-mortem de fêmeas bovinas, verificando sua influência sobre as estruturas ovarianas, alterações presentes no ovário, número e qualidade de oócitos recuperados. Foram coletados e avaliados 171 tratos reprodutivos em matadouro frigorífico. As endometrites foram diagnosticadas por meio de citologia endometrial e pela presença e características das secreções uterinas. Os diagnósticos foram confirmados pela análise histopatológica, sendo avaliada a existência de infiltrados de células inflamatórias no endométrio. Os ovários foram avaliados macroscopicamente quanto à estrutura e alterações ovarianas. A qualidade oocitária foi avaliada de acordo com o número de camadas de células do cumulus e aspecto do citoplasma, sendo assim, classificados como: grau I (GI), grau II (GII), grau III (GIII) e grau IV (GIV). Dentre as peças avaliadas, 8,20% (n = 14) apresentaram EC e 4,10% (n = 7) foram diagnosticadas com ES. Observou-se que a presença de endometrites não exerceu efeito sobre as estruturas e alterações ovarianas, assim como não alterou o número de oócitos recuperados. Em relação ao grau de qualidade oocitária, foi encontrado variação entre os diferentes graus de qualidade dentro dos grupos, no entanto, não houve correlação aparente com a presença de infecção uterina.(AU)


The objective of this study was to estimate the percentage of clinical (CE) and subclinical (SE) endometritis in post mortem bovine reproductive tracts, analyzing their influence on ovarian structures, changes in the ovary, and number and quality of oocytes collected. Endometritis were diagnosed by endometrial cytology, and by the presence and characteristics of uterine secretions. The diagnosis was confirmed through histopathological analysis, by evaluating the presence of infiltrated inflammatory cells in the endometrium. The ovaries were macroscopically evaluated for presence of physiological structures and alterations. The oocyte quality evaluation was performed according to the number of cumulus cells layers and cytoplasmic aspect, being classified as: grade I (GI), grade II (GII), grade III (GIII) and grade IV (GIV). Among the uterus evaluated, 8,20% (n = 14) of the animals presented EC, and 4,10% (n = 7) were diagnosed with ES. It was observed that the presence of endometritis had no effect on ovarian structures and alterations, nor did it influence the number of oocytes collected. Regarding the degree of oocyte quality, a variation was found between the different grades of quality within the groups, however, there was no apparent correlation with the presence of uterine infection.(AU)
