ABSTRACT

Purpose@#To report a case of exudative retinal detachment after using pembrolizumab in a patient with metastatic cutaneous melanoma.Case summary: A 67-year-old woman, diagnosed with malignant melanoma of the right thumb and axillary metastasis, presented with bilateral visual disturbance 3 days after adjuvant chemotherapy with pembrolizumab. Her best corrected visual acuity was 0.2 in the right eye and 0.7 in the left, while the intraocular pressure was 14 mmHg in both eyes. Fundus examination showed serous retinal detachment and choroidal detachment in the right eye, as well as a chorioretinal folding in both eyes. Optical coherence tomography showed exudative retinal detachment and choroidal detachment in the right eye, along with choroidal folding in both eyes. The pembrolizumab was stopped immediately, and the patient began treatment with systemic and topical steroids. After 1 month, the visual acuity improved and there was no exudative retinal detachment or choroidal detachment. However, 3 weeks later, exudative retinal detachment recurred in both eyes. The patient started treatment with oral steroids and cyclosporine, which resulted in the resolution of the exudative retinal detachment after 1 month. @*Conclusions@#Exudative retinal detachment may occur as a side effect of pembrolizumab treatment. Therefore, a differential diagnosis and appropriate treatment of ocular side effects are necessary.

ABSTRACT

Chronic neutrophilic leukemia is a rare myeloproliferative disorder. We have experienced a typical case of chronic neutrophilic leukemia in a 76-year-old man who complained abdominal distension due to hepatosplenomegaly. White blood cell count of peripheral blood was 50,500/nL with 90% segmented neutrophils. The underlying disease for a leukemoid reaction had not been detected. Leukocyte alkaline phosphatase score and the serum levels of vitamin B12 and uric acid were elevated. Chromosome study showed a normal karyotype without Philadelphia chromosome or bcr/abl rearrangement. Phorbol myristate acetate-activated respiratory burst activity of neutrophils measured with chemiluminescence showed increased activity.

ABSTRACT

The occurrence of disseminated tuberculosis in combination with acute leukemia is rare. A 28 year old male undergoing induction chemotherapy for AML presented with fever of unknown origin. Upon the studies to make the diagnosis this case turned out to be disseminated tuberculosis including meningitis. The chest CT showed multiple enlarged mediastinal lymphadenopathy. The Brain CT showed noncommunicating obstructive hydrocephalus. Disseminated tuberculosis was pathologically proven by theliver, bone marrow and mediastinal lymph node biopsies. As clinical course improved, radiological lesions were completely resolved after antituberculosis therapy. It is important to consider disseminated tuberculosis for the etiology of FUO inpatient with AML who had suffered from long standing fever.

ABSTRACT

BACKGROUND: Social support is defined as interpersonal transaction ; the giving of symbolic or material aid to another. It has been shown to have both buffering effects and direct beneficial effects on diverse health related outcomes. But there has been the dearth of well-validated measurement instruments. This article describes the development of Korean version of the Duke-UNC functional social support questionnaire(DUFSS), focusing on the aspect of social support. METHODS: The 13-item, self-administered questionnaire was translated to Korean language. It was evaluated on 177 patients attending the family medicine clinic of Seoul National University Hospital during one month(November, 1996.) Comparisons to the family APGAR, COOP/ WONCA functional status chart are made to assess validity. Factor analyses are performed. Correlation of the questionnire score with the sum of family APGAR and each items of COOP/ WONCA functional chart are measured Internal consistency are evaluated. RESULTS: The questionnaire score are correlated with significantly correlated with social activity item. The total Cronbachs alpha is calculated as 0.89. The four content areas developed by factor analyses are donfidant support(items 8, 9, 10), af fective support(items 4, 5, 6, 7), instrumental support(items 3, 12) and quantity of support(item 1). These shows similarity to American study results. CONCLUSIONS: The Korean version of the Duke-UNC functional social support questionnaire has been documented as having high reliability and moderate validity. It covers two dimensions well(confidant support, affective support) with a small enough number of questions. Instrumental support may need further study.
