ABSTRACT

BACKGROUND@#It is a great challenge for surgeons to resect pulmonary nodules with small volume, deep position and no solid components under video-assisted thoracoscopic surgery. The purpose of this study is to explore the feasibility and necessity of the localization of pulmonary nodules by injecting indocyanine green (ICG) under the guidance of magnetic navigation bronchoscope and the resection of small pulmonary nodules under the fluoroscope.@*METHODS@#Between December 2018 and August 2019, sixteen consecutive patients with 30 peripheral lung lesions in our hospital received fluorescent thoracoscopic pulmonary nodule resection. Electromagnetic navigation bronchoscope (ENB) was performed before surgery to guide ICG to the target lesion.@*RESULTS@#All patients underwent magnetic navigation-guided pulmonary nodule localization, and surgical resection was performed immediately after localization was completed. The average size of the nodules was (11.12±3.65) mm. The average navigation time was (12.06±2.74) minutes, and the average interval between dye labeling and lung resection was (25.00±5.29) minutes. All lesions were completely resected, the localization success rate was 100.00%, no bleeding and other complications occurred after the localization, the postoperative pathological results confirmed the accuracy of the staining.@*CONCLUSIONS@#Indocyanine green injection under the guidance of magnetic navigation bronchoscope is an effective way to locate pulmonary nodules, which can locate small and untouchable lesions in the lung. This method can help surgeons identify lesions more quickly and accurately. It is practical and worthy of promotion.

ABSTRACT

Objective@#To observe the effect of clean water perineum nursing in preventing urinary catheter-associated urinary tract infection.@*Methods@#From October 2015 to October 2016, 160 patients with indwelling urinary catheter were selected from the first affiliated Hospital of Shantou University Medical College, ICU and neurosurgery ICU. According to the order of admission, the patients were randomly divided into experimental group and control group. A total of 80 patients in the experimental group received clean water perineal nursing, while 80 patients in the control group received routine perineum nursing with 0.5% iodophor. The incidence of urinary tract infection was compared between the two groups on day 3rd, 7th and14th after indwelling urethral catheter.@*Results@#In the control group, the incidence of urinary tract infection on day 3rd, 7th and14th was 0, 1.3%(1/80) and 5.0%(4/80), respectively. In the experimental group, the incidence of urinary tract infection on day 3, 7 and 14 was 0, 2.5% (2/80) and 3.8%(3/80). The difference was not statistically significant (χ2=0.340, 0.149, P> 0.05).@*Conclusions@#Using clean water perineum nursing does not increase the incidence of urinary tract infection. This method is simple, effective, and has no adverse reactions. It is worth popularizing in clinic because it can reduce the consumption of iodophor disinfectant and reduce the workload of nurses.

ABSTRACT

Objective To study the clinical, laboratorial, histopathological, imaging features of two cases of hypothyroid myopathy. Method Clinical manifestations, thyroid function, electromyography, muscle MRI, muscle biopsy and follow-up results were collected, and analyzed with the literature. Result These two patients were middle-age to old age and the onset of disease was insidious. Their common clinical manifestations included subacute progressive weakness in the proximal muscles,myalgia after sports and reduction in tendon reflex.The blood test showed an increase in serum concentration of CK and TSH, and a decrease in FT3 and FT4. The electromyography showed suspicious myogenic damage.Muscle histopathological findings were largely nonspecific,such as type I fiber predominance and type 2 atrophy. The MRI revealed extensive muscular dystrophy and fatty filtration in the posterior group of thighs. Treatment of replacement therapy with L-T4 relieved the myopathic symptoms quickly. Conclusion When a patient presents with a subacute progressive weakness in the proximal muscles, the hypothyroidism should be consideration. Muscle histopathological findings may be nonspecific. The muscle MRI have a value of differential diagnosis and lesion assessment.

ABSTRACT

Objective To study the clinical, pathological, imaging features of two cases of central core disease (CCD) with different inheritance and to explore the similarities and differences between autosomal recessive CCD (AR-CCD) and autosomal dominant CCD (AD-CCD). Methods Clinical manifestations, family history, muscle MRI and muscle biopsy were collected. Targeted next generation sequencing (NGS) and sanger sequencing were applied for genetic analysis. Co-segregation analysis was further conducted in one family. Results Their common clinical manifestations included childhood early-onset proximal limbs muscle weakness and dystrophy accompanied with facial involvement. The MRI revealed extensive muscular dystrophy and fatty filtration in the both thighs, but not in rectus femoris. Pathology of skeletal muscle showed typical central cores in type Ⅰ muscle fibers and eccentric cores only in AR-CCD. Targeted NGS identified 3 missense mutations in RYR1, including one novel mutation. Conclusion The present study has described clinical and pathological features of two typical CCD patients with different inheritance, which may be associated with the different mutations in RYR1 gene. Targeted NGS apparently improves the genetic diagnosis of CCD.

ABSTRACT

Objective To investigate the expression of engulfment and cell motility protein 1 (ELMO1) in hepatocellular carcinoma(HCC) and its effects on cell migration.Methods The expression of ELMO1 in HCC cell lines and HCC tissues was assessed by Western blot.We used transient transfection with an ELMO1 expressing vector to over-express ELMO1 protein in SK-HEP-1 cells.Over-expression of ELMO1 was confirmed by Western blot.siRNAs specific to ELMO1 (ELMO1 siRNA) were used to knockdown ELMO1 expression.Rho Family Small GTPase Activation Assay, Western blots, transwell assay were used to determine the migration potential of cells.Student's test was employed for statistical analysis.Results The expression of ELMO1 protein was obviously up-regulated in HCC tissues and cell lines (P<0.05).Over-expression of ELMO1 promoted cell migration in SK-HEP-1 cells, while knock-down of ELMO1 showed the opposite effect.Conclusion ELMO1 up-regulation significantly correlates with cell migration in hepatocellular carcinoma.

ABSTRACT

Objective: To investigate the effect of epidural block analgesia on childbirth, mother and child. Methods: A retro-spective study was used. Totally 200 primipara were divided into two groups according to the analgesia method. The 100 cases in the observation group was undergone lumbar epidural puncture and cathetering at the 2-3 section when the cervix was with 2-3cm opening, 0. 075% ropivacaine hydrochloride and sufentanil 10ml was infused firstly, and then 0. 075% ropivacaine and sufentanil was infused continuously through a self-control analgesia pump at the speed of 6-8ml·h-1 till the cervix was nearly all opened. The other 100 cases in the control group were without anesthesia when the cervix was with 2-3cm opening. The analgesia effect, labor time, delivery mode, postpartum hemorrhage amount, fetal distress and postpartum hemorrhage of the two groups were compared. Results:The analgesic ef-fective rate of the observation group was 97. 0%, which was significantly higher than that of the control group (10. 0%, P0. 05). Conclusion:Ropivacaine at low concentration combined with sufentanil used in epidural block analgesia has promising analgesic effect, which can shorten labor time to promote the smooth delivery.

ABSTRACT

Objective To explore the effect and application value of carboprost methylate suppositorites combined with oxytocin in preventing scar uterus bleeding of patients with cesarean section surgery.Methods According to the digital table,100 patients received cesarean section with scarred uterus were randomly divided into the observation group and control group.The control group was given oxytocin,the observation group received carboprost methylate suppositorites combined with oxytocin.The bleeding in the two groups was observed.Results In the observation group,postpartum hemorrhage occurred in 1 case,2h postpartum haemorrhage amount was (120.73 ±45.28)mL,24h postpartum haemorrhage amount was (283.32 ± 56.78)mL,compared with the control group,the postpartum hemorrhage rate decreased,postpartum 2h,24h blood loss decreased,the differences were statistically significant (x2 =4.891,t =6.582,7.351,all P < 0.05).In the observation group,4 cases occurred nausea and vomiting,13 cases of diarrhea,high blood pressure in 7 cases,facial flush in 1 case,compared with the control group,there were no statistically significant differences in the incidence rates of adverse reactions (x2 =0.153,0.053,0.919,1.041,all P >0.05).Conclusion Application of carboprost methylate suppositorites combined with oxytocin in scar uterus patients with cesarean section can reduce postpartum blood loss in the operation,reduce the incidence of postpartum hemorrhage,and not increase adverse reactions.

ABSTRACT

A number of studies conducted to assess the association between Clara cell 10-kDa protein [CC10] +38A/G polymorphism and susceptibility to asthma have yielded inconsistent and inconclusive results, in the present study, the possible association was assessed by a meta-analysis. Relevant articles were identified for the period ranging from Jan 1998 up to March 2013. Pooled odds ratios [OR] with 95% confidence intervals [Cl] were appropriately derived from fixed effects or random-effects models. Ten case-control studies with a total of 1529 asthma cases and 2399 controls were included in this meta-analysis. The association between CC10 +38A/G polymorphism and asthma risk was determined in dominant model, recessive model, additive model, and codominant model. In dominant model, CC10 +38A/G polymorphism seemed to be associated with elevated asthma risk [OR = 1.62; 95% Cl, 1.23-2.12; P = 0.0005]. Subgroup analyses by ethnicity also found significant associations between this polymorphism and asthma risk in Asians and Caucasians. Results from other genetic models further identified this possible association. This meta-analysis suggests that CC10 +38A/G polymorphism confers asthma risk
