ABSTRACT

ObjectiveTo investigate the unmet needs for assistive technology for people with physical disabilities in Chengdu, and analyze the related factors. MethodsFrom November, 2023 to March, 2024, the persons with physical disabilities in Chengdu were selected from Sichuan Individuation service platform, and investigated using World Health Organization rapid Assistive Technology Assessment. ResultsA total of 558 questionnaires were set up, and 527 effective questionnaires retured. 26.8% of them reported unmet needs for aids, with the highest need for mobility aids (66.0%). Lack of support (54.9%), high price (26.3%) and lack of knowledge about aids (20.3%) were the main reasons for not obtaining the aids they needed. Loss of spouse (OR = 3.615), serious mobility impairment (OR > 2.926) and serious self-care impairment (OR > 2.781) were the risks of unmet needs for aids. ConclusionIt is important to popularize policies and products of aids, pay attention to personal adaptation for people with different barriers, and strengthen the service system, to meet the needs of people with disabilities.

ABSTRACT

ObjectiveTo investigate the demand, supply and satisfaction of the Chinese people on assistive technology. MethodsBased on the data from the World Health Organization rapid Assistive Technology Assessment (rATA) questionnaire in six provinces and cities in China, the assistive technology of the whole population in China was described from three aspects of demand, supply and satisfaction. ResultsA total of 15 234 valid questionnaires were obtained. In terms of demand, the proportion of visual impairment was the highest in all the functional impairment categories, the highest demands in the list were glasses (low-vision, short/long distance etc.) and walking sticks (canes/sticks, tripod and quadripod), while the highest demand outside the list was anti-skid pad. In terms of supply, the private sector was the main one, followed by government facility and public hospital, and most users took "paid out-of-pocket (self)" to obtain assistive technology, the distance to access the assistive technology was concentrated at "less than five kilometers", and the biggest obstacle to obtain assistive technology was lack of knowledge about assistive technology. Overall satisfaction with assistive technology was optimistic, the adaptation degree and service quality of assistive technology were the most unsatisfactory factors for users. ConclusionIn China, the satisfaction of assistive technology is optimistic, and there is a few amount of unmet assistive technology. It is necessary to propagandize assistive technology, and its manual should be readable, the assistive technical service team should be multidisciplinary, enterprises should promote independent innovation, and the professionals should be regulated.

ABSTRACT

@#Objective To investigate visual impairment students' quality of life and its influencing factors in Braille learning class at Quanzhou Special School. Methods November, 2020, 52 students (aged seven to 26) with the best corrected distance visual acuity of the better eye above 0.02 were investigated with near visual acuity, contrast sensitivity and Chinese-version Low Vision Quality of Life Questionnaire (CLVQOL). The subjects were divided into second grade blindness group, first grade low vision group and second grade low vision group according to the best corrected distance visual acuity of the better eye. Results There were significant differences in distance visual acuity (Z = 45.671, P < 0.001), near visual acuity (Z = 24.972, P < 0.001), and contrast sensitivity (CS) ( Z = 13.285, P = 0.001) among three groups. There was a correlation between near visual acuity and distance visual acuity (r = 0.74, P < 0.001), CS to distance visual acuity (r = -0.58, P < 0.001) and near visual acuity (r = -0.57, P < 0.001), score of CLVQOL and CS (r = 0.44, P < 0.001). There were significant differences in the total score (Z = 10.145, P = 0.006), distance visual acuity subscale (Z = 13.586, P = 0.001), psychological adjustment subscale (Z = 7.824, P = 0.020), reading and fine work subscale (Z = 7.923, P = 0.019) of CLVQOL among the three groups. Conclusion Quality of life is different with the visual impairment for students in special school, especially the distance visual acuity, psychological adjustment and fine reading. CS correlates to the quality of life of visually impaired students, which needs to be a part of evaluation of visual function.

ABSTRACT

Objective To analyze the effect ofmethylprednisolone stosstherapy on CD4+CD25+ regulatory T cell (Treg) and apoptosis factor CD95 expressions in the external peripheral blood of myasthenia gravis (MG) patients and explore the influencing factors of different clinical efficacy.Methods Thirty-one patients with MG,admitted to our hospital from January 2013 to September 2015,were included in this study;and 27 normal subjects were selected as control group.Methylprednisolone stosstherapy was given to the patient group.Expressions of Treg and CD95 in peripheral blood were detected by flow cytometry before and after treatment.Results Treg percentage of patients whose clinical absolute scale scores less or equal than 3 points and relative scale scores higher than 0.5 after methylprednisolonestosstherapy (13.39%+2.71%) was significantly higher than that ofhealthy controls (8.35%+1.87%,P<0.05);Treg percentage of patients whose clinical absolute scale scores greater than 3 as well as relative scale scores less than 0.5 after methylprednisolone stosstherapy (8.17%+1.31%) was slightly lower than that of healthy controls (P>0.05).CD95 percentage of patients whose clinical absolute scale scores less or equal than 3 points and relative scale scores higher than 0.5 after methylprednisolone stosstherapy (36.47%±5.32%) was not statistically different as compared with that of healthy controls (35.28%±5.58%,P>0.05),CD95 percentage of patients whose clinical absolute scale scores greater than 3 as well as relative scale scores less than 0.5 after methylprednisolone stosstherapy (34.97%±5.12%) was not statistically different as compared with that of healthy controls (P>0.05).Conclusion Treg ratio increases,CD95 change is not obvious in patients whose clinical curative effect is improved markedly after methylprednisolone stosstherapy.

ABSTRACT

Objective To investigate the correlation between microembolic signal (MES) and immune inflammation in patients with acute ischemic stroke. Methods The consecutive patients with acute ischemic stroke were enroled. According to the results of MES, they were divided into either a positive group or a negative group. The Immune inflammatory indexes, demographics, and baseline clinical data in both groups were compared. Multivariate logistic regression analysis was used to analyze the independent influencing factors of MES in acute ischemic stroke. Results A total of 237 patients were enroled, including 52 in the MES positive group and 185 in the MES negative group. There were significant differences in the levels of triglyceride (2. 130 ± 0. 933 mmol/L vs. 1. 811 ± 0. 962 mmol/L; t = 2. 126, P = 0. 035), plasma fibrinogen (2. 946 ± 0. 255 g/L vs. 2. 833 ± 0. 322 g/L; t = 2. 332, P = 0. 021 ), Lp-PLA2 level ( 288. 265 ± 27. 855 μg/L vs. 261. 652 ± 29. 961 μg/L; t = 2. 897, P = 0. 004 ), as wel as the proportions of CD4 + CD25high Treg (8. 695% ± 1. 461% vs. 9. 445% ± 1. 397% ; t = 3. 386, P = 0. 001), artery stenosis ≥70% (21. 15% vs. 5. 41% ; χ2 = 10. 592, P = 0. 001 ) and smal arterial occlusive stroke (9. 62% vs. 23. 24% ; χ2 = 4.667, P = 0. 031) between the MES positive group and the MES negative group. Multivariate logistic regression analysis showed that the increased plasma fibrinogen level (odds ratio [OR] 3. 257, 95%confidence interval [CI] 1. 124 - 9. 438; P = 0. 030), artery stenosis ≥ 70% (OR 3. 585, 95% CI 1. 394 -9. 219; P = 0. 008), and the decreased ratio of Treg (OR 3. 801, 95% CI 1. 190 - 12. 148; P = 0. 024) were the independent risk factors for positive MES, and smal arterial occlusive stroke was its independent protective factor (OR 0. 244, 95% CI 0. 072 - 0. 829; P = 0. 024). Conclusions MES may be associated with immune inflammation. The relationship between stroke and immune inflammation should be taken seriously.
