ABSTRACT

Objective To analyze the correlation between serum miR-939 and miR-15b expression and mi-crovascular injury in patients with diabetic retinopathy(DR).Methods A total of 176 patients with type 2 di-abetes diagnosed and treated in the Baoding Second Hospital from January 2021 to October 2022 were selected as the study objects.The subjects were divided into 74 patients without DR(NDR group),62 patients with non-proliferative DR(NPDR group)and 40 patients with proliferative DR(PDR group)according to whether or not DR occurred and the degree of lesions.Real-time fluorescent quantitative PCR was used to detect the relative expression levels of miR-939 and miR-15b in serum of all groups,the level of vascular endothelial growth factor(VEGF)was detected by enzyme-linked immunosorbent assay,and the count percentage of en-dothelial cells(ECs),endothelial progenitor cells(EPCs)and circulating progenitor cells(CPCs)was detected by flow cytometry.Serum levels of miR-939,miR-15b,VEGF and ECs,EPCs and CPCs were compared in 3 groups.Pearson correlation was used to analyze the correlation between serum miR-939 and miR-15b and VEGF,ECs,EPCs and CPCs.Multivariate Logistic regression was used to analyze the factors affecting the oc-currence of DR in patients with type 2 diabetes.Results The relative expression levels of miR-939 and miR-15b in PDR group and NPDR group were lower than those in NDR group,while the serum VEGF levels were higher than those in NDR group,with statistical significance(P<0.05).ECs in PDR group and NPDR group were higher than those in NDR group,while EPCs and CPCs were lower than those in NDR group,the differ-ence was statistically significant(P<0.05).Serum miR-939 was negatively correlated with VEGF and ECs(r=-0.407,-0.613,P<0.05),and positively correlated with EPCs and CPCs(r=0.481,0.486,P<0.05).Serum miR-15b was negatively correlated with VEGF and ECs(r=-0.539,-0.625,P<0.05),and positively correlated with EPCs and CPCs(r=0.451,0.483,P<0.05).Multivariate Logistic regression anal-ysis showed that the duration of type 2 diabetes,hemoglobin A1c,2-hour postprandial blood glucose,VEGF,miR-939 and miR-15b were the influencing factors for the occurrence of DR in type 2 diabetes patients(P<0.05).Conclusion The expression of miR-939 and miR-15b in serum of DR patients is closely related to VEGF,ECs,EPCs and CPCs,and the expression of miR-939 and miR-15b in serum of DR patients can provide a certain reference for early judgment and evaluation of the degree of microvascular injury.

ABSTRACT

Objective:To systematically sort out and cluster the existing indicators of key issues in the quality of postgraduate clinical degree education based on the bibliometric study, so as to build a multidimensional quality assessment index system that integrates scientificity, rationality and representativeness, and to provide a scientific measurement tool for assessing clinical professional postgraduate education in China.Methods:By mining the related functions of UCINET6 network analysis integration software and its one-dimensional and two-dimensional data analysis NetDraw program, the social network analysis (SNA) method was used to extract and cluster the education quality problem set of clinical professional degree postgraduates.Results:A three-dimensional evaluation index system was constructed. The first dimension concluded such 8 key issues in the quality of postgraduate education in clinical medicine as ability assessment, teaching system, teaching quality assurance system, professional cognition and career prospects, assessment and evaluation system and organization, and the pulse taking and diagnosis.Conclusion:The clinical graduate education quality evaluation index system is an effective measurement tool for education quality improvement, based on a multidimensional perspective, with key issues as priority areas for intervention, providing an effective evidence-based basis for ensuring the development of professional graduate education efforts from 2020-2025.

Objective To investigate the current status of professional identity among the undergraduates majoring in public affairs management and the related influence factors.Methods A stratified cluster sampling method was used to conduct a questionnaire survey among the 115 undergraduates majoring in public affairs management in a medical university in Harbin,China,and the contents of the questionnaires included general status and professional identity.The t-test,one-way analysis of variance,and multiple linear regression analysis were used to analyze the influencing factors of professional identity.In-depth interviews were conducted for some students to explore the profound influencing factors for professional identity among the students majoring in public affairs management.Results The overall level of professional identity was low among the undergraduates majoring in public affairs management in the medical university in Harbin,and the score of professional identity was 3.25 ± 0.56.The scores of cognitive dimension,emotional dimension,behavioral dimension,and applicable dimension were 3.41 ± 0.69,3.21 ± 0.78,3.24 ± 0.71,and 3.13 ± 0.72,respectively.The multivariate analysis showed that grade,academic performance,major selection,and children number in family had significant influence on their professional identity.The students who were at grade four,who had high academic record (b =-0.276,P< 0.01),who chose this major independently (b=-0.277,P<0.01),and who were the only child (b=-0.233,P< 0.05) had a high score of professional identity.As for the undergraduates at different grades,the undergraduates at grades two and three had a significantly lower score of professional identity than those at grade four (b=-0.296 and-0.369,P<0.05),while there was no significant difference between the undergraduates at grades one and four (b=-0.008,P>0.05).Furthermore,the students with a high score of professional identity had higher possibility to pursue the job related to this major than those with a low score (3.39 ± 0.53 vs.2.92 ± 0.49,t=4.414,P<0.01).Conclusion Universities should strengthen the publicity of the major of public affairs management,optimize the curriculum,and add job experience,so as to enhance professional identity among students with this major and provide talent for the field of public health management.

ABSTRACT

Objective To establish a simple diabetic peripheral neuropathy (DPN) rat model with the high fat-fed in GK rats. Methods A total of 30 GK rats (7-8 weeks) were fed with high-fat diet to establish the DPN model. Thirty normal Wistar rats were fed with ordinary diet (control group). The blood-sugar value, body mass, water-intake and food-intake were monitored every week in two groups. The serum level of glycosylated hemoglobin, the right sciatic nerve conduction velocity were detected at 8, 12 and 16 weeks respectively. The left sciatic nerve was used for HE and TUNEL staining. Results The manifestations of polydipsia, polyphagia and growth retardation were gradually appeared in GK rats. After 12 and 16 weeks, the blood-sugar and glycosylated hemoglobin were significantly increased in GK rats compared with those of normal Wistar rats (P 0.05). The sciatic nerve pathological features and Schwann cell apoptosis suggested that the model of DPN was successfully established (apoptosis index, P <0.01). Conclusion GK rats fed by high-fat diet are the satisfactory models of the DPN in experimental research. And 12-week is a suitable and economical time for molding.

ABSTRACT

Objective To investigate the effect of Tongluo Tangtai Recipe(TTR)on inflammatory cytokines in rats with diabetic peripheral neuropathy(DPN). Methods Rat model of spontaneous DPN was established by feeding high fat diet. The rats were divided into 5 groups , namely model group , western medicine(metformin 100 mg·kg-1·d-1+ methycobal 0.2 mg·kg-1·d-1) group, and low-, middle- and high-dose TTR groups (TTR in the dosage of 0.72, 1.44, 2.88 g·kg-1·d-1 respectively). Rats of the normal group and model group were treated with normal saline orally, and the medication groups were given the corresponding medicine according to the experimental design. After intragastric administration for 4 weeks, sciatic motor/sensory nerve conduction velocity was detected with electrophysiological examination method, and tumor necrosis factor alpha(TNF-α), interleukin-6(IL-6), C-reactive protein(CRP), myelin basic protein(MBP) levels in the serum were examined by enzyme-linked immunosorbent assay(ELISA). Results TTR in various dosages could reduce the serum levels of MBP and inflammatory cytokines of TNF-α, IL-6 and CRP, and increase sciatic nerve conduction velocity (P<0.05 or P<0.01 compared with those in the model group), the high dose showing the strongest effect. Conclusion TTR could inhibit inflammation reaction and nerve demyelination, and improve nerve structure and conduction function, which may contribute to the preventive and therapeutic mechanism of TTR for DPN.

ABSTRACT

Professor FU Zhonghua's unique clinical experience of Fu's subcutaneous needling (FSN) for cervical spondylosis (CS) is discussed in this paper, which is analyzed from the aspects of recognition of CS pathogenesis, treatment mechanism of FSN, advantage indications of FSN for CS and examples of medical cases. Professor FU introduced the theory of myofascial trigger points (MTrP) into the field of the management of CS. The site of neck MTrP should be carefully examined, and FSN needles for single use are used to sweep the affected area or subcutaneous layer of adjacent upper limb. This method can rapidly improve ischemia and hypoxia state of the relevant muscles and prompt the self-recovery of neck muscles. During FSN treatment, reperfusion approach is recommended to adopt to improve the qi and blood circulation and recovery of neck function.

ABSTRACT

<p><b>OBJECTIVE</b>To map the gene responsible for nonsyndromic hearing impairment in a consanguineous family.</p><p><b>METHODS</b>Firstly, X chromosome scanning was used to exclude X chromosome. Secondly, candidate gene analyzing and genome scanning were performed by homozygosity mapping. Then, additional markers flanking the tightly linked marker were tested to confirm linkage and decide the candidate region.</p><p><b>RESULTS</b>The nonsyndromic hearing impairment of this family was autosomal recessive. Twenty-five known genes were excluded. Autosomal genome scanning indicated that D17S1293 was tightly linked with disease gene. And further study mapped the disease gene to a 5.07 cM interval bounded by D17S1850 and D17S1818.</p><p><b>CONCLUSION</b>The disease gene of the family is mapped to a 5.07 cM interval between D17S1850 and D17S1818, which is a new locus of autosomal recessive nonsyndromic hearing impairment.</p>
