Resumen Introducción : Hay información escasa sobre evolución a largo plazo de pacientes hospitalizados por neumo nía COVID-19 moderada (NM) y grave (NG). El objetivo del estudio fue determinar impacto clínico, funcional respiratorio y tomográfico (TACAR) luego del alta a 12 meses del diagnóstico. Métodos : análisis según grupos NM y NG, desatura dores (PD) en prueba de caminata 6 min (PC6M) y patrón tomográfico símil fibrótico (SF). Comparamos resultados a 3 y 12 meses de seguimiento. Resultados : 194 pacientes enrolados, evaluados al año: 103 (53% ): masculinos (62.4%), edad 57.7 ± 10.9 años, comorbilidades (hipertensión arterial 38.8%, dia betes 29.6%, antecedentes respiratorios-AR- 18.4%). Com paramos variables a los 3 y 12 meses (media/DE): CVF 84%(19) a 88(19) (p = 0.01); 27% tuvo CVF<80% al año. En PC6M: 426 metros (108) a 447(92) (p = < 0.01). El 12.8% fue PD al año. NG tuvo mayor SF (40% vs. 27.9% p = 0.021). SF se relacionó con CVF <80% (p = 0.004) en toda la co horte, y NG (p < 0.001). Al año en análisis multivariado se asoció a CVF <80%, AR (OR 4.32, 1.15-16.25), diabetes (OR 2.96, 1.02-8.57) y patrón SF (OR 3.51, 1.25-9.88). PD se asoció a AR (OR 12.2, 2.41-61.85). Discusión : Se observó mejoría en todas las variables al año. Pero al año persisten alteraciones funcionales y tomográficas en <50% de los pacientes. El subgrupo de PD se relacionó a AR. Es importante el seguimiento protocolizado de los pacientes hospitalizados, especial mente los grupos NG, PD y SF.


Abstract Introduction : There is scarce information on longterm evolution of hospitalized patients with moderate (MP) and severe (SP) COVID-19 pneumonia. Objective: to de termine clinical, respiratory function, and tomographic (HRCT) impact after being discharged 12 months after diagnosis. Methods : Analysis according to MP and SP, desatura tor patients (DP) in 6-minute walking test (6MWT) and HRCT fibrotic-like pattern (FLP). Results compared at 3 and 12 months of follow-up. Results : 194 patients enrolled and one year later 103 (53%) were evaluated: gender male (62.4%), age 57.7 ± 10.9 years, comorbidities (arterial hypertension 38.8%, diabetes 29.6%, and respiratory diseases-RD-18.4%). Variables compared 3 months to 12 months (mean/SD): FVC: 84%( 19) to 88%( 19) (p= 0.01). A 27% of patients had FVC<80% at one year. In 6MWT:426 (108) to 447 (92) (p = <0.01). 12.8% are DP in one year. SP had a greater FLP than MP (40% vs. 27.9%, p = 0.021). The FLP group was related to FVC < 80% (p = 0.004) in all patients but only in SP (p < 0.001). After one year, in multivariate analysis, FVC < 80% was associated with RD (OR 4.32, 1.15-16.25), diabetes (OR 2.96, 1.02-8.57) and FLP (OR 3.51, 1.25-9.88). DP were associated with RD (OR 12.2, 2.41-61.85). Discussion : Improvement was observed in all vari ables when comparing 3 to 12 months. However, after one year, functional and tomographic alterations persist in less than 50% of patients. DP subgroup was related to RD. Protocolled follow-up of hospitalized patients is important, especially in SP, DP, and FLP groups.

ABSTRACT

El síndrome de linfocitosis infiltrativa difusa se produce en asociación con la infección por virus de la inmunodeficiencia humana; requiere cumplir con los criterios diagnósticos y descartar otras patologías infecciosas y autoinmunes. Se presenta el caso de una mujer de 47 años que consultó por edema parotídeo bilateral, síndrome sicca, tos y síndrome de impregnación. Se observó en la tomografía de tórax infiltrado en «vidrio esmerilado¼, parcheado y bilateral. Se realizó diagnóstico de virus de la inmunodeficiencia humana positivo y fibrobroncoscopia con lavado broncoalveolar sin desarrollo de patógenos. Se interpreta como neumonía intersticial linfoidea asociada a síndrome de linfocitosis infiltrativa difusa. Se inició terapia antirretroviral con buena evolución y desaparición de los síntomas y de los infiltrados pulmonares.


Diffuse infiltrative lymphocytosis syndrome occurs in association with HIV infection; it requires meeting the diagnostic criteria and ruling out other infectious and autoimmune pathologies. We present the case of a 47-year-old woman who consulted for bilateral parotid edema, sicca syndrome, cough and impregnation syndrome, which was observed in the chest tomography infiltrated in ground glass, patched and bilateral. A diagnosis of HIV positive and fiberoptic bronchoscopy with bronchoalveolar lavage was made without the development of pathogens. It is interpreted as lymphoid interstitial pneu monia associated with DILS. Antiretroviral therapy was started with good evolution and disappearance of symptoms and pulmonary infiltrates.

Resumen OBJETIVO: Evaluar los conocimientos, satisfacción y nivel de confianza de los médicos residentes de Ginecología y Obstetricia luego de haber participado en un taller de simulación de reparación de episiotomías y desgarros perineales severos. MATERIALES Y MÉTODOS: Estudio observacional y transversal efectuado en residentes de la especialidad de Ginecología y Obstetricia en junio del 2022. Se aplicaron cuestionarios de conocimientos antes y después del taller, una encuesta de satisfacción y nivel de confianza, así como una lista de cotejo para valorar el desempeño en la ejecución de la técnica de reparación de episiotomías y desgarros. Se utilizó estadística descriptiva y prueba de t pareada para la comparación antes y después de las muestras relacionadas. Se consideró significancia estadística una p < 0.05. RESULTADOS: Se incluyeron 37 médicos residentes de todos los grados académicos. La media de la evaluación preprueba fue de 14.89 ± 0.54 puntos y 15.81 ± 0.41 puntos en la posprueba (p = 0.233). Durante la práctica de reparación de la episiotomía, 34 de los 37 residentes reconocieron estructuras anatómicas y todos seleccionaron adecuadamente el material de sutura. En el taller de desgarros perineales 25 de 37 reconocieron las estructuras anatómicas y todos, menos uno, seleccionaron adecuadamente el material. CONCLUSIONES: El taller con modelos animales de simulación tiene un alto grado de satisfacción y nivel de confianza; sin embargo, en este estudio no se observó mejoría en los conocimientos. Este efecto positivo en los médicos en vías de especialización permite identificar, de manera oportuna, las lesiones del esfínter anal, llevar a cabo una correcta reparación y, en consecuencia, disminuir la incidencia de disfunciones asociadas con el traumatismo obstétrico perineal.


Abstract OBJECTIVE: To assess the knowledge, satisfaction and level of confidence of Gynaecology and Obstetrics residents after participating in a simulation workshop on episiotomy and severe perineal tear repair. MATERIALS AND METHODS: Observational and cross-sectional study carried out in residents of the speciality of Gynaecology and Obstetrics in June 2022. Knowledge questionnaires were administered before and after the workshop, a satisfaction and confidence level survey, as well as a checklist to assess the post-workshop episiotomy and tear repair technique. Descriptive statistics and paired t-test were used for pre- and post-test comparison. A p < 0.05 was considered statistically significant. RESULTS: Thirty-seven resident physicians of all academic grades were included. The mean pre-test assessment was 14.89 ± 0.54 points and 15.81 ± 0.41 points at post-test, p = 0.233. During episiotomy repair practice, 34 of the 37 residents recognized anatomical structures and all selected suture material appropriately. In the perineal tears workshop 25 of 37 recognized anatomical structures and all but one selected material appropriately. CONCLUSIONS: The workshop with simulation animal models has a high satisfaction and confidence level; however, no improvement in knowledge was observed in this study. This positive effect on trainee doctors allows early identification of anal sphincter injuries, correct repair and consequently a decrease in the incidence of dysfunction associated with perineal obstetric trauma.

ABSTRACT

RESUMEN La tuberculosis pericárdica es una manifestación poco frecuente de la tuberculosis extrapulmonar. Se presenta de manera insidiosa e inespecífica, lo que dificulta el diagnóstico, retrasa el tratamiento, y lleva a complicaciones graves, como el derrame pericárdico con signos de taponamiento cardíaco o pericarditis constrictiva. Se describe el caso de una paciente femenina de 18 años, embarazada, con antecedente de contacto estrecho con familiar con tuberculosis pulmonar, que consultó al servicio de urgencias por disnea. Entre los estudios solicitados, el ecocardiograma evidenció la presencia de un derrame pericárdico con compresión de cavidades, por lo que requirió pericardiocentesis. El cultivo del líquido pericárdico informó Mycobacterium tuberculosis y se inició trata miento antituberculosis y corticoides. Evolucionó con recurrencia del derrame a pesar de tratamiento médico adecuado y realización de ventana pericárdica.


ABSTRACT Pericardial tuberculosis (TBP) is a rare manifestation of extrapulmonary tuberculosis (TB). It presents insidiously and nonspecifically, making diagnosis difficult and delaying treatment, leading to serious complications such as pericardial effusion with signs of cardiac tamponade or constrictive pericarditis. The case of an 18-year-old female patient, pregnant, with a history of close contact with a relative with pulmonary TB is described; she consulted the Emergency Service for dyspnea. Among the studies requested, the echocardiogram showed pericardial effusion with compression of cavities, requiring pericardiocentesis. Pericardial fluid culture revealed Mycobacterium tuberculosis and antituberculous treat ment plus corticosteroids were started. It evolved with a recurrence of effusion despite adequate medical treatment and the performance of a pericardial window.

ABSTRACT

RESUMEN El síndrome antifosfolípido (SAF) es una entidad caracterizada por fenómenos trom bóticos (arteriales y/o venosos), pérdidas fetales y elevación sérica persistente de anticuerpos antifosfolípidos. Las complicaciones pulmonares más frecuentes del SAF son: tromboembolismo pulmonar, hipertensión pulmonar tromboembólica. La hemorragia alveolar es una ma nifestación infrecuente y potencialmente mortal (SAF catastrófico). El diagnóstico se confirma cuando en una muestra tomada mediante lavado bronquioalveolar (BAL), más del 20 % de los macrófagos son positivos para hemosiderina. Los hallazgos radiográfi cos más comúnmente muestran opacidades en vidrio deslustrado o de consolidación que suelen ser difusas, bilaterales más centrales que periféricas. La DLCO es otro método diagnóstico, con valores por encima del 120 % del valor predicho. Presentamos el caso de un paciente con SAF con antecedentes de trombosis venosa profunda (TVP) y tromboembolismo pulmonar (TEP) anticoagulado, que ingresa con diagnóstico de hemorragia alveolar (HA).


ABSTRACT Antiphospholipid syndrome (APS) is an entity characterized by thrombotic phenomena (arterial and/or venous), fetal losses, and persistent increase in the serum level of antiphospholipid antibodies. The most common pulmonary complications of APS are pulmonary thromboembolism and thromboembolic pulmonary hypertension. Alveolar hemorrhage is a rare, potentially life-threatening manifestation (catastrophic APS). The diagnosis is confirmed when more than 20 % of the macrophages in a sample taken by bronchoalveolar lavage (BAL) are positive for hemosiderin. Radiographic findings most commonly show ground-glass opacities or consolidations that are usually diffuse, bilateral, more central than peripheral. The DLCO is another diagnostic method, with values above 120 % of the predicted value. We present the case of a patient with APS with a history of deep vein thrombosis (DVT) and anticoagulated pulmonary thromboembolism (PTE), who was admitted with a diag nosis of alveolar hemorrhage (AH).

Resumen Introducción: Hay información escasa sobre la evolución de los enfermos hospitalizados por neumonía por COVID-19 a mediano plazo. El objetivo de este estudio fue determinar en pacientes con neumonía moderada (NM) o grave (NG) por COVID-19, el impacto clínico, funcional respiratorio y tomográfico a los 6 meses. Métodos: Se realizó análisis según grupos de NM y NG, desaturadores (PD) en la prueba de cami nata de 6 minutos y presencia del patrón tomográfico de alta resolución (TACAR) símil fibrótico. Se compararon los resultados a los 3 y 6 meses. Resultados: Se incluyeron 129 pacientes, edad 57±11 años, comorbilidades frecuentes (hipertensión arterial 38.1%, diabetes 30.4% y respiratorias 18.6%). Al comparar 3 y 6 meses se ob servó mejoría de la calidad de vida en NM y NG. Los PD presentaron menor metraje caminado, peor calidad de vida y mayor presencia de patrón símil fibrótico. Este patrón se relacionó con la caída de la FVC 80% en NM y NG (p = 0.048 y p = 0.007), y con PD (p = 0.002). En el análisis multivariado, el patrón símil fibrótico s e asoció a la reducción de la CVF con OR = 4.44 (1.94-10.18, p<0.01) y a la desaturación, OR = 5.01 (1.63-15.42, p<0.01). En esta cohorte se observó mayor compromiso funcional y tomográfico en los PD. El patrón símil fibrótico se relacionó con peor evolución funcional y oximétrica. Discusión: Es importante el seguimiento de los pacientes con NG, los PD y los que presentan patrón fibrótico al alta de neumonía por COVID-19.


Abstract Introduction: There is scarce information about middle-term evolution of hospitalized patients who suffer from pneumonia caused by COVID-19. The objective of this study is to determine the clinical, respiratory, tomographic and functional impact on COVID-19 patients with moderate (MP) to severe (SP) pneumonia after six months of acute infection. Methods: Analysis was carried out by MP and SP groups, desaturators during the 6-minute-walking test and the presence of fibrotic like pattern on HRCT. Outcomes at 3 and 6 months were compared. Results: The analysis included 129 patients, between 57 ± 11 years old. Frequent comorbidities were: arterial hypertension 38.1%, diabetes 30.4%, respiratory 18.6%). Comparing 3 and 6 months, improvement in quality of life was observed in MP and SP. The DP walked less meters in the MWT, worsened life quality and more fibrotic like pattern. The fibrotic pattern was related to the fall of CVF < 80% on MP and SP (p = 0.048 and p = 0.007), and with DP (p = 0.002). On multivariated analysis, the fibrotic like pattern was associated to the reduction of CVF with OR = 4.44 (1.94-10.18, p<0.01) and desaturation OR = 5.01(1.63-15.42, p < 0.01). On this cohort it was observed more functional and tomographic compromise on the DP The fibrotic like pattern was related to worse functional evolution and oximetry. Discussion: Follow-up after discharge of COVID-19 patients with SP, DP or fibrotic changes in HCRT is underlined.

ABSTRACT

Resumen El disfrute es una de las razones más fuertes para involucrarse en una actividad deportiva. ¿Pero cómo se puede definir conceptual y operacionalmente este constructo? El objetivo del presente estudio fue indagar con enfoque mixto los componentes del disfrute en el deporte. En el primer estudio participaron deportistas federados del Área Metropolitana de Buenos Aires (AMBA, Argentina) (n = 9). Se utilizó una entrevista semi-estructurada y se identificaron las categorías que describen la experiencia de disfrute, tales como, afecto positivo, involucramiento y experiencia autotélica. Los participantes refirieron además sentirse realizados a nivel personal. Por otro lado, en el segundo estudio participaron 362 jóvenes (57.2 % hombres y 42.8 % mujeres) de 14 a 29 años de distintos clubes del AMBA. Para este estudio se operacionalizaron las categorías del primer estudio, a través de 29 ítems, con una escala Likert de cinco puntos de acuerdo. El análisis factorial exploratorio arrojó dos factores que explicaron el 58.8 % de la varianza a través de 17 ítems. La confiabilidad de cada subescala fue alta. El disfrute puede conocerse a través de dos subescalas: afecto positivo . involucramiento. Un factor adicional emergió de los análisis: el sentido de realización personal. Los factores del disfrute correlacionaron positivamente con este último, con el compromiso en el deporte y con el Flow total. Posteriores estudios permitirán conocer otros indicadores de validez de la escala y su uso en diversos contextos.


Abstract Enjoyment is one of the strongest motives to engage in a sports activity, but how can this construct be conceptually and operationally defined? This study aimed to investigate, with a mixed approach, the components of enjoyment in sport. In the first study, federated athletes from the Metropolitan Area of Buenos Aires (MABA, Argentina) participated (n = 9). A semi-structured interview was conducted, and the categories that describe the enjoyment experience were identified, such as positive affect, involvement, and autotelic experience. Participants also reported feeling fulfilled on a personal level. On the other hand, the second study involved 362 young people (57.2% men and 42.8% women) aged 14 to 29 from different MABA clubs. For this study, the categories of the first study were operationalized through 29 items, with a Likert-type scale of five points of agreement. The exploratory factorial analysis yielded two factors that explained 58.8% of the variance through 17 items. The reliability of each subscale was high. Enjoyment can be assessed through two subscales: positive affect and involvement. An additional factor emerged from the analyses: the sense of personal fulfillment. The factors of enjoyment correlated positively with the latter, with the engagement in the sport and with the total flow. Subsequent studies will reveal other indicators of the validity of the scale and its use in various contexts.

ABSTRACT

Resumen En este trabajo se presenta una reflexión sobre una experiencia educativa con comunidades ubicadas en el casco urbano del municipio de Turbo (Colombia), que no tienen acceso al agua potable. Los participantes son 18 líderes comunitarios, 149 familias de tres barrios y profesionales de dentro y fuera de la región. Este proceso tuvo como objetivo el fortalecimiento de las prácticas del uso cotidiano y conservación del agua en las comunidades, y es respuesta a una investigación previa que analizó algunos retos que enfrentan las personas para acceder a fuentes de agua suficientes y adecuadas para suplir sus necesidades. En esta experiencia se privilegia el diálogo como posibilidad para el reconocimiento de las condiciones de acceso al agua potable y la construcción de acciones para la gestión y defensa del derecho al agua.


Abstract This paper presents a reflection on an educational experience with communities located in the urban area of the municipality of Turbo (Colombia), which do not have access to potable water. The participants are eighteen community leaders, one hundred and forty-nine families from three neighborhoods and professionals from inside and outside the territory. The main objective was to strengthen the practices of daily use and conservation of water in the communities. This process is related to a previous research that analyzed the challenges faced by people to access to sufficient and adequate water sources to meet their needs, among others. In this experience, dialogue was the possibility to recognize the conditions to have access to potable water, and to define collective actions for the management and defense of the right to water.

ABSTRACT

Resumen ANTECEDENTES: La displasia mesenquimal placentaria es una anomalía de la vasculatura y del tejido conectivo placentario que altera la estructura y la función de las vellosidades, se asocia con un desenlace perinatal adverso. CASO CLÍNICO: Paciente de 21 años, primigesta, con 15.2 semanas de embarazo establecidas por fetometría, con placenta corporal anterior, de morfología anormal, con múltiples imágenes hipoecogénicas que semejan racimos de uvas, sin flujo vascular en el Doppler color. La valoración ecográfica de las 33 semanas reportó fetometría para 27.2 semanas, anhidramnios y placentomagelia, con alteración en los parámetros de la flujometría Doppler. En la prueba sin estrés se diagnosticó restricción del crecimiento intrauterino estadio IV. La imagen del feto fue heterogénea en el flanco derecho de 4.7 x 4.1 cm. El embarazo finalizó por cesárea con una recién nacida con Apgar 8-9, de 930 gramos, placenta agrandada con vellosidades hidrópicas. El estudio histopatológico reportó: displasia mesenquimal placentaria. La resección del tumor abdominal fue electiva, por vía laparoscópica, al cuarto mes de vida, con diagnóstico histopatológico de hamartoma mesenquimal hepático. CONCLUSIÓN: La displasia mesenquimal placentaria es una anomalía placentaria benigna, relacionada con restricción del crecimiento intrauterino de inicio temprano que puede evolucionar, incluso, a la muerte del feto por hipoxia crónica debido a las anomalías en la vasculatura microscópica que afectan el desarrollo de las vellosidades terminales e interfiere en el intercambio de gases, nutrientes y electrolitos en el espacio intervelloso.


Abstract BACKGROUND: Placental mesenchymal dysplasia is an anomaly of the vasculature and placental connective tissue which alters the structure and function of the villi, being associated with an adverse perinatal outcome. CLINICAL CASE: Patient 21 years old, primigestation, with 15.2 weeks of pregnancy established by fetometry, with anterior body placenta, abnormal morphology, with multiple hypoechogenic images resembling clusters of grapes, without vascular flow in color Doppler. Ultrasound evaluation at 33 weeks, reporting fetometry for 27.2 weeks, anhydramnios and placentomagelia, with alteration in Doppler parameters, as well as in a non-stress test diagnosing fetal growth restriction stage IV. Fetus with heterogeneous image on the right flank of 4.7 x 4.1 cm. Abdominal pregnancy interruption was decided, obtaining a live female newborn, Apgar 8/9, weight 930 grams, enlarged placenta with hydropic villi. The histopathology study reports placental mesenchymal dysplasia. Abdominal tumor resection was performed electively by laparoscopy at 4 months of age with a histopathological diagnosis of hepatic mesenchymal hamartoma. CONCLUSION: Placental mesenchymal dysplasia is a benign placental anomaly, related to early-onset intrauterine growth restriction that can even progress to fetal death by chronic hypoxia due to abnormalities in the microscopic vasculature that affect the development of the terminal villi and interfere with the exchange of gases, nutrients and electrolytes in the intervillous space.

ABSTRACT

Objective: This in vitro study evaluated the effect of neolignan-containing solutions on dentin biomodification previously applied to the bonding procedure in adhesive restorations. Material and Methods: Neolignans, dehydrodieugenol B­CP1 and dehydrodieugenol B methyl ether­CP2, were isolated from Nectandra leucanthaand two aqueous solutions containing 0.13% neolignans, 0.2% propylene glycol and 3.0% ethanol were prepared. Bovine teeth were ground flat to obtain 2-mm thick specimens which received resin composite restorations (N=10). The neolignan solutions were applied before the bonding procedure (60 s). Experimental groups were: control, untreated group, 0.12% chlorhexidine gel, 0.13% CP1 solution, and 0.13% CP2 solution. A push-out bond strength test was conducted (0.5 mm/min). Bovine tooth sections (0.5×1.7×7.0 mm) were also obtained to assess the modulus of elasticity and mass change after treatment (N=15). A three-point bending test evaluated the elastic modulus of fully demineralized dentine beams after immersion in the solutions. The data were statistically analyzed (α = 0.05). Results: The bond strength of the restorations to dentin was significantly improved by the treatment with neolignan-containing solutions, irrespective of the evaluation time (p<0.05). After 6 months, a significant reduction in the bond strength was observed in the groups treated with the solutions (p>0.05), but the means were significantly higher than the control groups (p<0.05). The elastic modulus of demineralized dentin was significantly improved after the treatment with the solutions (p<0.05). All groups lost mass weight. Conclusion: The solutions improved the in vitro longevity of bonded restorations, possibly due to the dentin biomodification effect of the neolignans.(AU)


Objetivo: Este estudo in vitro avaliou o efeito de soluções contendo neolignanas na biomodificação da dentina aplicadas previamente à restaurações adesivas. Material e Métodos: Neolignanas, desidrodieugenol B­CP1 e éter metílico de desidrodieugenol B-CP2, foram isolados da espécie Nectandra leucantha e duas soluções aquosas contendo 0,13% de neolignanos, 0,2% de propilenoglicol e 3,0% de etanol foram preparadas. Dentes bovinos foram lixados para obter espécimes de 2 mm de espessura e preparos cavitários restaurados com resina composta (N=10). As soluções foram aplicadas em dentina antes do procedimento adesivo (60 s). Os grupos experimentais foram: controle, grupo não tratado, gel de clorexidina 0,12%, solução de CP1 a 0,13% e solução de CP2 a 0,13%. Foi realizado o teste de resistência de união push-out (0,5 mm/min). O módulo de elasticidade e a alteração de massa após tratamento da dentina (0,5×1,7×7,0 mm) foram também avaliados em teste de flexão de três pontos (N=15). Os dados foram analisados estatisticamente (α=0,05). Resultados: A resistência de união das restaurações à dentina melhorou significativamente com o tratamento com as soluções, independentemente do tempo de avaliação (p<0,05). Após 6 meses, foi observada redução significativa da resistência de união nos grupos tratados com as soluções (p>0,05), com médias significativamente maiores do que nos grupos controle (p<0,05). O módulo de elasticidade da dentina desmineralizada aumentou significativamente após tratamento com as soluções (p<0,05). Todos os grupos perderam massa, independentemente do tratamento. Conclusão: As soluções melhoraram in vitroa longevidade das restaurações adesivas, possivelmente devido ao efeito biomodificador da dentina das neolignanas(AU)

Introducción: A nivel mundial la tasa de natalidad desde las últimas décadas ha venido en declive, por lo que adquiere relevancia investigar sus cambios. Objetivos: Cuantificar la tendencia de nacimientos en Chile según rango etario de la madre y sexo biológico del recién nacido en el periodo 2015-2020. Metodología: Se realizó un estudio observacional y ecológico, con los datos obtenidos del banco mundial y DEIS, para la población y nacimientos respectivamente. Los datos se dividieron según las edades maternas en tres grupos: <18 años, 18-34 años y ≥ 35 años. No se requirió comité de ética debido a que no se interviene con personas. Los autores no presentan conflicto de interés. Resultados: Se observó una disminución de los nacimientos del 22,6%, pasando de 244.626 el 2015 a 189.250 el 2020. Los nacimientos en el grupo etario de madres menores a 18 años disminuyeron un 82,13%, reduciéndose de 17.505 a 3.129, el rango de edad materna correspondiente al segundo grupo de 18 a 34 años, ubicó la mayor cantidad de nacimientos, manteniéndose alrededor de un 77,75% (neto), el rango ≥ 35 años, presentó un alza de un 17%, aumentando de 34.648 nacimientos a 40.539. Discusión: En Chile hay un descenso en los nacimientos dado a que la maternidad se está postergando, lo cual puede deberse a la educación sexual implementada en el país y la mayor participación de mujeres en ámbito laboral, esto sugiere una transición de una demografía estable a una regresiva.


Introduction: The birth rate has been declining worldwide since the last decades, so it is important to investigate its changes. Objectives: To quantify the trend of births in Chile according to age range of the mother and biological sex of the newborn in the period 2015-2020. Methodology: An observational and ecological study was conducted, with data obtained from the World Bank and DEIS, for population and births respectively. Data were divided according to maternal ages into three groups: <18 years, 18-34 years and ≥ 35 years. No ethics committee was required due to the fact that we did not intervene with individuals. The authors have no conflict of interest. Results: A decrease in births of 22.6% was observed, from 244,626 in 2015 to 189,250 in 2020. Births in the age group of mothers under 18 years decreased by 82.13%, decreasing from 17,505 to 3,129, the maternal age range corresponding to the second group from 18 to 34 years, located the highest number of births, remaining around 77.75% (net), the range ≥ 35 years, presented a rise of 17%, increasing from 34. Discussion: In Chile there is a decrease in births due to the fact that motherhood is being postponed, which may be due to the sexual education implemented in the country and the greater participation of women in the labor market, suggesting a transition from a stable demography to a regressive one.

ABSTRACT

El aneurisma micótico surge de la destrucción infecciosa de la pared arterial y se diagnostica cuando hay un aneurisma en el contexto de inflamación y hemocultivos o biopsia positivos, aunque esto no es un requisito excluyente. Los agentes etiológicos de mayor frecuencia son bacterias (Staphylococcus, Salmonella)2 pero también se han notificado hongos (Aspergillus niger). La tuberculosis puede afectar cualquier órgano de la economía, el compromiso arterial es una complicación rara y potencialmente mortal, puede ocurrir por extensión directa de un foco adyacente o por diseminación hematógena o linfangítica de lesiones primarias. El compromiso en los grandes vasos puede presentarse de diversas formas, incluyendo pólipos, aortoarteritis estenosante y formación de aneurismas o pseudoaneurismas, siendo esta última la más común. Presentamos el diagnóstico de aneurisma de aorta abdominal infrarrenal con trombo intramural y pseudoaneurisma de aorta renal izquierda como hallazgo incidental en una paciente con tuberculosis ganglionar.


The mycotic aneurysm arises from the infectious destruction of the arterial wall and is diagnosed when there is an aneurysm within the context of inflammation and positive blood cultures or biopsies, though this is not an essential requirement. The most common etiologic agents are bacteria (Staphylococcus, Salmonella)2, but fungi have also been reported (Aspergillus niger). Tuberculosis may affect any organ of the body; arterial compromise is a rare and potentially mortal complication that may occur by direct extension of an adjacent infected site or by bloodstream or lymphangitic dissemination of primary lesions. The great vessels may be compromised in different ways, including polyps, stenotic aortoarteritis and formation of aneurysms or pseudoaneurysms, being the latter the most common one. We present the diagnosis of infrarenal abdominal aortic aneurysm with intramural thrombus and left renal aortic pseudoaneurysm as incidental finding in a patient with nodal tuberculosis.

ABSTRACT

Mouthwashes are an effective complementary method in the biofilm prevention and chemical control, when the practice of mechanical removal is insufficient. Among the active compounds most used in antiseptics are chlorhexidine, cetylpyridinium chloride, triclosan and essential oils. In order to reduce undesirable effects, such as teeth and restorations staining and the supragingival calculus formation, observed mainly after use for long periods, natural products of plant origin have been extensively studied and incorporated into oral hygiene formulations. Commonly used as a functional food or spice in world gastronomy, Coriandrum sativum (C. sativum), popularly known as coriander, is a natural therapeutic resource with several properties, including antifungal and antioxidant ones. Thus, the objective of this study was to carry out a literature review of articles published in the last 10 years that relate natural products, dentistry and C. sativum, in order to understand the role of naturally-occurring agents with potential therapeutic applications in the dental fields, as well as to evaluate the application of C. sativum in the treatment of oral diseases. Therefore, a bibliographic search was carried out in the ScienceDirect and PubMed databases, obtaining 20 articles. It was possible to confirm the antifungal properties and the mode of action of the essential oil from C. sativum in Candida species, as well as its relatively low cytotoxicity in human cells. These findings should encourage further studies for the development of mouthwashes based on C. sativum as an alternative to mouthwashes available on the market. (AU)


Os enxaguatórios bucais constituem um método complementar eficaz na prevenção e controle químico do biofilme, quando a prática de remoção mecânica se mostra insuficiente. Dentre os compostos ativos mais utilizados em antissépticos estão a Clorexidina, o cloreto de cetilpiridíneo, o triclosan e os óleos essenciais. Buscando a diminuição de efeitos indesejáveis, como manchamento de dentes e restaurações e a formação de cálculo supragengival, observados principalmente após a utilização por longos períodos, produtos naturais de origem vegetal têm sido amplamente estudados e incorporados nas formulações para higiene bucal. Comumente utilizado como alimento funcional ou especiaria na gastronomia mundial, o Coriandrum sativum (C. sativum), popularmente conhecido por coentro, apresenta-se como um recurso terapêutico natural com diversas propriedades, incluindo antifúngica e antioxidante. Assim, o objetivo deste estudo foi realizar uma revisão de literatura de artigos publicados nos últimos 10 anos que relacionam produtos naturais, a odontologia e o C. sativum, a fim de compreender o papel de agentes de ocorrência natural em aplicações terapêuticas potenciais na área odontológica, bem como avaliar a aplicação do C. sativum no tratamento de doenças bucais. Para isso foi realizada uma busca bibliográfica nas bases de dados ScienceDirect e PubMed, obtendo-se 20 artigos. Foi possível confirmar as propriedades antifúngicas e o modo de ação do óleo essencial de C. sativum em espécies de Candida, bem como sua citotoxicidade relativamente baixa em células humanas. Esses achados devem incentivar novos estudos para o desenvolvimento de enxaguatórios bucais baseados em C. sativum como alternativa aos enxaguatórios bucais disponíveis no mercado. (AU)

Resumen La determinación de adenosina deaminasa (ADA) es de utilidad en países donde la prevalencia de tuberculosis (TB) es alta/moderada. El objetivo del trabajo fue analizar el punto de corte de ADA en pacientes con tuberculosis en nuestra institución. Se incluyeron todos los pacientes con derrame pleural, desde mayo del 2016 a marzo 2019. Se excluyeron aquellos con serología positiva para HIV y los trasudados. Todos fueron agrupados en los siguientes diagnósticos: TB, neoplasia, paraneumónico u otro de causa no aclarada. Se efectuó determinación de ADA y cultivo para micobacterias a todas las muestras de líquido pleural. Se realizó una curva ROC para establecer el mejor punto de corte de ADA para el diagnóstico de TB. Se recolectó información de 309 pacientes, se incluyeron 220, 87 tuvieron diagnóstico de TB. El mejor punto de corte obtenido para el ADA fue de 52 U/l, con una sensibilidad de 79% y una especificidad de 90%. El área bajo la curva (AUC) resultó de 0.90. En los menores de 40 años, el mejor punto de corte fue de 41 U/l, por debajo del obtenido para el total de la población en estudio. En nuestra población, el punto de corte para el valor de ADA en exudado pleural para el diagnóstico de tuberculosis de 52 U/l, presentó la mayor especificidad y sensibilidad.


Abstract The determination of adenosine deaminase (ADA) is useful in countries where the prevalence of tuberculosis (TB) is high/ moderate. The objective of this study was to analyze the cut-off point for ADA in patients with TB at our institu tion. All patients with pleural effusion were included, from May 2016 to March 2019, except those with positive serology for HIV and transudates. They were grouped into the following diagnoses: TB, neoplasm, parapneumonic or other with an unclear cause. ADA determination and culture for mycobacteria were performed on all samples of pleural fluid. A ROC curve was performed to establish the best ADA cut-off point for the diagnosis of TB. Information was collected from 309 patients; 220 were included and 87 had a diagnosis of TB. The best cut-off point obtained for ADA was 52 U/l, with a sensitivity of 79% and a specificity of 90%. The area under the curve (AUC) was 0.90. In patients under 40 years old the best cut-off point was 41 U/l, below that obtained for the total study population. In our population the cut-off point for the ADA value in pleural exudate for the diagnosis of tuberculosis of 52 U/l presents the highest specificity and sensitivity.

Abstract Introduction: The multiple academic demands in the university environment generate high levels of stress among students. Although said stress may be useful to meet such demands, in many cases, it can have a negative impact on their health. Objective: To assess whether the implementation of active meditation in the daily routine of health sciences university students impacts their physical and psycho-emotional stress indicators. Materials and methods: Exploratory and quantitative pilot study carried out in a sample of 22 university students who agreed to participate in a 3-month active meditation elective course (18 sessions). Participants were asked to fill out the Global State Perception Questionnaire (CPGE) ex-ante and ex-post taking the course. Results: 18 participants completed the GSPC ex-ante and ex-post, but only 16 were valid for data processing. Most students reported an improvement in their perception of coping with different stressful situations after completing the course. For example, 87.5% reported remaining in a relaxed state during stressful situations, 81% stated feeling decreased mental exhaustion, and 81% said they were better at handling stressful situations. Conclusions: The implementation of active meditation in the daily routine of this group of university students decreased their physical and psycho-emotional indicators of stress. Therefore, it is necessary to carry out new studies with larger samples to confirm the positive effect of this type of course on the health of university students, which will undoubtedly lead to better academic performance as a result of a better mental state.


Resumen Introducción. Las múltiples exigencias académicas a las que los estudiantes universitarios se enfrentan les generan altos niveles de estrés; si bien el estrés puede ayudarles a afrontar tales exigencias, en muchos casos este puede causarles diversos problemas de salud. Objetivo. Evaluar si la implementación de la meditación activa en la rutina diaria de estudiantes universitarios de las ciencias de la salud tiene un efecto en sus indicadores físicos y psicoemocionales. Materiales y métodos. Estudio piloto exploratorio y cuantitativo realizado en una muestra de 22 estudiantes universitarios que aceptaron participar en un curso electivo de meditación activa de 3 meses (18 sesiones). A los participantes se les solicitó diligenciar el Cuestionario de Percepción Global de Estado (CPGE) ex ante y ex post al curso. Resultados. 18 participantes completaron el CPGE ex ante y ex post, pero solo 16 fueron válidos para el procesamiento de datos. La mayoría de los estudiantes reportó una mejora respecto a su percepción sobre cómo enfrentar diferentes situaciones estresantes luego de completar el curso, por ejemplo, el 87.5% manifestó permanecer en un estado de relajación en situaciones estresantes; el 81%, una disminución del agotamiento mental, y el 81%, un mejor manejo de las situaciones estresantes. Conclusiones. La incorporación de la meditación activa en la rutina diaria de estos estudiantes disminuyó sus indicadores físicos y psicoemocionales de estrés, por lo que es necesario realizar nuevos estudios con muestras más grandes que permitan confirmar el efecto positivo de este tipo de cursos en la salud de los estudiantes universitarios, lo que sin duda repercutirá en un mejor rendimiento académico producto de un mejor estado mental.

ABSTRACT The aim of the current study was to compare the effect of two lithium disilicate ceramic processing techniques (pressing and CAD/CAM) on the degree of conversion of resin cement when it is photoactivated and the microshear bond strength of resin cement to ceramics. Two ceramic discs were manufactured: one by the Press technique and another by CAD/CAM technique. Five Variolink Veneer resin cement samples were photoactivated through each ceramic disc and subjected to attenuated total reflectance Fourier transform infrared spectroscopy to evaluate their degree of conversion. To evaluate microshear bond strength, 20 ceramic bars were prepared: 10 using the Press technique and 10 using the CAD/CAM technique. The bars were air abraded with 50-μm Al2O3 particles, treated with 10% hydrofluoric acid for 20 s and subjected to RelyX Ceramic Primer application. Adper Scotchbond Multi-Purpose Adhesive was applied to the bars and cured for 10 s. Two 1-mm-height plastic tubes were placed on each bar and filled with Variolink Veneer resin cement. Each cylinder was cured for 20 s. Then the plastic tubes were removed and the microshear bond strength of each cylinder was tested. Data for degree of conversion (%) and microshear bond strength (MPa) were subjected to Student's t-test (α = 0.05). Both degree of conversion of resin cement photoactivated through ceramic and microshear bond strength of resin cement to ceramic were statistically higher in the group in which ceramic was prepared by CAD/CAM technique (44.74% and 22.18 MPa) than in the group in which the ceramic was prepared by the Press technique (25.71% and 19.83 MPa). The lithium disilicate ceramic processing techniques affected the degree of conversion of resin cement when it is photoactivated through the ceramics and the microshear bond strength of resin cement to ceramics.


RESUMO O objetivo do presente estudo foi comparar o efeito de duas técnicas de processamento da cerâmica de dissilicato de lítio (prensada e CAD/CAM) no grau de conversão do cimento resinoso quando fotoativado e na resistência de união ao microcisalhamento do cimento resinoso às cerâmicas. Foram fabricados dois discos de cerâmica: um pela técnica prensada e outro pela técnica CAD/CAM. Cinco amostras de cimento resinoso Variolink Veneer foram fotoativadas através de cada disco cerâmico e submetidas à espectroscopia no infravermelho por transformada de Fourier com reflectância total para avaliar seu grau de conversão. Para avaliar a resistência de união ao microcisalhamento, foram preparadas 20 barras de cerâmicas: 10 utilizando a técnica prensada e 10 utilizando a técnica CAD/CAM. As barras foram tratadas com partículas de Al2O3 (50 μm), com ácido fluorídrico a 10% por 20 s e submetidas à aplicação do RelyX Ceramic Primer. Em seguida, o adesivo multiuso Adper Scotchbond foi aplicado nas barras e fotoativado por 10 s. Dois tubos de plástico de 1 mm de comprimento foram colocados em cada barra e preenchidos com cimento resinoso Variolink Veneer. Cada cilindro foi fotoativado por 20 s. Os tubos de plástico foram removidos e a resistência de união ao microcisalhamento de cada cilindro foi testada. Os dados de grau de conversão (%) e resistência de união ao microcisalhamento (MPa) foram submetidos ao teste t de Student (α = 0,05). Tanto o grau de conversão do cimento resinoso fotoativado através da cerâmica quanto a resistência de união do cimento resinoso à cerâmica foram estatisticamente maiores no grupo em que a cerâmica foi confeccionada pela técnica CAD/CAM (44,74% e 22,18 MPa) do que no grupo em que a cerâmica foi confeccionada pela técnica prensada (25,71% e 19,83 MPa). As técnicas de processamento de cerâmica dissilicada de lítio afetaram o grau de conversão do cimento resinoso quando fotoativado através das cerâmicas e a resistência de união do microcisalhamento do cimento resinoso às cerâmicas.

ABSTRACT

La criptococosis afecta predominantemente a pacientes con compromiso de la inmunidad celular. Se presenta el caso de un paciente masculino de 45 años, con antecedentes de VIH y linfoma de Hodgkin, internado por fiebre persistente sin neutropenia. El PET-TC evidenció imágenes hipermetabólicas pulmonares y ganglionares se sospechó recaída de su enfermedad y/o proceso infeccioso. Se realizó BAL, se aisló en cultivo Crytococcus laurentii. Recibió tratamiento con anfotericina B y fluconazol, cultivos de control negativos. Se repitió TC de control sin infiltrados pulmonares y persistencia de adenopatías. Realizó tratamiento con ESHAP evolucionando con buena respuesta.


Cryptococcosis mostly affects patients with cell-mediated immunity involvement. We present the case of a male patient, 45 years old, with history of HIV and Hodgkin's lymphoma, who was admitted to the hospital due to persistent fever without neutropenia. The PET-TC (Positron Emission Tomography - Computed Tomography) showed hypermetabolic pulmonary and lymph node images. We suspected disease relapse and/or the presence of an infectious process. We performed a BAL (Bronchoalveolar Lavage) and Cryptococcus laurentii was isolated in culture. The patient received treatment with amphotericin B and fluconazole, with negative control cultures. The CT control was repeated without pulmonary infiltrates and persistence of adenopathies. The patient received treatment with ESHAP and evolved with good response.
