ABSTRACT

Objective:To investigate the predictive value of dynamic electrocardiography (ECG) for myocardial ischemia and arrhythmia in patients with coronary heart disease. Methods:A total of 87 patients with coronary heart disease who received treatment in Wenling Hospital of Traditional Chinese Medicine from January 2020 to January 2022 were included in this study. These patients were divided into a conventional ECG group (group A, n = 40) and a dynamic ECG group (group B, n = 47) according to different examination methods. The detection rate of myocardial ischemia in the two groups was determined. The diagnostic efficacy of routine ECG and dynamic ECG for myocardial ischemia was analyzed according to the results of coronary angiography. The detection rate of arrhythmia was determined in each group. Patient satisfaction was compared between the two groups. Results:There was no significant difference in the total detection rate of myocardial ischemia between the two groups ( P = 0.154). The detection rate of asymptomatic myocardial ischemia in group B [25% (25/47)] was significantly higher than that in group A [10% (10/40), χ2 = 5.24, P = 0.020). According to the results of coronary angiography, the detection rates of myocardial ischemia in group A and group B were 68% (27/40) and 87% (41/47), respectively. The sensitivity, specificity, and accuracy of group B in diagnosing myocardial ischemia were 80% (33/41), [67% (4/6), and 74% (35/47), respectively, all of which were significantly higher than 74% (20/27), [46% (6/13), 68% (27/40) in group A ( χ2 = 8.03, 5.89, 5.03, P = 0.003, 0.012, 0.005). The detection rates of premature ventricular contraction in bi-triplet rhythm, frequent paired premature atrial contraction in bi-triplet rhythm, right bundle-branch block, atrioventricular block, supraventricular tachycardia in group B were significantly higher compared with group A ( χ2 = 10.78, P = 0.001). According to the results of coronary angiography, the detection rates of arrhythmia in groups A and B were 65% (26/40) and 79% (37/47), respectively. The sensitivity, specificity, and accuracy of diagnosing arrhythmia in coronary heart disease in group B were 86% (32/37), 40% (4/10), and 81% (38/47), respectively. The sensitivity and accuracy of diagnosing arrhythmia in coronary heart disease in group B were significantly higher than those in group A [58% (15/26), 55% (22/40), χ2 = 7.30, 4.90, P = 0.005, 0.010). Patient satisfaction in group B [87% (41/47)] was significantly higher than that in group A [65% (26/40), χ2 = 6.24, P = 0.044]. Conclusion:Compared with conventional ECG detection, dynamic ECG has substantially higher detection and diagnosis rates of myocardial ischemia and arrhythmia, as well as a higher patient satisfaction rate, in patients with coronary heart disease, in particular in those with asymptomatic myocardial ischemia.

ABSTRACT

Exosomes, as a type of multivesicular bodies secreted by various cells in human body, exist in many kinds of body fluids in the body. Exosomes have the structure of lipid bilayer membranes to protect the structure and function stability of their contents and they carry a large number of active substances including microRNA (miRNA) and mRNA. Studies have shown that exosomes and their miRNA are closely associated with the development and progression of liver fibrosis and regulate such process by acting on hepatic stellate cells to change their activation, proliferation, apoptosis, and migration. This article elaborates on the biological characteristics of exosome miRNA and its association with liver fibrosis, analyzes its important role in the development and progression, diagnosis and treatment, and prognostic evaluation of liver fibrosis, and points out that exosome miRNA may become a new potential target for the diagnosis, treatment, and prognostic evaluation of liver fibrosis.

ABSTRACT

Objective To explore the efficacy and safety of double dosage of SHUGANJIEYU capsule on mod?erate depression. Methods The study was conducted by a multicenter, random, double-blind and parallel-controlled trial. Trial group received SHUGANJIEYU capsules 4# twice a day. Control group got two SHUGANJIEYU capsules and two simulated ones twice a day. The study lasted 56 days (8 weeks) for both groups. Primary efficacies were evalu? ated by the total score and score changed rate of Hamilton depression scale-17 (HAMD-17)and Hamilton anxiety scale (HAMA) as well safety by reported adverse events and laboratory tests for patients. Results One hundred twenty patients were recruited in trial group and 120 patients in control group. After a 8-week treatment, the remission ratio was 84.2% and 63.3% for the trial group and the control group, respectively according to the HAMD-17. The differ?ence in the remission ratio was significant (P0.05). Conclusion SHUGANJIEYU capsule ex?hibits a good therapeutic effect on the depressive symptoms and anxiety symptoms in patients with moderate depres?sion. Double doses of SHUGANJIEYU capsule can increase the effect of SHUGANJIEYU and accelerates SHUGAN?JIEYU-induced improvement of depression symptoms with an acceptable safety.

ABSTRACT

Objective To assess the quality of life(QOL)of advanced schistosomiasis patients, and understand its influenc-ing factors. Methods A questionnaire survey was carried out by using WHOQOL-BREF,and the information of demography, family, illness, health status, and health service was collected. Results Among the 217 advanced patients, the average age was (75.33±6.94)years,the ratio of male to female was 2∶5,89.86%of them were farmers,75.58%were illiterate or semi-illiterate, and 88.94%belonged to the splenomegaly type. Totally 61.29%of the cases had the scores over average for the overall QOL,but the scores for the health and well-being were lower. There were a significant difference among the average scores of different do-mains of QOL(χ2 =23.46,P<0.01). Both the year of being diagnosed and clinical classification was not associated with the scores of QOL. Regularly taking physical activities was significantly associated with each of all the 4 domains. The factors such as onset of acute disease in 2 weeks, taking therapeutic pills daily, marital status, age, etc. impacted the specific domains of QOL. The overall QOL and the 4 domains of the patients were at medium level;meanwhile, the score of physiological domain was lower than the scores of the other 3 domains. Conclusions The QOL(s)of advanced schistosomiasis patients in Qingpu District are rel-atively good. It is important to provide effective community health services and encourage the patients to take part in tempered sports or physical activities in order to keep their normal activities of daily living.

ABSTRACT

AIM: To detect the association between the polymorphism of Fc receptor ? chain gene at position-29 in promoter and systemic lupus erythematosus(SLE). METHODS: The genotypes at position -29 in promoter of Fc receptor ? chain gene were determined by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) method in 180 patients with SLE and 140 ethnically matched controls in southern China. RESULTS: The frequencies of TT genotype(33.3%) and T allele (54 4%) at position -29 in patients with SLE were significantly higher than those in controls (17 9%, respectively), whereas, the frequencies of GG genotype (24 4%) and G allele (45 6%) in patients with SLE were remarkably lower than those in controls (31 4% and 57 1%, respectively) ( P 0 05) . CONCLUSION: Our results indicate that the T allele at position -29 in promoter of Fc receptor gene probably contributes to the susceptibility to SLE, but does not play a role in the occurrence of lupus nephritis.
