ABSTRACT

Sentinel lymph nodes (SLNs) are the first station of lymph nodes that extend from the breast tumor to the axillary lymphatic drainage. The pathological status of these LNs can predict that of the entire axillary lymph node. Therefore, the accurate identification of SLNs is necessary for sentinel lymph node biopsy (SLNB) to replace axillary lymph node dissection (ALND). The quality of life and prognosis of breast cancer patients are related to proper surgical treatment after the precise identification of SLNs. Some of the SLN tracers that have been identified include radioisotope, nano-carbon, indocyanine green (ICG), and methylene blue (MB). However, these tracers have certain limitations, such as pigmentation, radiation dangers, and the requirement for costly detection equipment. Ultrasound contrast agents (UCAs) have good specificity and sensitivity, and thus can compensate for some shortcomings of the mentioned tracers. This technique is also being applied to SLNB in patients with breast cancer, and can even provide an initial judgment on SLN status. Contrast-enhanced ultrasound (CEUS) has the advantages of high distinguishability, simple operation, no radiation harm, low cost, and accurate localization; therefore, it is expected to replace the traditional biopsy methods. In addition, it can significantly enhance the accuracy of SLN localization and shorten the operation time.

ABSTRACT

Breast cancer is a malignant tumor that seriously endangers women's lives. The prognosis of breast cancer patients differs among molecular types. Compared with other subtypes, triple-negative breast cancer (TNBC) has been a research hotspot in recent years because of its high degree of malignancy, strong invasiveness, rapid progression, easy of recurrence, distant metastasis, poor prognosis, and high mortality. Many studies have found that long non-coding RNA (lncRNA) plays an important role in the occurrence, proliferation, migration, recurrence, chemotherapy resistance, and other characteristics of TNBC. Some lncRNAs are expected to become biomarkers in the diagnosis and prognosis of TNBC, and even new targets for its treatment. Based on a PubMed literature search, this review summarizes the progress in research on lncRNAs in TNBC and discusses their roles in TNBC diagnosis, prognosis, and chemotherapy with the hope of providing help for future research.

ABSTRACT

Objective:To study the clinical effect of thin replantation combined with vacuum assisted closure (VAC) in the treatment of large area retrograde skin avulsion injury.Methods:A total of 42 patients with large scale retrograde skin avulsion injury admitted to the trauma center of Zhuzhou Central Hospital from April 2017 to April 2019 were enrolled in this study, and were treated with VAC continuous negative pressure drainage after operation. The wound survival rate, wound survival area, wound infection, replantation skin performance and joint mobility were observed.Results:All of the 42 patients were followed up for 8-50(23.56±3.56)months. 35 patients survived the stage 1 skin grafting, the skin flap survived, and the wound had no obvious skin defect. After active dressing change, the wound healed well, and no second operation was needed. Small area necrosis occurred in the first stage wound of 7 patients due to large skin defect, and the wound healed well after the second stage surgical transplantation and enhanced dressing change. The Hospital for Special Surgery (HSS) score of 26 patients was 80-95(87.96±3.21), and the American Orthopaedic Foot and Ankle Society (AOFAS) score of 22 patients was 80-96(88.79±3.41). All patients had good skin sensation, elasticity, pressure resistance, wear resistance and color, and joint mobility was good without obvious limitation of movement.Conclusions:Thinning replantation combined with VAC negative pressure drainage in the treatment of large area retrograde skin avulsion injury can significantly promote the application of wound surface and skin graft, which is conducive to drain the drainage fluid out of the body, reduce the wound infection rate, improve the survival rate of skin grafting and improve joint function.

ABSTRACT

Objective To evaluate a model for axillary lymph node involvement combining CK19 mRNA with contrast enhanced ultrasound sonography (CEUS) score in operable breast cancer.Methods Operable breast cancer patients planned for sentinel lymph node (SLN) biopsy were enrolled.Preoperative CK19mRNA expressions in peripheral blood and CEUS score of axillary lymph nodes were tested before surgery.In the training set,postoperative sentinel lymph node (SLN) and non-sentinel lymph node (nSLN) pathological results were taken as the gold standard,effective modeling variables were screened,logistic regression was used to establish the prediction model.Parallel control studies were conducted between the validation set and the MSKCC model to evaluate the prediction accuracy and prediction efficiency.Results From Oct 2015 to Nov 2016,359 cases (training set) were enrolled and mathematical formulas for predicting SLN and nSLN were established,respectively.The sensitivity,specificity and AUC of predicting SLN involvement were 91.36%,94.92% and 0.979 respectively.The sensitivity,specificity and AUC of predicting nSLN metastasis were 91.04%,90.53% and 0.932 respectively.From Dec 2016 to Jul 2017,219 cases (verification set) were included.The sensitivity of SLN metastasis predicted by the model was 91.84%,the specificity was 96.69%,and the AUC was 0.979,significantly superior to the MSKCC model (0.739).The sensitivity,specificity and AUC of predicting nSLN metastasis were 95.35%,92.73% and 0.945 respectively,significantly superior to the MSKCC model (0.873).Concolusions Combined with peripheral blood CK19 mRNA and CEUS score,the prediction model for axillary lymph node involvement for operable breast cancer,SLN/nSLN involvement probability can be calculated and qualitative judgment can be made.The overall accuracy and AUC of this model are better than the prediction model of MSKCC.

ABSTRACT

Objective To compare the efficacy of vacuum sealing drainage( VSD) and the traditional treatment for breast abscess drain-age abscess.Methods A total of 59 cases with breast abscess in our hospital from February 2013 to December 2014 were randomly divided into two groups.The observation group underwent VSD and the control group treated by traditional abscess drainage of abscesses.The healing time,scarring length,the average length of hospital stay,number of dressing changes,pain degree were observed.The breast deformation and the recurrence of patients after 6 months were compared.Results The abscess healing time,the length of scar and the average days of hospi-talization were respectively (10.3 ±1.2) d,(1.5 ±0.2) cm,(12.7 ±3.5) d,the control group were respectively (16.5 ±2.6) d,(4.5 ± 0.6) cm,(26.9 ±3.3) d.The difference between two groups was statistically significant(P0.05).No breast deformation occurred in observation group,but 5 cases in the control group.The difference between two groups was statistically signifi-cant(P<0.05).Conclusion The effect of VSD for breast abscess is significant,which can accelerate the healing of the abscess and reduce the suffering of patients without affecting the appearance.

ABSTRACT

Objective To evaluate the value of PET-CT in patients with breast cancer and to determine if the PET-CT can provide additional information to predict early response to neoadjuvant chemotherapy (NAC).Methods NAC was given to 20 patients with breast cancer confirmed by biopsy puncture from September 2009 to March 2012.The PET-CT was carried out for all patients before NAC.TEC program with three weeks for one cycle was selected.After 6 days of the first cycle,the PET-CT was performed again.The changes of standard uptake value before and after the first cycle were compared.At the same time hand palpation was selected to detect the changes in tumor size before and after the first cycle of NAC.The changes of the standard uptake value and in tumor size need to refer to the pathology Miller & Payne classification methods to evaluate the efficacy of the NAC.Results The SUV were (7.51±1.76) Bq/ml and (4.98±1.61) Bq/ml before and after chemotherapy (t =7.916,P < 0.05) the maximum tumor diameters were (9.62±4.38) cm and (8.89±4.08) cm before and after NAC (t =2.154,P> 0.05).SUV had highly correlated with pathological MP classification (r =0.725,P =0.000); while for the tumor size there was no significant change (r =0.026,P =0.824).Conclusion PET-CT can predict the efficacy earlier and is more accurate than clinical efficacy standard for the NAC.

ABSTRACT

Objective To explore the effect of educational mode on children' s social adaptive capacity (SAC) through investigating its change of children into kindergarten one year later. MethodsFor this study, participants were divided into three groups, namely study group (SG), control group Ⅰ ( CG Ⅰ ) and control group Ⅱ( CG Ⅱ ). ①SG :30 ~ 47-month-old children at their entrance at one kindergarten of Xi' an city were study by the follow-up study method. There were 119 participants in this trial except 22 lost- visitors. The scores of children's SAC were evaluated at their entrance time and one year later respectively, according to the Social Adaptation Scale for Infant-Junior Middle School's Student Revised by Zuo Qi-hua. ②Control group(CG) :The children,whose classes were randomized,of five classes were divided into CG Ⅰ and the other four classes were CG Ⅱ. There were 107children aged at 30 ~ 46 months in CC Ⅰ , 94 children aged at 42 ~ 59 months in CG Ⅱ. The scores of their SAC were also evaluated according to the same scale on September 2006. The cross-section investigation were used to CG Ⅰ and CG Ⅱ. Results① The mean total score of SAC in SG was higher at one year later ( 10. 52 ± 0.77 ) than that at their entrance time( 10.22 ±0.52) (P<0.01) ,and the total score of SAC in SG was lower at their en trance time than that of CG Ⅰ (10.49 ±0.06)(P<0.01 ). ②There was no significant difference in children's SAC between SG at the final stage( 10. 52 ± 0.77 ) and CG Ⅱ(10.40 ± 0.67 ) (t = 1.500, P > 0.05). ③There were no significant difference in children's SAC between boarding care mode( 10.34 ± 0.68 ) and day care one( 10.32 ±0.56) (t = 0.182, P > 0.05 ). ④There were no significant difference in children' s SAC between male ( 10.42 ±0.73) and female( 10.51 ±0.68) in the kindergarten.. ConclusionThe gathered living mode can promote the child SAC development, and the boarding care mode or day care one in one year has no effect on children' s SAC.

ABSTRACT

Objective Baicalin reduces the severity of severe acute pancreatitis (SAP) in a rat model. This study was carried out to examine the effect of baicalin on TNF-α mRNA, caspase-3 and P-selectin protein expression in the pancreas of rats with SAP. Methods Rats with SAP were randomly assigned to untreated, baicalin-treated, octreotide-treated and sham-operated group. The TNF-α mRNA expression level, caspase-3 and P-selectin protein expression levels of pancreatic tissue were measured 3, 6 and 12 hours after operation. Results The caspase-3 protein expression level was significantly higher in the treated groups than that in the untreated group at 3 hours (p <0.01). P-selectin protein expression level in pancreas of the treated groups was significantly lower than that of the untreated group (p <0.05 or <0.01). TNF-α mRNA expression level in pancreas decreased in baicalintreated group while untreated group increased at different time points (p <0.05). Conclusion Baicalin increases caspase-3 expression and inhibits TNF-α mRNA and P-selectin protein expression in pancreatic tissue of SAP rats.

ABSTRACT

Objective To observe the protective effects of autologous bone marrow mesenchymal stem cells transplantation alone with bone marrow stem cells mobilization on liver cells in rats with severe acute paucreatitis in order to explore their mechanism. Method After the establishment of severe acute pancreatitis in rats made by intraperitoneal injection of L-arginine, 240 SD rats were randomly divided into sham-operated group (n = 48), model control group (n = 48), bone marrow mesenchymal stem cell transplanted (MSC) group (n = 48), granu-lacyte-colony stimulating factor treated (G-CSF) group (n = 48) and MSC + G-CSF (n = 48). The rats of MSC group were prepared by injection of 1.2 mL MSC into femoral vein 6 hours after SAP. The rats of G-CSF group were prepared by subcutaneous injection of G-CSF 40 μg/kg for 3 days before SAP. The rats of MSC + G-CSF group received MSC and G-CSF together. The rats of sham-operated group were injected with equal volume of nor-real saline. The rats in each group were sabdivided into 12 h, 24 h, 48 h and 72 h sub-groups (n = 12) according to the examinations in different intervals after operation. Of different subgroups, the morality rate, pathological changes, expression levels of Bax, Bcl-2 proteins and apoptosis indexes of livers were observed respectively. The contents of serum TNF-α,IL-6,ALT,AST,LDH and CRP were simultaneously determined to compare the difference among subgroups by variance analysis. Results Compared to the respective model group, the mortality rates of all treated 72 h subgroups showed no difference (P > 0.05), and no rats died before 48 h. The pathological injuries of liver cells were rather attenuated in rats of treated group than in rats of control group. The liver cell apoptosis in-dexes of 48 h and 72 h MSC + G-CSF subgroups were 107.1 ± 7.0, 110.3 ± 8.6, respectively; the expression of Bax in livers of 24 h,48 h and 72 h subgroups was 5.60±0.Z5, 5.69±0.22, 5.73±0.27, respectively;Bcl-2 protein of 48 h,72 h subgroups was 4.61±0.28, 4.43±0.28, respectively; compared with MSC and G-CSF subgroups the differences were significant (P < 0.05). The serum TNF-α, IL-6, ALT, AST, LDH and CRP de-creased obviously in 24 h/48 h treated subgroups in comparison with control group (P < 0.05). The MSC + G-CSF group showed more significant effects on those biomarkers than MSC or G-CSF alone after 48 hours (P < 0.05). Conclusions Autologous bone marrow mesenchymal stem cells transplantation alonewith bone marrow stem cells mobilization can significantly protect livers from severe damage during the course of severe acute panere-atitis, and the probable mechanisms are likely associated with the pathological regeneration, anti-inflammatory ef-fect and apoptosis inhibition of MSC.

ABSTRACT

Objective To observe the influence of Baicalin and Octreotide on lung injury of rat with severe acute pancreatitis(SAP) and discuss the therapeutic effect and mechanism of the two medicines on SAP.Methods The improved Aho(?)method was adopted to pre- pare SAP rat models via retrograde injection of 3.5% sodium taurocholate to the pancreatic duct.The 135 SAP rat models after being pre- pared were randomly divided into the model group,Baicalin treatment group and Octreotide treatment group with 45 rats in each group;an- other 45 were selected to be the sham operation group which only received abdomen opening surgery.The above - mentioned groups were then randomly divided into 3h,6h and 12h groups with 15 rats in each group.Observed respectively at 3h,6h and 12h after operation,the mortalities of all rat groups followed by batch execution of rats,and then observed the gross pathological and pathological changes of lung. The tissue microarrays technology was applied to prepare the lung tissue microarrays sections.The changes in Bax and bcl-2 protein ex- pression levels of lung tissue of each group were observed via immunohistochemical staining and meanwhile the TUNEL method was applied to observe the lung cell category and apoptotic index changes of lung tissue in each group.Results The 12h survival of model group was 66,67% while those of Baicalin treatment group and Octreotide treatment groups were both 100%,indicating a marked difference(P<0.05).Comparison of lung pathological score disclosed:The model group and Baicalin treatment group obviously exceeded the sham oper- ation group at various time points(P<0.01);at 6h and 12h the Baicalin treatment group was obviously less than the model group(P<0.05)and the Oetreotide treatment group less than the model group(P<0.01).Comparison of Bax protein of lung in each group dis- closed:The Baicalin treatment group and Octreotide treatment group obviously exceeded the sham operation group and model group at 3h (P<0.05),meanwhile the Baicalin treatment group obviously exceeded the Octreotide treatment group(P<0.01).Changes of lung bcl -2 protein expression level of all groups disclosed:The model group and Baicalin treatment group obviously exceeding the sham operation group(P<0.01)as well as the Octreotide treatment group(P<0.05)at different time points,the Baicalin treatment group was obviously less than the model group at 6h(P<0.05).The peak of apoptosis occurred to the Baicalin treatment group at 6h,but statistics showed no obvious difference among all groups at different time points(P>0.05).Conclusions(1)Both Baicalin and Octreotide have protecting effect on SAP lung injury and their therapeutic mechanism is possibly related to apoptosis.The application of tissue microarrays in SAP pathological examination can save manpower and material resources,cut down the experimental cost and improve the experimental efficien- cy,thus worth popularizing.

ABSTRACT

Objective To explore the antibacterial activity of the baicalin injection in vitro.Methods The broth doubling dilution method has been adopted.And the minimal inhibitory concentration(MIC)and minimum bactericidal concentration(MBC)of baicalin injection on 161 common pathogenic bacteria obtained from clinical practice were determined.Results The MIC range of baicalin against clinically common pathogenic bacteria is 0.1~50mg/ml and the MBC range is 0.2~50mg/ml.The MIC and MBC against staphylococci is remarkably lower than those against other bacteria,with the MIC range of 0.1~6.3mg/ml and MBC range of 0.2~12.5 respectivel.The MIC50,MIC90 are respectively 0.4mg/ml and 0.8mg/ml.Conclusions The baicalin injection possesses various levels of inhibitory effect on manifold pathogenic bacteria,especially the antibacterial effect on the staphylococci.

Objective To observe the acute toxic reaction and death of mice one day after intragastric administration of Baicalin capsule so as to appraise its safety. Methods The ICR mice were intragastrically administered with Baicalin powder in capsule at maximum concentration. There was no death of mice found and no LD_ 50 detected after administration. Hence the maximum dosage was identified. Results The maximum dosage of Baicalin powder is 15g/kg. Conclusions The Baicalin capsule is relatively safe.

Objective To observe the effect of emodin capsule on constipation model mice caused by diphenoxylate and compare it with the effect of Rhubarb & sodium bicarbonate tablet.Methods The ICR mice were divided into 6 groups as blank control group, diphenoxylate model group, emodin 25mg/kg,50mg/kg,100mg/kg group, and Rhubarb & sodium bicarbonate tablet 500mg/kg group. The blank control group and model group were given distilled water intragastrically while the rest 4 groups were given certain doses. The time of first dark stool defecation and 6-hour defecation quantity of each mouse were observed and recorded. BACKGROUND: Schwann cell can excrete many neurotrophic factors,protect neuron, and be nutritious to neuron. It also can promote the recover of injury of nerve.OBJECTIVE: To observe the expression of Schwann cell derived neurotrophic factor(SDNF) in developing lumber spinal cord of normal adult and different embryo rats as well as immature rats, and the effect on spinal cord in various developing period.DESIGN: Randomized and Controlled animal study.MATERIALS: The experiment was completed in the Animal Laboratory of Nanhua University in September 2003. Totally 30 Wister rats, in each gender,weighing 150-300 g, were selected from Animal Laboratory of Nanhua University.INTERVENTIONS: ① Selection and section: The normal lumbar spinal cord and dorsal root ganglia of normal adults rats with body mass in 180-220 g and 4-6 mature stages were fixed. Frozen sections were selected from 8 pregnant female rats including various embryol rats and immature rats. ② Immunohistochemical staining: Regarding protein antibody of SDNF as the first antibody,avidin-biotin peroxidase method was used to stain, the ash value was observed and the demi-quantitative value was worked out by scanning staining depth in order to analyze the expression of SDNF in lumbar spinal cord.MAIN OUTCOME MEASURES: ① Immunohistochemical staining of normal lumbar spinal cord; ② Immunohistochemical staining of developing lumbar spinal cord.RESULTS: Eight female rats were pregnant, and all samples were selected from the fetal rats and the immature rats. ① There appeared well-distributed SDNF, staining the whole white matter and Ⅲ, Ⅳ, Ⅴ stratum of the poster horn of gray matter of adult rats, and the dorsal root ganglia is much more intensively stained than lumber spinal cord. ② There was a inclination of early increase and late declination of SDNF immunoreactivity and the staining reaches a maximum at 21-day embryo, and then decreased at 7-day embryo and 12-day embryo after birth.CONCLUSION: There is lower expression of SDNF in lumber spinal cord of adult rats. But the expression of SDNF decreases gradually in the developing lumber spinal cord of rats. This means that SDNF may promote the development of spinal cord of embryo rats.

ABSTRACT

To investigate the serum level changes of tumor related factors in patients with mechanical trauma and with acute inflammation, and to inquire into the possible reason and its clinical significance, TSGF level was detected on day 1, 3 and 12 after trauma or inflammation in 60 patients with mechanical trauma (39 with mild trauma and 21 with severe trauma) and 37 patients with acute inflammation, while 40 healthy persons were selected as the normal control group. TSGF serum level was significantly higher in patients with trauma and with infectious inflammation than that in normal control group on day 1 and day 3( P 0 05).The results suggest that TSGF is related with the recovery of trauma and inflammatory response.
