ABSTRACT

Objective:To evaluate the clinical efficacy of five different Chinese patent medicines combined with conventional western medicine therapy in the treatment of coronary heart disease with anxiety and depression. Methods:Randomized controlled trials (RCTs) of Chinese patent medicine combined with conventional western medicine therapy and conventional western medicine therapy in the treatment of coronary heart disease with anxiety and depression were retrieved from China Academic Journal Database (Wanfang Data), China Biomedical Literature Service System (SinoMed), China National Knowledge Infrastructure (CNKI), Chinese Science and Technology Journal Database (Chongqing VIP), Cochrane Library, PubMed and Embase databases from January 1, 2010 to December 31, 2020. The Jadad score and Cochrane bias risk assessment tool were used to assess the bias risk of the included literature. The RevMan 5.3, Stata 16.0 and GeMTC 14.3 software were used for network meta-analysis.Results:A total of 32 articles involving 3 494 patients were included. In terms of clinical efficacy, the clinical efficacy of conventional western medicine combined with Zhenyuan Capsule [ OR (95% CI)=16.64 (6.38, 43.44)], Xinkeshu Tablets [ OR (95% CI)=4.67 (3.26, 6.68)], Wuling Capsule [ OR (95% CI)=4.65 (2.48, 8.72)], Guanxinjing Capsule [ OR (95% CI)=2.93 (1.37, 2.64)] weres better than that of conventional western medicine alone. The clinical efficacy of combined Zhenyuan Capsule was better than that of combined Xinkeshu Tablets [ OR (95% CI)=3.56 (1.28, 9.94)], Wuling Capsule [ OR (95% CI)=3.58 (1.13, 11.34)], Guanxinjing Capsule [ OR (95% CI)=5.69 (1.68, 19.32)] or Shugan Jieyu Capsule [ OR (95% CI)=9.29 (2.79, 30.96)]. Compared with Shugan Jieyu Capsule, Xinkeshu Tablets had better clinical efficacy [ OR (95% CI)=2.61 (1.16, 5.87)]. The SUCRA order of the effective rate of clinical efficacy was as follows conventional western medicine treatment combined with Zhenyuan Capsule (SUCRA=99.6)>with Xinkeshu Tablets (SUCRA=67.5) > with Wuling Capsule (SUCRA=65.0) > with Guanxinjing Capsule (SUCRA=41.6) > with Shugan Jieyu Capsule (SUCRA=25.8)>conventional western medicine treatment (SUCRA=0.5). Combined Xinkeshu Tablets [ MD (95% CI)=-8.85 (-14.16, -3.62)] was superior to conventional western medicine in reducing HAMD score. In terms of reducing HAMA score, compared with conventional western medicine therapy, the combination of Wuling Capsule [ MD (95% CI)=-7.61 (-14.82, -0.40)] and Xinkeshu Tablets [ MD=-6.18, 95% CI (-9.78, -2.58)] has better curative effect. The SUCRA of Chinese patent medicine in reducing HAMA of coronary heart disease complicated with anxiety and depression was combined with Wuling Capsule (SUCRA=82.8) > with Xinkeshu Tablets (SUCRA=78.2). In terms of adverse reactions, the safety of combined five Chinese patent medicines was better than that of conventional western medicine. Conclusions:The clinical efficacy and safety of conventional western medicine combined with five kinds of Chinese patent medicines in the treatment of coronary heart disease complicated with anxiety and depression were better than those of conventional western medicine alone. Among them, the combination of Zhenyuan Capsule was the most likely to be the best treatment. The combination of Xinkeshu Tablets was better in reducing HAMD scores, and the combination of Xinkeshu Tablets and Wuling Capsule is better in reducing HAMA scores. More high-quality RCT studies are needed to verify the conclusions.

ABSTRACT

Transport monitoring is an important link in the transport process of radioactive materials, involving the whole process, that is, before departure, during transportation and after arrival. To do well in transport monitoring, radiation monitoring organizations need to actively participate in the work, and also need to do a good job in the supervision of radioactive materials transport monitoring. Beijing is one of the radioactive materials distribution centers in China, the monitoring workload is large, the number of monitoring reports is large, the number of packages by air transportation is relatively large, and the transportation and monitoring for medical use is dominant. Through the introduction of radioactive materials transportation monitoring work, and analysis of 2006—2020 radioactive materials transportation monitoring situation, this paper discusses the problems that need to be paid attention to in the monitoring work and the ways to deal with them, so as to provide ideas for radiation monitoring organizations to carry out monitoring work and manage the transport of radioactive materials well.

ABSTRACT

The goal of diabetic drug treatment is to stabilize the blood sugar for a long time to close to the normal level, to correct the metabolic disorder and eliminate the symptoms. At present, glimepiride has become commonly used drugs for the treatment of diabetes with obesity. Compared with metformin, acarbose and rosiglitazone, glimepiride has different mechanisms of drug action, clinical combination showed synergistic hypoglycemic effect, good clinical curative effect. So, we use three treatments to study as group A [glimepiride and metformin]; group B [glimepiride and acarbose]; Group C [glimepiride and rosiglitazone]. From the analysis of drug economics, glimepiride and metformin scheme is better, has the lowest cost per unit cost effect. From the comparison of scheme is efficient, the best curative effect is rosiglitazone plus glimepiride, effective rate as 96.7%. At the same time, the drug can be rationally used to reduce the occurrence of some drug-induced diseases and adverse drug reactions

ABSTRACT

Objective Zika virus disease is an acute infectious disease caused by Zika virus transmitted through Aedes mosquitoes. To explore the therapeutic effect of integrated traditional Chinese and Western Medicine for Zika virus disease, the treatment process of the first imported case in China was reviewed. Methods The first imported Zika virus disease in China was admitted to Ganxian People's Hospital in Jiangxi Province on February 6th, 2016, and the patient received isolation treatment for 9 days and cured later. The effect of antiviral treatments including Xiyanping injection was evaluated based on clinical diagnosis and treatment process of the patient. Results A 34-year old male patient was admitted with chief complaint of fever for 9 days, orbital pain and itching rash for 4 days on February 6th, 2016. ① Epidemiological characteristics: the patient was bitted by mosquitoes during his business trip in Venezuela since January 1st, where Zika virus disease was spreading. On January 20th he had dizziness without fever, and the symptom disappeared after taking medicines without details. Paroxysmal dizziness, chills and mild fever without myalgia was experienced on January 28th. On February 3rd small red rash appeared in the neck, spreading to anterior part of chest, limbs and trunk, and the fever, fatigue, nausea was continued, and a new symptom of paroxysmal pain in back of ears and orbits appeared, during which he had not go to hospital. The symptoms relieved on February 4th. He returned to Ganxian County on February 5th, he had yellow stool 3 times with normal temperature, without abdominal pain, and red rash still appeared in the neck. He went to Ganxian People's Hospital on February 6th, 2016. ② Clinical manifestation: the vital signs showed a temperature of 36.8 ℃, a pulse rate of 80 bpm, a respiratory rate of 20 bpm, and a blood pressure of 110/70 mmHg (1 mmHg = 0.133 kPa). It was showed by physical examination that red rash appeared in the neck, and no superficial enlarged lymph nodes were found. Bilateral conjunctival congestion was obvious, physiological reflex existed and pathological reflex was not found. ③ Auxiliary lab test and examination: no abnormal finding were revealed throughout examination and laboratory tests, including routine blood test, liver function, renal function, serum myocardial enzyme, electrolyte, blood sugar, C-reactive protein (CRP), troponin I (TnI), and procalcitonin (PCT), except slight prolongation in activated partial thromboplastin time (APTT, 38.6 s) on February 6th; and slightly dense shadow in left lung in lung CT scan, considering inflammatory changes and slight emphysema (especially in the left lower lung) as well as bilateral renal calculus on February 8th. No significant abnormalities were found in electrocardiogram and B ultrasound test of liver, spleen, and pancreas. ④ Virus confirmation: Zika virus nucleic acid was positive reported by Jiangxi Province Center for Disease Control and Prevention (CDC) on February 7th and Chinese CDC on February 9th, respectively, though Dengue virus were negative reported by Ganzhou CDC on February 6th. Right after the first diagnosis, anyone who had been in close contact with the patient received medical monitoring. ⑤Treatment process: on February 6th, symptomatic treatment was prescribed since admitted into the infectious isolation wards and daily intravenous drip of Xiyanping injection 250 mg was prescribed for antiviral therapy. On February 7th, the patient had no fever, with occasional chills, neck rash was disappeared, orbital pain relieved and bilateral conjunctival hyperemia range was paler and narrowed, and his condition improved. Ibuprofen was administered for defervesce 3 times a day when his temperature reached to 37.5 ℃ at 16:00. On February 8th, the patient had no fever, times of chills was significantly reduced, without myalgia and rash, orbital pain and conjunctival hyperemia further recovered. On February 9th, bilateral eyes slightly tingling, mild conjunctival congestion, no fever chills or other discomfort was found. The chloramphenicol eye drops was prescribed for relieving sting pain with conjunctival congestion twice a day as recombinant human interferon alpha eye drops was out of store. The patient was comfortable from February 11th to February 13th. Blood and urine test for Zika were reported negative by the Chinese CDC and Jiangxi Province CDC. Because all the discharge criteria were satisfied, the patient was discharged on February 14th. Conclusions At present, there is no specific effective drug to prevent and treat Zika virus disease effectually. After receiving symptomatic treatment and antiviral treatments including Xiyanping injection, the patient's symptoms were relieved. Zika virus nucleic acid in blood and urine was negative. The patient was discharged. Combination of traditional Chinese medicine and Western medicine maybe a good method to prevent and treat Zika virus disease.

ABSTRACT

OBJECTIVE:To establish a new semi-automatic drug dispensing mode, with proper cost, which falls in between full-automatic drug dispensing mode and manual drug dispensing mode, good work efficiency, standard and simple operation meth-od and meets national laws and regulations. METHODS:A semi-automatic drug dispensing system was designed,in which the in-formation in the drug dispensing sheet could be automatically printed on the drug bag,and automatic bagging, packaging and deliv-ery of drugs were realized. Such drug dispensing system included hardware(mechanical structures such as drug turntable and drug funnel, transmission device, etc.)and software control systems(the program of interface with hospital information system, micro control unit software, computer software,etc.). Through commissioning, formal operation and statistics, based on 18 oral drug dis-pensing sheets with the same contents, calculated the time of drug dispensing and the number of drug dispensing errors by 3 phar-macists respectively in manual drug bag dispensing mode and semi-automatic drug dispensing mode,to evaluate the effect of the semi-automatic drug dispensing system. RESULTS:From commissioning in May 2012 to formal operation in September 2012,the system operated normally and utility model patents were obtained. In the two modes, the total time of drug dispensing was 481 and 397 min (t=6.82,P<0.001),the numbers of drug dispensing errors were 25 and 7 (χ2=9.353 8,P=0.002 2),respectively. There was statistical significance. CONCLUSIONS:The semi-automatic drug dispensing system has higher efficiency and less num-ber of drug dispensing errors compared with manual drug bag dispensing mode and lower cost compared to full-automatic drug dis-pensing system. It deserves promotion.

ABSTRACT

Objective To explore the efficacy and safety of the bronchoscopic high frequency electrocoagulation combined with balloon dilatation in treating tuberculosis inflammatory airway constriction. Methods According to the different methods of treatment, 55 patients with tuberculosis airway constriction were randomly divided into two groups, the balloon dilatation group (26 cases) and combination group (29 cases). The patients in balloon dilatation group underwent bronchoscopic balloon dilatation and the patients in combination group underwent bronchoscopic balloon dilatation combined with high frequency electrocoagulation. The patients of the two groups accepted endoscopic therapy once a week. Effective rate of recanalization for the narrow airway, frequency of effective treatment and the time of tuberculosis bacterium vanishing was recorded. Intraoperative and postoperative complications were also observed. Three months after the treatment, all patients accepted bronchoscopic to observe and assess the airway restenosis rate. Results After treatment, the effective rate in balloon dilatation group and combination group had no significant difference[69.2%(18/26) vs. 89.7% (26/29 )](P> 0.05 ),but frequency of effective treatment and time of tuberculosis bacterium vanishing had significant difference[(3.5 ±1.3) times vs. (1.5 ± 1.1) times, (23.3 ±3.6) d vs.(13.2 ±2.3) d](P<0.01). There was no significant difference on the intraoperative and postoperative complications between two groups (P>0.05). The airway restenosis rate was 33.3%(6/18) in balloon dilatation group and 7.7%(2/26) in combination group after treatment for 3 months (P <0.05). Conclusions Combination of bronchoscopic balloon dilatation and high frequency electrocoagulation is an efficacy and safety way for the tuberculosis inflammatory airway stenosis. ABSTRACT

Objective To understand the elonal expansion and genetic diversity of Salmonella en-terica semtype Paratyphi A (SPA) and to construct a typing method to determine the epidemic clones of the isolates. Methods Antimicrobial susceptibility testing was performed with 3980 SPA isolates by the cen-trolled Kirby-Bauer disc diffusion technique on Muller-Hinton agar plates. A total of 15 SPA with nalidixie acid resistance for mutations in gyrA, gyrB, gyrC and gyrE genes within the quinolone-resistant determina-tion region (QRDR) were examined. Subtyping of 121 isolates of SPA from seven counties in Yuxi were studied using pulsed-field gel eleetrophoresis (PFGE) analysis following digestion of chromosomal DNA with restriction endanucleases Spe Ⅰ and Xba Ⅰ. PFGE patterns were analyzed by duster analysis. Results The nalidixic acid-susceptible isolates predominated in 1999 but was replaced by nalidixic acid -resistant (NAR) isolates after 2000. Amplification by PCR and sequencing of the genes with subsets of 15 NAR strains re-vealed that the resistance mechanisms had resulted from single point mutations in the gyrA gene. Spe Ⅰ and Xba Ⅰ digestion of 121 isolates gave five and four different PFGE patterns with the predominance of the Spe Ⅰ 01 and Spe Ⅰ 02 (or the Xba Ⅰ 01) epidemic patterns, respectively. Spe Ⅰ 01 and Spe Ⅰ 02 consisted of 37.2% and 57.9% of isolates, respectively, or Xba Ⅰ 01 consisted of 95.0% of isolates. Conclusion The incidence of resistance to nalidixic acid of the isolates increased during the study period. PFGE patterns Spe Ⅰ 01 and Spe Ⅰ 02 (or Xba Ⅰ 01), the main clones of the epidemics, are highly prevalent in Yuxi. PFGE with Spe Ⅰ and Xba Ⅰ is a useful technique to differentiate SPA.
