ABSTRACT
Resumen Objetivo: Describir medidas de presión arterial sistólica (PAS), diastólica (PAD) y media (PAM) en recién nacidos sanos de término y pretérmino tardío para establecer parámetros de normalidad. Método: Estudio transversal realizado en el cunero fisiológico del Hospital Español, Ciudad de México. Se incluyeron 551 pacientes recién nacidos sanos. Las tomas de PA fueron realizadas dentro de las primeras 48 horas de vida con método oscilométrico. Posterior a la evaluación de normalidad, se realizó un análisis descriptivo de la población y cálculo de percentiles (25, 50 y 75) específicos para cada semana de gestación. Todos los análisis fueron realizados en STATA v14.2. Resultados: Los recién nacidos de sexo masculino tuvieron un valor medio de PAS de 64.6 mmHg en la semana 35 de gestación, este valor aumentó a 69.8 mmHg en la semana 40. El valor de la PAD fue de 42.6 mmHg en la semana 35 de gestación, disminuyendo a 40.8 mmHg para la semana 40. Los valores medios de PAS en los recién nacidos de sexo femenino fueron de 65.5 mmHg en la semana 35, teniendo un incremento a 73.5 mmHg en la semana 40. El valor de la PAD en la semana 35 de gestación fue 38 mmHg, incrementando a 41.3 mmHg en la semana 40. Conclusiones: Los valores de PA en recién nacidos sanos se modifican con la edad gestacional y el sexo. Estos resultados pueden servir como referencia para otros médicos ubicados en países o ciudades con alturas similares a la Ciudad de México.
Abstract Objective: Describe the measurements of systolic, diastolic and mean blood pressure in healthy term and late preterm newborns to establish normal values. Methods: Cross-sectional study carried out in the nursery of the Hospital Español, located in Mexico City. A sample of 551 healthy newborns were included in the study. Blood pressure (BP) measurements were taken within the first 48 hours of life with the oscillometric method. After the evaluation of normality, a descriptive analysis of the population and calculation of percentiles (25, 50 and 75) specific for each week of gestation was performed. All analyzes were performed in STATA v14.2. Results: Male newborns had a mean SBP value of 64.6 mmHg at week 35 of gestation, this value increased to 69.8 mmHg at week 40; the systolic blood pressure (SBP) value was 42.6 mmHg at week 35 of gestation, which decreased to 40.8 mmHg at week 40. The mean SBP values in female newborns were 65.5 mmHg at week 35, increasing to 73.5 mmHg at week 40; the diastolic blood pressure (DBP) value at week 35 of gestation was 38 mmHg, increasing to 41.3 mmHg at week 40. Conclusions: The BP values in healthy newborns are modified by the gestational age and sex of the patients. These results can serve as a reference for other physicians located in countries or cities with a similar altitude than the one in Mexico City.

Abstract Background: Rhabdoid tumors are malignant neoplasms of low prevalence, aggressive behavior, and high mortality. They were initially described as renal tumors, although tumors with the same histopathological and immunohistochemical characteristics have been discovered in other locations, mainly in the central nervous system. Few cases of mediastinal location have been reported internationally. This work aimed to describe the case of a mediastinal rhabdoid tumor. Case report: We describe the case of an 8-month-old male patient admitted to the pediatric department with dysphonia and laryngeal stridor progressing to severe respiratory distress. Contrast-enhanced computed tomography of the thorax showed a large mass with homogeneous soft tissue density, and smooth and well-defined borders, with suspicion of malignant neoplasm. Due to the oncological emergency compressing the airway, empirical chemotherapy was initiated. Subsequently, the patient underwent incomplete tumor resection due to its invasive nature. The pathology report showed morphology compatible with a rhabdoid tumor, which immunohistochemical and genetic studies corroborated. Chemotherapy and radiotherapy to the mediastinum were administered. However, the patient died three months after the initial treatment due to the aggressive behavior of the tumor. Conclusions: Rhabdoid tumors are aggressive and malignant entities difficult to control and have poor survival. Early diagnosis and aggressive treatment are required, although the 5-year survival does not exceed 40%. It is necessary to analyze and report more similar cases to establish specific treatment guidelines.
Resumen Introducción: Los tumores rabdoides son neoplasias malignas de baja prevalencia, con comportamiento agresivo y alta mortalidad. Inicialmente fueron descritos como renales, aunque posteriormente se han descrito tumores con las mismas características histopatológicas e inmunohistoquímicas en otros sitios, principalmente en el sistema nervioso central. Internacionalmente se han descrito pocos casos de localización mediastinal. El objetivo del presente trabajo fue describir el caso de un tumor rabdoide de localización mediastinal. Caso clínico: Se presenta el caso de un paciente de sexo masculino de 8 meses de edad que ingresó al servicio de pediatría con disfonía y estridor laríngeo que progresó a dificultad respiratoria severa. En la tomografía computarizada contrastada de tórax se observó una gran masa homogénea con densidad de tejidos blandos, de bordes lisos y bien definidos, por lo que se sospechó una neoplasia maligna. Debido a la urgencia oncológica compresiva de la vía aérea se inició con un esquema empírico de quimioterapia. Posteriormente se sometió a resección tumoral incompleta por carácter invasor. El reporte de patología mostró morfología compatible con un tumor rabdoide, el cual se corroboró con estudios de inmunohistoquímica y genética. Se administró un esquema de quimioterapia y radioterapia al mediastino. Sin embargo, el paciente falleció a los 3 meses del inicio de tratamiento debido al comportamiento agresivo del tumor. Conclusiones: Los tumores rabdoides son entidades agresivas y malignas de difícil control y con pobre supervivencia. A pesar de que se requiere un diagnóstico precoz y un tratamiento agresivo, no se ha logrado la supervivencia a 5 años mayor al 40%. Es necesario analizar una mayor cantidad de casos para establecer guías específicas de tratamiento.

Purpose@#The global prevalence of overactive bladder (OAB) is estimated at 11.8%. Despite existing treatment options such as sacral neuromodulation, a substantial number of patients remain untreated. One potential alternative is noninvasive transcutaneous electrical stimulation. This form of stimulation does not necessitate the implantation of an electrode, thereby eliminating the need for highly skilled surgeons, expensive implantable devices, or regular hospital visits. We hypothesized that alternative neural pathways can impact bladder contraction. @*Methods@#In this pilot study, we conducted transcutaneous electrical stimulation of the abdominal wall (T6-L1), the ear (vagus nerve), and the ankle (tibial nerve) of 3 anesthetized female Sprague-Dawley rats. Stimulation was administered within a range of 20 Hz to 20 kHz, and its impact on intravesical pressure was measured. We focused on 3 primary outcomes related to intravesical pressure: (1) the pressure change from the onset of a contraction to its peak, (2) the average duration of contraction, and (3) the number of contractions within a specified timeframe. These measurements were taken while the bladder was filled with either saline or acetic acid (serving as a model for OAB). @*Results@#Transcutaneous stimulation of the abdominal wall, ear, and ankle at a frequency of 20 Hz decreased the number of bladder contractions during infusion with acetic acid. As revealed by a comparison of various stimulation frequencies of the tibial nerve during bladder infusion with acetic acid, the duration of contraction was significantly shorter during stimulation at 1 kHz and 3 kHz relative to stimulation at 20 Hz (P = 0.025 and P = 0.044, respectively). @*Conclusions@#The application of transcutaneous electrical stimulation to the abdominal wall, ear, and tibial nerve could provide less invasive and more cost-effective treatment options for OAB relative to percutaneous tibial nerve stimulation and sacral neuromodulation. A follow-up study involving a larger sample size is recommended.

Resumen Introducción: Los gramnegativos continúan siendo los causantes de infecciones asociadas a la atención a la salud (IAAS). Material y métodos:: Analizamos la resistencia antimicrobiana de patógenos durante el 2013 vs. 2018 y lo comparamos con lo publicado en 2006 vs. 2012. Resultados: Identificamos nueve patógenos gramnegativos, de un total de 404 aislamientos, con una prevalencia en 2013 (N = 227 [0.22]) vs. 2018 (N = 177 [0.17]) y una incidencia por egresos (6,607 en el 2013 y 7,778 en el 2018) del 3.4 y 2.2% respectivamente. Destacaron tres patógenos: Klebsiella pneumoniae (129 [31.93%]), Pseudomonas aeruginosa (85 [21.03%]) y Escherichia coli (80 [19.80%]). Estos, llamados patógenos ESKAPE-E, prevalecieron como causantes de IAAS. Identificamos un aumento en los patrones de resistencia para muchos patógenos en 2018. Conclusión: La multirresistencia a patógenos ESKAPE-E es un serio problema de salud pública, por carecer de alternativas terapéuticas para enfrentar este reto. Los mapas de resistencia bacteriana ayudan en la prescripción antibiótica.
Abstract Background: Gram-negatives continue to be the cause of infections associated with health care (HCAI). Material and methods: We analyzed the antimicrobial resistance of pathogens during 2013 vs. 2018 and we compare it with what was published in 2006 vs. 2012. Results: We identified 9 gram-negative pathogens, out of a total of 404 isolates, with a prevalence in 2013 (N = 227 [0.22]) vs. 2018 (N = 177 [0.17]) and an incidence due to discharges (6,607 in 2013 and 7,778 in 2018) of 3.4 and 2.2%, respectively.Three pathogens stood out Klebsiella pneumoniae (129 [31.93%]), Pseudomonas aeruginosa (85 [21.03%]) and Escherichia coli (80 [19.80%]). These, called ESKAPE-E pathogens, prevailed as the cause of HCAI. We identified an increase in resistance patterns for many pathogens in 2018. Conclusion: Multi-resistance to ESKAPE-E pathogens is a serious public health problem, due to the lack of therapeutic alternatives to face this challenge. Bacterial resistance maps help in antibiotic prescription.

Candida auris es un hongo emergente, con gran relevancia en corto tiempo, como problema de salud pública mundial. Se reporta por primera vez en el Perú la presencia de Candida auris en 3 pacientes adultos internados en un hospital nacional de alta complejidad en el último trimestre del año 2020, durante la pandemia COVID-19. Los pacientes fueron hospitalizados en UCI; sin embargo, solo en 2 pacientes se aisló dicho germen durante su internamiento en UCI. Los pacientes tuvieron varias comorbilidades y tiempos prolongados de hospitalización desde su admisión hasta tener el primer cultivo positivo a C. auris. Todos los pacientes adquirieron una infección nosocomial bacteriana en algún momento de su hospitalización y recibieron antibióticos de amplio espectro. Todas las cepas aisladas fueron resistentes a fluconazol. El equipo de control de infecciones del hospital reforzó las medidas de contención y el Ministerio de Salud del Perú emitió una alerta epidemiológica.
Candida auris is an emerging fungus that has gained great relevance as a global public health problem in a short time. The presence of Candida auris in 3 adult patients admitted to a national hospital of high complexity in the last quarter of 2020 in the midst of the COVID-19 pandemic is reported for the first time in Peru. The patients were hospitalized in the ICU, however, this germ was isolated in only 2 patients while they were hospitalized in the ICU. The patients had various comorbidities and long hospitalization times from admission to having their first culture positive for C. auris. All patients acquired a bacterial nosocomial infection at some point during their hospitalization and received broad-spectrum antibiotics. All isolates were resistant to fluconazole. The hospital's infection control team reinforced containment measures and the Ministry of Health of Peru issued an epidemiological alert.

Objetivou-se avaliar diferentes modalidades ventilatórias em cães de diferentes idades submetidos à fração inspirada de oxigênio (FiO2) de 40% e 100%. Foram utilizados 36 cães de três grupos etários (GJ: 0-5; GA: 5-10 e GG: 10-15 anos), sem padronização de peso, sexo, raça e procedimento cirúrgico. Foram pré-medicados com acepromazina e morfina (0,02 e 0,5mg/kg), induzidos à anestesia geral com propofol dose-efeito, manutenção do plano anestésico com isoflurano em 1,3 V% e fornecimento de oxigênio conforme a FiO2 estabelecida para o grupo. Os animais foram submetidos a quatro diferentes modalidades ventilatórias: ventilação espontânea (VE), ventilação ciclada a volume (VCV), ventilação ciclada a pressão (VCP) e ventilação ciclada a pressão com PEEP (VCPP), e permaneceram 30 minutos em cada modalidade. Os parâmetros cardiovasculares mantiveram-se estáveis para todas as FiO2, modalidades ventilatórias e idades. Com relação aos parâmetros ventilatórios, na FiO2 100%, foram observados PaCO2 de 45mmHge e 29% de shunt, enquanto a FiO2 40% apresentou PaCO2 de 43 mmHg e 13% de shunt. Em relação às diferentes idades, os animais adultos e geriátricos apresentaram maiores valores de shunt (26% e 22%) e PaCO2 (44mm/Hg e 46mm/Hg). Conclui-se que a fração inspirada de 40% e a modalidade ventilatória ciclada a volume mostraram-se mais eficientes.(AU)
The objective was to evaluate different ventilatory modalities in dogs of different ages submitted to the inspired fraction of oxygen (FiO2) of 40% and 100%. Thirty-six dogs from three age groups (GJ 0-5, GA 5-10 and GG 10-15 years) were used, without standardization of weight, gender, race and surgical procedure. They were premedicated with acepromazine and morphine (0.02 and 0.5mg/kg), induced to general anesthesia with propofol dose/effect, maintenance of the anesthetic plane with isoflurane in 1.3V% and oxygen supply according to FiO2 established for the group. The animals were submitted to 4 different ventilation modalities, spontaneous ventilation (VS), volume-cycled ventilation (VCV), pressure-cycled ventilation (VCP) and pressure-cycled ventilation with PEEP (VCPP) and remained 30 minutes in each modality. The cardiovascular parameters remained stable for all FiO2, ventilatory modalities and ages. Regarding ventilatory parameters, in FiO2 and PaCO2 of 45mmHg and 29% of shunt, in FiO2 100%, PaCO2 of 43mmHg and 13% of shunt were observed. Regarding the different ages, adult and geriatric animals presented higher values of Shunt (26 and 22%) and PaCO2 (44 and 46mmHg). It was concluded that the inspired fraction of 40% and the volume-cycled ventilatory modality were more efficient.(AU)

Leishmaniasis is a parasitic disease of worldwide spread. It is caused by protozoa of the genus Leishmania and is transmitted to animals and humans through the bite of sand flies. In Brazil, leishmaniasis is one of the zoonoses of major importance and expansion. The objective of this work is to describe the clinical, pathological, immunohistochemical and molecular findings of cutaneous leishmaniasis by Leishmania enriettii in guinea pig (Cavia porcellus). Three animals had nodular and alopecia lesions on the muzzle, ears and ulcerated lesions on the distal extremities of the pelvic limbs. The males (2) also had diffuse thickening of the scrotal skin. Samples of the ulcerated cutaneous lesions were evaluated by cytology which were observed as amastigote forms of Leishmania. One of the animals was euthanized and necropsied. Histopathology showed abundant dermal infiltrate of macrophages, plasma cells, lymphocytes and multinucleated giant cells. Numerous macrophages contained parasitoid vacuoles with amastigote forms, evidenced by immunohistochemical examination. The molecular characterization based on the SSUrDNA gene identified the species as L. enrietti. The diagnosis of cutaneous leishmaniasis in these cases was based on pathological findings and confirmed by immunohistochemistry, PCR and sequencing.(AU)
A leishmaniose é uma doença parasitária de distribuição mundial. É causada por protozoários do gênero Leishmania e é transmitida para animais e seres humanos por meio da picada de flebotomíneos. No Brasil, a leishmaniose é uma das zoonoses de maior importância e expansão. O objetivo deste trabalho é descrever os achados clínicos, patológicos, imuno-histoquímicos e moleculares de leishmaniose cutânea por Leishmania enriettii em cobaia (Cavia porcellus). Três animais apresentavam lesões nodulares e alopécicas no focinho e orelhas, além de lesões ulceradas nas extremidades distais dos membros pélvicos. Nos machos (2), foi observado espessamento difuso da pele escrotal. Amostras das lesões cutâneas ulceradas foram avaliadas por citologia, nas quais foram observadas formas amastigotas de Leishmania. Um dos animais foi submetido à eutanásia e necropsiado. Na histopatologia, foi observado infiltrado dérmico abundante de macrófagos, plasmócitos, linfócitos e com células gigantes multinucleadas. Numerosos macrófagos continham vacúolos parasitóforos com formas amastigotas, evidenciados por meio do exame de imuno-histoquímica. A caracterização molecular baseada no gene de SSUrDNA identificou a espécie como L. enrietti. O diagnóstico de leishmaniose cutânea nesses casos foi baseado nos achados patológicos e confirmado pelas técnicas de imuno-histoquímica, PCR e sequenciamento.(AU)

RESUMEN Objetivo: proveer recomendaciones clínicas basadas en evidencia para el diagnóstico y tratamiento de la etapa aguda del ataque cerebrovascular isquémico en EsSalud. Materiales y métodos: se conformó un grupo elaborador de la guía (GEG) que incluyó médicos especialistas y metodólogos. El GEG formuló 8 preguntas clínicas a ser respondidas por la presente GPC. Se realizó búsquedas sistemáticas de revisiones sistemáticas y -cuando fue considerado pertinente- estudios primarios en Medline y Cochrane Controlled Register of Trials durante el 2018. Se seleccionó la evidencia para responder cada una de las preguntas clínicas planteadas. La certeza de la evidencia fue evaluada usando la metodología Grading of Recommendations Assessment, Development, and Evaluation (GRADE). En reuniones de trabajo periódicas, el GEG usó la metodología GRADE para revisar la evidencia y formular las recomendaciones, los puntos de buenas prácticas clínicas y el flujograma de diagnóstico y tratamiento. Finalmente, la GPC fue aprobada con Resolución N° 128-IETSI-ESSALUD-2019. Resultados: la presente GPC abordó 8 preguntas clínicas, divididas en cuatro temas: tamizaje, diagnóstico, tratamiento, soporte y rehabilitación. En base a estas preguntas se formularon 28 recomendaciones (8 fuertes y 20 condicionales), 38 puntos de buena práctica clínica, 1 nota de implementación y 2 flujogramas. Conclusión: el presente artículo resume la metodología y las conclusiones basadas en evidencias de la GPC para el diagnóstico y tratamiento de la etapa aguda del ataque cerebrovascular isquémico en EsSalud.
ABSTRACT Objective: to provide evidence based clinical recommendations for the diagnosis and therapy of the acute stage of ischemic cerebrovascular accident in EsSalud. Materials and methods : a group for producing the guideline was conveyed, including specialized physicians and methodologists. The group prepared 8 clinical questions to be answered by the guideline. Systematic searches of previous reviews were performed, and - when it was deemed necessary - primary studies in Medline and the Cochrane Controlled Registry of Trials for 2018 were reviewed. Evidence was selected aiming to respond each one of the proposed clinical questions. Certainty of the evidence was determined using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) methodology. In periodic work sessions, the group used GRADE methodology for reviewing the evidence and generated recommendations, good clinical practice items, and designed flow charts for both diagnosis and therapy. Finally, the guide was approved with the decree N° 128-IETSI-ESSALUD-2019. Results : this clinical practice guideline focused in 8 clinical questions, which were divided into four topics: screening, diagnosis, therapy, support and rehabilitation. On the basis of these questions, 28 recommendations were formulated (8 strong and 20 conditional), 38 items for good clinical practice, 1 implementation note, and 2 flow charts. Conclusion : this paper summarizes both evidence based methodology and conclusions from a new clinical practice guide for diagnosis and therapy of the acute stage of a cerebrovascular accident in EsSalud.

La presencia de bacterias patógenas, como Escherichia coli, afecta la calidad e inocuidad de las hortalizas que se consumen en fresco y se relaciona con graves problemas de salud. El objetivo de este trabajo fue determinar si 3 cepas diferentes de E. coli tienen la capacidad de penetrar y permanecer en plantas y frutos de tomate. Se siguió un diseño experimental completamente al azar, para lo cual se estableció un cultivo de tomate (variedad «Cid¼) en condiciones de invernadero y se evaluaron 3 tratamientos, T1 (E. coli O157: H7), T2 (E. coli de cultivo de tomate -#91;EcT-#93; O157: H16), T3 (E. coli de cultivo de espinaca -#91;EcH-#93; EcH O105ab) y un testigo T4, con 100 plantas cada uno y 4 formas de inoculación: en el sustrato, en el tallo, en el pecíolo y en el pedúnculo. Se realizaron muestreos en etapa vegetativa, floración, fructificación y madurez fisiológica para cuantificar en placa las UFC/g y saber si las bacterias lograban moverse y recuperarse en la raíz, el tallo, la flor y el fruto. Los grupos filogenéticos a los que correspondieron las bacterias recuperadas fueron confirmados mediante pruebas bioquímicas, serotipificación y PCR. A los 120 días la recuperación de bacterias en la planta fue del 23% (E. coli O157: H7), 28% (EcT O157: H16) y 55% (EcH O105ab) con la inoculación al sustrato, mientras que con la inoculación por punción la recuperación fue (en igual orden) del 5%, 3% y 4% a los 30 días; del 37%, 35% y 30% a los 90 días; y del 42%, 39% y 13% a los 65 días. Las cepas utilizadas mostraron la capacidad de entrar en la planta de tomate y de permanecer en ella y transportarse hasta llegar al fruto, sin producir síntomas que indiquen su presencia.
The presence of pathogenic bacteria, such as Escherichia coli affects the quality and safety of vegetables that are consumed fresh and is associated with serious health problems. The objective of this study was to determine if three different strains of E. coli can penetrate and remain in plants and tomato fruits. A completely randomized experimental design was followed for which a tomato crop ("Cid" variety) was established under greenhouse conditions and three treatments were evaluated, T1 (E. coli O157: H7), T2 (E. coli from tomato cultivation -#91;EcT-#93; O157: H16), T3 (E. coli from spinach cultivation -#91;EcH-#93; O105ab) and a T4 control, with 100 plants each and four forms of inoculation: in the substrate, steam, petiole and the peduncle. Samples were carried out in vegetative stage, flowering, fruiting and physiological maturity to quantify in petri dish CFU/g and know if the bacteria managed to move around and recover in root, stem, flower and fruit. The phylogenetic groups that corresponded to the bacteria recovered were confirmed by biochemical tests, serotyping and PCR. At 120 days the recovery of bacteria in the plant was 23% (E. coli O157: H7), 28% (EcT O157: H16) and 55% (EcH O105ab) whit inoculation to the substrate while the inoculation by puncture the recovery was (in the same order) of 5%, 3%, and 4% at 30 days; 37%, 35% and 30% at 90 days; and 42%, 39% and 13% at 65 days. The strains submit the ability to enter the tomato plant and to stay in it and transported to the fruit, without producing that indicate their presence.

Fermentation profile and microbial population were assessed in soybean silages without any additive (control), with inoculant (I), with I + powdered molasses (I+M), and with powdered molasses only (M). Soybean plants were harvested at the R6 stage and ensiled in 2kg-capacity laboratory silos. The additives were added to the natural matter base of silages. The assessed fermentation periods were 1, 3, 7, 14, 28, and 56 days. A 4 × 6 factorial arrangement (4 additives × 6 fermentation periods) in a completely randomized design with 3 replicates was used. Lactic, acetic, and butyric acids concentrations were influenced by additives and periods (P< 0.05). It was observed higher lactic acid values to control silages, on the 56th day. Lower average values of acetic and butyric acids were observed to I+M and M silages. It was observed quadratic effect to pH values with a reduction estimated of 0.5504, 0.5358, 0.6312 and 0.6680 units to pH values to control, I, I+M, and M silages in the first 10 days. A maximum lactic acid bacteria population was observed at the 28th day of fermentation in silages with inoculant. The inoculant and powdered molasses improve the fermentation profile of soybean silages.(AU)

As plantas de soja foram colhidas no estádio R6 e ensiladas em silos laboratoriais com capacidade de 2kg. Os aditivos foram adicionados às silagens na matéria natural. Os períodos de fermentação avaliados foram: 1, 3, 7, 14, 28 e 56 dias. Utilizou-se um esquema fatorial 4 × 6 (quatro aditivos × seis períodos), em um delineamento completamente ao acaso, com três repetições. Observou-se efeito de aditivos e de período (P<0,05) sobre os ácidos láctico, acético e butírico. Foi constatado maior teor de ácido láctico na silagem controle aos 56 dias. Para os teores dos ácidos acético e butírico, registraram-se menores valores médios nas silagens I+M e M. Observou-se efeito quadrático para o pH das silagens, com estimativa de redução de pH nos primeiros 10 dias igual a 0,5504; 0,5358; 0,6312 e 0,6680 nas silagens controle, I, I+M e M, respectivamente. A máxima população de bactéria ácido-láctica foi observada aos 28 dias nas silagens inoculadas. O inoculante e o melaço em pó melhoram o perfil fermentativo de silagens de soja.(AU)

Se determinaron areas superficiales entre 705 y 906 m2/g. Los carbones fueron amorfos y se detectó la presencia de agentes antibacterianos, tales como Ag, Cl, Cu y Si. Se determinó que para el As y Pb, cuyas concentraciones iniciales en el agua contaminada (agua del Río Tumbes-Perú) fueron 56,7 y 224,0 μg/L, respectivamente, el porcentaje de adsorción fue cercano al 100%. También se encontró que la relación entre el pH de carga cero de los carbones y pH del agua del río durante los experimentos juega un rol determinante en la adsorción de los elementos analizados. Por otro lado, la capacidad antibacteriana fue evaluada satisfactoriamente frente a las siguientes cepas de bacterias gram negativas fecales: Escherichia coli (ATCC® 25922™), Salmonella typhimurium (ATCC® 14028™) y Shigella flexneri (ATCC® 12022™). Esta capacidad se basa en la presencia superficial en los carbones de los agentes antibacterianos mencionados.
Abstract Comercial activated carbon samples (A, B, C, and D) used in filters for the treatment of water were characterized and evaluated in the decontamination of heavy metals present in river water and in the elimination of coliform microorganisms. The carbon samples had microporous and mesoporous structures. Surface areas of between 705 and 906 m2/g were found. The carbon samples were amorphous and the presence of antibacterial agents such as Ag, Cl, Cu, and Si was detected. It was determined that for As and Pb, whose initial concentrations in contaminated water (water of the Tumbes river-Peru) were 56.7 and 224.0 μg/L, respectively, the percentage of adsorption was close to 100%. The relationship between point of zero charge pH of the activated carbons and pH of the river water during the experiments plays a determinant role in the adsorption of the analyzed elements. The antibacterial capacity was evaluated satisfactorily against the following strains of fecal gram negative bacteria: Escherichia coli (ATCC® 25922™), Salmonella typhimurium (ATCC® 14028™), and Shigella flexneri (ATCC® 12022™). This ability is based on the surface presence in the carbons ofthe mentioned antibacterial agents.
Resumo Carvões ativados comerciais (A, B, C e D) usado em filtros para o tratamento de água foram caracterizados e avaliados na descontaminação de metais pesados da água do rio e para a remoção de microrganismos coliformes. Os carvões comerciais mostraram estruturas microporosas e mesoporosas. Foram determinadas as áreas superficiais entre 705 e 906 m2/g. As amostras de carvão foram amorfas e foi detectada a presença de agentes antibacterianos, tais como Ag, Cl, Cu e Si. Determinou-se que, no caso de As e Pb, cujas concentrações iniciais na água contaminada (água do rio Tumbes-Peru) foram de 56,7 e 224,0 μg/L respectivamente, a taxa de adsorção foi de quase 100%. Também foi econtrado que e a relação entre o pH de carga zero dos carvões e o pH da água do rio durante as experiências desempenha um papel decisivo na adsorção dos elementos analisados. Objetivou-se determinar a viabilidade de dois protocolos de sedação para ventilação prolongada em cães e seus efeitos hemodinâmicos e metabólicos. Doze cães, alocados aleatoriamente em dois grupos (n=6), receberam infusão contínua de midazolam (0,5mg/kg/h), fentanil (10µg/kg/h) e propofol (18mg/kg/h) no GMF ou cetamina (0,6mg/kg/h), morfina (0,26mg/kg/h) e propofol (18mg/kg/h) no GCM, durante 24 horas. Os cães foram ventilados mecanicamente com FiO2 de 40%, mantendo-se a normocapnia. A FC diminuiu 32% no GMF e 34% no GCM ao longo do tempo, reduzindo o IC em 24% no GMF e em 29% no GCM. A CaO2, o CvmO2, a DO2 e o VO2 diminuíram no GCM (5%, 16%, 31% e 7%) e no GMF (4%, 19%, 26% e 15%), respectivamente. A TEO2 aumentou 32% no GMF e 36% no GCM, sem diferenças entre grupos, porém a calorimetria indireta demonstrou diminuição do VO2, minimizando a redução da DO2. Não houve diferença entre os tempos para extubação, deambulação e recuperação total, com médias globais (minutos) de 33,8±15,9, 134,8±60,7 e 208±77,5, respectivamente. Conclui-se que ambos os protocolos permitiram a ventilação mecânica, com redução do IC e da DO2, porém sem prejuízos hemodinâmicos e metabólicos, podendo ser utilizados com segurança em cães hígidos.(AU)

Twelve dogs, were randomly allocated in two groups (n=6) who received constant rate infusion (CRI) of midazolam (0,5mg/kg/h), fentanyl (10µg/kg/h) and propofol (18mg/kg/h) in GMF or ketamine (0,6 mg/kg/h), morphine (0,26mg/kg/h) and propofol (18mg/kg/h) in GCM, during 24 hours. The dogs were mechanically ventilated to normocapnia with FiO2 of 40%. Heart rate decreased 32% in GMF and 34% in GCM during infusion time reducing CI in 24% at GMF and 29% at GCM. CaO2, CmvO2, DO2 and VO2 decreased in GCM (5%, 16%, 31% and 7% respectively) and GMF (4%, 19%, 26% and 15% respectively). Extraction ratio increased 32% in GMF and 36% in GCM without differences between groups; however, decreased VO2, evaluated for indirect calorimetry, suggests minimization of DO2 reduction. No differences between time to extubation, sternal recumbency and total recovery time were observed between groups, with an average of 33,8±15,9, 134,8±60,7 e 208±77,5 minutes respectively. We conclude that both protocols allowed mechanical ventilation with IC and DO2 reduction without metabolic and hemodynamic impairment, and can be safely used in healthy dogs.(AU)

Abstract Flood events in riparian forests of southern Brazil, can be characterized as unpredictable and of low magnitude with an average duration of less than 15 days. Inga marginata is an evergreen tree which grows in Southeast South America on a wide range of environments, including riparian forests. In this paper, the interactive effects of the light environment and soil flooding on morphological parameters of I. marginata were examined. Seedlings were acclimated in two contrasting light conditions: sun or shade for 30 days. Sun and shade plants were subjected to soil flooding for two periods; five or 15 days. After 5 days, the interaction between flooding and light did not affect growth, chlorophyll content and dry mass or the root-shoot ratio. After 15 days, flooded plants from the sunny treatment had a lower shoot dry mass compared to control sun plants and flooded plants from the shaded treatment. Moreover, the higher dry mass observed for shade plants compared to sun plants, following flooding, can also be directly associated with a higher content of soluble sugars. Shade plants of I. marginata showed a greater acclimation to soil waterlogging. This acclimation appears to be associated with a larger accumulation of soluble sugars compared to non-flooded plants. The responses observed on the shade plants appear to be decisive to indicate the use of I. marginata in degraded areas.
Resumo As inundações em florestas ripárias do Sul do Brasil, podem ser caracterizadas como imprevisíveis e de baixa magnitude com uma duração média de menos de 15 dias. Inga marginata é uma árvore que cresce no sudeste da América do Sul em uma grande variedade de ambientes, incluindo matas ciliares. Neste trabalho, os efeitos combinados da luminosidade e do e alagamento do solo nos parâmetros morfológicos de I. marginata foram examinados. As plântulas foram aclimatadas em duas condições contrastantes de luminosidade: sol e sombra por 30 dias. Plantas de sol e sombra foram submetidas ao alagamento do solo por dois períodos; cinco ou 15 dias. Após 5 dias, a interação entre a inundação e luminosidade não afetou o crescimento, teor de clorofila e massa seca e a razão raiz-parte aérea. Após 15 dias, plantas de sol sob inundação apresentaram menor massa seca na parte aérea em relação as plantas controle de sol e as plantas alagadas de sombra. Além disso, a maior massa seca observada nas plantas de sombra em comparação com plantas de sol, sob inundação, pode ser diretamente associado com um maior teor de açúcares solúveis. Plantas de sombra de I. marginata mostraram uma maior aclimatação ao encharcamento do solo. Esta aclimatação parece estar associada com um maior acúmulo de açúcares solúveis em comparação com as plantas não-inundadas. As respostas observadas nas plantas de sombra parecem ser determinantes para a indicação do uso de I. marginata em áreas degradadas.

Objetivou-se correlacionar a necessidade de resgate analgésico pós-operatório por meio das escalas analógica visual (EVA), de Glasgow, Colorado e Melbourne, por meio de um avaliador experiente (AE) e outro não experiente (ANE), em cadelas submetidas à mastectomia unilateral total. Foram utilizadas 24 cadelas, hígidas, internadas 24 horas antes do procedimento cirúrgico, para avaliação do seu comportamento, com o auxílio das escalas descritas acima no momento basal (M0). Foram pré-medicadas com acepromazina e morfina (0,02 e 0,5mg/kg) e induzidas à anestesia geral com propofol (4mg/kg), mantidas em plano anestésico com CAM de isoflurano 1%. A manutenção analgésica transoperatória foi realizada com cetamina e fentanil (10µg/kg/min e 10µg/kg/h). As demais avaliações ocorreram em uma, duas, quatro, seis, oito, 12 e 24 horas de pós-operatório, sendo os resgates realizados com morfina (0,5mg/kg), pela via intramuscular, quando fosse observada uma pontuação maior ou igual a 50, seis, dois e nove pontos, respectivamente, para as escalas descritas, quando observada pelo AE e quando ao menos duas das escalas demonstrassem esses valores. Houve aumento dos escores de dor do M1 ao M12 para o AE e para o ANE para a EVA. Na análise de Colorado, maiores pontuações de dor ocorreram em relação ao M0 entre o M2 e o M8 para o AE e do M1 ao M12 para o ANE. Na análise de Glasgow, maiores escores foram detectados entre o M1 e o M12 para o AE e do M1 ao M24 para o ANE. E para a de Melbourne, maiores valores foram observados do M1 e do M24 para o AE e o ANE. A melhor correlação entre as escalas foi de 0,775 entre Glasgow e Colorado e entre os avaliadores de 0,925 para a Glasgow. Conclui-se que a escala de Glasgow apresentou-se mais sensível para detectar resgates analgésicos em cadelas submetidas à mastectomia total unilateral, que a inexperiência do avaliador não compromete a qualidade das avaliações de dor e sugere-se reduzir a pontuação da EVA e Melbourne para aumentar a sua capacidade em detectar resgates analgésicos pós-operatórios.(AU)
The objective was to relate the need for analgesic postoperative recovery through Visual Analogue Scale (VAS), Glasgow, Colorado and Melbourne, by an experienced assessor (AE) and other non-experienced ones (ANE), in bitches undergoing total unilateral mastectomy. Otherwise healthy bitches, a total of 24, were admitted 24 hours before the surgical procedure for assessment of behavior with the help of the above scales to determine the baseline (M0) moment. They were pre-medicated with morphine and acepromazine (0,02 and 0,5 mg/kg) and general anesthesia was induced with propofol (4 mg/kg) and maintained with isoflurane at 1% MAC. The analgesic during surgery was maintained with ketamine and fentanyl (10 µg/kg/min and 10 µg/kg/h). The other evaluations were performed at 1, 2, 4, 6, 8, 12 and 24 hours postoperatively, and redemptions made with intramuscular morphine (0,5 mg/kg) when a greater than or equal score of 50, 6, 2 and 9 points was observed respectively for the described ranges, as observed by the AE and when at least two scales demonstrated these values. There was an increase of M1 to M12 pain scores for AE and the ANE for VAS. In Colorado analyses, the highest painful scores occurred in relation to M0 between M2 and M8 to the AE and M1 to M12 for ANE. In Glasgow analyses, higher scores were detected between M1 to M12 for AE and M1 to M24 for ANE. And for the Melbourne highest values were observed in M1 and M24 for AE and ANE. The best correlation between the scales was 0,775 between Glasgow and Colorado and of the evaluators of 0,925 to Glasgow. The Glasgow scale was shown to be more sensitive to detect painkiller redemptions in dogs undergoing total unilateral mastectomy, the inexperience of the appraiser does not compromise the quality of painful reviews, and it is suggested to reduce the score VAS and Melbourne to increase it is ability to detect rescue postoperative analgesics.(AU)

La monoparesia motora pura (MMP) es un déficit motor aislado en una extremidad, superior o inferior, producido por un evento vinculado a enfermedad cerebrovascular. Es un trastorno raro y podría confundirse con otras causas de déficit motor por lo que la diferenciación con otras etiologías se debe realizar a través de una adecuada evaluación clínica y de estudios de neuroimágenes. Se reporta el caso de una paciente de 56 años de edad con antecedente de hipertensión arterial, que debuta súbitamente con una monoparesia motora pura braquial izquierda. En la IRM cerebral se evidenció hiperintensidad en los protocolos de FLAIR y difusión e hipointensidad en el coeficiente de difusión aparente (ADC) en el territorio de arteria cerebral media derecha, segmento M4, correspondiente al área prerolándica en la distribución somatotópica del miembro superior. Se discuten diferentes aspectos etiológicos y de tratamiento de este cuadro. En pacientes que presenten déficit motor agudo de una extremidad, con historia de factores de riesgo vascular, debe considerarse activamente la posibilidad diagnóstica de MMP.
The pure motor monoparesis (PMM) is an isolated motor deficit in an upper or lower extremity usually related to a cerebro-vascular disease. It is a rare condition that can be easily confounded with other pathologies, reason for which should be carefully differentiated through clinical assessment and neuroimaging studies. We report the case of a 56 year-old woman who suddenly presented pure left brachial monoparesis as a manifestation of a right middle cerebral artery stroke. Brain MRI showed a hyperintensity in FLAIR and diffusion protocols, and a subintensity in the apparent diffusion coefficient (ADC) in the territory of M4 segment of the right middle cerebral artery, corresponding to the prerolandic area in the somatotopical distribution of the upper limb. Different etiologies of an management strategies for this condition are discussed. The diagnosis of PMM due to ischemic stroke should be considered in patients presenting an acute motor deficit in one extremity, braquial or crural, and with history of vascular risk factors.

Moyamoya disease is characterized by progressive stenosis of the internal carotid artery and its main branches. The cause of the disease is unknown, ischemic or hemorrhagic stroke are the main manifestations (the former is more common) that disproportionately affect children and young adults, and is consider a diagnostic challenge. Aims: Evaluation of leaves methanol extracts from Plectranthus barbatus and Plectranthus ecklonii (Lamiaceae) against oral pathogens. Place and Duration of the Study: P. barbatus and P. ecklonii, cultivated in Botanic Garden of the University of Lisbon, were collected during winter 2009. Methodology: Methanol extracts were prepared and the compounds separated and identified by HPLC-DAD and mass spectrometry. The anticariogenic activity was determined by measuring the inhibition activity towards the growth of the pathogens Streptococcus mutans and S. sobrinus together with the inhibitory activity against the enzyme glucosyltransferase (GTF) involved in the biosynthesis of glucans. Results: Phytochemical analysis of the extracts revealed the presence of two abietane diterpenoids in P. barbatus and two quinone methides together with rosmarinic acid in P. ecklonii. The two Plectranthus extracts showed bacteriostatic activity with minimum inhibitory concentrations (MIC) of 0.3 mg/mL. The minimum bactericidal concentrations (MBC) obtained for both extracts were 0.6 mg/mL against S. sobrinus and 0.8 mg/mL against S. mutans. After exposing both strains during 2h to P. ecklonii extract, 80% of inhibition against viable cells on a 24h old biofilm was observed. When methanol extracts of P. barbatus and P. ecklonii were used to inhibit the growth of the two bacterial strains in biofilm, IC50 (inhibitory concentration) values were 1.9 mg/mL and 0.57 mg/mL against S. sobrinus biofilm and 0.7 mg/mL and 0.8 mg/mL against S. mutans biofilm, respectively. P. barbatus IC50 values for the biofilm formation were 0.63 mg/mL and 0.13 mg/mL against S. sobrinus and S. mutans, respectively. P. ecklonii IC50 values for the biofilm formation were 0.07 mg/mL and 0.12 mg/mL against S. sobrinus and S. mutans biofilm. GTF from S. sobrinus was inhibited in 30% when 0.3 mg/mL of P. barbatus extract was used. Conclusions: These extracts are important in the control of biofilms and useful in the prevention of oral diseases.

Objetivos: Explorar los factores asociados a mortalidad en pacientes con infarto cerebral que fueron hospitalizados en el Instituto Nacional de Ciencias Neurológicas. Material y Métodos: Se revisaron las historias clínicas de pacientes con diagnóstico de infarto cerebral hospitalizados en el Instituto Nacional de Ciencias Neurológicas, durante Enero del 2008 hasta Diciembre del 2009. Se describen las variables demográficas y clínicas de los pacientes, además se utilizó el modelo de regresión logística para explorar los factores asociados a mortalidad en los pacientes. Resultados: Se analizaron 461 historias clínicas de pacientes con infarto cerebral. El promedio de edad fue de 67 años, el 56% fueron hombres. La mortalidad fue del 7,6%, IC 95% (5,3-10,4); los factores asociados a mortalidad que tuvieron significancia estadística fueron: el déficit sensitivo OR=2,7, cefalea OR=2,75, trastorno de conciencia OR=12, escala NIHSS al ingreso OR=1,25 y el territorio vascular anterior OR=2,65. Conclusiones: en éste estudio exploratorio los factores asociados a mortalidad intrahospitalaria por infarto cerebral fueron la presencia de déficit sensitivo, cefalea, pérdida de conciencia, déficit neurológico severo y territorio vascular de la circulación anterior.
Objectives: To explore associated factors to mortality of patients with cerebral infarction. Methods: We reviewed clinical records of patients with diagnosis of cerebral infarction hospitalized at the Instituto Nacional de Ciencias Neurologicas, January 2008 to December 2010. We describe the demographic and clinical variables. In addition, the logistic regression model was used to determine the factors associated with mortality in these patients. Results: We reviewed clinical records of 461 patients with brain infarction. The median age was 67 years and 56% were men. We calculated a ratio of 7.6% dead, 95% IC (5.3-10.4); the factors significantly associated are the sensitive deficit OR = 2.7, headache OR = 2.75, consciousness disorder OR = 12, NIHSS scale OR = 1.25 and anterior vascular territory OR = 2.65. Conclusions: in this exploratory study factors associated with mortality by cerebral infarction were the presence of sensitive deficit, headache, loss of consciousness, severe neurological deficit and vascular territory of the anterior circulation.

A realização deste trabalho teve como objetivo avaliar a eficácia do fipronil em Rhipicephalus (Boophilus) microplus, durante e após 14 tratamentos, no período de dezembro de 2006 a abril de 2009. O experimento foi realizado em uma propriedade rural do município de Lages, SC, utilizando-se 20 bovinos mestiços charolês mantidos em campo nativo, naturalmente infestados com R. (B.) microplus. A cada 14 dias, foram realizadas contagens das fêmeas de carrapato, maior ou igual a 4,5mm. Os animais foram tratados com fipronil 1mg/kg via pour on, quando a média do número de carrapatos foi igual ou superior a 40 fêmeas. Ao final do experimento, para análise da eficácia do fipronil 1%, foi realizado o teste de estábulo, utilizando 10 animais infestados com larvas provenientes de teleóginas coletadas de bovinos da propriedade. Os animais foram randomizados, de acordo com a produção inicial de teleóginas, estabelecendo-se dois grupos: controle (n=5) e tratado (n=5). A redução da média do número de fêmeas de R. (B.) microplus nas contagens após tratamentos foi de 100% nos três primeiros, com um leve declínio até o sexto tratamento. No sétimo tratamento a redução foi de 91,3%. Ao final do experimento a eficácia do fipronil 1%, avaliada pelo teste de estábulo, na redução do número de teleóginas, foi de 79,3%. A porcentagem média de inibição de reprodução calculada foi de 22,47%. Concluiu-se que, após seis tratamentos com fipronil, média de um a cada 2,7 meses, o princípio ativo deve ser substituído e que, após 14 tratamentos, o R. (B.) microplus adquiriu resistência parcial a esse carrapaticida.
This study aimed to evaluate the effectiveness of fipronil in Rhipicephalus (Boophilus) microplus during and after 14 treatments from December 2006 to April 2009. The experiment was performed on a rural property in the city of Lages/SC, using 20 crossbred Charolais cattle maintained on open pasture naturally infected with R. (B.) microplus. Every 14 days, tick counts of the females greater than or equal to 4.5mm were conducted. When tick counts averaged 40 females or more the animals were treated with applications of a 1% applied to the dorsal formulation of fipronil 1mg/kg. At the end of the experiment, barn testing was performed on 10 animals infested with larvae of the Engorging females collected from the cattle on the property, for analysis of the effectiveness of 1% fipronil. The animals were randomized according to the initial production of Engorging females, establishing five animals in the control group and five in the treated group. The reduction in the average number of females of R. (B.) microplus was 100% during the first three treatments, with a slight decline up to the sixth treatment. On the seventh treatment the reduction was 91.3%. At the end of the experiment the effectiveness of 1% fipronil for reducing the number of ticks was 79.3%, assessed by the barn testing. The average percentage of inhibition of reproduction was calculated at 22.47%. It was concluded that after six treatments with fipronil, with an average of one treatment every 2.7 months, there must be a change in the active ingredient and that after 14 treatments R. (B.) microplus acquired partial resistance to this acaricide.
