Centro especializado ambulatorio de Cienfuegos: una mirada desde la gestión hospitalaria en su primera década / Specialized ambulatory center of Cienfuegos province: a view to hospital management in its first decade

La celebración en el 2019 de una década de trabajo ininterrumpido del primer hospital ligero diseñado en Cuba fue motivo para la realización de este trabajo, que tuvo como objetivo describir los principales resultados en los aspectos de meso y microgestión hospitalaria del Centro Especializado Ambulatorio Héroes de Playa Girón de Cienfuegos, desde el 2009 hasta el 2019. Múltiples investigaciones mostraron la utilidad de la gestión por procesos para garantizar la calidad de la atención médica y la integración de los programas de seguridad institucional y del paciente en las funciones gerenciales y asistenciales. La primera gestión hospitalaria del Centro Especializado Ambulatorio, sobre la base de las mejores experiencias nacionales e internacionales, cumplió con los objetivos del diseño para el cual fue creado al integrar calidad, racionalidad y eficiencia. Esta experiencia puede ser ejemplo para otras instituciones que precisen de un centro de atención ambulatoria y muy corta estadía en la concepción de un hospital universitario(AU)

The celebration in 2019 of a decade of continuous work of the first ambulatory hospital designed in Cuba motivated the production of this work, that had as an objective to describe the main results in the aspects of hospital meso and micro-management of Héroes de Playa Girón Specialized Ambulatory Center of Cienfuegos province, from 2009 to 2019. Several research works showed the usefulness of management by processes to guarantee the quality of medical care and the integration of institutional and patients safety programs in management and care functions. The first hospital management of the Specialized Ambulatory Center, based on the best national and international experiences, accomplished the objectives for what it was design to by integrating quality, rationality and efficiency. This experience would be an example for other institutions that need an ambulatory care center and a short time in the creation of a university hospital(AU)

Historia de la Anestesiología y Reanimación en Cienfuegos / History of Anestesiology ad Resuscitation in Cienfuegos

RESUMEN A partir de la introducción de la anestesia en la isla, el 11 de marzo de 1847 se diò un progresivo avance en el desarrollo de la cirugía. En Cienfuegos sería un hito cuando el cirujano Alfredo Méndez la introdujo entre 1901 y 1902; y su uso se ha incrementado en la actualidad. Se describieron las principales acciones que se han desarrollado por la especialidad de Anestesiología y Reanimación en sus hospitales generales en Cienfuegos. Se revisó el libro de la cátedra Anestesiología Clínica, en su primera y segunda ediciones. Se realizaron entrevistas a protagonistas que formaron parte del desarrollo de la especialidad en Cienfuegos. Se encontraron los principales hechos que fueron desarrollando la especialidad de Anestesiología y Reanimación en la provincia desde principios del siglo xx con la aplicación de cloroformo y éter por parte del cirujano Alfredo Méndez y cómo continúa su desarrollo con médicos dedicados a la anestesia en el comienzo de la segunda mitad de la década del 50; el desarrollo en el Hospital civil y las clínicas privadas, hasta llegar al triunfo de la Revolución y hasta nuestros días en pleno siglo xxi. La anestesia en Cienfuegos se introdujo a comienzos del siglo xx, teniendo un quehacer constante en el desarrollo de la esta disciplina, así como la integración de la especialidad a la Sociedad Cubana de Anestesiología, Reanimación y Dolor, constituyendo su grupo provincial.

ABSTRACT As of the introduction of anesthesia on the Island, on March 11, 1847 there was a progressive advance in the development of surgery. In Cienfuegos it would be a milestone when the surgeon Alfredo Méndez introduced it between 1901 and 1902; and its use has increased at present. The main actions that have been developed by the Anesthesiology and Resuscitation specialty in the general hospitals in Cienfuegos were described. The book of the Clinical Anesthesiology, in its first and second editions, was revised. Interviews were conducted with protagonists who were part of the development of the specialty in Cienfuegos. The main facts that were developing the specialty of Anesthesiology and Resuscitation in the province were found since the beginning of the 20th century with the application of chloroform and ether by Alfredo Méndez surgeon and how it continues its development with doctors dedicated to anesthesia in the beginning of the second half of the 1950s; the development in the Civil Hospital and private clinics, until the triumph of the Revolution and until nowadays in the XXI century. Anesthesia in Cienfuegos was introduced at the beginning of the 20th century, having a constant task in the development of this discipline, as well as the integration of the specialty to the Anesthesiology Resuscitation and Pain Cuban Society, being its provincial group and chapter.

Primer Centro Especializado Ambulatorio en Cuba y sus resultados en tres años de trabajo / First Specialized Ambulatory Center in Cuba and its results in three years

Este trabajo tiene el propósito de exponer los resultados del trabajo del Centro Especializado Ambulatorio Héroes de Playa Girón adjunto al Hospital General Universitario Dr. Gustavo Aldereguía Lima tras tres años de su puesta en marcha. Estudio descriptivo para el cual se tuvieron en cuenta los principales indicadores del Hospital General desde el 2007 hasta el 2012. A partir de septiembre de 2009, apertura del Centro Especializado Ambulatorio, se incluyeron sus indicadores conjuntamente con los anteriores. Los datos se obtuvieron de los informes estadísticos anuales de ambos recintos hospitalarios. La apertura del Centro Especializado permitió un incremento en la cirugía mayor ambulatoria en el Hospital General; en el Centro Especializado, en el 2010, 2011 y 2012, se realizaron el 36,2; 43,9 y el 41,1 por ciento respectivamente del total de la cirugía mayor electiva ambulatoria realizada en el Hospital General. Las intervenciones realizadas con mayor frecuencia fueron: herniorrafías, hemorroidectomía, colecistectomía por cirugía mínimamente invasiva, extirpación de tumores de partes blandas, intervenciones ginecológicas del suelo pélvico, cirugías ortopédicas y entre otras. El Centro Especializado asumió el 20 por ciento del total de pacientes vistos en la consulta del Hospital General. En los tres años de trabajo se hospitalizaron 3 119 pacientes, la mayoría en el servicio de rehabilitación y la enfermedad más frecuente para rehabilitar fue la cerebrovascular. El Centro Especializado Ambulatorio, desde su apertura, contribuye a disminuir la carga asistencial del Hospital General, ofrece un servicio de alta calidad y juega un importante papel en el desarrollo de la cirugía ambulatoria en Cienfuegos(AU)

This paper was aimed at putting forward the work results of Heroes de Playa Giron specialized ambulatory center subordinated to Dr Gustavo Aldereguia Lima general university hospital after three years of operation. This descriptive study took into account the main indicators of the general hospital from 2007 through 2012. As from September 2009, the date of opening of the specialized ambulatory center, these indicators were jointly included with the above-mentioned. Data were collected from the yearly statistical reports of both hospital facilities. The opening of the Center allowed increasing the number of ambulatory major surgeries in the general hospital, since 36.2 percent, 43.9 percent and 41.1 percent of the total number of ambulatory elective major surgeries recorded in the hospital were performed in 2010, 2011 and 2012, respectively, in this specialized center. The most frequent were herniorraphies, hemorrhoidectomy, colecistectomy through minimally invasive technique, excision of soft tissue tumors, gynecological treatments of the pelvic floor, orthopedic surgeries and others. The Specialized Center gave service to 20 percent of the total number of patients seen at the outpatient service in the general hospital. In three years, 3 119 patients were admitted to the hospital, most of them in the rehabilitation service and the most treated disease was the cerebrovascular one. Since its opening, the Specialized Ambulatory Center has contributed to reduce the medical assistance burden of the general hospital, provides high quality service and plays a key role in the development of ambulatory surgery in Cienfuegos province(AU)

Puesta en marcha del primer Centro Especializado Ambulatorio en Cuba / First specialized outpatient service center in Cuba

Introducción Poner en marcha una institución de salud es una tarea compleja que demanda una buena planificación. Objetivos Identificar los principales problemas para la puesta en marcha del primer Centro Especializado Ambulatorio en Cuba y determinar su orden de prioridad. Métodos Para la identificación de los problemas, se realizó una lluvia de ideas en la que intervinieron miembros del consejo de dirección y jefes de servicios seleccionados. Se aplicaron métodos de ponderación para reducir la lista de problemas y seleccionar un número determinado de ellos. Para la priorización de estos problemas se utilizó el método de ranqueo y para el diagnóstico causal se usó la técnica de Ishikawa o espina de pescado, que permitió definir cuáles de las causas son de carácter interno y cuáles de carácter externo. Resultados Se identificaron cuatro problemas fundamentales con el siguiente orden de prioridad: necesidad de la puesta en marcha de los servicios ingenieros, calculo de insumos médicos y no médicos, incorporación de los recursos humanos y departamentos que trabajarían en el Centro y por último, incorporación de los servicios médicos según objeto social previsto. Conclusiones La planificación realizada y los métodos utilizados garantizaron el cumplimiento de los objetivos propuestos y la puesta en marcha del Centro Especializado Ambulatorio Héroes de Playa Girón anexo al Hospital General Universitario Dr Gustavo Aldereguía Lima, en Cienfuegos

Introduction Putting into operation a health institution is a complex task requiring good planning. Objectives To identify the main problems for the putting into operation the first specialized outpatient service center in Cuba and to determine their order of priority. Methods For the problem identification, a brainstorming activity was performed in which members of the managing board and selected heads of services were involved. Several weighing methods to reduce the listing of problems and to select a certain number of them were used. The ranking method served to prioritize problems followed by the causal diagnosis, based on the Ishikawa or fishbone technique, which allowed defining both the internal and the external causes. Results Four fundamental problems were detected following the order of priority: need for putting engineering services into operation, calculation of the required medical and non-medical inputs, incorporation of the human resources and departments, and finally, the incorporation of the various medical services in line with the agreed social object of the institution. Conclusions The planning and the used methods assured the fulfilment of the objectives and the putting into operation of Heroes de Playa Girón specialized outpatient service center, attached to Dr Gustavo Aldereguía Lima general university hospital in Cienfuegos
