ABSTRACT
Objective @#To compare the ultrasound signs of symptomatic joint lesions in rheumatoid arthritis (RA) and gouty arthritis ( GA) , musculoskeletal ultrasound ( MSUS) was utilized. @*Methods @#A retrospective analysis was performed for 85 hospitalized patients with RA and 55 hospitalized patients with GA in the same period , and the differences in general data , diseased joints and ultrasound signs between the two groups were compared. @*Results@#The gender, age and diseased joints of the two groups were statistically significant (all P < 0. 05) . The detection rate of knee joint lesions was the highest; the RA group had high sensitivity , high specificity of meniscal injury , and high diagnostic efficiency of bone erosion , while the diagnostic performance of the three combined ultrasound signs of punctate strong echo , double track sign and tophi in the GA group was higher than that of any individual diagnosis , and the sensitivity and specificity were also higher. The course of disease in the RA group was positively correlated with bone erosion (P < 0. 05) , and the course in the GA group was positively correlated with tophi (P < 0. 05) .@*Conclusion @#The ultrasound signs of RA and GA are different , and MSUS has good value in the diagnosis and differential diagnosis of the two.

Pyroptosis, an inflammatory caspase-dependent programmed cell death, plays a vital role in maintaining tissue homeostasis and activating inflammatory responses. Orthodontic tooth movement (OTM) is an aseptic force-induced inflammatory bone remodeling process mediated by the activation of periodontal ligament (PDL) progenitor cells. However, whether and how force induces PDL progenitor cell pyroptosis, thereby influencing OTM and alveolar bone remodeling remains unknown. In this study, we found that mechanical force induced the expression of pyroptosis-related markers in rat OTM and alveolar bone remodeling process. Blocking or enhancing pyroptosis level could suppress or promote OTM and alveolar bone remodeling respectively. Using Caspase-1-/- mice, we further demonstrated that the functional role of the force-induced pyroptosis in PDL progenitor cells depended on Caspase-1. Moreover, mechanical force could also induce pyroptosis in human ex-vivo force-treated PDL progenitor cells and in compressive force-loaded PDL progenitor cells in vitro, which influenced osteoclastogenesis. Mechanistically, transient receptor potential subfamily V member 4 signaling was involved in force-induced Caspase-1-dependent pyroptosis in PDL progenitor cells. Overall, this study suggested a novel mechanism contributing to the modulation of osteoclastogenesis and alveolar bone remodeling under mechanical stimuli, indicating a promising approach to accelerate OTM by targeting Caspase-1.

Objective To explore the value of three-dimensional speckle tracking imaging (3D-STI)in early dectection of left ventricular myocardial dysfunction in patients with breast cancer after receiving Anthracycline.Methods A group of 63 breast cancer postoperative patients were recruited and received a six-cycle epirubicin-based chemotherapy.Electrocardiogram,conventional echocardiography and 3D-STI parameters were measured before chemotherapy and during 24-48 hours after the cumulative dose of 120 mg/m2 ,240 mg/m2 and 360 mg/m2 .The receiver operating characteristics (ROC ) curves of 3D-STl parameters,such as global area strain(GAS),global longitudinal strain(GLS),global circumferential strain (GCS),and global radial strain(GRS)were drawn to determine optimal sensitivity and specificity.Twenty-one healthy female volunteers served as control subjects.Results Compared with before chemotherapy and control subjects,GAS and GLS were significantly reduced at the cumulative dose of 240 mg/m2 ,360 mg/m2 (P <0.05).The area under ROC of GAS was 0.974,and its optimal cut-off value was -3 1 .5%,with a specificity of 87.1 % and a sensitivity of 92.9%.Moreover,correlation analysis showed GAS was significantly associated with cumulative doxorubicin dose(r = -0.834,P < 0.01 ).Conclusions During chemotherapy,GAS can detect minor LV myocardial dysfunction associated with cardiotoxicity of anthracycline.3D-STI is useful in early dectection of left ventricular myocardial dysfunction in patients with breast cancer after receiving anthracycline.

<p><b>OBJECTIVE</b>To investigate the effects of maxillary protraction combined with repetitive rapid palatal expansions and constrictions (RPE/C) vs. rapid palatal expansion (RPE) alone using cone-beam computed tomography (CBCT).</p><p><b>METHODS</b>Twenty four subjects with maxillary retrusion were recruited and block randomized into either the control group (n = 12) or the RPE/C (n = 12) group.</p><p><b>CONTROL GROUP</b>facemask protraction after RPE. RPE/C group: facemask protraction after RPE/C. 3D reconstruction, landmarks identifying, superimposition and cephalometric analysis were performed to compare the pre-treatment and post- treatment CBCT images.</p><p><b>RESULTS</b>One subject in the RPE/C group was lost to follow up during the treatment. Maxilla moved forward [(2.5±1.0) mm] after maxillary protraction with RPE/C, which was significantly greater than that in the control group [(1.6±0.8) mm] (P < 0.05). The distance of basion to subspinale (Ba-A) increased [(3.1±1.0) mm] in the RPE/C group, which was significantly greater than that in the control group [(2.2 ± 0.9) mm] (P < 0.05). The amount of forward movement of upper first molars was significantly greater in the RPE/C group (P < 0.05).</p><p><b>CONCLUSIONS</b>Maxillary protraction with RPE/C might positively affect the forward movement of maxilla compared with the RPE alone protocol with the early treatment of maxillary retrusion patients.</p>

Objective To evaluate regional and global left ventricular systolic function after coronary artery bypass grafting (CABG) by speckle tracking imaging (STI).Methods Transthoracic echocardiography (TTE) was performed in 32 patients with coronary heart disease (CHD) preoperative and 3 months after CABG.The two-dimensional data were obtained in apical 4-chamble,2-chamble and long axis view.Peak systolic longitudinal strain (SLs),global longitudinal systolic strain (GLS) and average global strain(GLS-Avg) were measured using STI.30 healthy volunteers were involved as controls.Results SLs in the control group increased from the basal to the apical ventricular.SLs in the control group and CHD group apical segment were significantly higher than the basal and middle segments.Compared to the control group,SLs,GLS in part of myocardial ischemic segments were lower in CHD group before CABG (P <0.05).SLs and GSL in the corresponding segments showed significantly improving 3 months after CABG in CHD group (P < 0.05).There was good relationship between GSL-Avg and left ventricular ejection fraction.Conclusions STI technology can be used to objectively and quantitatively evaluate the change of regional and global left ventricular systolic function.Therefore,it provide a non-invasive means for evaluation of curative effect and prognosis judgement of CHD after CABG.
