Resumo

This study aimed to elucidate the sedative effect and physiological changes associated with the intramuscular injection of detomidine combined with morphine in horses. Six healthy crossbred horses, aged 2 to 10 years, were included. A crossover experimental design was used to compare the effects of intramuscular injection of 30 µg/kg of detomidine alone (IMD) and intramuscular administration of 30 µg/kg of detomidine and 0.1 mg/kg of morphine (IMDM). The degree of sedation, height of head above ground, were assessed at the time points before and 5, 10, 20, 30, 40, 50, 60, 75, 90, 105, and 120 minutes after drug administration, and heart rate, respiratory rate, systolic blood pressure, rectum temperature and intestinal motility were assessed at the time points before and 10, 20, 30, 40, 50, 60, 75, 90, 105, and 120 minutes after drug administration. The physiological parameters were analyzed using the Kruskal-Wallis test with Dunn's post-hoc test and analysis of variance with t-test for independent samples and the sedation scores using the Friedman test and Mann Whitney U-test. P-values <0.05 indicated a statistically significant difference. IMDM promoted a higher sedative effect as compared to IMD, but the sedation occurred inconsistently. Additionally, a reduction in intestinal motility was observed with IMDM at 60, 75, 90, and 105 minutes after administration. IMDM promoted more variable sedation and prolonged reduction in the intestinal motility in the horses as compared to IMD.


Este trabalho teve por objetivo elucidar o efeito sedativo e alterações fisiológicas em cavalos associados à injeção intramuscular de detomidina combinada com morfina. Seis cavalos saudáveis e sem raça definida, com idade de dois a 10 anos, foram incluídos. Delineamento experimental cruzado foi usado para comparar os efeitos da administração intramuscular de 30 µg/kg detomidina isolada (IMD) e da administração intramuscular de 30 µg/kg de detomidina associada à morfina a 0,1 mg/kg (IMDM). Avaliou-se o grau de sedação, a altura da cabeça acima do solo nos momentos antes (M0) e 5, 10, 20, 30, 40, 50, 60, 75, 90, 105 e 120 minutos após administração dos fármacos, e frequência cardíaca, frequência respiratória, pressão arterial sistólica, temperatura retal e motilidade intestinal foram avaliados nos momentos antes (M0) e 10, 20, 30, 40, 50, 60, 75, 90, 105 e 120 minutos após administração dos fármacos. Para análise estatística foram utilizados: teste de Kruskal-Wallis, com pós-teste de Dunn, e ANOVA associada ao teste t para amostras independentes para os parâmetros fisiológicos; e teste de Friedman e teste de Mann Whitney para os escores de sedação. Valores de P < 0.05 indicaram diferença significativa. A administração de detomidina associada à morfina pela via intramuscular (IM) promoveu maior efeito sedativo, quando comparada à administração isolada de detomidina IM. Contudo, a sedação ocorreu de forma inconsistente. Também foi observado, no IMDM, redução da motilidade intestinal aos 60, 75, 90 e 105 minutos de avaliação. A associação de morfina e detomidina pela via intramuscular promove sedação de forma variável em cavalos, e resulta em redução da motilidade intestinal, mais prolongada quando comparada a administração isolada de detomidina intramuscular.

This study aimed to carry out a retrospective study of vagal indigestion cases diagnosed in cattle admitted at the Garanhuns Bovine Clinic, Federal Rural University of Pernambuco campus. This syndrome, caused by dysfunctions of the vagus nerve and characterized by motility disorders of the pre-stomachs and abomasum, represented 5.5% (70/1279) of digestive cases diagnosed in a period of 10 years and had an unfavorable prognosis in 78.3% of cases. Type II vagal indigestion was the most prevalent, accounting for 40% of cases, followed by type I (24.3%) and types III and IV, which accounted for 18.6% and 10.0% of cases, respectively. Vagal indigestion in 67.1% (47/70) of cases occurred as a result of other illnesses, such as traumatic reticuloperitonitis (27.7%), lung diseases (12.8%), gastric impaction (10.6%), abomasal ulcer (10.6%), lymphosarcoma (6.4%), and liver abscesses (6.4%). Motor changes in the gastrointestinal tract, such as hypomotility, abdominal distension, and bloat, as well as their consequences, were the most frequent clinical signs. Laboratory, ultrasonographic, and anatomopathological alterations mainly originate from the primary illnesses present in each case. The approach of this disease is essential due to its clinical and economic importance for livestock to expand the knowledge of its etiopathogenesis, thus contributing to a more accurate diagnosis by veterinarians working in the field of internal medicine for cattle.


Objetivou-se realizar um estudo retrospectivo dos casos de indigestão vagal diagnosticados em bovinos atendidos na Clínica de Bovinos de Garanhuns, campus da Universidade Federal Rural de Pernambuco. Essa síndrome, causada por disfunções do nervo vago e caracterizada por transtornos de motilidade dos pré-estômagos e abomaso, representou 5,5% (70/1279) dos casos digestivos diagnosticados num período de 10 anos e apresentou prognóstico desfavorável em 78,3% dos casos. A indigestão vagal tipo II foi a mais prevalente, representando 40% dos casos, seguida do tipo I (24,3%) e dos tipos III e IV, que corresponderam a 18,6% e 10,0%, respectivamente. Em 67,1% (47/70) dos casos, a indigestão vagal ocorreu em consequência de outras enfermidades, tais como reticuloperitonite traumática (27,7%), doenças pulmonares (12,8%), compactação gástrica (10,6%), úlcera de abomaso (10,6%), linfossarcoma (6,4%) e abscessos hepáticos (6,4%). As alterações motoras do trato gastrointestinal, tais como hipomotilidade, distensão abdominal e timpania, assim como suas consequências foram os sinais clínicos mais frequentes. As alterações laboratoriais, de imagem e anatomopatológicas são oriundas principalmente das enfermidades primárias presentes em cada caso. Devido sua importância clínica e econômica para a bovinocultura, é primordial a abordagem dessa enfermidade, visando ampliar o conhecimento da sua etiopatogenia contribuindo dessa forma para um diagnóstico mais preciso pelos médicos veterinários atuantes na área de medicina interna de bovinos.

Resumo

A ocorrência de processos fisiopatológicos que cursam com desidratação da ingesta no trato gastrointestinal dos equinos é comum na rotina clínica. Fatores como diminuição da motilidade intestinal e sobrecarga intraluminal de conteúdo desidratado podem levar a compactação em segmentos como estômago, ceco e cólons. Este estudo objetivou realizar a comparação entre soluções eletrolíticas enterais hipotônica (SeHIPO) e isotônica (SeISO) e a solução Ringer com lactato de sódio (RL IV) sobre o teor de umidade das fezes de equinos submetidos a um período de desidratação experimental (PD). Foram utilizados seis equinos adultos, todas fêmeas com idades entre 10 e 15 anos, média de 440 kg de peso corpóreo. O PD constou de 36 horas de jejum hídrico e alimentar associadas a duas administrações intravenosas de furosemida, sendo a primeira imediatamente no início (T-36) e a segunda 12 horas após o início do PD. Os tratamentos utilizados foram: SeHIPO e SeISO, ambas administradas por via nasogástrica em fluxo contínuo (HETfc), e RL IV administrada pela via intravenosa. Todos os tratamentos foram administrados a uma taxa de infusão contínua de 15mL kg-1 h-1 durante 8 horas consecutivas. O delineamento experimental utilizado foi o crossover6x3, onde cada animal foi submetido, em sistema de rodízio, aos três tratamentos em momentos distintos. As soluções eletrolíticas enterais demonstraram maior eficácia na recomposição do teor de umidade das fezes quando comparadas à terapia RL IV. A hidratação enteral com soluções isotônicas e hipotônicas administrada em fluxo contínuo são eficazes em restaurar o teor de umidade das fezes, podendo ofertar uma opção econômica, segura e eficiente na reidratação de pacientes e nas afecções que cursam como obstruções intraluminais simples.


The occurrence of pathophysiological processes that curse with digesta dryness in the gastrointestinal tract of horses is common in clinical routine, factors such as decreased intestinal motility and intraluminal overload of dry content can lead to compaction in segments such as cecum and colon. This study aimed to compare a hypotonic enteral solution (SeHIPO), an isotonic enteral solution (SeISO) and a Ringer with sodium lactate solution (RL IV) over the moisture content of equine feces submitted to an experimental dehydration protocol. Six adult horses were used, all females aged between 10 and 15 years, average body weight of 440 kg. The PD consisted of a 36 hours period of water and food fasting associated with two intravenous administrations of furosemide, the first immediately at the beginning (T-36) and the second 12 hours after the beginning of the PD. The treatments used were: SeHIPO (hypotonic enteral solution administered via nasogastric), SeISO (enteral isotonic solution administered via nasogastric) and RL IV (Ringer's solution with sodium lactate administered intravenously), all treatments were administered by continuous infusion at a rate of 15mL kg-1 h-1 for 8 consecutive hours. The experimental design used was the 6x3 crossover, where each animal is submitted, in a rotation system, to the three treatments at different times. Enteral fluid therapy with isotonic and hypotonic solutions administered in continuous flow are effective in restoring the moisture content of feces, and may offer an economical, safe, and efficient option for rehydrating patients and in conditions that progress as simple intraluminal obstructions.

Background: Equine colic syndrome comprises numerous conditions associated with abdominal pain in horses. Impaction, a common cause of this manifestation, is strongly related to these animals' diet. Highly fibrous diets such as sugarcane can predispose horses to colic. The clinical condition can be worsened by fermentative processes, which lead to dysbiosis, circulatory disorders and even endotoxemia. The aim of this study was to report 4 cases of colic syndrome among 8 horses that underwent an experiment to adapt them to a sugarcane-based diet, and to correlate the animals' clinical conditions to the forage they ingested. Cases: Eight male castrated Mangalarga Marchador horses, between 5.5 and 7 years old, were subjected to an experiment to test the feasibility of sugarcane as forage. Four of these horses were taken to the Large Animal Veterinary Hospital (HVGA) of the Federal Rural University of Rio de Janeiro for treatment of abdominal signs of discomfort a few days after the exclusive consumption of sugarcane, in a proportion of 1.75% of live weight in dry matter. The animals' symptoms ranged from behavioral signs indicative of pain to changes in vital parameters and structure of the feces, as well as changes revealed by transrectal palpation. Three of the 4 cases presented impaction in the small colon, and 1 of the horses also presented impaction in the right dorsal colon and rostral displacement of the pelvic flexure, with accumulation of contents in the right ventral colon and sternal flexure. Two of the cases were treated medically, while the other 2 required surgical intervention. The clinical condition of all the patients evolved favorably and they were discharged between 2 and 18 days. Discussion: Colic originating in the digestive system is a syndrome strongly associated with management, especially with respect to confinement, nutrition, and parasite control. During the experiment, 4 of the 8 horses fed with sugarcane presented with colic syndrome. The low quality of sugarcane fiber is due to the high degree of lignification of the plant cell wall, which favors accumulation of ingesta. The poor digestibility and sweet taste of this roughage favor increased consumption. Furthermore, its high sucrose content, associated with an increased rate of passage in the small intestine, alters the intestinal microbiome, and hence, the fermentation byproducts and pH of the ingesta. High intestinal content, allied to longer retention times in the colon and activation of the renin-angiotensin-aldosterone system, promote greater dryness of the ingesta, predisposing the occurrence of impactions in the most distal portion of the large intestine. Intestinal distension and mesenteric traction caused by the accumulation of contents and gases trigger pain, which can worsen due to displacement of the large colon. Small colon impaction, which is easily identified by transrectal palpation, evolves gradually and its treatment, both clinical and surgical, tends to have a favorable prognosis. The need for alternative food sources for horses is a growing demand; however, sugarcane as an exclusive roughage has been shown to be unsafe for horses. The low quality of the fiber and the high sucrose content of this forage can alter the digestive physiology of horses through changes in the passage rate, microbiome and motility of digesta, predisposing them to intestinal dysfunction, ingesta compaction and displacement of the large colon.

Foram utilizados 16 equinos adultos hígidos, sem raça definida, de ambos os sexos, quatro machos e 12 fêmeas, com idade variando entre quatro e 14 anos e peso corporal médio de 248,40 + 2,28 kg, divididos em quatro grupos, quatro animais por grupo: Grupo I (ENTGL): fluidoterapia enteral com eletrólitos associada a glutamina; Grupo II (PARGL): Nutrição parenteral total (NPT) associada a glutamina; Grupo III (ENTFL): fluidoterapia enteral com eletrólitos; Grupo IV (PARFL): fluidoterapia parenteral. O delineamento experimental foi inteiramente ao acaso, em um esquema fatorial 4x12 (grupos x tempo de colheita), para cada fase, e suas médias comparadas pelo teste de Duncan ao nível de 5% de significância. Independente do grupo experimental ocorreu redução da motilidade gastrintestinal durante a fase de inanição, mais pronunciada nos grupos PARGL e PARFL. Uma vez restabelecida a alimentação a motilidade gastrintestinal retornou à normalidade.

The experimental design was entirely randomized, in a 4x12 factorial scheme (groups x harvest time), for each phase, and their means compared by the Duncan test at the level of 5% significance. Regardless of the experimental group, there was a reduction in gastrointestinal motility during the starvation phase, which was more pronounced in the PARGL and PARFL groups. Once the food was restored, gastrointestinal motility returned to normal.

This study set out to determine the impacts of a commercial equine enteral nutrition product on fecal pH, buffering capacity (BC) and physical examination variables. Eight healthy horses were randomly allocated to one of two simultaneous experimental groups in a 4×4 Latin square design. Horses were submitted to 12 hours of solid fasting, then fed increasing doses of the test product via nasogastric tube, as follows: 0% (pure water), 50%, 75% and 100% of the daily recommended dose. Test product doses were diluted in water (1:3) and delivered by bolus feeding. Fecal samples were taken directly from the rectal ampulla prior to (T0) and within 3, 6, 12, 24, 36 and 48 hours of product administration (T3, T6, T12, T24, T36 and T48 respectively). Within 24 to 36 hours of product administration, fecal pH was near 6 (p = 0.01). However, dose variation had no effect on pH. Product dose and sample collection time had a significant impact (p = 0.00) on buffering capacity at pH 6. The more dramatic drop in pH occurred within 24 to 36 hours of product administration, except in horses receiving the 0% dose (water). At pH 5, buffering capacity was affected by dose but not by sample collection time. Soft fecal consistency, increased intestinal motility and fat droplets in fecal samples were noted in most horses. Fecal pH and buffering capacity assessment are indirect tests. Still, results obtained from these tests were deemed useful for detection of intestinal changes, particularly when combined with physical examination. The product had an impact on faecal pH, buffering capacity and intestinal motility, therefore, it is recommended that the formulation be revised.(AU)


O objetivo desse estudo foi avaliar o desempenho de uma formulação comercial para nutrição enteral em equinos quanto aos aspectos de pH fecal, capacidade tamponante (CT) nas fezes e exame físico. Para tal, utilizaram-se 8 equinos hígidos, distribuídos aleatoriamente em dois Quadrados Latinos 4 X 4 simultâneos, que permaneceram em jejum alimentar 12 horas antes de receberem via sonda nasogástrica doses crescentes de 0% (somente água), 50%, 75% e 100% da quantidade total diária recomendada do produto, com a diluição estabelecida de três partes de água para cada parte de produto. Foram colhidas amostras de fezes diretamente da ampola retal antes da administração do produto (T0), 3 (T3), 6 (T6), 12 (T12), 24 (T24), 36 (36) e 48 (T48) horas após. Entre 24 a 36 horas após a administração do produto, o pH fecal estava próximo a 6 (p = 0,01). No entanto, a variação da dose não teve efeito sobre o pH. A dose do produto e o tempo de coleta da amostra tiveram um impacto significativo (p = 0,00) na capacidade tamponante das fezes em pH 6. A queda mais dramática no pH ocorreu dentro de 24 a 36 horas da administração do produto, exceto em cavalos que receberam a dose de 0% (água). Em pH 5, a capacidade tamponante das fezes foi afetada pela dose, mas não pelo tempo de coleta da amostra. Consistência fecal mole, aumento da motilidade intestinal e gotículas de gordura nas amostras fecais foram observados na maioria dos cavalos. A avaliação do pH fecal e da capacidade de tamponante são testes indiretos. Ainda assim, os resultados obtidos com essas análises foram considerados úteis para a detecção de alterações intestinais, principalmente quando combinados com o exame físico. O produto teve impacto no pH fecal, capacidade tamponante das fezes e motilidade intestinal, portanto, recomenda-se que a formulação seja revisada.(AU)

Thus, a case of RGIS resultingfrom environmental change in a domestic rabbit (Oryctolagus cuniculus) is reported.Case: A 4-month-old male domestic rabbit weighing 0.962 kg was referred to a university veterinary hospital. The patient had a history of absence of defecation and anorexia for the past two days. The owner reported that the patient wasapathetic but became aggressive when manipulated. The changes started after a move of residence. On physical evaluation, an increase in abdominal volume and a painful response to touch on the abdomen were observed. Complementaryexaminations were performed, such as a blood count and an ultrasound study. The blood tests showed no alterations,but the ultrasound evaluation showed the presence of free abdominal fluid, dilated intestinal loops due to fluid content,reduced gastrointestinal motility, and a hyperechoic structure associated with acoustic shading in the small intestine, allfindings suggestive of obstruction. Given the failure of clinical management, the patient was referred for an exploratorylaparotomy procedure followed by enterotomy. The obstruction point was located near the ileocecal junction. After surgery,analgesics, antibiotics, fluid therapy, anti-inflammatory drugs, intestinal motility inducers, and probiotics were prescribed.One week after the surgical procedure, the patient showed improvement in the clinical condition, with normal appetite,defecation, and docility...

Ultrasonographic examinations of 30 healthy neonates in their 20 days of life were performed to assess the thickness of jejunal segments and intestinal motility. There was no correlation between the thickness and the gender of the newborn and motility remained almost constant, reaching an average of 11.2 peristaltic movements per minute. A result of 0.21 cm ± 0.04 cm was obtained. The information obtained is relevant in relation to the small intestine of neonatal patients, as well as about the intestinal motility.(AU)

Resumo

Purpose To investigate the effect of probiotics on spontaneous contractions of smooth muscle isolated from jejunum and ileum of rat model. Methods Four rat groups were created (n=8, in each) including control (Group 1), control+probiotic (Group 2), short bowel (Group 3), and short bowel+probiotic (Group 4). Groups 1 and 2 underwent sham operation, Groups 3 and 4 underwent massive bowel resection. Bifidobacterium Lactis was administered in Groups 2 and 4 daily (P.O.) for three weeks. On postoperative week 3, rats were sacrificed, and jejunum and ileum smooth muscle were isolated for organ bath. Muscle contraction changes were analyzed before and after addition of antagonists. Results Short bowel group exhibited increased amplitude and frequency of spontaneous contractions. The addition of probiotics significantly decreased enhanced amplitude and frequency of bowel contraction in short bowel group and returned to control values. L-NNA increased amplitude and frequency of contractions in all groups. While indomethacin and nimesulide increased the amplitude in all groups, the frequency was only increased in jejunum. Hexamethonium and tetrodotoxin did not change the contraction characteristics in all groups. Conclusion We suggest that early use of probiotics may significantly regulate bowel motility, and accordingly improve absorption of nutrients in short bowel syndrome.(AU)

O íleo paralítico é uma obstrução do tipo funcional, na qual o lúmen intestinal está patente, comprometendo a passagem da ingesta no intestino. Este relato objetiva descrever três casos em vacas com aptidão leiteira, atendidos na rotina hospitalar e diagnosticados com íleo paralítico. Em seus históricos, os proprietários queixavam-se que os animais apresentavam redução do apetite, timpania ruminal, diminuição da produção de leite e eliminação das fezes. Ao exame físico, alguns sinais clínicos apresentaram destaque, como apatia, desidratação, redução da motilidade ruminal e intestinal, fezes em pequena quantidade e com muco, distensão do abdômen e ao balotamento constatou-se a presença de líquido. Na análise do fluido ruminal todos os animais apresentaram o teor de cloreto elevado (>30 mEq/L), caracterizando, dessa forma, um processo obstrutivo. Diante dos achados, suspeitou-se inicialmente de um quadro de obstrução intestinal. Nos casos, a conduta adotada foi realizar uma laparotomia exploratória através do flanco direito, porém constatou-se, que não existia qualquer segmento com obstrução de natureza mecânica, que justificasse as alterações físicas e laboratoriais encontradas. Diante destes resultados, configurou-se um quadro clínico indicativo de íleo paralítico. As vacasforam submetidas a um protocolo terapêutico pós-cirúrgico composto por antibioticoterapia, anti-inflamatório, cálcio, procinético e tratamento de suporte. Os animais manifestaram uma resposta favorável a conduta terapêutica, com restabelecimento da função gastrointestinal e dos demais parâmetros fisiológicos, recebendo alta após uma evolução clínica variando entre dez a doze dias.

In the analysis of the ruminal fluid, the chloride content in both was high (>30 mEq/L), characterizing an obstructive process. In the face of the findings, a diagnosis of intestinal obstruction was initially suspected. In animals, the adopted approach was to perform an exploratory laparotomy through the right flank, but it was verified that there was no segment with mechanical obstruction that justified the physical and laboratorial alterations found. In view of these results, a clinical diagnosis indicative of paralytic ileus was established. The three animals were submitted to a post-surgical therapeutic protocol consisting of antibiotic therapy, anti-inflammatory, calcium, pro-kinetic and supportive treatment. The animals showed a favourable response to therapeutic treatment, with restoration of gastrointestinal function and other physiological parameters, and was discharged after a clinical evolution ranging from ten to twelve days.

Resumo

The aim of this study was to evaluate the effects of Psyllium (PSY) and Carboxymethylcellulose (CMC) administration on fecal elimination of sand in horses with asymptomatic sand accumulations. Eight horses were selected from sandy areas and randomly divided into 2 groups of four animals. The subjects were treated either with CMC or PSY. The presence of intestinal sand was confirmed through radiography and glove sedimentation test. The study was performed in two phases, with a 7-day interval. In phase I, all the animals received 8 liters of warm water; in phase II, the CMC group received 8 liters of water + 1g/kg of CMC, whereas the PSY group received 8 liters of water + 1g/kg of PSY. All administrations were performed through nasogastric intubation and fractionated in 2 equal volume administrations with an interval of two hours. General and specific physical examination of the digestive system were performed in conjunction with abdominal ultrasonography before the administrations and after 6, 12, 24, 36 and 48 hours, aiming to evaluate intestinal motility and presence of sand. All the feces eliminated by the animals within the 72 hours following the administrations were quantified, diluted and sedimented in order to calculate the sand output (g/kg of feces). All the animals were also subjected to radiographic examination to quantify sand accumulation prior to phase I and after 72 hours of phases I and II. No adverse effects were observed after the treatments. It was possible to notice higher sand elimination in both groups during the phase I, whereas no difference was observed in sand elimination rates between the groups in phase II. The radiographic scores presented differences between the initial timepoint and 72h in phases I and II for both groups. Based on the sand elimination rates and radiographic score, this study demonstrated that sand output was greater after administration of water alone, compared to CMC and Psyllium, leading to the inference that removal of the sandy environment and prevention of sand re-ingestion are effective measures for the elimination of sand from the colon of horses with asymptomatic sand accumulations.(AU)


O objetivo deste estudo foi avaliar os efeitos da administração do psyllium (PSY) e da carboximetilcelulose (CMC) sobre a eliminação fecal de areia em equinos com sablose assintomática. Oito equinos com confirmação radiográfica de sablose assintomática foram divididos em dois grupos (grupo CMC e grupo PSY). O estudo foi realizado em duas fases, com intervalo de sete dias. Na fase 1, todos os animais receberam 8L de água; na fase 2, o grupo CMC recebeu 8L de água + 1g/kg de CMC e o grupo PSY recebeu 8L de água + 1g/kg de PSY. Antes da administração de cada solução e após seis, 12, 24, 36 e 48 horas, foram realizados exame físico e ultrassonografia abdominal. Todas as fezes eliminadas em 72 horas foram avaliadas para quantificar a eliminação de areia (g/kg de fezes). Antes da fase 1 e após 72 horas das fases 1 e 2, o exame radiográfico foi realizado para quantificar o escore de acúmulo de areia. Houve maior eliminação de areia após a administração de água em comparação com a administração de CMC, e não se observou diferença entre a CMC e o PSY. Uma redução significativa nos escores radiográficos de acúmulo de areia foi observada após a administração de água, bem como a manutenção dos escores após a administração da CMC e do PSY. Com base na produção de areia e no escore radiográfico, este estudo sugere que a remoção do ambiente arenoso, impedindo a reingestão de areia, é uma medida eficaz para a eliminação da areia do cólon de cavalos com acúmulos de areia assintomáticos.(AU)

Background: Megaesophagus is a chronic dilation of the esophagus rarely found in horses. It’s a non-specific disease that is associated with several causes, and esophageal hypomotility is the dysfunction that most commonly results in organ dilation. In the literature, there are few reports of megaesophagus in horses and, to date, no cases in mule have been reported. The objective of this work is to describe a case of a donkey with thoracic megaesophagus. Case: A 16 year-old donkey, castrated male, mixed breed weighing 195 kg, was referred for clinical care with a history of 5 days of anorexia. On physical examination, apathy, cachexia, 8% dehydration, moderate enophthalmos, ptialism, bilateral nasal discharge, dry and bristling hair were observed. Due to the poor general condition, a nasogastric tube was chosen to perform enteral nutrition, however, it was not possible to progress the tube to the stomach. In order to confirm the suspicion of a possible esophageal obstruction, gastroscopy was performed, where it was possible to observe an esophageal dilation filled with bulky food located in the thoracic portion of the esophagus. In an attempt to stimulate esophageal motility, in order to promote the progression of the material present in the region of dilation, intramuscular metoclopramide was administered (two applications every 6 h), however the treatment had no effect. Due to the unfavorable prognosis and financial limitations of the owner, euthanasia was performed, which was followed by autopsy and histopathological examination. At necropsy, a marked dilation of the esophagus was observed in the thoracic portion, which was filled with approximately 500 grams of bulky food (grass). In the mucosa of this area...

Resumo

Due to the growing increase in the dental routine in equine medicine, along with the need for patient sedation to perform this procedure, this study aimed to assess sedation by detomidine alone and/or associated with lidocaine under continuous infusion. Twenty-two horses from the dental routine of the veterinary hospital of the institution of origin were used. A jugular catheter was implanted on the day of study and animals were allocated into two distinct groups (n=11): GD, which received detomidine (20 µg kg−1), followed by continuous infusion (20 µg kg−1 h−1); and GDL, animals which received detomidine (10 µg kg−1) and lidocaine (1.3 mg kg−1), followed by continuous infusion of detomidine (10 µg kg−1 h−1) and lidocaine (25 µg kg−1 min−1). The assessed moments were as follows: M0, before catheter placement; M1, 5 minutes after bolus and beginning of infusion; M2, placement of mouth speculum; and M3, wear with an electric rasp. The assessed parameters were heart rate (HR), respiratory rate (f), systolic blood pressure (SBP), and intestinal motility. In addition to the clinical parameters, sedation was assessed by measuring head height in relation to the ground before (M0) and after treatment administration (M1) and during the dental procedure using a scale adapted for dental procedures. Statistical analysis was performed using the Shapiro-Wilk normality test by applying one-way RM ANOVA, followed by the Dunnett test for comparison in relation to M0, t-test, and Mann-Whitney test between groups (p ≤ 0.05). HR decreased by 20% at M2 and M3 compared to M0 only in GD, while SBP increased 35% at the same moments compared to M0 in GD and 27, 42, and 27% at M1, M2, and M3 compared to M0 in GDL, respectively. Intestinal motility decreased by 75% at all moments compared to M0 in both groups. Head height decreased by 67% in GD and 60% in GDL, with no difference between groups. Sedation scores provided by the scale did not differ between groups, with values of 1 [0­3] in GD and 3 [0­4] in GDL at M2 and 1 [0­3] in GD and 1 [1.5­3] in GDL at M3. Thus, sedation by the association between detomidine and lidocaine allowed reducing detomidine dose in half but offering similar results for dental procedures.(AU)


Devido ao crescente aumento da rotina odontológica na medicina equina, juntamente com a necessidade de sedação dos pacientes para a realização destes procedimentos, objetivou-se avaliar a sedação promovida pela detomidina isolada e, ou, associada com lidocaína sob regime de infusão contínua. Foram utilizados 22 equinos provindos da rotina odontológica do hospital veterinário da instituição de origem. No dia do estudo, foi implantado um cateter na jugular, alocando-se os animais em 2 grupos distintos (n = 11): GD, os quais receberam detomidina (20 µg kg-1) seguido de infusão contínua (20 µg kg-1 h-1); GDL, os quais receberam detomidina (10 µg kg-1) e lidocaína (1,3 mg kg-1), seguido de infusão contínua de detomidina (10 µg kg-1 h-1) e lidocaína (25 µg kg-1 min-1). Os momentos avaliados foram: M0, antes da colocação do cateter; M1, 5 minutos após o bolus e início da infusão; M2, no momento da colocação do abre bocas; M3, momento do desgaste com a grosa elétrica. Os parâmetros avaliados foram: frequência cardíaca (FC), frequência respiratória (f), pressão arterial sistólica (PAS) e motilidade intestinal. Além dos parâmetros clínicos, foi avaliada a sedação medindo-se a altura de cabeça em relação ao solo antes (M0) e após a administração dos tratamentos (M1), assim como durante o procedimento odontológico através de uma escala adaptada para procedimentos odontológicos. A análise estatística foi realizada através do teste de normalidade Shapiro-Wilk, aplicando-se One Way RM ANOVA seguido por Dunnet para comparação em relação ao M0, teste de t e Mann-Whitney entre grupos (p ≤ 0,05). A FC reduziu 20% no M2 e M3 em relação ao M0 somente no GD, já a PAS aumentou 35% nos mesmos momentos em relação ao M0 no GD e no M1, M2 e M3 do GDL também houve um aumento de 27%, 42% e 27%, respectivamente, em relação ao M0. A motilidade intestinal reduziu 75% em todos os momentos em relação ao M0 em ambos os grupos. A altura da cabeça diminuiu 67% no GD e 60% no GDL, sem diferença entre os grupos. Os escores de sedação fornecidos pela escala não diferiram entre os grupos, sendo este no M2 de 1 [0 ­ 3], no GD, e 3 [0 ­ 4], no GDL, e no M3, no GD 1 [0 ­ 3], e 1 [1,5 ­ 3], no GDL. Conclui-se que a sedação ofertada pela associação de detomidina e lidocaína reduziu a dose de detomidina pela metade, oferecendo sedação similar para procedimentos odontológicos.(AU)
