Resumo

Background: The congenital flexural deformity is common in cattle, often affecting the metacarpophalangeal joint of thethoracic limbs. The deformity may be mild, moderate, or severe, and the therapy depends on the limb’s degree of flexionand the affected joint. In severe deformities, tenotomy of the flexor tendons and desmotomy of the suspensor ligament isrecommended. However, this surgical technique may not be sufficient to promote limb extension, and other interventionsmay be necessary. Thus, the purpose of this report is to describe a technique to correct severe flexural deformities of themetacarpophalangeal joint in calves.Case: A 3-month-old, female, Dutch-bred calf weighing 46 kg was referred for treatment of congenital flexural deformity.On attendance, the patient presented severe deformity in the right thoracic limb and mild in the left thoracic limb both at theheight of the metacarpophalangeal joints. During palpation it was possible to notice that the flexor tendons were contractedin both limbs. Radiographic exams were performed to rule out the presence of other diseases, confirming the diagnosis offlexural deformity. The patient was referred to surgery to correct the anatomical anomaly. The animal was submitted to general anesthesia and placed in right lateral decubitus. In the left thoracic limb, an incision was made in the medial region ofthe metacarpal bone, the tissues were divulsioned until the superficial digital and deep digital flexor tendons were exposed;these structures were sectioned with a scalpel, and the limb was extended, returning to the standard anatomical position. Foram utilizados oito animais, da raça quarto de milha e seus mestiços, de diferentes idades, peso e sexo, selecionados e questionados sobre o tempo que exerciam a atividade de vaquejada e o tipo de treinamento a qual eram submetidos, que refere-se a maneira como praticavam a prova correndo de que lado do boi, em uma prova de vaquejada no município de Cristino Castro, Sul do Piauí, Brasil, onde os participantes eram provenientes de diferentes haras da Microrregião do Alto Médio Gurgueia, Sul do Piauí, Brasil. Em cada animal foi realizada ultrassonografia das zonas IB, IIB e IIIB dos tendões flexores digital superficial (TFDS) e digital profundo (TFDP), obtendo-se as medidas dos diâmetros médio lateral (DML) e dorso palmar (DDP) através do plano transverso em cada uma das diferentes zonas. Os dados foram submetidos a análise estatística (ANOVA), utilizando-se o teste t a nível de significância de (p<0,05), observando-se que não houve diferença significativa entre as medidas avaliadas. De acordo com os resultados obtidos, cavalos atletas que correm a direita do boi não apresentam diferença significativa nos diâmetros médio lateral e dorso palmar dos tendões flexores entre os antímeros direito e esquerdo do membro torácico.(AU)

Eight animals of the quarter-mile breed and their crossbreed, of different ages, weight and sex, were selected and asked about the time they were engaged in the activity of the vaquejada and the type of training they were submitted to as they practiced running on which side of the ox, in a vaquejada test in the municipality of Cristino Castro, South of Piauí, Brazil, where the participants came from different farms of the microregion of the Alto Médio Gurgueia, South of Piauí, Brazil. The ultrasonography of the IB, IIB and IIIB zones of the superficial digital (TFDS) and deep digital flexor tendons (TFDP) were performed in each animal, obtaining the measurements of the lateral diameters (DML) and palmar dorsum (DDP) through the transverse plane in each of the different zones. Data were submitted to statistical analysis (ANOVA), using the t test at the significance level of (p <0.05), observing that there was no significant difference between the measures evaluated. According to the results, horses for vaquejada athletes do not present significant difference in the lateral diameters and palmar dorsum of the flexor tendons between the right and left thoracic limb antimeres.(AU)

Resumo

The gross anatomy of the digital flexors tendons and the distal accessory ligament of the Catalan Pyrenean Horse, an equine trait breed from Catalonia (NE Spain), is presented here. Research has been based on the dissection of isolated 10 right forelimbs, 10 left forelimbs, six right hindlimbs and five left hindlimbs (total 31 distal limbs) from clinically normal yearlings, about 12 months of age. Tendons dimensions tended to be bigger than those reported in the literature, probably due to breed, age, bodyweight, withers height and exercise programme. Distal accessory ligament was sometimes absent. Data intend to serve as standard manual data for this breed but also for other trait breeds, being the present study the first one on the subject.(AU)


A anatomia dos tendões flexores digitais e o ligamento acessório distal do Cavalo Pirineu Catalão, uma raça de equinos de tração da Catalunha (NE Espanha) está apresentada aqui. A pesquisa se baseou na dissecação  para isolamento de 10 membros anteriores direitos, 10 membros anteriores esquerdos, seis membros posteriores direitos e cinco membros posteriores esquerdos (total de 31 membros) de animais clinicamente normais, com cerca de 12 meses de idade. As dimensões dos tendões tendem a ser maiores do que as relatadas na literatura, provavelmente devido à raça, idade, peso corporal, altura de cernelha e programa de exercício. O ligamento acessório distal às vezes estava ausente. Os dados pretendem servir como padrões para essa raça, mas também para outras raças de tração, sendo o presente estudo o primeiro sobre o assunto.(AU)

Resumo

Flexural deformity in growing animals is a condition in which a joint is kept in a flexed or abnormally extended position. In foals, it is characterised mainly by deviation of the limbs. As such, the aim is to report a case of flexural deformity in a pelvic limb of Quarter Horse. The animal was treated at the Jerônimo Dix-Huit Rosado Maia Veterinary Hospital (HOVET-UFERSA): a female, with dappled coat, approximately two years of age, presenting with interphalangeal flexural deformity of the left pelvic limb. In the anamnesis, the owner reported that the animal had shown the deformity from six months of age, and on physical examination, presented grade IV lameness of the left pelvic limb, excessive wear of the toe, with the limb offering no support. Upon palpation, contracture of the superficial and deep digital flexor tendons was identified. Surgical treatment was therefore chosen, including tenotomy of the deep digital flexor tendon together with desmotomy of the lower check ligament. Medicinal therapy based on antibiotics and an anti-inflammatory was carried out, in addition to corrective trimming of the hoof and a horseshoe with toe extension; cleaning and changing the dressing was performed every 48 hours, and active physical therapy was given through a daily 15-minute walk. After 15 days hospitalisation, the animal was discharged, showing considerable improvement in it


A deformidade flexuralé uma condição de animais em crescimento, na qual uma articulação é mantida em posição flexionada ou estendida anormalmente. Em potros, caracteriza-se principalmente pelo desvio dos membros. Desta forma, objetiva-se relatar um caso de deformidade flexuralem membro pélvico de equino Quarto de Milha. Foi atendido no Hospital Veterinário Jerônimo Dix-Huit Rosado Maia (HOVET-UFERSA), umafêmea, da raça Quarto de Milha, pelagem tordilha, com aproximadamente dois anos de idade, apresentando deformidade flexuralinterfalangeana em membro pélvico esquerdo. Na anamnese, o proprietário relatou que o animal apresentava encastelamento do membro desde os 6 meses de idade. Ao exame físico, apresentou claudicação grau IV no membro pélvico esquerdo, desgaste excessivo de pinça, e sem apoio do membro. À palpação foi identificada contratura dos tendões flexores digitais superficial e profundo. Desta forma, optou-se pelo tratamento cirúrgico de tenotomia do tendão flexor digital

This study set out to determine normal values for acoustic radiation force impulse (ARFI) elastography of the superficial digital flexor tendon, deep digital flexor tendon, accessory ligament of the deep digital flexor tendon and suspensory ligament in horses. Twenty-four horses were submitted to conventional sonographic assessment and elastography imaging (quantitative ARFI method) of distal forelimb tendons and ligaments at nine anatomical regions. Elastograms depicted tendons and ligaments in orange to red tones consistent with greater stiffness. Shear wave velocity was higher in the longitudinal compared to the cross-sectional plane but did not differ significantly between the left and right forelimbs. The suspensory ligament was stiffer compared to remaining structures in the cross-sectional plane. Ligaments were stiffer than tendons and the deep digital flexor tendon was stiffer than the superficial digital flexor tendon in the longitudinal plane. Target structures were stiffer in male compared to female horses and stiffness tended to increase with age. ARFI elastography proved to be a feasible and reproducible imaging modality for assessment of distal forelimb tendons and ligaments in horses. Qualitative features derived from stiffness quantification in these structures can be applied to horses with musculoskeletal disorders, as well as to other animal species.(AU)


Este trabalho objetivou estabelecer valores de referência por meio da elastografia ARFI ("Acoustic Radiation Force Impulse") do tendão flexor digital superficial, tendão flexor digital profundo, ligamento acessório inferior e ligamento suspensor do boleto em equinos hígidos. Vinte e quatro equinos foram submetidos aos exames ultrassonográfico e elastográfico (método ARFI quantitativo) dos tendões e ligamentos da porção distal do membro torácico, em nove regiões anatômicas diferentes. Os elastogramas mostraram tendões e ligamentos de coloração alaranjada a vermelha, que representam maior rigidez. Não houve diferenças em relação ao membro avaliado; as velocidades de cisalhamento foram maiores nos planos longitudinais. No plano transversal, os ligamentos suspensores foram mais rígidos em comparação aos demais. No plano longitudinal, os ligamentos mostraram-se mais rígidos que os tendões, e o tendão flexor digital profundo, mais rígido que o tendão flexor digital superficial. As estruturas mostraram-se mais rígidas nos machos, e houve uma tendência à maior rigidez em animais mais velhos. A elastografia ARFI comprovou ser um método viável e reprodutível para avaliação dos tendões e ligamentos dos membros torácicos de equinos. As características qualitativas obtidas a partir da rigidez dessas estruturas podem ser utilizadas para testes em equinos com distúrbios musculoesqueléticos e também em outras espécies animais.(AU)

Resumo

Background: Tendon injuries are common in horses and are commonly associated with lameness and athletic career disruption.Adhesions formed between the tendons and the surrounding tissues compromise the sliding and movement of the structures,compromising their functionality. Therefore, the control of adhesion formation is critical to restore the structural integrity of thetendon, as well as its biomechanical function. The aim of this study was to evaluate the behavior of the biosynthetic cellulosemembrane implanted in foals with surgically induced tendinitis of the superficial digital flexor.Materials, Methods & Results: Six healthy foals were used, which underwent tendinitis induction in the superficial digital flexorof the right and left forelimbs. The lesions was induced by local ischemia by crushing the tendon with hemostatic forceps. Thebiosynthetic cellulose membrane was implanted only in the right forelimb, involving the superficial digital flexor tendon in theregion of the ischemia and the left forelimb was used as control. After surgery, both forelimb were immobilized with syntheticplaster cast for 15 days. Ultrasonography was performed in six foals immediately before (M0), at 15° (M1) at and 30° (M2) day,and three were evaluate at 45° (M3) and 60° (M4) day after surgery. Incisional biopsies were performed in three animals on the30° day and in three animals on the 60° day after surgery. Histopathological examination involved the analysis of tissue disorganization, presence and type of inflammatory infiltrate and neovascularization, according to the score of 0 to 3. Ultrasonographyallowed visualization of the membrane, which was characterized as a continuous hyperechoic line at the edges of the tendon at15 days (M1). In addition, at 30 and 45 days after surgery, interruption of the hyperechoic line and reduction of echogenicitywere observed, and no echogenic lines were observed at 60 days after surgery...(AU)

Background: The common calcaneus tendon is formed by the common tendon of the femoral biceps muscle, tendon of the superficial digital flexor muscle and gastrocnemius muscle tendon, and its rupture is related to acute traumas in small animals. Among some applications described, surgical repair can be adopted in the correction of tendon ruptures and the use of grafts may be indicated in some severe lesions, where it is not possible to perform tenorrhaphy. This study aims to report the case of a feline with rupture of the common calcaneus tendon in which the tenorrafia associated with the autogenous fascia lata graft was performed as adjuvant to the cicatrization of the same.Case: A 24-month-old, undefined male feline with approximately 3 kg was treated at the FCAV Veterinary Hospital, UNESP Jaboticabal, State of São Paulo, Brazil, with a history of falling, for approximately two weeks, and has since plantigrade posture. Upon physical examination, it was possible to identify a discontinuity of the common calcaneus tendon. The radiographs were performed, which did not show any bone changes. A hemogram and biochemical profile were performed, which were within normal limits. The patient underwent surgery, and a complete lesion of the common calcaneus tendon was identified, with a chronic aspect. Approximation of the stumps was impossible, so a flap of fascia lata, about two centimeters long and one centimeter wide, was removed, which was used as a bed and tendon support for tendon growth. After removal all of the present fibrosis, the tendon stumps were approximated by means of modified Kessler suture with mononylon thread, and fascia lata flap of the ipsilateral limb was sutured with separate single points in both tendon ends and in their lateral and medial portion. In the tibiotarsic joint, the external skeletal fixator type II was used for immobilization, during a period of 30 days.[...]

The carpus is a complex articulation, which is often involved in injuries in equine athletes. The objective of this study was to suggest a didactic approach for ultrasonography training for the examination of the carpal joint in horses. Ultrasonograhy examination was performed in a healthy 14-year-old horse. The images were compared with those of a dissected anatomic specimen of the carpal region of a senior horse and with those reported in the literature. Identifiable structures were as follows: (dorsal) tendon of the muscle extensor carpi radialis, tendon of the muscle extensor carpi obliquus, tendon of the muscle commom digital extensor, dorsal synovial outpouchings, joint capsule and fat cushion, (lateral) tendon of the muscle lateral digital extensor, lateral collateral ligament (deep and superficial components), (medial) medial collateral ligament (deep and superficial components) and (palmar) palmar intercarpal ligament, carpal sheath, carpal tunnel, superficial digital flexor tendon, proximal origin of the deep digital flexor tendon, and palmar carpal ligament. Prior knowledge of the anatomy in combination with the study of anatomical specimens is essential for recognizing musculoskeletal structures during ultrasound examination, contributing to the training and learning processes.


O carpo é uma articulação complexa que, com frequência, é sede das doenças que acometem cavalos atletas. O objetivo deste estudo foi sugerir uma abordagem didática para o treinamento de exame ultrassonográfico do carpo em equinos. Foi realizado o exame ultrassonográfico de um equino hígido de catorze anos, as imagens foram comparadas com uma peça anatômica dissecada da mesma região de um cadáver de equino sênior e com dados da literatura. As estruturas passíveis de identificação foram: (dorsal) tendão do músculo extensor carporradial, tendão do músculo extensor oblíquo do carpo, tendão do músculo extensor digital comum, recessos sinoviais dorsais, cápsula articular, coxim de gordura, (lateral) tendão do músculo extensor digital lateral, ligamento colateral lateral (componente profundo e superficial), (medial) ligamento colateral medial (componente profundo e superficial), (palmar) ligamento intercarpiano palmar, bainha carpal, canal carpal, tendão do músculo flexor digital superficial, origem proximal do tendão do músculo flexor digital profundo e o ligamento carpal palmar. Background: Common calcaneal tendon (CCT) is usually affected by injuries in dogs and less frequently in cats. The Achilles mechanism consists of the convergence of three distinct structures: the tendons of the gastrocnemius muscle, superficial digital flexor muscle and the common tendons of the biceps femoris, gracilis, and semitendinosus muscles. Orthopedic examination showed plantigrade stance, hyperflexion of the right tibial-tarsal joint (TTJ) and swelling with fibrous thickening of the distal portion of the CCT. A tenotomia do flexor digital superficial e a desmotomia de seu acessório são rotineiramente empregadas na correção cirúrgica de deformidades flexoras metacarpofalângicas. Objetivando-se avaliar os efeitos de tais práticas cirúrgicas sobre a morfologia dos cascos de equinos, realizou-se a tenotomia do flexor digital superficial e a desmotomia de seu acessório respectivamente nos membros torácicos direito e esquerdo de nove equinos. No pré-operatório, aos 15, 30 e 60 dias após a cirurgia foram tomadas as medidas da pinça, talão lateral, comprimento e largura do casco visto pela sola, além da angulação dos mesmos nos membros operados. O casco deformou-se em resposta a ambos os procedimentos cirúrgicos, aumentando o comprimento do talão, diminuindo o comprimento e aumentando a largura do casco visto pela sola. A tenotomia promoveu ainda elevação do ângulo do casco. Os procedimentos cirúrgicos estudados alteraram significativamente o formato do casco.


The superficial digital flexor tenotomy and desmotomy of its accessories are routinely used in surgical correction of flexural metacarpophalangeal deformities. Aiming to assess the effects of these surgical procedures on hooves morphology, the superficial digital flexor tenotomy and desmotomy of its acessories was proceeded respectively in the right and left forelimbs of nine horses. Preoperatively, 15, 30 and 60 days after surgery, the measurements of the toe, lateral heel, length and width of the sole and the angulation of the same were taken. The hoof deformed in response to both surgical procedures, increasing the length of the toe, decreasing length and increasing the width of the hoof seen from the sole. The tenotomy also promoted the hoof angle elevation. The studied surgical procedures significantly altered the hoove shape by reducing its length and increasing its width, as well as increasing the height of heels.
