ABSTRACT
ObjectiveTo systematically evaluate the distribution of traditional Chinese medicine(TCM) syndromes of primary osteoporosis(POP) in China by using evidence-based medicine methods, and to understand the distribution law of the syndromes. MethodChina National Knowledge Infrastructure(CNKI), VIP Chinese Science and Technology Journal Database(VIP), WanFang Data Knowledge Service Platform(WanFang) and China Biology Medicine(CBM) were searched to obtain representative literature, and each database was searched from the 1994 World Health Organization defined diagnostic criteria for osteoporosis until May 1, 2023. Two researchers independently screened literature according to the criteria, extracted data, and cross-checked them. Meta analysis was conducted using R4.1.3, and subgroup analysis was performed. ResultA total of 56 Chinese papers were included, involving 14 415 patients. After standardized classification of syndromes, 11 articles were excluded, and Meta analysis results of the ultimately included 45 Chinese articles showed that the distribution frequencies of liver-kidney Yin deficiency syndrome, spleen-kidney Yang deficiency syndrome and kidney deficiency and blood stasis syndrome in 12 723 patients were 27%[95% confidence interval(CI) 0.24-0.31], 32%(95% CI 0.29-0.36), 36%(95% CI 0.30-0.42). Subgroup analysis showed that there was a statistically significant difference in the distribution of the three TCM syndromes in the north and south(P<0.05). In addition, the incidence of TCM syndrome fractures in different types of POP was 15%(95% CI 0.09-0.24) for liver-kidney Yin deficiency syndrome, 20%(95% CI 0.12-0.30) for spleen-kidney Yang deficiency syndrome, and 31%(95% CI 0.25-0.39) for kidney deficiency and blood stasis syndrome. ConclusionThe distribution of syndromes in POP patients is mainly kidney deficiency, accompanied by liver and spleen dysfunction. Liver-kidney Yin deficiency syndrome, spleen-kidney Yang deficiency syndrome and kidney deficiency and blood stasis syndrome are all the main syndromes of POP and osteoporotic fractures, and kidney deficiency and blood stasis syndrome is most closely related to the development of osteoporotic fractures. The reference standards for syndrome determination among the included studies are inconsistent, and in the future, it is necessary to focus on their determination standards to obtain consensus research results, at the same time, conduct large-scale syndrome research to obtain representative research results, providing a basis for clinical practice and research.

Osteoporosis (OP) and osteoarthritis (OA) are common bone diseases in clinic. OP is a systemic skeletal disease, and OA is a chronic degenerative joint disease with high prevalence and disability rates. With the advent of the aging population, the incidence rate of OA and OP is increasing year by year, and they have become common diseases of the elderly. The quality of life and physical and mental health of patients are severely affected by the above two bone diseases. Chinese medicine has a long history of treating bone diseases, with a good clinical effect on preventing and treating OP, OA, and other bone diseases with few side effects. It is one of the commonly used methods to treat bone diseases. Polysaccharides, as one of the active substances of Chinese medicine, have various pharmacological activities and a wide range of sources with low toxicity, and their effect cannot be ignored. The role of polysaccharides in the treatment of bone diseases has been deeply studied. It has been found that the mechanism of Chinese medicine polysaccharides in treating OP and OA involves multiple levels, targets, and pathways. Through the analysis and summary of the relevant literature on the mechanism of Chinese medicine polysaccharides in treating OP and OA, it was found that Chinese medicine polysaccharides mainly treated OP by regulating the bone dynamic balance between osteoblasts and osteoclasts and affecting bone marrow mesenchymal stem cells and bone microstructure. The mechanism of Chinese medicine polysaccharides in the treatment of OA is related to the regulation of chondrocyte growth, the increase in the proteoglycan and collagen content in the cartilage matrix, and the reduction of oxygen free radical content and inflammatory mediator level. This study aimed to further explore the internal relationship among mechanisms of Chinese medicine polysaccharides in the treatment of bone diseases, to provide relevant ideas for the study of Chinese medicine polysaccharides in the treatment of bone diseases.
ABSTRACT
OBJECTIVE To optimize the e xtraction technology of Guizhi shaoyao zhimu decoction (GSZD). METHODS The contents of 9 components in GSZD were determined by HPLC ,such as ephedrine hydrochloride ,pseudoephedrine hydrochloride , mangiferin,paeoniflorin,liquiritin,5-O-methylvisammioside,glycyrrhizic acid ,cinnamic acid ,6-gingerol. On the basis of single factor experiment ,taking material-liquid ratio ,extraction times and extraction time as inspection factors ,taking the contents of above 9 components and the yield of dry extract as evaluation indicators ,the analytic hierarchy process and entropy weight method were used to determine the composite weight of each index and calculate the comprehensive score ;the extraction technology parameters of GSZD were optimized by Box-Behnken response surface method ,and the validation tests were conducted. RESULTS The composite weight of the contents of ephedrine hydrochloride ,pseudoephedrine hydrochloride ,mangiferin,paeoniflorin, glycyrrhizin,5-O-methylvisa- midol ,glycyrrhizinate,cinnamic acid ,6-gingerol and the yield of dry extract were respectively 0.12,0.10,0.05,0.12,0.14,0.06,0.13,0.15,0.10,0.03. The optimal extraction technology of GSZD is that the ratio of material to liquid is 1 ∶ 14(g/mL),extraction is 2 times,and the extraction time is 3.0 h;average comprehensive score of the 3 verification tests was 95.879,and RSD was 0.50%(n=3),the deviation from the predicted comprehensive score (94.328)was 1.64%. CONCLUSIONS In this study ,the optimal extraction technology of GSZD is determined.
ABSTRACT
Objective:To investigate the effect of pilose antler polypeptide combined with Schwann cells modified by glial cell line-derived neurotrophic factor (GDNF)gene on the proliferation of human bone marrow mesenchymal stem cells (BMSCs)in vitro .Methods:According to the conventional method,the bone marrow (10 mL)was extracted from the healthy volunteers and was inoculated into the culture flask.The primary cultured cells were completely fused.The BMSCs were harvested at the 3rd generation and the cells were adjusted to 5 × 106 mL-1 . 4 μL GDNF gene modified Schwann cells was added into GDNF group,4 μL (10 mg· L-1 )PAP combined with GDNF gene modified Schwann cells was added into combination group,and only same amount of medium was added into control group.The proliferative activities,cell nuclear antigen (PCNA)levels and apoptotic rates of BMSCs in various groups were detected by enzyme-linked immunosorbent assay,ELISA method and Annexin Ⅴ-FIFC/PI cell apoptosis detection kit,respectively.Results:After primary culture for 48 h,most of the cells adhered to the wall,and the morphology of the cells changed into polygonal shape and few of them showed spindle.The passaged cells showed spindle spindle,the cells were confirmed as BMSCs,and all of them were non-hematopoietic stem cells.Compared with control group,the proliferative activities and the PCNA level of the BMSCs in GDNF group and combination group were increased (P <0.05)and the apoptotic rates were decreased (P <0.05);compared with GDNF group,the proliferative activity and the PCNA level of the BMSCs in combination group were increased (P < 0.05 )and the apoptotic rate was decreased (P < 0.05 ).Conclusion:PAP combined with Schwann cells modified by GDNF gene can promote the proliferation of human BMSCs in vitro .
