ABSTRACT

Introduction: Total hip arthroplasty (THA) or total hipreplacement is the most effective, economical surgicalprocedure for femoral neck fractures or hip joint diseasesamong elderly patients. The main purpose is to reduce painas well as reduce joint function. Hence, the aim of the presentstudy was to assess the functional outcome between cementedand uncemented total hip arthroplasty.Material and Methods: The present study was anobservational study which was hospital‑based conductedamong 140 cases divided into two groups with 70 cases ineach group. All the patients of 50–80 years in which THR wasindicated were taken in this study. Patients with neurovasculardeficit and active infection were excluded from this study.In Group 1, cemented THR was done while in Group 2uncemented THR was done. Pain was evaluated using Harriship score.Results: In cemented group, majority of the cases wereavascular necrosis 24(34%) followed by fracture neck offemur 10 (14%), fracture-dislocation of hip 6 (8%) and inuncemented group, maximum cases were avascular necrosis36 (51%) followed by fracture neck of femur 21 (30%) andfracture-dislocation of hip 14 (20%). mean difference amongcemented group was 82.41±7.2 at 6 weeks followed by81.31±7.5 in uncemented group and this difference was foundto be statistically significant at p value 0.001.Conclusion: Cemented implants showed better functionaloutcomes than uncemented in total hip arthroplasty at 6weeks, 3 months and at 6 months.

ABSTRACT

Introduction: Pain in the hip joint is one of the importantcauses that disable locomotion. Pain in the hip joint may bedue to various causes ranging from fracture neck of femurin the elderly to rheumatoid arthritis in the younger age.There are many ways and methods by which this cripplingpain in the hip can be treated. This includes analgesics, usinga walking stick or bilateral axillary crutches, arthrodesis,excision arthroplasty and hip arthroplasty. Hence, the aimof the present study was to assess the short term clinical andradiological outcome of full hydroxyapatite coated stems intotal hip and bipolar arthroplasty.Material and Methods: The present study was a retrospectivestudy which was done among 44 hip arthroplasty bothbipolar and total hip replacement with fully hydroxyapatitecoated stem in 32 patients. Preoperatively, the patients wereevaluated using the Harris hip score, except in acute neck offemur fracture patients. This score takes into account pain,function, absence of deformity and range of motions. All 32hips were operated through posterior approach. In that 18 Leftside and 14 right side.Results: In the present study, 32 hips were evaluated bothclinically and radiologically clinical evaluation was done withthe help of Harris hip score and it was found that excellentscore was achieved among 84.3%, good among 12.5%, fairamong 0% and poor among 3.1% of the patients. Accordingto radiological examination, based on the position of stem,center was achieved among 68.75% patients followed byvarus among 18.75% and valgus among 12.5% of the patients.Conclusion: About 84% of patients had radiologically silenthip with excellent clinical score. With HA coated stem, thereis no clinically significant anterior thigh pain due good stemstability
