ABSTRACT

La investigación tuvo por objetivo identificar, analizar y sistematizar las experiencias teóricas con respecto al estudio de casos como metodología investigativa. El objetivo primario fue evidenciar el estigma y el estereotipo sobre los adultos mayores contrastado con la realidad actual. Para esto se realizó una serie de estudios de casos a adultos mayores de manera remota y presencial debido a la reciente pandemia de COVID-19 donde fueron seleccionados los más destacados. Los resultados obtenidos fueron utilizados para comparar con la estigmatización social atribuida a este grupo etario. Se considera que mediante la entrega de herramientas necesarias para el desarrollo social a las personas mayores para que logren manejar su salud y su estilo de vida, un adulto mayor puede ser capaz de gestionar su autonomía e incluirse en la sociedad de forma eficaz y placentera[AU]


The objective of the research w as to identify, analyze and systematize the theoretical experiences regarding the case study as a research methodology. The primary objective was to demonstrate the stigma and stereotype about the elderly over reality. For this, a series of case studies were carried out on elderly people remotely and in person due to the recent COVID-19 pandemic, where the most prominent were selected. The results obtained were used to contrast the social stigmatization attributed to this age group. We believe that by providing the necessary tools for social development to older people so that they can manage their health and lifestyle, an older adult may be able to manage their autonomy and include themselves in society in an effective and pleasant way[AU]


A pesquisa teve como objetivo identificar, analisar e sistematizar as experiências teóricas sobre o estudo de caso como metodologia de pesquisa. O objetivo principal foi destacar o estigma e estereótipo sobre os idosos sobre a realidade. Para isso, foi realizada uma série de estudos de caso em idosos de forma remota e presencial devido à recente pandemia de COVID-19, onde foram selecionados os mais destacados. Os resultados obtidos foram utilizados para contrastar com a estigmatização social atribuída a essa faixa etária. Em conclusão, propomos que, ao fornecer as ferramentas necessárias para o desenvolvimento social dos idosos, para que eles possam gerenciar sua saúde e estilo de vida, o idoso poderá gerenciar sua autonomia e se incluir na sociedade de maneira eficaz e agradável[AU]

ABSTRACT

Adrenal incidentalomas are an increasingly common pathology. Although historically they have been considered largely non-functioning, recent evidence suggests that the usually performed study is incomplete and/or not sensitive enough. In the last decade the clinical spectrum of adrenal hypercortisolism has expanded considerably, including milder cases which are also associated with cardiovascular morbidity and even mortality. Furthermore, primary aldosteronism has also expanded beyond the classic phenotype with advanced vascular damage, resistant hypertension and hypokalemia, currently including asymptomatic, normotensive and normokalemic patients. For this reason, a correct protocolized study is essential in all adrenal incidentalomas, including a precise radiological characterization, as well as a systematic hormonal evaluation using more sensitive cut points. The findings of this workup are relevant, because they allow a more individualized approach to the medical and surgical management of these patients.


Los incidentalomas suprarrenales son una patología cada vez más frecuente. Si bien históricamente han sido considerados no funcionantes en su gran mayoría, evidencia reciente sugiere que el estudio habitual es incompleto y/o poco sensible. En la última década el espectro clínico del hipercortisolismo de origen adrenal se ha ampliado de forma considerable, incluyendo casos leves que también se asocian a morbilidad cardiovascular e incluso mortalidad. Por otro lado, el hiperaldosteronismo primario también ha expandido su fenotipo más allá del clásicamente descrito con daño vascular avanzado, hipertensión resistente e hipokalemia, abarcando en la actualidad a pacientes asintomáticos, normotensos y normokalemicos. Por esta razón es imprescindible un correcto estudio protocolizado en todo incidentaloma suprarrenal, incluyendo una precisa caracterización radiológica, así como una evaluación hormonal sistemática utilizando puntos de corte más sensibles. Los hallazgos de este estudio son relevantes, pues permiten guiar de forma más individualizada el manejo médico y quirúrgico de estos pacientes.

ABSTRACT

Resumen Se entiende el temperamento como el conjunto de diferencias individuales en reactividad y autorregulación, manteniéndose relativamente estable a través del tiempo, y siendo influenciado por la herencia, la maduración biológica y el contacto con el ambiente. En el presente estudio se buscó establecer si se dan diferencias en las dimensiones de temperamento de extraversión, afecto negativo y autorregulación según nivel socioeconómico, en niños chilenos asistentes a sala cuna. Se evaluaron 103 niños de ambos sexos, a los 12 y a los 30 meses de edad, con los cuestionarios IBQ-R-VSF y ECBQ-VSF, los cuales respondieron los padres. Los resultados mostraron que entre los 12 y 30 meses la extraversión se mantuvo constante, mientras que las dimensiones de afecto negativo y autorregulación disminuyeron de manera significativa. A los 12 meses los niños de NSE alto mostraron una mayor autorregulación, mientras que a los 30 meses los niños de NSE bajo evidenciaron un mayor afecto negativo. Se discuten los resultados en base a la evidencia encontrada sobre las diferentes realidades en los distintos niveles socioeconómicos.


Abstract Temperament is understood as the set of individual differences in reactivity and self-regulation, considered as stable through time, and influenced by the heritability, the biological maturation, and the interaction with the environment. The aim of this study was to determine if the dimensions of temperament (extraversion, negative affect, and self-regulation) show differences according to socioeconomic status in Chilean children attending to nurseries. For this study, 103 children from both genders were evaluated at 12 and 30 months old, using the IBQ-R-VSF and ECBQ-VSF questionnaires, which were answered by their parent. Results showed that between 12 and 30 months, extraversion remained constant, while the dimensions of negative affect and self-regulation decreased significantly. Children from high socioeconomic status showed higher self-regulation at age of 12 months, while the ones from low socioeconomic status decreased their negative affect at 30 months old. Results are discussed based on evidence about realities from different socioeconomic statuses.

ABSTRACT

Introducción: El tabaco es la primera causa de morbimortalidad prevenible en el mundo. En Chile la prevalencia de consumo es 40,6 por ciento, la mayor a nivel continental. El consejo breve o consejería breve (CB) es una intervención simple, efectiva y de bajo costo para el cese de consumo de tabaco, que Atención Primaria en Salud (APS) ha demostrado disminuir en la prevalencia de tabaquismo. Existe escasa información sobre la utilización de CB en APS en Chile. Métodos: Estudio transversal de metodología cuantitativa. Se encuestó a 604 pacientes pertenecientes a 2 centros APS de Santiago, tras haber recibido atención clínica. Los datos fueron analizados con SPSS 21 y calculadora web Graph Pad. Resultados: La prevalencia de tabaquismo entre los encuestados fue de 32,5por ciento, de ellos, el 78,6 por ciento reportó deseo de dejar de fumar. Al 43,7 por ciento de los encuestados se le preguntó respecto al consumo de tabaco en la atención reciente, y del total de fumadores, a 37,1 por ciento se les aconsejó dejar de fumar. Los pacientes fumadores calificaron la recepción de CB como "indiferente", "agradable" o "muy agradable" en un 94,9 por ciento, y en un 90,5 por ciento, entre aquellos que no desean dejar de fumar. Conclusión: El tabaquismo es un problema escasamente abordado en las atenciones clínicas APS, lo que contrasta con la alta prevalencia de consumo nacional y de los pacientes consultantes. La mayoría de los fumadores desea dejar de fumar, y la minoría considera "desagradable" o "muy desagradable" ser aconsejada para cesar el consumo, aun en aquellos fumadores que no quieren dejar de fumar.(AU)


Introduction: Smoking is the leading cause of preventable morbidity and mortality worldwide. In Chile smoking prevalence is 40.6 percent, the highest on the continent. Brief advice or brief counseling (BC) is a simple, effective and inexpensive intervention for tobacco consumption cessation, that has been shown decrease smoking prevalence in Primary Health Care (PHC). There is little information on the use of BC in PHC in Chile. Methods: Cross-sectional study of quantitative methodology. 604 patients were surveyed from 2 PHC centers of Santiago, after receiving clinical care. Data was analyzed with SPSS 21 and Graph Pad web calculator. Results: The smoking prevalence among respondents was 32.5 percent, 78.6 percent of them expressed desire to quit 43.7 percent of respondents were asked about consumption of tobacco in a recent consultation, and a total 36.9 percent of smokers were advised to quit. Smokers patients rated reciving CB as "indifferent", "pleasant" or "very pleasant" in 94,9 percent and 90,5 percent among those who did not want to quit. Conclusion: Smoking is a problem rarely addressed in PHC, this contrasts with the high prevalence of domestic consumption and consulting patients. Most smokers want to quit and only a minority considered "unpleasant" or "very unpleasant" being advised to cease consumption, even in those smokers who do not want to quit.(AU)

ABSTRACT

Background: In a rapidly changing culture like ours, with emphasis on productivity, there is a strong need to find the meaning of health care work using learning instances that privilege reflection and face to face contact with others. The Diploma in Health and Humanization (DSH), was developed as an interdisciplinary space for training on issues related to humanization. Aim: To analyze the experience of DSH aiming to identify the elements that students considered key factors for the success of the program. Material and Methods: We conducted a focus group with DSH graduates, identifying factors associated with satisfaction. Transcripts were coded and analyzed by two independent reviewers. Results: DSH graduates valued a safe space, personal interaction, dialogue and respect as learning tools of the DSH. They also appreciates the opportunity to have emotional interactions among students and between them and the teacher as well as the opportunity to share personal stories and their own search for meaning. Discussion: DSH is a learning experience in which their graduates value the ability to think about their vocation and the affective interaction with peers and teachers. We hope to contribute to the development of face to face courses in the area of humanization. Face to face methodology is an excellent teaching technique for contents related to the meaning of work, and more specifically, to a group of learners that require affective communication and a personal connection of their work with their own values and beliefs.
