ABSTRACT
Objective To investigate the effect of antinuclear antibodies(ANAs)on hormone response in patients with autoimmune hepatitis(AIH)-primary biliary cholangitis(PBC)overlap syndrome(AIH-PBC OS)and AIH-only within half a year.Methods A retrospective analysis of 77 patients with autoimmune liver disease(AILD)admitted to First Clinical Medical College of Lanzhou University from January 2018 to December 2021,all of whom were confirmed by liver biopsy and receiving glucocorticoid treatment.Among them,46 patients were in AIH-PBC OS group and 31 were in AIH-only group.The general clinical characteristics,liver puncture-related indexes,autoantibodies and immunoglobulin indexes of patients in each group at the time of diagnosis were collected and compared,and the biochemical and immunoglobulin indexes of patients at the time of hormone use and at the time of review within 6 months were also collected,and the hormone response within 6 months was evaluated according to the levels of glutamic transaminase(AST),glutamic alanine transaminase(ALT)and immunoglobulin G(IgG),and the effect of ANAs on hormone response outcomes in both groups over a six-month period was analyzed.Multifactorial ordered logistic analysis was performed to evaluate the effect of ANAs on hormone response between two groups.Results There was no statistically significant difference in the percentage of AIH-PBC OS and AIH-only patients among both ANAs-positive and-negative AILD patients(55.6%vs.44.4%and 65.6%vs.34.4%,P>0.05).Among 46 patients with AIH-PBC OS,there were 25 in ANAs-positive group and 21 in ANAs-negative group.The rate of complete hormone response within 6 months was lower than that of ANAs-negative group(44.0%vs.76.2%),while the rate of hormone non-response was higher than that of ANAs-negative group(20.0%vs.0),and the difference was statistically significant(P<0.05).There were 20 cases of ANAs-positive and 11 cases of ANAs-negative in the 31 AIH-only patients.There was no statistically significant difference in the results of hormone response within 6 months between the two groups(P>0.05).Multifactorial ordered logistic analysis showed that AIH-PBC OS patients were more likely to have a higher likelihood of 6-month hormone non-response rate in ANAs-positive patients,and the difference was statistically different(P<0.05).And there was no significant effect of ANAs type on hormone response outcome in AIH-only patients(P>0.05).Conclusion AIH-PBC OS ANAs-positive patients have a poor hormone response within half a year.In AIH-only patients,ANAs have no significant effect on hormone response results.
ABSTRACT
By summarizing and exploring the theoretic connotation, key of functions and effect mechanism of acupoint compatibility, the effect of acupoint compatibility is concluded as the increase of "effect value" and the expansion of "effect domain". The increase of "effect value" is the concrete embodiment by the value of medical assessment scale, the value of objective index detection in clinical trial and the value of index detection in experiment research. The expansion of "effect domain" is the increase of effect target and the extension of effect scope. The paper interprets the scientific connotation of acupoint compatibility therapy from a new perspective, and emphasizes the innovative approaches to research while bringing forth new ideas on the research method. It is anticipated that a novel breakthrough can be achieved in the study of acupoint compatibility and the improvement of acupuncture-moxibustion efficacy.

Through collecting the 30-year medical experience of professor , the methods of acupuncture manipulation practice were summarized so as to provide the reference for the medical staffs in acupuncture teaching and clinical work. Professor proposes four key elements of acupuncture manipulation practice, i.e. the style of needling practice, the consciousness of needling practice, the strength of needling practice and of needling practice. Acupuncture manipulation is a highly operational clinical skill. The knowledge learning from the experience of the instructors is the important way to improve acupuncture techniques. Besides, beginners need to improve their techniques through learning the theories, a large amount of exercises as well as the self-perception.
ABSTRACT
<p><b>OBJECTIVE</b>To summarize and analyze the compatibility laws for acupoint selection of acupuncture in treating diabetic gastroparasis (DGP) in modern literatures of clinical researches.</p><p><b>METHODS</b>Retrieved were literatures related to treating DGP by acupuncture or acupuncture combined other therapies from PubMed, CNKI, and WF from 1982 to 2014. Analyzed were frequency of acupoint use, meridians selected acupoints belonged to, regions selected, association laws of selected compatible acupoints.</p><p><b>RESULTS</b>Retrieved were 35 with compatibility frequencies more than 15 listed as follows: compatibility frequency of Zusanli (ST36) and Zhongwan (CV12) was 33 (94.29%); compatibility frequency of Zusanli (ST36) and Neiguan (PC6) was 23 (65.71%), compatibility frequency of Zhongwan (CV12) and Neiguan (PC6) was 22 (62.86%), compatibility frequency of Zusanli (ST36), Zhongwan (CV12), and Neiguan (PC6) was 22 (62.86%); compatibility frequency of Zusanli (ST36) and Sanyinjiao (SP6) was 16 (45.71%); compatibility frequency of Zhongwan (CV12) and Sanyinjiao (SP6) was 16 (45.71%); compatibility frequency of Zusanli (ST36), Zhongwan (CV12), and Sanyinjiao (SP6) was 15 (42.86%). Meridians selected acupoints belonged to were sequenced as Foot Yangming Stomach channel, Ren channel, Foot Taiyang Bladder channel, and so on. Acupoints selected were mainly in lower limbs, chest and abdomen, waist and back. The compatibility of Zusanli (ST36) and Zhongwan (CV12) was the most often used with the highest frequency.</p><p><b>CONCLUSIONS</b>The compatibility laws for acupoint selection of acupuncture in treating DGP were mainly dominated as upper-lower selection, three regions selection, local selection, anterior-posterior selection. The compatibility laws for acupoint selection of acupuncture along meridians were mainly dominated as the convergence points and exterior-interior meridian points.</p>

The effectiveness and safety of acupuncture-moxibustion for the treatment of agrypnia was systematically reviewed. The clinical randomized controlled trial (RCT) of acupuncture-moxibustion for agrypnia were collected. The literature and document on acupuncture-moxibustion RCT for the treatment of agrypnia that published from January of 2001 to March of 2012 was searched with computer in PubMed, CNKI, Wanfang and VIP database. According to Cochrane Handbook 5. 1. 0, the bias risk and quality assessment were performed on every included trial and RevMan 5. 0 software was applied to make the Meta analysis. Totally 14 researches were included, involving 2662 cases. The Meta analysis showed that the differences of total effective rate between acupuncture-moxibustion and clinical routine treatment were significant [RR = 1.16, 95% CI (1.12, 1.19), Z = 9.32, P < 0.000 01]. The results indicate that total effective rate of acupuncture-moxibustion for agrypnia is obviously superior to that of clinical routine treatment, meaning clinical efficacy of acupuncture-moxibustion is served credit.

<p><b>OBJECTIVE</b>To investigate the effects of different acupoint combination on prevention and treatment of stress gastric ulcer and to search for a better acupoint combination.</p><p><b>METHODS</b>The theoretical basis and experimental research on treatment of stomach diseases with the combination of he-(sea) points and front-mu points as well as back-shu points and front-mu points are analyzed in this article.</p><p><b>RESULTS</b>The combination of he-sea points and front-mu points is suitable to be applied on acute diseases, febrile diseases and diseases of fu organs. While the combination of back-shu points and front-mu points is more applicable on chronic gastric diseases.</p><p><b>CONCLUSION</b>It is proved that the combination of he-sea point (Zusanli ST 36) and front-mu point (Zhongwan CV 12) has reliable effect on prevention and treatment of stress gastric ulcer.</p>

The modern physicians have different views on acupoint combination and acupuncture-moxibustion prescription and confuse them in clinical practice. It is significant to clarify the conception, connotation and relationship between them so as to normalize the therapeutic program of acupuncture and moxibustion and promote the standardization of acupuncture and moxibustion. Through the collection of relevant literature and analysis on the differences in the understandings among physicians, the conception, connotation and relationship between acupoint combination and acupuncture-moxibustion prescription are summarized. It is viewed that the acupoint combination is based on TCM theory. Under the guide of acupoint selection, in combination of the characters of clinical practice and acupoint indications, two or more than two acupoints of the same function are combined to enhance the collaborative effects of acupoints so as to achieve specific efficacy and improve clinical efficacy. Regarding acupuncture-moxibustion prescription, on the basis of disorder and syndrome differentiation of patients, the concrete therapeutic program is put forward, including acupoint composition and therapeutic method. Acupoint combination is the basic element of acupuncture-moxibustion prescription. Acupuncture-moxibustion prescription is the specific application of acupoint combination.

By reviewing the historical evolution of the major catalogue adjustment of the discipline of acupuncture-moxibustion and Tuina for the graduate students, the current situation of the development of the discipline of acupuncture-moxibustion and Tuina is analyzed deeply in this paper. Based on the basic requirements of the setup and the adjustment of the top-grade discipline in the Discipline Catalogue Setup and Administrative Measures for the Academic Degree Award and Personnel Training (Academic Degree [2009] No. 10 Document) issued by the Academic Degree Committee of the State Council and the Ministry of Education, with the strictly analysis, the author proposed that the discipline of acupuncture-moxibustion and Tuina should be set up to be the top-grade discipline. The author stated that the discipline of acupuncture-moxibustion and Tuina possessed the explicit study object and had many assignable second-grade disciplines. It could be acknowledged extensively in the academic field in terms of the setup of the top-grade discipline. The author expounded adequately the necessity of the stable and urgent requirements for the talents of the discipline of acupuncture-moxibustion and Tuina in the society, which provided the reference evidences for the adjustments of the discipline and major catalogue in the future.

<p><b>OBJECTIVE</b>To assess the efficacy of acupuncture and moxibustion on constipation.</p><p><b>METHODS</b>A retrieval on literatures concerning treatment of constipation with acupuncture was carried out in databases of VIP, CNKI, WANFANG and PubMed. And meta-analyses were conducted on randomized controlled trial (RCT) and controlled clinical trial (CCT) which met the enrolling requirements.</p><p><b>RESULTS</b>A total number of 15 papers involving 1 052 patients were concluded. The result indicated that the curative rate of acupuncture and moxibustion on constipation is better than ordinary medication (RR = 1.92, 95% CI 1.61-2.30, Z = 7.18, P < 0.000 01). And statistical significance can be found between acupuncture-moxibustion treatment and the routine medicine treatment (RR = 1.26, 95% CI 1.18-1.34, Z = 7.26, P < 0.000 01). In the comparison of abdominal pain, defecation duration and general symptom scores, statistical significance can be found between the differences of acupuncture and moxibustion group and control group (abdominal pain: WMD = -0.22, 95% CI-0.32-0.12, Z = 4.28, P < 0.000 1; defecation duration: WMD = -0.47, 95% CI-0.79-0.15, Z = 2.85, P < 0.004; general symptom scores: WMD = -0.41, 95% CI-0.79-0.03, Z = 2.13, P = 0.03).</p><p><b>CONCLUSION</b>Acupuncture and moxibustion is effective to treat constipation. It has certain advantage when compare with the routine medication treatment. However, since singleness still exists in the index of assessment on therapeutic effect of constipation, and the number of RCT and CCT literatures, especially high-quality, large samples and multi-center reports were still not enough, further studies are still necessary for approving the above conclusions.</p>

<p><b>OBJECTIVE</b>To observe the therapeutic effect of tranquilizing and allaying excitement needling method on insomnia of heart and spleen deficiency and the effect of brain blood flow.</p><p><b>METHODS</b>Sixty cases were randomly divided into a tranquilizing and allaying excitement needling method group (observation group) and an eight confluence points selected group (control group), 30 cases in each group. The observation group was treated by acupuncture at Sishencong (EX-HN 1), Shenmen (HT 7), and Sanyinjiao (SP 6) with tranquilizing and allaying excitement needling method. The control group was treated by acupuncture at Shenmai (BL 62) and Zhaohai (KI 6). Their therapeutic effects and changes of brain blood flow were observed.</p><p><b>RESULTS</b>The total effective rate was 93.3% (28/30) in the observation group which was better than 83.3% (25/30) in the control group (P < 0.05). After treatment, the peak velocity of systolic (Vp) and diastolic blood flow velocity (Vd) of middle cerebral artery, basilar artery and vertebral artery were increased in the both groups (P < 0.01, P < 0.05), with more obvious increase in the observation group (P < 0.01, P < 0.05), except the Vd of left vertebral artery.</p><p><b>CONCLUSION</b>Both the tranquilizing and allaying excitement needling method and the eight confluence points selected needling method can improve the clinical signs and symptoms. Meanwhile, the therapeutic effect of acupuncture is related with improvement of brain blood flow. However, the tranquilizing and allaying excitement needling method has better therapeutic effect on insomnia of heart and spleen deficiency.</p>

<p><b>OBJECTIVE</b>To explore the mechanism of acupuncture for treatment of asthma.</p><p><b>METHODS</b>Forty SD rats were randomly divided into 5 groups: blank control group, normal saline control group (NS control group), asthma model group, asthma model with acupuncture group (asthma acupuncture group) and asthma model with binding group (asthma binding group). The asthma acupuncture group was treated with acupuncture at "Dazhui" (GV 14), "Feishu" (BL 13) and "Fengmen" (BL 12); the asthma binding group was only binding without acupuncture and no intervention was given in the other groups. Surfactant protein-A (SP-A) expression was examined by Western-Blot.</p><p><b>RESULTS</b>Airway resistance in asthma model group was significantly higher than that in blank control group and NS control group from 3 to 6 min after asthma provocation (all P<0.01). Western-Blot detection showed that SP-A expression in bronchoalveolar lavage fluid (BALF) of the rats in asthma model group was significantly lower than that in blank control group and NS control group (both P<0.05), and which in asthma acupuncture group was significantly higher than that in asthma model group (P<0.05).</p><p><b>CONCLUSION</b>The mechanism of prevention and treatment of acupuncture for allergic asthma is related to regulating SP-A expression of airway in asthmatic rats.</p>

The article discusses uniform reinforcing-reducing acupuncture manipulation techniques from literature and clinical application, and indicates that the acupuncture manipulation method can be divided into single and double manipulation. The single manipulation should be uniform and soft lifting-thrusting or twirling the needle, and the double manipulation should be uniform and soft lifting-thrusting, in combination with twirling the needle. At the same time, this article puts forward differences between uniform reinforcing-reducing method and qi-inducing method, holding that "qi-inducing method" in Miraculous Pivot is a method promoting flow of qi, and "the uniform needling method" in Yijing Xiaoxue described the methods of inserting needle and manipulating needle, with no meanings of reinforcing and reducing.

The characteristic of "Qinglong Baiwei" needling method is " first toward the left, then toward the right, slowly move or adjust with the hand"; and "Baihu Yaotou" needling method emphasizes "Tuifang Jinyuan" and "Yaozhen "; key point of "Canggui Tanxue" needling method is "Zuanti Sifang"; Chifeng Yingyuan" needling method emphasizes "Siwei Feixuan".

<p><b>OBJECTIVE</b>To define the concept of meridian theory.</p><p><b>METHODS</b>Characteristics of meridian theory were systematically analyzed by the comparison of differences of doctrine and theory concepts and according to practicality, abstraction and logicality of scientific theory.</p><p><b>RESULTS</b>Meridians have various characteristics of scientific theory, so it should be a scientific theory.</p><p><b>CONCLUSION</b>Meridians are not a doctrine, but is a scientific theory. That the meridians are defined as scientific theory is of important practical significance for studies of meridian essence and standardization of acupuncture and moxibustion, and innovation of theoretical system of acupuncture and moxibustion.</p>

<p><b>OBJECTIVE</b>To explore the relation of syndrome differentiation of zang-and fu-organs with combination of acupoints.</p><p><b>METHODS</b>Relation of syndrome differentiation with combination of shu-yuan points, and combination of he-mu points are analyzed through understanding intension of combination of he-mu points, combination of shu-yuan points and syndrome differentiation, in combination with Prof. WANG fu-chun's clinical experience of m any years.</p><p><b>RESULTS</b>Combination of he-mu points and combination of shu-yuan points have better therapeutic effect on diseases of zang- and fu-organs.</p><p><b>CONCLUSION</b>Combination of he-mu points and combination of shu-yuan points are suitable to treatment of diseases an d syndromes of zang- and fu-organs, and have better therapeutic effects under guidance of syndrome differentiation of zang- and fu-organs, with characteristics of less points selected and easily acceptance for the patients.</p>
