ABSTRACT

Priapism is a rare disease, but needs early intervention and appropriate management. We present 2 cases of ischemic low-flow priapism after intracavernosal injection of vasoactive drug. 43-year-old man and 61-year-old man visited our clinic complaining of continuous plan and erection persisting for several days after self cavernosal injection for increasing their erectile function. We performed blood gas analysis of cavernosal blood and color doppler ultrasound. And so we confirmed the diagnosis of low-flow priapism. Detumescence was finally achieved after the proximal shunt operation. Their erectile function was not recovered and they became permanent impotence.

ABSTRACT

Urethral injury studied from Jan. 1976 to Jun. 1980 at Department of Urology, Busan National University Hospital was reviewed respectively with particular attention to etiology, diagnosis and operative procedure. During the period total number of patients with urethral injury was 104, of whom 67 were anterior urethral injury and the remaining 37 were posterior urethral injury. The following results were obtained. 1. The rate of the urethral injury to genitourinary tract was 49. 5 per cent. 2. In anterior urethral injury, the most common cause was industrial accident, the mode of injury was straddling. In posterior urethral injury, the most common case was traffic accident, the mode of injury was blunting. 3. According to the result of treatment in anterior urethral injury, Foley catheter indwelling was not effective in about half cases, but good results were by modified Badenoch`s pull-through urethroplasty just after injury or after long-term cystostomy. In posterior urethral injury, we had poor results with primary realignment just after trauma, but good results with classic Badenoch`s pull-through urethroplasty after long-term cystostomy.
