ABSTRACT

Objective:To investigate the clinical effects of Jiakang Pingxiao prescription combined with methiimidazole on hyperthyroidism. Methods:A total of 100 patients with hyperthyroidism admitted to Shanxian Central Hospital from February 2018 to January 2021 were included in this study. They were randomly divided into a study group and a control group, with 50 patients in each group. The control group was treated with methiimidazole, and the study group was treated with Jiakang Pingxiao prescription combined with methiimidazole. Thyroid function, serum levels of osteocalcin (OCN), β-CTx, hypersensitive C-reactive protein, and interleukin-6 (IL-6) were compared between the two groups. Results:After treatment, serum levels of thyroid stimulating hormone (TSH), free triiodothyronine (FT3), free thyroxine (FT4) in the study group were (3.10 ± 1.36) mU/L, (5.76 ± 1.25) pmol/L, (15.22 ± 1.95) pmol/L, respectively, which were significantly lower than (4.88 ± 1.47) mU/L, (7.13 ± 1.32) pmol/L, (19.07 ± 2.02) pmol/L in the control group ( t = 5.27, 4.71, 6.29, all P < 0.05). Serum OCN, β-CTx, hS-CRP, and IL-6 in the study group were (17.36 ± 2.62) μg/L, (0.32 ± 0.04) μg/L, (4.07 ± 0.86) mg/L, and (1.38 ± 0.21) pg/L, respectively, which were significantly lower than (26.05 ± 2.88) μg/L, (0.51 ± 0.09) μg/L, (6.23 ± 0.91) mg/L, (1.89 ± 0.28) pg/L in the control group ( t = 12.37, 10.40, 7.39, 8.57, all P < 0.05). The incidence of adverse reactions in the study group was significantly lower than that in the control group [6.00% (3/50) vs. 12.00% (3/50), χ2 = 14.78, P < 0.05). Conclusion:Jikang Pingxiao prescription combined with methiimidazole can effectively reduce the inflammatory responses in patients with hyperthyroidism, inhibit the expression of OCN and β-CTX in the serum, and improve thyroid function. The combined method is scientific and reasonable, and is suitable for clinical application. It has good therapeutic effects on hyperthyroidism and is worthy of clinical promotion.

ABSTRACT

OBJECTIVE@#To investigate the gut microbiota in newly diagnosed IgA nephropathy patients with chronic kidney disease (CKD) stages 1-2 and the association between the gut microbiota and the clinical risk factors of IgA nephropathy.@*METHODS@#Fresh fecal samples were collected from nineteen newly diagnosed IgA nephropathy patients with CKD stages 1-2 and fifteen age- and sex-matched healthy controls. Fecal bacterial DNA was extracted and microbiota composition were characterized using 16S ribosomal RNA (16S rRNA) high-throughput sequencing for the V3-V4 region. The Illumina Miseq platform was used to analyze the results of 16S rRNA high-throughput sequencing of fecal flora. At the same time, the clinical risk factors of IgA nephropathy patients were collected to investigate the association between the gut microbiota and the clinical risk factors.@*RESULTS@#(1) At the phylum level, the abundance of Bacteroidetes was significantly reduced (P=0.046), and the abundance of Actinobacteria was significantly increased (P=0.001). At the genus level, the abundance of Escherichia-Shigella, Bifidobacte-rium, Dorea and others were significantly increased (P < 0.05). The abundance of Lachnospira, Coprococcus_2 and Sutterella was significantly reduced (P < 0.05). (2) There was no significant difference in the abundance of gut microbiota between the newly diagnosed IgA nephropathy patients and the healthy control group (P>0.05), but there were differences in the structure of the gut microbiota between the two groups. The results of LEfSe analysis showed that there were 16 differential bacteria in the newly diagnosed IgA nephropathy patients and healthy controls. Among them, the abundance of the newly diagnosed IgA nephropathy patients was increased in Enterobacteriales, Actinobacteria, Escherichia-Shigella, etc. The healthy control group was increased in Bacteroidetes and Lachnospira. (3) The result of redundancy analysis (RDA) showed that Bifidobacterium was positively correlated with serum IgA levels, 24-hour urinary protein levels and the presence of hypertension. Lachnoclostridium was positively correlated with the presence of hypertension. Escherichia-Shigella was positively correlated with urine red blood cells account. Bifidobacterium was positively correlated with the proliferation of capillaries. Faecalibacterium was positively correlated with cell/fibrocytic crescents. Ruminococcus_2 was positively correlated with mesangial cell proliferation, glomerular segmental sclerosis and renal tubular atrophy/interstitial fibrosis.@*CONCLUSION@#The gut microbiota in the newly diagnosed IgA nephropathy patients with CKD stages 1-2 is different from that of the healthy controls. Most importantly, some gut bacteria are related to the clinical risk factors of IgA nephropathy. Further research is needed to understand the potential role of these bacteria in IgA nephropathy.

ABSTRACT

Objective:To evaluate endoscopic nasobiliary drainage (ENBD) combined with nasojejunal tube feeding for elderly patients with severe acute cholangitis.Methods:Data of 43 elderly patients with severe acute cholangitis, who received ENBD combined with nasojejunal tube feeding from January 1, 2016 to May 31, 2018 at Affiliated Hangzhou First People′s Hospital, Zhejiang University School of Medicine were retrospectively analyzed and were included in the observation group, and 43 other patients who received ENBD combined with conventional therapy in the same period were included in the control group with the matching principle of 1∶1. Liver function indices (ALT and AST), nutritional status (Hb, TP and ALB) and inflammation indices (WBC, NEU% and CRP) of the two groups before the operation, 3 days and 7 days of nutritional support after the operation were compared. Adverse reactions (abdominal distention and diarrhea), mortality, hospitalization time and expenses of the two groups were also compared.Results:There were no significant differences in gender composition, mean age, preoperative APACHE-Ⅱ score, NRS2002 score, liver function index, nutritional index, or inflammatory index between the observation group and the control group ( P>0.05). The baseline data of the two groups were comparable. After 3 days of nutritional support, ALT, AST, TP were 21.0 (15.0, 35.5) U/L, 26.0 (21.0, 36.5) U/L, and 64.2±5.2 g/L, respectively in the observation group, and 47.0 (29.5, 82.5) U/L ( P<0.05), 47.0 (29.0, 75.0) U/L ( P<0.05), and 60.5±6.4 g/L ( P<0.05), respectively in the control group. The levels of other indicators were not statistically different at this time point ( P>0.05). At 7 days postoperative nutritional support, ALT, AST, TP, ALB and CRP of the observation group were 22.0 (14.0, 31.5) U/L, 26.0 (20.5, 38.5) U/L, 67.6±5.4 g/L, 34.6±3.7 g/L, and 28.0 (18.5, 35.5) mg/L, respectively, and 43.0 (18.0, 59.5) U/L ( P<0.01), 34.0 (24.0, 60.5) U/L ( P=0.02), 64.5±5.7 g/L ( P=0.01), 31.5±7.0 g/L ( P=0.02), and 34.0 (24.0, 66.5) mg/L ( P<0.05) in the control group. There were no significant differences in the levels of other indicators between the two groups at this time point ( P>0.05). In the observation group, the incidence of diarrhea, abdominal distension, mortality, hospitalization time and hospitalization expenses were 32.6% (14/43), 30.2% (13/43), 9.3% (4/43), 16.0±7.0 days and 40±10 thousand yuan, respectively, and in the control group, the above indicators were 4.7% (2/43) ( P<0.05), 7.0% (3/43) ( P<0.05), 11.6% (5/43) ( P=0.72), 19.3±3.7 days ( P<0.05)) and 53±23 thousand yuan ( P<0.05), respectively. Conclusion:For elderly patients with severe acute cholangitis, enteral nutrition with ENBD can effectively improve the nutritional status, reduce inflammatory reaction, the impact on liver function, and hospital costs, and shorten the hospitalization time, which is suitable for further clinical application.

ABSTRACT

Objective:To investigate the clinical effect of Wenshen-Gushu Decoction combined with conventional western medicine on postmenopausal osteoporosis patients with kidney yang deficiency type. Methods:A total of 105 postmenopausal osteoporosis patients admitted in Huxi Hospital Affiliated to Jining Medical College from January 2017 to December 2019 were selected. According to the random number table method, they were divided into the control group (52 cases) and observation group (53 cases). The control group was treated with conventional western medicine, and the observation group was treated with self-made Wenshen-Gushu Decoction on the basis of the control group. Both groups were treated for 8 weeks. The TCM Symptom Score, Visual Analogue Scale (VAS), bone mineral density of troch (Troch), bone mineral density of ward’s triangle (ward’s), bone mineral density of L 1-4 and serum levels of steroid hormone receptor coactivator-3 (SRC-3), B-cell lymphoma gene-2 (Bcl-2) and transcription element coactivator (PGC-1a) were compared between two groups before and after treatment. The incidence of adverse reactions in the treatment process was observed, and the clinical efficacy was evaluated. Results:The total effective rate of observation group was 79.3% (42/53), and that of control group was 61.5% (32/52), where the difference between the two groups was statistically significant ( χ2=3.955, P=0.047). After treatment, the scores of lumbar and knee soreness, lumbar and spinal pain, lower limb flaccidity, walking difficulty and the VAS scores in the observation group were significantly lower than those in the control group ( t=21.956, 20.472, 18.591, 13.439, 19.594, 9.035, P<0.001). After treatment, the Troch [(0.70 ± 0.07) g/cm 2vs. (0.67 ± 0.06) g/cm 2, t=2.356] and Ward’s [(0.57 ± 0.06) g/cm 2vs. (0.510 ± 0.05) g/cm 2, t=5.561] and the bone mineral density of L 1-4 [(0.88 ± 0.07) g/cm 2vs. (0.80 ± 0.05) g/cm 2, t=6.727] in the observation group were significantly higher than those in the control group ( P<0.05). The serum levels of SRC-3 [(0.34 ± 0.08) ng/L vs. (0.27 ± 0.07) ng/L, t=4.768], Bcl-2 [(9.56 ± 2.15) μg/L vs. (8.45 ± 2.07) μg/L, t=1.917] and PGC-1a[(0.33 ± 0.06) g/L vs. (0.25 ± 0.04) g/L, t=8.023] in the observation group were significantly higher than those in the control group ( P<0.05). No serious adverse reactions occurred in both groups during the treatment. Conclusion:Wenshen-Gushu Decoction combined with conventional western medicine shows clinical effect and can reduce clinical symptoms, improve bone mineral density, and upregulate the levels of SRC-3, Bcl-2, PGC-1a for the postmenopausal osteoporosis patients with kidney yang deficiency typea with safety.

ABSTRACT

Objective To investigate the effect of cyclic stretch on adhesion of vascular smooth muscle cells (VSMCs) with platelet-derived microparticles (PMPs), and the role of PMPs in VSMC autophagy. Methods Cyclic stretch with the magnitude of 5% (simulating physiological mechanical stretch) or 15% (simulating pathological mechanical stretch) was subjected to VSMCs in vitro by using FX-5000T cyclic stretch loading system, and the adhesion of PMPs in VSMCs was detected by using flow cytometry. Immunofluorescence was used to detect the expression of autophagy microtubule associated protein light chain 3 (LC3) after 24 h stimulation with PMPs. Western blotting was used to detect the expression of autophagy related protein (Atg) in VSMCs after 24 h stimulation by PMPs. Results Compared with 5% cyclic stretch, 15% cyclic stretch significantly increased the adhesion ability of VSMCs with PMPs. Immunofluorescence and Western blotting result revealed that PMPs stimulation significantly increased the expression of autophagy marker protein LC3 in VSMCs. Furthermore, the protein expressions of Atg5, Atg7 and Atg12 were all significantly increased in VSMCs stimulated with PMPs. Conclusions High cyclic stretch may enhance the autophagy of VSMCs by promoting the adhesion of PMPs, which will subsequently increase the expressions of Atg5, Atg7, Atg12 and LC3.

ABSTRACT

Objective To explore the efficacy and safety of endoscopic retrograde cholangiopancreatography ( ERCP ) for the diagnosis and treatment of children with pancreaticobiliary diseases. Methods Clinical data of 115 children with pancreaticobiliary diseases who underwent ERCP between July 2006 and June 2016 at Hangzhou First People' s Hospital were reviewed. Anesthesia types, therapeutic methods,success rate and postoperative complications were summarized. Results A total of 221 ERCP procedures were performed on 115 patients. Eighty one cases were diagnosed as common bile duct stones, 22 bile duct cysts, 3 biliary ascariasis, 66 chronic pancreatitis, 45 pancreas divisum, and 43 acute pancreatitis. Thirty-one procedures were performed under general anesthesia. Dissection of pancreatic duct and biliary duct, calculus removal, insertion of pancreatic duct and biliary duct, and draining in pancreatic duct or biliary duct were performed. The success rate of the ERCP procedure was 98. 6% ( 218/221) with complication rate of 7. 2%( 16/221 ) . In terms of postoperative complications, post-ERCP pancreatitisoccurred in 7 ( 3. 2%) cases, bleeding in 5 ( 2. 3%) , and postoperative cholangitis in 4 ( 1. 8%) . All the complications were cured after treatment. Conclusion Congenital anatomic anomalies are common in children with pancreaticobiliary diseases. Early diagnosis and treatment of ERCP by an experienced endoscopist for children with pancreaticobiliary diseases is effective and safe.

@#AIM: To analyze the accuracy of non-contact tonometer(NCT)and ICare rebound tonometer(RBT)in measuring intraocular pressure in glaucoma patients. <p>METHODS:A retrospective proportional control study. Totally 113 patients(185 eyes)with glaucoma were studied. Intraocular pressure was measured by RBT, NCT and Goldmann Applanation tonometer(GAT)at 9:00 and 16:00 and recorded as two sets of data, respectively. Their accuracy is evaluated by the rank sum test, and the international standard 8621 guide, Bland-Altman consistency analysis analyzed the consistency of the two groups of measurements, and the data were divided into two groups: abnormal intraocular pressure group(<10mmHg or >21mmHg)and normal intraocular pressure group(10-21mmHg). The consistency was analyzed between different groups. The spearman correlation analysis is adopted to analyze the correlation between ophthalmotonometers.<p>RESULTS: There were differences among the GAT, NCT and RBT measurements(<i>P</i><0.01). There was a strong correlation between the intraocular pressure measured value obtained by GAT and measured values obtained by RBT and NCT(<i>rs</i>=0.71, 0.77, <i>P</i><0.001). The measured values of NCT and GAT are close to each other without statistically significant differences(<i>P</i>=0.92). Compared with GAT, RBT has a higher intraocular pressure measured value, and there are significant differences(<i>P</i><0.05). However, compared with GAT, both NCT and RBT have a wide range of limits of agreement, and they are(-6.2-6.0mmHg)and(-5.2-7.6mmHg)respectively. In normal intraocular pressure groups, the ranges of limits of agreement are(-5.9-5.9mmHg)and(-4.3-7.5mmHg)respectively. In abnormal intraocular pressure groups, the ranges of limits of agreement are(-7.3-6.4mmHg)and(-7.5-5.6mmHg)respectively. The evaluation carried out in accordance with ISO 8612 guides indicates that the abnormal values more than the 95% of consistency restriction of NCT and RBT in the three groups are(3.9%, 11.3%, 12.2%)and(26.3%, 11.3%, 12.2%)respectively.<p>CONCLUSION: Both NCT and RBT cannot simply replace the GAT to measure the intraocular pressure of glaucoma patients. In glaucoma patients, with the deviation of the measured value from the normal intraocular pressure range, the measurement error of NCT and RBT also increase.

ABSTRACT

Objective To investigate the mechanical response of Emerin, a nuclear envelope protein, and its role in apoptosis of vascular smooth muscle cells (VSMCs) during cyclic stretch, and the potential effect of transcriptional factors in this process. Methods Physiological cyclic stretch with the magnitude of 5% and frequency of 1.25 Hz was subjected to VSMCs in vitro by using FX-5000T cyclic stretch loading system. VSMCs cultured under the same conditions but without applying mechanical stretch were used as the static control. The apoptosis of VSMCs was detected by using Cleaved-caspase3 ELISA kit, and the expression of Emerin was revealed by using Western blotting. The effects of Emerin on activities of 345 kinds of transcriptional factors in VSMCs were demonstrated with Protein/DNA array after Emerin specific RNA interference (RNAi) under static condition, and the potential transcriptional factors involved in VSMC apoptosis were analyzed with Ingenurity Pathway Analysis (IPA) software. Furthermore, the binding abilities of Emerin to the motif of 2 kinds of apoptosis-related transcriptional factors were detected with chromatin immunoprecipitation (CHIP) and qPCR. ResultsCompared with the static control, the apoptosis of VSMCs was significantly decreased by 5% cyclic stretch, which suggested a protective effect of physiological cyclic stretch. The expressions of Emerin in VSMCs was remarkably increased with 5% cyclic stretch applied for 6 h, 12 h and 24 h. Specific RNAi under static condition decreased the expressions of Emerin but increased the apoptosis of VSMCs. Emerin siRNA transfection remarkably increased (more than 2 times) the activities of 10 transcriptional factors that participated in cellular apoptosis, i.e. CREB-BP1, p300, p55, MAX, NRF-1, STAT1, STAT3, TEF1, TR and BZP. CHIP-qPCR result revealed that the binding ability of Emerin to specific mofit of STAT1 or STAT3 was significantly repressed with Emerin RNAi. Conclusions Physiological cyclic stretch could increase the expression of Emerin which might modulate the binding of Emerin to motifs of apoptosis-related transcriptional factors such as STAT1 and STAT3, regulate the activities of these factors, and then subsequently repress the VSMC apoptosis. The investigation on mechanobiological mechanisms of VSMC apoptosis induced by cyclic stretch may contribute to further understanding the physiological and pathological mechanisms of vascular homeostasis and vascular remodeling.

OBJECTIVE:To provide reference for strengthening drug regulation in drug administration department. METH-ODS:Drug regulatory statistical yearbooks during 2011-2015 were collected. Literature analysis,content analysis,comparative analysis and secondary analysis were conducted to analyze and explore the drug production license,business license,advertising ap-proval,protection of TCM varieties,drug complaints,investigation and punishment of drug cases,etc. in statistical yearbook. RE-SULTS & CONCLUSIONS:The number of drug manufacturers and operating enterprises had been increasing year by year,while the retail chain stores in certified enterprises showed tendency to go beyond the single retail pharmacy. Compared with the down-ward trend of number of approved drug advertisements,the number of protected varieties of TCM decreased more obviously. The increase in number of drug complaints did not cause the number of investigated and punished drug cases at the same time,which showed a downward trend. It is suggested to further improve the quality and efficiency of drug regulatory work in China by strict drug production license approval,reforming drug advertising regulation and TCM varieties protection system,intensifying efforts to investigate and punish drug complaints and cases,and playing the supervision of public opinion role of the media and the masses.

Objective The study aimed to construct a composite score method based on multidimensional quality indi-cators and conduct simulation trials to validate the method. Besides,the quality of breast cancer care for both hospitals and sur-geons was evaluated by the method. Methods The two-parameter logistic latent variable model was constructed as measure-ment model;the latent variables in the measurement model were further incorporated into multilevel structural model as depend-ent variables and one pseudo level was designed for representing multiple latent variables. MCMC method was used to estimate model parameters. Three level and two-dimensional latent variable model was used to analyze the actual data. Results The sim-ulation study showed that the number of quality indicators and surgeons should not be less than 20 to obtain efficient estimate of quality of care;the multilevel and multidimensional latent variable model was applied to analyze the data;surgeons and hospitals who provided superior quality of breast cancer diagnosis and operative procedure were obtained. Conclusion The newly con-structed multilevel and multidimensional latent variable model could effectively address the hieratical structure in quality of care data as well as the multidimensional nature of quality of care,thus,the model can be used to comprehensively and rationally as-sess the quality of care;comprehensive evaluation of quality of care provided ground for linking the ranking of hospitals and per-formance appraisal of surgeons to the quality of care.

ABSTRACT

OBJECTIVE:To establish an effective technical system to achieve the informatization management in drug produc-tion process,and provide reference for the drug informatization management during production process in China. METHODS:Through analyzing the common problems existing in informatization management in drug production process,above problems were solved by interactive integration marketing system(IIMS)based on mobile internet. Informatization supervision was conducted for the production progress of a batch of drugs in a pharmaceutical production enterprise,and feasibility of the technical system was in-vestigated. RESULTS:There were problems commonly existed in the production process,such as supervision and data collection out of touch,no advanced means,delayed and closed informatization transmission,high cost of informatization collection,hard to develop,etc. IIMS consisted of transit label,informatization collection terminal and monitoring management platform had achieved batch production records paperless data interaction and solved informatization interaction and supervision issues in process of drug quality. In the actual study,IIMS totally supervised 15 links from raw materials purchasing to finished products,and all data were successfully collected. IIMS achieved the real-time remote control of data to make the government easy to regulate,without chang-ing the operational processes,and did not cause economic burden to the enterprises. CONCLUSIONS:The technical system is suit-able for the informatization management in drug production process in pharmaceutical production enterprise,and it can be extended to the scientific research institutes and drug supervision departments in the further.

Methodology: AG490 was applied to C666-1 cells. Cell proliferation was detected by MTT assay. Cell cycle and apoptosis were detected by flow cytometry. The expression of STAT3 and p-STAT3 protein was detected by immobilized Western blot


Results: AG490 could effectively inhibit proliferation of C666-1 cells in vitro, and the inhibitory effect was characterized by time and concentration dependence. AG490 induced apoptosis of nasopharyngeal carcinoma cells, and apoptosis rate increased with prolongation of the time. AG490 could inhibit expression of STAT3 and p-STAT3 protein in C666-1 cells


Conclusion: AG490 can down-regulate expression of STAT3 and p-STAT3 protein in C666-1 cells, inhibit proliferation of nasopharyngeal carcinoma cells and promote apoptosis of nasopharyngeal carcinoma cells

ABSTRACT

Objective To investigate the role of microRNAs (miRs) in the proliferation of vascular smooth muscle cells (VSMCs)induced by endothelial insulin-like growth factor-1 (IGF-1) under low shear stress (LowSS). Methods Endothelial cells (ECs) and VSMCs were co-cultured and exposed to normal shear stress (NSS, 1.5 Pa) and LowSS (0.5 Pa) for 12 h with parallel plate flow chamber system, respectively. Real-time PCR was used to examine the expression levels of miRs. The target genes of miR-133b were predicted by multiple algorithms. The expression of polypyrimidine tract binding protein 1 (Ptbp1) and N-myc downstream regulated 1 (Ndrg1) in VSMCs was detected by Western blotting. The VSMC proliferation was detected by EdU flow cytometry assay. Results After treated with recombinant IGF-1, the expression of both miR-133b and miR-378a in VSMCs was increased. Compared with NSS, LowSS significantly induced the expression of miR-133b in the co-cultured VSMCs, but had no obvious effect on miR-378a. In VSMCs, the protein and mRNA levels of Ptbp1 and Ndrg1 were down-regulated by miR-133b mimics. miR-133b inhibitor up-regulated the mRNA levels of Ptbp1 and Ndrg1. miR-133b overexpression promoted the proliferation of VSMCs significantly. Conclusions IGF-1 secreted by ECs in response to LowSS can upregulate the expression of miR-133b in the co-cultured VSMCs, which subsequently depresses the expression of Ptbp1 and Ndrg1, and induces the proliferation of VSMCs eventually. The research findings provide a potential new target for cardiovascular disease therapy.

ABSTRACT

Objective To investigate the effect of pathologically elevated cyclic strain induced by hypertension on proliferation of vascular smooth muscle cells (VSMCs) and the role of long non-coding RNA (IncRNA)-XR007793 during this process. Methods Flexcell-4000 tension system was used to apply physiologically (5% magnitude) and pathologically (15% magnitude) cyclic strain with frequency of 1.25 Hz on VSMCs for 24 h respectively. qRT-PCR was used to detect the expression of XR007793 and 4 co-expressed genes: signal transducer and activator of transcription 2 (STAT2), cell division cycle associated 8 (CDCA8), proto-oncogene LMO2 and interferon regulatory factor (IRF7). Western blot was used to detect the proliferating cell nuclear antigen (PCNA) level in VSMCs. RNA inference was used to inhibit XR007793 expression. The cell cycle of VSMCs was measured by flow cytometry in static condition and the cell proliferation was detected by Brdu-Elisa in cyclic strain loading condition. Results Compared with 5% cyclic strain, 15% cyclic strain remarkably decreased the XR007793 level and increased the proliferation of VSMCs,along with the increasing expression of STAT2 and CDCA8. XR007793 specific siRNA transfection under static condition decreased the expression of XR007793 and increased the VSMC proliferation. Under 15% cyclic strain, XR007793 specific siRNA transfection also increased the VSMC proliferation and the expression of CDCA8 compared with the non-specific siRNA control. Conclusions Pathologically elevated cyclic strain decreases the XR007793 expression level and increases the CDCA8 expression level to modulate VSMC proliferation. These results provide new experimental evidence for the study of mechanobiological mechanism during hypertension and potential targets for hypertension therapy.

Methods:The effects of water amount,extraction duration and extraction times were investigated by orthogonal design using the contents of forsythia-side A and chlorogenic acid as the indices. Results: The optimum extraction process was as follows: adding 8-fold amount of water, and extracting 1. 5 h for the first time, and then adding 6-fold amount of water, extracting 1 h for the second and third time, respective-ly. Conclusion:The extraction technology is simple, reasonable and reliable.

ABSTRACT

Background:Fibroscan is the noninvasive method widely used to evaluate quantitatively the liver fibrosis and monitor the long-term efficacy of anti-fibrosis therapy. Aims:To study the use of Fibroscan for evaluating the efficacy of combined therapy with FuFang BieJia RuanGan tablet and antiviral drugs in patients with hepatitis B virus( HBV)-related cirrhosis. Methods:A total of 90 patients with HBV-related cirrhosis from March 2013 to September 2014 at Shanghai Ren Ji Hospital were recruited,and divided into treatment group and control group. Patients in treatment group received FuFang BieJia RuanGan tablet,and patients in control group received conventional liver-protective drugs,all the patients took nucleoside antiviral drugs at the same time. The treatment courses in both groups were 6 months. Liver stiffness measurement( LSM)was detected by Fibroscan before and after treatment. Biochemical parameters,width of portal vein and clinical symptoms were recorded. Results:After treatment,LSM was significantly decreased in both groups( P 0. 05). LSM was closely associated with Child-Pugh score both before and after treatment(r=0. 484,P<0. 01;r=0. 523,P<0. 01). Patients with Child-Pugh A had lower LSM than those with Child-Pugh B or Child-Pugh C(P<0. 01). Conclusions:FuFang BieJia RuanGan tablet combined with oral antiviral drugs can remarkably improve the liver function of cirrhotic patients and prevent progression of cirrhosis. Dynamic detection of LSM can be used for monitoring drug efficacy and disease progression in patients with cirrhosis.

ABSTRACT

OBJECTIVE:To observe clinical efficacy and safety of tanshinone ⅡA sulfonate combined with alprostadil in the treatment of acute cerebral infarction. METHODS:56 patients with acute cerebral infarction were randomly divided into treatment group(36 cases)and control group(20 cases). Control group was given tanshinone ⅡA sulfonate 60 mg intravenously,qd,for 10 days. Treatment was additionally given Alprostadil injection 10 μg intravenously,30 min drop lrote,qd,the two group for 10 days,on the basis of control group. Therapeutic efficacy and BI were observed at the end of a treatment course. RESULTS:The ef-fective rate of treatment group and control group were 91.67% and 75.00%,with statistical significance(P<0.05). BI score of 2 groups after treatment were better than before,and the treatment group was better than the control group after treatment,with statis-tical significance (P<0.05). No obvious ADR wad found in both groups. CONCLUSIONS:Tanshinone ⅡA sulfonate combined with alprostadil is effective in the treatment of acute cerebral infarction with good safety.

<p><b>OBJECTIVE</b>To validate transient elastography (Fibroscan) in assessment of hepatic fibrosis in autoimmune hepatitis (AIH).</p><p><b>METHODS</b>Liver stiffness was assessed using Fibroscan in totally 30 patients with AIH. We compared the results of Fibroscan with the Scheuer fibrosis stage in liver biopsy in each patient.</p><p><b>RESULTS</b>4 patients were shown as liver fibrosis stage S0, 6 as S1, 5 as S2, 11 as S3 and 4 as S4. Failure of the Fibroscan measurement occurred in 1 case (3.3%) because of her increased body mass index (BMI). The stiffness of Fibroscan was significantly correlated with the liver biopsy fibrosis stage (r = 0.801, P less than 0.001). The liver stiffnesses between mild and moderate fibrosis (S0-2) and advanced fibrosis (S3-4) were significantly different (t = -3.937, P = 0.001).</p><p><b>CONCLUSION</b>Transient elastography (Fibroscan) is a promising non-invasive method for detection of fibrosis in patients with autoimmune hepatitis. Its use for the follow up and management of these patients and should be evaluated further.</p>

ABSTRACT

<p><b>OBJECTIVE</b>This paper is to investigate the effects of steroid or IFN alpha-2b on apoptosis and cell pathway of fibroblasts from keloids, hypertrophic scars and normal skins and different responses of different fibroblasts.</p><p><b>METHODS</b>6 samples from keloid, hypertrophic scar and normal skin were collected respectively and fibroblasts from different sources were cultured in vitro. After different fibroblasts were treated with dexamethasone (0.1 mg/ml) or IFN alpha-2b (1000 U/ml), Bax and Bcl-2 protein expressions were detected in situ by immunohistochemical staining; DNA ladders of different fibroblasts were observed by gel electrophoresis; and relative activated (phospho-) ERK1/2 and JNK pathways were detected by method of FACE ELISA.</p><p><b>RESULTS</b>Dexamethasone could induce apoptosis of fibroblasts from keloids, hypertrophic scars and normal skins through activating (phospho-) ERK1/2 and JNK pathways; IFN alpha-2b could not induce apoptosis of fibroblasts from different sources. IFN alpha-2b could inhibit (phospho-) ERK1/2 pathway and could not affect (phospho-) JNK pathways of fibroblasts from keloid and hypertrophic scar. IFN alpha-2b could affect neither (phospho-) ERK1/2 pathway nor (phospho-) JNK pathways of fibroblasts from normal skin.</p><p><b>CONCLUSIONS</b>The responses of different fibroblasts to steroid or IFN alpha-2b were different.</p>
