ABSTRACT
@#Plants are alternative source of natural medicines due to secondary active metabolites. Fagonia cretica extracts and Gradient High-Pressure Liquid Chromatography fractionations were checked against multidrug-resistant gastrointestinal pathogens including, Salmonella typhi, Escherichia coli and Shigella flexneri. ESI-MS/MS analysis of bioactive HPLC fractions was performed to elucidate antibacterial compounds. F. cretica extracts exhibited potential antibacterial activity. Twenty-four (24) HPLC fractions were obtained from methanol, ethanol and aqueous extracts of F. cretica. Eighteen (18) fractions showed antibacterial activity, while no activity was observed by the remaining six (6) fractions. HPLC fractions, F1 (25g ± 0.20 mm) and F2 (15f ± 0.12 mm) of aqueous extract exhibited activity against multidrug resistant GI pathogens. Gallic acid, quinic acid, cyclo-l-leu-l-pro, vidalenolone, liquirtigenin, rosmarinic acid and cerebronic acid were identified in F1 fraction of aqueous extract, while succinic acid, cyclo (l-Leul-Pro) and liquirtigenin were identified in F2 fraction of aqueous extract through ESI-MS/MS analysis. F. cretica extracts and HPLC fractions showed potential activity against MDR GI pathogens. Vidalenolone, Cyclo-1-leu-1-pro and Cerebronic acid are first time reported in F. cretica. Further characterization of bioactive compounds from F. cretica may be helpful to elucidate antibacterial therapeutic molecules.

Current research was aim to assess the prevalence of tapeworms among 5-12years school children residing in district Lower Dir, Pakistan from January 2019-December 2019. The wet mount preparation in saline/iodine/methods were used for stool examination. Data was analyzed using appropriate descriptive, static methods. Of the 400 children studied 71.7% were infected with one or more species of intestinal parasites. Single infection of cestode species was found in 69 individuals with 17.2% prevalence and multiple parasitic infections were identified in 19.7% (n=79/400) individuals. The multiple infection were comprised as 10% (n=40) double, 6.75% (n=27) triple and 3% (n=12) quadruple. A total of 9 species of helminths and one species of protozoan infection. Among the helminths Ascaris lumbricoides was the most prevalent 33.1% (n=95), Taenia saginata 22.6% (n=65), hookworm 19.8% (n=57), Hymenolepis nana 18.8% (n=54), Enterobius vermicularis and Hymenolepis diminuta 1.39% (n=4each), Trichuris trichura 1.04% (n=3), Toxocara spp 0.69% (n=2) and Schistosoma japonicum 0.34% (n=1) were reported. One protozoan species was Cryptosporidium spp 0.69% (n=2) in current study. In case of A.lumbricoides, hookworm, E.vermicularis, T.trichura, T.saginata, H.nana and H.diminuta the male children of below 8 years of age were highly infected. Other infections are reported in the same prevalence with slight difference if any. We conclude that there is a need for mass scale campaigns to create awareness regarding health and hygiene in children and the need for development of effective poverty control programs because deworming alone is not adequate to control parasitic infections.

Trypanosomosis or surra is caused by the haemoflagellate parasite, Trypanosoma evansi and is an important disease of animals, including domestic and wild herbivores and carnivores, in tropical countries. The invariant surface glycoproteins (ISGs) are blood stream stage specific and are uniformly distributed over the entire surface of the trypanosomes. In the present study, the extracellular domain (ED) region of ISG-75 from T. evansi, consisting of 1320 nt, encoding a polypeptide of 440 amino acids, has been heterologously expressed in Escherichia coli. Further, the immunoreactivity of recombinant ISG-75 (rISG-75) was characterized in immunoblot and ELISA using T. evansi hyper immune sera raised in experimental animals. The protein was found immunoreactive when compared with a panel of antigens (VSG RoTat 1.2 and whole cell lysate) using bovine serum samples from field. The diagnostic potential of rISG-75 was evaluated in ELISA with large number of bovine field serum samples. The optimum sensitivity and specificity were 98.47 and 99.1, respectively. The present finding showed that the expressed protein has potential use in the serodiagnosis of trypanosomosis.

Background and Aims: Providing a necessary care for a sick person outside home 'in hospes or hospital' dates back to nearly 300 century BC. In the present day hospital care facilities has been taken an institutional shape both in public and private sector. A hospital bed is both a scarce and expensive commodity in healthcare. Administrators running hospitals are in a dire need of objective measures and methods for efficient management of their limited financial resources. Bed utilization rates can be of immense help in realistic and effective decision making. The present study was undertaken to explore utilization of bed in a specialized tertiary care hospital in the Dhaka city. Methods: Hospital records of the year were reviewed- age, gender, disease profile, duration of hospital stay, outcome of treatment were recorded and bed occupancy rate was calculated. Data were presented as number, percentage and/ or mean SD, as appropriate. The dada were managed by Statistical Package for Social Science (SPSS) for Windows Version 10. Results: The results showed in the year 2001 total number of admissions were 13,305 of which 9953 (74.8%) were male and 3352 (25.2%) female. Average monthly admission was 1109. Maximum number of admissions (1304) was observed in the month of September of that year. Male admission rate was higher than female admission throughout the year. Among all the admission 27.2% were of road traffic accident cases. Among the admitted patients there was 57.3% discharge with advice, 1.9% death, 14.6% discharge on request bond, 12.7% discharge on request. Of all the admission there 12.5% found to be absconded. Bed occupancy rate was 79.75% and average length of stay in the hospital 18.47 days. Conclusions: The present data suggest that (i) in terms of bed occupancy rate the NITOR found to run in optimal capacity which, however, might be attributed to the relative high rate of ascendance and discharges on requests; (ii) average length of stay of patients appeared to be relatively longer and (iii) the management need to look into the issue and take appropriate measures to reduce patients unwanted long duration of stay and make the tertiary care hospital improve the quality of services.
ABSTRACT
To examine the misoprostol efficacy and safety in induction of labor of women with severe preeclampsia at or near term when the cervix is unfavorable. A prospective comparative study was conducted in Al Thawara General Hospital Sana'a, Yemen, from June 2009 to June 2010. One hundred and thirteen women met the inclusion criteria. They were divided into 2 groups. The study group [n=56] and the control group [n=57]. The study group were given 50 micro g of Misoprostol intravaginally every 4 hours to a maximum of 6 doses. Maternal age, gestational age, parity, initial cervical status, the rate of cesarean section, and neonatal outcomes were analyzed and compared to the control group. The vaginal delivery was achieved in 69.6% in the study group versus 15.8% in the control group [p<0.0001]. The overall cesarean section was performed in 30.3% of the study group versus 84.2% [p<0.0001]. The mean time from insertion to delivery was 12.12 +/- 2.1 hours and the mean dosing was 2.77 +/- 1.3. There were no significant differences between the 2 groups in the frequency of abnormal fetal heart rate, Apgar score and admission to the neonatal intensive care unit. Misoprostol when given intravaginally in 50 micro g 4-hourly dosing regimen is an effective agent for ripening the cervix in this group of women

Aims: The morphological study was aimed to finedout the ovarian weight in dif ferent age group in Bangladeshi people to increase the knowledge regarding weight variation in our population. Subject & Methods: This cross - sectional study was carried out on Sixty two postmortem tissue contain ovary and fallopian tube along with surrounding structures were collected from 62- female cadaver of different age group by block dissection and fixed in 10% formal saline solution. Gross and fine dissections were carried out to see the morphological parameter of weight of the ovary in different age groups. In the present study, findings were compared with findings of the other researchers. Results: Maximum mean weight of the right ovary is found in this study in group C (46-80) years is 5.78gm and minimum weight is found in group A (2-13 years ) is 3.36gm .But incase left ovary Maximum in group B(14-45)years is 5.01 and minimum group a (2-13)is 2.72gm. statistatical analysis significant different between two groups was calculated by using students "T"Test. In the present study it is observed that the weight of the ovary is not equal on both side of same individual.

Aims of the present study were to find out the histological pattern of atypically presenting nephrotic syndrome children and their response with oral corticosteroid therapy. This prospective study was carried out in the Paediatric Nephrology unit of Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, for a period of two years, from September 2001 to September 2003. Forty children with nephrotic syndrome with atypical presentations such as gross haematuria, renal impairment, hypertension and hypocomplementaemia were enrolled for the study. For each patient proper investigations were done to screen out infection and necessary treatment was given accordingly. Renal biopsy was done in all cases after treating infection and controlling hypertension and specimen were evaluated by light and immunoflourescence microscopy. All the patients were treated with oral prenisolone 60 mg/m2/day for 4 weeks and response were observed by heat coagulation test. In present study it was observed that mean age of presentation was 8.73+/-4.36 years. Hypertension, gross haematuria, impaired renal function and hypo-complementaemia were observed in 50%, 45%, 19% and 15% cases respectively. When histopathologies were considered, 90% cases had other than minimal change disease (MCD) and Mesangial proliferative glomerulonephritis (MesPGN) was the most common histopathology (47.5%). Regarding steroid therapy only 43.2% patients were responsive to oral prednisolone therapy within 28 days. Atypically presented nephrotic syndrome usually manifested with higher age of onset, gross haematuria, hypertension, renal insufficiency and hypocomplementaemia. Mesangial proliferative glomerulonephritis was the predominant histological type. The majority (56.8%) of these atypically presented nephrotic syndromes were resistant to steroid therapy.
ABSTRACT
The aim of this study was to establish the standard weight of the normal spleen in Bangladeshi people. One hundred and twenty human cadavers of which eighty seven male and thirty three female were dissected to remove spleen with associated structures. Collected specimens were tagged with specific identification number, and divided into five groups according to age, sex and height of the individual. Gross and fine dissections were carried out after fixing the specimen in 10% formal saline solution. Weight of the spleen was measured by analytical balance and expressed in gram and findings of the study were compared with the findings of national and global studies. This cross sectional descriptive study was carried out in the Department of Anatomy of Mymensingh Medical College, Mymensingh. In this study maximum mean weight of spleen was 86.35 gram in male and 85.33 gram in female in 31 – 45 years age group. Minimum mean weight of spleen was 47.37 gram in male and 38.83 gram in female up to 15 years age group. The weight of spleen increases with age of the individual. Mean weight of spleen in male was 73.43 gram, which was higher than the mean weight of female spleen (59.17 gram). According to height of individual the mean weight of spleen was maximum 84.32 gram in 165.01 to 180 cm height group and minimum in 54.87 gram in up to 120 cm height group which indicate that weight of the spleen increases with height of the individual. In conclusion, the weight of the spleen depends on the age, sex and body height of the individual.

This cross sectional descriptive morphological study was done to see the site of origin of cystic artery supplying the gallbladder in Bangladeshi people to increase the knowledge regarding variation anatomy in our population. Sixty post mortem tissue blocks containing liver and gallbladder along with surrounding structures were collected from 40 male and 20 female cadavers of different age groups and fixed in 10% formal saline solution. This study was carried out in the Department of Anatomy in Mymensingh Medical College, Mymensingh , from July 2007 to June 2008. Gross and fine dissections were carried out to study the different origin of cystic artery and its topographic relationship with adjacent structures. In the present study, findings were compared with the findings of Western researchers. In the present study, the so-called typical origin of the cystic artery from right hepatic artery was 90% and in 10% cases it was found to arise from other sources. Out of 10%, 3% arise from left hepatic artery, 3% from junction between right and left hepatic artery, 2% from hepatic artery proper and 2% from gastro duodenal artery.

Cytomegalovirus (CMV) infection is a very rare infection in children as well as in infancy. In CMV infection extra renal manifestations in central nervous system (CNS), eyes and hematological are prominent and common than renal manifestation. We are describing two case reports one at the age of 5 months and another at one month with features of both extra renal and renal manifestations of nephritic syndrome. Our first case presented with predominant features of CNS manifestations like convulsion and spastic monoplegia, sensory type of deafness and absence of light reflexes along with nephritic features. Deafness and absence of light reflex were confirmed by audiometric and ophthalmological examination and brain atrophy was confirmed by CT scan. Our second case had features of hypothyroidism along with nephritic features. Hypothyroid status was confirmed by elevated serum TSH level and reduced T4 level. In both cases nephritic features were confirmed at bed site urine examination by heat coagulation test and other relevant investigations. CMV infection was confirmed in both cases by detecting anti CMV IgM by ELISA method. Both patients were clinically improved with intravenous gangcyclovir therapy. In these two cases reports, clinical, serological and therapeutic observation established the causal relationship between the CMV infection and nephrotic syndrome.

Leptospirosis is one of the important zoonoses of man and animals. In recent years, it has increasingly been reported from different parts of the world including India. Sera from 536 individuals with pyrexia of unknown origin (PUO) belonging to different age group, sex and occupation and having varying levels of exposure to animals, forest and flood were examined for the presence of antibodies to pathogenic Leptospira by commercially available kit. Overall 22.57% individuals with PUO were found to be positive for leptospirosis with 27.93% among male and 11.79% among female patients. This difference among..genders was statistically significant. The seropositivity was higher among patients in the age groups of 21-30 years followed by 31-40 and 41-50 years. Seropositivity was higher among individuals having contact with animals, working in forest and flood affected areas.

Toxoplasmosis is one of the important zoonoses of man and has been known to cause serious problems particularly in females. A study on seroprevalence of toxoplasmosis was undertaken amongst the human population in Assam to determine the level of exposure of the population to the infection by using commercial ELISA kits. Of the 241 sera belonging to different age groups, sex and religion and having varying levels of exposure to the animals examined, 23 (9.54%) were positive for toxoplasmosis. No significant difference in the prevalence amongst males and females was observed. Some occupational groups like veterinarians, pet keepers and farmers were found to infect more frequently. Although the overall prevalence rate of toxoplasmosis was relatively low, higher prevalence rate of toxoplasmosis amongst the exposed groups warrants due care by these groups when they are handling the animals.

The objective of this study was to investigate the angiogenic potential of human chorion-derived stem cells (CDSC) cultured in medium containing bFGF and VEGF (EDM50). Total RNA was extracted from cells cultured in FD+10% FBS and EDM50. Quantitative RT-PCR was carried out to score the differential mRNA expression of genes involve in angiogenesis and endothelial differentiation. Our finding demonstrated that all angiogenic and endothelial associated genes were expressed higher in EDM50. Expression level of ANG-1, eNOS and VEGFR2 were significantly higher in EDM50 compared to FD+10% FBS. Our results suggested that human CDSC cultured in EDM50 can be used for angiogenesis purpose in regenerative medicine.
ABSTRACT
The aim of the study is to determine the neuronal and glial gene expression of cultured human amniotic epithelial cells (HAECs) in serial passages. HAECs obtained from human term placentae were cultured in F12:DMEM (1:1) + 10% FBS +10ng/ml EGF in serial passages. Quantitative RT-PCR was used to assess the gene expression analysis. The results showed that the cultured HAECs expressed the neural stem cell genes (Nestin, NSE and Vimentin), mature neuronal genes (TH, MAP-2, beta-III-tubulin and NFM) and glial genes (CNPase, MBP and Olig). These neural stem cell genes increased with serial passages while the genes expression for mature neuronal and glial cells were downregulated. These results suggested that HAECs may promote or involve in neurogenesis and gliagenesis.

The aim of the study is to evaluate the stemness gene expression of cultured human amniotic epithelial cells (HAECs) in serial passages. Quantitative RT-PCR was used to assess the gene expression analysis. The results showed that cultured HAECs expressed and downregulated the stemness genes expression for Oct-4, Sox-2, Nanog3, FGF4, Rex-1, FZD-9, BST-1 ABCG2. However, vimentin and nestin gene expression were upregulated. The results suggested that cultured HAECs may have pluripotent and multipotent properties.
ABSTRACT
Angiogenic induction was made to promote angiogenesis by differentiating stem cells towards endothelial cells. However, the stemness property of induced cells has not been revealed yet. Hence, we aim to evaluate the differential mRNA expression of stemness genes in human chorion-derived stem cells (CDSC) after being cultured in EDM50 comprised bFGF and VEGF. Results indicated that CDSC cultured in EMD50 expressed significantly higher mRNA level of Sox-2, FZD9, BST-1 and Nestin. In addition Oct-4, FGF-4 and ABCG-2 were also upregulated. Our finding suggested that CDSC after angiogenic induction enhanced its stem cell properties. This could be contributed for the mechanism of stem cell therapy in ischemic problem.

The study was done to find out the length of human vermiform appendix in Bangladeshi people to magnify the knowledge regarding the diverse length of human vermiform appendix in our population. A total 100 vermiform appendix were measured with different age and sex during routine postmortem examination in the autopsy laboratory of forensic medicine department of Mymensingh Medical College. This cross sectional study was done by convenient sampling technique. For convenience of differentiating the length of vermiform appendix in relation to different age and sex, findings were classified in four groups (up to 20 years, 21 to 35 years, 36 to 55 years and 56 to 70 years). Length of vermiform appendix was 2.6 cm to 14 cm. Mean length was 7.9 cm in female and 7.56 cm in male. The highest mean length of vermiform appendix in group A was 9.17 cm and lowest was 5.93 cm in group D.

The pathogenesis of diabetic retinopathy is still unclear. The relative role of duration of diabetes, glycemic and insulinemic status in the etiopathogenesis of retinopathy is to be clearly understood. Hypertension, hyperlipidemia, pregnancy and age at diagnosis have been thought to be factors associated with diabetic retinopathy. Taking advantage of the availability of a group of young lean, normotensive and generally normolipidemic subjects in Bangladeshi population, the present study was undertaken to investigate the relationship between insulin secretory capacity and microvascular damage in the pathogenesis of diabetic retinopathy. A total number of 91 diabetic subjects, diabetes diagnosed before the age of 30 years, were recruited form the out-patient Department, BIRDEM Hospital. Diabetic subjects recruited were new- to previously diagnosed cases with duration of diabetes around 8 years. Insulin secretory capacity was assessed by C-peptide and C-peptide was estimated by ELISA method. Urinary albumin was measured by Immunoturbidetric methods. The subjects were grouped on the basis of duration of diabetes and presence (DR) and absence (NDR) of diabetic retinopathy. Subjects were also divided on the basis of albumin-creatinine ratio (ACR) level. ACR level was observed sharply increased with the duration of diabetes and showed a peak after 8 years. In the ACR positive cases 95.25% of diabetic patient had retinopathy. Both retinopathic and ACR positive subjects had low insulin secretory capacity as evident by absolute C-peptide level and C-peptide glucose ratio. Retinopathy was found increased with generalized vasculopathy. Both microvasculopathy and retinopathy seemed to occur at around 8 years. Insulin secretory capacity in particular seemed to have predominant determinant effect in the etiopathogenesis of diabetic retinopathy.
