ABSTRACT
Objective To investigate a convenient and quantitative solution to activation levels and functional characterization of CAR-T cells by inserting T cell activity-responsive promoter (TARP) nanoluciferase reporter gene system into a lentiviral plasmid containing the gene encoding the chimeric antigen receptor (CAR). Methods The recombinant plasmid was constructed by using whole gene synthesis and molecular cloning techniques. The lentivirus was packaged and was infected with human primary T lymphocytes. Flow cytometry was used to detected the positive rate of lentivirus-infected T cells. The functional characterization of CAR-T cells was identified by luciferase reporter gene system, Western blot, flow cytometry, and small animal live imaging techniques. Results The results of enzyme digestion identification and the plasmid sequencing showed that the recombinant plasmids were constructed, and flow cytometry displayed the normal preparation of CAR-T cells. This system could dynamically respond to the activation of CAR-T cells by luciferase reporter gene system. The functional assay in vitro confirmed that the system could reflect the exhaustion of CAR-T cells, and the small animal live imaging results demonstrated that the system can be used as a tracer of CAR-T cells in mice. Conclusion TARP nanoluciferase reporter gene system provides a more convenient, sensitive and quantitative method for evaluating CAR-T cells activation level, exhaustion phenotype and tracing.

Objective To investigate the relationship between uncertainty in illness and recovery quality. Methods A cross sectional study was conducted from June to December 2015. The Chinese version Mishel Uncertainty in Illness Scale was used to assess uncertainty in illness of 168 patients with unconfirmed breast tumor undergoing general anesthesia. The following datarecovery time, time stay in postanesthesiacar unit, modified Observer′s Assessment of Alertness/Sedation Scale score etc were recorded. Results The breast tumor patients had a medium degree of illness uncertainty (81.11 ± 12.57) points. Educational background, marital status, family medical history, the severity of disease affect scores of illness uncertainty (P<0.05 or 0.01). Uncertainty in illnesswas negative correlated with recovery quality (P<0.05 or 0.01). Conclusions Uncertainty in illness of patients isrelated to recovery quality. Nursesshould instruct breast tumor patients to manage symptoms to decrease uncertainty in illness.

Objective To evaluate the effect of dezocine on the c-fos expression in neurons in the midbrain periaqueductal gray in a rat model of incisional pain.Methods Thirty-six pathogen-free healthy adult male Wistar rats,weighing 250-300 g,were divided into 3 groups (n =12 each) using a random number table:control group (group C),incisional pain group (group I) and dezocine group (group D).A 1 cm longitudinal incision was made through skin,fascia and muscle of the plantar aspect of the right hind paw in sevoflurane-anesthetized rats.In group C,the rats were only anesthetized and underwent no operation.In group I,0.9% sodium chloride solution 2 ml was injected via the caudal vein at 15 min before the model was established.In group D,dezocine 1 mg/kg (diluted to 2 ml in 0.9% sodium chloride solution) was injected via the caudal vein at 15 min before the model was established.At 24 h before operation (T0) and 2,6 and 24 h after operation (T1-3),the mechanical paw withdrawal threshold (MWT) and cumulative pain score were measured.After measurement of the pain threshold at T3,the whole brain was removed for determination of the c-fos expression in neurons in the midbrain periaqueductal gray by immunohistochemistry.Results Compared with group C,the MWT was significantly decreased,cumulative pain scores were increased,and the expression of c-fos in neurons in the midbrain periaqueductal gray was upregulated at T1-3 in I and D groups (P<0.05).Compared with group I,the MWT was significantly increased,the cumulative pain score was decreased,and the expression of c-fos protein in neurons in the midbrain periaqueductal gray was down-regulated at T1.3 in group D (P<0.05).Conclusion Dezocine mitigates incisional pain through inhibiting the expression of c-fos in neurons in the midbrain periaqueductal gray of rats.
ABSTRACT
Immobilization of enzymes is important and widely applied in biocatalysis. Streptomyces platensis gene gox, encoding an extracellular L-glutamate oxidase (Gox), was fused to cellulose binding domain (CBDcex) from Cellulomonas fimi and the recombinant protein Gox-CBD was expressed in Escherichia coli. The fusion protein (Gox-CBD) was immobilized onto microcrystalline cellulose. The preparation conditions, binding capacity, properties and stability of the immobilized enzyme were studied. Under the condition of 4 ℃, for 1 hour, the fusion protein Gox-CBD was able to bind microcrystalline cellulose at a ratio of 9.0 mg of protein per gram of microcrystalline cellulose. Enzymatic properties of free and immobilized L-glutamic oxidase (Gox-CBD) were compared. The specific activity of the immobilized enzyme decreased, but its thermal stability increased a lot compared with that of the free Gox-CBD. After incubation at 60 ℃ for 30 min, 70% of the total activity remained whereas the free recombinant Gox completely lost its activity. The immobilized protein was tightly bound to microcrystalline cellulose at pH below 10 or more than 5 mmol/L NaCl. The fusion protein of Gox-CBD can be specifically immobilized on the microcrystalline cellulose on a single step. Therefore, our findings can provide a novel strategy for protein purification and enzyme immobilization.

Objective To investigate the therapeutic effects of titanium artificial auditory ossicles in closed type mastoid radical tympanoplasty ,in comparison with ceramic artificial auditory ossicles .Methods A retrospec‐tive analysis was done on of 150 patients (155 ears) with the closed type of mastoid radical tympanoplasty using arti‐ficial auditory ossicles for hearing reconstruction (from January 2008 to December 2012) ,cund were ,and followed up for 12 months .Based on the two kinds of artificial auditory ossicle materials ,the group was divided into two :122 cases (127 ears) for the titanium auditory ossicle group and 28 cases (28 ears) for the ceramic auditory ossicle group .We compared the average air conduction hearing threshold and air bone gaps (ABG) at 0 .5 ,1 .0 ,2 .0 ,and 4 .0 kHz ,at preoperative 6 months and postoperative 6 ,12 months .Results For the Titanium artificial auditory os‐sicle group:117 ears (92 .13% ) had hearing threshold improvement >15 dB HL ,106 ears of the ABG change ≤20 dB ,which were statistically significant .The total success rate of hearing reconstruction was 83 .46% (106/127) . For the ceramic auditory ossicle group:5 ears (17 .86% ) had hearing threshold improvement >15 dB HL ,3 ears (10 .71% ) of the ABG change ≤20 dB ,which was no statistical significance between the two groups .Conclusion The hearing reconstruction effects of titanium artificial auditory ossicle in closed type mastoid radical tympanoplasty is excellent .It is suitable for application in closed-mastoidectomy tympanoplasty ideal artificial auditory ossicle .

OBJECTIVE@#To investigate the leukotriene D4 synthase gene A (LTD4S A)-444 C polymorphism in persistent allergic rhinitis (AR) of Chinese Han nationality and to evaluate its relevance to clinical responsiveness of leukotriene receptor antagonist.@*METHOD@#There were 150 patients [87 males, 63 females, average age (38 +/- 14)] diagnosed with persistent AR in Allergy clinic in our hospital from March 2010 to March 2012; 146 healthy controls (78 males, 68 females, mean age (39 +/- 12)). We detected LT D4SA-444C polymorphism and allele frequencies with Polymerase Chain Reaction (PCR) and-Restriction Fragment Length polymorphism (RELP). The treatment group received monotherapy leukotriene receptor antagonist (montelukast) for 4 weeks. Urinary leukotriene D4 (LTD4) levels were detected by enzyme-linked immunosorbent assay (ELISA) before and after treatment, respectively. We evaluated anti-leukotriene treatment response according to the changement of symptoms, signs PTS and urinary LTD4. We tested correlation between LT D4S gene-444C allele frequency and the treatment response by multivariate analysis of variance.@*RESULT@#(1) LTD4S gene-444 genotype AA/CC, AC/CC frequency is 70.7% (106/150) and 29.3% (44/150), allele A, C frequencies is 67.3% (101/150) and 32.7% (49/150) in AR group, and LTD4S gene-444 genotype AA/CC, AC/CC frequency is 76.7% (112/146) and 23.3% (34/ 146), allele A, C frequencies is 74.0% (108/146) and 26.0% (38/146) in healthy control group, there is not statistically significant difference between two groups. (2) Among 150 AR patients, compared to patients with AA/CC genotype, the genotype AC/CC patients are younger [average age (35 +/- 9), and (50 +/- 18) respectively, F = 5.891, P < 0.05], with earlier age of onset [(31 +/- 4), and (46 +/- 6) respectively, F = 6.985, P < 0.05], longer course of disease [(8.7 +/- 2.1), and (3.1 +/- 2.0) respectively, F = 11.43, P < 0.05], higher symptom scores (8.2 +/- 0.2; 4.8 +/- 0.3), higher signs score (7.3 +/- 3.3; 3.4 +/- 5.1), and the difference was statistically significant. (3) After 4 weeks of montelukast treatment in AR patients, treatment response of anti-leukotriene in genotype AC/ CC patients is better than those in AA/CC genotype patients (F = 11.01, P < 0.05), the differences of treatment response between two groups were correlated with LTD4 levels in vivo, clinical symptoms and signs of patients.@*CONCLUSION@#In a Chinese Han population the LTD4SA-444B polymorphism might be one of the factors in the clinical response to leukotriene receptor antagonists in persistent AR patients.

Efforts on directed evolution of sortase A to optimize its catalytic properties have been undertaken and shown the promise. To facilitate screening of sortase A mutants with expected catalytic properties, a novel ligation efficiency monitoring system, including reporter substrates GFP-LPETG and GGGYK-Biotin, was developed. GFP-LPETG, wild type sortase A, and a recently reported high activity sortase A mutant were prepared recombinantly from Escherichia coli BL21 (DE3). Taking advantage of the newly designed reporter system, the ligation efficiency catalyzed by wild type and mutant form of sortase A could be sensitively monitored via SDS-PAGE directly. Consistent with previous report, the mutant sortase A displayed much higher catalytic activity compared to wild type enzyme, indicating the new reporter system is easily and fast handled and sensitive. The application of this reporter system into systemic screening will facilitate future directed optimization of sortase A.

OBJECTIVE@#To investigate the expression of major histocompatibility complex class I chain-related gene A (MICA) in laryngeal squamous cell carcinoma(LSCC), and its clinical significance.@*METHOD@#Immunohistochemistry and RT-PCR were used to detect the expression of the MICA in LSCC and normal tissue samples. The relationship between the expression of MICA and the clinicopathologic features features were was analyzed.@*RESULT@#Compared to the expression of MICA in normal tissues samples, the expression of MICA in LSCC tissue was significantly increased (P < 0.01). MICA expression level in carcinoma tissue was closely related to the tumor-differentiation degree and TNM staging.@*CONCLUSION@#Our study suggests that MICA may play an important role in the invasion and metastasis of LSCC, and could be a potential tumor maker for LSCC.

OBJECTIVE@#To detect the levels and effects of protective bioactive substances such as NO, SOD and GSH in patients with obstructive sleep apnea syndrome (OSAHS) combining hypertension.@*METHOD@#To collect the data from 99 snoring patients, which were divided into control group, OSAHS group, and OSAHS combined with hypertension group. The serum levels of NO), SOD and GSH in these patients were detected perioperatively, correlation analysis were carried out between these parameters.@*RESULT@#The levels of NO, SOD and GSH in control group, OSAHS group and OSAHS combined with hypertension group reduced in turn with statistically significance, and had inverse correlations with apnea hyponea index (AHDI) and lowest saturation of blood oxygen. After operation, the levels of NO), SOD and GSH were increased and the blood pressure was decreased.@*CONCLUSION@#The oxidative stress state is serious in patients with OSAHS. The oxidative stress process is more severe in patients with hypertension, which may play a role in the devolepment of hypertension in OSAHS patients.

OBJECTIVE@#To evaluate the treatment responses of persistent allergic rhinitis with and without eosinophils (EOS) in nasal discharge to inhaled glucocorticosteroid (CS), and therefore to verify whether low eosinophil level in nasal discharge can predict poor response to treatment with CS.@*METHOD@#Forty-two symptomatic allergic rhinitis patients, who had not received CS therapy in three months preceding the study, were examined before and 2 month, 4 months and 6 months after treatment with CS. At each visit, all patients underwent symptom scoring and physical sign scoring. The level of eosinophil cationic protein (ECP) in the nasal discharge supernatants was measured by radioimmunoassay. The patients were divided into 2 groups according to nasal discharge EOS percentages, an EOS group (EOS > or = 3%) and a non-EOS group (EOS 0.05), but the ECP level did improve (F = 3.78, P < 0.05). The average daily dose of CS was not different between the two groups at any visits.@*CONCLUSION@#In persistent allergic rhinitis with low EOS in nasal discharge, CS therapy for 6 months failed to improve symptoms and physical signs.

OBJECTIVE@#Investigate the effect of three-grade preventive health education and lifestyle intervention in the treatment of allergic rhinitis (AR).@*METHOD@#Two hundred and ten selected cases needed for triple prevention were randomly divided into three groups, each group included 70 cases were undertaken for a three-year randomized controlled study. Group A, treated with Budesonide nasal spray. Group B, combined Budesonide nasal spray with Hydrochloric acid left Kabbah Sting nasal spray. Group C, taking health education management and lifestyle intervention on the basis of group B's therapy. A health lecture or health problems counseling and the dissemination of health education information were undertaken, quarterly. It was mainly for health knowledge awareness, and healthy behaviors formation rate guidance. Lifestyle intervention included a balanced diet, avoiding the allergens of living environment and aerobic exercise (daily 0.5-1.0 h). The score of the signs and symptoms in each group were obtained respectively at the beginning of study, 1 year after intervention and 3 years after intervention, as well as the comparison of patient compliance of follow-up.@*RESULT@#The improved score of the signs and symptoms, endoscopy and radiological results were used to evaluate the treatment effect. There was no significant difference among the score of signs and symptoms in three groups. Comparing in group, before intervention,1 year after intervention and 3 years after intervention, the signs and symptoms of patients in three groups had improvement at different degree. The score of four symptoms (rhinobyon, rhinorrhoea, rhinocnesmus, sneezing) and signs were significant lower than before the intervention, there were a significant difference (P < 0.05). There were 8 patients in group A (11.43%), 6 patients in group 13 (8.57%) and 1 patient in group C (1.43%) lost to follow-up at 3 years after the intervention. The patient compliance of group C was significantly higher than groups A and B.@*CONCLUSION@#Triple prevention health education for AR can significantly improve the treatment compliance of AR patients for treatment, while ensuring clinical efficacy.

OBJECTIVE@#To evaluate the changes of epidermal growth factor (EGF) in serum before and after endoscopic surgery in patients with chronic sinusitis, and then to explore its clinical significance.@*METHOD@#Serum was collected before and after operation and EGF concentrations were measured by the radioimmunoassay at different times.@*RESULT@#EGF levels in patients with chronic sinusitis (1.24 +/- 0.41) microg/L were higher than that in control group (0.92 +/- 0.23) microg/L, but there were no significantly difference between them (P > 0.05). The concentrations of EGF were reduced after surgery, and the lowest were evaluated after 4 weeks. Significant difference of EGF concentrations were found between before and 4 weeks after operation (P < 0.01). The concentration of EGF were raised 6 weeks after surgery, and returned to normal after 8 weeks.@*CONCLUSION@#The changes of EGF in serum before and after endoscopic surgery may relate with the epithelization process of sinus mucosa after endoscopic surgery.

OBJECTIVE@#To investigate the changes of leukotriene D4 (LTD4) in nasal discharge and plasma of patients with persistent allergic rhinitis (AR) and the effects of antihistamine.@*METHOD@#The investigation was a prospective, randomized controlled trial. Forty AR patients (group C) were divided randomly into two subgroup. One group received oral antihistamine 10 mg everyday for one week (group CA) and another group received no loratadine tablets 10 mg everyday for one week (group CB). Fifteen age matched healthy (group D) people were enrolled as control. The level of LTD4 and interleukin-5 (IL-5) in both nasal discharge and plasma by using enzyme linked immunosorbent assay (ELISA) and enzyme immunoassay (EIA), cell counts and cell differentials in nasal discharge, were measured before and after three month. The clinical symptom and life quality scores of group C were also investigated.@*RESULT@#The concentrations of LTD4 in nasal discharge [(794 +/- 305) pg] and plasma [(5219 +/- those in group D [(347 +/- 169) pg, (2283 +/- 489) ng/L, all P 1185) ng/L] in group C were significantly higher than those in group D [(347 +/- 169) pg, (2283 +/- 489) ng/L, all P 0.05).@*CONCLUSION@#The results suggested that LTD4 was involved in airway inflammation in AR. Antihistamine was not effective enough in decreasing the levels of LTD4 in both nasal discharge and plasma of AR patients.

OBJECTIVE@#To explore the head and neck tumors underwent primary tumor resection and bilateral neck dissection in the same period in the safety, indications, and surgical difficulty.@*METHOD@#Retrospective analysis of 134 cases received primary tumor resection and bilateral neck dissection for head and neck cancer, the way of bilateral neck dissection were: one side was radical neck dissection and another was functional neck dissection (29 cases), one side was radical neck dissection and another was lateral neck dissection (34 cases), bilateral functional neck dissection (14 cases), one side was functional neck dissection and another was lateral neck dissection (48 cases), bilateral sides of lateral neck dissection (6 cases).@*RESULT@#There was no operative death in 134 cases, complications for the wound bleeding in 3 cases, chylous leakage in 4 cases, pharyngeal fistula and infection in 1 case, stress ulcer in 5 cases, 1 case died, cerebral infarction in 1 case.@*CONCLUSION@#The head and neck tumors underwent simultaneous bilateral neck dissection is safe. Appropriate cleaning method selection to reduce cervical lymph node metastasis could reduce the suffering of patients.

OBJECTIVE@#To compare the efficacy and safety of histamine H1 receptor antagonist loratadine with Leukotriene receptor antagonist Ibudilast in steroid resistant allergic rhinitis in a randomized controlled clinical trial.@*METHOD@#Thirty-five cases were treated by Ibudilast, and 34 cases by loratadine. Score system was used to compare the therapeutic effect of these two drugs on clinical symptoms and signs.@*RESULT@#Ibudilast shows a better curative effect than loratadine in the improvement of the total scores on clinical symptom and signs(P<0.05). Scores of symptoms and signs in Ibudilast group after 3, 7, 14 days decreased significantly by means of square analysis of single factor (P<0.01). No complication was observed.@*CONCLUSION@#Ibudilast can effectively alleviate the clinical symptoms and signs of steroid resistant allergic rhinitis with confirmed efficacy and safety, thus is recommended in steroid resistant allergic rhinitis. Increased doses or prolonged treatment of steroid is inappropriate.

OBJECTIVE@#To study the effects of long term use of beclomethasone dipropionate (BDP) nasal spray on bone density with perennial allergic rhinitis (AR) in adults.@*METHOD@#A 5-year randomized study was conducted on the effects of BDP nasal spray on serum calcium, phosphorus, alkaline phosphatase, and bone density determined before and after the treatment in 36 adult patients with perennial AR. 20-45 years of age, were randomly divided into 3 groups. That is group A (nasal spray 1 - or =5 year). The data were analyzed by paired t test.@*RESULT@#The perennial AR were followed up for more than > or =1 year, > or =3 year and > or =5 year to observe the influences of nasal spray BDP. There were no significant difference between the data examined before and after the treatment (P > 0.05). Bone development is not influenced by nasal spray BDP < or =400 microg/d within 5 years.@*CONCLUSION@#Long term use of BDP nasal spray in adult patients does not lead to osteoporosis if the lowest effective steroid dose is given.

ABSTRACT
ObjectiveTo define the components of membranous labyrinth proteins which take part in the development of autoimmunity in experimental animals with autoimmune inner- ear disease(AIED). MethodsAn animal modee of AIED was es tablished by using homologous crude inner ear antigen to immune the guinea pig. The autoantibodies against inner ear antigenin the sera of animals were examined by immunoblotting assay. ResultsThe components of membranous labyrinth proteins were very complicated, which had five main bands in electrophoresis; most of immunized animals had only developed two sorts of autoantibod ies which were anti-68 kD and 136 kD membranous labyrinth proteins. There was no positive reaction in sera of control animals. Conclusion This results showed that the immune reactions against the 68 kD and 136 kD proteins of membranous labyrinth may be autoimmune bases for AIED in animals.
ABSTRACT
Objective To improve the diagnostic quality of tumors composed of smooth muscles in alimentary tract.Methods The clinical and X-ray date of 40 cases of smooth musles tumors confirmed by surgery and pathological examination.Results Fifteen cases in the esophagus (including leiomyoma 14,leiomyosarcoma 1)showed the following X-ray appearence:filling defect,unfolding of mucous membrane,widening of the canal and pericanal mass 25 cases in the stomach and intestine(including leiomyoma 6,leiomyoplastoma 7,leiomyosarcoma 12)showed the following X-ray changes depending on the way of tumor,grows,cyst formation caused by tumor necrosis,compression of the stomach or intestine and adjacent organs,rarely seen calcification in tumor.The X-ray changes of small intermascular leiomyoma were short of specific X-ray signs.Conclusion X-ray exeamination is a reliable diagnostic method for smooth muscles tumors in alimentary tract.
