ABSTRACT

Abstract Background Local anesthetic puncture is often related to the experience of pain. This study aimed to systematically analyze the literature on changes in pain perception during the anesthetic puncture of dental local anesthesia after Photobiomodulation Therapy (PBMT). Material and methods An electronic search was performed in eight primary databases (Embase, LILACS, BBO, LIVIVO, MedLine via PubMed, SciELO, Scopus, and Web of Science) and three additional ones (EASY, Google Scholar, and OATD) to partially capture the "gray literature". The PICO strategy was used to identify randomized clinical trials evaluating the analgesic effect of PBMT in the anesthetic puncture site of dental local anesthesia compared to placebo or control groups, without restrictions on publication language and year. Two reviewers extracted the data and assessed the individual risk of bias of the eligible studies using the Cochrane Collaboration Risk of Bias Tool version 2.0. Results The electronic search found 3,485 records, of which eight met the eligibility criteria and were included in the qualitative synthesis. The studies were published from 2011 to 2022. None of the included studies had a low risk of bias. PBMT groups showed no significant difference in pain scores compared to placebo and control groups of most studies. Conclusion Based on a low to very low certainty of evidence, PBMT seems to have no effect on pain perception during anesthetic puncture in patients undergoing dental local anesthesia.

ABSTRACT

Purpose: The current study aimed at evaluating the repair of a partial defect of the trachea with a muscle flap, an advanced technique that employs combined suture patterns. Methods: Sixteen healthy male New Zealand white rabbits were used as an experimental model. A partial defect in the trachea within the ventral region of the fourth to eighth tracheal ring was created. Subsequently, repair was initiated with a flap of the sternocephalicus muscle. The animals were divided into four groups for postoperative evaluation using clinical, tracheoscopic, and histopathological analyses. Each group was separated according to the time of euthanasia, programmed at interval of seven (G7), 15 (G15), 30 (G30), and 60 days (G60). Results: One animal from the G60 group died, whereas the other animals had good surgical recovery without serious changes in the breathing pattern. The major clinical signs observed were stridor and coughing. Tracheoscopy revealed secretions in the tracheal lumen, exuberant granulation, and stenosis. Histopathological analysis showed growth of the ciliary respiratory epithelium at the flap site 30 days after implantation. Conclusions: Partial repair showed satisfactory results owing to the anatomical location of the muscle, adequate vascular support, and structural and physiological maintenance without serious changes in the respiratory system.

ABSTRACT

Abstract Working with an interprofessional focus is increasingly necessary, in view of the growing complexity of the population's health needs. This study aims to assess interprofessional collaboration and the teamwork climate in primary health care (PHC) and determine whether there is a relationship between these two variables. The AITCS-II instrument was used to measure interprofessional collaboration, while to diagnose teamwork climate, the ECTE instrument was used, a version adapted to the SUS context of the Teamwork Climate Inventory instrument. These two instruments were applied online together with a questionnaire for the sociodemographic characterization of the 544 participants, who belonged to 97 Family Health Strategy (FHS) teams in a Brazilian municipality. The obtained data were submitted to a multilevel analysis. A positive correlation was observed between interprofessional collaboration and three of the four teamwork climate factors. The better the work climate, the better the interprofessional collaboration in the corresponding team, and this characteristic stands out in relation to other individual analyzed characteristics.


Resumo Trabalhar com foco interprofissional é cada vez mais necessário, tendo em vista a crescente complexidade das necessidades de saúde da população. Este estudo tem como objetivo avaliar a colaboração interprofissional e o clima de trabalho em equipe na atenção primária à saúde (APS) e verificar possível relação entre estas duas variáveis. Para mensurar a colaboração interprofissional foi utilizado o instrumento AITCS-II, enquanto para o diagnóstico do clima de trabalho em equipe foi utilizado o instrumento ECTE, versão adaptada para o contexto SUS do instrumento Teamwork Climate Inventory. Esses dois instrumentos foram aplicados on-line juntamente com um questionário para caracterização sociodemográfica dos 544 participantes, pertencentes a 97 equipes da Estratégia Saúde da Família (ESF) de um município brasileiro. Os dados obtidos foram submetidos a uma análise multinível. Foi observada uma correlação positiva entre a colaboração interprofissional e três dos quatro fatores do clima de trabalho em equipe. Quanto melhor o clima de trabalho, melhor a colaboração interprofissional na equipe correspondente, e essa característica se destaca em relação às demais características individuais analisadas.

ABSTRACT Objective: To assess the relationship between celiac disease and the prevalence of dental caries in children and adolescents and also evaluate if dental enamel defects in celiac children predispose to dental caries. Material and Methods: Searches were performed in the following databases: PubMed, Scopus, Cochrane Library, Latin American and Caribbean Literature on Health Sciences, and OpenGrey. The risk of bias was assessed using the Newcastle-Ottawa Scale. DMFT/dmft and DMFS/dmfs data of observational studies that compared the prevalence of caries between children and adolescents with celiac disease and healthy individuals. Meta-analysis was performed using a random effects model. Heterogeneity between studies was estimated using Cochran's Q test, and inconsistency was measured using I2 statistics. Results: Of the 121 studies retrieved, 17 were selected, and 12 were included in the meta-analysis. The prevalence of caries in the primary dentition (dmft) did not differ between celiac patients and controls [SMD = -0.35; 95% CI (-0.83; 0.13); p = 0,15; I2 = 89%]. There was also no difference in the prevalence of caries in permanent teeth (DMFT) between groups [SMD = -0.44; 95% CI (-1.02; 0.14); p = 0.14; I2 = 95%]. Conclusion: Celiac disease is not a determinant factor in the development of dental caries in children and adolescents compared to the control group.

ABSTRACT Third molar extraction surgery is a frequentprocedure in dentistry. Like any surgical procedure, it may lead to inflammatory responses, and postoperative pain is one of its main complications. Furthermore, temporomandibular disorder (TMD) is a collective termfor several clinicalproblems involving orofacial structures. Patients withparafunction are more sensitive to mechanical stimuli such as pressure during surgical procedures. Aim: To analyze postoperative pain in patients with and without bruxism subjected to third molar extraction surgery. Materials and Method: This was an observational study including four groups with a 1:1:1:! allocation ratio, conducted following ethical approval. Patients classified as ASA I with an indication for lower third molar extraction were recruited. Bruxism was self-reported. Two surgical techniques were used: one with only forceps and levers (ST1) and another with osteotomy and odontosection (ST2). Results: Four groups (bruxism and surgical techniques) were enrolled, each with a convenience sample (n=34). Postoperative pain levels were higher in patients with than without bruxism (p<0.05). The comparison between surgical techniques showed significantly higher pain levels only on the seventh day for ST2 groups (p<0.05). Oral mucosaflap incisions did not cause significantly higher persistence and pain levels. Conclusions: Bruxism, osteotomy, and odontosection may have increased postoperative pain levels, whereas performing an oral mucosa flap did not cause significant differences. Nevertheless, these preliminary data should be interpreted carefully. Randomized controlled trials are required to reinforce the findings of this study.


RESUMO A cirurgia de extragao de terceiros molares é um procedimento frequente na odontologia. Como em qualquer procedimento cirúrgico, pode levar a respostas inflamatorias. A dor pós-operatória é uma das principais complicagoes após a cirurgia de extragao de terceiros molares. Além disso, disfungao temporomandibular (DTM) é um termo coletivo para vários problemas clínicos envolvendo estruturas orofaciais. Pacientes com parafungao sao mais sensíveis a estímulos mecánicos como pressao durante procedimentos cirúrgicos. Objetivo: Analisar a dor pós-operatória em pacientes com e sem bruxismo submetidos á cirurgia de extragao de terceiros molares. Material e Método: Um estudo observacional incluindo quatro grupos com uma proporgao de alocagao de 1:1:1:1 foi realizado após aprovagao ética. Foram recrutados pacientes classificados como ASA I com indicagao de exodontia de terceiros molares inferiores. O bruxismo foi autorreferido e foram realizadas duas técnicas cirúrgicas: uma com apenas fórceps e alavancas (ST1) e outra com osteotomia e odontosecgao (ST2). Resultados: Foram incluidos quatro grupos (bruxismo e técnicas cirúrgicas), cada um com uma amostra de conveniencia (n=34). Os níveis de dor pós-operatória foram maiores em pacientes com bruxismo (p<0,05). A comparagao entre as técnicas cirúrgicas mostrou níveis de dor significativamente maiores apenas no sétimo diapara os grupos ST2 (p<0,05). Incisoes de retalhos de mucosa oral nao mostraram níveis de dor significativamente maiores. Conclusoes: Bruxismo, osteotomia e odontosecgaopodem aumentar os níveis de dor pós-operatória, enquanto a realizagao de retalho de mucosa oral nao apresenta diferengas significativas. No entanto, a interpretagao cuidadosa desses dados preliminares é recomendada, e ensaios clínicos randomizados sao necessários para fortalecer os achados deste estudo.

ABSTRACT

Abstract Lipolytic substance injections to reduce localized fat have been extensively used because it is a low-invasive method. This review aimed to evaluate the efficacy and safety of deoxycholic acid in submental fat reduction compared to a placebo and investigate the potential industry sponsorship bias in the results of randomized clinical trials on this topic. Ten electronic databases were extensively searched for randomized clinical trials without restriction on language and year of publication. Two reviewers extracted the data and assessed the individual risk of bias in the studies with the RoB 2.0 tool. The industry sponsorship bias was evaluated according to citations in the articles regarding industry funding/sponsorship throughout the texts. Fixed and random effects meta-analyses were performed, and the results were reported in Risk Ratio (RR) at a 95% Confidence Interval (95% CI). The initial search provided 5756 results, of which only five were included. Only two studies had a low risk of bias. All studies showed a potential industry bias. The meta-analysis showed that patients treated with deoxycholic acid had significant positive results for all efficacy outcomes and a higher risk of fibrosis, pain, erythema, numbness, swelling, edema, pruritus, nodules, headache, and paresthesia. The low to moderate certainty of evidence found allows concluding that deoxycholic acid is effective in submental fat reduction, causing well-tolerated adverse effects. However, all eligible studies showed a potential industry bias.

Resumo A interprofissionalidade foi tema central do nono edital do Programa de Educação pelo Trabalho para a Saúde. O objetivo deste estudo foi descrever a disponibilidade dos estudantes que participaram do programa para a aprendizagem interprofissional. A pesquisa, de abordagem quantitativa e de perspectiva descritivo-exploratória, coletou dados através da readiness for interprofessional learning scale, adaptada para o português do Brasil. Os resultados indicam que há disponibilidade para o trabalho em equipe e a colaboração. Também foram encontrados percentuais significativos relacionados à identidade profissional, o que indica disponibilidade para a aprendizagem interprofissional, embora aspectos relacionados à competição tenham aparecido através de percentuais menores de concordância em relação à aprendizagem compartilhada e interdependência das práticas profissionais. Para a atenção centrada no usuário também foram encontrados resultados que indicam boa disponibilidade entre os participantes; porém, aspectos relacionados à colaboração do usuário na produção dos serviços de saúde ainda expressam a tendência de uma lógica centrada nos profissionais. Conclui-se que há disponibilidade positiva para a educação interprofissional, que precisa ser cada vez mais estimulada nas políticas e estratégias de reformas curriculares.


Abstract Interprofessionality was the central theme of the ninth call for proposals of the Education by Working for Health Program. The objective of this study was to describe the readiness of students who participated in the program for interprofessional learning. The research, of a quantitative approach and descriptive-exploratory perspective, collected data by applying the readiness for interprofessional learning scale (RIPLS), adapted for Brazilian Portuguese. The results indicate an important readiness for teamwork and collaboration. Relevant percentages were also found regarding professional identity, indicating good readiness for interprofessional learning, although aspects related to competition appeared in lower percentages of agreement regarding shared learning and interdependence of professional practices. For user-centered care, results were also found indicating good readiness among participants; however, aspects related to the collaboration of the user in the production of health services still express the tendency of a professional-centered logic. In conclusion, there is a positive readiness for interprofessional education, which needs to be increasingly stimulated in policies and strategies of curricular reforms.

ABSTRACT

Introdução: Os lipossarcomas retroperitoneais são neoplasias mesenquimais raras, sendo mais comuns os bem diferenciados e os desdiferenciados. O subtipo bem diferenciado pode sofrer desdiferenciação para tumores de maior grau. São neoplasias difíceis de tratar cirurgicamente, pois apresentam altas taxas de recorrência local, alguns subtipos podem metastizar e são pouco sensíveis à radioterapia e à quimioterapia. Relato do caso: Paciente feminina, 45 anos, apresentou dor abdominal e massa abdominal palpável em 2017. Foi submetida à ressecção de lipossarcoma bem diferenciado de retroperitônio, sem intercorrências. Em 2020, manifestou dor abdominal e perda ponderal. A tomografia mostrou múltiplas massas volumosas abdominais, com biópsia sugestiva de lipossarcoma desdiferenciado. Foi submetida à radioterapia neoadjuvante e, em seguida, à ressecção cirúrgica das massas e ileocolectomia direita. Em 2022, apresentou quadro sugestivo de obstrução intestinal, sendo submetida à laparotomia que evidenciou intenso bloqueio de alças intestinais, fístula duodenal, tumor retroperitonial e peritonite fecal. Procedeu-se à ressecção de neoplasia retroperitoneal, ileostomia e rafia de fístula. O histopatológico mostrou lipossarcoma desdiferenciado recidivado. A paciente evoluiu com complicações operatórias e infecciosas, necessitando de cuidados intensivos e antibioticoterapia. Após melhora clínica, recebeu alta com dieta enteral e segue em acompanhamento ambulatorial. Conclusão: O lipossarcoma de retroperitônio pode sofrer desdiferenciação, recidivas multifocais e múltiplas recorrências, necessitando de várias abordagens cirúrgicas, o que aumenta a morbidade e o risco de complicações. A cirurgia com margens amplas continua sendo a principal modalidade terapêutica.


ABSTRACT Introduction: Retroperitoneal liposarcomas are rare mesenchymal neoplasms, with well-differentiated and dedifferentiated liposarcomas being most common. The well differentiated subtype can undergo dedifferentiation to higher grade tumors. These are difficult neoplasms to treat surgically because they have high rates of local recurrence, some subtypes can metastasize, and are poorly responsive to radiotherapy and chemotherapy. Case report: Female patient, 45 years old, presented abdominal pain and palpable abdominal mass in 2017. She underwent resection of well-differentiated liposarcoma of the retroperitoneum, without intercurrences. In 2020, she manifested abdominal pain and weight loss. Tomography showed multiple voluminous abdominal masses, with biopsy suggestive of dedifferentiated liposarcoma. The patient was submitted to neoadjuvant radiotherapy, followed by surgical resection of the masses and right ileocolectomy. In 2022, she presented symptoms suggestive of intestinal obstruction, and underwent laparotomy that revealed intense blockage of intestinal loops, duodenal fistula, retroperitoneal tumor, and fecal peritonitis. Retroperitoneal neoplasm resection, ileostomy and fistula closure were performed. Histopathology showed relapsed dedifferentiated liposarcoma. The patient evolved with operative and infectious complications, requiring intensive care and antibiotic therapy. After clinical improvement, the patient was discharged with enteral diet and continues under outpatient follow-up. Conclusion: Retroperitoneal liposarcoma may undergo multifocal dedifferentiation and recurrence, requiring several surgical approaches, increasing morbidity and the risk of complications. Wide margin surgery remains the main therapeutic modality.


Introducción: Los liposarcomas retroperitoneales son neoplasias mesenquimatosas raras, siendo los más comunes los liposarcomas bien diferenciados y desdiferenciados. El subtipo bien diferenciado puede sufrir desdiferenciación hacia tumores de mayor grado. Estas neoplasias son difíciles de tratar quirúrgicamente porque presentan altas tasas de recidiva local, algunos subtipos pueden hacer metástasis y responden mal a la radioterapia y la quimioterapia. Informe del caso: Mujer de 45 años, en 2017 presenta dolor abdominal y masa abdominal palpable. Fue sometida a la resección de un liposarcoma bien diferenciado del retroperitoneo, sin intercurrencias. En 2020, manifestó dolor abdominal y pérdida de peso. La tomografía mostró múltiples masas abdominales voluminosas, con biopsia sugestiva de liposarcoma desdiferenciado. Fue sometida a radioterapia neoadyuvante y luego a resección quirúrgica de las masas y a ileocolectomía derecha. En 2022, presentó síntomas de obstrucción intestinal y fue sometida a una laparotomía que reveló obstrucción de las asas intestinales, fístula duodenal, tumor retroperitoneal y peritonitis fecal. Se realizó la resección de la neoplasia retroperitoneal, la ileostomía y la fistulización. La histopatología mostró un liposarcoma desdiferenciado. La paciente evolucionó con complicaciones operatorias e infecciosas, requiriendo cuidados intensivos y terapia antibiótica. Tras la mejora clínica, la paciente fue dada de alta con dieta enteral y está en seguimiento. Conclusión: El liposarcoma retroperitoneal puede sufrir desdiferenciación multifocal y recurrencia, requiriendo varios a tratamientos quirúrgicos, aumentando la morbilidad y el riesgo de complicaciones. La cirugía con márgenes amplios sigue siendo la terapia principal.

ABSTRACT

The study analyzed the influence of temperature on the depth of cure of bulk-fill composite resins. Three discs (ISO 4049/2000) from each group were made, and four restorative materials were investigated: Opus Bulk-Fill Flow, Opus Bulk-Fill APS, Filtek™ One Bulk-Fill, and Filtek™ Bulk-Fill Flow. They were light-cured (Gnatus ± 1,200 mW/cm2) for 20 seconds, varying the temperature (23°C and 5°C) and simulating the use of the product both at room temperature and under refrigeration. The materials were inserted in aluminum matrices with 10 mm in depth and 4 mm in diameter. A clear film strip and a glass microscope slide were positioned at the top and bottom surfaces. The material was condensed and light-cured with the tip of the light source in close contact, for 20 seconds on the top surface. Immediately after irradiation, the samples were removed from the mold, and the uncured part was removed with a plastic spatula. The measurement was performed with a micrometer ± 0.1 mm, and the value was divided by 2. The data were analyzed with two-way ANOVA at a significance of 0.05. There was a statistical difference in the temperature between the results obtained in Opus Bulk-Fill APS (p <0.001) and Filtek™ Bulk-Fill Flow (p = 0.018) resins. For the temperature of 5°C, Filtek™ Bulk-Fill Flow showed a statistical difference compared to the other resins, while Opus Bulk-Fill Flow and Opus Bulk-Fill APS did not. For the temperature of 23°C, Filtek™ Bulk-Fill Flow maintained the statistical difference from the others, but Filtek™ One Bulk-Fill and Opus Bulk-Fill APS did not present statistical differences. Storing the Filtek™ Bulk-Fill Flow composite resin in the refrigerator caused a greater depth of cure than the other resins, and the depth of cure decreased at room temperature. As for Opus Bulk-Fill APS, the depth of cure decreased proportionally to the temperature decrease. The temperature may affect the depth of cure of some composite resins.

ABSTRACT Objective: To evaluate a group of Brazilian dentists on their knowledge of Molar Incisor Hypomineralization (MIH) and Hypomineralized Second Primary Molars (HSPM) related to clinical aspects, consequences, and diagnostic criteria. Material and Methods: In this cross-sectional, the participants were invited by e-mail and Whatsapp® to answer a questionnaire about their knowledge of hypomineralization enamel defects (MIH/HSPM) on the Google Forms® platform. The questionnaire comprised eight questions about personal data and multiple-choice questions about their knowledge concerning clinical aspects, diagnostic criteria of MIH/HSPM and differential diagnosis through clinical images. Chi-square test was applied with the significance level set at 5%. Results: Most participants (n = 492; 91.1%) reported having knowledge about MIH/HSPM. The general dentists gave more incorrect answers (n = 40; 65.6 %;) about dental tissues affected by MIH/HSPM. Overall, 83.3% of the dentists gave the correct answer to which dentitions are associated with this condition. In addition, most dentists presented knowledge about the consequences related to possible fractures (n= 487; 90.2%) and about an increased risk of caries (n= 479; 88.9%) in the affected teeth. Regarding the differential diagnosis performed through clinical images, most participants gave incorrect answers (p≤0.001). Conclusion: The participants presented knowledge about the dentition associated with this condition and possible consequences related to the teeth affected by MIH/HSPM; however, they showed difficulties concerning clinical diagnostic criteria.

ABSTRACT OBJECTIVE This study aims to assess covid-19 morbidity, mortality, and severity from 2020 to 2021 in five Brazilian Amazon states with the highest records of wildfires. METHODS A distributed lag non-linear model was applied to estimate the potential exposure risk association with particulate matter smaller than 2.5-µm in diameter (PM2.5). Daily mean temperature, relative humidity, percentual of community mobility, number of hospital beds, days of the week, and holidays were considered in the final models for controlling the confounding factors. RESULTS The states of Para, Mato Grosso, and Amazonas have reported the highest values of overall cases, deaths, and severe cases of covid-19. The worrying growth in the percentual rates in 2020/2021 for the incidence, severity, and mortality were highlighted in Rondônia and Mato Grosso. The growth in 2020/2021 in the estimations of PM2.5 concentrations was higher in Mato Grosso, with an increase of 24.4%, followed by Rondônia (14.9%). CONCLUSION This study establishes an association between wildfire-generated PM2.5 and increasing covid-19 incidence, mortality, and severity within the studied area. The findings showed that the risk of covid-19 morbidity and mortality is nearly two times higher among individuals exposed to high concentrations of PM2.5. The attributable fraction to PM2.5 in the studied area represents an important role in the risk associated with covid-19 in the Brazilian Amazon region.

Resumo O leiomiossarcoma de veia cava inferior (LVCI) é um raro tumor maligno mesenquimal. Seu tratamento cirúrgico é um desafio, pois necessita combinar margens cirúrgicas livres com reconstrução vascular, usando prótese ou enxerto autólogo, sutura primária ou ligadura simples sem reconstrução da veia. A ligadura é possível graças ao lento crescimento do tumor, permitindo o desenvolvimento de circulação venosa colateral. Apresentamos um caso de LVCI tratado por ressecção radical sem reconstrução vascular. Paciente feminina, 48 anos, com dor abdominal em hipocôndrio direito, astenia e sintomas dispépticos pós-prandiais. Tomografia de abdome revelou massa de formação expansiva localizada no segmento infra-hepático da veia cava inferior com redução da luz do vaso. Na cirurgia, o clampeamento da veia não indicou repercussões hemodinâmicas, sugerindo formação de circulação colateral suficiente. Decidiu-se pela ressecção radical em toda a porção da veia cava retro-hepática e ligadura da veia cava sem reconstrução vascular. A paciente evoluiu sem intercorrências.


Abstract Inferior vena cava leiomyosarcoma (IVCL) is a rare malignant mesenchymal tumor. Surgical treatment is a challenge because it must combine free surgical margins with vascular reconstruction, using prosthetic or autologous grafts, primary suture, or simple ligation without vein reconstruction. The ligation option is possible thanks to the slow growth of the tumor, allowing collateral venous circulation to develop. We present a case of an IVCL treated with radical resection without vascular reconstruction. The patient was a 48-year-old female with abdominal pain in the right upper quadrant, asthenia, and postprandial dyspeptic symptoms. Abdominal tomography revealed a mass with an expansive formation located in the infrahepatic segment of the inferior vena cava and reduced vessel lumen. During surgery, vein clamping did not provoke hemodynamic repercussions, suggesting sufficient collateral circulation formation. It was decided to perform a radical resection of the entire portion of the retrohepatic vena cava and ligate the vena cava without vascular reconstruction. The patient recovered without complications.

ABSTRACT

Abstract The study aimed to analyze the spatial distribution of sedentary behavior and unhealthy eating habits, and to assess its relationship with the neighborhood environment. Cross-sectional study with data of Surveillance System of Risk and Protective Factors for Chronic Diseases, carried out in Belo Horizonte, Minas Gerais. Watching television for four hours or more per day was identified as sedentary behavior. Unhealthy eating habits were defined based on regular consumption of sodas, excess fat meat, and red meat, and irregular consumption of fruits and vegetables. Georeferenced data of places for physical activity, food establishments, population and residential density, homicide rate, mean total income, and social vulnerability index were entered into the Vigitel database. The coverage area by basic health units was used as the geographical unit of neighborhood. SaTScan was used to analyze the spatial distribution. Spatial analysis identified a significant cluster of high prevalence of sedentary behavior and unhealthy eating habits, after adjusting for sociodemographic characteristics. The comparison of environmental characteristics inside and outside clusters showed significant differences in the physical and social environment. Physical and social environment might be related to clusters of high prevalence of sedentary behavior and unhealthy eating habits.


Resumo O objetivo foi analisar a distribuição espacial do comportamento sedentário e do hábito alimentar não saudável e verificar a relação com o ambiente da vizinhança. Estudo transversal com dados do Sistema de Vigilância de fatores de risco e proteção para doenças crônicas, realizado em Belo Horizonte, Minas Gerais. Considerou-se comportamento sedentário o hábito de assistir televisão quatro ou mais horas por dia. O hábito alimentar não saudável foi avaliado pelo consumo regular de carne com excesso de gordura, refrigerante e carne vermelha e irregular de frutas e hortaliças. Informações georreferenciadas dos locais para a prática de atividade física, estabelecimentos com venda de alimentos, densidade populacional e residencial, taxa de homicídio, renda e índice de vulnerabilidade social foram inseridas na base do Vigitel. A área de abrangência da unidade básica de saúde foi usada como unidade geográfica da vizinhança. A análise espacial identificou cluster significativo de alta prevalência de comportamento sedentário e hábito alimentar não saudável, mesmo após ajuste. Os ambientes físico e social podem estar relacionados a cluster de alta prevalência de comportamento sedentário e hábito alimentar não saudável.

Introducción:La afectación pulpar irreversible de los dientes primarios representa un desafío para la endodoncia en odontopediatría. Sobre la base del enfoque de esterilización de lesiones y reparación de tejidos (LSTR- Lesion Sterelization and Tissue Repair), se ha propuesto una técnica endodóncica no instrumental que utiliza pastas antibióticas. La pasta antibiótica CTZ, que contiene una mezcla de cloranfenicol, tetraciclina, óxido de zinc y eugenol, tiene potencial para uso en la técnica endodóncica no instrumental en pacientes pediátricos. Objetivo: El objetivo de este trabajo es presentar una revisión narrativa de la literatura sobre la pasta antibiótica CTZ, en lo relacionado a capacidad antimicrobiana, biocompatibilidad y aspectos clínicos. Materiales y métodos: Se realizaron búsquedas en las bases de datos PUBMED, BVS, WEB OF SCIENCE, COCHRANE y SCOPUS utilizando los términos "ctz paste", "ctz", "tooth, deciduous" y "primary molars" sin restricción de idioma, con fecha limitada de 2015 hasta enero de 2021. Resultados: Después de eliminar los duplicados, se obtuvieron 13 artículos. La pasta CTZ mostró una actividad antimicrobiana satisfactoria en Enterecoccus faecalis, Staphylococcus aureus, Klebisiella pneumonia, Escherichia coli y Candida albicans. Los estudios in vitro e in vivo han demostrado que la pasta tiene biocompatibilidad. La frecuencia de éxito clínico varió del 37% al 100% y el éxito radiográfico del 29,7% al 97,4%. Conclusiones: Aunque se necesitan más estudios que contribuyan a un mejor nivel de la evidencia, puede afirmase que la pasta CTZ una es alternativa para los casos de afectación pulpar en dientes primarios, inclusive en el ámbito de la salud pública

Introduction: The irreversible pulp changes in primary teeth represent a challenge to endodontics in pediatric dentistry. Based on the Lesion Sterilization and Tissue Repair approach (LSTR), a non-instrumental endodontic treatment (NIET) of primary teeth root canals using an antibiotic paste has been proposed. The CTZ antibiotic paste, which contains a mixture of chloramphenicol, tetracycline, zinc oxide and eugenol, has potential for use in pediatric patients. Objective: The objective of this work is to present a literature review on the CTZ antibiotic paste, covering antimicrobial capacity, biocompatibility, and clinical aspects. Methods: An electronic search on PUBMED, BVS, WEB OF SCIENCE, COCHRANE y SCOPUS databases using the terms "ctz paste", "ctz", "deciduous teeth" and "primary molars" without language restrictions and publication date between 2015 and January 2021. Results: After removing duplicates, a total of 13 articles were included. The CTZ paste showed satisfactory antimicrobial activity on Enterecoccus faecalis, Staphylococcus aureus, Klebisiella pneumonia, Escherichia coli andCandida albicans. In vivo and in vitro studies have demonstrated that the paste presents good biocompatibility. The frequency of clinical success varied from 37% to 100% and radiographic success from 29,7% to 97,4%. Conclusion: Although more studies are needed to contribute to the body of evidence, the CTZ antibiotic paste demonstrated antimicrobial activity, biocompatibility and satisfactory clinical results and may be an alternative for cases of pulp involved primary teeth, including in public health.

ABSTRACT

Se ha declarado una pandemia ante la propagación de un nuevo virus con alta contagiosidad, llamado síndrome respiratorio agudo severo coronavirus 2 (severe acute respiratory syndrome coronavirus 2, SARS-CoV2). El mundo ha quedado detenido ante la rápida expansión del virus, con una letalidad que en algunos países llega a 15%. En Chile, el gobierno ha tomado medidas rápidas y agresivas que han permitido mantener la curva de contagios a un nivel que permita atender de manera adecuada a la población. Dentro de estas medidas, se contempla la suspensión de cirugías y consultas ambulatorias. Como cirujanos ortopédicos, nos hemos visto afectados por estas medidas, y existe confusión respecto a cuál es la conducta más adecuada. Quisimos hacer esta guía para resumir parte de las evidencias disponibles y orientar a los cirujanos ortopédicos respecto a esta patología. El comportamiento de esta guía es dinámico, dadas las múltiples opiniones, experiencias y evidencias, que surgen diariamente, por lo que recomendamos mantenerlo como referencia, no como certeza.


A pandemic has been declared due to a new highly contagious virus called severe acute respiratory syndrome coronavirus 2 (SARS-CoV2). The world has come to a halt due to the rapid expansion of a virus whose lethality has reached 15% in some countries. In Chile, the government has taken decisive, aggressive measures in an attempt to control disease spread and provide healthcare to those who need it. These decisions include the suspension of elective surgeries and other ambulatory procedures. As Orthopedic surgeons we have been affected by these measures and there is doubt regarding the best course of action. We prepared this guide to summarize available evidence and orient our colleagues regarding this pathology. This guide is meant to be dynamic, as new opinions, evidence and experiences arise every day. Therefore, we advise the reader to keep it as a reference, not an undisputable truth.

ABSTRACT

ABSTRACT Objective: to map the scientific production on interprofessional relationships in health in the first year of the COVID-19 pandemic. Method: this is a scoping review performed in PubMed, Scopus, LILACS, CINAHL, Web of Science, Google Scholar and Science Direct databases, covering the period of publication in 2020, using the acronym PCC (Population = health professionals; Concept = interprofessional relationships; Context = health services) and respective search strategies. Results: fourteen scientific articles were selected and the content discussed in the manuscripts was standardized, analyzed and organized into categories of affinities and similarities of their results: 1 - Interprofessional collaboration; 2 - Collaborative practice; 3 - Interprofessional work; 4 - Interactive and interprofessional learning. Conclusion: the pandemic demanded quick and effective responses that were only possible through collaboration and interprofessionalism dimensions. Interprofessional work in health during the first year of the COVID-19 pandemic confirms the importance of interprofessional work and its dimensions for the provision of more comprehensive, resolute and safer health services.


RESUMEN Objetivo: mapear la producción científica sobre las relaciones interprofesionales en salud en el primer año de la pandemia del COVID-19. Método: revisión del alcance, realizada en las bases de datos PubMed, Scopus, LILACS, CINAHL, Web of Science, Google Scholar y Science Direct, cubriendo el período de publicación 2020, utilizando las siglas del PCC (Población = profesionales de la salud; Concepto = relaciones interprofesionales; Contexto = salud servicios) y las respectivas estrategias de búsqueda. Resultados: se seleccionaron 14 artículos científicos y el contenido discutido en los manuscritos fue estandarizado, analizado y organizado en categorías de afinidades y similitudes de sus resultados: 1 - Colaboración interprofesional; 2 - Práctica colaborativa; 3 - Trabajo interprofesional; 4 - Aprendizaje interactivo e interprofesional. Conclusión: la pandemia exigía respuestas rápidas y eficaces que solo eran posibles a través de las dimensiones de la colaboración y la interprofesionalidad. El trabajo interprofesional en salud durante el primer año de la pandemia COVID-19 confirma la importancia del trabajo interprofesional y sus dimensiones para la prestación de servicios de salud más integrales, decididos y seguros.

Resultados: foram selecionados 14 artigos científicos e o conteúdo discutido nos manuscritos foi uniformizado, analisado e organizado em categorias de afinidades e semelhanças de seus resultados: 1 - Colaboração interprofissional; 2 - Prática colaborativa; 3 - Trabalho interprofissional; 4 - Aprendizagem interativa e interprofissional. Conclusão: a pandemia demandou respostas rápidas e eficazes que só foram possíveis através das dimensões da colaboração e da interprofissionalidade. O trabalho interprofissional em saúde durante o primeiro ano de pandemia de COVID-19 confirma a importância do trabalho interprofissional e suas dimensões para a oferta de serviços de saúde mais integrais, resolutivos e seguros.

RESUMO A partir do conceito de imanência em sua leitura deleuziana, este trabalho objetivou pensar a criação de si como forma de afirmar nossa existência e nosso desejo. O exercício analítico realizado discutiu o aspecto problemático de se justificar a linguagem por seu caráter hermenêutico e seus reflexos em nossa potência de agir. Esse é o resultado de uma produção subjetiva que, ao capturar o desejo, nos distancia daquilo que "podemos" e acaba por diminuir nossa capacidade de ação no mundo das práticas corporais de movimento.


ABSTRACT Based on the concept of immanence in its Deleuzian reading, this work aimed to think about the creation of ourselves as a way of affirming our existence and our desire. The conducted analytical exercise discussed the problematic aspect of justifying language by its hermeneutic character and its reflexes on our power to act. This is the result of a subjective production, by capturing the desire, that distances us from what may be able to be done and ends up decreasing our capacity for action in the world of body movement practices.


RESUMEN Partiendo del concepto de inmanencia en su lectura deleuziana, el objetivo de este trabajo es pensar "la creación de si" como una forma de afirmar nuestra existencia y nuestro deseo. El ejercicio analítico realizado discutió el aspecto problemático de justificar el lenguaje por su carácter hermenéutico y sus reflejos sobre nuestra capacidad de actuar. Ese es el resultado de una producción subjetiva que, al capturar el deseo, nos aleja de ese que "podemos" y acaba por disminuir nuestra capacidad de actuación en el mundo de las prácticas corporales de movimientos.

ABSTRACT Objective: To analyze the way of life of the unhoused people to enhance health care in the pandemic. Methods: A qualitative, interdisciplinary research, with participant observation and 24 interviews with the unhoused people. Empirical categories and bibliographic search on this population and COVID-19 guided simple actions aimed at care. Results: The group at greatest risk for COVID-19 use drugs compulsively; starves constantly; discontinues drug treatment for tuberculosis, HIV, and diabetes; has underdiagnosis of Depression; has difficulty sheltering and uses inhaled drugs. This way of life increases the risk of worsening COVID-19 and brings great challenges to health services. Several proposals to guide care considered these results and the new routine caused by the pandemic. Final considerations: The way of life of the studied population increased their vulnerability in the pandemic, as well as the perception of risk of disease transmission by the population in general.


RESUMEN Objetivo: Analizar el modo de vida de la población en situación de calle (PSC) para potenciar el cuidado en salud en la pandemia. Métodos: Investigación cualitativa, interdisciplinaria, con observación participante, y 24 entrevistas con población en situación de calle. Categorías empíricas y búsqueda bibliográfica sobre dicha población y COVID-19 determinaron propuestas simples orientadas al cuidado. Resultados: El grupo más vulnerable a COVID-19 usa drogas compulsivamente, pasa hambre, interrumpe tratamiento médico para tuberculosis, VIH y diabetes; tiene subdiagnóstico de depresión, dificultad para hallar refugio y usa drogas inhalantes. Dicho modo de vida incrementa el riesgo de agravamiento del COVID-19, imponiendo mayores desafíos a los servicios de salud. Diversas propuestas de orientación del cuidado consideraron estos resultados y las nuevas rutinas impuestas por la pandemia. Consideraciones finales: El modo de vida de la población estudiada incrementó su vulnerabilidad en pandemia, así como la percepción de riesgo de transmisión de la enfermedad a la población en general.


RESUMO Objetivo: Analisar o modo de vida da População em Situação de Rua para potencializar o cuidado em saúde na pandemia. Métodos: Pesquisa qualitativa, interdisciplinar, com observação participante e 24 entrevistas com população em situação de rua. Categorias empíricas e busca bibliográfica sobre essa população e COVID-19 pautaram proposições simples voltadas ao cuidado. Resultados: O grupo de maior risco para COVID-19 faz uso compulsivo de drogas; passa fome constantemente; interrompe tratamento medicamentoso para Tuberculose, HIV e Diabetes; tem subdiagnóstico de Depressão; tem dificuldade de abrigamento; usa drogas inalatórias. Esse modo de vida aumenta risco de agravamento da COVID-19 e impõe maiores desafios aos serviços de saúde. Diversas propostas para orientar o cuidado levaram em conta esses resultados e a nova rotina impressa pela pandemia. Considerações Finais: O modo de vida da população estudada ampliou sua vulnerabilidade na pandemia, bem como a percepção de risco de transmissão da doença pela população em geral.

ABSTRACT: Objective: To analyze the spatial distribution of the prevalence of leisure-time physical activity (LTPA) in a Brazilian urban area and its association with the characteristics of the physical and social environments. Methods: A cross-sectional study conducted with data from the Surveillance System for Risk and Protective Factors for Chronic Diseases from the years 2008-2010, in Belo Horizonte, state of Minas Gerais, Brazil. The outcome was the practice of LTPA and the independent variables were residential and population density, the density of places for physical activity, homicide rates, average family income, and health vulnerability index. The spatial scanning technique was employed to identify clusters with a high prevalence of PA at leisure time. The Mann-Whitney test was used to compare variables inside and outside the cluster. Results: The sample included 5,779 participants, 33.3% (SE = 0.73) of whom reported sufficient PA during leisure time. We identified a significant cluster of a high prevalence of LTPA. After adjustments, the cluster presented a radius of 3,041.99 meters and 603 individuals, and 293 (48.6%) of them reported sufficient LTPA. The probability of performing sufficient LTPA in the cluster was 27% higher (PR = 1.27; p = 0.002) than in the coverage areas of primary healthcare units outside the cluster. There was a higher density of places for LTPA practice, higher population and residential density, and higher family income in the cluster. Conclusion: The results evidenced a cluster of high prevalence of LTPA in a privileged physical and socioeconomic environment in Belo Horizonte, even after adjustments, demonstrating that reducing inequalities can increase LTPA.


RESUMO: Objetivo: Analisar a distribuição espacial da prevalência da prática de atividade física (AF) no lazer suficiente em uma área urbana brasileira, no período de 2008 a 2010, e verificar se ela é influenciada por características dos ambientes físico e social. Métodos: Estudo transversal com dados do sistema de Vigilância de Fatores de Risco para doenças crônicas não transmissíveis, realizado em Belo Horizonte (MG), entre os anos 2008 e 2010. O desfecho foi a realização de AF no lazer e as exposições foram densidade residencial e populacional, densidade de locais de prática de AF no lazer, taxa de homicídio, renda familiar média e índice de vulnerabilidade da saúde. Foi empregada a técnica de varredura espacial para identificar clusters de alta prevalência de AF no lazer. Utilizou-se o teste de Mann-Whitney para comparar variáveis ambientais dentro e fora do cluster. Resultados: A amostra foi constituída de 5.779 participantes, dos quais 33,3% referiram praticar AF no lazer suficientemente. Identificou-se um cluster significativo de alta prevalência de prática de AF no lazer. Após ajustes, ele apresentou raio de 3.041,99 m e 603 indivíduos, dos quais 293 (48,6%) realizavam AF no lazer suficiente. A probabilidade de um indivíduo praticar AF no lazer suficiente no cluster foi 27% maior (razão de prevalência — RP = 1,27; p = 0,002) do que nas áreas de abrangência das unidades básicas de saúde não pertencentes a ele. Esse cluster teve maior densidade de locais para a prática de AF no lazer, maior densidade populacional e residencial e maior renda familiar. Conclusão: Constatou-se cluster de alta prevalência de AF no lazer em um contexto físico e econômico privilegiado de Belo Horizonte, o que demonstra que diminuir desigualdades pode aumentar a prática de AF no lazer.
