ABSTRACT

Adenosquamous carcinoma of the pancreas is a rare tumor, which is difficult to diagnose. A 65-year-old man was admitted complaining of abdominal pain. Abdominal computed tomography (CT) demonstrated a cystic mass in the pancreatic body. Endoscopic ultrasound (EUS) showed an approximate 1.0 cm-sized hypoechoic cystic mass with a solid portion in the pancreatic body and narrowing of the main pancreatic duct. Under the impression of acute pancreatitis with a pseudocyst diagnosis, medical therapy including pancreatic stent insertion was administered to the patient for 4 weeks. A distal pancreatectomy was performed with the suspicion of pancreatic cancer due to the remaining mass on a follow-up CT scan, and the tumor was histopathologically diagnosed as stage I (TNM classification) adenosquamous carcinoma of the pancreatic body. The patient was discharged without any problems and has been on follow-up without recurrence. We report a case of adenosquamous carcinoma of the pancreas that was diagnosed relatively early with the help of EUS.

ABSTRACT

Neuroendocrine carcinoma of the bile duct is rare, and a large cell neuroendocrine carcinoma is extremely rare. We report the first case of a large cell neuroendocrine carcinoma in the hilar bile duct. A 73-year-old woman presented with continuous dull pain in the epigastrium. We diagnosed a Klatskin's tumor, Bismuth type II by computed tomography. An initial endoscopic cytology of the common hepatic duct showed a poorly differentiated adenocarcinoma. She underwent an extended right hepatectomy. Histologically, the tumor was diagnosed as a large cell neuroendocrine carcinoma. She received adjuvant radiotherapy for tumor involvement of the resected margin. At 12 months after surgery, lymph node metastases were present in the retroperitoneum, but she was still alive at 23 months after surgery. The role of additional therapy, such as radiotherapy, must be investigated to improve patient prognoses.

ABSTRACT

Pancreatic arteriovenous malformation (AVM) is an extremely rare condition with various clinical manifestations. We report herein a case of recurrent acute pancreatitis due to pancreatic AVM in a 49-year-old man. This patient presented with epigastric pain that had developed after consuming alcohol 2 days prior to admission. Serum amylase and lipase levels were elevated and computed tomography revealed focal low-attenuation lesions with peripancreatic infiltrations in the pancreatic tail and multiple collateral vessels around the low-attenuation lesions. He was diagnosed with acute pancreatitis and pancreatic AVM. Although he had stopped drinking after the first attack of acute pancreatitis, his pancreatitis recurred twice within 3 months. He underwent a distal pancreatectomy after the third attack of acute pancreatitis. He was free of symptoms for 2 years after the pancreatectomy.

A 17-year-old girl presented with primary amenorrhea, short stature, and clitomegaly. Her karyotype showed Turner mosaicism of 45,X/46,XY,idic(Y)(q11.23)del(Y)(q11.23). Laparoscopic bilateral gonadectomy was performed and there was testicular tissue in left ovary.

ABSTRACT

A lymphangioma is a benign tumor which is composed of lymphatic vessels and spaces containing chylous, serosanguinous or serous material. A lymphangioma in the gastrointestinal tract is usually small sized and incidentally diagnosed because it does not cause significant symptoms. However, when abdominal pain, bleeding, intussusception or intestinal obstruction develope, it requires surgical resection. Recently, we experienced a case of multiple jejunal lymphangioma with recurrent bleeding in a 53 years old renal transplant recipient presenting with chronic anemia. Small bowel series, capsule endoscopy and computerized tomograpy scan revealed a 4.5 x 6.5 cm sized mass on jejunum. The patient underwent a laparoscopic segmental resection of jejunum without any complication.

ABSTRACT

Dermatofibrosarcoma protuberans (DFPS) is a locally aggressive skin tumor with a very low incidence in the general population. This tumor has a remarkable tendency to recur, However, a metastasis is rare. We report a case of DFPS with a pulmonary metastasis in 28-year-old man. The pulmonary metastasis developed 5 years after a complete resection of the primary skin tumor. We reviewed the clinical manifestations and treatment of DFPS, and highlight the need for a long-term follow-up examination for metastases after a wide excision of these lesions.

ABSTRACT

Barrett's esophagus is defined as the replacement of normal squamous epithelium of distal esophagus with metaplastic columnar epithelium. It is a well known risk factor of esophageal adenocarcinoma. If high grade dysplasia or early stage esophageal cancer develops in a patient with Barrett's esophagus, esophagectomy shoud be performed. However, operative procedures have various complications and the patients may suffer resulting in poor quality of life. Therefore, if the cancer is detected at an early stage such as superficial mucosal lesion, it is possible to resect the tumor with the use of endoscopic technique. Furthermore, endoscopic mucosal resection (EMR) also can be performed for the patients with mucosal or submucosal cancer who can not receive esophagectomy due to old age or poor general condition. We experienced a case of esophageal adenocarcinoma from Barrett's esophagus which had been successfully resected endoscopically.

ABSTRACT

Diabetes mellitus is well known to be associated with various structural and functional liver abnormalities. If diabetic patients are accompanied by hepatomegaly or abnormal findings from a liver function test, the most common pathological findings are steatosis and glycogenosis. The steatosis is characterized by deposition of macrovesicular fat droplets in the hepatocytes, which is common in obese, type 2 diabetes mellitus. If macrovesicular steatosis is combined with mixed inflammatory infiltrate, without evidence of alcoholic hepatitis, the case could be diagnosed as nonalcoholic steatohepatitis (NASH). NASH has the possibility of progressing to cirrhosis. Secondary glycogenosis is common in uncontrolled type 1 diabetes mellitus, and is completely reversible. A 22-year-old male, with uncontrolled type 1 diabetes mellitus, was admitted with anorexia, nausea and right upper quadrant pain. Hepatomegaly and elevated aminotransferases were noted. He was diagnosed as diabetic glycogenosis using computed tomogram and liver biopsy. The hepato megaly and liver function test abnormalities were markedly improved with glycemic control

ABSTRACT

Hydrosalpinx is an entity commonly encountered in gynecologic practice. However, giant hydrosalpinx is rare. Although the exact pathophysiology is unknown, most feel that hydrosalpinx is the result of chronic pathological condition of the fallopian tube when the fimbrial end of the tube is occluded and distal part distended with fluid. The occlusion usually occurs secondary to pelvic inflammatory disease or endometriosis or adjacent appendicular inflammation. Hydrosalpinx is associated with infertility, torsion and hemorrhagic infarction. We present an unusual case of giant hydrosalpinx, which was misdiagnosed as ovarian cyst with a review of brief literature.
