ABSTRACT

Objective:To investigate clinical and laboratory characteristics of secondary hemophagocytic lymphohistiocytosis (sHLH) associated with secondary cutaneous T-cell lymphoma (CTCL) .Methods:CTCL patients with clinically suspected sHLH were collected from Department of Hematology, Wuhan No.1 Hospital from January 2016 to October 2021, and were evaluated according to the HLH-2004 diagnostic criteria and HScore. Results:Seven CTCL patients were confirmedly diagnosed with sHLH, including 2 with primary cutaneous γδT-cell lymphoma (PC-GDTCL) , 3 with cutaneous extranodal natural killer/T-cell lymphoma (C-ENKTCL) , and 2 with primary cutaneous anaplastic large cell lymphoma (PC-ALCL) . All the 7 patients received chemotherapy, but 6 died finally, and the median overall survival duration was 26.5 days (range: 14 - 60 days) after the confirmed diagnosis of CTCL complicated by sHLH. HLH-related gene mutations, which were located in the PRF1 and LYST genes, were identified in 2 patients; lymphoma-related gene mutations were identified in the KRAS and KMT2D genes in 1 PC-GDTCL patient, and in the JAK3 and SAMHD1 genes in another PC-GDTCL patient. Conclusions:CTCL complicated by sHLH usually progresses rapidly, so early diagnosis and treatment are needed. Bone marrow biopsy and mutation screening of lymphoma- and HLH-related genes at initial diagnosis and during disease progression may facilitate early diagnosis.

ABSTRACT

Objective:To observe the effects of rolling method massager on local tissue morphology, tissue and serum TNF-α and IL-1β in rabbits with skeletal muscle injury at different time points; To investigate the mechanism of temporal effect of rolling method action on skeletal muscle injury. Methods:Totally 72 New Zealand rabbits were divided into blank group, model group and rolling method treatment group according to random number table method, with 24 rabbits in each group. Rabbits in each group were divided into 1 d, 3 d, 5 d, 7 d, 9 d and 11 d subgroups according to the time point of injury, with 4 rabbits in each group. Blunt contusion was used to model the model group and the rolling method treatment group. Each subgroup of the rolling method treatment group was subjected to rolling method intervention for 3 d, using a homemade rolling method massager, 2 times/d, 3 min/time. At 24 h after the completion of the intervention, the histomorphological changes were observed by HE staining, and the TNF-α and IL-1β contents in serum and damaged skeletal muscle tissues were detected by ELISA.Results:Compared with the blank group, the inflammatory cell infiltration in the model group was obvious, edema was severe, and myofibers were broken; the inflammatory cell infiltration in the 1 d rolling method treatment group was intensified, myocytes were apoptotic, and myofibers were broken and necrosed more seriously; the inflammation in the 7 d rolling method treatment group was obviously improved with the best effect, and the difference with normal healthy muscle tissue was smaller. After modeling, TNF-α and IL-1β levels in skeletal muscle tissues and serum TNF-α levels were higher in the 3 d model group than in the 1 d model group ( P<0.05). Compared with the blank group, TNF-α and IL-1β levels in skeletal muscle tissues and serum increased in each subgroup of the model group and each subgroup of the rolling method treatment group ( P<0.01); Compared with the 1 d model group, TNF-α and IL-1β levels in skeletal muscle tissues and serum TNF-α levels increased in the 1 d rolling method treatment group. The levels of TNF-α and IL-1β in the 3 d, 5 d, 7 d, 9 d and 11 d rolling method treatment group were lower than those in the model group subgroup ( P<0.05). TNF-α and IL-1β levels in skeletal muscle tissues and serum TNF-α levels were higher in the 1 d, 3 d and 5 d rolling method treatment group than in the 7 d rolling method treatment group ( P<0.05). TNF-α levels in skeletal muscle tissues were higher in the 1 d and 3 d rolling method treatment group than in the 7 d rolling method treatment group ( P<0.05). Conclusion:The inflammatory factors in the rolling treated group were significantly higher at 1 d after skeletal muscle injury, indicating that treatment with the rolling method was inappropriate at this time; seven days after injury, the application of rolling method can reduce the inflammatory effect, accelerate the repair of skeletal muscle, and improve the quality of functional recovery.

ABSTRACT

17α hydroxylase is a key enzyme for the conversion of progesterone to prepare various progestational drug intermediates. To improve the specific hydroxylation capability of this enzyme in steroid biocatalysis, the CYP260A1 derived from cellulose-mucilaginous bacteria Sorangium cellulosum Soce56 and the Fpr and bovine adrenal-derived Adx4-108 derived from Escherichia coli str. K-12 were used to construct a new electron transfer system for the conversion of progesterone. Selective mutation of CYP260A1 resulted in a mutant S276I with significantly enhanced 17α hydroxylase activity, and the yield of 17α-OH progesterone reached 58% after optimization of the catalytic system in vitro. In addition, the effect of phosphorylation of the ferredoxin Adx4-108 on 17α hydroxyl activity was evaluated using a targeted mutation technique, and the results showed that the mutation Adx4-108T69E transferred electrons to S276I more efficiently, which further enhanced the catalytic specificity in the C17 position of progesterone, and the yield of 17α-OH progesterone was eventually increased to 74%. This study provides a new option for the production of 17α-OH progesterone by specific transformation of bacterial-derived 17α hydroxylase, and lays a theoretical foundation for the industrial production of progesterone analogs using biotransformation method.

ABSTRACT

[Objective]To explore the pathological mechanism and treatment of polycystic ovary syndrome in traditional Chinese from the angle of five viscera, and to enrich the theory in differentiation syndrome of solid-hollow viscera.[Methods]Based on ancient literature and modern clinic investigation,we discuss polycystic ovary syndrome from five viscera,at the same time, different viscera has its own etiology and pathogenesis,so according to five viscera syndrome differentiation,we can take different treatment and prescription respectively. [Results]The generation, transmission, mutual interaction of qi,blood, essence, body fluids in five viscera are dynamic. The pathological mechanism of polycystic ovary syndrome is based on the imbalance of qi,blood, essence, body fluids in internal organs, while phlegm wet and blood stasis are considered as treetop.Consequently, which are seen as the radical treatment such as to tonify and diffuse the lung, replenish qi and to calm down the heart, invigorate the spleen and nourish blood, to clear and soothe the liver, to tonify the kidney and secure the essence. While dampness drying, phlegm resolving, and blood activation are the treatment of treetop aspect, then they can reach the purpose of simultaneous treatment of treetop and root.As a result, the physiological function of five viscera will recover, clinical symptoms such as menstrual disorder, infertility, hirsutism and acne will be improved.[Conclusion]In clinic,we should start from the overall, broaden the train of thought, develop and innovate the application in differentiation syndrome of solid-hollow viscera.

ABSTRACT

<p><b>OBJECTIVE</b>To establish objectively biomechanical criteria for temporomandibular joint disc tissue engineerings.</p><p><b>METHODS</b>Eight temporomandibular joint discs from a four-month-old goat were separated completely. A cylindrical sample with diameter of 3 mm in the anterior, intermediate, and posterior bands was obtained, and the samples underwent pre-compression test with three cycles under a speed of 0.025 mm · s⁻¹. With the end condition for the maximum load of less than 4.9 N and 1.5 mm compression displacement, the formal compression test was performed with the same speed. Moreover, this test obtained the stress-strain relationship and elastic modulus of each disc band. Ultrastructure and collagen fiber orientation of the district zone were observed by scanning electron microscope and polarized light microscope.</p><p><b>RESULTS</b>1) The physiological strain range of an articular disc was within 10%, and the quasilinear constitutive equation of articular disc tissues can be fitted with the polynomial function: y = ax + bx² + cx³. 2) The elastic modulus of the intermediate disc zone was the largest (P < 0.05). No significant difference existed between the anterior and posterior bands (P = 0.361). 3) Scanning electron microscope and polarized light microscope showed an annular disc collagen network structure, which was the internal part of the goat temporomandibular joint disc. The collagen arrangement of intermediate bands was tensely arranged anterior-posteriorly. The collagen of anterior and posterior bands went through mediolaterally with intersection of anterior-posterior branch fibersd.</p><p><b>CONCLUSION</b>Annular collagen network structure is the basis for goat temporomandibular joint disc compression resistance properties. The intermediate band demonstrated higher compression resistance performance than the anterior and posterior bands.</p>

Objective To evaluate the effect of remifentanil preconditioning on hepatic ischemiarepeffusion (I/R) injury in rats with liver cirrhosis. Methods Healthy male Sprague-Dawley rats, weighing 160180 g,received 40% tetrachloride carbon 3 ml/kg (in peanut oil) intragastrically twice a week for 8 weeks to induce liver cirrhosis.Thirty-five rats with liver cirrhosis were randomly divided into 5 groups (n =7 each) using a random number table:sham operation group (group S),group I/R,and preconditioning with different does of remifentanil groups (R1,R2 and R3 groups).Hepatic I/R injury was induced by clamping the branches of hepatic artery, portal vein and common bile duct in the left and median hepatic lobes for 30 min followed by 90 min reperfusion in anesthetized rats with liver cirrhosis. In R1,R2 and R3 groups, remifentanil was infused intravenously at 0.4,2.0 and 10.0 μg· kg-1· min-1 for 15 min, respectively, and the rats were exposed to ischemia for 30 min followed by 30 reperfusion starting from 10 min after infusion was stopped. At 90 min of reperfusion, blood samples were collected from the abdominal aorta for determination of serum alanine aminotransferase (ALT) and aspartate aminotransferase (AST) activities. The rats were then sacrificed and livers were removed for determination of superoxide dismutase (SOD) activity, malondialdehyde (MDA) content, and expression of activated caspase-3 (using immunohistochemistry and Western blot analysis) and for detection of cell apoptosis in hepatic tissues.Apoptotic index was calculated. Results Compared with group S,the serum ALT and AST activities,MDA content and apoptotic index were significantly increased,SOD activity was decreased, and the expression of activated caspase-3 was up-regulated in I/R,R1,R2 and R3 groups.Compared with group I/R,no significant changes were found in the indexes mentioned above in group I/R,and the serum ALT and AST activities,MDA coment and apoptotic index were significantly decreased,SOD activity was increased, and the expression of activated caspase-3 was down-regulated in R2 and R3 groups. Conclusion Remifentanil preconditioning can reduce hepatic I/R injury in rats with liver cirrhosis, and the mechanism is related to inhibition of the lipid peroxidation and cell apoptosis.

ABSTRACT

Objective To establish the CTRP4 transgenic mouse model and investigate the function of the novel adipocytokine CTRP4.Methods CTRP4 overexpressing vector in pCAGGS was firstly constructed and then microinjected into zygote to establish the founder transgenic mice .F1 heterozygotes were generated by founder mice mating with wildtype mice, and the CTRP4 transgenic homozygotes were generated by F 1 littermates. The genotype was confirmed by PCR and test cross method .The expression level of CTRP 4 in transgenic mice was detected by western blot .Result The human CTRP4 transgenic homozygote mice line was established , and the expression level of CTRP 4 was confirmed raletively high in detected tissues including heart , liver, brain and kidney . Conclusion The human CTRP4 transgenic mice was successfully established .

ABSTRACT

For decades, there have been no systematic studies on the reference intervals of Chinese populations. With the support of the Ministry of Health, the Chinese Society of Laboratory Medicine launches a research project investigating the Chinese reference intervals for common clinical laboratory tests.Up to now,the first batch of reference intervals for common liver function, electrolytes and blood cell analysis have been published in forms of healthy professional standards.Medical institutions are suggested to use these new reference intervals in clinical practice.
