ABSTRACT

The primary chemical components of Astragalus membranaceus include polysaccharides, saponins, flavonoids, and amino acids. Recent studies have shown that Astragalus membranaceus has multiple functions, including improving immune function and exerting antioxidative, anti-radiation, anti-tumor, antibacterial, antiviral, and hormone-like effects. Astragalus membranaceus and its extracts are widely used in clinical practice because they have obvious therapeutic effects against various autoimmune diseases and relatively less adverse reaction. Multiple sclerosis (MS) is an autoimmune disease of central nervous system (CNS), which mainly caused by immune disorder that leads to inflammatory demyelination, inflammatory cell infiltration, and axonal degeneration in the CNS. In this review, the authors analyzed the clinical manifestations of MS and experimental autoimmune encephalomyelitis (EAE) and focused on the efficacy of Astragalus membranaceus and its chemical components in the treatment of MS/EAE.

ABSTRACT

Based on the clinical characteristics of multiple sclerosis (MS) in traditional Chinese medicine (TCM) and western medicine and literature analysis, this paper aims to formulate the diagnostic criteria of TCM and western medicine for MS. Moreover, the modeling methods of experimental autoimmune encephalomyelitis (EAE), animals for the modeling, and characteristics of the models were analyzed and summarized, and the consistency between the EAE models and the diagnostic criteria of TCM and western medicine was evaluated. The results showed that animal models had low consistency with the clinical characteristics in TCM (highest consistency 68%) and western medicine (highest consistency 60%). Pathological models account for the majority of animal models for MS research, but there is a lack of intuitive performance indicators. Thus, it is difficult to comprehensively evaluate the models. The mental state, limb numbness, lack of strength, loss of muscle tone, tremor, and balance disorders of the mice are among the diagnostic criteria in western medicine. In TCM diagnostic criteria, the major symptoms which are reflected in animal behavior, such as physical fatigue, lack of strength, mental fatigue, distinclination to talk, and weak heavy numb limbs, are consistent with the western diagnostic criteria. The minor symptoms, including mental decline, bitter taste in mouth, frequent and urgent urination, fecal incontinence, and aggravated fever, are not well reflected in the models. According to TCM, MS is caused by deficiency of kidney essence and external contraction of pathogen, but no index is available for evaluating the external contraction of pathogen in existing animal models. The key to experimental research on MS is to establish an appropriate animal model based on the clinical pathogenesis and characteristics. However, there is a lack of MS animal model with TCM characteristics for syndrome classification. Therefore, renewed efforts should be made to prepare animal models with both TCM and western medicine characteristics that can be used in both basic experiments and clinical research.

ABSTRACT

Objective:To investigate the clinical efficacy and prognosis of transjugular intrahepatic portosystemic shunt (TIPS) and drug combined with endoscopic treatment in patients with liver cirrhosis and esophagogastric variceal bleeding (EGVB).Methods:From January 2012 to December 2013, at the First Affiliated Hospital of Xi′an Jiaotong University, the data of 147 patients with liver cirrhosis and EGVB undergoing TIPS or drug combined with endoscopic treatment were retrospectively collected, with 87 cases in TIPS treatment group and 60 in drug combined with endoscopic treatment group. The 5 years follow-up data were analyzed, and the overall survival rates, rebleeding-free survival rates and hepatic encephalopathy-free survival rates at 6 weeks, 1 year, 2 years and 5 years after treatment of two groups were compared. Independent sample t test, Mann-Whitney U test, chi-square test, Fisher exact test, Z test, log-rank test and trend test were used for statistical analysis. Results:There were no significant differences in age, gender, etiology, Child-Pugh classification, initial liver function, coagulation function, liver ascites, previous history of hepatic encephalopathy, blood pressure and preoperative blood transfusion history between the TIPS treatment group and combination of drugs and endoscopy treatment group (all P>0.05). Forty-one patients died within 5 years, of which 20 (48.8%) died of rebleeding and 6 (14.6%) died of hepatic encephalopathy. There were no significant differences in 6-week, 1-year and 2-year overall survival rates between the TIPS group and drug combined with endoscopic treatment group (all P>0.05), however the 5-year overall survival rate of the TIPS treatment group was higher than that of the drug combined with endoscopic treatment group (78.4% vs. 63.2%), and the difference was statistically significant ( Z=2.06, P=0.048). The 6-week, 1-year, 2-year, 5-year rebleeding-free survival rates of the TIPS group were 97.7%, 96.5%, 88.9% and 70.9%, respectively, which were all higher than those of the drug combined with endoscopic treatment group (86.7%, 53.3%, 43.3% and 27.1%), and the differences were statistically significant ( Z=2.35, 6.39, 6.26 and 4.80, all P<0.05). There were no significant differences in hepatic encephalopathy-free survival rates at 6 weeks, 1 year and 2 years after treatment between the TIPS group and drug combined with endoscopic treatment group (all P>0.05), however the 5-year hepatic encephalopathy-free survival rate of the TIPS treatment group was lower than that of the drug combined with endoscopic treatment group (67.7% vs. 86.7%), and the difference was statistically significant ( Z=2.28, P=0.030). The lower the Child-Pugh classification, the higher the cumulative 5-year survival rate ( χ2=6.75, P<0.01). There was no statistically significant difference in the 5-year overall survival rate in patients with the same Child-Pugh classification between the TIPS group and the drug combined with endoscopic treatment group (all P>0.05). Conclusions:The efficacy of TIPS is better than that of the drug combined with endoscopic treatment in treating EGVB. Even the long-term risk of hepatic encephalopathy of TIPS is higher, the short-term, middle-term and long-term rebleeding rate are decreased. Patients with Child-Pugh grade C do not need to avoid TIPS when choosing the treatment, the earlier the TIPS used, the better survival benefit will be obtained.

ABSTRACT

OBJECTIVE:To investigate the correlation between the morphological characteristics and chemical constituents of garden ginseng (GG),mountain transplanted ginseng (MTG)and mountain-grown ginseng (MGG),so as to provide evidence for elucidating the scientific connotation of “evaluating quality from morphological characteristics ”. METHODS :A total of 30 batches of GG ,MTG and MGG were collected. The contents of 11 saponins(ginsenoside Rg 1,Re,Rg2,F1,Rb1,Rb2,Rb3,Rc,Rd,Ro, Rf)were determined by UPLC. The determination was performed on Acquity UPLC ® BEH C 18 column with mobile phase consisted of 0.1% Formic acid solution- 0.1% Acetonitrile formate solution (gradient elution )at the flow rate of 0.5 mL/min. The column temperature was set at 35 ℃. The detection wavelength was set at 203 nm,and sample size was 5 μL. The content of starch was determined by colorimetry ,and the absorbance value was measured at 559 nm with 0.5% iodine-potassium iodide as the color reagent. SPSS 23.0 statistical software was used to perform Pearson relationship analysis for appearance characteristic indexes (number of stem scars ,rhizome length ,texture)with the contents of 11 kinds of saponins ,the content of total saponins ,the content ratio of panaxatriol saponin (PPT)/panaxadiol saponin (PPD),the content of starch. RESULTS :The linear relationship between UPLC and colorimetry was good (r>0.999). RSDs of precision ,reproducibility and stability tests were all lower than 5% . Average recoveries were 95.17% -105.20%(RSD<5% ,n=6). Except for ginsenoside Rf ,the contents of other 10 ginsenosides were positively correlated with the number of stem scars ,so were the contents of 11 gensenosides with the rhizome length(r>0,P<0.05),and they were negatively correlated with the firmness of the texture (r<0,P<0.05). The content of starch in P. ginseng was positively correlated with texture firmness (r=0.95,P<0.01). The ratio of PPT/PPD in P. ginseng was negatively associated with the number of stem scars and the rhizome length (r<0,P<0.05). CONCLUSIONS :The number of stem scars ,the rhizome length and texture porosity of P. ginseng are positively associated with the contents of medicinal material ginsenoside;the increase of PPD content in P. ginseng was higher than that of PPT with the increase of growth years. The traditional“evaluating quality from morphological characteristics ”for P. ginseng has a certain scientific connotation ,which is more stem scars ,longer rhizome and loose texture.

ABSTRACT

Objective: To optimize the extraction process of total flavonoid from different kinds of teas and com- pare the total flavonoid content in teas. Methods: The UV spectrophotometry was used to determine the total flavonoids in tea. The effects of ethanol concentration,material-liquid ratio, extraction temperature and extraction time on the ex- traction rate were investigated. The extraction processes were optimized by the orthogonal experiment and variance analy- sis,and the total flavonoid content was compared for different extraction processes. Results: The conditions for the opti- mal extraction processes were as follows:60% ethanol concentration,1:40(g/ml)material-liquid ratio,80℃ extraction temperature and 70 min extraction time for black tea;60% ethanol concentration,1:40(g/ml)material-liquid ratio, 90℃ extraction temperature and 70 min extraction time for oolong tea;and 70% ethanol concentration,1:40(g/ml)ma- terial-liquid ratio,80℃ extraction temperature and 70 min extraction time for green tea. Under the conditions of the opti- mized extraction processes, the content of total flavonoids in black tea was the highest, followed by the oolong tea, with the least content in green tea. Conclusion: The optimization of the extraction process could significantly increase the ex- traction of in tea,and the total flavonoid content obviously differed in the different kinds of teas. The present results pro-vide an experimental basis for future studies on the extraction of total flavonoid in teas to further explore the influencing factors to the extraction.

ABSTRACT

Background@#Little study has investigated the differences between Talatomyces marneffei (T. marneffei) respiratory infection and tuberculosis and the prognostic factors of such infection. This study investigated the characteristics and prognostic factors of T. marneffei infections with respiratory lesions and the causes of misdiagnosis.@*Methods@#Clinical characteristics and prognoses of patients with T. marneffei infections with respiratory system lesion were investigated. T. marneffei diagnosis followed isolation from clinical specimens using standard culture, cytology, and histopathology. Survival curves were estimated by using Kaplan-Meier analysis, with log-rank test to compare differences in survival rates between groups. Univariate and multivariate Cox regression analyses were also performed to assess significant differences in clinical characteristics of overall survival.@*Results@#Of 126 patients diagnosed with T. marneffei infections, 63 (50.0%) had T. marneffei respiratory system infections; 38.1% (24/63) were misdiagnosed as having tuberculosis. Human immunodeficiency virus (HIV) infection, CD4/CD8 < 0.5, percentage of CD4+ T cells <42.8%, and length of time from onset to confirmation of diagnosis >105 days were potential risk factors for poor prognoses. Length of time from onset to confirmation of diagnosis persisted as an independent predictor of all-cause mortality in multivariate analysis (odds ratio: 0.083, 95.0% confidence interval: 0.021–0.326, P < 0.001). However, the size of the lung lesions, dyspnea, thoracalgia, mediastinal lymphadenopathy, and pleural effusion did not significantly predict overall survival. There was no significant difference in prognosis according to the type of treatment.@*Conclusions@#T. marneffei infections involving the respiratory system are common. The critical determinants of prognosis are HIV infection, CD4/CD8, percentage of CD4+ T cells, type of treatment, and the time range from onset to confirmation of diagnosis. Rapid and accurate diagnosis is crucial for improving prognosis.

ABSTRACT

This research aims to develop an UHPLC method, based on core-shell column(i.e. superficially porous particles), for simultaneous determination of eight isoflavonoids including formononetin,(6αR,11αR)-3-hydroxy-9,10-dimethoxypterocarpan, calycosin-7-O-β-D-glucopyranoside,(3R)-7,2-dihydroxy-3,4-dimethoxyisoflavone, calycosin, ononin,(6αR,11αR)-9,10-dimethoxypterocarpan-3-O-β-D-glucopyranoside, and(3R)-7,2-dihydroxy-3,4-dimethoxyisoflavan-7-O-β-D-glucopyranoside in Astragali Radix. The analysis was performed on an Agilent Poroshell EC-C_(18 )column(2.1 mm×100 mm, 2.7 μm) with 0.2% formic acid solution(A)-acetonitrile(B) as mobile phase for gradient elution. The flow rate was 0.5 mL·min~(-1), with column temperature of 40 ℃ and the wavelengths were set at 260 and 280 nm. According to the results, all calibration curves showed good linearity(R~2>0.999 8) within the tested concentration ranges. Both the intra-and inter-day precisions for 8 isoflavonoids were less than 0.80%, with the mean recovery at the range of 94.71%-104.6%. Thus, the newly developed UHPLC method using core-shell column owned the advantages in terms of rapid analysis, low column pressure and less solvent consumption, thus enabling the usage of conventional HPLC systems. Meanwhile, quantitative evaluation was carried out for 22 batches of commercial Astragali Radix. It has been found that great variations occurred for the content of the individual isoflavonoids among different batches; in contrast, the total content of total 8 isoflavonoids(>0.1%) was stable in most samples, indicating that it was reasonable to involve all isoflavonoids as the chemical markers for the quality control of Astragali Radix.

ABSTRACT

Objective To explore the application value of low ligation of inferior mesenteric artery(IMA) in laparoscopic Dixon of nonⅠsigmoid colon mesangial(SMC). Methods Seventy-one cases of nonⅠSMC rectal cancer patients who evaluated for feasibility of laparoscopic Dixon in the Department of Gastrointestinal Surgery Affiliated Tumor Hospital of Xinjiang Medical University from September 2017 to July 2018 were selected in this study, and they were randomly divided into two groups:the group of left colic artery reserved(low ligation group)and the group of left colic artery not retained(high ligation group). A three-dimensional compu-ted tomography angiography of inferior mesenteric artery was obtained in all patients to guide the intraoperative accurate ligation of vessels. The baseline data, intraoperative and postoperative quantitative parameters of the two groups were compared. Results Three patients in the group of left colic artery reserved were forced to turn to high ligation for it could not meet the requirement of anastomo-sis. The low ligation group showed significant difference to the high ligation group on operation time[170(160,180) vs. 180 (170, 210)](P=0. 026),but there was no significant difference in the aspect of gender, age,BMI,distance between tumor and anal bor-der,TNM stage, intraoperative blood loss, the splenic free rate, prophylactic stoma rate,Length of specimen, maximum diameter of tumor, tissue typing, total lymph node count,D3 lymph node count, postoperative first exhaust time, days in hospital since surgery and anastomotic fistula rate(P>0. 05). Conclusion Low ligation of inferior mesenteric artery can shorten the operation time and signifi-cantly lessen unplanned bowel length and reduce postoperative risk of anastomotic leakage. All of these are beneficial to patients′post-operative recovery and have important clinical guiding value.

ABSTRACT

To investigate the efficacy of bismuth containing quadruple therapies on Helicobacter pylori (Hp) eradication in patients with history of antibiotic treatment.
Methods: Hp infected patients (n=327) were allocated into 3 groups. Group A (n=52), patients had no antibiotic history and they took medicine of proton pump inhibitors (PPI) and livzon triple (clarithromycin, tinidazole, and bismuth); group B (n=80), patients had the antibiotic history except for amoxicillin and clarithromycin, and they were treated with PPI, amoxicillin, clarithromycin, and bismuth; group C (n=195), patients suffered failures of Hp therapy or with history of antibiotic abuse, and they were treated with PPI, doxycycline, furazolidone, and bismuth.
 Results: Both the intention-to-treat (ITT) analysis (group A 63.5%, group B 76.2%, group C 82.6%, P<0.05) and the pre-protocol (PP) analysis (group A 76.7%, group B 92.4%, group C 96.4%, P<0.05) showed significant difference among the 3 groups, revealing higher elimination in group B and C. The side-effects (20.2%) were mild and tolerable (group A, 28.0%; group B, 10.7%; group C, 22.0%).
Conclusion: Proton pump inhibitors together with the livzon triple regimen have a low rate of Hp eradication and a higher incidence of adverse reactions. The quadruple therapy containing clarithromycin and metronidazole drugs can achieve the satisfactory outcomes based on patient's antibiotic history. For patients with multiple antibiotics, the quadruple therapy containing furazolidone and doxycycline may achieve the satisfactory outcomes, but the adverse resction would be relatively higher.

ABSTRACT

Isothermal nucleic acid amplifications, as powerful as polymerase chain reaction but functioning at a constant temperature, are considered to be very promising technique in achieving point-of-care gene diagnostics. However, until now, their practical applications are still seriously lagged by the bad reliability resulting from the problems such as false positive amplification and low signal amplitude. In this work, a universal transduction method in which any sequence ( including loop-mediated isothermal amplification products) could be transduced via a hairpin transducer into a catalyst of a well-engineered circuit (catalytic hairpin assembly, CHA) was established. Because CHA circuit could amplify tens to hundreds fold with especially high sequence specificity, it could provide both accuracy and high amplitude for sequence detection. And for a new targeting sequence, the only sequence needed to be changed was the hairpin transducer. Due to the importance of the transducer, we provided and verified a universal designing rule-set to guarantee the transducing efficiency ( signal to background ratio) of the transducer. Transducers designed following this rule set were then proved to be very efficient in detecting pathogen gene targets. As less as near single molecule ( 20 copies ) of pathogen genes could be detected with significant fluorescent and electrochemical signals.

ABSTRACT

Objective:The aim of this study is to explore the utilization of basic public health services by elder-ly migrants and its influencing factors, and to provide data support for the equalization of public health services. Methods:The framework of the Behavioral Model of Health Services Utilization elaborated by Anderson was used as the theoretical basis. Basing on the data of 2015 National Dynamic Monitoring of Mobile Population, the multivariate logistic regression model was used to analyze the materials coming from in-depth interviews. Results: The utilization rate of basic public health services by the elderly migrant was low and there are differences as compared to other groups. The type of household registration, living duration and the scope of mobility were the main factors influencing the utilization of health services by the elderly migrants. The allocation of funds to primary health care services is in-consistent with the workload, and other difficulties for the community service agencies. Conclusions:The accessibility of basic public health services for the elderly migrants is poor, the health consciousness is weak, and health education should be strengthened. The elderly migrants coming from rural areas, having long-term residence and migrating long distance should be paid more attention too. The administrative departments should strengthen the statistics on elderly migrants, ensure a fair allocation of funds for the basic public health service, and optimize the top-level design of in-formation technology to improve the use of the basic public health services for elderly migrants.

Results There were 38 rehabilitation hospitals in Guangdong province including 18 non-profit and 20 for-profit ones. Only one was tertiary rehabilitation hospital. There were totally 4524 open beds, hospitals with 50 to199 beds ac-counted for 68.4%. There were 37,262 rehabilitation equipments worth more than 10,000 Yuan, in which less than 500,000 Yuan was the most. There were 776 rehabilitation doctors,1051 nurses,749 therapists, and 248 oth-er staff members. The annual hospital admissions were more than 55,000, hospital bed utilization rate was 74.6%,bed turnover rates was 12.75,the working time of bed was 272.3 days, and the average length of stay in hospital was 19.5 days. Conclusion These rehabilitation institutions cannot meet the need of people in Guangdong province. Many problems ex-isted,such as low hospital scales, unbalanced geographical distribution, low rates of hospital beds utilization and turnover, inadequate hospital management and imperfect health system. The service and management capabilities of rehabilitation institutions should be strengthened.

ABSTRACT

Objective:To investigate the correlation between certain unhealthy lifestyles and the incidence of gastroesophageal reflux disease (GERD),thus to provide the lifestyle guidelines for GERD patients. Methods:Retrospective study were conducted for 402 GERD and 276 non-GERD out-patients in Department of Gastroenterology,Third Xiangya Hospital,Central South University from August,2014 to August,2015 based on questionnaire survey, then the correlation of unhealthy lifestyles with GERD were analyzed. Results:The top 10 common symptoms for GERD were as follows:reflux, acid regurgitation, postprandial fullness, heartburn, swallow obstruction or pain, epigastric burning sensation, paraesthesia pharynges, poststernal pain, chronic laryngopharyngitis, and chronic cough. The top 8 unhealthy habits closely related to GERD were as follows:fast-eating,over-eating,spicy preferred diet, sweets preferred diet, anxious, soup preferred diet,high-fat diet, and hot eating.Single-factor analysis showed that GERD was markedly correlated to gender (male),age (≥ 60 years),BMI,smoking, alcohol fast-eating,over-eating,hot-eating,spicy preferred diet,high-fat diet, acid preferred diet, sweets preferred diet, hard food preference, strong tea preference, coffee preference, immediately on bed after meal, difficult defecation, dyscoimesis, anxious, and too tight belt, respectively (P<0.05).Logistic multiple regression analysis indicated that the largest risk factor for GERD was the fast-eating (OR=3.214,95% CI 2.171 to 4.759,P<0.001) followed by the overeating (OR=2.936,95% CI 1.981 to 4.350,P<0.001),elderly population (OR=2.047,95% CI 1.291 to 3.244,P=0.002),too tight belt (OR=2.003,95% CI 1.013 to 3.961,P=0.046),and hot-eating (OR=1.570,95% CI 1.044 to 2.362,P=0.030).Conclusion:The elderly people are at high risk for GERD,and unhealthy habits like fast-eating,over-eating,too tight belt, and hot-eating is closely related to GERD.The lifestyles such as chewing food thoroughly, splitting the meals up,warm and cool diet, keeping patients out of the too tight belt are necessary for GERD patients.

ABSTRACT

Objective To analyze formalin-fixed and paraffin-embedded(FFPE) sample by next generation sequencing(NGS) technology for high throughput DNA sequencing and related bioinformatics analysis, and to explore the reliability and clinic significance of miRNAs in the detection of sample from FFPE gastric cancer tissue. Methods FFPE and fresh frozen samples of the same tissue were excised from 2 cases of gastric cancer in 2015 and 2016.RNA was extracted from the samples and the small RNA library was isolated and constructed, and high throughput DNA sequencing was conducted with Illumina sequencing platform. The DNA sequencing results were analyzed by quantifying the expression of known miRNAs families sequence, and the expression profiling of known miRNAs was constructed.Spearman correlation analysis was performed on the miRNAs of the samples to assess the difference in the expression of miRNAs.Results The total RNA extracted from the fresh frozen sample had higher quality and complete RNA coverage, and the results showed that the 28S was 1.3-1.4 times of 18S.The RNA extracted from the FFPE sample showed different degrees of degradation, and the peaks of 28S and 18S were not significant. The spearman correlation of miRNAs expression profiling in frozen and FFPE samples was 0.90 and 0.86 respectively, and the expression levels of miRNAs in two samples were consistent and had significant correlation. Conclusion miRNAs in FFPE sample can retain certain level of integrity and can be used in tumor test by NGS.

ABSTRACT

The key to maximize the sensitivity of inner filter effect ( IFE)-based assay is to enlarge the overlap between the absorption spectra of the absorber and the excitation or emission spectra of the fluorophore. In this work, Mn-doped ZnS quantum dots ( QDs) were chosen for IFE-based detection of β-glucuronidase ( GUS) , since the excitation of QDs was perfectly overlapped with the absorption of the substrate of GUS, namely 4-nitrophenyl-β-D-glucuronide ( PNPG) . In addition, the phosphorescence emission from Mn-doped ZnS QDs could eliminate the fluorescence background from biological samples. Therefore, a simple turn-on phosphorescent GUS assay was developed, with the linear range of 10-300 U/L and limit of detection of 7 U/L (S/N=3).
