Introduction (Panimula)@#Ang Rehiyong Awtonomo ng Bangsamoro sa Muslim Mindanao ay nagpapaigting sa kaparatan ng mga tao tulad ng katutubo at higit sa lahat ay mga muslim. Ang muslim ay ang tumatalima sa Islam at katuruan nito sa pamamagitan ng pagtanggap o pagsasaksi ( ٱلشَّھَادَة ) sa kaisahan ( توحید ) ng Allah ( سُبْحَانَھُوَتَعَالَى ) at bunga nito ang isang muslim ay nagiging bahagi ng relihiyon ( دین ) ng kapayapaan sapagkat ang kapayapaan sa arabe ( سلام ) ay nag mula sa kaparehas na salitang ugat ng Islam, ang ( سِلم ). Ngunit paano bibigyan ng mukha ang kapayapaan sa mga baklang sasailalim sa rehiyong ito sa kulturang hindi hinihikayat (Haram) ang pagpapahayag (gender expression) ng pagiging bakla? @*Objective (Layunin)@#Ang pananaw sa bakla ay hindi basta salita bagkus ay isang buong pananaw (holistikong pagtingin sa tao) o pilosopiyang nakapaloob sa kulturang Pilipino na maaring magamit upang maging tulay sa pilipinong kultura ng mga muslim na huhubog sa ating sariling karanasan ng totoong kapayapaan ang malayang kapayapaan @*Methodology (Pamamaraan)@#Ang papel na ito ay magtatangka na pagtalabin ang konsepto ng bakla sa konsepto ng Pilipinong muslim gamit ang pamamaraan at dulog ng paglalahad ng kasaysayan na may saysay (dulog ng Qasas قَصَص ) na may pagtindig sa tradisyon ni Ibn Khaldun ang pagsasaysay ng makatotohanang pagsasaysay na sa yaong dulog ay maaring magdulot ng tatlong bunga. @*Result (Bunga)@#(1) una, ito ay maaring magpalalim sa pag unawa (Verstehen) sa bawat isa at maging daan tungo sa karanasan ng malayang kapayapaan dahil ang paggamit ng kultura sa talakayan ay nakaugat at mas malapit sa sariling kultura't pananampalatayang Pilipino (2) ang paggamit ng katutubong kultura ay maaring maging mas mabisa sa pagpapalaganap ng kapayapaan dahil malapit ito sa kultura ng taong tatanggap (3) ang katutubong kultura ay malilinang dahil kasabay ng pananampalatayang Pilipino ay magagamit ito sa pagtindig ng kapayapaan na naaayon sa konteksto at pananaw ng mga Pilipino @*Conclusion (Tugatog)@#Sa pamamagitang ng paglalahad ng saysay sa kasaysayan makikita natin ang pagkakapareha ng katangiang bakla bilang banayad at mabuti na tumutugon sa katangian ng muslim bilang mabuti na may katangiang لِنْتَ at بِٱلْقِسْطِ na maaring magamit bilang tulay ng dalawan tradisyon.

A 72 years old lady presented with a gradually increasing abdominal mass for the last 25 years, with more rapid increase in the last 05 years. Examination revealed a large mass spanning the whole abdomen. CAT scan revealed a huge retroperitoneal tumour, with tortuous ureters passing through the tumour and three tiny nodules in the right lung field. A subtotal piecemeal excision of the tumour was done after stenting the ureters with single J stents. The excised tumour mass weighed 46 Kg. The patient had a prolonged ileus that recovered in 5 days. Histopathology revealed the mass to be dedifferentiated liposarcoma. The patient and the family were satisfied with the short-term outcome of the surgery. She was ambulating with little support and was able to manage her daily affairs
ABSTRACT
To analyze the outcome of open mini-cholecystectomy in terms of operative success and complications. Retrospective review of record. Shifa International Hospital, Islamabad, from January 2002 to September 2005. all the patients who underwent mini-open cholecystectomy for cholelithiasis during the study period were reviewed. The data was collected on a computerized proforma. The patients undergoing classical open cholecystectomy were excluded. Statistical analysis was performed using SPSS for Windows version 11. A total of 200 cases were operated [180 females, 20 males; M:F ratio 1:9]. The mean age was 43 years [range 21-72 years]. The average operating time was 45 minutes. Complications included biliary leak in one patient [0.5%], bleeding 1 [0.5%], wound infection 4 [2%] and death in one case [0.5%]. The overall rate of complications was 3.5%. There were 5 patients who required extension of the incision for reasons like carcinoma gallbladder [1], control of bleeding [1], bad adhesions [1], difficult anatomy [1] and biliary leak from the liver bed [1]. The mean hospital stay was 1.5 days [range 1-3 days]. Mini-open cholecystectomy appears to be a safe and has its application where laparoscopic facilities are not available

Early appendectomy is traditionally considered the treatment of choice for acute appendicitis. Recently many reports have shown that postponing surgery overnight for such cases till the following morning is safe without increase in the rate of complications. In our hospital, too, it has been our practice for those patients admitted with non-complicated acute appendicitis who come in the evening or night to delay their surgery till the following day. A retrospective study was undertaken to see the impact of this policy on s the rate of complicated cases, postoperative complications and hospital stay. It included 270 patients under going surgery for acute appendicitis from January, 1995 through December 1999. The patients were grouped into two categories; Group A included [84] cases who were operated within 6 hours of admission and Group B comprised the rest [186] who were operated after 6 hours. In Group A, 85.7% cases had acute uncomplicated appendicitis, 9.5% complicated [perforated and gangrenous] and 4.76% had normal appendices. In Group B, there were 83.87% cases of acute appendicitis, 8.6% complicated and 6.45% normal appendices. In Group A 13.1% cases had postoperative complications as against 5.09% in Group B. The mean hospital stay, too, was comparable in both the groups [4.16 days in Group A and 4.8 days in Group B]. The data suggests that elective overnight delay in appendectomy is safe without any increase in the rate of complications, postoperative morbidity and hospital stay

The treatment of pilonidal sinus is still not definitive, however primary suturing after excision is becoming the preferred method of treatment than open excision. In our setup, such cases are managed by open as well as closed excisions. A study was carried out to evaluate the outcome of both procedures. Ninety-seven patients were available for analysis during the period from January 1993 through December 1998. Sixty-three patients were operated by open [Group-A] and 34 by excision and simple closure [Group-B]. The patients in group-B had shorter hospital stay [5.5 vs 8 days, p< 0.05], faster recovery [5.3 vs 8.6 weeks, p < 0.01] and earlier return to work [4.4 vs. 5.5 weeks, p < 0.05] despite problems of patient's compliance in the early post-operative period. Closed excision was found to be better than open excision in chronic pilonidal sinus and results may be further improved by better post-operative care and regular follow-up

Solitary Caecal Diverticulitis is a relatively uncommon condition presenting with clinical features similar to acute appendicitis. It is usually diagnosed intraoperatively with an otherwise normal appendix. We report an even rarer case of acute solitary caecal diverticulitis associated with acute appendicitis. An evaluation of techniques for preoperative diagnosis and current status of management plans for this condition is presented

A prospective study of 210 cases undergoing early appendicectomies for suspected acute appendicitis is presented. The diagnosis was based on detailed history and clinical examination with limited investigations. There was a 7.2 percent incidence of negative appendicectomies with a 13.3 percent rate of complications. The overall morbidity was 9.5 percent. The incidence of post-operative complications was 37.2 percent in the complicated group as opposed to 8.9 percent in the un-complicated appendicitis and zero percent in negative explorations. An aggressive and prompt approach is recommended to lower the incidence of perforated appendicitis and hence postoperative morbidity
