ABSTRACT

Background: There is a substantial increase in the coincidence of diabetes mellitus and cardiomyopathy. The cardiomyopathy may occur in patients who have no evidence of large vessel disease or abnormalities. The early and commonest hemodynamic derangement of diabetic cardiomyopathy is left ventricular diastolic dysfunction. So, the present study was undertaken to assess the prevalence of diastolic dysfunction in patients with type 2 diabetes and to assess the correlation of diastolic dysfunction and HbA1c% levels. Materials and methods: A total of 100 diabetic patients with minimum 5 years duration of diabetes were selected from Malla Reddy Hospital, Suraram from August 2020 to June 2022. Patients with minimum history of 5 years of type 2 diabetes were scrutinized for Doppler echo cardiography and HbA1c levels. Results: Diastolic dysfunction of left ventricle was observed in 58 patients out of 100, of which 54 (93.1%) patients had HbA1c% of > 6.4. 2 (3.4%) patients belong to HbA1c% group of 5.7-6.4. and 2(3.4%) patients belong to HbA1c% of < 5. Conclusion: Our findings indicate that myocardial damage in patients with diabetes affects diastolic function before systolic function. Diabetic cardiomyopathy is characterized by an early diastolic dysfunction and a later systolic dysfunction. Impaired diastolic function was not affected by sex or type of diabetes. Even young patients with diabetics with normal systolic ventricular function have diastolic dysfunction, which serves as a marker of a diabetic cardiomyopathy. Diastolic seems not to correlate with disease duration. HbA1c% can be a very good indicator of long term prognosis. Strong corelation exists between diastolic dysfunction and HbA1c.

ABSTRACT

Background: Non communicable diseases like obesity and diabetes are increasing worldwide, healthcare physicians are also not immune to this morbidity. The objective of this study is to find the prevalence of obesity and diabetes among healthcare physicians. Methods: The study was done at a tertiary care hospital in Pondicherry. All adult health care physicians of both sexes working in the hospital and giving informed consent to participate in the study were included. Pregnant physicians and those who are not willing to give written consent for participation in the study were excluded from the study. The study was done as a cross sectional study using a pretested standardized questionnaire. Age, sex, demographic data, height, weight, diet habits, family history of diabetes, exercise, medication in all the health care physicians were recorded and studied. Results: Of the total one hundred health care physicians, there were 50 male and 50 female physicians. Among the female physicians, there were 17 of age 41 to 50 years. There were 27 male and 34 female physicians with body mass index of 25 to 29.9. Six males and four females had diabetes mellitus. Six physicians were doing regular exercise. Four physicians were taking both oral anti hyperglycemic drugs and insulin. Conclusions: Doctors are aware and educated part of the society but there is high prevalence of obesity and diabetes mellitus among healthcare physicians. Doctor have to be motivated to take care of their health and to prevent lifestyle disease complication. Further regular screening for diabetes and obesity has to be done for doctors.

ABSTRACT

Background: Coronary heart disease is an important cause of morbidity and mortality in postmenopausal women. The present study aimed to compare cardiovascular risk factors: blood pressure, alterations in lipid profile and structural changes in heart in pre- and postmenopausal women

Method: It was a hospital based study including 41 women selected on basis of menstrual history. After taking detailed history, clinical examination was performed and fasting blood sample was collected for lipid profile and blood sugar estimation. Atherogenic indices like Castelli's Risk Index-I and II, Atherogenic Index of Plasma and Atherogenic Coefficient were calculated from lipid profile values. Cardiac evaluation was done by echocardiography

Results: A statistically significant increase in blood pressure in postmenopausal women was observed. A statistically significant increase was also noted in total cholesterol [p<0.01], low density lipoprotein [p<0.02] and Castelli Risk Index-II [p<0.04] n postmenopausal women. An increase in Interventricular Septal Thickness in diastole [p<0.0003], Aortic Diameter [p<0.01], Left Atrial Diameter [p<0.008], Relative Wall Thickness [p<0.005], Left Ventricular Mass [p<0.004] and Left Ventricular Mass indexed to body Surface Area [p<0.002] and Height 2.7 [p<0.001] was also found to be statistically significant in post menopausal women

Conclusion: Menopause leads to dyslipidemia with atherogenic lipid profile with structural changes in left ventricle structure and mass

ABSTRACT

Seckel syndrome is an extremely rare inherited disorder characterized by growth delays prior to birth resulting in low birth weight. Growth delays continue after birth resulting in short stature (dwarfism). This syndrome is associated with an abnormally small head, varying degrees of mental retardation and unusual "beak like" protrusion of nose. Other facial features may include abnormally large eyes, a narrow face, malformed ears and an unusually small jaw. This syndrome has an autosomal recessive pattern of inheritance. A case of the Seckel syndrome is presented.

ABSTRACT

Lymphangiomas are congenital malformations of lymphatic vessels filled with a clear protein-rich fluid containing few lymph cells. It can also occur in association with hemangioma. Large lymphangioma extending into the tissue spaces of neck is referred to as cystic hygroma. Herewith, we present a case of cystic hygroma associated with lymphangioma of tongue leading to macroglossia in a 5-year-old boy.

ABSTRACT

Cemento-osseous dysplasia is probably the most common fibro-osseous lesion encountered in clinical practice. It is thought to be a reactive phenomenon that arises from elements within the periodontal ligament. Here we present a case of 37-year-old female patient, who reported for a regular dental check-up. Incidentally, mixed radiopaque-radiolucent lesions were found bilaterally in the radiograph. Based on the clinical examination and radiographic evaluation, we arrived at a diagnosis of florid cemento-osseous dysplasia.
