ABSTRACT
We report two males and one female, aged 71, 81 and 73 years old respectively, previously operated of a renal cell carcinoma. During the follow up of the 71 years old male, the CAT scan showed three lesions in the pancreas. Also, the CAT scan in the female showed one lesion in the tail of the pancreas. The 81 years old male was admitted to the emergency room due to a upper gastrointestinal bleeding. Endoscopy showed a proliferating lesion in the second portion of the duodenum whose biopsy showed a clear cell carcinoma. The CAT scan showed also a tumoral mass in the head of the pancreas. All three patients were subjected to surgical resection of the tumors without postoperative complications or mortality

We report a man in whom a 15 cm. renal tumor was excised at the age of 49. The pathological examination showed a clear cell carcinoma. Five years later, he presented with headache, vomiting and unilateral palpebral ptosis. Imaging studies showed a sellar tumor with pituitary apoplexy. The tumor was excised and the pathological study disclosed a clear cell tumor, positive for vimentin, cytokeratins AE1 and AE3 and immunohistochemically negative for LH, TSH, ACTH and GH. Considering the similar histopathological features, it was considered as a metastasis of the renal tumor. The patient was supplemented with thyroid, adrenal and gonadal hormones. Seven years later, he presented a new tumor in the remaining kidney, that corresponded to a cystic papillary renal cell carcinoma. Afterwards, he presented a transitional urinary bladder tumor. Mortality associated to renal cell tumors is 90 percent at 5 years, and pituitary metastases are extraordinarily uncommon

Se presenta el caso de un recién nacido de término, sexo femenino, de 3.380 g de peso al nacer, producto de un embarazo fisiológico de madre sana, que evolucionó desde el primer minuto de vida con insuficiencia respiratoria y respuesta parcial a ventilación con presión positiva y oxígeno. Una radiografía de tórax mostró sombras pulmonares difusas sin cardiomegalia, sospechándose un drenaje venoso pulmonar anómalo total obstructivo, siendo descartado por ecocardiografía. Los gases arteriales demostraron hipoxemia y acidosis metabólica; las radiografías posteriores mostraron hiperinsuflación pulmonar con sombras reticulonodulares difusas. Se planteó el diagnóstico de linfangiectasia pulmonar congénita y se mantuvo el tratamiento para hipertensión pulmonar con ventilación mecánica, drogas vasoactivas y bicarbonato endovenoso, pese a lo cual presentó deterioro hemodinámico progresivo, falleciendo a las 13 horas de vida. La autopsia confirmó el diagnóstico de linfangiectasia pulmonar congénita. Se presenta el caso clínico con sus hallazgos radiológicos, anatomopatológicos y una breve revisión del tema

In 179 patients subjected to 186 renal tranplants, 30 renal biopsies were performed due to the presence of a proteinuria over 3.5 g/24 h or due to a reduction in glomerular filtration rate. Six of these biopsies, coming from 5 patients, disclosed morphological alterations compatible with focal segmental glomerulosclerosis. Five of these were due to recurrence of the primary disease (in 4 patients) and in all, massive proteinuria appeared from 1 to 23 days after transplantation. Two patients with three transplants evolved to renal failure and required dialysis in a period 12 months as a mean. The third patient, developed a nephrotic syndrome without renal failure and died 14 months after the renal transplant due to stroke. In the fourth patient, the nephrotic syndrome disappeared 38 days after the transplant and remained with minimal proteinuria until his last follow up visit two years later. The primary disease of the fifth patient is unknown, the nephrotic syndrome appeared 68 months after the transplant and remitted spontaneously in 2 months. The renal biopsy showed focal and segmental lesions with partial effacement of epithelial foot processes. It is concluded that focal segmental glomerulosclerosis recurrence in renal transplant occurs with aerly massive proteinuria and frequently leads to renal failure and graft loss in no more than two years

Se investigó el efecto del enalapril en la gestación en ratas. A un grupo de 11 ratas se les administró 15 mg/kg/día desde el día primeroal noveno de gestación (E10-20); 12 ratas sirvieron de control (C); 15 animales fueron sacrificados al día 20 y 19 continuaron hasta el parto. Las placentas fueron 15% menores en el grupo E10-20 (p<0.05) y tenían una estructura cordonal hipocelular simple. En el grupo E1-9 el tipo estructural predominante fue una combinación de estructuras simple y compleja. En el día 20 de gestación los fetos de los grupos tratados eran de menor tamaño que los controles (-5% en E1-9 y -16% en E10-20, p<0.05). Las diferencias desaparecieron a los 13 días postparto. En dos fetos tratados se observó calcificación incompleta del cráneo. Estimamos que estos resultados, analizados en el contexto de los daños observados en el embarazo humano, agregan información que fundamenta la abstención del uso de inhibidores de enzima convertidora en el embarazo

En 36 pacientes con Ca vesical se usó BCG en instalación local. Se evaluó en 18 de ellos aspectos de respuesta clínica y morfológica frente a dosis variable decreciente de BCG (50 mg a 2 mg). Se usó cepa Pasteur con alta viabilidad. La respuesta anátomo-clínica encontrada fue independiente de la dosis empleada. La presencia de inflamación vesical granulomatosa post tratamiento coincidió en mayor frecuencia con la desaparición persistente del tumor, no así la inflamación vesical inespecífica que tuvo más recurrencia

Se presenta un estudio retrospectivo de 24 pacientes que desarrollan enfermedad tubular aguda postrasplante renal donante cadáver. Doce enfermos presentaron una crisis de rechazo agudo en este período. El seguimiento clínico riguroso, el diagnóstico y tratamiento precoz de esta complicación permiten preservar el injerto en todos los casos
