ABSTRACT

High-quality hospital development calls for high-quality hospital culture.Hospital culture is the core of hospi-tal management,which deeply affects the development direction and operation mode of the hospital,throughout the whole process of high-quality development of the hospital,but also an indispensable component of social culture.This paper discusses and con-siders the definition and value of hospital culture,the dilemma of the ideological level and management level of hospital culture construction,and countermeasures,which provides a reference for hospital management and promotes the high-quality develop-ment of hospitals.

ABSTRACT

OBJECTIVE To assess the treatment reliability of covered stent for carotid artery blowout after head and neck tumors resection. METHODS Five cases with postoperative rupture of carotid artery invaded by the head and neck tumor were reviewed. They presented with life threatening massive neck or oral bleedings. All of them were treated with self expanding covered stents through intervention therapy approach. RESULTS The covered stent were successfully deployed in the target arteries in all cases,the instant isolation effect was quite satisfactory. After treatment, angiography showed successful occlusion of the pseudoaneurysm, patency of carotid artery lumen, and significant improvement of clinical symptoms without neurologic dysfunction. Following up 2 to 36 months, 3 patients were alive with no disease, two patients died of recurrence. CONCLUSION For the treatment of carotid blowout, endovascular occlusion with covered stent is a minimally-invasive, safe and reliable methods.

ABSTRACT

Objective To study changes of serum IL-17 and IL-35 levels in patients with heart failure. Methods 60 patients with heart failure (observation group)were selected as research subjects.60 patients accord-ing to different severity were divided into acute period heart failure (34 cases)and stable stage heart failure (26 ca-ses);60 patients graded according to the NYHA standards were divided into 24 cases of heart failure with grade Ⅱ, 20 cases of grade Ⅲ ,16 cases of grade Ⅳ.According to the different primary diseases :expansion cardiomyopathy group (20 cases in group A),the coronary heart disease group (group B,24 cases),hypertensive heart disease group (group C,16 cases).During the same period,42 healthy elderly people in our hospital were selected as control group. The serum IL-17,IL-35 levels were tested,and the serum IL-17,IL-35 levels in patients with heart failure were ana-lyzed.Results Serum level of IL-17 in the observation group was higher than the control group,and the difference was significant [(15.61 ±4.02)pg/mL vs (9.49 ±3.96)pg/mL,t =9.018,P 0.05). The serum IL-35 level of A group was lower than that of B group and C group,there were significant differences (F =8.978,P 0.05).Serum IL-17 and serum IL-35 level was negatively correlated (r =-0.429,P =0.009).Conclusion High expression of IL-17 in elderly patients with heart failure,while IL-35 decreased in elderly patients with heart failure, IL-17,IL-35 are closely related to the senile congestive heart failure and the severity of illness.Serum IL-17 is nega-tively correlated with the level of serum IL-35.

ABSTRACT

Objective To establish a rabbit posttraumatic acute diffuse brain swelling (PADBS) model and investigate the mechanism of action.Methods Fifty New Zealand rabbits were assigned to control group (n =10) and model group (n =40) according to random number table.The animal model of sinus balloon compression was established under intracranial pressure monitoring by using intracranial pressure probe.The model group was subdivided equally at 1.5 hours after compression,1.5 hours after decompression,3 hours after decompression and 4.5 hours after decompression,for which intracranial pressure,brain water content,pathological mechanism and ultrastructure were measured dynamically.Results The success rate of modeling was 83% (33/40).Intracranial pressure was (4.9 ± 0.8)mmHg in control group,(50.1 ± 4.3) mmHg in 1.5 hours after compression group,(45.2 ± 1.7) mmHg in 1.5 hours after decompression group,(48.6 ± 2.2) mmHg in 3 hours after decompression group,and (59.1 ±2.5)mmHg in 4.5 hours after decompression group (P <0.05).Brain water content was (75.0 ± 0.6) % in control gorup,(76.7 ± 0.8) % in 1.5 hours after compression group,(77.3 ± 0.5) % in 1.5 hours after decompression group,(78.5 ± 0.6) % in 3 hours after decompression group,and (79.4 ± 0.7) % in 4.5 hours after decompression group (P < 0.05).Vasogenic brain oedema was seen 1.5 hours after decompression.Cytotoxicity brain swelling generated with brain tissue destroyed 3 hours after decompression.The vicious cycle of high intracranial pressure and brain tissue destruction occurred 4.5 hours after decompression.Conclusion Under intracranial pressure probe monitoring,the rabbit model of PADBS by sinus balloon compression has stable pressure of the sinus balloon and has good reliability and repeatability,which provides a reliable evidence for further study on the possible mechanism and treatment methods of PADBS.

ABSTRACT

Objective To introduce the application and assess the outcome of ProPlan CMF software in virtual surgery of mandibular resection and reconstruction with vascularized fibular graft.Methods Nineteen patients were performed mandibular resection,and immediate reconstruction with free fibular graft by application of ProPlan CMF software between March 1,2013 and January 31,2014,including 7 cases of osteoradionecrosis,6 cases of adamantoblastoma,4 cases of carcinoma of gingiva,2 cases of intra-osseous carcinoma of mandible.There were 12 males and 7 females,with an age range of 21-73 years (median,54 years).In each case,three-dimensional spiral CT scan of mandible and fibula was obtained before operation.The CT data were imported into the ProPlan CMF software and the virtual surgical planning was performed.After that,the mandibular rapid prototyping and surgical guide plates were made according to customized design.The segmental resection of mandible,titanium plate bending,fibular molding,insetting and fixing were then carried out by using these surgical guide plates.Results Operations were undertaken successfully according to preoperative design in all nineteen patients and negative margin was detected by intraoperative frozen section.The fibula was cut,shaped and fixed accurately.The operations took 4.5-7.0 hours (mean,5.5 hours).Primary healing of incision was obtained without complications except fistula in 2 cases of osteoradionecrosis,which were resolved with local wound care after 4 weeks.19 patients were followed up between 3 months to 1 year and all patients were alive with disease free.All patients were satisfactory with their maxillofacial appearance,occlusion and function.The ranges of mouth opening was 2.0-4.0 cm.The postoperative panoramic radiographs or CT scan showed perfect situation and contour of fibular graft.Healing of the donor site occurred without significant complications.All patients started early postoperative ambulation,and walked normally after 2 or 3 months.Conclusion The application of ProPlan CMF software in virtual surgical design and guide plates creation can simplify the operation,and improve the accuracy of mandibular resection and reconstruction,especially the spatial relationship of the mandible and the fibula graft.It is worth promotion widely in clinical practice.

ABSTRACT

Objective To assess the reliability and useness of using a coupling device for end-to-end venous anastomosis in patients undergoing free-tissue transfer in head and neck reconstruction.Methods Twelve patients undergoing surgical resection and head and neck defects were repaired with fibular flap,anterolateral thigh flap and radial forearm flap.The microvascular anastomotic device was used to perform venous anastomosis in these cases.Flap survival and thrombosis of the venous anastomoses were determined.Results There were no flap losses due to venous thrombosis in this series.The venous anastomosis was usually coupled in about 5.3 minutes and appear as an obviously time savings compared with performing a hand-sewn venous anastomosis.Conclusion The microvascular coupler is excellent in end-to-end venous anastomosis and certainly less taxing on the surgeon when compared with standard suture techniques.

ABSTRACT

Ezrin is an important membrane-cytoskeleton linker protein.It is highly related with tumor invasion and metastasis and indicates poor prognosis.It is reported that aberrant Ezrin expressed in many carcinomas,such as osteosarcoma and breast carcinoma.However,the complex mechanisms of Ezrin in tumor invasion and metastasis remain unclear.It is involved in several different tumor associated signal pathway based on different tumor types,including adhesion molecule signal transduction,Rho and Akt signal transduction.Therefore,research on the signal transduction of Ezrin has great significance for the understanding of cancer progression and Ezrin is probable to be a new treatment target.

ABSTRACT

<p><b>OBJECTIVE</b>To demonstrate the feasibility of Ni-Ti shape memory alloy mesh stent for the correction of nasal deformities caused by cleft lip.</p><p><b>METHODS</b>Six patients with cleft lip-nose deformities, aged from 18 to 32, were treated by implantation of Ni-Ti shape memory alloy mesh stent through intra-nasal incisions.</p><p><b>RESULTS</b>The symmetry of the alar and nasal tip were cosmetically acceptable. Satisfactory relationship between the lip and the nose were obtained in all the patients. There were not any postoperative surgical complications by 6 to 12 months' follow-up.</p><p><b>CONCLUSION</b>The Ni-Ti shape memory alloy mesh stent is suitable for use in the correction of adult cleft-lip nose deformities.</p>

ABSTRACT

Objective To study the blood_flow_activating and blood_stasis_removi ng actions of Qushang Tablet (QT) and its safety. Methods Thirty rats were randomly allocated to Group A (normal control), Group B (models control) and G r oup C (treated with QT). Rat models with traumatic blood_stasis syndrome were in duc ed by the bruise of falling iron hammer. Effects of QT on blood rheology of rat model were studied. Acute and chronic toxic effect of QT was also studied. Res ults QT had obvious effect on blood rheology of rat model and could improve th e concentration, viscosity, aggregation and coagulability of blood. No acute and chronic toxic effect of QT was found. Conclusion QT exerts obvious blood_fl ow_activating and blood_stasis_removing actions and is safe to be applied in cli nic.
