RESUMEN

Introduction Syncope is defined as a transient, self-limited loss of consciousness with an inability to maintain postural tone that is followed by spontaneous recovery. We revisit situational syncope focusing on one situation, Mass. Methods We interrogated our electronic syncope database for key terms associated with situational syncope. From the most commonly encountered situation, Mass, we interrogated the results of tilt testing performed to identify evidence of orthostatic hypotension. Results There were 110 cases of situational syncope identified with 56.3% (n=62) taking place at mass. All had tilt table testing performed and 15.4% (n=17) had evidence of orthostatic hypotension. Conclusion The multiple sudden changes in position during mass from sitting to kneeling to standing can precipitate an episode of orthostatic hypotension. Consideration should be given as to whether it is safe for older mass goers to be subjected to such significant orthostatic stress.

In order to correlate symptoms, osteoid volume, and aluminum deposition in bone, 46 methacrylate-embedded biopsy specimens from 26 hemodialysis patients were examined. Osteoid volume was measured using computer-assisted morphometric analysis, and aluminum was detected using the Aluminon stain. Positive staining for aluminum was present in biopsies from 21 patients. Osteoid volume did not correlate with duration of dialysis therapy or ingestion of aluminum hydroxide but displayed a logarithmic relationship with dialysate aluminum exposure. Patients with bone pain at the time of biopsy had a greater degree of hyperosteoidosis than asymptomatic subjects. Osteoid volume in patients with fractures and positive aluminum staining decreased on withdrawal from aluminum-rich dialysate. The Aluminon staining technic is a convenient method of confirming aluminum overload.

RESUMEN

Two cases of ischaemic optic neuropathy, which occurred as a complication of oncological neck surgery, are reported. These cases are submitted because of the apparent scarcity in the literature of this complication after head and neck surgery. They are also unusual because they presented with different clinical manifestations of ischaemic optic neuropathy after separate forms of bilateral neck dissection. A literature review identifies a small number of similar cases and risk factors and preventative measures are discussed.
