RESUMO

HeXEHRS is a FHIR-based cloud EHR service designed to support healthcare in depopulated areas, powered by digital twin technology. Its core functionalities encompass standard EHR tasks including data exchange for healthcare processes. In the first year of this national project, we present the design and define the functionalities of the system.

RESUMO

The Survivorship Passport (SurPass) for childhood cancer survivors provides a personalized treatment summary together with a care plan for long-term screening of possible late effects. HL7 FHIR connectivity of Electronic Health Record (EHR) systems with the SurPass has been proposed to reduce the burden of collecting and organizing the relevant information. We present the results of testing and validation efforts conducted across six clinics in Austria, Belgium, Germany, Italy, Lithuania, and Spain. We also discuss ways in which this experience can be used to reduce efforts for the SurPass integration in other clinics across Europe.

Background: The electronic health record (EHR) has been widely implemented internationally as a tool to improve health and healthcare delivery. However, EHR implementation has been comparatively slow amongst hospitals in the Arabian Gulf countries. This gradual uptake may be linked to prevailing opinions amongst medical practitioners. Until now, no systematic review has been conducted to identify the impact of EHRs on doctor-patient relationships and attitudes in the Arabian Gulf countries. Objective: To understand the impact of EHR use on patient-doctor relationships and communication in the Arabian Gulf countries. Design: A systematic review of English language publications was performed using PRISMA chart guidelines between 1990 and 2023. Methods: Electronic database search (Ovid MEDLINE, Global Health, HMIC, EMRIM, and PsycINFO) and reference searching restricted to the six Arabian Gulf countries only. MeSH terms and keywords related to electronic health records, doctor-patient communication, and relationship were used. Newcastle-Ottawa Scale (NOS) quality assessment was performed. Results: 18 studies fulfilled the criteria to be included in the systematic review. They were published between 1992 and 2023. Overall, a positive impact of EHR uptake was reported within the Gulf countries studied. This included improvement in the quality and performance of physicians, as well as improved accuracy in monitoring patient health. On the other hand, a notable negative impact was a general perception of physician attention shifted away from the patients themselves and towards data entry tasks (e.g., details of the patients and their education at the time of the consultation). Conclusion: The implementation of EHR systems is beneficial for effective care delivery by doctors in Gulf countries despite some patients' perception of decreased attention. The use of EHR assists doctors with recording patient details, including medication and treatment procedures, as well as their outcomes. Based on this study, the authors conclude that widespread EHR implementation is highly recommended, yet specific training should be provided, and the subsequent effect on adoption rates by all users must be evaluated (particularly physicians). The COVID-19 Pandemic showed the great value of EHR in accessing information and consulting patients remotely.

BACKGROUND: Patients develop pressure injuries (PIs) in the hospital owing to low mobility, exposure to localized pressure, circulatory conditions, and other predisposing factors. Over 2.5 million Americans develop PIs annually. The Center for Medicare and Medicaid considers hospital-acquired PIs (HAPIs) as the most frequent preventable event, and they are the second most common claim in lawsuits. With the growing use of electronic health records (EHRs) in hospitals, an opportunity exists to build machine learning models to identify and predict HAPI rather than relying on occasional manual assessments by human experts. However, accurate computational models rely on high-quality HAPI data labels. Unfortunately, the different data sources within EHRs can provide conflicting information on HAPI occurrence in the same patient. Furthermore, the existing definitions of HAPI disagree with each other, even within the same patient population. The inconsistent criteria make it impossible to benchmark machine learning methods to predict HAPI. OBJECTIVE: The objective of this project was threefold. We aimed to identify discrepancies in HAPI sources within EHRs, to develop a comprehensive definition for HAPI classification using data from all EHR sources, and to illustrate the importance of an improved HAPI definition. METHODS: We assessed the congruence among HAPI occurrences documented in clinical notes, diagnosis codes, procedure codes, and chart events from the Medical Information Mart for Intensive Care III database. We analyzed the criteria used for the 3 existing HAPI definitions and their adherence to the regulatory guidelines. We proposed the Emory HAPI (EHAPI), which is an improved and more comprehensive HAPI definition. We then evaluated the importance of the labels in training a HAPI classification model using tree-based and sequential neural network classifiers. RESULTS: We illustrate the complexity of defining HAPI, with <13% of hospital stays having at least 3 PI indications documented across 4 data sources. Although chart events were the most common indicator, it was the only PI documentation for >49% of the stays. We demonstrate a lack of congruence across existing HAPI definitions and EHAPI, with only 219 stays having a consensus positive label. Our analysis highlights the importance of our improved HAPI definition, with classifiers trained using our labels outperforming others on a small manually labeled set from nurse annotators and a consensus set in which all definitions agreed on the label. CONCLUSIONS: Standardized HAPI definitions are important for accurately assessing HAPI nursing quality metric and determining HAPI incidence for preventive measures. We demonstrate the complexity of defining an occurrence of HAPI, given the conflicting and incomplete EHR data. Our EHAPI definition has favorable properties, making it a suitable candidate for HAPI classification tasks.

OBJECTIVE: To evaluate the completeness of diagnosis recording in problem lists in a hospital electronic health record (EHR) system during the COVID-19 pandemic. DESIGN: Retrospective chart review with manual review of free text electronic case notes. SETTING: Major teaching hospital trust in London, one year after the launch of a comprehensive EHR system (Epic), during the first peak of the COVID-19 pandemic in the UK. PARTICIPANTS: 516 patients with suspected or confirmed COVID-19. MAIN OUTCOME MEASURES: Percentage of diagnoses already included in the structured problem list. RESULTS: Prior to review, these patients had a combined total of 2841 diagnoses recorded in their EHR problem lists. 1722 additional diagnoses were identified, increasing the mean number of recorded problems per patient from 5.51 to 8.84. The overall percentage of diagnoses originally included in the problem list was 62.3% (2841 / 4563, 95% confidence interval 60.8%, 63.7%). CONCLUSIONS: Diagnoses and other clinical information stored in a structured way in electronic health records is extremely useful for supporting clinical decisions, improving patient care and enabling better research. However, recording of medical diagnoses on the structured problem list for inpatients is incomplete, with almost 40% of important diagnoses mentioned only in the free text notes.

INTRODUCTION: The technologies used to treat the millions who receive care in intensive care unit (ICUs) each year have steadily advanced. However, the quality of ICU-based communication has remained suboptimal, particularly concerning for Black patients and their family members. Therefore we developed a mobile app intervention for ICU clinicians and family members called ICUconnect that assists with delivering need-based care. OBJECTIVE: To describe the methods and early experiences of a clustered randomized clinical trial (RCT) being conducted to compare ICUconnect vs. usual care. METHODS AND ANALYSIS: The goal of this two-arm, parallel group clustered RCT is to determine the clinical impact of the ICUconnect intervention in improving outcomes overall and for each racial subgroup on reducing racial disparities in core palliative care outcomes over a 3-month follow up period. ICU attending physicians are randomized to either ICUconnect or usual care, with outcomes obtained from family members of ICU patients. The primary outcome is change in unmet palliative care needs measured by the NEST instrument between baseline and 3 days post-randomization. Secondary outcomes include goal concordance of care and interpersonal processes of care at 3 days post-randomization; length of stay; as well as symptoms of depression, anxiety, and post-traumatic stress disorder at 3 months post-randomization. We will use hierarchical linear models to compare outcomes between the ICUconnect and usual care arms within all participants and assess for differential intervention effects in Blacks and Whites by adding a patient-race interaction term. We hypothesize that both compared to usual care as well as among Blacks compared to Whites, ICUconnect will reduce unmet palliative care needs, psychological distress and healthcare resource utilization while improving goal concordance and interpersonal processes of care. In this manuscript, we also describe steps taken to adapt the ICUconnect intervention to the COVID-19 pandemic healthcare setting. ENROLLMENT STATUS: A total of 36 (90%) of 40 ICU physicians have been randomized and 83 (52%) of 160 patient-family dyads have been enrolled to date. Enrollment will continue until the end of 2021.

INTRODUCTION: The number of older adults who receive life support in an intensive care unit (ICU), now 2 million per year, is increasing while survival remains unchanged. Because the quality of ICU-based palliative care is highly variable, we developed a mobile app intervention that integrates into the electronic health records (EHR) system called PCplanner (Palliative Care planner) with the goal of improving collaborative primary and specialist palliative care delivery in ICU settings. OBJECTIVE: To describe the methods of a randomized clinical trial (RCT) being conducted to compare PCplanner vs. usual care. METHODS AND ANALYSIS: The goal of this two-arm, parallel group mixed methods RCT is to determine the clinical impact of the PCplanner intervention on outcomes of interest to patients, family members, clinicians, and policymakers over a 3-month follow up period. The primary outcome is change in unmet palliative care needs measured by the NEST instrument between baseline and 1 week post-randomization. Secondary outcomes include goal concordance of care, patient-centeredness of care, and quality of communication at 1 week post-randomization; length of stay; as well as symptoms of depression, anxiety, and post-traumatic stress disorder at 3 months post-randomization. We will use general linear models for repeated measures to compare outcomes across the main effects and interactions of the factors. We hypothesize that compared to usual care, PCplanner will have a greater impact on the quality of ICU-based palliative care delivery across domains of core palliative care needs, psychological distress, patient-centeredness, and healthcare resource utilization.

Use of shared electronic health records opens a whole range of new possibilities for flexible and fruitful cooperation among health personnel in different health institutions, to the benefit of the patients. There are, however, unsolved legal and security challenges. The overall aim of this article is to highlight legal and security challenges that should be considered before using shared electronic cooperation platforms and health record systems to avoid legal and security "surprises" subsequent to the implementation. Practical lessons learned from the use of a web-based ulcer record system involving patients, community nurses, GPs, and hospital nurses and doctors in specialist health care are used to illustrate challenges we faced. Discussion of possible legal and security challenges is critical for successful implementation of shared electronic collaboration systems. Key challenges include (1) allocation of responsibility, (2) documentation routines, (3) and integrated or federated access control. We discuss and suggest how challenges of legal and security aspects can be handled. This discussion may be useful for both current and future users, as well as policy makers.

With the government backing the concept of electronic health records (EHR), and with technology being used in every walk of life, more and more hospitals are looking into implementing EHR systems. A national survey of U.S. hospitals was conducted in February/March 2005 to identify the status of EHR systems in hospitals regarding the core functionalities implemented (as identified by the Institute of Medicine), and to determine the perceived benefits, risks, and barriers to adoption of EHR systems in relation to the size of the hospital. The results showed that 37 percent of the hospitals that participated had some components in all of the core functionalities of an EHR system, while 27 percent were using at least some of the core functionalities. There was a significant relationship in some of the perceived benefits and barriers to adoption of EHR systems based on the size of the hospital. With regard to perceived benefits, a large correlation was found between the "medical staff's work efficiency and time management" and size of the hospital. As to perceived barriers to adoption, the cost of hardware and the size of the hospital were interwoven.
