RESUMO

PURPOSE: To report the clinical results obtained with fixed short-span (single crowns [SCs] and fixed partial prostheses [FPPs]) implant-supported hybrid composite restorations fabricated through tilting stereolithography (TSLA). METHODS: This retrospective clinical study included 85 patients who had been restored with 95 fixed short-span implant-supported hybrid composite (Irix Max®, DWS Systems) restorations (70 SCs and 25 FPPs up to three units) fabricated with TSLA. The full-digital model-free workflow was based on intraoral implant scanning, computer-assisted design (CAD) and 3D printing using TSLA (Dfab®, DWS Systems). The primary outcomes were the marginal adaptation, the quality of the occlusal and interproximal contact points, and the chromatic integration of the restorations, assessed independently by two experienced operators (a prosthodontist and a periodontist). A score from 1 to 5 (with 5 as the highest value, 4 for satisfactory quality, 3 for acceptable quality, and 2 and 1 as the lowest values, expressing unsatisfactory quality) was assigned by each operator to each restoration at delivery. The secondary outcomes were the survival and success of the restorations at the 1-year follow-up. The restoration was defined as successful in the absence of any complications throughout the follow-up period. A statistical analysis was conducted. RESULTS: For the quality of the marginal closure and occlusal and interproximal contact points, the 3D-printed hybrid composite restorations scored highly; the aesthetic integration was satisfactory. One year after placement, all restorations survived, with a low incidence (4.2% overall, 5.7% SCs) of complications (two abutment screw loosenings, two decementation of the restorations, and one upper portion of the hybrid abutment decemented from the titanium base), for a success rate of 95.8%. CONCLUSIONS: Within the limits of this study (retrospective design, follow-up limited to 1 year from the delivery, and only cemented restorations included) fixed short-span implant-supported hybrid composite crowns and bridges fabricated through TSLA were clinically precise, presenting a low incidence of complications at 1 year. STATEMENT OF CLINICAL RELEVANCE: The use of TSLA printing technology can open new perspectives for the treatment of small edentulous gaps with definitive implant-supported prosthetic restorations.

RESUMO

PURPOSE: To evaluate the trueness, precision, time efficiency, and cost of three different workflows for manufacturing single crowns (SCs). METHODS: A plaster model with a prepared tooth (#15) was scanned with an industrial scanner, and an SC was designed in computer-assisted-design (CAD) software. Ten SCs were printed with a hybrid composite (additive chairside) and a stereolithographic (SLA) printer (Dfab®), 10 SCs were milled in lithium disilicate (subtractive chairside) using a chairside milling unit (inLab MC XL®), and 10 SCs were milled in zirconia (lab-based) using a five-axis laboratory machine (DWX-52D®). All SCs were scanned with the same scanner after polymerization/sinterization. Each scan was superimposed to the marginal area of the original CAD file to evaluate trueness: absolute average (ABS AVG), root mean square (RMS), and (90˚-10˚)/2 percentile were calculated for each group. Marginal adaptation and quality of the occlusal and interproximal contact points were also investigated by two prosthodontists on 3D printed and plaster models. Finally, the three workflows' time efficiency and costs were evaluated. RESULTS: Additive chairside and subtractive lab-based SCs had significantly better marginal trueness than subtractive chairside SCs in all three parameters (ABS AVG, p < 0.01; RMS, p < 0.01; [90˚-10˚]/2, p < 0.01). However, the two prosthodontists found no significant differences between the three manufacturing procedures in the quality of the marginal closure (p = 0.186), interproximal (p = 0.319), and occlusal contacts (p = 0.218). Both time efficiency and cost show a trend favoring the chairside additive workflow. CONCLUSIONS: Chairside additive technology seems to represent a valid alternative for manufacturing definitive SCs, given the high marginal trueness, precision, workflow efficiency and low costs. STATEMENT OF CLINICAL RELEVANCE: Additive chairside manufacturing of definitive hybrid composite SCs is now possible and shows high accuracy, time efficiency, and competitive cost.

PURPOSE: To present a novel protocol for authentic three-dimensional (3D) planning of dental implants, using artificial intelligence (AI) and augmented reality (AR). METHODS: The novel protocol consists of (1) 3D data acquisition, with an intraoral scanner (IOS) and cone-beam computed tomography (CBCT); (2) application of AI for CBCT segmentation to obtain standard tessellation language (STL) models and automatic alignment with IOS models; (3) loading of selected STL models within the AR system and surgical planning with holograms; (4) surgical guide design with open-source computer-assisted-design (CAD) software; and (5) surgery on the patient. RESULTS: This novel protocol is effective and time-efficient when used for planning simple cases of static guided implant surgery in the partially edentulous patient. The clinician can plan the implants in an authentic 3D environment, without using any radiological guided surgery software. The precision of implant placement looks clinically acceptable, with minor deviations. CONCLUSIONS: AI and AR technologies can be successfully used in guided implant surgery for authentic 3D planning that may replace conventional software. However, further clinical studies are needed to validate this protocol. STATEMENT OF CLINICAL RELEVANCE: The combined use of AI and AR may change the perspectives of modern guided implant surgery for authentic 3D planning that may replace conventional software.

BACKGROUND: The risk of periodontal diseases development increased in patients with malocclusion undergoing orthodontic treatment. Thus, the aim of this study was to examine the efficacy of the use of adjunctive rinses with nano-Argentum to standard oral hygiene regimen in subjects wearing fixed orthodontic appliances. MATERIALS AND METHODS: Eighty patients were observed for 1 year. They were divided in two groups: in test group patients were instructed to rinse with non-ionic colloidal silver solution according to protocol for 6 months as an adjunct to standard; in control group the patients followed the standard oral hygiene regimen. Index of efficiency of oral hygiene (PHPm), community periodontal index (CPI) and papillary-marginal-alveolar index (PMA) were evaluated before treatment and after 1 and 6 months. For statistics analysis, Mann-Whitney, Kruskal-Wallis tests and Pearson criterion were used. RESULTS: Baseline hygiene levels in two groups had no differences. Oral hygiene indices were significantly lower in the test group in comparison with control after 1 month (PHPm = 0.38 ± 0.18 and 1.19 ± 0.45, respectively, p < 0.01; PMA = 11.78 ± 8.5 and 47.25 ± 20.9, respectively, p < 0.05; CPI = 0.65 ± 0.53 and 1.53 ± 0.77, respectively, p < 0.01) and 6 months (PHPm = 0.5 ± 0.2 and 1.2 ± 0.4, respectively, p < 0.01; PMA = 11.62 ± 19.6 and 66.33 ± 27.9, respectively, p < 0.01; CPI = 0.63 ± 0.73 and 1.68 ± 0.78, respectively, p < 0.01). CONCLUSIONS: The use of the test solution as an adjunct to standard oral hygiene provided a significant beneficial effect in terms of oral hygiene in patients undergoing orthodontic treatment.

PURPOSE: To report on the results obtained with computer-aided-design/ computer-assisted-manufacturing (CAD/CAM) custom-made synthetic hydroxyapatite/beta-tricalcium-phosphate (HA/beta-TCP) bone grafts in alveolar ridge augmentation for dental implant placement. METHODS: The procedure included: (1) cone-beam computed tomography (CBCT) of the bone defect; (2) virtual design of the custom-made onlay bone grafts; (3) milling of grafts from a pre-formed block of synthetic HA/beta-TCP; and (4) bone reconstructive surgery. Implants were placed 8 months later. The patients were followed for 3 years. The study outcomes were: (1) intra- and immediate post-operative complications; (2) 8-month vertical and horizontal bone gain; (3) implant survival; (4) implant-crown success; and (5) peri-implant marginal bone loss (MBL). RESULTS: Twenty-six patients underwent ridge augmentation with custom-made CAD/CAM HA/beta-TCP onlay grafts. Eight months later, these patients were rehabilitated with dental implants. During surgery, 25/26 (96.1%) of the grafts adapted well to the bone defect. Immediate post-operative complications were pain and swelling (2/26 patients: 7.6%), and bone graft exposure (3/26: 11.5%); one exposure led to infection, removal of the graft, and failure of the procedure. Excellent integration of the other grafts was observed 8 months after the regenerative procedure, with mean vertical and horizontal bone gains of 2.10 mm (± 0.35) and 2.96 mm (± 0.45), respectively. Twenty-five implants were placed and restored with single crowns. Three years later, all implants were in function. The 3-year implant crown success rate and peri-implant MBL were 92.0% and 0.7 mm (±0.19), respectively. CONCLUSIONS: With custom-made CAD/CAM synthetic HA/beta-TCP onlay grafts reconstruction of small vertical and/or horizontal defects of the alveolar ridge was obtained; this enabled implant placement, with high implant-crown success rate after 3 years. Further studies are needed to validate this technique. STATEMENT OF CLINICAL RELEVANCE: Custom-made CAD/CAM synthetic HA/beta-TCP onlay grafts may represent an option for regeneration of small bone defects prior to implant placement.

Background: Pediatric dentists face difficulties in treating tooth decay of children with difficulty in opening the mouth. This is especially true, as the main disease is accompanied by such symptoms as arthritis, osteoarthritis, myositis, myalgia, sclerosis, and oral atrophy. Aim: The aim was to increase the level of treatment of fissure tooth decay for children with rheumatic diseases, using the silicone key method (SKM). Materials and Methods: A dental examination of 82 children was carried out, with diagnoses: systemic lupus erythematosus, juvenile systemic scleroderma, juvenile dermatomyositis, and juvenile rheumatoid arthritis. Evaluation of the intensity of caries using indices: decayed, missing, and filled teeth (DMFT)(average). Assessment and measurement of the degree of mouth opening (MMO) by Sh. M. Batashvili. The SKM was used to treat fissure tooth decay. Results: During the dental examination of the oral cavity of all children with RD, multiple caries lesions were found. In the first age group of index indicators (DMFT) (on average) - 4.88, in the second age group DMFT (on average) - 7.58 - these values corresponded to high scores for the degree of dental caries, which requires frequent appointment to the dentist. Oral hygiene of children with difficult opening of the mouth is unsatisfactory, which leads to a rapid increase of caries. Restriction of moth opening <4 mm was found in 57 children with RD (69, 51%) that made it difficult to treat caries. Conclusion: Using SKM will improve the quality of caries treatment and provide a stable result of treatment in patients with difficulty opening the mouth in severe somatic pathologies.

PURPOSE: Additive manufacturing (AM) in prosthodontics is used as an alternative to casting or milling. Various techniques and materials are available for the additive manufacturing of the fixed and removable tooth-supported restorations, but there is a lack of evidence on the accuracy of AM fixed implant-supported prostheses. Recent studies investigated the accuracy of ceramic AM prostheses. Therefore, the aim of this systematic review was to evaluate the accuracy of additively manufactured metal, ceramic or polymers, and screw- or cement-retained fixed implant-supported prostheses. MATERIALS AND METHODS: Two calibrated investigators performed an electronic search of relevant publications in the English language following selected PICOS criteria and using a well-defined search strategy (latest search date-1st of June, 2021). Based on the exclusion criteria (no control group, less than five samples per group, 3D printing of the implant abutment part, only subjective evaluation of accuracy, etc.) studies were not included in the review. Quantitative data of accuracy evaluation such as marginal gap, strain analysis, and linear measurements was extracted and interpreted. QUADAS-2 tool was used to assess the risk of methodological bias of all included studies. RESULTS: Sixteen in vitro studies were selected for the final analysis. Six of the selected studies evaluated screw-retained restorations and 10 cement-retained implant-supported restorations. Only 4 publications concluded that AM restorations were more accurate than conventionally made (cast or milled) ones. The most common finding was that AM restorations were more accurate than cast and demonstrated less or similar accuracy compared to milled ones (n = 10 studies). Detected marginal discrepancies mean values of the AM prosthesis varied from 23 to more than 200 µm, but most of them were categorized as clinically acceptable. CONCLUSIONS: AM implant-supported fixed prostheses demonstrate similar accuracy compared to conventional and computer-aided design and computer-aided manufacturing techniques in vitro. Detected inaccuracies of AM restorations do not exceed clinically acceptable limits. Clinical studies with longer follow-up periods are needed to show the reliability of AM prostheses.

OBJECTIVES: To present a solid index (SI), a 3D-printed replica of a patient's preexisting complete denture, which allows the dentist to accurately capture the position of the implants and the dental technician to model a bar for overdenture (OD) in CAD software with the use of reverse implant libraries. MATERIALS AND METHODS: A patient in need of rehabilitation of a fully edentulous mandible was restored with an implant OD supported by a polyetheretherketone (PEEK) milled bar. The position of the implants was captured through a physical impression using an SI, with the transfers screwed in. The analogs were then connected, and the SI was scanned upside down to directly capture the position of the implants in the space. This scan was used to design the bar in CAD software using reverse implant libraries, without the need for pouring any SI-derived cast or for using any scanbody. The bar was milled in PEEK. RESULTS: When delivered to the patient, the bar was clinically precise, screwing perfectly onto the implants without any tension or misfit. CONCLUSIONS: The present proof-of-concept article supports the use of an SI and reverse implant libraries for the fabrication of a bar for OD. Further clinical studies are needed to confirm these results.

PURPOSE: To compare the trueness and precision of 3D-printed versus milled monolithic zirconia crowns (MZCs). METHODS: A model of a maxilla with a prepared premolar was scanned with an industrial scanner (ATOSQ®, Gom) and an MZC was designed in computer-assisted-design (CAD) software (DentalCad®, Exocad). From that standard tessellation language (STL) file, 10 MZCs (test) were 3D-printed with a Lithography-based Ceramic Manufacturing (LCM) printer (CerafabS65®, Lithoz) and 10 MZCs (control) were milled using a 5-axis machine (DWX-52D®, DGShape). All MZCs were sintered and scanned with the aforementioned scanner. The surface data of each sample (overall crown, marginal area, occlusal surface) were superimposed to the original CAD file (ControlX®, Geomagic) to evaluate trueness: (90-10)/2, absolute average (ABS AVG) and root mean square (RMS) values were obtained for test and control groups (MathLab®, Mathworks) and used for analysis. Finally, the clinical precision (marginal adaptation, interproximal contacts) of test and control MZCs was investigated on a split-cast model printed (Solflex350®, Voco) from the CAD project, and compared. RESULTS: The milled MZCs had a significantly higher trueness than the 3D-printed ones, overall [(90-10)/2 printed 37.8 µm vs milled 21.2 µm; ABS AVG printed 27.2 µm vs milled 15.1 µm; RMS printed 33.2 µm vs milled 20.5 µm; p = 0.000005], at the margins [(90-10)/2 printed 25.6 µm vs milled 12.4 µm; ABS AVG printed 17.8 µm vs milled 9.4 µm; RMS printed 22.8 µm vs milled 15.6 µm; p= 0.000011] and at the occlusal level [(90-10)/2 printed 50.4 µm vs milled 21.9 µm; ABS AVG printed 29.6 µm vs milled 14.7 µm; RMS printed 38.9 µm vs milled 22.5 µm; p = 0.000005]. However, with regard to precision, both test and control groups scored highly, with no significant difference either in the quality of interproximal contact points (p = 0.355) or marginal closure (p = 0.355). CONCLUSIONS: Milled MZCs had a statistically higher trueness than 3D-printed ones; all crowns, however, showed high precision, compatible with the clinical use. CLINICAL SIGNIFICANCE: Although milled MZCs remain more accurate than 3D-printed ones, the LCM technique seems able to guarantee the production of clinically precise zirconia crowns.

PURPOSE: To investigate and compare the production tolerances of six different commercially available implant scan bodies (SBs), with the null hypothesis that there would be no tolerances in the production or significant differences between the different SBs. MATERIALS AND METHODS: Six different implant SBs (IO 6A-B and IO 2B-B, Nobel Biocare; RC 4.1 mm 025.4915 and RN 4.8 mm 048.168, Straumann; KR 352KR1A0, BTK BIOTEC; and AANISR4013T, MegaGen) were evaluated. Five specimens of each SB type (a total of 30 samples) were screwed onto the corresponding implant analogs and underwent dimensional analysis with optical microscopy (QVI Smartscope Flash 200, Optical Gaging Products) and precision probing (R 0.25, Renishaw). The outcome variables were SB height, diameter, and angle of the flat face on the top (plane). All measurements were compared with the corresponding computer-assisted design library measurements used as a reference to assess the manufacturing tolerances. Statistical analyses were performed to compare the results obtained with the different SBs. RESULTS: Tolerances in the manufacture of the SBs were reported in height, diameter, and plane measurements, and statistically significant differences between the different types of SBs were found. Therefore, the null hypothesis was rejected. Most of the deviations and tolerances were reported in height measurements with conical connection implants. CONCLUSIONS: Tolerances in the production and statistically significant differences were found among the six commercially available SBs evaluated in this study. Additional studies with larger sample sizes and other types of SBs are needed.

BACKGROUND: The literature has not yet validated the use of intraoral scanners (IOSs) for full-arch (FA) implant impression. Hence, the aim of this in vitro study was to assess and compare the trueness of 12 different IOSs in FA implant impression. METHODS: A stone-cast model of a totally edentulous maxilla with 6 implant analogues and scanbodies (SBs) was scanned with a desktop scanner (Freedom UHD®) to capture a reference model (RM), and with 12 IOSs (ITERO ELEMENTS 5D®; PRIMESCAN® and OMNICAM®; CS 3700® and CS 3600®; TRIOS3®; i-500®; EMERALD S® and EMERALD®; VIRTUO VIVO® and DWIO®; RUNEYES QUICKSCAN®). Ten scans were taken using each IOS, and each was compared to the RM, to evaluate trueness. A mesh/mesh method and a nurbs/nurbs method were used to evaluate the overall trueness of the scans; linear and cross distances between the SBs were used to evaluate the local trueness of the scans. The analysis was performed using reverse engineering software (Studio®, Geomagics; Magics®, Materialise). A statistical evaluation was performed. RESULTS: With the mesh/mesh method, the best results were obtained by CS 3700® (mean error 30.4 µm) followed by ITERO ELEMENTS 5D® (31.4 µm), i-500® (32.2 µm), TRIOS 3® (36.4 µm), CS 3600® (36.5 µm), PRIMESCAN® (38.4 µm), VIRTUO VIVO® (43.8 µm), RUNEYES® (44.4 µm), EMERALD S® (52.9 µm), EMERALD® (76.1 µm), OMNICAM® (79.6 µm) and DWIO® (98.4 µm). With the nurbs/nurbs method, the best results were obtained by ITERO ELEMENTS 5D® (mean error 16.1 µm), followed by PRIMESCAN® (19.3 µm), TRIOS 3® (20.2 µm), i-500® (20.8 µm), CS 3700® (21.9 µm), CS 3600® (24.4 µm), VIRTUO VIVO® (32.0 µm), RUNEYES® (33.9 µm), EMERALD S® (36.8 µm), OMNICAM® (47.0 µm), EMERALD® (51.9 µm) and DWIO® (69.9 µm). Statistically significant differences were found between the IOSs. Linear and cross distances between the SBs (local trueness analysis) confirmed the data that emerged from the overall trueness evaluation. CONCLUSIONS: Different levels of trueness were found among the IOSs evaluated in this study. Further studies are needed to confirm these results.

PURPOSE: To compare the reliability of five different intraoral scanners (IOSs) in the capture of implant scanbodies (SBs) and to verify the dimensional congruence between the meshes (MEs) of the SBs and the corresponding library file (LF). METHODS: A gypsum cast of a fully edentulous maxilla with six implant analogues and SBs screwed on was scanned with five different IOSs (PRIMESCAN®, CS 3700®, MEDIT i-500®, ITERO ELEMENTS 5D®, and Emerald S®). Ten scans were taken for each IOS. The resulting MEs were imported to reverse engineering software for 3D analysis, consisting of the superimposition of the SB LF onto each SB ME. Then, a quantitative and qualitative evaluation of the deviations between MEs and LF was performed. A careful statistical analysis was performed. RESULTS: PRIMESCAN® showed the highest congruence between SB MEs and LF, with the lowest mean absolute deviation (25.5 ± 5.0 µm), immediately followed by CS 3700® (27.0 ± 4.3 µm); the difference between them was not significant (p = 0.1235). PRIMESCAN® showed a significantly higher congruence than MEDIT i-500® (29.8 ± 4.8 µm, p < 0.0001), ITERO ELEMENTS 5D® (34.2 ± 9.3 µm, p < 0.0001), and Emerald S® (38.3 ± 7.8 µm, p < 0.0001). CS 3700® had a significantly higher congruence than MEDIT i-500® (p = 0.0004), ITERO ELEMENTS 5D® (p < 0.0001), and Emerald S® (p < 0.0001). Significant differences were also found between MEDIT i-500® and ITERO ELEMENTS 5D® (p < 0.0001), MEDIT i-500® and Emerald S® (p < 0.0001), and ITERO ELEMENTS 5D® and Emerald S® (p < 0.0001). Significant differences were found among different SBs when scanned with the same IOS. The deviations of the IOSs showed different directions and patterns. With PRIMESCAN®, ITERO ELEMENTS 5D®, and Emerald S®, the MEs were included inside the LF; with CS 3700®, the LF was included in the MEs. MEDIT i-500® showed interpolation between the MEs and LF, with no clear direction for the deviation. CONCLUSIONS: Statistically different levels of congruence were found between the SB MEs and the corresponding LF when using different IOSs. Significant differences were also found between different SBs when scanned with the same IOS. Finally, the qualitative evaluation revealed different directions and patterns for the five IOSs.

BACKGROUND: Artificial intelligence (AI) is a branch of computer science concerned with building smart software or machines capable of performing tasks that typically require human intelligence. We present a protocol for the use of AI to fabricate implant-supported monolithic zirconia crowns (MZCs) cemented on customized hybrid abutments. METHODS: The study protocol consisted of: (1) intraoral scan of the implant position; (2) design of the individual abutment and temporary crown using computer-aided design (CAD) software; (3) milling of the zirconia abutment and the temporary polymethyl-methacrylate (PMMA) crown, with extraoral cementation of the zirconia abutment on the relative titanium bonding base, to generate an individual hybrid abutment; (4) clinical application of the hybrid abutment and the temporary PMMA crown; (5) intraoral scan of the hybrid abutment; (6) CAD of the final crown with automated margin line design using AI; (7) milling, sintering and characterisation of the final MZC; and (8) clinical application of the MZC. The outcome variables were mathematical (quality of the fabrication of the individual zirconia abutment) and clinical, such as (1) quality of the marginal adaptation, (2) of interproximal contact points and (3) of occlusal contacts, (4) chromatic integration, (5) survival and (6) success of MZCs. A careful statistical analysis was performed. RESULTS: 90 patients (35 males, 55 females; mean age 53.3 ± 13.7 years) restored with 106 implant-supported MZCs were included in the study. The follow-up varied from 6 months to 3 years. The quality of the fabrication of individual hybrid abutments revealed a mean deviation of 44 µm (± 6.3) between the original CAD design of the zirconia abutment, and the mesh of the zirconia abutment captured intraorally at the end of the provisionalization. At the delivery of the MZCs, the marginal adaptation, quality of interproximal and occlusal contacts, and aesthetic integration were excellent. The three-year cumulative survival and success of the MZCs were 99.0% and 91.3%, respectively. CONCLUSIONS: AI seems to represent a reliable tool for the restoration of single implants with MZCs cemented on customised hybrid abutments via a full digital workflow. Further studies are needed to confirm these positive results.

Purpose: To present an experimental strategy for successfully capturing the margins of prepared single teeth with an intraoral scanner (IOS). Methods: The protocol was as follows: (1) an intraoral impression was captured with an IOS, without taking care of the visibility of the margins; (2) a partial analog impression was taken by means of a 3D-printed custom tray filled with polyvinylsiloxane light, after the removal of a retraction cord; (3) the hollow portion of the analog impression, with the preparation margins clearly visible, was scanned extraorally with the same IOS; (4) the scan of the analog impression was imported into computer-assisted-design (CAD) software, where its normals were inverted; (5) the scan with inverted normals was registered on the first intraoral scan, and replaced it; (6) the technician designed the final restoration, which was fabricated and delivered for application. The study outcomes were: (1) the marginal adaptation of the final crown; (2) the quality of interproximal contacts; and (3) the quality of occlusal contacts. Results: Thirty patients (18 males, 12 females; mean age 51.3 ± 11.6 years) were selected for this study. All these patients were restored with a monolithic translucent zirconia crown, fabricated following the aforementioned protocol. The clinical precision and the marginal adaptation of the crowns were optimal, interproximal contact points were perfect, and the only necessary adaptations were occlusal, with some minor precontacts that had to be polished. Conclusions: The present protocol seems to be compatible with the fabrication of clinically precise zirconia crowns. Further studies are needed to confirm these results.

PURPOSE: To present the application of custom-made 3D-printed subperiosteal implants for fixed prosthetic restoration of the atrophic posterior mandible of elderly patients. METHODS: Between January 2017 and June 2018, all partially edentulous patients aged over 65 years, with two or more missing teeth in the posterior atrophic mandible, and who did not want to undergo bone regenerative procedures, were included in this study. These patients were rehabilitated with custom-made subperiosteal implants, designed from cone beam computed tomography (CBCT) and fabricated in titanium by means of direct metal laser sintering (DMLS). The outcome measures were fit and stability of the implants at placement, duration of the intervention, implant survival, and early and late complications. All patients were followed for 1 year after surgery. RESULTS: Ten patients (four males, six females; mean age 69.6, SD ± 2.8, median 69, 95% CI 67.9-71.6) were included in the study. The fit of the implants was satisfactory, with a mean rating of 7 out of 10 (SD ± 1.6, median 7, 95% CI 6-8). Only two implants had insufficient fit, because of the presence of scattering in the CBCT; however, they were adapted to the sites during the interventions. The mean duration of the intervention was 44.3 min (SD ± 19.4, median 37, 95% CI 32.3-56.3). At the one-year follow-up, no implants were lost (survival rate 100%). One implant presented immediate postoperative complications with pain, discomfort and swelling, and two patients experienced late complications, having their provisional restorations fractured during the temporisation phase. All these complications were minor in nature, but the final complication rate amounted to 30% (three of ten patients). CONCLUSIONS: Although this study has limits (small patient sample and short follow-up), DMLS has proven to be an effective method for fabricating accurate subperiosteal implants, with high survival rates. This may represent an alternative treatment procedure in elderly patients with a severely atrophic posterior mandible, since it allows avoidance of regenerative bone therapies. Further studies are needed to confirm these outcomes.

BACKGROUND: The aim of this study was to compare free fluoride concentration and total fluoride concentration in mouthwashes. METHODS: Fluorine-containing mouthwashes from various companies and manufacturers (Colgate Total Plax Classic Mint®, Colgate-Palmolive, New York, USA; Colgate Total Plax Gentle Mint®, Colgate-Palmolive, New York, USA; Colgate Total Plax Fresh Mint®, Colgate-Palmolive, New York, USA; Oral B Advantage®, Procter&Gamble, Cincinnati, USA; Reach Fresh Mint®, Johnson&Johnson, New Brunswick, USA; Foramen®, Laboratorios Foramen, Guarnizo, Spain; Lacalut Sensitive®, Dr. THEISS, Homburg, Germany; Sensodyne®, GlaxoSmithKline, London, UK; Vesna F®, Vita, Saint Petersburg, Russia; Lacalut Fresh®, Dr. THEISS, Homburg, Germany) were selected as study objects. Fluoride measurements were carried out using the fluoride selective electrode. RESULTS: Free fluoride:total fluoride ratio was more than 80% for six samples (Colgate Total Plax Gentle Mint® - 88%, Colgate Total Plax Fresh Mint® - 99%, Oral B Advantage® - 92%, Reach Fresh Mint® - 92 and 89% for the mouthwash of another batch, Lacalut Sensitive® - 94%) and less than 63% for three samples (Colgate Total Plax Classic Mint® - 56%, Foramen® - 62%, Vesna F® - 61%). Two samples had more than 70% and less than 80% of unbound fluoride, respectively (Sensodyne® - 77%, another batch of Oral B Advantage® mouthwash - 74%). Rinse containing sodium monofluorophosphate (Na2PO3F) (Vesna F®) had more than 50% of free fluoride, while the rinse containing amine fluoride (AmF) (Lacalut Sensitive®) had 94%. The difference in the free fluoride:total fluoride ratio can be explained by binding of fluoride ions by components contained in mouthwashes, such as coloring agents and polymeric compounds. The lowest concentration of free fluoride ions (0.000093 mol/L) was observed for aluminum fluoride (AlF3) rinse (Lacalut Fresh®), while the total fluoride amount was not determined due to possible generation of strong fluoride complexes. This implies that fluoride ions will not be uptaken by tooth tissue and may even be washed away from it, compromising the efficacy of mouthwashes. CONCLUSIONS: The differences in free fluoride: total fluoride ratio between analyzed mouthwashes reveal a need to develop a method for evaluation of free fluorides in mouthwashes for proper updating of national and international guidelines.

BACKGROUND: This retrospective clinical study aims to present results of experience with a novel guided surgery system with a sleeveless, open-frame structure, in which the surgical handpiece (not the drills used for preparation) is guided. METHODS: This study was based on an evaluation of the records of partially edentulous patients who had been treated with a sleeveless open-frame guided surgery system (TWIN-Guide®, 2Ingis, Brussels, Belgium), between January 2015 and December 2017. Inclusion criteria were patients with good systemic/oral health and a minimum follow-up of 1 year. Exclusion criteria were patients who had been treated without a guide, or with a guide with sleeves, patients with systemic/oral diseases and who did not have a follow-up of 1 year. The main outcomes were surgical (fit and stability of the surgical guide, duration of the intervention, implant stability, and any intra-operative or immediate post-operative complication), biologic, and prosthetic. RESULTS: Thirty-eight patients (24 males, 14 females; mean age 56.5 ± 14.0 years) were included in the study. These patients had been treated with 110 implants inserted by means of 40 sleeveless, open-frame guides. With regard to fit and stability, 34 guides were excellent, 4 acceptable, and 2 inadequate for use. The mean duration of the intervention was 23.7 (± 6.7) minutes. Immediately after placement, 2 fixtures were not stable and had to be removed. Two patients experienced pain/swelling after surgery. The 108 surviving implants were restored with 36 single crowns and 32 fixed partial prostheses (24 two-unit and 8 three-unit bridges); these restorations survived until the 1-year follow-up, with a low incidence of biologic and prosthetic complications. CONCLUSIONS: Within the limits of this study, this novel guided surgery system with sleeveless, open frame-structure guides seems to be clinically reliable; further studies on a larger sample of patients are needed to confirm these outcomes.

PURPOSE: To present a digital method that combines intraoral and face scanning for the computer-assisted design/computer-assisted manufacturing (CAD/CAM) fabrication of implant-supported bars for maxillary overdentures. METHODS: Over a 2-year period, all patients presenting to a private dental clinic with a removable complete denture in the maxilla, seeking rehabilitation with implants, were considered for inclusion in this study. Inclusion criteria were fully edentulous maxilla, functional problems with the preexisting denture, opposing dentition, and sufficient bone volume to insert four implants. Exclusion criteria were age < 55 years, need for bone augmentation, uncompensated diabetes mellitus, immunocompromised status, radio- and/or chemotherapy, and previous treatment with oral and/or intravenous aminobisphosphonates. All patients were rehabilitated with a maxillary overdenture supported by a CAD/CAM polyether-ether-ketone (PEEK) implant-supported bar. The outcomes of the study were the passive fit/adaptation of the bar, the 1-year implant survival, and the success rates of the implant-supported overdentures. RESULTS: 15 patients (6 males, 9 females; mean age 68.8 ± 4.7 years) received 60 implants and were rehabilitated with a maxillary overdenture supported by a PEEK bar, designed and milled from an intraoral digital impression. The intraoral scans were integrated with face scans, in order to design each bar with all available patient data (soft tissues, prosthesis, implants, and face) in the correct spatial position. When testing the 3D-printed resin bar, 12 bars out of 15 (80%) had a perfect passive adaptation and fit; in contrast, 3 out of 15 (20%) did not have a sufficient passive fit or adaptation. No implants were lost, for a 1-year survival of 100% (60/60 surviving implants). However, some complications (two fixtures with peri-implantitis in the same patient and two repaired overdentures in two different patients) occurred. This determined a 1-year success rate of 80% for the implant-supported overdenture. CONCLUSIONS: In this study, the combination of intraoral and face scans allowed to successfully restore fully edentulous patients with maxillary overdentures supported by 4 implants and a CAD/CAM PEEK bar. Further studies are needed to confirm these outcomes.

In dental districts, successful bone regeneration using biphasic calcium phosphate materials was recently explored. The present study aimed to perform a comparative study between 3D-printed scaffolds produced by laser light stereo-lithography (SLA) and traditionally sintered biphasic calcium phosphate scaffolds by an integrated morphological, morphometric and mechanical analysis. METHODS: Biphasic calcium phosphate (30% HA/70% ß-TCP) samples, produced by SLA-3D-printing or by traditional sintering methods, were tested. The experimental sequence included: (1) Microtomography (microCT) analyses, to serve as control-references for the 3D morphometric analysis; (2) loading tests in continuous mode, with compression up to fracture, to reconstruct their mechanical characteristics; and (3) microCT of the same samples after the loading tests, for the prediction of the morphometric changes induced by compressive loading of the selected materials. All the biomaterials were also studied by complementary scanning electron microscopy to evaluate fracture regions and surfaces. RESULTS: The characterization of the 3D mineralized microarchitecture showed that the SLA-3D-printed biomaterials offer performances comparable to and in some cases better than the traditionally sintered ones, with higher mean thickness of struts and pores. Interestingly, the SLA-3D-printed samples had a higher ultimate strength than the sintered ones, with a smaller plastic region. Moreover, by SEM observation, it was observed that fractures in the SLA-3D-printed samples were localized in the structure nodes or on the external shells of the rods, while all the traditionally sintered samples revealed a ductile fracture surface. CONCLUSIONS: The reduction of the region of plastic deformation in the SLA-3D-printed samples with respect to traditionally sintered biomaterials is expected to positively influence, in vivo, the cell adhesion. Both microCT and SEM imaging revealed that the studied biomaterials exhibit a structure more similar to human jaw than the sintered biomaterials.

BACKGROUND: Until now, a few studies have addressed the accuracy of intraoral scanners (IOSs) in implantology. Hence, the aim of this in vitro study was to assess the accuracy of 5 different IOSs in the impressions of single and multiple implants, and to compare them. METHODS: Plaster models were prepared, representative of a partially edentulous maxilla (PEM) to be restored with a single crown (SC) and a partial prosthesis (PP), and a totally edentulous maxilla (TEM) to be restored with a full-arch (FA). These models were scanned with a desktop scanner, to capture reference models (RMs), and with 5 IOSs (CS 3600®, Trios3®, Omnicam®, DWIO®, Emerald®); 10 scans were taken for each model, using each IOS. All IOS datasets were loaded into a reverse-engineering software where they were superimposed on the corresponding RMs, to evaluate trueness, and superimposed on each other within groups, to determine precision. A statistical analysis was performed. RESULTS: In the SC, CS 3600® had the best trueness (15.2 ± 0.8 µm), followed by Trios3® (22.3 ± 0.5 µm), DWIO® (27.8 ± 3.2 µm), Omnicam® (28.4 ± 4.5 µm), Emerald® (43.1 ± 11.5 µm). In the PP, CS 3600® had the best trueness (23 ± 1.1 µm), followed by Trios3® (28.5 ± 0.5 µm), Omnicam® (38.1 ± 8.8 µm), Emerald® (49.3 ± 5.5 µm), DWIO® (49.8 ± 5 µm). In the FA, CS 3600® had the best trueness (44.9 ± 8.9 µm), followed by Trios3® (46.3 ± 4.9 µm), Emerald® (66.3 ± 5.6 µm), Omnicam® (70.4 ± 11.9 µm), DWIO® (92.1 ± 24.1 µm). Significant differences were found between the IOSs; a significant difference in trueness was found between the contexts (SC vs. PP vs. FA). In the SC, CS 3600® had the best precision (11.3 ± 1.1 µm), followed by Trios3® (15.2 ± 0.8 µm), DWIO® (27.1 ± 10.7 µm), Omnicam® (30.6 ± 3.3 µm), Emerald® (32.8 ± 10.7 µm). In the PP, CS 3600® had the best precision (17 ± 2.3 µm), followed by Trios3® (21 ± 1.9 µm), Emerald® (29.9 ± 8.9 µm), DWIO® (34.8 ± 10.8 µm), Omnicam® (43.2 ± 9.4 µm). In the FA, Trios3® had the best precision (35.6 ± 3.4 µm), followed by CS 3600® (35.7 ± 4.3 µm), Emerald® (61.5 ± 18.1 µm), Omnicam® (89.3 ± 14 µm), DWIO® (111 ± 24.8 µm). Significant differences were found between the IOSs; a significant difference in precision was found between the contexts (SC vs. PP vs. FA). CONCLUSIONS: The IOSs showed significant differences between them, both in trueness and in precision. The mathematical error increased in the transition from SC to PP up to FA, both in trueness than in precision.
