ABSTRACT

Objective:To explore the clinical effect of cosmetic reconstruction for partial defect of distal segment of digits. Methods:Form January 2018 to January 2021, the Department of Hand Surgery of Institute for Hygiene of Ordnance Industry(The 521 Hospital of Weapon Industry) admitted 129 patients with partial defect of distal segment of thumb or fingers with phalange or tendon exposure. The patients were 111 males and 18 females with an average age of 34(17-59) years old. The sizes of nailbed defect were 0.4 cm×1.1 cm-1.8 cm×2.0 cm, the length of phalange defect was 0.4-1.8 cm, and the sizes of the soft tissue defect were 1.6 cm×1.8 cm-3.2 cm×4.8 cm. Great toe tissue flaps were used to reconstruct the partial defect of distal segment of thumb or fingers after debridement. Wounds of fibular flap of great toe in 77 cases were directly sutured in 17 patients. The donor sites in rest 60 great toes were narrowed first and then repaired with skin grafts in 10 cases, with artificial dermis in 28 cases and with transverse V-Y advancement flaps of ipsilateral great toes for 22 cases. Forty-nine of 52 donor site wounds for hallux toenail flap were repaired with artificial dermis and 3 with free peroneal artery perforator flaps. The method was outpatient follow-up. Postoperative follow-up lasted until July 2022. The check-items for follow-up included: occurrence of necrosis, appearance, shape and texture of the flap, appearance of the reconstructed nails, TPD of the reconstructed digit pulps, tolerance to cold on the scars of flaps, flexion and extension of the reconstructed digits. The healing time of phalanges of the reconstructed digits was evaluated by X-rays. The appearance, sensation, the tolerance to cold of great toe and the movement of donor foot were also assessed. Results:Postoperative follow-up lasted for 18 to 24 months, with an average of 21 months. A total of 128 flaps survived. Necrosis occurred in 1 fibular hallux flap, the necrosis was cured with a reverse digital proper artery island flap. Thereafter, all flaps healed well. The appearance, shape, texture and nails of reconstructed digits were close to the contralateral digits. The reconstructed thumb and finger were evaluated according to Zook, 127 cases were excellent and 2 cases were good.TPD of the pulps of the reconstructed digits was 4-10 mm. The mean score of the Vancouver scar scale(VSS) was 0.6 for scars of the reconstructed digits. The mean score of the Visual analog scale(VAS) was 0.3 for the tolerance to cold. Flexion and extension function recovered well in all the reconstructed digits. According to the Evaluation Criteria of Upper limb Function Hand Surgery Society of Chinese Medical Association, the function of hand was excellent in 127 cases and 1 in each of good and fair. X-rays of all digits showed the phalange healing of the reconstructed digits, with an average healing time of 2 months after surgery, without phalange resorption, infection, nonunion nor stress fracture. There was no difference between the length of the donor great toe and the contralateral toe, except the donor site of the great toe nail root. There was no significant visual difference between the appearance of the donor great toe and the contralateral toe. TPD of the pulps of donor great toe was 4-8 mm. The mean score of the VSS was 1.4 for scars in the donor great toe. The mean score of the VAS was 0.7 for the tolerance to cold of the donor great toe. There was no stress fracture at donor site, and the functions of donor foot were not affected when walking, running, jumping and tiptoeing in all patients. Conclusion:It is an ideal method for reconstructing a partial defect of distal segment of digit by great toe flap. It can not only reconstruct the partial defect of distal segment of the digit, but also results in a good appearance and satisfactory functions of the reconstructed digit. Damage to the donor site is minimum. The length of the donor great toe is unchanged, and it has little impact on sensation and appearance of the donor great toe. Meanwhile, there is no adverse effect on walking, running and jumping with the donor foot.

ABSTRACT

OBJECTIVE To analyze the current status, hotspots and development trends of research on the treatment of non-alcoholic fatty liver disease (NAFLD), providing reference for subsequent research. METHODS Searching the Web of Science database, the literature related to the treatment of NAFLD from the establishment of the database to December 31, 2022 were collected. CiteSpace 6.1.R6 was used to construct a visual atlas, perform collaborative network analysis on authors, countries and institutions, and conduct keyword co-occurrence, clustering and emergence analysis to explore its research status and hotspots. RESULTS A total of 3 882 articles were included, and the number of publications had been increasing year by year. The top three countries in terms of publication volume were China, the United States, and Japan. The author with the highest volume of publications was Sanyal from the United States (37 articles), while the institution with the highest volume of publications was the University of California, San Diego (75 articles). A closely connected research team abroad mainly conducted large-scale randomized controlled trials (RCT) to evaluate the effectiveness and safety of various interventions, including medication and lifestyle, in treating NAFLD. However, domestic researches mainly focused on basic researches about the treatment of NAFLD with effective medicinal ingredients, and were characterized by traditional Chinese medicine. There were relatively few high-quality large-scale RCT studies related to it. Keyword analysis showed that researches in various countries mainly focused on regulating liver oxidative stress and inflammation, improving the overall balance of glucose and lipid metabolism. Except for hypoglycemic drugs, drugs that act on various comprehensive metabolic homeostasis targets in the body had entered clinical research, and had enormous therapeutic potential. CONCLUSIONS The research on NAFLD treatment continues to grow in popularity and tends to research targets and drugs for regulating systemic metabolic homeostasis. As the main force of research, China should strengthen communication with the international community, grasp the trends and directions of basic research, attach importance to clinical research, and continuously tap the therapeutic potential of traditional Chinese medicine.

ABSTRACT

Background: ZNF460 is a member of the zinc finger protein family transcription factors, and is involved in the regulation of a variety of cellular functions. It has been demonstrated to be closely related to digestive system cancers. Aims: To analyze the expression level of ZNF460 in hepatocellular carcinoma (HCC), and explore the effect and potential mechanisms of ZNF460 on tumor cell proliferation. Methods: Immunohistochemical staining was used to determine the expression level of ZNF460 in tissue microarray of 76 HCC and paired adjacent tissues, and the correlation between ZNF460 expression and TNM staging was analyzed. The effect of ZNF460 on HCC cell proliferation was evaluated by CCK‑ 8 and colony formation assays. Genes positively related to ZNF460 expression in HCC were screened through LinkedOmics database, and the KEGG and GSEA pathway enrichment analyses were performed; the possible downstream molecules of ZNF460 were explored and verified by overexpression or knockdown of ZNF460 in HCC cells combined with luciferase reporter assay and other experiments. Results: ZNF460 was highly expressed in HCC and was positively correlated with TNM staging. ZNF460 promoted the proliferation of HCC cells, and the underlying mechanism might be associated with the transcriptional activation of ZDHHC7, the coding gene of palmitoyl transferase DHHC7 and the subsequent STAT3 palmitoylation and phosphorylation. Conclusions: ZNF460 is highly expressed in HCC and promotes cell proliferation and tumor progression via STAT3 activation. It might be a promising molecular marker and therapeutic target for HCC.

ABSTRACT

Myocardial dysfunction is the most serious complication of sepsis. Sepsis-induced myocardial dysfunction (SMD) is often associated with gastrointestinal dysfunction, but its pathophysiological significance remains unclear. The present study found that patients with SMD had higher plasma gastrin concentrations than those without SMD. In mice, knockdown of the gastrin receptor, cholecystokinin B receptor (Cckbr), aggravated lipopolysaccharide (LPS)-induced cardiac dysfunction and increased inflammation in the heart, whereas the intravenous administration of gastrin ameliorated SMD and cardiac injury. Macrophage infiltration plays a significant role in SMD because depletion of macrophages by the intravenous injection of clodronate liposomes, 48 h prior to LPS administration, alleviated LPS-induced cardiac injury in Cckbr-deficient mice. The intravenous injection of bone marrow macrophages (BMMs) overexpressing Cckbr reduced LPS-induced myocardial dysfunction. Furthermore, gastrin treatment inhibited toll-like receptor 4 (TLR4) expression through the peroxisome proliferator-activated receptor α (PPAR-α) signaling pathway in BMMs. Thus, our findings provide insights into the mechanism of the protective role of gastrin/CCKBR in SMD, which could be used to develop new treatment modalities for SMD.

ABSTRACT

OBJECTIVE To study the regulator y mech anism of glaucocalyxin A (GLA) on autophagy and apoptosis of HCCLM3 hepatocellular carcinoma cells. METHODS HCCLM3 cells were taken ,and control group ,GLA 2.5 μg/mL group,GLA 5 μg/mL group and GLA 10 μg/mL group were mainly set according to different experimental purposes. In control group, only complete medium was added ;in each administration group ,complete medium containing the corresponding final concentration of GLA was added. Cell cycle distribution and apoptosis were detected by flow cytometry ;mitochondrial morphology and autophagy were observed by transmission electron microscope (only control group ,GLA 5 μg/mL group);JC-1 staining and fluorescence inverted microscope were used to observe and detect the mitochondrial membrane potential of the cells ;Western blot assay was used to detect the protein expression of Bcl- 2, Bax, Beclin1 and cleaved caspase- 3 proteins in the cells ; the co-immunoprecipitation method was used to detect the binding and dissociation of Bcl- 2 and Beclin 1(only GLA 5 μg/mL group, GLA 10 μg/mL group). RESULTS Compared with control group ,GLA 5 μg/mL and GLA 10 μg/mL could induce a significant arrest of the cell cycle in the G 2-M phase for HCCLM 3,a significant decrease in mitochondrial membrane potential ,an increase in apoptosis as well as significant promotion of the protein expression of Bax ,cleaved caspase- 3 and Beclin 1,and significant inhibition of the protein expression of Bcl- 2(P<0.01). GLA 5 μg/mL also significantly changed mitochondrial morphology and increased autophagosomes. The results of co-immunoprecipitation showed that compared with GLA 5 μg/mL,GLA 10 μg/mL could enhance the binding of Bcl- 2 and Beclin 1. CONCLUSIONS GLA can regulate the autophagy and apoptosis of HCCLM 3 cells by Bcl-2/Beclin1 target. The effect is closely related to the dose of GLA.

ABSTRACT

The administration of nanoparticles (NPs) first faces the challenges of evading renal filtration and clearance of reticuloendothelial system (RES). After that, NPs infiltrate through the expanded endothelial space and penetrated the dense stroma of tumor microenvironment to tumor cells. As long as possible to prolong the time of NPs remaining in tumor tissue, NPs release active agent and induce pharmacological action. This review provides a comprehensive summary of the physical and chemical properties of NPs and the influence of various biological factors in tumor microenvironment, and discusses how to improve the final efficacy through adjusting the characteristics and structure of NPs. Perspectives and future directions are also provided.

ABSTRACT

Objective:High-flow nasal cannula (HFNC) is a new method to treat adult respiratory distress. This study aims to explore the early predictors of the inefficiency of HFNC in patients with respiratory distress. Methods:A total of 162 patients with respiratory distress were treated with HFNC oxygen therapy in the Department of Respiratory and Critical Care Medicine, Emergency and Critical Care Medicine. The age, sex, weight, history, diagnosis, vital signs, blood oxygen saturation/inhaled oxygen concentration (SpO 2/FiO 2, SF) ratio, and modified respiratory distress score (mRDAI) , duration of HFNC oxygen therapy, replacement of advanced oxygen therapy support time, adverse reactions and other data were extracted from the medical electronic medical records. Results:A total of 154 valid samples were included, with a median age of 55 (inter-quartile range 47-72), including 59 patients (38.3%) with acute bronchiolitis, 64 patients (41.6%) with bacterial pneumonia, 31 patients (20.1%) with atypical or viral pneumonia; 129 patients (83.8%) with effective HFNC oxygen therapy and 25 patients (16.2%) with ineffective HFNC oxygen therapy. SF ratio in ineffective HFNC oxygen therapy patients at admission was lower, pH value was lower, partial pressure of carbon dioxide was higher ( P values were 0.008, 0.012, 0.001). RR, mRDAI score and SF ratio in the first hour of effective HFNC oxygen therapy patients improved significantly ( t values were 1.732, 9.783, 37.591, P<0.05 or 0.01). SF ratio in the first hour of HFNC was lower than 195 (area under curve 0.842, 95% CI 0.743-0.942, P<0.01). It was the critical value for the ineffectiveness of oxygen therapy. Conclusions:When patients with respiratory distress are supported by HFNC oxygen therapy, the lower initial fingertip oxygen saturation, higher blood gas arterial partial pressure of carbon dioxide and lower SF ratio are the early predictors of failure of HFNC oxygen therapy.

ABSTRACT

Objective:To investigate the change of peptidylarginine deiminase 4 (PAD4) expression in the neutrophils of peripheral blood of patients with anti-neutrophil cytoplasmic myeloperoxidase antibody-associated vasculitis (MPO-AAV), and to analyze the relationship between the change and disease activity.Methods:Thirty-nine untreated patients with active MPO-AAV (patient group) and thirty-nine healthy volunteers (control group) were enrolled into this study. The PAD4 expressed on neutrophils was detected by flow cytometry (FCM). The serum neutrophil extracellular traps (NETs), fragments from the activated complement C5 (C5a) and anti-neutrophil cytoplasmic myeloperoxidase antibody (MPO-ANCA) were measured by enzyme-linked immuno sorbent assay (ELISA). Their disease activity was evaluated by Birmingham vasculitis activity score (BVAS). All the detected results were compared between the 2 groups by t test, Spearman correlation analysis and multivariate linear regression analysis were employed to analyze the relationship between BVAS and the Lab test results in patient group. Results:The proportion of neutrophil expressing PAD4, the mean fluorescence intensity (MIF) of PAD4, the levels of NETs and C5a in patient group were significantly higher than those in the control group [(71±11)% vs (26±6)%, t=22.456, P<0.01; (33±4) vs (14±4), t=18.668, P<0.01; (0.62±0.22) vs (0.26±0.15), t=8.466, P<0.01 and (4.6±1.0) vs (2.9±1.0), t=7.697, P<0.01, respectively]. In patient group, BVAS was positively associated with the proportion of PAD4 + neutrophil, MFI of PAD4, the serum level of NETs, C5a and MPO-ANCA ( r=0.843, P<0.01; r=0.821, P<0.01; r=0.411 1, P<0.01; r=0.613, P<0.01 and P=0.790, P<0.01, respectively), however, the results of multiple linear regression analysis showed only the percentage of PAD4 + neutrophils and the level of MPO-ANCA were independent influencing factors on BVAS ( β=0.324 6, P<0.01 and β=0.796, P<0.01, respectively). Conclusion:The percentage of neutrophils expressed PAD4 in the peripheral blood of patient with active MPO-AAV is significantly increased, and it is an independent factor affecting the disease activity. Intervention on this expression might be a potential new pathway for MPO-AAV treatment.

ABSTRACT

The drug cues of multi-sensory have higher reliability and ecological validity, and play a key role in the substance and behavioral addiction. However, the comprehensive discussion on the relation-ship of addiction and the disorder of emotional audiovisual integration was very limited so far. Reviewing re-cent studies on multi-sensory integration of substance and behavioral addictions,it had been found that both two addiction groups had disorders of audiovisual integration, especially the negative emotion. Neuroimaging studies found that the abnormal activation on prefrontal cortical during emotional audiovisual integration com-monly tended to live in substance and behavioral addictions, suggesting that the disorder of emotional audiovi-sual integration for addictions might be related to the weakening of cognitive control. Further improvements of the neural mechanism of emotional audiovisual integration disorder of addictions,as well as the more ecologi-cally predictive indicators and clinical diagnosis criteria are needed.

ABSTRACT

Objective To evaluate the effect of six-character formula breathing exercises combined with long-term home oxygen therapy in the rehabilitation of patients with chronic obstructive pulmonary disease (COPD). Methods A total of 200 COPD patients in Shanghai First People′s Hospital were divided into control group and observation group by odd and even numbers with 100 cases each. The patients in control group received routine long-term home oxygen therapy and the patients in observation group received six-character formula breathing exercises as an adjunct to long-term home oxygen therapy. The patients completed the Modified Medical Research Council (mMRC) score at 1 month, 3 months and 9 months respectively. At 6 months and 12 months, the patients completed the mMRC score, the frequency of acute exacerbation of chronic obstructive pulmonary disease (AECOPD), the number of re-admissions, the partial pressure of carbon dioxide in arterial blood (PaCO2) and St. George′s Respiratory Disease Questionnaire (SGRQ) scores. At 12 months, the patients were evaluated for the six-minute walking test. Results The duration of oxygen in observation group was (6.3 ± 1.7) h/d, was shorter than control group (7.5 ± 2.1) h/d (t =2.040, P =0.043). The frequency of AECOPD in observation group at 12 months was (3.12 ± 0.49) times/case, was lower than control group (4.95±0.65) times/case (t=22.482, P<0.01). The frequency of re-admission in observation group was (1.09 ± 0.41) times/case, was lower than control group (1.22 ± 0.39) times/case (t=2.215, P=0.028). The mMRC scores in observation group at 3 months and 9 months was (2.733±0.750), (2.749 ± 0.938), was lower than control group (3.186 ± 0.734), (3.197 ± 0.848) (t=4.317, 3.543, P<0.01). The SGRQ scores in observation group at 6 months and 12 months was (54.678 ± 9.318), (56.656 ± 7.294) , was lower than control group (61.603 ± 8.989), (63.006 ± 7.387) (t=5.349, 6.117, P<0.01). PaCO2 in observation group at 12 months was (6.422 ± 0.864) kPa, was better than control group (6.856 ± 0.904) kPa (t=3.471, P=0.01). The walking distance of 6 minutes in observation group was (265.483 ± 19.277) meters, was longer than control group (232.966 ± 18.883) meters (t = 12.050, P <0.01). Conclusions Six-character formula breathing exercises assisted with long-term home oxygen therapy can reduce the use of oxygen, reduce the frequency of AECOPD and readmission, improve the symptoms of COPD patients, and delay the pathological process of COPD.
