OBJECTIVE: The aim of this investigation was to compare clinical performance and in vitro wear of temporary CAD/CAM and cartridge crowns. This study is an approach to estimate the influence of in vivo use and laboratory simulation on temporary crowns. MATERIALS AND METHODS: A total of 90 crowns were fabricated from each temporary CAD/CAM or cartridge material. Also, 10 crowns of each material were clinically applied for 14 days, and 80 identical duplicate restorations were investigated in the laboratory after storage in water (14 days; 37 °C) and subsequent thermal cycling and mechanical loading (TCML, 240.000 × 50N ML, 600 × 5°C/55 °C). After in vivo application or in vitro aging, facture force, superficial wear (mean and maximum), surface roughness (Ra, Rz), thermal weight loss (TGA), and heat of reaction (DSC) were determined for all crowns. STATISTICS: Bonferroni post hoc test; one-way analysis of variance (ANOVA); α = 0.05). RESULTS: The fracture resistance of the temporary materials varied between 1196.4 (CAD in vivo) and 1598.3 N (cartridge crown in vitro). Mean (maximum) wear data between 204.7 (386.7 µm; cartridge in vitro) and 353.0 µm (621.8 µm; CAD in vitro) were found. Ra values ranged between 4.4 and 4.9 µm and Rz values between 36.0 and 40.8 µm. DSC and TG analysis revealed small differences between the materials but a strong influence of the aging process. CONCLUSIONS: Comparison of in vivo and in vitro aging led to no significant differences in fracture force and wear but differences in roughness, DSC, and TGA. SEM evaluation confirmed comparability. Comparison of CAD/CAM and cartridge temporary materials partially showed significant differences. CLINICAL RELEVANCE: In vitro aging methods might be helpful to estimate materials' properties before principal clinical application. CAD/CAM and cartridge temporary materials provided comparable good clinical performance.

To compare an Er:YAG laser-activated bleaching system with different light-activated in-office bleaching systems for color change, surface roughness, and post-bleaching enamel bond strength. 51 enamel slabs were prepared from the sound buccal enamel of extracted bovine teeth. The teeth were randomly divided into three groups according to different light-activated office bleaching systems (n = 17): diode laser (Epic, Biolase) (940 nm, 7 W, continuous mode), Er:YAG laser (LightWalker, Fotona) (2940 nm, 50 mJ, 10 Hz, 1000 µs), and LED (Radii Plus) (440-480 nm, 1500 mW/cm2). All systems were used with their compatible bleaching agents according to manufacturers' recommendations. The tooth color and surface roughness (Ra) were assessed at baseline and after bleaching using a spectrophotometer and a surface profilometer, respectively. The color change was determined by the CIE L*a*b* system (ΔE, ΔL*, Δa*, Δb*). Kruskal-Wallis test was used for color change whereas Kruskal-Wallis and Wilcoxon tests were used to analyze the roughness data. For shear bond strength test (SBS), composite cylinders were bonded on bleached enamel samples 14 days after bleaching procedures and stored in water (37 °C). Specimens were then debonded with a universal testing machine at 1 mm/min and data were analyzed by using Kruskal-Wallis test. All the tested bleaching systems were effective on color change (ΔE > 3.3) and produced similar color change (p > 0.05). There were no significant differences among the Ra values of the groups neither at baseline nor after bleaching (p > 0.05). However, comparing the baseline and after bleaching Ra values, a significant increase was observed for all tested groups (p < 0.05). Significant differences were also found among all systems for SBS (p < 0.05). The highest values were obtained in Er:YAG group, whereas the LED group revealed the lowest values (p < 0.05). All tested bleaching systems were effective on tooth whitening, whereas they all led to an increase on surface roughness. Although the current Er:YAG laser-activated bleaching system did not differ from other tested bleaching systems according to color change and surface roughness, it appears to provide better results in terms of SBS.

PURPOSE: To evaluate the antimicrobial efficiency of three cleaning solutions and their effect on the physical properties of a denture base material. MATERIALS AND METHODS: A heat-cured polymethyl-methacrylate (PMMA) denture base material (Meliodent) and three cleaning solutions (alkaline-peroxide, 30 minutes; 1% sodium-hypochlorite, 10 minutes; and 0.1% polymeric-guanidine solution, 5 minutes) were used. For antifungal activity test, 40 disc-shaped specimens were fabricated and allocated into a control group (distilled water) and 3 experimental groups (n = 10) according to the cleaning solutions. Antifungal activity against Candida albicans (ATCC 2091) was assessed with colony-forming units. An additional 40 rectangular plate specimens were fabricated for mechanical tests. Ten specimens were kept intact to be used as the control group for flexural strength test. The remaining 30 specimens were distributed into three groups according to the cleaning solutions. The surface roughness and Vickers hardness of the specimens were consecutively measured after 48 hours of water storage at 37 ± 2°C (t0), two disinfection cycles (t1), and 7 days of storage (t2) in one of the solutions. Finally, all 40 rectangular specimens were subjected to flexural strength test. Data were analyzed with Kruskal-Wallis test for antifungal activity, ANOVA for flexural strength test, and analysis of covariance for surface roughness and hardness tests. Significance was set at 0.05. RESULTS: The antifungal activities of polymeric guanidine and sodium hypochlorite were comparable to each other and significantly higher than alkaline peroxide (p < 0.05). The changes in the surface roughness of the specimens were statistically comparable among the cleaning solutions and time periods (p > 0.05); however, the decrease in the Vickers hardness of the specimens stored in sodium hypochlorite was significantly higher from t0 to t1 and t0 to t2 (p < 0.05) than other groups, resulting in comparable hardness changes. The flexural strengths of all groups were comparable with the control after t2 (p > 0.05). CONCLUSION: The use of polymeric guanidine disinfectant solution could be an alternative method for cleaning PMMA denture base materials.

BACKGROUND AND OBJECTIVE: The laser debonding procedure of adhesively luted all-ceramic restorations is based on the ablation of resin cement due to the transmitted laser energy through the ceramic. The purpose of this study was to determine the effect of Er:YAG laser irradiation transmitted through a dental ceramic on five different resin cements. MATERIALS AND METHODS: Five different resin cements were evaluated in this study: G-Cem LinkAce, Multilink Automix, Variolink II, Panavia F, and Rely X Unicem U100. Disc shaped resin cement specimens (n = 10) were fabricated for each group. A ceramic disc was placed between the resin cement discs and the tip of the handpiece of Er:YAG laser device. The resin cement discs were irradiated through the ceramic and the volume of the resin cement discs were measured using a micro-CT system before and after Er:YAG laser irradiation. The volume loss of the resin cement discs was calculated and analyzed with one-way ANOVA and Tukey-HSD tests. RESULTS: The highest volume loss was determined in G-Cem (1.1 ± 0.6 mm3 ) and Multilink (1.3 ± 0.1 mm3 ) (P < 0.05) groups, and the lowest volume loss was determined in Rely X (0.3 ± 0.07 mm3 ), Variolink (0.4 ± 0.2 mm3 ), and Panavia (0.6 ± 0.2 mm3 ) groups (P < 0.05). All resin cements were affected by the laser irradiation resulting in the volume loss of the cement; however, there are significant differences among different resin cements. CONCLUSIONS: All the resin cements tested in this study were effected by the Er:YAG laser irradiation and there were significant differences among the resin cements with regard to ablation volume. The aim of this study was to evaluate the increase in temperature induced by various light sources during in-office bleaching treatment, under simulated blood microcirculation in pulp conditions. Ten freshly extracted human maxillary central incisors were used for the study. The roots of the teeth were removed from approximately 2 mm below the cementoenamel junction and fixed on an apparatus for the simulation of blood microcirculation in pulp. A J-type thermocouple wire was inserted into the pulp chamber through an artificial access at the lingual surfaces of the teeth, and another thermocouple wire was fixed on the labial surface of the teeth meanwhile. An in-office bleaching agent, intense red in color and with 30% water content, was applied to the labial surfaces of the teeth, and repeating measurements were made for each tooth using three different light sources: Er:YAG laser (40 mJ, 10 Hz, 20 s), 810-nm diode laser (4 W, 20 s, CW), and high-intensity light-emitting diodes (LED) (1,100 mW/cm(2), 20 s) as the control. Temperature increase in the pulp chamber and within the bleaching gel during light application were recorded and statistically evaluated. The highest pulp temperature increases were recorded for the diode laser group (2.61 °C), followed by the Er:YAG laser (1.86 °C) and LED (1.02 °C) groups (p < 0.05; analysis of variance (ANOVA), Tukey's honestly significant difference (HSD)). Contradictorily, the lowest gel temperature increases were recorded for diode laser (6.21 °C) and followed by LED (12.38 °C) and Er:YAG (20.11 °C) groups (p < 0.05; ANOVA, Tukey's HSD). Despite the significant differences among the groups, the temperature increases recorded for all groups were below the critical value of 5.6 °C that can cause irreversible harmful changes in pulp tissue. It can be concluded that, with regard to temperature increase, all the light sources evaluated in this study can be used safely for in-office bleaching treatment within the described parameters.

The aim of this study was to evaluate the effect of different surface treatments on the microtensile bond strength (µTBS) of two different self-adhesive flowable composite materials (SACs) to dentin. Forty extracted human molars were horizontally flattened to expose the dentin surfaces. The teeth were divided into eight groups according to the surface treatments (acid etching, Er:YAG laser quantum square pulse (QSP) mode, and Er:YAG laser medium-short pulse (MSP) mode at 10 Hz, 1.2 W, 120 mJ, and control-SiC) and SAC type (Vertise Flow and Fusio Liquid Dentin) (n = 5). Light cured 4-mm-high composite build-ups were made on the dentin surfaces, and the specimens were sectioned into serial sticks (n = 15). Later, the specimens were attached to microtensile testing machine and tensile force was applied at a crosshead speed of 1 mm/min until failure occurred. The failure modes were evaluated using a stereomicroscope at ×40 magnification, and one sample from each group was used for scanning electron microscopy (SEM) evaluation. The data were analyzed with two-way ANOVA and Tukey's HSD test (α = 0.05). In all surface treatment groups along with the control groups, Vertise Flow showed better µTBS than the Fusio Liquid Dentin with the exception of the Er:YAG MSP mode surface treatment (p < 0.05). Etching with phosphoric acid and the Er:YAG QSP mode surface treatment significantly increased the µTBS of Vertise Flow (p < 0.05). Etching with phosphoric acid, Er:YAG QSP mode surface treatment, and Er:YAG MSP mode surface treatment significantly increased the µTBS of Fusio Liquid Dentin (p < 0.05). The µTBS of Vertise Flow was higher than the µTBS of Fusio Liquid Dentin. The Er:YAG laser irradiation increased the µTBS of both self-adhesive flowable resin composites in two pulse modes tested (MSP or QSP). However, the highest µTBS was recorded when Vertise Flow was applied with acid etching.

PURPOSE: Commercial fiber-reinforced dowel systems are marketed as having better adhesion and sealing ability than conventional metallic dowel systems. The aim of this in vitro study was to evaluate the microleakage of teeth restored with nine dowel systems. MATERIALS AND METHODS: Ninety mandibular second premolar teeth were decoronated, and nine homogenous groups were composed of ten teeth each. Root canal and dowel space preparations were made, and eight fiber-reinforced composite dowel systems and one stainless steel dowel system were used to fabricate dowel restorations. Microleakage measurements of the restored teeth were made with a modified fluid filtration method, and data were collected. One sample Kolmogorov-Smirnov, one-way ANOVA, and Tukey-HSD tests were performed on the relative microleakage data of the groups. RESULTS: Microleakage values were reported relative to those for teeth with unfilled canals. The highest and lowest relative microleakage values were recorded for the metallic Parapost (7.06 × 10(-4) %) and fiber-reinforced Everstick (3.55 × 10(-4) %) groups, respectively. Significant differences in relative microleakage between the fiber-reinforced dowels and stainless steel dowels were observed. Significant differences among the fiber-reinforced dowel groups were observed as well. CONCLUSIONS: The sealing ability of all fiber-reinforced composite dowels is not better than that of stainless steel dowels, and there are significant differences among different fiber-reinforced dowel systems as well. Differences among commercial dowel systems must be taken into consideration when making a selection.

The neodymium/yttrium-aluminum-garnet (Nd/YAG) laser has been suggested to repair broken prostheses in the mouth. This study investigated the effects of different dentin thicknesses and air cooling on pulpal temperature rise during laser welding. Three intact human maxillary molars were prepared for full-veneer crown. For each tooth, dentin thicknesses in mesiobuccal cusp was 2, 3, or 4 mm. Twenty dies were duplicated from each of the prepared teeth. For metal copings with 0.5-mm thickness, wax patterns were prepared with dip wax technique directly onto each of dies. All patterns were sprued and invested. The castings were made using a nickel-chromium alloy (Nicromed Premium, Neodontics). A hole with 0.5-mm diameter was prepared on the mesiobuccal cusp of each crown. The Nd/YAG laser (9.85 W; 1 Hz repetition rate; fluence, 1.230 J/cm(2); Fidelis Plus 3, Fotona) was used for welding with or without air cooling (n = 10). The temperature rise was measured in pulpal chamber with a J-type thermocouple wire that was connected to a data logger. Differences between start and highest temperature reading were taken, and temperature rise values were compared using two-way analysis of variance and Tukey's honestly significant difference tests (α = .05). Pulpal temperature rise varied significantly depending on the dentin thickness and air cooling (p < 0.05). The non-air cooling group induced significantly the highest temperature increases. There were no significant differences between 2- and 3-mm dentin thicknesses groups (p > 0.05); however, pulpal temperature rise was the lowest for 4-mm dentin thickness group (p < 0.05). The highest values of thermal increase were found in the pulp chamber (6.8°C) when no air cooling was used in 2-mm dentin thickness group. Laser welding on base metal castings with Nd/YAG laser can be applied with air cooling to avoid temperature rises known to adversely affect pulpal health when dentin thickness is 2 or 3 mm.

Zirconia-based ceramics offer strong restorations in dentistry, but the adhesive bond strength of resin cements to such ceramics is not optimal. This study evaluated the influence of surface treatments on the bond strength of resin cement to yttrium-stabilized tetragonal zirconia (Y-TZP) ceramic. Seventy-five plates of Y-TZP ceramic were randomly assigned to five groups (n = 15) according to the surface treatments [airborne particle abrasion, neodymium-doped yttrium aluminum garnet (Nd:YAG) laser irradiation (Fidelis Plus 3, Fotona; 2 W, 200 mJ, 10 Hz, with two different pulse durations 180 or 320 µs), glaze applied, and then 9.5 % hydrofluoric acid gel conditioned, control]. One specimen from each group was randomly selected, and specimens were evaluated with x-ray diffraction and SEM analysis. The resin cement (Clearfil Esthetic Cement, Kuraray) was adhered onto the zirconia surfaces with its corresponding adhesive components. Shear bond strength of each sample was measured using a universal testing machine at a crosshead speed of 1 mm/min. Bond strengths were analyzed through one-way ANOVA/Tukey tests. Surface treatments significantly modified the topography of the Y-TZP ceramic. The Nd:YAG laser-irradiated specimens resulted in both increased surface roughness and bond strength of the resin cement. The highest surface roughness and bond strength values were achieved with short pulse duration. Nd:YAG laser irradiation increased both surface roughness of Y-TZP surfaces and bond strength of resin cement to the zirconia surface.

OBJECTIVE: Fiber-reinforced composite dowels are suggested to be a better alternative to metal dowels. This in vitro study evaluated the fracture resistance and fracture modes of teeth restored with nine different dowel systems. MATERIALS AND METHODS: Ninety mandibular pre-molar teeth were decoronated and nine homogenous groups were composed. Root canal and dowel canal preparations were made and nine different dowel systems were used to fabricate restorations. Core build-ups were made with a composite resin core material. Specimens were mounted in acrylic resin blocks and continuous compressive force was applied until fracture occurred. Fracture resistance and fracture mode data were collected. One-sample Kolmogorov-Smirnov and one-way ANOVA tests were performed for the fracture resistance data of the groups. RESULTS: There were no significant differences among the fracture resistances of the groups. All specimens of the pre-fabricated stainless steel dowel group fractured catastrophically. However, even in the worst-case, five specimens of the fiber-reinforced composite groups had favorable fracture modes. CONCLUSIONS: The teeth restored with fiber reinforced composite dowels were as resistant to fracture as teeth restored with stainless steel dowels. Fracture modes of teeth restored with fiber reinforced composite dowels were more advantageous than teeth restored with pre-fabricated stainless steel dowels.

The aim of this study was to evaluate the intrapulpal temperature changes during the curing of different bulk-fill restorative materials. Ten mandibular molar teeth were selected and occlusal surfaces were removed to obtain a standard 0.5 mm occlusal dentin thickness. Five bulk-fill restorative materials and a conventional resin composite (control) were applied. The intrapulpal temperature changes during the curing of these materials were determined by a device simulating pulpal blood microcirculation. The difference between the initial and maximum temperature values (Δt), was recorded. The data were statistically analyzed with one-way ANOVA and Tukey's HSD test (p<0.05). There were statistically significant differences between materials (p<0.001). The light-curing bulkfill restoratives exhibited the highest Δt values. Equia Forte showed the lowest Δt values among all the groups (p<0.05). Bulk-fill restorative materials causes significantly different temperature changes in the pulp chamber according to curing type. Therefore, clinicians should be considered when using these materials.

This study evaluated the fracture resistance of endodontically treated teeth restored with different base materials and mesioocclusal-distal (MOD) ceramic inlays. Fifty mandibular molars were assigned into five groups (n=10 per group). Group1 (control) comprised intact molar teeth without any treatment. Teeth in other groups were subjected to root canal treatment and restored with MOD ceramic inlays on different base materials. In Group 2, base material was zinc phosphate cement; Group 3's was glass ionomer cement; Group 4's was composite resin, and Group 5's was composite resin reinforced with fiber. Finally, a continuous occlusal load was applied until fracture occurred. Mean fracture resistance of Group 1 (3,027 N) was significantly higher than the other groups (890, 1,070, 1,670, 1,226 N respectively). Fracture resistance of Group 4 was statistically comparable with Group 5 and significantly higher than Groups 2 and 3 (p<0.05; Tukey's HSD). Use of different base materials under ceramic inlay restorations could affect the fracture resistance of endodontically treated teeth.

OBJECTIVE: To evaluate and compare intrapulpal temperature rise with three different light-curing units by using a study model simulating pulpal blood microcirculation. MATERIALS AND METHODS: The roots of 10 extracted intact maxillary central incisors were separated approximately 2 mm below the cement-enamel junction. The crowns of these teeth were fixed on an apparatus for the simulation of blood microcirculation in pulp. A J-type thermocouple wire was inserted into the pulp chamber through a drilled access on the palatal surfaces of the teeth. Four measurements were made using each tooth for four different modes: group 1, 1000 mW/cm(2) for 15 seconds; group 2, 1200 mW/cm(2) for 10 seconds; group 3, 1400 mW/cm(2) for 8 seconds; and group 4, 3200 mW/cm(2) for 3 seconds. The tip of the light source was positioned at 2 mm to the incisor's labial surface. RESULTS: The highest temperature rise was recorded in group 1 (2.6°C ± 0.54°C), followed by group 2 (2.57°C ± 0.62°C) and group 3 (2.35°C ± 0.61°C). The lowest temperature rise value was found in group 4 (1.74°C ± 0.52°C); this value represented significantly lower ΔT values when compared to group 1 and group 2 (P  =  .01 and P  =  .013, respectively). CONCLUSIONS: The lowest intrapulpal temperature rise was induced by 3200 mW/cm(2) for 3 seconds of irradiation. Despite the significant differences among the groups, the temperature increases recorded for all groups were below the critical value of 5.6°C.

OBJECTIVE: The aim of this study was to determine the erbium-doped yttrium aluminum garnet (Er:YAG) laser transmission ratio through different dental ceramics with different thicknesses. BACKGROUND DATA: Laser debonding procedure of adhesively luted all-ceramic restorations is based on the transmission of laser energy through the ceramic and the ablation of resin cement, because of the transmitted laser energy. METHODS: Five different dental ceramics were evaluated in this study: sintered zirconium-oxide core ceramic, monolithic zirconium-oxide ceramic, feldspathic ceramic, leucite-reinforced glass ceramic, and lithium disilicate-reinforced glass ceramic. Two ceramic discs with different thicknesses (0.5 and 1 mm) were fabricated for each group. Ceramic discs were placed between the sensor membrane of the laser power meter and the tip of the contact handpiece of an Er:YAG laser device with the aid of a custom- made acrylic holder. The transmission ratio of Er:YAG laser energy (500 mJ, 2 Hz, 1 W, 1000 µs) through different ceramic discs was measured with the power meter. Ten measurements were made for each group and the results were analyzed with two way analysis of variance (ANOVA) and Tukey honestly significant difference (HSD) tests. RESULTS: The highest transmission ratio was determined for lithium disilicate-reinforced ceramic with 0.5 mm thickness (88%) and the lowest was determined for feldspathic ceramic with 1 mm thickness (44%). The differences among the different ceramics and between the different thicknesses were significant (p<0.05). CONCLUSIONS: Ceramic type and thickness should be taken into consideration to adjust the laser irradiation parameters during laser debonding of adhesively luted all-ceramic restorations.

The aim of this study was to evaluate temperature changes in the pulp chamber during polymerization of four different pulp-capping materials using a LED-light-curing-unit in the contact and noncontact positions. A pulpal circulation mechanism was simulated to measure increases in temperature in four pulp-capping materials that were applied to the occlusal dentin surface. Two different distances were used between the tip of the unit and the material surface during polymerization; 0 and 2 mm. The data were statistically analyzed using factorial-ANOVA, one-way-ANOVA, and Tukey's HSD test. There were statistically differences between contact and noncontact groups (p<0.05). Contact groups produced significantly lower temperature rise compared with noncontact groups. The highest and lowest mean temperature increases were calculated for the Biner LC and the TheraCal LC, respectively. Considering the effects of temperature rising, the distance of the light curing units from restorations should not be overlooked as well as the types of the materials.

PURPOSE: Translucency of all-ceramic restorations is an important factor which affects the final appearance and esthetic outcome of the restoration. The aim of this study was to evaluate the effect of the shade of coloring liquid on the translucency of zirconia framework. MATERIALS AND METHODS: Thirty zirconium oxide core plate (15 × 12 × 0.5 mm) were divided into 6 groups of 5 plates each. Each group was classified according to the shade of coloring liquid based on Vita Classic Scale (A2, A3, B1, C2, and D2), and each sample was immersed in coloring liquid for 3 seconds as recommended by the manufacturer, except for the control group. Contrast ratio, as a translucency parameter, was calculated using a spectrophotometer and the data were analyzed with oneway analysis of variance (ANOVA) and Tukey's honestly significant differences (HSD) tests (α=.05). RESULTS: Significant differences in translucency among the control and test groups, and the B1 shaded group and other shades was observed. There were no significant differences among A2, A3, C2, and D2 shaded groups. CONCLUSION: The translucency of the zirconium oxide cores was affected by the coloring procedure and significant differences in the translucency measurements were identified between specific shades.

OBJECTIVE: This study aimed to evaluate the antimicrobial efficiency of PDT and the effect of different irradiation durations on the antimicrobial efficiency of PDT. MATERIALS AND METHODS: Sixty freshly extracted human teeth with a single root were decoronated and distributed into five groups. The control group received no treatment. Group 1 was treated with a 5% sodium hypochlorite (NaOCl) solution. Groups 2, 3, and 4 were treated with methylene-blue photosensitizer and 660-nm diode laser irradiation for 1, 2, and 4 min, respectively. The root canals were instrumented and irrigated with NaOCl, ethylenediamine-tetraacetic acid, and a saline solution, followed by autoclaving. All the roots were inoculated with an Enterococcus faecalis suspension and brain heart infusion broth and stored for 21 days to allow biofilm formation. Microbiological data on microorganism load were collected before and after the disinfection procedures and analyzed with the Wilcoxon ranged test, the Kruskal-Wallis test, and the Dunn's test. RESULTS: The microorganism load in the control group increased. The lowest reduction in the microorganism load was observed in the 1-min irradiation group (Group 2 = 99.8%), which was very close to the results of the other experimental groups (99.9%). There were no significant differences among the groups. CONCLUSIONS: PDT is as effective as conventional 5% NaOCl irrigation with regard to antimicrobial efficiency against Enterococcus faecalis.

RESUMEN

This clinical report describes the prosthodontic treatment for an 18-year-old man diagnosed with amelogenesis imperfecta. The aim of treatment was to reduce dental sensitivity and to restore esthetics and masticatory function. Metal-ceramic fixed partial dentures were placed on posterior teeth to modify the occlusion, and porcelain laminate veneers were placed to improve the esthetics of the maxillary anterior teeth. Clinical examination 12 months after treatment revealed no evidence of disorders associated with the restored teeth or their supporting structures.
