ABSTRACT

The present study was undertaken to explore the role of autonomic nerves in the regulation of sodium transporters and water channels in the salivary gland. Rats were denervated of their sympathetic or parasympathetic nerves to the submandibular gland. One week later, the expression of Na,K-ATPase, epithelial sodium channels (ENaC), and aquaporins (AQP) was examined in the denervated and contralateral glands. The sympathetic denervation slightly but significantly decreased the expression of alpha1 subunit of Na,K-ATPase, whereas the parasympathetic denervation increased it. The expression of alpha-subunit of ENaC was significantly increased in both the denervated and contralateral glands either by the sympathetic or parasympathetic denervation. The sympathetic denervation significantly increased the expression of AQP5 in both the denervated and contralateral glands, whereas the parasympathetic denervation decreased it. It is suggested that the autonomic nerves have a tonic effect on the regulation of sodium transporters and AQP water channels in the salivary gland.

ABSTRACT

We have experienced 40 cases of the tibial shaft fractures treated with Küntscher nail from 1979 to 1986. Authors analysed these cases and our own experimental study concerned with the shape of Küntscher nail. The shape of Küntscher nail for the tibial shaft fractures should be designed according to the type and location of the fracture. The proximally bent and distally straight nail is used for the extension fracture, proximally and distally bent nail or entirely bent nail is inserted for the flexion fracture of the tibia. The large nail(over 13 mm in diameter) may produce injury to the patella because it has minimal flexibility. In order to permit easy driven down of nail and prevent this injury, the nail should b. bent into three to four segments and the length of the longest segment should not exceed the permissible length of straight nail (Permissible length is distance from entrance of nail to posterior cortex of the upper fragment, where tip of the (nail impinged-about 12cm) The midpoint of the middle segment of dual dent nail is placed at the fracture site. The middle segment of the nail may bent anteriorly for flexion fracture and posteriorly for extension fracture, securing the dynamic fixation of the fracture.

ABSTRACT

The authors performed the interfascicular nerve grafting in the gap of injured nerves using the microsurgical technique to obtain the accurate coaptation of the fascicles. The authors experienced the 33 cases(26 patients) of the interfascicular nerve grafting in the median(11), ulnar(9), radial(2), common peroneal(3) and digital(8) nerves at the Department of Orthopaedic Surgery, College of Medicine, Kyung Hee University from Jan. 1980 to June 1984. The patients were followed up from 12 to 26 months (average 20 1/2 months). The following results were obtained; 1. The shorter the length of the donor nerve and the younger the age of the patients are, the better the recovery of the nerve function is. 2. The medial and lateral cutaneous nerves of forearm as donor nerve are suitable as well as the sural and superficial radial nerves. 3. The end result of each patient was evaluated by Seddon's classification. The excellent motor recovery was 72.0% (18/25) and the excellent sensory recovery was 81.8% (27/33). 4. The interfascicular nerve grafting is an excellent method in treating the widely separated, severed peripheral nerve injury.
