ABSTRACT

Objective:To study the expression and clinical significance of serum neutrophil elatinase-associated lipocalin (NGAL), monocyte chemoattractant protein-1 (MCP-1) and tumor necrosis factor-like weak inducer of apoptosis (TWEAK) in patients with lupus nephritis (LN).Methods:97 LN patients who received treatment in Hanzhong 3201 Hospital from January 2016 to December 2019 were selected as study objects. Among them, 38 cases were in the active group and 59 cases were in the inactive group, and 51 cases for physical examination were selected as the control group. The levels of serum NGAL, MCP-1, TWEAK and LN renal function was examined and compared in each group. The diagnostic value of serum NGAL, MCP-1 and TWEAK expression on LN disease activity was evaluated by receiver operating characteristic (ROC) curve.Results:The levels of serum NGAL, MCP-1 , TWEAK, serum creatine (Scr), blood urea nitrogen (BUN), 24 h urinary protein and dsDNA in the active and inactive groups were higher than those in the control group, and the above indexes in the active group were higher than those in the inactive group ( P<0.05). Pearson correlation analysis showed that serum NGAL, MCP-1 and TWEAK levels in LN patients were positively correlated with systemic lupus erythematosus disease activity index (SLEDAI) score, Scr, BUN, 24 h urine protein and dsDNA levels ( P<0.05). ROC curve analysis showed that the area under curve (AUC) of combined serum NGAL, MCP-1 and TWEAK in predicting disease activity of LN patients was 0.865, and the specificity, sensitivity, AUC and Youden index were higher than single index. Conclusions:The expression of NGAL, MCP-1 and TWEAK in LN patients during the active period is increased and closely related to the activity of disease. It is of high application value for predicting LN activity, and can be used as biomarkers for clinical prediction of LN activity.

Objective@#To observe the effect of atorvastatin combined with insulin glargine on renal function in patients with early diabetic nephropathy.@*Methods@#From January 2016 to March 2019, 100 patients with early diabetic nephropathy admitted to Hanzhong 3201 Hospital Affiliated with Xi′an Jiaotong University Medical School were selected as subjects. According to the random number table, patients were divided into control group and observation group, with 50 cases in each group. All patients underwent diet control, blood pressure control and symptomatic treatment. Patients in the control group were treated with insulin glargine to control blood glucose. Patients in observation group were given atorvastatin on this basis. After 16 weeks of treatment, the therapeutic effects of the two groups were observed, as well as the change in urinary albumin excretion rate (UAER), serum creatinine (Scr), C-reactive protein (CRP), total cholesterol (TC), and triglyceride (TG). Adverse reactions were observed during treatment in both groups.@*Results@#After treatment, the levels of UAER, Scr, CRP, TC and TG of the two groups were lower than those before treatment, and the above indexes of the observation group were lower than those of the control group. The difference were statistically significant (P<0.05). During the treatment period, the incidence of adverse reactions in control group and observation group was 4.00%(2/50) and 12.00%(6/50), and there was no significant difference (P>0.05).@*Conclusions@#Atorvastatin combined with insulin glargine in the treatment of early diabetic nephropathy can effectively reduce the levels of UAER, Scr, CRP, TC and TG, and has good safety.

ABSTRACT

OBJECTIVES@#To explore the psychological status of medical staff in the epidemic period of coronavirus disease 2019 (COVID-19), and to analyze its influential factors.@*METHODS@#A total of 373 medical staff from Xiangya Hospital and the Second Xiangya Hospital of Central South University were enrolled for this study. The General Sociological Data Questionnaire, Symptom Check-List 90 (SCL-90), and self-designed public opinion response questionnaire were used to assess general sociological data, mental health scores, and ability to respond to COVID-19 related public opinion information of medical staff. The mental health scores of medical staff with different general sociological data and public opinion information coping abilities were compared. Influential factors of mental health were analyzed.@*RESULTS@#The average score of 10 factors in SCL-90 of 373 medical staff was less than 2 points. 14.21% medical staff had one or more factor scores more than two points, including 11.26% with terror symptoms, 7.77% with compulsive symptoms, and 5.63% with anxiety. The main sources of COVID-19 information for medical staff included WeChat, microblog, Jinri toutiao, TV and radio. 66.22% medical staff regularly verified information about COVID-19 through official websites or formal channels. A great deal of COVID-19 information in WeChat could make medical staff nervous (34.05%), anxious (30.29%), and insecure (29.22%). 68.63% medical staff sometimes were worried about getting infected because they knew information about COVID-19. Different departments of medical staff, getting cough or having a fever recently, and the degree of fear of infection had an impact on the SCL-90 score of medical staff, the differences were all statistically significant (all <0.05). Stepwise regression analysis showed that the impact of COVID-19 information on their life in WeChat, getting cough or having a fever recently, insomnia-early caused by COVID-19 information in WeChat, different departments, and the degree of fear of infection COVID-19 were the influential factors for the mental health of medical staff (all <0.05).@*CONCLUSIONS@#During the epidemic of COVID-19, medical staff suffered from psychological problems to various degrees. It is necessary to establish a psychological assistance platform and guide the direction of public opinion correctly to promote the mental health of medical staff.

ABSTRACT

Objective To explore the clinical effect of using small needle knife in autologous fat grafting for the treatment of atrophic scar.Methods Twelve patients with atrophic scar were included in this study during March 2014 and December 2016 in the Hangzhou First People's Hospital,Zhejiang University School of Medicine.We used small needle knife during fat grafting to treat the atrophic scar.Observer Scar Assessment Scale (POSAS) was used to assess the clinical effect of the patients.Results Aesthetic and functional improvements were observed.Resolution of pain and increases in scar elasticity were objectively assessable.Improvement of both clinical evaluation and patient perception was obtained.Conclusions Use of small needle knife during fat grafting is a good alternative for the treatment of atrophic scars.

ABSTRACT

Objective To evaluate the advantages of confocal laser endomicroscopy(CLE)for function repairing of ulcerative colitis(UC). Methods Thirty patients with UC who were diagnosed and treated in the First Affiliated Hospital of Zhejiang University Medical College between July 2014 and December 2016 were enrolled in the study group. The control group consisted of 10 patients who were diagnosed as having colonic polyps with colonoscopy and underwent polypectomy in the same period. Both groups were examined with white light endoscopy and CLE,then the fluorescein leakage score was compared. Results There were 10 out of 30 cases in the study group whose whole intestinal mucosa were judged as normal under white light endoscopy. In the other 20 cases,there were parts of intestinal mucosa being judged as abnormal. All of the 10 cases in the control group were with normal colon mucosa. There was a significant difference on fluorescein leakage score by CLE between the study group which showed normal by white light endoscopy and the control group(P<0.05). The fluorescein leakage score by CLE was significant difference between the abnormal intestinal segment and normal intestinal segment in 20 patients with active UC(P<0.05). Spearman correlation analysis showed that there was non-ranked correlation between the fluorescein leakage score of CLE and histopathological findings of biopsy in UC patients with normal mucosa of the left colon under white light endoscopy(rs=0.394,P>0.05). Conclusion In the process of mucosal healing of UC patients,structural repair can be found firstly through CLE. Mucosal healing under white light endoscopy cannot represent the functional recovery. CLE is more effective than the histopathology in the evaluation of mucosal barrier function.

ABSTRACT

Objective To evaluate the efficacy of follicular unit extraction (FUE) in the repair of scarring eyebrow loss.Methods A total of 30 patients with scarring eyebrow loss were treated with FUE technique in the Department of Plastic Surgery of Zhengzhou First People's Hospital,including 18 females and 12 males with 41 eyebrow defects.Fine soft hairs with a length of 5-10 mm were extracted from the back of the ear and temporal region of the patients,and then were split into individual follicular units under a microscope at 6 × magnification.The transplant density ranged from 20 to 30 FU/cm2.Results During the 3-18 months of follow-up after the surgery,eyebrows in the recipient areas grew in a natural direction and showed good appearance.The survival rate of hair grafts was up to 90%,and the patients were satisfied with the results.Conclusion FUE may be an ideal method for the treatment of scarring eyebrow loss with less injury and high survival rate of hair grafts.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the platelet count in prediction of the efficacy of infliximab for active Crohn's disease.</p><p><b>METHODS</b>A total of 74 patients with Crohn's disease, including 50 males and 24 females with mean age of 27 years, receiving infliximab therapy from January 2011 to June 2015 were retrospectively included. Clinical variables including routine blood test, erythrocyte sedimentation rate, C-reactive protein and Crohn's disease activity index were measured before treatment and after 3rd, 6th treatments.</p><p><b>RESULTS</b>Compared with baseline, the Crohn's disease activity index score, white blood cell count, platelet count, erythrocyte sedimentation rate, C-reactive protein significantly decreased (all (P<0.01), while hemoglobin and red blood cell hematocrit increased (P<0.01) in patients after 3 treatments of infliximab. These variables were further improved after 6 treatments. Further analysis showed that platelet count was positively associated with disease severity of Crohn's disease. Fifty-three patients achieved clinical remission (Crohn's disease activity index score<150) after 3 treatments of infliximab. Logistic regression analysis showed that baseline platelet count, Crohn's disease activity index, and disease course were three predictors for clinical remission of Crohn's disease.</p><p><b>CONCLUSION</b>Platelet count is positively associated with disease severity of Crohn's disease, and baseline platelet count is an important predicator for the early response of infliximab therapy.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the therapeutic effect of soft tissue expansion combined with follicular unit extraction( FUE) for burn cicatricial bald.</p><p><b>METHODS</b>48 patients with burn cicatricial bald (> 25 cm2) were treated in three stages. The expanders were implanted on the first stage. After expansion for 8 weeks, the expanders were taken out and local flaps were transferred. One year later, follicular unit extraction( FUE) was applied on the bald area.</p><p><b>RESULTS</b>48 cases were followed up for 5 years with satisfactory cosmetic results. The VAS assessment of satisfaction on hair appearance after three-staged surgery was 8.2 ± 2.1.</p><p><b>CONCLUSIONS</b>Soft tissue expansion combined with FUE has a reliable effect for burn cicatricial bald.</p>

ABSTRACT

Objective To observe the therapeutic effect of Dioscorea opposita polysaccharide on gastric ulcer in rats,and to investigate the effects of Dioscorea opposita polysaccharide on level of epidermal growth factor ( EGF) in stomach tissues .Methods 50 rats were randomly divided into five groups [ A group ( normal control group ) , B group(model group),C group(Dioscorea opposita polysaccharide group ),D group(teprenone group)].The healing condition of gastric ulcer and EGF level were observed and compared .Results (1)The ulcer index of C group was obviously reduced compared with B group (P0.05).Conclusion Dioscorea opposita polysaccharide has protective effects for gastric mucosa .The mechanism of protection may be related to promote synthesis of EGF .

ABSTRACT

<p><b>OBJECTIVE</b>To summarize our initial experience with laparoendoscopic single-site (LESS) retroperitoneal lymph node dissection (RPLND) for treatment of nonseminomatous testicular cancer.</p><p><b>METHODS</b>From September 2010 to June 2011, 3 patients (aged 19-27 years) with right testicle enlargement and elevated alpha-fetoprotein level underwent right radical orchidectomy. Histopathological analysis revealed nonseminomatous germ cell tumor. LESS-RPLND was performed 3 weeks after orchiectomy. The homemade port was inserted through a 3-cm right pararectal incision in the right lower quadrant for unilateral RPLND using nerve-sparing technique and modified right-sided template removal similar to those in open RPLND.</p><p><b>RESULTS</b>The operation was successfully performed with a mean operative time of 240 min and a mean estimated blood loss of 50 ml. No conversion to open or conventional laparoscopic surgery was required. No major perioperative complications were observed. For the first case, the number of lymph nodes obtained for final histopathological examination was 11, and two positive nodes were detected. For the other 2 cases, no positive nodes were detected. Chemotherapy was administered in the first case. Alpha-fetoprotein level decreased close to the baseline one week postoperatively and no relapse occurred in these cases 3 month after RPLND. Follow-up at 1 year after the surgery showed good tumor control and preservation of the sexual function.</p><p><b>CONCLUSION</b>LESS-RPLND is safe and feasible for treatment of nonseminomatous testicular cancer, and the pararectal incision provides an ideal surgical approach with good cosmetic result, but the long-term effect needs to be tested by further large population-based study.</p>

ABSTRACT

Objective To discuss the effect of an advanced technique by shortening the levator palpebrae superioris muscle in treatment of moderate and severe congenital blepharoptosis. Methods 30 cases of moderate and severe blepharoptosis (including 2 cases of recurrent ptosis) were treated by shortening the levator palpebrae superioris associated with resection of tarsus. Removal portion of tarsus and levator palpebrae superioris muscle was designed according to the width of tarsus and the severity of blepharoptosis. Tarsus and levator palpebrae superioris muscle and conjunctiva palpebrae were resected on the basis of above design. The results of postoperation were followed up. Results Among the 30 cases, except for undercorrection in 3 cases, all others had a good outcome. Eyelid was raised to normal level by this technique with a natural eyelid contour. Only some presented a slight dysraphism in the early stage of postoperation,and no other severe complications occurred. Conclusions Shortening of the levator palpebrae superioris muscle can be applied to treat moderate and severe congenital blepharoptosis, and correct the deformity and achieve cosmetic effect simultaneously.In order to achieve satisfactory effects on function and shape, we should master the main points of operation.

ABSTRACT

OBJECTIVE To investigate the significance of Epworth sleepiness scale(ESS)and modified Epworth sleepiness scale(MESS),and evaluate the feasibility of MESS in assessment of obstructive sleep apnea hypopnea syndrome(OSAHS).METHODS The ESS and MESS of 232 patients with OSAHS was recorded in their first visiting.The scores of ESS and MESS were analyzed with the parameters of polysomnography(PSG). RESULTS Both ESS and MESS can reflect the degree of sleepiness and anoxemia in OSAHS patients,but the MESS was more efficient than ESS.The difference of MESS score between normal group and severe OSAHS group was greater than that of ESS score.CONCLUSION MESS is concerned the living habit of Chinese,and can partly reflect the station of patient with OSAHS.MESS can also be a primary screening method for suspected patients with severe OSAHS.

ABSTRACT

The effects of exopolysaccharides from Aphanothece halophytica(.EAH) on immunological function were studied by immunological techniques. The results showed that EAH could not only enhance the mitogenic response of mice's thymus cells, murine bone marrow cells proliferation,murine mixed lymphocyte reaction and the activity of NK cells in mice,but promote the interleukinl(IL-1)production and increase the activity of 1L-2 as well. The study suggests that EAH can improve the immunological function of mice.
